Restorative Physical Function and Patient-Reported Outcomes Following Acute Lateral Ankle Sprain in High School Athletes

Abstract:ABSTRACT Context Limited longitudinal data exists on patient-reported outcomes (PROs) following acute lateral ankle sprain (LAS). The impact of prospective hop testing on PROs at return to play (RTP) and 6-months post RTP is unclear. Objective Determine if high school athletes with a LAS who return to baseline physical function as measured by a single-leg hop for distance (SLHOP) have better PROs relative to individuals who return to symmetry. Design Cohort Study Setting Ten high schools over two years. Patients or Other Participants Two hundred six high school athletes who sustained a LAS were included. Baseline SLHOP testing was completed pre-injury. PROs were recorded at time of injury, RTP, and 6-months post RTP. Participants were classified as symmetry (n=134) or restorative (n=72). Symmetry was defined as achieving a SLHOP performance within 10% of the uninjured limb at RTP. Restorative was defined as achieving a SLHOP performance within 10% of pre-injury levels. Main Outcome Measures FAAM-ADL, FAAM-Sport, PROMIS Anxiety, and PROMIS Depression were analyzed using a two-way non-parametric ANOVA. The interaction term of group by time was the main comparison of interest and was interpreted if significant. Post-hoc multiple comparisons were performed using the Tukey-Kramer test. Results The restorative group reported better FAAM scores at RTP and 6-months post RTP (p<0.05) relative to the symmetry group. There were no differences in PROMIS Anxiety scores at time of injury, RTP, or 6-months post RTP between groups (p>0.05). There were no differences in PROMIS Depression scores at time of injury between groups (p=0.34), but the restorative group had worse PROMIS Depression scores at RTP (p=0.03). Conclusions The restorative group reported better FAAM-ADL and FAAM-Sport scores at RTP and 6-months post RTP relative to the symmetry group. Restoring individuals to baseline physical function rather than limb symmetry may ensure better PROs following a LAS.

Which Functional Tests and Self-Reported Questionnaires Can Help Clinicians Make Valid Return to Sport Decisions in Patients With Chronic Ankle Instability? A Narrative Review and Expert Opinion

Abstract:Lateral ankle sprain is the most common injury in sports, with up to 40% of patients developing chronic ankle instability (CAI). One possible cause underlying this high rate of recurrence or feeling of giving way may be a premature return to sport (RTS). Indeed, except for time-based parameters, there are no specific criteria to guide clinicians in their RTS decisions in patients with CAI. A recent international consensus highlighted the relevance and importance of including patient-reported ankle function questionnaires combined with functional tests targeting ankle impairments in this population. Thus, the aim of this narrative review and expert opinion was to identify the most relevant functional performance tests and self-reported questionnaires to help clinicians in their RTS decision-making process following recurrent ankle sprains or surgical ankle stabilization. The PubMed (MEDLINE), PEDro, Cochrane Library and ScienceDirect databases were searched to identify published articles. Results showed that the single leg stance test on firm surfaces, the modified version of the star excursion balance test, the side hop test and the figure-of-8 test appeared to be the most relevant functional performance tests to target ankle impairments in patients with CAI. A combination of the Foot and Ankle Ability Measure (FAAM) and the Ankle Ligament Reconstruction-Return to Sport after Injury (ALR-RSI) questionnaires were the most relevant self-reported questionnaires to assess patient function in the context of CAI. Although these functional tests and questionnaires provide a solid foundation for clinicians to validate their RTS decisions in patient with CAI, objective scientific criteria with cut-off scores are still lacking. In addition to the proposed test cluster, an analysis of the context, in particular characteristics related to sports (e.g., fatigue, cognitive constraints), to obtain more information about the patient's risk of recurrent injury could be of added value when making a RTS decision in patients with CAI. In order to evaluate the strength of evertors under ecological conditions, it would also be interesting to assess the ability to control weight-bearing ankle inversion in a unipodal stance. Further studies are needed to assess the relevance of this proposed test cluster in RTS decision-making following lateral ankle sprain injury and CAI.
Assessment of Patient-Reported Outcomes at Return-to-Sport Following Lateral Ankle Sprain Injuries: A Report from the Athletic Training Practice-Based Research Network.

