Abstract:Background: Aim of this study was to define the prognostic value of stress myocardial perfusion imaging by cadmium zinc telluride (CZT) single-photon emission computed tomography (SPECT) for prediction of adverse cardiovascular events in patients with known or suspected coronary artery disease (CAD). Methods and results: Studies published until November 2022 were identified by database search. We included studies using stress myocardial perfusion imaging by CZT-SPECT to evaluate subjects with known or suspected CAD and providing primary data of adverse cardiovascular events. Total of 12 studies were finally included recruiting 36,415 patients. Pooled hazard ratio (HR) for the occurrence of adverse events was 2.17 (95% confidence interval, CI, 1.78-2.65) and heterogeneity was 66.1% (P = 0.001). Five studies reported data on adjusted HR for the occurrence of adverse events. Pooled HR was 1.69 (95% CI, 1.44-1.98) and heterogeneity was 44.9% (P = 0.123). Seven studies reported data on unadjusted HR for the occurrence of adverse events. Pooled HR was 2.72 (95% CI, 2.00-3.70). Nine studies reported data useful to calculate separately the incidence rate of adverse events in patients with abnormal and normal myocardial perfusion. Pooled incidence rate ratio was 2.38 (95% CI, 1.39-4.06) and heterogeneity was 84.6% (P < 0.001). The funnel plot showed no evidence of asymmetry (P = 0.517). At meta-regression analysis, we found an association between HR for adverse events and presence of angina symptoms and family history of CAD. Conclusions: Stress myocardial perfusion imaging by CZT-SPECT is a valuable noninvasive prognostic indicator for adverse cardiovascular events in patients with known or suspected CAD.
Head-to-head comparison of diagnostic accuracy of stress-only myocardial perfusion imaging with conventional and cadmium-zinc telluride single-photon emission computed tomography in women with suspected coronary artery disease

Abstract:Background: Breast attenuation may impact the diagnostic accuracy of stress myocardial perfusion imaging (MPI) with single-photon emission computed tomography (SPECT). We compared the performance of conventional (C)-SPECT and cadmium-zinc-telluride (CZT)-SPECT systems in women with low-intermediate likelihood of coronary artery disease (CAD). Methods and results: A total of 109 consecutive women underwent stress-optional rest MPI by both C-SPECT and CZT-SPECT. In the overall study population, a weak albeit significant correlation between total perfusion defect (TPD) measured by C-SPECT and CZT-SPECT was observed (r = 0.38, P < .001) and at Bland-Altman analysis the mean difference in TPD (C-SPECT minus CZT-SPECT) was 2.40% (P < .001). Overall concordance of semi-quantitative diagnostic performance between C-SPECT and CZT-SPECT was observed in 52 (48%) women with a κ value of 0.09. Normalcy rate was significantly higher using CZT-SPECT compared to C-SPECT (P < .001). Machine learning analysis performed through the implementation of J48 algorithm proved that CZT-SPECT has higher sensitivity, specificity, and accuracy than C-SPECT. Conclusions: In women with low-intermediate likelihood of CAD, there is a poor concordance of diagnostic performance between C-SPECT and CZT-SPECT, and CZT-SPECT allows better normalcy rate detection compared to C-SPECT.
Incremental Value of Coronary Flow Reserve Assessed by CZT-SPECT in the Diagnosis of Coronary Ar-Tery Disease

