Abstract:Dengue is a vector-borne disease transmitted to humans by vectors of genus Aedes causing a global threat to health, social, and economic sectors in many of the tropical countries including Sri Lanka. In Sri Lanka, the tropical climate, marked by seasonal weather primarily influenced by monsoons, fosters optimal conditions for the virus to spread efficiently. This heightened transmission results in increased per-capita vector density. In this work, we investigate the dynamic influence of environmental conditions on dengue emergence in Colombo district − the geographical region with the highest recorded dengue threat in Sri Lanka. An iterative approach is employed to dynamically estimate dengue cases leveraging the Markov chain Monte Carlo simulations, utilizing the dynamics of four seasons per year influenced by monsoon weather patterns governing in the region. The developed algorithm allows to estimate the risk of dengue outbreaks in 2017 and 2019 with high precision, facilitating accurate forecasts of upcoming disease emergence patterns for better preparedness. The uncertainty quantification not only validated the accuracy of outbreak estimates but also showcased the model's capacity to capture extreme cases and revealed undisclosed external factors such as human mobility and environmental pollution that might affect dengue transmission in the Colombo district of Sri Lanka.

Abstract:Epidemics of communicable diseases place a huge burden on public health infrastructures across the world. Producing accurate and actionable forecasts of infectious disease incidence at short and long time scales will improve public health response to outbreaks. However, scientists and public health officials face many obstacles in trying to create accurate and actionable real-time forecasts of infectious disease incidence. Dengue is a mosquito-borne virus that annually infects over 400 million people worldwide. We developed a real-time forecasting model for dengue hemorrhagic fever in the 77 provinces of Thailand. We created an operational and computational infrastructure that generated multi-step predictions of dengue incidence in Thai provinces every two weeks throughout 2014. These predictions show mixed performance across provinces, out-performing na\"ive seasonal models in over half of provinces at a 1.5 month horizon. Additionally, to assess the degree to which delays in case reporting make long-range prediction a challenging task, we compared the performance of our real-time predictions with predictions made with fully reported data. This paper provides valuable lessons for the implementation of real-time predictions in the context of public health decision making.

Abstract:Abstract Background Dengue epidemics is affected by vector-human interactive dynamics. Infectious disease prevention and control emphasize the timing intervention at the right diffusion phase. In such a way, control measures can be cost-effective, and epidemic incidents can be controlled before devastated consequence occurs. However, timing relations between a measurable signal and the onset of the pandemic are complex to be discovered, and the typical lag period regression is difficult to capture in these complex relations. This study investigates the dynamic diffusion pattern of the disease in terms of a probability distribution. We estimate the parameters of an epidemic compartment model with the cross-infection of patients and mosquitoes in various infection cycles. We comprehensively study the incorporated meteorological and mosquito factors that may affect the epidemic of dengue fever to predict dengue fever epidemics. Results We develop a dual-parameter estimation algorithm for a composite model of the partial differential equations for vector-susceptible-infectious-recovered with exogeneity compartment model, Markov chain Montel Carlo method, and boundary element method to evaluate the epidemic periodicity under the effect of environmental factors of dengue fever, given the time series data of 2000–2016 from three cities with a population of 4.7 million. The established computer model of “energy accumulation-delayed diffusion-epidemics” is proven to be effective to predict the future trend of reported and unreported infected incidents. Our artificial intelligent algorithm can inform the authority to cease the larvae at the highest vector infection time. We find that the estimated dengue report rate is about 20%, which is close to the number of official announcements, and the percentage of infected vectors increases exponentially yearly. We suggest that the executive authorities should seriously consider the accumulated effect among infected populations. This established epidemic prediction model of dengue fever can be used to simulate and evaluate the best time to prevent and control dengue fever. Conclusions Given our developed model, government epidemic prevention teams can apply this platform before they physically carry out the prevention work. The optimal suggestions from these models can be promptly accommodated when real-time data have been continuously corrected from clinics and related agents.