Abstract:Context There is limited evidence regarding the assessment of single-item patient-reported outcomes when patients are medically cleared to return-to-sport following a lateral ankle sprain (LAS) injury. Objective To evaluate self-report of improvement in health status, pain, function, and disability at return-to-sport following a LAS. Design Descriptive study. Setting Sixty-nine athletic training facilities across 24 states. Patients or Other Participants Six hundred and thirty-seven patients (male=53.2%) were included in our final analysis. Patients were diagnosed with a LAS, restricted from sport following injury, and subsequently medically cleared to return-to-sport within 60 days were included. Main Outcome Measures Descriptive statistics were used to summarize scores for health status (Global Rating of Change), pain (Numeric Pain Rating Scale), function (Global Rating of Function), and disability (Global Rating of Disability). Mann-Whitney U Tests were used to compare score differences between sexes. A Kaplan-Meier analysis was performed to provide a visual depiction of sex differences in time to return-to-sport. Results Most patients sustained a LAS injury while participating in basketball, football, or soccer and cleared to return-to-sport 8 days following injury. Over two-thirds of patients reported a meaningful improvement in health status between time of injury and return-to-sport. However, many reported some level of deficits related to pain (65.1%), function (86.2%), and disability (35.8%) at return-to-sport. No differences were noted between males and females for pain (p=.90), function (p=.68), change (p=.45), or disability (p=.21) at return-to-sport. Males, however, returned to sport slightly sooner than females (p=.025). Conclusions Despite self-perceived improvements in health status from time of injury, patients typically return-to-sport with deficits in pain, function, and disability following a LAS. Patients may be returning to unrestricted sport participation before they feel their bodies have fully recovered from the injury. Level of Evidence Level III; retrospective cohort study.

Abstract:BACKGROUND: Lateral ankle sprains (LAS) are among the most common sports- related injuries and the reinjury rate is very high.OBJECTIVES: This study aimed to evaluate the prevalence of some intrinsic risk factors among professional football and basketball players with or without history of acute or recurrent ankle sprain.PATIENTS AND METHODS: One hundred and six professional football and basketball players who were referred for pre-participation examinations were recruited in this study. Prepared checklist was completed for each participant. Athletes were asked for any history of previous ankle sprain and the severity (based of self-description of signs and symptoms by the athlete), level and number of injuries in the last two years. All players were assessed for measures of foot posture index- 6, foot length and width, Beighton generalized joint laxity score, anterior drawer and talar tilt tests, star excursion and single leg balance tests and goniometric assessment of ankle plantarflexion, ankle dorsiflexion and first metatarsophalangeal dorsiflexion.RESULTS: Forty eight basketball players (45.3%) and 58 football players (54.7%) with mean (SD) age of 19.8 (4.5) years participated. About 58.5% and 14.2% of athletes had a history of ankle sprain and recurrent sprain in at least one extremity, respectively. Sprains were more prevalent in basketball players and in dominant leg. There was no significant difference in assessed risk factors between athletes with and without history of ankle sprain, except for positive single leg balance test which was more prevalent in athletes with history of ankle sprain and also for positive talar tilt test and decreased ankle plantarflexion range of motion in acute and recurrent injury of left ankle.CONCLUSIONS: Some intrinsic risk factors including lateral ankle ligaments laxity, balance and ankle plantarflexion seem to be related to acute or recurrent LAS in athletes. Further research is needed to reveal the role of different arthrokinematics following lateral ankle sprain.
Adolescent Athletes with Stronger Athletic Identity Perceptions Have Weaker Fear Avoidance Perceptions During Musculoskeletal Injury Rehabilitation Return to Sports Preparation

