Abstract:BACKGROUND Accurate assessment of the natural history of late-life depression requires frequent observation over time. In later life, depressive disorders fulfilling rigorous diagnostic criteria are relatively rare, while subthreshold disorders are common. The primary aim was to study the natural history of late-life depression, systematically comparing those who did with those who did not fulfill rigorous diagnostic criteria. METHODS Within the Longitudinal Aging Study Amsterdam, a large cohort of depressed elderly persons (n = 277) was identified and followed up for 6 years, using 14 observations. Depression was measured using self-reports (the Center for Epidemiological Studies Depression Scale) and diagnostic interviews (the Diagnostic Interview Schedule). The natural history was assessed for symptom severity (Center for Epidemiological Studies Depression Scale score), symptom duration, clinical course type, and stability of diagnoses. RESULTS The average symptom severity remained above the 85th percentile of the population average for 6 years. Symptoms were short-lived in only 14%. There were remissions in 23%, an unfavorable but fluctuating course in 44%, and a severe chronic course in 32% (percentages do not total 100 because of rounding). Comparing the outcome, there was a clear gradient in which those with subthreshold disorders had the best outcome, followed by those with major depressive disorder, dysthymic disorder, and double depression. However, the prognosis of subthreshold disorders was unfavorable in most cases, while this group was at high risk of developing DSM affective disorders. CONCLUSIONS The natural history of late-life depression in the community is poor. DSM affective disorders are relatively rare among elderly persons, but do identify those with the worst prognosis. However, subthreshold depression is serious and chronic in many cases.

Abstract:Reluctance to talk about corruption is an important barrier to action. Yet the stakes of not addressing corruption in the health sector are higher than ever. Corruption includes wrongdoing by individuals, but it is also a problem of weak institutions captured by political interests, and underfunded, unreliable administrative systems and healthcare delivery models. We urgently need to focus on corruption as a health systems problem. In addition to supporting research to better understand the context and implications of corruption in health systems, this article suggests actions that public health professionals can do now to fight corruption.

Abstract:Objectives: Corruption undermines the quality of healthcare and leads to inequitable access to essential health products. WHO, Global Fund, United Nations Development Programme (UNDP) and World Bank are engaged in anti-corruption in health sectors globally. Throughout the COVID-19 pandemic, weakened health systems and overlooked regulatory processes have increased corruption risks. The objective of this study is thus to explore the strengths and weaknesses of these organisations' anti-corruption mechanisms and their trajectories since the pandemic began. Design, setting and participants: 25 semistructured key informant interviews with a total of 27 participants were conducted via Zoom between April and July 2021 with informants from WHO, World Bank, Global Fund and UNDP, other non-governmental organisations involved in anti-corruption and academic institutions. Key informant selection was guided by purposive and snowball sampling. Detailed interview notes were qualitatively coded by three researchers. Data analysis followed an inductive-deductive hybrid thematic analysis framework. Results: The findings demonstrate that WHO, World Bank, Global Fund and UNDP have shifted from criminalisation/punitive approaches to anti-corruption to preventative ones and that anti-corruption initiatives are strong when they are well funded, explicitly address corruption and are complemented by strong monitoring and evaluation mechanisms. Weaknesses in the organisations' approaches to anti-corruption include one-size-fits-all approaches, lack of political will to address corruption and zero-tolerance policies for corruption. The COVID-19 pandemic has highlighted the necessity of improving anti-corruption by promoting strong accountability and transparency in health systems. Conclusions: Results from this study highlight the strengths, weaknesses and recent trajectories of anti-corruption in the Global Fund, World Bank, UNDP and WHO. This study underscores the importance of implementing strong and robust anti-corruption mechanisms specifically geared towards corruption prevention that remain resilient even in times of emergency.

Abstract:Macrophages from osteopetrotic mice caused less bone resorption than normal sib macrophages in culture. The bone-resorbing activity lay in the culture supernatant, and reduced activity in cultures of macrophages from osteopetrotic mice correlated with reduced glass spreading and reduced latex phagocytosis. We suggest that macrophages from osteopetrotic mice show defective phagocytic recognition of glass, latex, and bone and that this defective recognition may reflect the cause of the failure of resorption which results in osteopetrosis.

