Abstract:Throughout history, the maritime nations of the world have employed surgeons in seagoing service. The history of women in surgery and the military is well described, but no prior report exists describing the gender breakdown of past and current sea-going U.S. Navy general surgeons. Using literature review, primary sources, personal interviews, and correspondence with Navy Medicine administrative leaders, this historical review describes the evolution of women providing surgical care at sea. During the 1800s and early 1900s, some of the earliest women surgeons in England, America, and Russia began their surgical careers in military service or providing combat casualty care. Women at sea served unpaid nursing roles in the 1700s and provided informal medical care in the 1800s. In 1913 and 1941 two different women held medical leadership roles aboard seagoing vessels. Four years after Congress allowed women to serve aboard combatant vessels, Dr Beth Jaklic became the first woman to serve as Ship’s Surgeon aboard a U.S. Navy warship in 1997. From 1997-2020, 19% of surgeons serving on aircraft carriers were women and one-half of the 20 general surgeons assigned to maritime surgical billets were women in 2022. War and the military environment historically have offered opportunities for women to break boundaries in the world of surgery. Navy Medicine’s experience with women surgeons at sea serves as a positive example to the broader surgical community, especially “austere” practices and subspecialties with limited female representation.

Abstract:Various well-known people associated with the history of the presidency of the United States have experienced neurologic disease or injury, especially trauma to the head or spine. Nancy Reagan, however, as the wife of President Ronald Reagan and First Lady, would leave a significant and lasting mark on the progress of neurosurgical science and education. Recognized for endeavors against drug abuse, Alzheimer disease, and polio, her interest in neurosurgical research is less well known. Nancy's father Loyal Davis was a remarkable neurosurgeon and educator of extraordinary influence. When Barrow Neurological Institute (BNI) founder John Green experienced complications after an illness, Davis served as BNI director during 1966 - 1967. After Davis's death in 1982, Robert Spetzler, who had been a student of Davis at Northwestern University Medical School and was then BNI director, convinced Green, despite his misgivings, to support a neurosurgical laboratory recognizing Davis. In 1988, Nancy Reagan, then First Lady, dedicated the Loyal and Edith Davis Neurosurgical Research Laboratory. At the dedication, she remarked on her years growing up in the home of a pioneering neurosurgeon and remarked that "my father believed deeply in the importance of research to develop new methods for treating patients." Green and Spetzler's unified efforts honored the extraordinary career of Davis in a manner he would have appreciated, were supported by a First Lady with deep involvement in politics and philanthropy dedicated to promoting advances in medicine, and are part of neurosurgery's unique heritage.

Abstract:Abstract Greta Gerwig’s 2023 Barbie blockbuster provided an infectious reminder that women are capable of the success of their male counterparts, as well as emphasizing the impressiveness of female empowerment and leadership. Luckily, in dermatology, we have had this leadership from the beginning. According to the 2022 UK consensus of consultant physicians, women make up 51% of higher specialty trainees, and there have been more women than men in training since 2013. However, despite this, only 41% of the consultant physician workforce are women while 59% are men (Royal College of Physicians of Edinburgh. Focus on Physicians. Census of Consultant Physicians and Higher Specialty Trainees in the UK 2016–17. Edinburgh: Royal College of Physicians, 2017). However, dermatology stands out as a female-predominant area of specialization. In the UK in 2016, 57% of consultants and 75% of higher specialty trainees in dermatology were women [Royal College of Physicians. The UK 2022 census of consultant physicians. There is likely a multitude of reasons contributing to this, but the historical influence of prominent female leaders in the specialty cannot be ignored. Presented here are a select group of pioneering female dermatologists whose careers and contributions to the field of dermatology paved the way for the development of one of the few female-dominated medical specialties. Helen Ollendorff-Curth was a female pioneer of genodermatology and is commemorated with four eponyms: the Ollendorff probe sign, the Curth criteria, Buschke–Ollendorff syndrome, and ichthyosis hystrix, Curth–Macklin type (IHCM). Loretta Joy Cummins was the first woman to pass the Dermatology Board examination in the USA and the first to be president of the New England Dermatological Society. Cummins founded a fund for Massachusetts General Hospital and a scholarship fund for women at Tufts Medical School. Agnes Blackadder was the first female consultant dermatologist in the UK, when she was appointed consultant dermatologist at St John’s Hospital, London in 1907. Daisy Maude Orleman Robinson’s achievements include becoming the first female dermatologist in the USA in 1905, as well as being the first female dermatologist to present a case at a dermatological meeting, to publish a scholarly paper in dermatology, and to present a case at an international dermatology meeting. These influential women were catalysts for creating and cultivating a specialty that promotes female leadership in the world of dermatology, medicine and science.