Abstract:Background: Adolescent athletes who sustain a musculoskeletal sports injury are at high re-injury risk. This prospective study evaluated athletic identity perception and fear avoidance perception relationships during the return to sport preparation phase of musculoskeletal injury rehabilitation. Methods: From 140 consecutive physical therapy outpatients, 50 (26 females, 24 males) adolescent athletes (mean age 16.8, range 14 to 22 y) completed the 7-item Athletic Identity Measurement Scale (AIMS), the 10-item Athletic Fear Avoidance Questionnaire (AFAQ), and the AFAQ with 2 additional movement-related fear and pain questions (AFAQ+). Correlational analysis was performed of overall AIMS and AIMS subscale scores (social identity, exclusivity, negative affectivity), with AFAQ and AFAQ+ scores ( P ≤0.05). Results: Adolescent athletes were receiving treatment for musculoskeletal injuries sustained during soccer (n=10), lacrosse (n=7), baseball (n=6), basketball (n=5), volleyball (n=5), track or cross country (n=4), American football (n=4), field hockey or ice hockey (n=3), softball (n=2), tennis (n=2), and gymnastics (n=2). Lower extremity conditions included anterior cruciate ligament reconstruction (n=23) or other lower extremity conditions (n=17). Upper extremity conditions included shoulder dislocation or labral repair (n=6), elbow sprain or fracture (n=3), and clavicle fracture (n=1). The AIMS score displayed moderate inverse relationships with AFAQ ( r =-0.40, P =0.008) and AFAQ+ ( r =-0.41, P <0.004) scores. The "social identity" AIMS subscale score displayed a moderate inverse relationship with AFAQ ( r =- 0.48, P <0.001) and AFAQ+ ( r =-0.46, P =0.001) scores. The "exclusivity" AIMS subscale score displayed moderate inverse relationships with AFAQ ( r =-0.40, P =0.005) and AFAQ+ ( r =-0.46, P =0.001) scores. The "negative affectivity" subscale did not display significant relationships with AFAQ or AFAQ+ scores. Subject age displayed moderate inverse relationships with the AIMS "social identity" ( r =-0.56, P <0.001) and "exclusivity" ( r =-0.42, P =0.004) subscale scores and weak direct relationships with AFAQ (r=0.30, P =0.04) and AFAQ+ (r=0.32, P =0.02) scores. Conclusion: Adolescent athletes with stronger athletic identity perceptions during the return to the sports preparation phase of musculoskeletal injury rehabilitation had weaker fear avoidance perceptions. As age increased from early to late adolescence, athletic identity perceptions became weaker, and fear avoidance perceptions became stronger. To decrease re-injury rates, early identification and surveillance of injured adolescent athletes with stronger, more exclusive athletic identity perceptions and weaker fear avoidance perceptions may influence rehabilitation progression and return to play decisions. Abstract:Acute ankle sprains represent one of the most common traumatic injuries to the musculoskeletal system. Many individuals with these injuries experience unresolved symptoms such as instability and recurrent sprains, leading to chronic ankle instability (CAI), which affects their ability to maintain an active lifestyle. While rehabilitation programs focusing on sensorimotor, neuromuscular, strength and balance training are primary treatments, some patients require surgery when rehabilitation fails. A critical analysis of the patient-reported outcome tools (PROs) used to assess CAI surgical outcomes raises some concerns about their measurement properties in CAI patients, which may ultimately affect the quality of evidence supporting current surgical practice. The aim of this research is to develop and validate a new PRO for the assessment of ankle instability and CAI treatment outcomes, following recent methodological guidelines, with the implicit aim of contributing to the generation of scientifically meaningful evidence for clinical practice in patients with ankle instability.

Usefulness of Current Patient-Reported Outcome Scales for ACL Injury: A Mixed-Methods Evaluation of Stakeholder-Perceived Utility of Specific Constructs and Items Across the Rehabilitation Timeline

Abstract:Background: Numerous patient-reported outcome measures (PROMs) have been used in patients with anterior cruciate ligament reconstruction (ACLR), often with overlapping constructs of interest and limited content validity. Inefficient scale application increases burden and diminishes overall usefulness for both the patient and practitioner. Purpose: To isolate specific PROM items across a diverse set of constructs that patients and practitioners perceive as having the greatest value at various stages of recovery and return to sport (RTS) in patients after ACLR. Study design: Cross-sectional study. Methods: A combined 77 stakeholders participated in this 2-phase mixed-methods investigation. In phase 1, a total of 27 patients and 21 practitioners selected individual PROM items from various constructs that had the greatest utility or importance. In phase 2, the highest rated items were further tested in a head-to-head comparison with 29 stakeholders who attended the 2022 ACL Injury Research Retreat. In addition to the utility assessment, practitioners answered other questions related to importance and timing of PROM assessments. Results: In phase 1, both patients and practitioners shared the same top item in 6 of the 8 (75%) constructs assessed. In phase 2, the construct of psychological burden was rated as "extremely important" by 59% of respondents, followed by physical function (54%), symptoms (35%), and donor site issues (10%). The PROM items of confidence, perceived likelihood of reinjury, and difficulty stopping quickly were rated by a respective 93%, 89%, and 86% of the sample as either "very useful" or "extremely useful." All constructs except donor site issues were rated by most stakeholders to be absolutely necessary to evaluate treatment progress and RTS readiness at the 6-month postoperative time and at RTS. Conclusion: Overall, psychological burden, with specific items related to confidence and reinjury likelihood, were rated as most important and useful by both patients and practitioners. The second most important and useful PROM item was related to higher intensity function (eg, decelerating or jumping/landing activities during sports).