Abstract:Objective To investigate the incremental value of coronary flow reserve ( CFR) as-sessed by cadmium zinc telluride(CZT)-SPECT as an adjunct to myocardial perfusion imaging (MPI) in the diagnosis of coronary artery disease (CAD). Methods Data of 132 patients (89 males, 43 females; 40-81 years) with or suspected with CAD who successfully underwent rest and stress MPI and CFR from November 2017 to October 2018 in Zhongshan Hospital Affiliated to Fudan University were retrospectively analyzed. Based on coronary angiography (CAG) as the " gold standard" , the value of MPI and MPI+CFR in the di-agnosis of CAD was evaluated and compared. χ2 test or Fisher exact probability test was used for data analy-sis. Results Of 132 patients, 61 (46.2％) were CAD with stenosis of at least 75％ in one vessel (47.5％, 29/ 61), two vessels (34.4％, 21/ 61), or three vessels (18.0％, 11/ 61). A total of 104 (26.3％) vessels with stenosis of at least 75％, 25 (6.3％) vessels with stenosis of 65％-74％, and 30 (7.6％) vessels with stenosis of 50％-64％ were found in 396 vessels. For detecting coronary stenosis of at least 75％, the sensi-tivity and accuracy of MPI on per-patient analysis were 86.89％(53/ 61) and 68.94％(91/ 132), which in-creased to 96.72％(59/ 61; χ2 = 3.921, P<0.05) and 87.88％(116/ 132; χ2 = 13.984, P<0.01) by MPI+CFR. On per-vessel analysis, the sensitivity and accuracy of MPI were 72.12％ (75/ 104) and 77. 53％(307/ 396) and increased to 96.15％(100/ 104; χ2 = 22. 511, P<0.01) and 85.10％(337/ 396; χ2 = 7.479, P<0.05) by MPI+CFR. The sensitivity of MPI for predicting one, two, and three vessels disease were 72. 41％(21/ 29), 42.86％ (9/ 21), and 5/ 11 and were improved to 93.10％ (27/ 29; χ2 = 4.350, P =0. 037), 90.48％(19/ 21; χ2 = 10.714, P = 0.001), and 11/ 11 (P = 0.012) by MPI+CFR. For coronary with stenosis of 65％-74％, the sensitivity of MPI was 24. 00％(6/ 25) and was improved to 64.00％(16/25; χ2 = 8.117, P= 0.004) by MPI+CFR. For coronary with stenosis of 50％-64％, the sensitivity of MPI was 40.00％(12/ 30) and was improved to 76.67％(23/ 30; χ2 = 8.297, P= 0.004) by MPI+CFR. Conclu-sion As an adjunct to MPI, CFR can significantly improve the sensitivity and accuracy in the diagnosis of CAD, particularly for patients with mild stenosis and multivessel CAD.

Abstract:We measured myocardial blood flow (MBF) and myocardial perfusion reserve (MPR) by a dynamic low-dose CZT-SPECT protocol in patients with suspected or known coronary artery disease (CAD) and investigated the capability of dynamic data in predicting obstructive CAD. A total of 173 patients with suspected or known CAD underwent dynamic CZT-SPECT after the injection of 155 MBq and 370 MBq of 99mTc-sestamibi for rest and stress imaging, respectively. Standard rest and stress imaging were performed at the end of each dynamic scan. A total perfusion defect (TPD) < 5% were considered normal. Obstructive CAD was defined as ≥ 70% stenosis at coronary angiography.

Abstract:Purpose: Left ventricular ejection fraction (LVEF) strongly predicts cardiac events. However, conflicting findings exist regarding the prognostic value of the LVEF reserve (ΔLVEF) when measured by gated single-photon emission computed tomography myocardial perfusion imaging (SPECT G-MPI). In particular, data related to the prognostic value of ΔLVEF when measured by SPECT in patients with reduced LVEF are scarce. In this study, we aimed to evaluate the prognostic value of ΔLVEF when acquired by SPECT G-MPI in patients with coronary artery disease (CAD) and a LVEF Stress < 60%. Methods: We retrospectively recruited 260 consecutive patients diagnosed with CAD by coronary angiography (CAG) and a LVEF Stress 0% and ΔLVEF ≤ 0%), and survival analyses were conducted. The primary endpoints were major adverse cardiac events (MACEs), a composite of all-cause death, nonfatal myocardial infarction, unplanned coronary revascularization, and hospitalization for unstable angina. Results: We observed 69 MACEs (26.5%). The cumulative incidence of MACEs in patients with ΔLVEF ≤ 0% was significantly higher than in patients with ΔLVEF > 0% ( P = 0.042). Multivariate Cox regression further revealed that a ΔLVEF ≤ 0% represented an independent predictor of MACEs (adjusted hazard ratio [HR]: 1.276; 95% confidence interval [CI]: (1.006, 1.618), P = 0.045). Adding a ΔLVEF ≤ 0% to traditional myocardial perfusion and function variables evaluated by MPI significantly improved the ability to predict MACEs ( P = 0.044). Conclusions: Determining ΔLVEF by SPECT G-MPI was associated with MACEs and improved risk stratification compared to prediction models based on traditional perfusion and functional parameters in CAD patients with left ventricular dysfunction, particularly those with no or mild myocardial ischemia.