"Who would know better than the girls in white?" Nurses as experts in postwar magazine advertising, 1945-1950

Abstract:American advertising in the period immediately following the Second World War portrayed nurses as trusted advisers and capable professionals and frequently pictured them performing skilled work, including dispensing medicine and assisting in surgery. Advertisements published in a range of magazines whose target audiences varied by gender, race, age, and class show that nurses in postwar advertisements embodied two broad categories of representation: (a) medical authority, scientific progress, and hospital cleanliness; and (b) feminine expertise, especially in female and family health. Typically portrayed as young white women--although older nurses were occasionally depicted and black nurses appeared in advertisements intended for black audiences-nurses were especially prominent in advertisements for menstrual and beauty products, as well as products related to children's health. Although previous scholarly examinations of nurses in postwar mass media have emphasized their portrayal as hypersexual and incompetent, this investigation posits postwar advertising as a forum that emphasized nurses' professionalism, as well as complex expectations surrounding women's professional and domestic roles.

Abstract:Nurses running hospitals is not new. Florence Nightingale could be considered the first hospital administrator. What is changing is the growth of RN chief executive officers (CEOs), from 10% in 2004 to 18% in 2010. Furthermore, nearly 20% of chief nursing officers (CNOs) aspire to be CEOs. Is this a natural growth of CNO to vice president of patient care services to chief operating officer...to RN CEO? The research on RN CEOs is very small. Therefore, the author set out to obtain a journalistic snapshot of Arizona's 12 hospital RN CEOs through interviews. Of the 12, 3 were corporate CEOs at the system level, they saw over several hospitals, and 9 were CEOs over 1 to 2 hospitals. This article discusses some of the finding from these interviews.

Abstract:Abstract Agnes Blackadder, born in Dundee on 4 December 1875, is widely credited as the first female consultant dermatologist in Britain. In 1895 she graduated from St Andrews University, the first woman to do so, with a Master of Arts. Following this she matriculated at Queen Margaret College in Glasgow, going on to obtain her medical degree in 1901. She was known to be a particularly voracious learner, taking multiple subjects simultaneously during her time at St Andrews, resulting in her graduating prematurely. Furthermore, during her medical education she obtained prizes across multiple fields, such as surgery, midwifery and ophthalmology. In 1907 Blackadder moved to London, where she was appointed as a consultant dermatologist at St John’s Hospital for Skin Diseases. Not only is she considered to be the first female consultant dermatologist in Britain, but she was among the first female physicians to be appointed as a consultant in a hospital serving both male and female patients. Her very presence within the medical community was, at the time, considered to be radical. Moreover, her interest in dermatology, radiology and electrotherapeutics diverged from those considered to be more acceptable for female physicians, namely paediatrics and women’s health. Central to Blackadder’s life was her advocacy for gender equality, frequently highlighting the persistent gender disparities in the fields of both medicine and academia. In 1912 she worked as part of the medical team who assessed the impact of force feeding of imprisoned suffragettes participating in hunger strikes. She published her findings, describing this practice as ‘severe physical and mental torture’, and strongly advised against its use. During World War I she served at Royaumont Hospital in France, working as a radiographer. Her research interests continued to be both progressive and wide ranging; she was among the first to identify the radiographic appearances of gas gangrene, and used this to help improve diagnostic accuracy. She observed the impact of music on health and documented its therapeutic implications. She published Music, Health and Character soon after her return to London, and this resulted in the formation of the Council for Music in Hospitals. The exceptional career of Blackadder spanned over 50 years and she continued to practise medicine into her eighth decade before she passed away in 1964, aged 88. Her legacy has left an indelible mark on both medicine and women’s rights. She succeeded in creating space for women in the fields of dermatology and academia, paving the way for subsequent generations to pursue their aspirations.