Abstract:The authors used data from the 1970 National Fertility Survey and Centers for Disease Control surveillance of surgical sterilizations to estimate the cumulative prevalences of hysterectomy and tubal sterilization among women of reproductive age in the United States between 1971 and 1978. In 1978, the cumulative prevalence rate of tubal sterilization was more than twice as high for women aged 15--44 years as it was in 1971 and at least three times as high for women under 30. Although the increase in the cumulative prevalence rate of hysterectomy was not as marked, by 1978, 19% of women aged 40--44 had undergone hysterectomy. The authors used the cumulative prevalence rate to estimate the population at risk for surgical sterilization, and calculated the corrected incidence rates for these procedures. While corrected incidence rates of tubal sterilization among women aged 15--44 doubled between 1971 and 1978, corrected hysterectomy rates remained stable. The largest age-specific increase in incidence rates of tubal sterilization was among women 40--44, with rates six times higher in 1978 than in 1971. These findings can be used to recompute incidence rates of endometrial and cervical cancers, abortions, and ectopic pregnancies, allowing more precise analysis of related trends.

Abstract:Background: As called for by the Sustainable Development Goals, governments, development partners and civil society are working on anti-corruption, transparency and accountability approaches to control corruption and advance Universal Health Coverage.Objectives: The objective of this review is to summarize concepts, frameworks, and approaches used to identify corruption risks and consequences of corruption on health systems and outcomes. We also inventory interventions to fight corruption and increase transparency and accountability.Methods: We performed a critical review based on a systematic search of literature in PubMed and Web of Science and reviewed background papers and presentations from two international technical meetings on the topic of anti-corruption and health. We identified concepts, frameworks and approaches and summarized updated evidence of types and causes corruption in the health sector.Results: Corruption, or the abuse of power for private gain, in health systems includes bribes and kickbacks, embezzlement, fraud, political influence/nepotism and informal payments, among other behaviors. Drivers of corruption include individual and systems level factors such as financial pressures, poorly managed conflicts of interest, and weak regulatory and enforcement systems. We identify six typologies and frameworks that model relationships influencing the scope and seriousness of corruption, and show how anti-corruption strategies such as transparency, accountability, and civic participation can affect corruption risk. Little research exists on the effectiveness of anti-corruption measures; however, interventions such as community monitoring and insurance fraud control programs show promise.Conclusions: Corruption undermines the capacity of health systems to contribute to better health, economic growth and development. Interventions and resources on prevention and control of corruption are essential components of health system strengthening for Universal Health Coverage.

Abstract:Corruption is a global wicked problem that threatens the achievement of health, social and economic development goals, including Sustainable Development Goal # 3: Ensuring healthy lives and promoting well-being for all. The COVID-19 pandemic and its resulting strain on health systems has heightened risks of corruption both generally and specifically within health systems. Over the past years, international organizations, including those instrumental to the global COVID-19 response, have increased efforts to address corruption within their operations and related programs. However, as attention to anti-corruption efforts is relatively recent within international organizations, there is a lack of literature examining how these organizations address corruption and the impact of their anti-corruption efforts. This study addresses this gap by examining how accountability, transparency, and anti-corruption are taken up by international organizations within their own operations and the reported outcomes of such efforts. The following international organizations were selected as the focus of this document analysis: the World Health Organization, the Global Fund, the United Nations Development Programme, and the World Bank Group. Documents were identified through a targeted search of each organization's website. Documents were then analyzed combining elements of content analysis and thematic analysis. The findings demonstrate that accountability and transparency mechanisms have been employed by each of the four international organizations to address corruption. Further, these organizations commonly employed oversight mechanisms, including risk assessments, investigations, and audits to monitor their internal and external operations for fraud and corruption. All organizations used sanction strategies meant to reprimand identified transgressors and deter future corruption. Findings also demonstrate a marked increase in anti-corruption efforts by these international organizations in recent years. Though this is promising, there remains a distinct absence of evidence demonstrating the impact of such efforts on the prevalence and severity of corruption in international organizations.