Abstract:Context: Lateral ankle sprain (LAS) patients often have deficient patient-reported outcomes (PROs) at return-to-activity (RTA), potentially increasing risk for recurrent LAS and ankle pain. Additionally, applied care strategies are known to correct impairments, but their ability to mitigate risk for long-term consequences remains unknown. Objective: To determine if applied care strategies and PRO scores at RTA and 6-months post-RTA predict recurrent LAS and ankle pain 12 months after an acute LAS. Design: Prospective cohort study. Setting: Online survey. Patients or other participants: We enrolled 63 individuals within one week of sustaining an acute LAS. Main outcome measures: Participants completed online surveys about their health history and recent LAS. At RTA and 6 months post-RTA, participants completed online surveys regarding demographics, applied care strategies, and patient reported outcomes (PROs): Foot and Ankle Disability Index, Identification of Functional Ankle Instability, Godin Leisure-Time Exercise Questionnaire, Short Form-8. At 12 months post-RTA, we asked participants if they sustained recurrent LAS. Chi-squares determined if recurrent LAS and ankle pain at 12-months were related to applied care strategies or ankle pain at RTA. Independent t-tests compared demographics and PROs at RTA and 6-months between participants with and without a recurrent LAS or ankle pain at 12-months. Logistic regression and area under the receiver operating characteristic (AUROC) analyses determined if demographics, applied care strategies, ankle pain at RTA, and PRO scores at RTA and 6-months predicted recurrent LAS and ankle pain at 12-months. Results: Participants with a recurrent LAS had a lower walking boot use (P=0.05) and were taller than those without (P=0.03). Increased height and lack of walking boot use were predictive of recurrent LAS (P<0.01, R2=0.33, AUROC=0.81[0.68, 0.95]). Conclusions: LAS patients who are taller and do not use a walking boot might have greater risk for a recurrent LAS withing 12 months of RTA.

Abstract:Chronic Ankle Instability (CAI) can affect the ankle function, range of motion (ROM) and posture control ability of athletes. However, among elite college athletes aged 18-25, the relationship between dysfunction, ROM limitation and abnormal posture control ability has not been well described. PURPOSE: The purpose of this study is to analyze the relationship between dysfunction, ROM limitation and abnormal posture control ability of CAI college elite athletes, and to provide a basis for formulating intervention strategies for functional recovery. METHODS: According to the CAI inclusion criteria recommended by the International ankle consortium (IAC), 55 (male : 45, female : 10) 17-25-year-old CAI college elite athletes were selected for cross-sectional study. Cumberland Ankle Instability Tool (CAIT) was used to represent function, the weight-bearing lunge test (WBLT) was used to represent ROM, and Y-balance test was used to represent posture control ability. Pearson correlation analysis was performed on the relevant test results, r ≥ 0.8 was strongly correlated, 0.8 > r ≥ 0.6 was strongly correlated, 0.6 > r ≥ 0.4 was moderately correlated, and test level α = 0.05. To avoid the effect of lower limb length on the YBT test, the results are standardized. SUMMARY OF RESULTS: The average age of the subjects was 19.9 ± 5.7, and there was no correlation between gender (p = 0.408) and the degree of injury, p > 0.05. CAIT was weakly correlated with WBLT (r = 0.288, p = 0.031) and YBT anterior standard value (r = 0.329, p = 0.013), p < 0.05. Moderate correlations with YBT posterolateral standard (r = 0.461, p < 0.001), YBT posteromedial standard (r = 0.420, p = 0.001), YBT conflation (r = 0.459, p < 0.001) were significant, p < 0.01. WBLT was weakly correlated with CAIT (r = 0.288, p = 0.031) and YBT anterior standard value (r = 0.338, p = 0.011), p < 0.05. There was no correlation with YBT posterolateral standard value (p = 0.353), posteromedial standard value (p = 0.1), and conflation (p < 0.068),p > 0.05. CONCLUSION: There is a significant positive correlation between the function of CAI college elite athletes and the posture control in ROM and ANT directions, and a very significant positive correlation with PLAT, PMED direction and overall posture control. There was a significant positive correlation between ROM and ANT direction posture control.
Generalized 3D Kernel Computation Method and Its Application in PET-Insert System