Incremental prognostic value of stress/rest gated perfusion SPECT in patients with coronary artery disease--subanalysis of the J-ACCESS study

Abstract:Background: This study aimed to reveal the incremental prognostic implications of perfusion/function variables by stress/rest gated single-photon emission computed tomography (SPECT) over clinical risks in patients with known coronary artery disease (CAD). Methods and results: Using the Japanese Assessment of Cardiac Events and Survival Study by Quantitative Gated SPECT (J-ACCESS) database, the 3-year follow-up data of 2,200 patients who had established CAD were analyzed. Major cardiac events (cardiac death, myocardial infarction, heart failure, and unstable angina) were observed in 167 (7.6%) patients. Multivariate logistic regression analysis identified peripheral artery disease, diabetes mellitus, no use of statins, typical chest pain, pharmacological stress test, heart rate at rest, left ventricular end-systolic volume index derived from gated SPECT (LVESVI), and summed difference score (SDS) as independent significant predictors of the major cardiac events, with odds ratios of 1.025 to 2.291 (P=0.0309-0.0008). Global chi-square values increased by combining the independent predictors, and the greatest values (nearly 110) were observed when LVESVI or SDS was added to the pre-scan clinical information. Conclusions: Perfusion/function measures by stress/rest gated SPECT contribute to a significant improvement in risk stratification and secondary prevention strategy in combination with pre-scan clinical risks in patients with known CAD.

Abstract:Objective To prospectively compare cadmium-zinc-telluride (CZT) SPECT gated myocardial perfusion imaging (GMPI),conventional SPECT MPI and cardiac MRI for the assessment of left ventricular volume and ejection fraction in patients with heart failure.Methods From July 2016 to October 2016,a total of 35 patients (27 males,8 females,average age:(52.7±14.9) years) with heart failure were consecutively included.All patients underwent CZT SPECT GMPI,conventional SPECT GMPI and cardiac MRI within 7 d.LVEDV,LVESV and LVEF of three imaging modalities were calculated.One-way analysis of variance,Pearson correlation analysis and Bland-Altman analysis were used.Results CZT SPECT showed excellent correlation with conventional SPECT for LVEDV,LVESV and LVEF (r values:0.983,0.986 and 0.910,respectively;all P＜0.001).Bland-Altman analysis revealed good agreement between CZT SPECT and conventional SPECT for LVEDV,LVESV and LVEF.The correlation between CZT SPECT and cardiac MRI for LVEDV,LVESV and LVEF were all significant (r values:0.864,0.896 and 0.836,respectively;all P＜0.001).Compared with cardiac MRI,CZT SPECT showed systemic underestimation of LVEDV and LVESV and good agreement of LVEF by Bland-Altman analysis.Conclusions CZT SPECT has high clinical value for patients with heart failure.Despite underestimating LVEDV and LVESV,it correlated well with cardiac MRI.It also has a good agreement with conventional SPECT on left ventricular volume and LVEF.