Abstract:Various ocular manifestations have been described in Wegener's granulomatosis. Orbital involvement is common and manifestations in the anterior segment of the eye such as keratitis, scleritis, and conjunctivitis may occur. Involvement of the posterior segment presenting with major vascular occlusions or anterior ischemic neuropathy is rare. This case report presents a patient with bilateral cotton-wool spots at the posterior pole associated with histologically proven Wegener's granulomatosis. This finding, as an initial ocular manifestation of the disease, is very uncommon and may be interpreted as focal retinal inflammatory vasculitis. This was supported by the rapid positive response to systemic combined immunosuppressive and steroidal treatment during a 30-month period of review. The presence of cotton-wool spots in association with systemic signs of the disease should be considered in establishing the diagnosis of Wegener's granulomatosis.
A system dynamics modelling and analytical framework for imported dengue outbreak surveillance and risk mapping

Abstract:System Dynamics (SD) models have been used to understand complex, multi-faceted dengue transmission dynamics, but a gap persists between research and actionable public health tools for decision-making. Spain is an at-risk country of imported dengue outbreaks, but only qualitative assessments are available to guide public health action and control. We propose a modular SD model combining temperature-dependent vector population, transmission parameters, and epidemiological interactions to simulate outbreaks from imported cases accounting for heterogeneous local climate-related transmission patterns. Under our assumptions, 15 provinces sustain vector populations capable of generating outbreaks from imported cases, with heterogeneous risk profiles regarding seasonality, magnitude and risk window shifting from late Spring to early Autum. Results being relative to given vector-to-human populations allow flexibility when translating outcomes between geographic scales. The model and the framework are meant to serve public health by incorporating transmission dynamics and quantitative-qualitative input to the evidence-based decision-making chain. It is a flexible tool that can easily adapt to changing contexts, parametrizations and epidemiological settings thanks to the modular approach.

Abstract:Dengue is estimated to have approximately 390 million infections annually out of which an estimated 96 million manifest (Bhatt et al., 2013). WHO estimates almost half the global population to be at risk from this neglected tropical infectious disease. The enormous economic burden of the disease is evident when considering that an estimated 264 disability-adjusted life years (DALYs) per million population is lost due to dengue each year (World Health Organization, 2012). In Sri Lanka, it was estimated that within the Colombo district, where the nation’s capital is situated, a financial burden of US$ 971,360 was imposed upon the national health system in 2012 alone just for the execution of preventive measures (Thalagala et al., 2016). Considering all of these factors, optimizing resource allocation and reducing the economic burden of dengue should be a key element of any long term strategy for dealing with the disease. In this context, the ability to predict dengue outbreaks for a particular region 2 weeks in advance would lead to better resource mobilization and would be invaluable asset for the public health sector. Additionally, a disease outbreak forecasting model developed for dengue need not be limited to tackling resource allocation for that disease only, but can also be used to forecast outbreaks of other arboviral diseases such as Ebola, Zika or Chikungunya. In Sri Lanka at least, Chikungunya is already prevalent and it is just a matter of time before Zika comes to Sri Lanka given that cases have been detected in the Asian region including Singapore. In an epidemic or outbreak of an infectious disease, we would need to develop a deep understanding of human mobility patterns within the infected regions to identify the potential hotspots and also to forecast to which regions are most likely to be infected next. For the purpose of understanding human mobility in disease propagation, Mobile Network Big Data (MNBD) has become a low cost data exhaust that provides rich insight into human mobility patterns with better spatial and temporal granularity when compared to statistical methods which rely mostly on macro level population parameters. In this work, we evaluate multiple machine learning techniques such as Neural Networks (NN), Support Vector Machines (SVM), Random Forests and XGBoost to determine which technique performs best. A comparison of the model performance between different techniques is provided. We go on to use a genetic algorithm based optimization to further improve the accuracy of these models. Our work shows that Call Detail Records (CDR) can be used to derive proxy indicators for human movement patterns which are applicable across multiple machine learning models. Our results show that human mobility has an impact on dengue incidence, even in dengue endemic regions. The forecasting models developed in this work can be utilized to effect a significant impact on the issue of allocating resources effectively to combat dengue which in turn would lead to reduced economic burden as well as reduced mortality and morbidity.
Estimating the Attack Ratio of Dengue Epidemics under Time-varying Force of Infection using Aggregated Notification Data

