Abstract:Gastric schwannomas are rare mesenchymal tumors of the gastrointestinal tract. They are usually misdiagnosed as other submucosal tumors preoperatively. Experience of the imaging features of gastric schwannomas is extremely limited. In this report, we summarize the features of a series of endoscopic ultrasound (EUS) images of gastric schwannomas in an effort to improve the diagnosis and differential diagnosis rate. We retrospectively reviewed the endosonographic features of four patients with gastric schwannomas and their computed tomography imaging results. Gastric schwannomas had heterogeneous hypoechogenicity or isoechogenicity, and a well-demarcated margin. The tumors originated from the fourth layer. Cystic changes and calcification were uncommon. Marginal hypoechoic haloes were observed in two patients. The results described here were different from those of previous studies. In the EUS evaluation, the internal echogenicity of gastric schwannomas was heterogeneous and low, but slightly higher than that of muscularis propria. These features might help us differentiate gastric schwannomas from other submucosal tumors. Further investigation is needed to differentiate these mesenchymal tumors.

Abstract:Objective To discuss CT and endoscopic ultrasonography performance and diagnostic value in patients with gastric schwannoma.Methods CT and the upper gastrointestinal endoscopic ultrasonography features of 8 cases of gastric schwannoma proved by surgical pathology were retrospectively analyzed.Results One case located in the fundus of the gaster,3 located in the antetheca of corpusventriculi,2 located in the lesser curvature of corpusventriculi,2 located in the antrum of gaster.CT showed the gastric wall exogenous well-defined homogeneous solid mass,focal nodules or mass type 7 cases,massive type 1 case,1 case had speckled calcification of the margins,4 cases appeared small flake and thin strips hypodense cystic in the mass.Post contrast imaging showed the masses of 8 cases were mildto-moderate enhanced.The upper gastrointestinal endoscopic ultrasonography performed hypoechoic mass from the gastric muscularis propria.Conclusions CT and endoscopic ultrasonography examinations can definite tumors positions,size,boundary and tumor necrosis,cystic change or calcification,help to improve preoperative diagnosis accuracy rate of the tumor.

Abstract:Background This study aimed to investigate the clinical features, diagnostic criteria, treatment options, and prognosis of patients with gastric schwannoma (GS). Methods We collected the clinical data of all patients pathologically diagnosed with GS in Zhejiang Provincial People's Hospital from May 2012 to October 2021. Results A total of 26 cases of GS were analyzed clinicopathologically, where the sizes of the tumor were found to be in the range of 1–6 cm (mean: 3.16 cm, median: 3.05 cm). A computed tomography (CT) scan analysis revealed that most masses were either moderately progressive or uniformly enhanced. According to ultrasound gastroscopy results, most of them were hypoechoic masses. There were 23 cases of surgery and three cases of endoscopic submucosal tumor dissection. Immunohistochemistry demonstrated that S100 was positive in 26 patients, immunomarker SOX10 was positive in five, whereas CD34, CD117, and SMA were negative in most patients. CK (Pan), Dog-1, and Desmin were also found negative. All 26 cases were followed up after the conclusion of the study where no evidence of recurrence or metastasis was observed. Conclusions GS is a unique form of peripheral schwannoma. The diagnosis of this type of tumor depends on the pathology and immunohistochemistry of the individual. The key to treating this type of tumor is endoscopy and surgery. Follow up and related literature review showed that GS was a benign tumor with little possibility of malignant transformation.

Abstract:Schwannomas origin from Schwann cells sheath and generally are benign, slow-growing, and asymptomatic neoplasms which frequently appear in the head and neck. Although gastrointestinal schwannoma is really rare, the most affected organ in GI system is the stomach. Gastric schwannoma forms 0.2% of all gastric tumors. This neoplasm is always detected as a submucosal mass, the same as other gastrointestinal stromal tumors. Although these tumors have almost the same presentations, they are completely different at therapeutic options and prognoses. Hence, it is important to distinguish them apart and make an accurate diagnosis to optimize treatment outcomes. Herein, we report a case of 28-year-old woman with frequent vomiting and abdominal pain caused by 5 × 6 cm schwannoma in the antrum of the stomach. This is a rare case of gastric outlet obstruction due to a massive schwannoma. In addition, all other probable submucosal masses will be discussed at different aspects.