Abstract:We are developing insert devices for existing PET scanners to improve the image resolution with almost the same sensitivity as current PET scanners. The insert device can be used to zoom into a particular organ of interest. Introduction of the insert inside the scanner leads to three types of coincidences: insert-insert (II), insert-scanner (IS) and scanner-scanner (SS). In typical whole-body PET scanners, coincidences recorded in the scanner are sorted into parallel-beam sinograms and images are reconstructed using linear or iterative techniques. In the PET-insert system, the coincidences of type IS have an inherent fan-beam geometry. Reconstruction using parallel-beam sinograms introduces severe streaking artifacts in the images. The coincidences sorted into fan-beam sinograms reduce the artifacts in the reconstructed images. The approach to compute the kernel was derived from CT as there exists an analogy between the PET-insert geometry and a fourth generation CT scanner geometry. In this approach, the weights in the kernel are computed using the intersection of a cone with a voxel. We previously developed two dimensional reconstruction algorithms for this novel system geometry. We extend this work to three dimensions in this paper. A maximum-likelihood expectation-maximization algorithm was used to reconstruct the data. The kernel was validated with a contrast recovery study using a digital phantom. Images reconstructed from experimental data show good quality without any visible artifacts.
Identification of Ankle Injury Risk Factors in Professional Soccer Players Through a Preseason Functional Assessment

Abstract:Background: Etiologically, the risk of an ankle injury depends on extrinsic and intrinsic factors, such as muscle strength asymmetry, decreased flexibility, and decreased proprioception, as well as patient age and history of injuries. Purpose/Hypothesis: The purpose of this study was to identify risk factors present in the preseason assessment that may predispose professional soccer players to ankle injuries. We hypothesized that analysis of these parameters could relate the incidence of injuries to the deficits found during the preseason period, enabling the identification of risk factors to predict the occurrence of injuries. Study Design: Cohort study; Level of evidence, 2. Methods: A total of 89 professional soccer athletes were evaluated in the preseason period; the evaluation included athlete history and anthropometric data collection, an isokinetic ankle evaluation, and functional tests: the Dorsiflexion Lunge Test and Y-Balance Test (YBT). The athletes were monitored during the competitive period, and the incidence of injuries was surveyed. The association of quantitative variables and injury outcomes was analyzed using the Student t test for independent samples, with P < .05. For the association of categorical variables and injury outcomes, the chi-square test was performed, with P < .05. Results: A higher incidence of ankle injuries was associated with lower YBT scores in the dominant ( P = .04) and nondominant ( P = .01) limbs. A higher body mass index was also associated with a higher injury occurrence ( P = .01). Conclusion: Functional tests, such as the YBT, are indicated tools for assessing the physical capacities and possible risks of ankle sprains, as they can evaluate the ankle functional capacity in a complex way, identifying athletes more prone to ankle injuries. Athletes’ body mass index should also be taken into account to prevent such injuries.