Abstract:Quantifying the attack ratio of disease is key to epidemiological inference and Public Health planning. For multi-serotype pathogens, however, different levels of serotype-specific immunity make it difficult to assess the population at risk. In this paper we propose a Bayesian method for estimation of the attack ratio of an epidemic and the initial fraction of susceptibles using aggregated incidence data. We derive the probability distribution of the effective reproductive number, R t , and use MCMC to obtain posterior distributions of the parameters of a single-strain SIR transmission model with time-varying force of infection. Our method is showcased in a data set consisting of 18 years of dengue incidence in the city of Rio de Janeiro, Brazil. We demonstrate that it is possible to learn about the initial fraction of susceptibles and the attack ratio even in the absence of serotype specific data. On the other hand, the information provided by this approach is limited, stressing the need for detailed serological surveys to characterise the distribution of serotype-specific immunity in the population.

Abstract:Dengue, a zoonotic viral disease transmitted by Aedes mosquitoes, poses a significant public health concern throughout the Lao People's Democratic Republic (Lao PDR). This study aimed to describe spatial–temporal patterns and quantify the effects of environmental and climate variables on dengue transmission at the district level. The dengue data from 2015 to 2020 across 148 districts of Lao PDR were obtained from the Lao PDR National Center for Laboratory and Epidemiology (NCLE). The association between monthly dengue occurrences and environmental and climate variations was investigated using a multivariable Zero-inflated Poisson regression model developed in a Bayesian framework. The study analyzed a total of 72,471 dengue cases with an incidence rate of 174 per 100,000 population. Each year, incidence peaked from June to September and a large spike was observed in 2019. The Bayesian spatio-temporal model revealed a 9.1% decrease (95% credible interval [CrI] 8.9%, 9.2%) in dengue incidence for a 0.1 unit increase in monthly normalized difference vegetation index at a 1-month lag and a 5.7% decrease (95% CrI 5.3%, 6.2%) for a 1 cm increase in monthly precipitation at a 6-month lag. Conversely, dengue incidence increased by 43% (95% CrI 41%, 45%) for a 1 °C increase in monthly mean temperature at a 3-month lag. After accounting for covariates, the most significant high-risk spatial clusters were detected in the southern regions of Lao PDR. Probability analysis highlighted elevated trends in 45 districts, emphasizing the importance of targeted control strategies in high-risk areas. This research underscores the impact of climate and environmental factors on dengue transmission, emphasizing the need for proactive public health interventions tailored to specific contexts in Lao PDR.

Abstract:Aedes aegypti is a known vector of Dengue, Chikungunya and Zika and the goal of this study is to propose the first mathematical model to describe the dynamic transmission of these three diseases. We present two preliminary models that consist of the SEIR model for the human populations and an SEI model for the vector to describe (a) the single transmission dynamics of dengue, Chikungunya or Zika, and (b) any possible coinfection between two diseases in the same population. In order to do that, we obtain an analytical solution of the system of 17 and 30 coupled differential equations for each model respectively, and later obtain the eigenvalues by analyzing the Jacobian matrix in order to begin the development of a surveillance system to prevent the spread of these three diseases.