Abstract:Rationale: Pancreatic schwannomas are extremely rare and are difficult to diagnose preoperatively. Over the past 50 years, only 96 cases of pancreatic schwannoma have been reported in English literature. Herein, we report a case of pancreatic schwannoma treated with enucleation. Patient concerns: A 66-year-old woman visited a local hospital due to ventosities. Ultrasonography and computed tomography revealed a pancreatic mass. She visited our hospital for further diagnosis and treatment. Diagnosis and interventions: Magnetic resonance imaging revealed a tumor in the pancreatic body, and a solid pseudopapillary tumor was considered preoperatively. During the surgery, a pancreatic mass was found growing in the pancreatic body and tail. A successful tumor enucleation was performed. The mass was 7 x 6 x 3 cm in size with a thin capsule. Pathological examination revealed that the tumor was mainly composed of spindle-shaped cells with a palisading arrangement and no atypia. Both hypercellular and hypocellular areas were visible. Immunohistochemical staining showed that protein S-100 was strongly positive. The tumor was diagnosed as a benign schwannoma originating from the pancreatic body and tail. Outcomes: Postoperatively, the patient showed good recovery. During the 24-month follow-up period, the patient remained well and free of complications. Lessons: Pancreatic schwannomas are extremely rare and difficult to diagnose using imaging examinations. Enucleation is a safe and efficacious treatment for exophytic pancreatic schwannomas.

Abstract:Schwannomas are tumors arising from the nervous system that also occur infrequently in the gastrointestinal tract, most commonly in the stomach. This report characterizes 29 patients with benign or malignant gastric schwannomas. Surgical data and clinical follow-up information were available for 28 cases with a median postoperative duration of 57 months. Clinicopathological and immunohistochemical characteristics of benign and malignant schwannomas were analysed. Four cases (13.7%) were histologically diagnosed with malignant schwannoma. All tumors were positive for S-100 and CD56 proteins, displaying a diffuse staining pattern. Vimentin was expressed in 100% cases and all schwannomas were negative for smooth muscle actin, c-kit, and HMB-45. A significant difference was observed between the group of benign and malignant schwannomas as regards recurrences and metastasis after complete resection (P = 0.015). The survival time of patients with benign schwannomas was longer than the malignant group (P = 0.013), so gastric malignant schwannomas have a potential for recurrence and metastasis, with subsequently short survival. Complete resection with an attempt to remove all tumor tissue with negative margins is of paramount importance in the management of gastric schwannomas, particularly when they turn out to be malignant.

Abstract:Schwannomas of the cervical esophagus are extremely rare, as fewer than a dozen reports have been published in the literature. Therefore, their clinical characteristics and management have not been definitively elucidated. We report 2 cases of cervical esophageal schwannoma (CES) in which the patients-a 52-year-old woman and a 53-year-old woman-were initially misdiagnosed clinically. The correct diagnosis was later established on the basis of contrast-enhanced computed tomography (CT) and intraoperative frozen-section analysis. In both cases, the tumor was enucleated, and the esophagus was closed by primary intention. Both patients resumed an oral diet 2 weeks postoperatively. Follow-up detected no evidence of recurrence. Our review of the literature revealed that CES is a benign mesenchymal tumor that can be misdiagnosed both clinically and pathologically. Preoperative contrast-enhanced CT and intraoperative frozen-section analysis help in the planning for conservative enucleation, which precludes the need for esophageal resection and its associated morbidity.