Abstract:Corruption is recognized by the global community as a threat to development generally and to achieving health goals, such as the United Nations Sustainable Development Goal # 3: ensuring healthy lives and promoting well-being for all. As such, international organizations such as the World Health Organizations and the United Nations Development Program are creating an evidence base on how best to address corruption in health systems. At present, the risk of corruption is even more apparent, given the need for quick and nimble responses to the COVID-19 pandemic, which may include a relaxation of standards and the rapid mobilization of large funds. As international organizations and governments attempt to respond to the ever-changing demands of this pandemic, there is a need to acknowledge and address the increased opportunity for corruption.In order to explore how such risks of corruption are addressed in international organizations, this paper focuses on the question: How are international organizations implementing measures to promote accountability and transparency, and anti-corruption, in their own operations? The following international organizations were selected as the focus of this paper given their current involvement in anti-corruption, transparency, and accountability in the health sector: the World Health Organization, the United Nations Development Program, the World Bank Group, and the Global Fund to Fight Aids, Tuberculosis and Malaria. Our findings demonstrate that there has been a clear increase in the volume and scope of anti-corruption, accountability, and transparency measures implemented by these international organizations in recent years. However, the efficacy of these measures remains unclear. Further research is needed to determine how these measures are achieving their transparency, accountability, and anti-corruption goals.

Abstract:At the government, hospital, and health-care provider level, corruption plays a major role in health-care systems in Africa. The returns on health investments of international financial institutions, health organisations, and donors might be very low when mismanagement and dysfunctional structures of health-care systems are not addressed. More funding might even aggravate corruption. We discuss corruption and its effects on cancer care within the African health-care system in a sociocultural context. The contribution of high-income countries in stimulating corruption is also described. Corrupt African governments cannot be expected to take the initiative to eradicate corruption. Therefore, international financial institutions, health organisations, and financial donors should use their power to demand policy reforms of health-care systems in Africa troubled by the issue of corruption. These modifications will ameliorate the access and quality of cancer care for patients across the continent, and ultimately improve the outcome of health care to all patients.

Abstract:Corruption is of central interest to business ethics but its meaning is often assumed to be self-evident and universal. In this paper we seek to re-politicize and unsettle the dominant meaning of corruption by showing how it is culturally specific, relationally derived and varies over time. In particular, we show how corruption's meaning changes depending on its relationship with Western-style liberal democracy and non-Western local experience with its implementation. We chose this focus because promoting democracy is a central plank of the international anti-corruption and development agenda and yet the relationship between corruption and democracy is rarely specified. Adopting a critical-discursive approach that draws on poststructuralism and postcolonialism, we explore how the meaning of corruption constructed in The Jakarta Post (TJP) changed in relation to Indonesia's experience in implementing democratic reform, a condition of the international financial aid it received following the 1997 Asian Financial Crisis. In the 1990s, corruption was seen as an illness, and democracy the cure; from 2000 to 2011 experience with democracy brought disillusion—democracy had not cured corruption but caused it to spread; while from 2012 to 2014 democracy was constructed as a valued end in its own right, but needed protection from corruption in order to survive. From translating the international development agenda in a relatively straightforward way, TJP moved towards constructions of increasing complexity and ambivalence. This demonstrates how corruption's meaning is fundamentally contingent and unstable—even dominant meanings have the potential to be contested, showing how they are an effect of power and raising the possibility of alternatives.

Abstract:Corruption is one of the most intractable problems that the world is faced with and its reported impact is widespread and pervasive. Since the mid-1990s, international efforts to combat this problem expanded significantly, driven by the involvement governments, international financial institutions and non-governmental organisations. The objective of this article is to use Michel Foucault's work in a critical analysis of the international fight against corruption. This analysis is centred on Foucault's concept of governmentality, as well as his notions of knowledge, power and ethics. The primary focus is on the role played by the World Bank, the International Monetary Fund and Transparency International in the proliferation of international anti-corruption discourse and practices. Furthermore, South Africa will be used as a case study to unpack the historical and contextual factors that play a role in the legitimisation of corruption. As will be shown in this assessment, without adequately understanding the local factors that stimulate corruption and the other societal characteristics that serve to compound its pervasiveness, internationally driven anti-corruption initiatives have limited scope for success. Corruption will persist where there is a lack of resonance between international anti-corruption ideals and local norms, values and ideologies.