Abstract:Background: Cadmium-zinc-telluride (CZT)-based detectors exhibit higher diagnostic sensitivity in myocardial perfusion imaging (MPI) than conventional Anger-MPI for detection of coronary artery disease (CAD); however, reduced specificity and diagnostic accuracy of CZT-MPI were observed. This study aims to compare these different camera systems and to examine the degree of inter-rater reproducibility among readers with varying experience in MPI. Methods: 83 patients who underwent double stress/rest examinations using both a CZT and conventional SPECT cameras within one visit were included. Anonymized and randomized MPI-images were distributed to 15 international readers using a standardized questionnaire. Subsequent coronary angiography findings of ten patients served as a reference for analysis of sensitivity and specificity. Results: Image quality was significantly better in CZT-MPI with significantly lower breast attenuation (P < 0.05). CZT-MPI exhibited higher sensitivity than Anger-MPI (87.5% vs. 62.5%) and significantly reduced specificity (40% vs. 100%). Readers experienced with both camera systems had the highest inter-rater agreement indicating higher reproducibility (CZT 0.54 vs. conv. 0.49, P < 0.05). Conclusions: Higher diagnostic sensitivity of CZT-MPI offers advantages in detection of CAD yet potentially of at the cost of reduced specificity, therefore it requires special training and a differentiated evaluation approach, especially for non-experienced readers with such camera systems.

Abstract:Abstract Background Maximal hyperemia capacity and myocardial flow reserve (MFR) have emerged as critical indicators of cardiovascular prognosis, with reduced vascular functionality associated with adverse outcomes (1). PET-CT serves as the gold standard for MFR assessment in clinical practice. In recent years, SPECT equipment with high-performance CZT detectors has been utilized for quantitative assessment of coronary flow at rest and under maximal hyperemia. The potential of MFR in guiding clinical decision-making for coronary artery disease (CAD) management is increasingly recognized. However, the clinical feasibility and utility of CZT-derived MFR data remains under active investigation. Objective This study aims to describe the feasibility of quantitatively measuring myocardial flow reserve obtained using SPECT equipment with CZT detectors in patients undergoing scintigraphy to assess coronary disease. Methodology One hundred patients with suspected or known CAD underwent pharmacological stress myocardial perfusion scintigraphy (adenosine) and rest imaging using a D-SPECT device (Spectrum Dynamics) between October 7th, 2021, and December 30th, 2022. Dynamic tomographic images were acquired with the administration of 99mTc-sestamibi via an injection pump (Bracco) at a flow rate of 1 ml/second. Cedars Sinai's QPS kinetic software was used to calculate MFR, with values < 2 considered abnormal. Statistical analysis was performed using a significance level of 5% and the Student's T-type comparison test. Results Twenty patients (71 men), with a mean age of 69.4 years, were evaluated. Thirteen exams presented global MFR < 2, with an additional 11 patients showing global MFR > 2 and regional MFR in any of the three coronary territories below 2. Notably, six patients with MFR < 2 did not demonstrate abnormal myocardial perfusion imaging in SPECT. Patients with MFR values < 2 had a lower mean left ventricular ejection fraction compared to those with MFR > 2 (52% vs. 62%; p = 0.02). All exams were of good quality and interpretable. Motion correction was required in most exams (>90%). In two patients, the radiotracer and adenosine were administered through external jugular venous access due to unavailability of venous access in the cubital fossa. Conclusion Myocardial flow reserve (MFR) assessment can be effectively conducted using SPECT equipment equipped with CZT detectors, enabling the identification of patients with diminished MFR both globally and within specific coronary territories. These findings provide valuable insights into the management of patients with suspected or confirmed coronary artery disease (CAD). However, further investigation is warranted to elucidate the precise clinical implications of MFR assessment with CZT SPECT in guiding management strategies for patients with CAD.

Quantitative myocardial contrast echocardiography during pharmacological stress for diagnosis of coronary artery disease: a systematic review and meta-analysis of diagnostic accuracy studies