Abstract:BACKGROUNDPrimary schwannoma is a rare submucosal tumor of the esophagus, which is most often benign, and surgery is the only effective treatment. So far, only a few cases have been reported. Herein, we reported a single case diagnosed with primary esophageal schwannoma that was totally removed by submucosal tunneling endoscopic resection (STER).CASE SUMMARYA 62-year-old man presented to the hospital with a history of resection of a malignant gastric tumor and mild dysphagia. Endoscopic examination revealed a large submucosal elevated lesion in the esophagus 25-30 cm from the incisors. Endoscopic ultrasonography detected a 45 mm x 35 mm x 31 mm hypoechoic lesion; chest computed tomography showed a mass of approximately 55 mm x 35 mm x 29 mm. A preliminary examination showed features suggestive of a stromal tumor. Pathological findings indicated esophageal schwannoma. Next, STER alone was performed to completely resect the mass, and the patient recovered well post-surgery. Afterward, the patient was discharged and showed no tumor recurrence at 33 mo of follow-up.CONCLUSIONEndoscopic resection is still an effective treatment for large esophageal schwannomas (> 30 mm) under meticulous morphological evaluation.
A rare case of gastric schwannoma mimicking GIST: diagnostic and therapeutic approach

Abstract:Gastric schwannomas are rare, benign neurogenic tumors originating from Schwann cells within the gastrointestinal tract, comprising only 0.2% of all gastric tumors. This report presents the case of a 32-year-old female patient who experienced severe periumbilical pain, nausea, and vomiting, ultimately diagnosed with gastric schwannoma. Initial imaging and endoscopic evaluations suggested a gastrointestinal stromal tumor (GIST), but postoperative histopathological analysis confirmed schwannoma, showing S-100 positivity and negativity for CD117, DOG-1, SMA, Desmin, and CD34. The patient underwent successful central gastrectomy with negative surgical margins and no metastasis. Despite a postoperative complication of small bowel obstruction, which was managed conservatively, the patient remained symptom-free with no recurrence over the follow-up period. This case underscores the importance of differential diagnosis, distinguishing schwannomas from GISTs and other submucosal lesions through thorough histopathological and immunohistochemical analyses, and highlights the efficacy of complete surgical resection in preventing recurrence.

Abstract:The majority of esophageal tumor lesions are esophageal cancer. The benign primary esophageal tumors which account for about 2% of all esophageal tumors are uncommon. Over 80% of benign esophageal tumors present as smooth muscle tumors and less commonly as esophageal schwannoma. [ 1 , 2 ] Esophageal schwannoma are characteristically neurogenic tumors of mediastinal origin. It occurs rarely and is difficult to diagnose by imaging. They are usually benign and exhibit 1 or 2 histological patterns: Antoni A and B. [ 3 ] Esophageal schwannoma are usually seen in Asian patients, with a predominance of women and middle-aged (approximately 50–60 years old) patients. [ 4 ] One of the most common symptoms is dysphagia. Histological and immunohistochemical studies are required for diagnosis, and surgical resection is the primary treatment for the disease.

Abstract:Primary benign schwannoma of the mesentery is extremely rare. To date, only 9 cases have been reported in the English literature, while mesenteric schwannoma with ossified degeneration has not been reported thus far. In the present study, we present the first giant ossified benign mesenteric schwannoma in a 58-year-old female. Ultrasound, computed tomography and magnetic resonance imaging were used, but it was still difficult to determine the definitive location and diagnose the mass. By laparotomy, a 10.0 cm × 9.0 cm × 9.0 cm giant mass was found in the mesentery and was then completely resected. Microscopically, the tumour located in the mesentery mainly consisted of spindle-shaped cells with a palisading arrangement. Some areas of the tumour were ossified, and a true metaplastic bone formation was observed, with the presence of bone lamellae and osteoblasts. Immunohistochemical investigation of the tumour located in the mesentery showed that the staining for the S-100 protein was strongly positive, while the stainings of SMA, CD34, CD117 and DOG-1 were negative. The cell proliferation index, measured with Ki67 staining, was less than 3%. Finally, a giant ossified benign mesenteric schwannoma was diagnosed. After surgery, the patient was followed up for a period of 43 mo, during which she remained well, with no evidence of tumour recurrence.
Clinicopathological features of esophageal schwannomas in mainland China: systematic review of the literature