Patients or other participants: Thirty-nine volunteers with CAI who scored 11 or greater on the Identification of Functional Ankle Instability questionnaire were randomly assigned to 1 of 3 groups: balance-training protocol (7 males, 6 females; age = 23.5 ± 6.5 years, height = 175.0 ± 8.5 cm, mass = 72.8 ± 10.9 kg), strength-training protocol (8 males, 5 females; age = 24.6 ± 7.7 years, height = 173.2 ± 9.0 cm, mass = 76.0 ± 16.2 kg), or control (6 males, 7 females; age = 24.8 ± 9.0 years, height = 175.5 ± 8.4 cm, mass = 79.1 ± 16.8 kg). Intervention(s): Each group met for 20 minutes, 3 times each week, for 6 weeks. The control group completed a mild to moderately strenuous bicycle workout. Main outcome measure(s): Global patient-reported outcomes, regional ankle function, and perceived instability were measured using the Disablement in the Physically Active Scale, the Fear-Avoidance Beliefs Questionnaire, the Foot and Ankle Ability Measure, and a visual analog scale for perceived instability. Participants completed the questionnaires at pretest and 6 weeks posttest. A multivariate repeated-measures analysis of variance with follow-up univariate analysis was conducted. The α level was set a priori at .05. Results: No time-by-group interaction was found ( P = .78, η2 = 0.09). However, we observed a main effect for time ( P = .001, η2 = 0.49). Follow-up univariate analyses revealed differences between the pretest and posttest for the Disablement in the Physically Active Scale ( P = .02, η2 = 0.15), Fear-Avoidance Beliefs Questionnaire ( P = .001, η2 = 0.27), Foot and Ankle Ability Measure-Activities of Daily Living subscale ( P = .003, η2 = 0.22), Foot and Ankle Ability Measure-Sport subscale ( P = .001, η2 = 0.36), and visual analog scale ( P = .008, η2 = 0.18). Conclusions: Statistically, after the 6-week intervention, all groups improved in global and regional health-related quality of life. Clinicians should compare patient-reported outcomes with clinical measures to have a better understanding of progression during rehabilitation.

Abstract:Ligamentous instability, ankle muscle weakness, foot ankle alignment, and generalized joint laxity may be predisposing factors for ankle ligament injuries. The purpose of this study was to examine the reliability of these risk factors before and after the season in healthy individuals and to determine if any significant differ ences developed during the athletic season (range, 12 to 16 weeks). Twenty-one healthy college-aged ath letes were tested for generalized joint laxity, anatomic alignment of the foot and ankle, ligamentous stability, and isokinetic strength of the ankle muscles. This study showed that generalized joint laxity, ankle ligamentous stability, and ankle strength measurements demon strated high correlation coefficients ( r > 0.75). The high correlation coefficients suggested reliable measures. Some of the range of motion measurements had lower correlation coefficients, which suggested more variabil ity in these measurements. After establishing the reli ability in 24 of the 28 measurements with standardized methods, further work is underway to evaluate the role of these factors in inversion ankle sprains.

Abstract:Background: Chronic ankle instability (CAI) is the most common injury among the football players and it occurs with the players who have history of repetitive ankle sprain. This will result in limitation of functional ability and sport performance of a football player. However, most of the sport rehabilitation focused on the sport performance of an athletic after ankle sprain injury and the functional ability was not stressed on during the return to sport process. It is because the relationship between the level functional ability and sport performance among the football players with CAI is still unclear. Objectives: To determine the relationship between the level of functional ability and sports performance among the football players with chronic ankle instability (CAI). Study Design: Correlation study. Methods: Forty two (n=42) of football players were involved in this study by convenience sampling design. Football players filled in the questionnaire which was Foot and Ankle Ability Measure (FAAM) which included the activities of daily living (ADL) subscale and sports subscale. The total score of each subscale for all the players were calculated and Pearson’s Correlation Coefficient was used to analyze the relationship. Results: A total of twenty five (60%) of the football players had higher ADL score than the sports score while seventeen (40%) of them had higher sports score than ADL score. The mean score for ADL subscale was 79.63±20.64 (%) whereas for sports subscale was 75.59±25.43 (%). Pearson’s correlation was .92 (r=.92) and it is significant at .01 level (p <.00). Conclusion: There is a significant relationship between the level of functional ability and sports performance among the football players with chronic ankle instability (CAI). Implication: It provides a guideline for the physiotherapy or football player to plan a more structured rehabilitation in order to improve the quality of return to sport performance.
[Immunomodulatory properties of statins and cancer risk].