Abstract:American Behavioral Scientist, Ahead of Print. In recent years, digital technologies have been recognized as essential for combating corruption in healthcare by academics and practitioners alike. Our study focuses on India, where healthcare policies increasingly emphasize digital public health services. We analyze multi-stakeholder perspectives emerging in the country post the 2020 launch of a national digital health plan. The study uses interviews, participant observation, and official video transcripts to critically appraise these perspectives. Although government actors view digital innovation as a panacea for issues like inclusion, transparency, and service delivery, other stakeholders raise concerns on wide-ranging issues. These include infrastructural and design challenges, unclear policies on data privacy, and the opaque role of private companies in providing digital solutions. Particularly, grassroots actors warn of new opportunities for corruption due to extensive digitization. This article explores the dichotomy between policy intentions and practical realities, highlighting the complex relationship between digital transformation in healthcare and corruption control.

Abstract:Background Improving anti-corruption, transparency and accountability mechanisms (ACTA) in the health sector is critical for progressing towards universal health coverage. Mechanisms to foster ACTA, are rarely enforced in many LMICs and infractions and circumvention of rules are commonplace. We explored particular structures, Ward Development Committees (WDCs), in Kano, Nigeria, that are considered to be successful by different stakeholders, seeking to explore the ways in which they are able to tackle health sector corruption. Methods We used iterative qualitative research methodology employing Khan and Roy's 2022 Power, Capabilities and Interest (PCI) framework involving a series of steps. The process began with a co-creation workshop that included 'story arcs' (participant-created narratives on the mechanisms through which the Committees act on corruption), and the political economy aspects that shape how they operate, followed by key informant interviews. Given the sensitive nature of the topic, we emphasized a close engagement and building trust with stakeholders. Our methodology included debriefing meetings, post meeting individual reflections (to ensure reflexibility on the complex set of emerging issues) and a researchers' workshop, to review gaps in findings and identify explanatory theories. Results WDCs use a range of strategies for impact, interacting with stakeholders from across sectors in contextually appropriate ways to foster ACTA. Specific factors underlying success are multi-sectorality, the enabling and relatively receptive policy environment, integration with the health governance structures, longstanding and synergistic donor support, and cultural value of volunteerism. The WDCs gradually developed competencies that empowered them to serve as agents for ACTA. Conclusion Incorporating community based structured into health governance structures is essential for improving ACTA. Our study suggest that governance actors should examine how similar grassroot-led mechanisms can be strengthened elsewhere, while ensuring these are adapted to the local health systems and socio-political contexts.

Abstract:Background: The goal of the public procurement of pharmaceuticals is to purchase sufficient quantities of high-quality pharmaceuticals at cost-effective prices for a given population. This goal can be undercut if corruption infiltrates the procurement process. Good procurement practices can help mitigate the risks of corruption and support equitable access to affordable and high-quality medicines.Objectives: This paper aims to 1) examine manifestations of corruption in the pharmaceutical procurement process and key factors behind them, and 2) identify how to design and implement effective anti-corruption, transparency and accountability mechanisms within this process.Methods: This paper was informed by a narrative literature review from 1996 to the present. The search focused on publications that addressed the issue of pharmaceutical procurement and governance and corruption issues. Our search included peer-reviewed literature, books, grey literature such as working papers, reports published by international organizations and donor agencies, and some media articles. Some documents used in this paper were already known to the authors.Results: Procurement is highly vulnerable to corruption particularly in the health sector. What is more, corruption in the procurement process does not appear to be limited to any one level of government or type of health system. The better integration of accountability, transparency and anti-corruption mechanisms in the procurement process is needed to reduce the risk of corruption.Conclusions: Lessons learned suggest that anti-corruption, transparency and accountability mechanisms in the pharmaceutical procurement process, such as open contracting and integrity pacts are helpful towards reducing the risk of corruption.