Abstract:Education of health and social care professionals is essential in preparation to confidently and effectively support families affected by dementia to undertake advance care planning. This article describes a training needs analysis of Admiral Nurses, dementia specialists, in facilitating advance care planning for future care. Methods: A questionnaire survey was completed by Admiral Nurses attending end-of-life care masterclasses in 2017 and 2018. Both quantitative (years registered as a nurse, years as an Admiral Nurse and subjective level of confidence in completing advance care plannings) and qualitative data (interventions perceived to increase confidence) were collected. Findings: There were 75 completed responses (two incomplete returns). There was no correlation between levels of confidence and years registered as a nurse. However, there was a small positive correlation between confidence and number of years as an Admiral Nurse ( r = 0.23; p < 0.05). Themes identified qualitatively to enhance practice were advanced communication skills, supervised practice, resources to frame conversations and a guide and template for advance care planning. Conclusion: Although Admiral Nurses are specialists in dementia, several educational initiatives could be employed to better enable them to support advance care planning for families affected by dementia. This training needs analysis is contributed towards developing an educational intervention for Admiral Nurses to improve advance care planning support.

Abstract:Dr. Marilyn A. Ray, nurse scholar and retired United States Air Force (USAF) veteran and former flight nurse, began her nursing scholarship in Canada. She was influenced by the experiences and interprofessional scholarly ideas that she encountered along her career trajectory. Her early love of the air and space led her to the United States Air Force Nurse Corps, where she served as a flight nurse during the Vietnam war era, followed by leadership positions in nursing education, administration, practice, and research. Dr. Ray's contributions to nursing knowledge includes two nursing theories and a caring inquiry methodology. Dr. Ray is helping to create a new caring science certificate program at Florida Atlantic University, Christine E. Lynn College of Nursing. In this column, Dr. Ray shares the story of her scholarly influences and how they helped her care for her husband and gain insight into her contributions to nursing knowledge development.

Abstract:BACKGROUND: Women are underrepresented in medicine and academic anesthesiology, and especially in leadership positions. We sought to characterize career achievement milestones of female versus male academic anesthesiology chairs to understand possible gender-related differences in pathways to leadership. METHODS: We conducted a retrospective observational cross-sectional analysis. In November 2019, curricula vitae (CVs) were requested from then-current members of the US Association of Academic Anesthesiology Chairs. Data reflecting accomplishments up to the time of chair appointment were systematically extracted from CVs and analyzed using a mixed methods approach with qualitative content analysis supplemented by descriptive statistics and bivariate statistical testing. Missing data were not imputed. RESULTS: Seventy-two CVs were received from eligible individuals (response rate 67.3%). The respondent sample was 12.5% women (n = 9), 87.5% men (n = 63), and no transgender or nonbinary people; this is similar to the known gender balance in anesthesiology chairs in the United States. No statistically significant differences in objective markers of academic achievement at the time of chair appointment were evident for female versus male chairs, including time elapsed between the first faculty appointment and assumption of the chair role (median 25 vs 18 years, P = .06), number of publications at the time the chair was assumed (101 vs 69, P = .28), or proportion who had ever held a National Institutes of Health (NIH) grant as principal investigator (44.4% vs 25.4%, 0.25). Four phenotypes of career paths were discernible in the data: the clinician-administrator, the educator, the investigator, and the well-rounded scholar; these did not differ by gender. CONCLUSIONS: Female chairpersons who were members of the Association of Academic Anesthesiology Chairs in the United States demonstrated similar patterns of academic achievement as compared to male chairpersons at the time the position of chair was assumed, suggesting that they were equally qualified for the role as compared to men. Four patterns of career achievements were evident in the chairperson group, suggesting multiple viable pathways to this leadership position.