Abstract:Aims: We conducted a meta-analysis to evaluate the accuracy of quantitative stress myocardial contrast echocardiography (MCE) in coronary artery disease (CAD). Methods and results: Database search was performed through January 2008. We included studies evaluating accuracy of quantitative stress MCE for detection of CAD compared with coronary angiography or single-photon emission computed tomography (SPECT) and measuring reserve parameters of A, beta, and Abeta. Data from studies were verified and supplemented by the authors of each study. Using random effects meta-analysis, we estimated weighted mean difference (WMD), likelihood ratios (LRs), diagnostic odds ratios (DORs), and summary area under curve (AUC), all with 95% confidence interval (CI). Of 1443 studies, 13 including 627 patients (age range, 38-75 years) and comparing MCE with angiography (n = 10), SPECT (n = 1), or both (n = 2) were eligible. WMD (95% CI) were significantly less in CAD group than no-CAD group: 0.12 (0.06-0.18) (P < 0.001), 1.38 (1.28-1.52) (P < 0.001), and 1.47 (1.18-1.76) (P < 0.001) for A, beta, and Abeta reserves, respectively. Pooled LRs for positive test were 1.33 (1.13-1.57), 3.76 (2.43-5.80), and 3.64 (2.87-4.78) and LRs for negative test were 0.68 (0.55-0.83), 0.30 (0.24-0.38), and 0.27 (0.22-0.34) for A, beta, and Abeta reserves, respectively. Pooled DORs were 2.09 (1.42-3.07), 15.11 (7.90-28.91), and 14.73 (9.61-22.57) and AUCs were 0.637 (0.594-0.677), 0.851 (0.828-0.872), and 0.859 (0.842-0.750) for A, beta, and Abeta reserves, respectively. Conclusion: Evidence supports the use of quantitative MCE as a non-invasive test for detection of CAD. Standardizing MCE quantification analysis and adherence to reporting standards for diagnostic tests could enhance the quality of evidence in this field.
Left Ventricular Functional Changes after Adenosine Vasodilator Stress Evaluated by Gated Single-Photon Emission Computed Tomography

Abstract:OBJECTIVES:Gated single-photon emission computed tomography (SPECT) myocardial perfusion imaging is useful in assessing left ventricular (LV) myocardial perfusion and function. This study evaluated the LV functional changes after adenosine vasodilator stress, using gated SPECT.METHODS:The study population consisted of 70 patients who underwent adenosine-mediated stress and rest SPECT. All patients underwent coronary angiography. Semi-quantitative assessment of perfusion was analyzed and produced the summed rest score (SRS), the summed stress score (SSS) and the summed difference score (SDS). The global LV function parameters [ejection fraction (EF), end-diastolic volume (EDV), end-systolic volume (ESV)] and regional LV function [the summed motion score (SMS) and the summed thickening score (STS)] were quantified by gated SPECT.RESULTS:Patients were divided into 2 groups: group 1 comprised 16 patients with worsening of LVEF (LVEFrest-LVEFado ≥5%), and group 2 comprised the other 54 patients. Compared with group 2, patients in group 1 had a significantly higher SSS and SDS (9.1 ± 6.8 vs. 5.6 ± 4.5 and 6.6 ± 3.8 vs. 3.6 ± 4.0, respectively; p < 0.05) and the severity of coronary artery stenosis was more serious (p < 0.05).CONCLUSION:Worsening of LVEF after adenosine-induced vasodilator stress, as shown by (99m)Tc-MIBI gated SPECT, is a valuable nonperfusion marker of significant CAD.

Abstract:To evaluate myocardial strain in patients with different coronary artery disease-reporting and data system (CAD-RADS) levels using the computed tomography (CT) feature tracking technology and to investigate the relationship of myocardial strain with coronary artery calcium scores (CACs) and the degree of coronary artery stenosis. We prospectively enrolled 237 consecutive patients to undergo coronary CT angiography. The participants were divided into the following groups: control (n = 87), CAD-RADS 1 (n = 43), CAD-RADS 2 (n = 43), CAD-RADS 3 (n = 38), and CAD-RADS 4 and above (n = 26). Myocardial strains were analyzed by commercial software, and CACs and coronary stenosis were assessed on post-processing stations. Differences between multiple groups were analyzed using one-way analysis of variance or the Kruskal–Wallis test. Logistic regression were used to analyze the effects of dichotomous variables. As the CAD-RADS level increased, the global circumferential strain (GCS), global longitudinal strain (GLS) and global radial strain (GRS) of the left ventricle based on CT gradually decreased. A significant correlation was observed between global myocardial strain and CACs (GRS: r = − 0.219, GCS: r = 0.189, GLS: r = 0.491; P < 0.05). The independent predictors of obstructive CAD were age ( β = 0.065, odds ratio [ OR ] = 1.067, P = 0.005), left ventricular ejection fraction ( β = 0.145, OR = 1.156, P = 0.047), and GLS ( β = 0.232, OR = 1.261, P = 0.01). CT-derived GLS of the left ventricle is correlated with CAD-RADS levels and CACs. It may be a better indicator than CACs to reflect the severity of CAD.