Abstract:Objective: Esophageal schwannoma (ES) are rare and mostly benign neurogenic tumors. The clinical misdiagnosis rate of it is high. In this study, the clinicopathologic features of ES in mainland China were studied to better understand the disease and improve the diagnosis and treatment rate. Methods: A systematic review was conducted in accordance with PRISMA guidelines. The keywords "esophageal schwannoma", "esophageal neurinoma" and "esophageal neurilemoma" were searched for databases such as Pubmed, EMbase, Wanfang Database and Chinese National Knowledge Infrastructure. The search time frame for database was until July 2019. Combined with our patient, the clinicopathological data and the diagnosis and treatment of ES were summarized. Results: ES occurs in the upper part of the mediastinum and in the thoracic esophagus in most patients in the neck, upper and middle segments. CT and PET/CT examinations can be used for diagnosis, but the differentiation value of both benign and malignant ES is similar. The histopathological findings of forceps biopsy specimens are often difficult to diagnose, and deep tissue biopsies may increase pathological accuracy. EUS-FNA is also recommended for ES diagnosis, but it may also be misdiagnosed. Pathological features include a fusiform arrangement in a palisade-like structure or a tumor cell arranged in a network to form a loose structure. ES characteristic immunohistochemistry results showed that S-100 protein has strong immunological activity. Conclusion: The definitive diagnosis requires immunohistochemistry, especially immunological reaction with S-100 protein. The appropriate treatment plan should be selected according to the diameter of the lesion. The overall prognosis of ES is good, but attention should be paid to follow-up.

Abstract:Esophageal schwannoma was extremely rare and difficult to differentiate from other submucosal tumors. We present a case of a 55-year-old woman who suffered from dysphagia and retrosternal pain for a month. Endoscopic ultrasonography revealed a submucosal tumor, located 23u25cm from the incisors arising from the esophagus. The patient underwent subtotal esophagectomy. The histopathological finding confirmed the diagnosis of esophageal schwannoma. Her postoperative course was uneventful and free of recurrence 2 years after the operation.

Abstract:Objective To study the diagnostic and therapeutic value of endoscopy for gastrointestinal (GI) schwannoma.Methods We retrospectively studied data of 13 patients who underwent endoscopy and were confirmed as having GI schwannoma by pathology in our hospital during January 2006 and July 2011.Results Schwannoma of 8 patiets located in stomach,3 in esophagus and 2 in colon.Endoscopic ultrasonography showed schwannoma originated from muscularis propria,characterized by hypoecho.Twelve patients received endoscopic treatment,and 11 were successfully removed.The en bloc resection rate was 91.7％.Among them,6 were treated by endoscopic submucosal excavation,4 by endoscopic full-thickness resection and one by submucosal tunnel endoscopic resection.Another patient with schwannoma located in esophagus with a maximum diameter of 4.7 cm was transferred to thoracic surgery since the tumor had a wide base even after partial resection.No bleeding,inflammation or abscess occurred.The average follow-up time was 23.3 ± 10.8 months ( range,6-36 months).No metastasis or recurrence was found during follow-up.Conclusion Endoscopy and endoscopic ultrasonography,safe and efficacious,are valuable for the diagnosis and treatment of GI schwannoma.