Abstract:Pediatric and Developmental Pathology, Ahead of Print. Maude Abbott was a pioneering female Canadian physician who became a world authority on medical museums and congenital heart disease. Abbott spent almost all her career in highly sexist, discriminatory work environments. This paper reviews Abbott's life and accomplishments, but, more importantly, analyzes her pathway to success in the masculine world of early 20th-century academic pathology. Abbott, though well-trained as a pathologist, never provided clinical service, but instead worked as museum curator at McGill University. She established the International Association of Medical Museums (predecessor to the International Academy of Pathology), edited its journal, and essentially ran the organization. Abbott, surrounded by influential males, dealt differently with each. In general, she recognized that male doctors believed women lacked the gravitas to lead major initiatives but that she could circumnavigate this supposed impediment by co-leading projects with male counterparts, preferably ones too busy to get in her way. She repeatedly used this approach, and by doing most of the work but sharing credit, succeeded in gaining reputation, accomplishment, and advancement. Abbott's pioneering work on congenital heart disease established her as one of the founders of pediatric pathology, and, overall, her career promoted the entry of women physicians into the pathology profession.

A Life on the Frontlines: The Legacy of Norman Bethune (1890-1939)

Abstract:Henry Norman Bethune was a prominent Canadian thoracic surgeon who came to fame during the 1930s. After being made a Fellow of the Royal College of Surgeons of Edinburgh, Bethune became head of thoracic surgery in a hospital in Cartierville, Canada. During this time, he pioneered surgical techniques, published research findings, and invented surgical instruments. Not content with being only a physician, innovator, and humanitarian, Bethune also found himself in medical services on the frontlines of wars in both Spain and China. In Spain, Bethune emphasized the need for prompt blood transfusions and developed mobile blood transfusion services. After the start of the Second Sino-Japanese War, Bethune traveled to China and quickly organized a mobile operating unit. Following discussions with Chinese leaders, Bethune performed surgeries on the frontlines of conflict in northern China, where his exceptional loyalty to duty became famous throughout the region. Although he met his end at an early age due to septicemia in 1939, his medical legacy carries on in multiple countries and serves to inspire a future generation of medical practitioners.

Abstract:In 1946, Public Law 588 of the 79th Congress established the Office of Naval Research (ONR). Its mission was to plan, foster and encourage scientific research in support of Naval problems. The establishment of ONR predates the National Science Foundation and initiated the refocusing of scientific infrastructure in the United States following World War II. At the time, ONR was the only source for federal support of basic research in the United States. Dorothy Gilford was one of the first Heads of the Probability and Statistics program at the Office of Naval Research (1955 to 1962), and she went on to serve as Director of the Mathematical Sciences Division (1962 to 1968). During her time at ONR, Dorothy influenced many areas of statistics and mathematics and was ahead of her time in promoting interdisciplinary projects. Dorothy continued her career at the National Center for Education Statistics (1969 to 1974). She was active in starting international comparisons of education outcomes in different countries, which has influenced educational policy in the United States. Dorothy went on to serve in many capacities at the National Academy of Sciences, including Director of Human Resources Studies (1975 to 1978), Senior Statistician on the Committee on National Statistics (1978 to 1988) and Director of the Board on International Comparative Studies in Education (1988 to 1994). The following is a conversation we had with Dorothy Gilford in March of 2004. We found her to be an interesting person and a remarkable statistician. We hope you agree.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objective: To examine trends in the gender composition of residents and faculty in Otolaryngology-Head and Neck Surgery residency programs in the United States and to investigate the correlation between women's representation in leadership positions and the proportion of women faculty and residents. Methods: A literature review was first performed to analyze trends in the gender composition of residents and faculty in Otolaryngology-Head and Neck Surgery (OHNS) residency programs. Current residency programs were then identified using the Electronic Residency Application Service 2023 Participating Specialties and Programs website. The following data was collected from each program website: gender of associated medical school dean, gender of department chair, gender of residency program director, and total number and gender of fellowship directors, faculty, and residents. Wilcoxon rank sum test and Fischer's exact test were used to analyze relationships between the number of women in leadership positions and the proportion of women faculty and residents. Results: An increase in the number of women chairs, residency program directors, residents, and faculty over the past decade is documented across published literature. One hundred twenty-three current academic residency programs were identified. Women accounted for 42%, 30%, 27%, and 8% of current residents, residency program directors, faculty, and department chairs, respectively. Department chair gender was significantly correlated with number of women faculty (P = .01). Any women in a leadership position were correlated with a statistically significant increase in median percent of women faculty (P = .006). Conclusion: Further understanding of how the mentorship of women promotes gender equity is necessary to promote gender diversity in OHNS.