Background Myocardial blood flow (MBF) analysis using stress cardiac magnetic resonance (CMR) has been shown to detect obstructive coronary artery disease (CAD); however, evaluation of its diagnostic performance has primarily been limited to single-center studies. AQUA-MBF (Assessment of QUAntitative MBF using stress CMR) is an international study with the goal of assessing the diagnostic performance of stress MBF for the detection of CAD. In this study, we aim to determine how stress MBF assessment compared against visual analysis (VA) of stress CMR images for the detection of CAD. Methods. 144 individuals (89 (62%) men, age 62±16 years, 97 (68%) hypertension, 54 (38%) diabetes, 92 (64%) hyperlipidemia) from 9 centers who underwent dual sequence stress CMR (1.5T or 3.0T GE Healthcare) and also had either a coronary computed tomography angiography (CTA, n=31), invasive coronary angiogram (ICA, n=95), or low pre-test probability for CAD (n=18) were included. CAD was defined as the presence of: (1) a stenosis ≥50% in the left main coronary artery or ≥70% in the 1 major vessel based on ICA or CTA or (2) an invasive fractional flow reserve (FFR) ≤ 0.8. Absence of obstructive coronary disease (NOCAD) was defined as a no history of myocardial infarction and stenosis <50% by ICA or CTA, 50–70% stenosis with FFR>0.8, or a young individual with no cardiac risk factors. Myocardial perfusion imaging was performed during first pass perfusion of a gadolinium-based contrast agent following the administration of adenosine or regadenoson with a low-resolution image acquired to assess the arterial input function and 2–3 short axis slices acquired to assess myocardial perfusion. VA was performed by 2 experienced cardiologists who assigned a grade of 1–5 based on the probability a study was abnormal. Stress MBF values were determined for each of the 16 myocardial segments using Fermi deconvolution (CircleCVI). The global stress MBF was calculated as the average value of two segments with the lowest values from each of the three coronary artery territories. Unpaired t-test was used to compare stress MBF values between CAD vs NOCAD. Receiver-operating characteristics curves were used to determine diagnostic performance. Results. 60 patients had CAD (20: 1-vessel, 26: 2-vessel, and 14: 3-vessel) (Figure 1A) while 84 had NOCAD (Figure 1B). The global stress MBF in CAD was lower than in NOCAD (1.63±0.52ml/g/min vs 2.41±0.68ml/g/min, p<0.0001) (Figure 2A). Stress MBF had a higher AUC than VA (reader 1 – 0.83 vs 0.73, p = 0.07: reader 2–0.83 vs 0.71, p = 0.02). The optimal stress MBF cut-off value for detecting CAD was 2.05ml/g/min (Figure 2B). Conclusions. In this multicenter study, we show that global stress MBF reported as a single value can identify patients with CAD at least as accurately as VA performed by physicians experienced in the interpretation of stress CMR images.