Abstract:Understanding the past, current, and future dynamics of dengue epidemics is challenging yet increasingly important. To date, many techniques across statistics, mathematics, and machine learning have provided us with quantitative tools for studying dengue epidemics. Here, using data from provinces in northern Peru across 2010 to 2021, we provide a new interdisciplinary pipeline that draws on a new and existing techniques to provide comprehensive understanding and robust prediction of dengue epidemic dynamics. Wavelet analyses can unveil spatiotemporal patterns in epidemic dynamics across annual and multi- annual time periods. Here, these included climatic forcing and greater spatial similarity in large outbreak years. Space-varying epidemic drivers included climatic influences and shorter pairwise distances driv- ing greater epidemic similarity in more northerly coastal provinces. Then, using a Bayesian model, we can probabilistically quantify the timing, structure, and intensity of such climatic influences on Dengue Incidence Rates (DIRs), while simultaneously considering other influences. Recognising that a single model is generally sub-optimal for any forecasting task, we demonstrate how to form trained and untrained probabilistic ensembles for forecasting dengue cases in settings reflective of real-world conditions. We introduce a suite of climate-informed and covariate-free deep learning approaches that leverage big data and foundational time series, temporal convolutional networks, and conformal infer- ence. We complement these modern techniques with statistically principled training and assessment of ensemble frameworks, while explicitly considering strong benchmark models, computational costs, pub- lic health priorities, and data availability limitations. In doing so, we show how ensemble frameworks consistently outperform individual models across space and time, and produce sharp and accurate fore- casts with robust, reliable descriptions of uncertainty. We report interpretable classification metrics for detection of outbreaks to communicate our outputs with the wider public and public health authorities. Looking forward, whether the objective is to understand and/or to predict epidemic dynamics, our modelling pipeline can be used in any dengue setting to robustly inform the decision-making and planning of public health authorities.

Abstract:Background: Dengue is the fastest spreading vector-borne viral disease, resulting in an estimated 390 million infections annually. Precise prediction of many attributes related to dengue is still a challenge due to the complex dynamics of the disease. Important attributes to predict include: the risk of and risk factors for an infection; infection severity; and the timing and magnitude of outbreaks. In this work, we build a model for predicting the risk of dengue transmission using high-resolution weather data. The level of dengue transmission risk depends on the vector density, hence we predict risk via vector prediction. Methods and findings: We make use of surveillance data on Aedes aegypti larvae collected by the Taiwan Centers for Disease Control as part of the national routine entomological surveillance of dengue, and weather data simulated using the IBM's Containerized Forecasting Workflow, a high spatial- and temporal-resolution forecasting system. We propose a two stage risk prediction system for assessing dengue transmission via Aedes aegypti mosquitoes. In stage one, we perform a logistic regression to determine whether larvae are present or absent at the locations of interest using weather attributes as the explanatory variables. The results are then aggregated to an administrative division, with presence in the division determined by a threshold percentage of larvae positive locations resulting from a bootstrap approach. In stage two, larvae counts are estimated for the predicted larvae positive divisions from stage one, using a zero-inflated negative binomial model. This model identifies the larvae positive locations with 71% accuracy and predicts the larvae numbers producing a coverage probability of 98% over 95% nominal prediction intervals. This two-stage model improves the overall accuracy of identifying larvae positive locations by 29%, and the mean squared error of predicted larvae numbers by 9.6%, against a single-stage approach which uses a zero-inflated binomial regression approach. Conclusions: We demonstrate a risk prediction system using high resolution weather data can provide valuable insight to the distribution of risk over a geographical region. The work also shows that a two-stage approach is beneficial in predicting risk in non-homogeneous regions, where the risk is localised.