Abstract:Doreen Norton was a delightful, widely respected nurse who devoted her life to improving the care of elderly people. She researched the neglected problem of pressure sores, revolutionised their nursing care, and thus achieved international fame. Her Pressure Sore Scale was established as a management tool and is still used today. She was a key member of the design team that produced the ‘King’s Fund Bed’, researched equipment required on geriatric wards, assessed all geriatric long stay units in Scotland and established research as a valuable nursing tool within her profession and health authorities. She lectured extensively and her publications attracted worldwide acclamation. After her retirement, she was subsequently appointed to the world’s first Chair of Gerontological Nursing in Cleveland, Ohio.

Abstract:Two nurses were among the 2,200 crew and passengers on board the Titanic when it sank on its maiden voyage to New York on April 141912. Hospital matron Catherine Wallis, who worked with passengers in third class, was among the 1,500 people who died. Caroline Endres, the private nurse of a first-class passenger, survived. The familiar story of the sinking of the ship should not overshadow the personal tragedies, that still resonate with many families today.

Nevertheless, gender gaps in leadership, research publications, and compensation persist. The experience reflects a twenty-year period in dean positions at research universities in three states. The lessons learned relate to 1) forming a strategic vision, 2) using vision and strategic priorities to inform decisions and actions, and 3) communicating the vision and strategic framework. The conclusion looks to the future development of a cadre of energized and supported future leaders who will assume deanships in nursing.

Abstract:Background: Women have historically been underrepresented in surgical leadership and in specialty organizations. This study longitudinally examines representation of women across committee leadership within the American Pediatric Surgical Association (APSA). Methods: Annual APSA committee chair and vice-chair rosters (2003-2023) were obtained. If not self-reported in the membership roster, gender was classified based upon review of publicly available data. Proportions of women who served as chairs and vice-chairs were quantified by committee and by year. Results: Overall, the proportion of women serving as APSA committee chairs and vice-chairs increased from 11% to 48% during the study period (p = 0.001). In examining each position, the proportion of women chairs increased from 12% to 40% (p = 0.042), while women vice-chairs increased from 10% to 58% (p = 0.009). The committees with the highest cumulative proportion of women chairs were Benjy Brooks (100%), Diversity Equity & Inclusion (100%), Finance (100%), Global Pediatric Surgery (67%), and Wellness (67%). Four committees- Trauma, Access to Surgery for Kids, Practice, and Professional Development - were not led by a woman chair in the entire study period. Additionally, five committees that traditionally have had significant impact on organizational workflow and agendas all had cumulative proportions of women chairs of less than 50% - Education (33%), Publications (28.6%), Outcomes (19%), Surgical Quality & Safety (18.8%) and Program (9.5%). Conclusion: These results demonstrate encouraging trends in the gender diversity of APSA leadership. However, this progress does not appear to be evenly distributed; leadership of key committees continues to lack substantial women's representation. Level of evidence: III.