Diagnostic accuracy for CZT gamma camera compared to conventional gamma camera technique with myocardial perfusion single-photon emission computed tomography: Assessment of myocardial infarction and function

Abstract:Background: The solid-state cadmium-zinc-telluride (CZT) gamma camera for myocardial perfusion single-photon emission computed tomography (MPS) has theoretical advantages compared to the conventional gamma camera technique. This includes more sensitive detectors and better energy resolution. We aimed to explore the diagnostic performance of gated MPS with a CZT gamma camera compared to a conventional gamma camera for detection of myocardial infarct (MI) and assessment of left ventricular (LV) volumes and ejection fraction (LVEF), using cardiac magnetic resonance (CMR) as the reference method. Methods: Seventy-three patients (26% female) with known or suspected chronic coronary syndrome were examined with gated MPS using both a CZT gamma camera and a conventional gamma camera as well as with CMR. Presence and extent of MI on MPS and late gadolinium enhancement (LGE) CMR was evaluated. For LV volumes, LVEF and LV mass, gated MPS images and cine CMR images were evaluated. Results: MI was found in 42 patients on CMR. The overall sensitivity, specificity, positive and negative predictive values for the CZT and the conventional gamma camera were the same (67%, 100%, 100% and 69%). For infarct size > 3% on CMR, the sensitivity was 82% for the CZT and 73% for the conventional gamma camera, respectively. LV volumes were significantly underestimated by MPS compared to CMR (P ≤ .002 for all measures). The underestimation was slightly less pronounced for the CZT compared to the conventional gamma camera (2-10 mL, P ≤ .03 for all measures). For LVEF, however, accuracy was high for both gamma cameras. Conclusion: Differences between a CZT and a conventional gamma camera for detection of MI and assessment of LV volumes and LVEF are small and do not appear to be clinically significant.

Abstract:Background: The value of semiquantitative resting myocardial perfusion imaging (MPI) in coronary artery disease (CAD) is limited. At present, quantitative MPI can be performed by a new cadmium zinc tellurium single-photon emission computed tomography (CZT-SPECT) scan. The quantitative index of resting myocardial blood flow (MBF) has received little attention, and its manifestations and clinical value in the presence of unstable coronary blood flow have not been clarified. Purpose: In patients with ST-segment elevation myocardial infarction (STEMI), whether resting MBF can provide additional value of blood flow than semi-quantitative resting MPI is not sure. We also explored the influencing factors of resting MBF. Methods: This was a retrospective clinical study. We included 75 patients with STEMI in the subacute phase who underwent resting MPI and dynamic scans after reperfusion therapy. General patient information, STEMI-related data, MPI, gated MPI (G-MPI), and resting MBF data were collected and recorded. According to the clinically provided culprit vessels, the resting MBF was divided into ischemic MBF and non-ischemic MBF. The paired Wilcoxon signed-rank test was used for resting MBF. The receiver operating characteristic (ROC) curves were used to determine the optimal threshold for ischemia, and multiple linear regression analysis was used to analyze the influencing factors of resting MBF. Results: There was a statistically significant difference between the ischemic MBF and non-ischemic MBF [0.59 (0.47–0.72) vs. 0.76 (0.64–0.93), p < 0.0001]. The ROC curve analysis revealed that resting MBF could identify ischemia to a certain extent, with a cutoff value of 0.5975, area under the curve (AUC) = 0.666, sensitivity = 55.8%, and specificity = 68.7%. Male sex and summed rest score (SRS) were influencing factors for resting MBF. Conclusion: To a certain extent, resting MBF can suggest residual ischemia after reperfusion therapy in patients with STEMI. There was a negative correlation between male sex, SRS, and ischemic MBF. A lower resting MBF may be associated with more severe myocardial ischemia.

Assessing the diagnostic value of left ventricular synchrony indices derived from phase analysis by D-SPECT in identifying obstructive coronary artery disease