Abstract:Background: Disease surveillance systems are essential for effective disease intervention and control by monitoring disease prevalence as time series. To evaluate the severity of an epidemic, statistical methods are widely used to forecast the trend, seasonality, and the possible number of infections of a disease. However, most statistical methods are limited in revealing the underlying dynamics of disease transmission, which may be affected by various impact factors, such as environmental, meteorological, and physiological factors. In this study, we focus on investigating malaria transmission dynamics based on time series data. Methods: A data-driven nonlinear stochastic model is proposed to infer and predict the dynamics of malaria transmission based on the time series of prevalence data. Specifically, the dynamics of malaria transmission is modeled based on the notion of vectorial capacity (VCAP) and entomological inoculation rate (EIR). A particle Markov chain Monte Carlo (PMCMC) method is employed to estimate the model parameters. Accordingly, a one-step-ahead prediction method is proposed to project the number of future malaria infections. Finally, two case studies are carried out on the inference and prediction of Plasmodium vivax transmission in Tengchong and Longling, Yunnan province, China. Results: The results show that the trained data-driven stochastic model can well fit the historical time series of P. vivax prevalence data in both counties from 2007 to 2010. Moreover, with well-trained model parameters, the proposed one-step-ahead prediction method can achieve better performances than that of the seasonal autoregressive integrated moving average model with respect to predicting the number of future malaria infections. Conclusions: By involving dynamically changing impact factors, the proposed data-driven model together with the PMCMC method can successfully (i) depict the dynamics of malaria transmission, and (ii) achieve accurate one-step-ahead prediction about malaria infections. Such a data-driven method has the potential to investigate malaria transmission dynamics in other malaria-endemic countries/regions.

Abstract:Vector-transmitted diseases such as dengue fever and chikungunya have been spreading rapidly in many parts of the world. The Zika virus has been known since 1947 and invaded South America in 2013. It can be transmitted not only by (mosquito) vectors but also directly through sexual contact. Zika has developed into a serious global health problem because, while most cases are asymptomatic or very light, babies born to Zika - infected mothers may develop microcephaly and other very serious birth defects. We formulate and analyze two epidemic models for vector-transmitted diseases, one appropriate for dengue and chikungunya fever outbreaks and one that includes direct transmission appropriate for Zika virus outbreaks. This is especially important because the Zika virus is the first example of a disease that can be spread both indirectly through a vector and directly (through sexual contact). In both cases, we obtain expressions for the basic reproduction number and show how to use the initial exponential growth rate to estimate the basic reproduction number. However, for the model that includes direct transmission some additional data would be needed to identify the fraction of cases transmitted directly. Data for the 2015 Zika virus outbreak in Barranquilla, Colombia has been used to fit parameters to the model developed here and to estimate the basic reproduction number.

The Critical Role of Early Dengue Surveillance and Limitations of Clinical Reporting - Implications for Non-Endemic Countries

Abstract:The increasing dengue burden and epidemic severity worldwide have highlighted the need to improve surveillance. In non-endemic areas such as Taiwan, where outbreaks start mostly with imported cases from Southeast Asia, a closer examination of surveillance dynamics to detect cases early is necessary. To evaluate problems with dengue surveillance and investigate the involvement of different factors at various epidemic stages, we investigated 632 laboratory-confirmed indigenous dengue cases in Kaohsiung City, Taiwan during 2009-2010. The estimated sensitivity of clinical surveillance was 82.4% (521/632). Initially, the modified serological surveillance (targeting only the contacts of laboratory-confirmed dengue cases) identified clinically unrecognized afebrile cases in younger patients who visited private clinics and accounted for 30.4% (35/115) of the early-stage cases. Multivariate regression indicated that hospital/medical center visits [Adjusted Odds Ratio (aOR): 11.6, 95% confidence interval (CI): 6.3-21.4], middle epidemic stage [aOR: 2.4 (1.2-4.7)], fever [aOR: 2.3 (2.3-12.9)], and musculo-articular pain [aOR: 1.9 (1.05-3.3)] were significantly associated with clinical reporting. However, cases with pruritus/rash [aOR: 0.47 (0.26-0.83)] and diarrhea [aOR: 0.47 (0.26-0.85)] were underreported. In conclusion, multiple factors contributed to dengue surveillance problems. To prevent a large-scale epidemic and minimize severe dengue cases, there is a need for integrated surveillance incorporating entomological, clinical, serological, and virological surveillance systems to detect early cases, followed by immediate prevention and control measures and continuous evaluation to ensure effectiveness. This effort will be particularly important for an arbovirus, such as Zika virus, with a high asymptomatic infection ratio. For dengue- non-endemic countries, we recommend serological surveillance be implemented in areas with high Aedes mosquito indices or many breeding sites. Syndromic surveillance, spatial analysis and monitoring changes in epidemiological characteristics using a geographical information system, as well as epidemic prediction models involving epidemiological, meteorological and environmental variables will be helpful for early risk communication to increase awareness.