Abstract:Statins are cholesterol-lowering drugs widely prescribed for cardiovascular prevention. They are also immunomodulatory and increase peripheral regulatory T cell numbers and function, in vivo. This may be beneficial by decreasing inflammatory response within the atheroma and stabilizing atherosclerotic plaque, however this novel pleiotropic effect might impair host tumor-specific effector T cell responses. Indeed, in some randomized trials, notwithstanding their short duration, statins have been found to increase cancer incidence especially in the elderly and women. In these situations, the decrease in cardiovascular mortality can be matched by an equal increase in cancer mortality, leaving all-cause mortality unchanged. Furthermore, statins have been shown to interfere with cancer immunotherapy. Preventive pharmacological treatment can sometimes be justified because of reduced cardiovascular events even if total mortality does not change, but if it leads to an increase in cancer incidence, it might be not easily accepted. Physicians need to weigh the global risks and benefits when prescribing statins.

Abstract:Background Despite being the most commonly incurred sports injury with a high recurrence rate, there are no guidelines to inform return to sport (RTS) decisions following acute lateral ankle sprain injuries. We aimed to develop a list of assessment items to address this gap. Methods We used a three-round Delphi survey approach to develop consensus of opinion among 155 globally diverse health professionals working in elite field or court sports. This involved surveys that were structured in question format with both closed-response and open-response options. We asked panellists to indicate their agreement about whether or not assessment items should support the RTS decision after an acute lateral ankle sprain injury. The second and third round surveys included quantitative and qualitative feedback from the previous round. We defined a priori consensus being reached at >70% agree or disagree responses. Results Sixteen assessment items reached consensus to be included in the RTS decision after an acute lateral ankle sprain injury. They were mapped to five domains with 98% panellist agreement—PAASS: P ain (during sport participation and over the last 24 hours), A nkle impairments (range of motion; muscle strength, endurance and power), A thlete perception (perceived ankle confidence/reassurance and stability; psychological readiness), S ensorimotor control (proprioception; dynamic postural control/balance), S port/functional performance (hopping, jumping and agility; sport-specific drills; ability to complete a full training session). Conclusion Expert opinion indicated that pain severity, ankle impairments, sensorimotor control, athlete perception/readiness and sport/functional performance should be assessed to inform the RTS decision following an acute lateral ankle sprain injury. Abstract:Background: High ankle sprains are common athletic injuries and can be associated with long-term sequelae. Regardless of operative or nonoperative treatment, there is a paucity of data in the literature about the long-term outcomes of high ankle sprains. Hypothesis: Nonoperative treatment of high ankle sprains utilizing a standardized protocol will result in good long-term outcomes. Study design: Case series; Level of evidence, 4. Methods: Patients who experienced a high ankle sprain without radiographic diastasis of the syndesmosis were identified from a previous study database and contacted for long-term follow-up. All patients were high school or National Collegiate Athletic Association Division IA athletes at initial injury and were treated nonoperatively with the same standardized protocol. Patients completed a questionnaire that included documentation of any interim ankle injuries, 2 different patient-reported outcome scores, and ankle radiographs to conduct Kellgren-Lawrence scoring for ankle osteoarthritis. Results: In total, 76 cases in 74 patients were identified in the database. A total of 40 patients were successfully contacted, and 31 patients (24 collegiate and 7 high school athletes) with 33 high ankle sprains completed the survey (31/40; 77.5%). The mean age at follow-up was 45 years (range, 34-50 years), with a mean time from injury to follow-up of 25 years. Overall, 93.5% (n = 29) of the respondents were male, and 42% (n = 13) of the respondents reported an ipsilateral ankle injury since their initial injury, with 16% (n = 5) having ankle or Achilles surgery. The mean Patient-Reported Outcomes Measurement Information System-10 score was 53.4 (SD, 8.3; range, 37.4-67.7), PROMIS median (IQR), 54.1 (39.9, 68.3), and the mean Self-reported Foot and Ankle Score score was 42.7 (SD, 5.86). Follow-up ankle radiographs were obtained in 11 (35%) of the respondents; 27% had Kellgren-Lawrence grade >2 osteoarthritis, and 36% had signs of heterotopic ossification on imaging. The mean tibiofibular clear space was 4.5 mm, and the mean tibiofibular overlap was 7.15 mm, with 27% of patients demonstrating some tibiotalar narrowing. Conclusion: At long-term follow-up, nonoperative management of high ankle sprains without diastasis on imaging was associated with acceptable patient-reported functional outcomes and low rates of subsequent ankle injuries. There was a high incidence of arthritis, but most cases were not clinically significant. This case series shows the natural history of nonoperatively treated high ankle sprains and may serve as a comparison for different management techniques in the future.

