Dramatic tissue response after a single granulocyte transfusion

Abstract:A 7-year-old Hispanic girl with acute lymphocytic leukemia and chemotherapy (vincristine, cyclophosphamide, and l-asparaginase) -induced neutropenia received an intramuscular injection of l-asparaginase in her right arm. The following day she developed pain and erythema at the injection site, which, over the next 24 hours, rapidly extended to involve the entire right arm. Upon admission to our hospital, the patient had a high fever (40°C) and a WBC count of 0.3 ¥ 10 per L with an absolute neutrophil count (ANC) of 40 ¥ 10 per L. She looked toxic and was somnolent. CT scan revealed necrotizing fasciitis involving the entire arm. She was transferred to the ICU, and broad-spectrum antibiotics were initiated. In consultation with an orthopedic surgeon, it was decided that she would undergo emergent surgical debridement. Wound cultures from the debrided tissue were positive for the presence of Pseudomonas aeruginosa, sensitive to piperacillin-tazobactam and clindamycin; thus, antibiotic coverage was narrowed appropriately. The debrided material, which was sent for pathologic examination, is shown in the figure (Panel A1 = necrotic debris with bacteria but no neutrophils [low power]; Panel A2 = Gram-negative bacteria as proven by Gram stain [high power]). The histology is significant for necrotic debris containing Gram-negative bacteria with an absence of neutrophils. The patient received a granulocyte transfusion on the day of admission, immediately after surgical debridement, from a donor who was stimulated with G-CSF and dexamethasone. Because the patient weighed only 22 kg, it was possible to collect a therapeutic dose of neutrophils (2.2 ¥ 10) 6 hours after stimulation. The following day surgical debridement was again performed, because the arm remained swollen causing fear of a compartment syndrome. The material was A1 B1