Abstract:Effective public health strategies for preventing infectious diseases require a deep understanding of transmission dynamics. However, epidemiological data alone is often insufficient for guiding public health action, as transmission patterns emerge from complex human-environment interactions. Additionally, collecting empirical data is frequently limited by cost and logistical challenges, especially in low-resource settings. Consequently, health policy increasingly relies on inference models that account for spatial and temporal dimensions, confounding factors, and uncertainty. In this work, we introduce a novel hierarchical mechanistic Bayesian model based on ecological diffusion to improve epidemiological inference for spreading pathogens. This model is specifically designed to reveal underlying spatiotemporal dynamics from empirical data, explicitly accounting for sampling bias and uncertainty. To demonstrate its effectiveness, we apply the model to simulated data of a drug-resistant pathogen and formally compare its performance to state-of-the-art epidemiological regression models. Our findings show that the model accurately identifies transmission patterns and is particularly effective at pinpointing transmission hotspots and predicting spread pathways - critical capabilities for controlling drug-resistant pathogens. While reliable monitoring remains challenging in low-resource environments, this model addresses demographic sampling bias and spatiotemporal complexities, though concentrated sampling in presumed high-risk areas can sometimes distort transmission insights. The proposed framework can inform targeted public health interventions in the early stages of resistance emergence, offering valuable guidance for resource-constrained settings.

Abstract:Dengue fever, a tropical vector-borne disease, is a leading cause of hospitalization and death in many parts of the world, especially in Asia and Latin America. In places where timely and accurate dengue activity surveillance is available, decision-makers possess valuable information that may allow them to better design and implement public health measures, and improve the allocation of limited public health resources. In addition, robust and reliable near-term forecasts of likely epidemic outcomes may further help anticipate increased demand on healthcare infrastructure and may promote a culture of preparedness. Here, we propose ensemble modeling approaches that combine forecasts produced with a variety of independent mechanistic, statistical, and machine learning component models to forecast reported dengue case counts 1-, 2-, and 3-months ahead of current time at the province level in multiple countries. We assess the ensemble and each component models' monthly predictive ability in a fully out-of-sample and retrospective fashion, in over 180 locations around the world - all provinces of Brazil, Colombia, Malaysia, Mexico, and Thailand, as well as Iquitos, Peru, and San Juan, Puerto Rico - during at least 2-3 years. Additionally, we evaluate ensemble approaches in a multi-model, real-time, and prospective dengue forecasting platform - where issues of data availability and data completeness introduce important limitations - during an 11-month time period in the years 2022 and 2023. We show that our ensemble modeling approaches lead to reliable and robust prediction estimates when compared to baseline estimates produced with available information at the time of prediction. This can be contrasted with the high variability in the forecasting ability of each individual component model, across locations and time. Furthermore, we find that no individual model leads to optimal and robust predictions across time horizons and locations, and while the ensemble models do not always achieve the best prediction performance in any given location, they consistently provide reliable disease estimates - they rank in the top 3 performing models across locations and time periods - both retrospectively and prospectively.