Abstract:BACKGROUND Schwannomas are uncommon tumors originating from Schwann cells, forming the neural sheath. They account for approximately 2%-6% of all mesenchymal tumors and are most commonly identified in peripheral nerve trunks, with rarity in the gastrointestinal tract. Among gastrointestinal locations, the stomach harbors the majority of nerve sheath tumors, while such occurrences in the sigmoid colon are exceptionally infrequent. CASE SUMMARY This study presented a clinical case involving a 60-year-old female patient who, during colonoscopy, was diagnosed with a submucosal lesion that was later identified as a nerve sheath tumor. The patient underwent surgical resection, and the diagnosis was confirmed through immunohistochemistry. This study highlighted an exceptionally uncommon occurrence of a nerve sheath tumor in the sigmoid colon, which was effectively managed within our department. Additionally, a comprehensive review of relevant studies was conducted. CONCLUSION The preoperative diagnosis of nerve sheath tumors poses challenges, as the definitive diagnosis still relies on pathology and immunohistochemistry. Although categorized as benign, these tumors have the potential to demonstrate malignant behavior. Consequently, the optimal treatment approach entails the complete surgical excision of the tumor, ensuring the absence of residual lesions at the margins.

Abstract:The application of the classical "linear" model of incentive pay to the case when the noise is multiplicative to effort generates two predictions for a given strength of incentives: 1) more risk-averse workers will put in less effort, and 2) setting a performance target will weaken the negative risk aversion--effort link. The data from a real-effort laboratory experiment involving 85 student participants support both these predictions. Implications of the model and empirical findings to the literature on, and practice of, personnel management are discussed.

Abstract:Benign tumors of the esophagus are uncommon, and primary esophageal schwannoma of the esophagus is even rarer, accounting for 2% of cases. Less than 30 cases have been reported in the literature. Here we report a case of a young man with a symptomatic giant esophageal schwannoma, which was completely removed by laparoscopic three-staged esophagectomy. This is the first case of such a large lesion being removed by a minimally invasive approach. Our patient was a 22-year-old male, presented with dysphagia and dyspnea for five years. An endoscopy and CT scan suggested a giant leiomyoma. Postoperative biopsy revealed a primary esophageal schwannoma. We present our case of benign esophageal tumor measuring about 10.5 cm in greatest dimension. This is the first schwannoma resection performed with the Mckeown technique among the limited case reports in the literature. Esophageal schwannoma must be kept as a possible diagnosis in patients presenting with benign esophageal tumors. Esophagectomy is the mainstay of treatment for giant esophageal schwannomas. Clinical Treatment of Gastrinoma: A Case Report and Review of the Literature

Abstract:Gastrinoma is a gastrin-secreting tumor that is associated with Zollinger-Ellison syndrome. The majority of cases occur in the pancreas, followed by the duodenum. Early diagnosis is difficult due to the relative rarity of the tumor and the lack of specific symptoms. In the current study, a 68-year-old female patient presented at the Second Affiliated Hospital, Zhejiang University (Hangzhou, China) due to intermittent abdominal pain and watery diarrhea. The patient was treated by surgical resection and was pathologically diagnosed with a well-differentiated pancreatic neuroendocrine tumor (gastrinoma; grade 1). No evidence of recurrence was observed during 1 year of follow-up. Furthermore, a review of the Chinese literature was performed, which analyzed an additional 17 published cases of gastrinoma. The tumor size ranged between 0.5x0.5 cm and 7.5x6.3x5.1 cm. The pancreas was the most common site of occurrence, accounting for 72% (13/18) of cases, followed by the duodenum (28%; 5/18). The most common initial symptom was abdominal pain (89%; 16/18), followed by diarrhea (56%; 10/18). In 18 cases, including the present case and 17 previous cases, the level of gastrin ranged between 137 and 1,550 pg/ml (normal range, 5‑100 pg/ml). Of the 17 previous cases, 11 patients underwent surgery and 6 patients received conservative therapy due to metastasis or patient choice. Overall, gastrinoma remains a rare disease. Complete removal of the lesion is the standard curative treatment and conservative treatment is only recommended for patients unsuitable for surgery or for those with widespread metastasis.