Abstract:This study aimed to assess the diagnostic efficacy of left ventricular synchrony (LVS) for detecting coronary artery disease (CAD). We explored whether the LVS index derived from phase analysis of D-SPECT provides superior diagnostic value compared to conventional perfusion analysis in identifying obstructive CAD. Patients with suspected or confirmed CAD underwent drug-stress/rest gated D-SPECT myocardial perfusion imaging (MPI) and coronary angiography (CAG). A 50% stenosis was set as the threshold for obstructive CAD. 110 participants were enrolled in this analysis. There were significant differences in phase standard deviation (PSD), phase histogram bandwidth (PHB) and entropy among the four groups. Patients without cardiac disease and those with mild-moderate stenosis exhibited no noticeable contraction asynchrony. However, LVS indices demonstrated a gradual increase with the progression of coronary stenosis when compared to NC (P < 0.001). Obstructive CAD was identified in 43 out of 110 participants (39%). Optimal cutoff values for diagnosing obstructive CAD during stress were determined as 7.6° for PSD, 24° for PHB, and 37% for entropy, respectively. Notably, PSD, PHB, and entropy indices exhibited higher sensitivity compared to MPI. The integration of the stress-induced LVS indices into routine MPI analysis resulted in a significantly greater area under the curve (AUC), leading to improved diagnostic performance and enhanced differential capacity. Stress-induced LVS indices increase with the severity of coronary artery stenosis by D-SPECT phase analysis. Further, the indices-derived phase analysis exhibits superior sensitivity and discriminatory ability compared to MPI in detecting obstructive CAD.

Abstract:Introduction: Coronary artery disease (CAD) is a leading cause of death and disability in developed countries. While exercise testing is recommended for diagnosing stable angina pectoris, its limited sensitivity and specificity have been questioned. Myocardial strain is a promising predictor of significant CAD. Aim: To evaluate the utility of myocardial strain obtained by 2D speckle tracking at rest and under stress combined with stress magnetocardiography for detecting CAD in patients with stable or low-risk unstable angina pectoris. Methods: A total of 108 patients meeting the inclusion criteria underwent coronary angiography within 48 h of admission. Myocardial strain was assessed using 2D speckle tracking at rest and during dobutamine stress alongside stress magnetocardiography. Results: Global longitudinal strain at stress showed a moderate correlation with significant CAD (r = 0.41, p <0.0001) and with coronary occlusion severity (r = 0.62, p <0.0001). Strain at stress had a sensitivity of 74.1% and specificity of 76.7% for detecting CAD at a cut-off value of −19.1. The ST fluctuation rate from magnetocardiography demonstrated the highest sensitivity for CAD detection. Conclusions: Longitudinal strain parameters and stress magnetocardiography are effective non-invasive methods for predicting CAD in patients with stable angina, potentially reducing the need for invasive assessments.

Abstract:Background: GadaCAD2 was 1 of 2 international, multicenter, prospective, Phase 3 clinical trials that led to U.S. Food and Drug Administration approval of gadobutrol to assess myocardial perfusion and late gadolinium enhancement (LGE) in adults with known or suspected coronary artery disease (CAD). Objectives: A prespecified secondary objective was to determine if stress perfusion cardiovascular magnetic resonance (CMR) was noninferior to single-photon emission computed tomography (SPECT) for detecting significant CAD and for excluding significant CAD. Methods: Participants with known or suspected CAD underwent a research rest and stress perfusion CMR that was compared with a gated SPECT performed using standard clinical protocols. For CMR, adenosine or regadenoson served as vasodilators. The total dose of gadobutrol was 0.1 mmol/kg body weight. The standard of reference was a 70% stenosis defined by quantitative coronary angiography (QCA). A negative coronary computed tomography angiography could exclude CAD. Analysis was per patient. CMR, SPECT, and QCA were evaluated by independent central core lab readers blinded to clinical information. Results: Participants were predominantly male (61.4% male; mean age 58.9 ± 10.2 years) and were recruited from the United States (75.0%), Australia (14.7%), Singapore (5.7%), and Canada (4.6%). The prevalence of significant CAD was 24.5% (n = 72 of 294). Stress perfusion CMR was statistically superior to gated SPECT for specificity (P = 0.002), area under the receiver operating characteristic curve (P < 0.001), accuracy (P = 0.003), positive predictive value (P < 0.001), and negative predictive value (P = 0.041). The sensitivity of CMR for a 70% QCA stenosis was noninferior and nonsuperior to gated SPECT. Conclusions: Vasodilator stress perfusion CMR, as performed with gadobutrol 0.1 mmol/kg body weight, had superior diagnostic accuracy for diagnosis and exclusion of significant CAD vs gated SPECT.