Abstract:Abstract Timely and accurate knowledge of Dengue incidence is of value to public health professionals because it helps to enable the precise communication of risk, improved allocation of resources to potential interventions, and improved planning for the provision of clinical care of severe cases. Therefore, many national public health organizations make local Dengue incidence data publicly available for individuals and organizations to use to manage current risk. The availability of these data has also resulted in active research into the forecasting of Dengue incidence as a way to increase the public health value of incidence data. Here, we robustly assess time-series-based forecasting approaches against a null model (historical average incidence) for the forecasting of incidence up to four months ahead. We used publicly available data from multiple countries: Brazil, Mexico, Singapore, Sri Lanka, and Thailand; and found that our time series methods are more accurate than the null model across all populations, especially for 1-and 2-month ahead forecasts. We tested whether the inclusion of climatic data improved forecast accuracy and found only modest, if any improvements. We also tested whether national timeseries forecasts are more accurate if made from aggregate sub-national forecasts, and found mixed results. We used our forecasting results to illustrate the high value of increased reporting speed. This framework and test data are available as an R package. The non-mechanistic approaches described here motivates further research into the use of disease-dynamic models to increase the accuracy of medium-term Dengue forecasting across multiple populations. Author summary Dengue is a mosquito-borne disease caused by the Dengue virus. Since the Second World War it has evolved into a global problem, securing a foothold in more than 100 countries. Each year, hundreds of millions of people become infected, and upwards of 10,000 die from the disease. Thus, being able to accurately forecast the number of cases likely to emerge in particular locations is vital for public health professionals to be able to develop appropriate plans. In this study, we have refined a technique that allows us to forecast the number of cases of Dengue in a particular location, up to four months in advance. We test the approach using state-level and national-level data from Brazil, Mexico, Singapore, Sri Lanka, and Thailand. We found that the model can generally make useful forecasts, particularly on a two-month horizon. We tested whether information about climatic conditions improved the forecast, and found only modest improvements to the forecast. Our results highlight the need for both timely and accurate reports. We also anticipate that this approach may be more generally useful to the scientific community; thus, we are releasing a framework, which will allow interested parties to replicate our work, as well as apply it to other sources of Dengue data, as well as other infectious diseases in general.
Distribution of components of basal lamina and dystrophin-dystroglycan complex in the rat pineal gland: differences from the brain tissue and between the subdivisions of the gland.

Abstract:The pineal gland is an evagination of the brain tissue, a circumventricular neuroendocrine organ. Our immunohistochemical study investigates basal lamina components (laminin, agrin, perlecan, fibronectin), their receptor, the dystrophin-dystroglycan complex (beta-dystroglycan, dystrophin utrophin), aquaporins (-4,-9) and cellular markers (S100, neurofilament, GFAP, glutamine synthetase) in the adult rat corpus pineale. The aim was to compare the immunohistochemical features of the cerebral and pineal vessels and their environment, and to compare their features in the distal and proximal subdivisions of the so-called 'superficial pineal gland'. In contrast to the cerebral vessels, pineal vessels proved to be immunonegative to alpha1-dystrobrevin, but immunoreactive to laminin. An inner, dense, and an outer, loose layer of laminin as two basal laminae were present. The gap between them contained agrin and perlecan. Basal lamina components enmeshed the pinealocytes, too. Components of dystrophin-dystroglycan complex were also distributed along the vessels. Dystrophin, utrophin and agrin gave a 'patchy' distribution rather than a continuous one. The vessels were interconnected by wing-like structures, composed of basal lamina-components: a delicate network forming nests for cells. Cells immunostained with glutamine synthetase, S100-protein or neurofilament protein contacted the vessels, as well as GFAP- or aquaporin-immunostained astrocytes. Within the body a smaller, proximal, GFAP-and aquaporin-containing subdivision, and a larger, distal, GFAP-and aquaporin-free subdivision could be distinguished. The vascular localization of agrin and utrophin, as well as dystrophin, delineated vessels unequally, preferring the proximal or distal end of the body, respectively.
