Abstract:Although the more regular use of inhaled corticosteroids (ICS) in patients with moderate to severe asthma has led to a reduction in admission rates for exacerbations, rates of emergency department (ED) visits remain unacceptably and inexplicably high. Previous work has shown that nonwhite race, low socioeconomic status, and greater asthma severity are associated with ED visits, but these factors do not fully explain the variation in the use of emergency services. In this issue of the Southern Medical Journal, Wells et al further explore the conundrum by examining factors associated with ED visits in an underserved and low income population using retrospective data obtained from three ambulatory care centers. The purpose of the studywas to provide data for quality improvement programs designed to reduce ED utilization among patients with asthma. The study was designed as a caseYcontrol analysis wherein cases consisted of asthmatics with one or more ED visit during a 2-year time period, and controls consisted of asthmatics with one or more outpatient clinic visits but no ED visits during the same 2 years. The authors found that cases were more likely to have been prescribed an ICS in addition to a second controller medication and less likely to have been receiving ICS monotherapy or have been given short-acting A-agonists (SABA) than controls in the year before the index visit. The authors propose that individuals prescribed an ICS plus additional therapy were more likely to use the ED as a reflection of greater disease severity. This assumption is supported by the finding that cases also were more likely to have received a referral for asthma specialty care in the 12 months previously, but it is surprising that they were less likely to have received a prescription for SABA. SABA are recommended for all asthma patients as the most effective means of quickly treating increased symptoms, and lack of availability would certainly put patients at increased risk for needing ED services. Because records from other healthcare systemswere not available for this study, it is possible that these patients were prescribed SABA elsewhere, but this finding raises concerns about drawing a cause-and-effect conclusion about the relation between prescribed medications and ED visitation. Consistent with our present understanding, cases alsowere less likely to receive an influenza vaccination despite clear evidence that the vaccine is safe and effective and that infection is associated with an increased risk of hospitalization for asthma. The lack of provision of the influenza vaccine may be an indicator of less consistent use of primary care services among these individuals, as suggested by the long time to primary care follow-up ofmore than 2 months, but it also could be a reflection of patient preferences as fears about vaccinations continue to hamper the broad use of these critical public health interventions. It is also noted that nonsmokers were more likely than smokers to seek ED care. As the authors hypothesize, smoking may be a self-limited vice in this population that is suspected to have more severe disease; however, this runs counter to the literature, which suggests that asthmatic smokers are more likely to have severe symptoms and a decreased response to ICS therapy. An alternative theory is that the smoking population may be misdiagnosed as having asthma. Because bronchodilator reversibility testing and methacholine challenge test results are not available, it is possible that smoking individuals included in the study may not be asthmatic but rather had acute bronchitis or bronchiolitis associated with a better prognosis. Interestingly, race was not found to correlate with ED visits in the Wells et al study. Although there is debate in the literature on this point, data tend to favor nonwhite race being associated with increased ED use and hospitalization among asthmatics, even when controlling for socioeconomic differences.