Abstract:Purpose: To systematically review existing literature regarding the ankle ligament reconstruction-return to sport after injury (ALR-RSI) scale and to assess its correlation with Return to sport and functional outcomes as well as feasibility, reliability and consistency. Methods: A systematic review of the literature based on the Preferred Reporting Items for Systematic Reviews and Meta Analyses (PRISMA) was conducted using PubMed, Embase and Cochrane Library. Studies that evaluated psychological readiness to return to sport after ankle ligament reconstruction or repair for the treatment of chronic lateral ankle instability using the ALR-RSI scale were included. The results from each study were pooled, and weighted means and overall rates were calculated. Results: In total, 157 patients (53.2% male, mean age: 34.2 years) from three articles were included. Overall, 85.0% of patients reported successful return to sport, but only 48.9% of patients returned to the preoperative sporting level. All studies reported a significant difference in psychological scores between patients who returned to sport and those who did not. Pooled mean patient-reported outcome measures, reported as the American Orthopaedic Foot and Ankle Society Ankle-Hindfoot (AOFAS, three studies) Score and Karlsson-Peterson Score (three studies), were 82.7 (range: 29-100) and 81.7 (range: 25-100), respectively. The ALR-RSI scale demonstrated strong correlations with the AOFAS Score and Karlsson-Peterson Score. Conclusion: Patients who returned to sport after ankle ligament reconstruction or repair exhibited higher psychological readiness compared to those who did not. The ALR-RSI scale showed strong correlations with ankle function. Evaluation of psychological readiness using the ALR-RSI scale may provide an additional tool in the assessment of patients who underwent ankle ligament reconstruction or repair. Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 9, September 2024. Background:Readiness for return to sports involves both physical and psychological aspects of recovery; however, the relationship between psychological and physical variables after anterior cruciate ligament (ACL) reconstruction (ACLR) is poorly understood.Hypothesis:ACLR patients with a higher psychological readiness would demonstrate better functional testing results at 6 months.Study Design:Cross-sectional study; Level of evidence, 3.Methods:Participants were evaluated at 6 months after ACLR with various patient-reported outcome metrics: Hospital for Special Surgery Pediatric Functional Activity Brief Scale, pediatric or adult International Knee Documentation Committee Questionnaire (IKDC), Patient-Reported Outcomes Measurement Information System (PROMIS) - Psychological Stress Experience and ACL - Return to Sport After Injury (ACL-RSI) scale. Functional testing included quadriceps, hamstrings, and gluteal strength testing; Y-balance test; single-leg single hop, crossover hop, and triple hop tests; and timed 6-m hop test. Pearson correlation coefficient and multivariable regression were used to determine associations between the limb symmetry index (LSI) on functional testing and patient-reported outcomes. Those with LSI deficits 20% (worse performance).Results:A total of 229 participants (89 male, 140 female) with a median age of 17 years (range, 10.3-30.6 years) were enrolled. IKDC had a moderate negative correlation with PROMIS - Psychological Stress Experience (r = -0.39; 95% CI = -0.49, -0.27; P < .001) and a moderate positive correlation with ACL-RSI (r = 0.55; 95% CI = 0.46, 0.64; P < .001). A total of 151 patients completed functional testing. ACL-RSI demonstrated a positive correlation with single-hop LSI (r = 0.21; 95% CI = 0.05, 0.35; P = .01) and timed 6-m hop (r = 0.28; 95% CI, 0.42; P = .001). When adjusting for sex, age, and graft type, patients who had <20% deficit on the single-hop test scored 16.6 points higher on ACL-RSI (P = .001), and those with <20% deficit on crossover hop testing scored a mean 13.9 points higher on ACL-RSI (P = .04)Conclusion:Higher psychological readiness for return to sport was associated with better performance and greater symmetry on hop testing 6 months after ACLR, suggesting a potential link between physical and psychological recovery.
