Abstract:Background: Periodontal disease, an oral disease that initiates with plaque biofilm infection, affects 10% of the global population. Due to the complexity of tooth root anatomy, biofilm resistance and antibiotic resistance, traditional mechanical debridement and antibiotic removal of biofilms are not ideal. Nitric oxide (NO) gas therapy and its multifunctional therapy are effective methods to clear biofilms. However, large and controlled delivery of NO gas molecules is currently a great challenge. Methods: The core-shell structure of Ag2S@ZIF-90/Arg/ICG was developed and characterized in detail. The ability of Ag2S@ZIF-90/Arg/ICG to produce heat, ROS and NO under 808 nm NIR excitation was detected by an infrared thermal camera, probes and Griess assay. In vitro anti-biofilm effects were evaluated by CFU, Dead/Live staining and MTT assays. Hematoxylin-eosin staining, Masson staining and immunofluorescence staining were used to analyze the therapeutic effects in vivo. Results: Antibacterial photothermal therapy (aPTT) and antibacterial photodynamic therapy (aPDT) could be excited by 808 nm NIR light, and the produced heat and ROS further triggered the release of NO gas molecules simultaneously. The antibiofilm effect had a 4-log reduction in vitro. The produced NO caused biofilm dispersion through the degradation of the c-di-AMP pathway and improved biofilm eradication performance. In addition, Ag2S@ZIF-90/Arg/ICG had the best therapeutic effect on periodontitis and NIR II imaging ability in vivo. Conclusions: We successfully prepared a novel nanocomposite with NO synergistic aPTT and aPDT. It had an outstanding therapeutic effect in treating deep tissue biofilm infection. This study not only enriches the research on compound therapy with NO gas therapy but also provides a new solution for other biofilm infection diseases.

Abstract:Periodontitis is a persistent inflammatory condition caused by plaque biofilm. To achieve successful periodontal tissue repair and halt disease progression, it is imperative to concurrently address the demands for precise antibacterial, anti-inflammatory, and osteogenic interventions within the profound lesions. Herein, we synthesized an intelligent and efficient nanoplatform for the treatment of periodontitis, combining the synergistic therapeutic effects of carbon monoxide (CO) and nitric oxide (NO) gas therapy with the application of photodynamic therapy (PDT). Benefiting from the rapid adhesion to bacteria, responsive CO/NO releasing and ROS generation, the nanoplatform achieved a powerful bactericidal capacity and conspicuous biofilm elimination efficiency under 808 nm near-infrared (NIR) light irradiation. In addition, the combination of CO and NO holds paramount significance, and they serve as crucial endogenous signaling molecules capable of regulating the TLR4/NF-κB/NLRP3 pathway. This combination not only achieved critical protective effects but also fostered the promotion of osteoblast differentiation.

Abstract:Catatonia has been increasingly recognized as a comorbid syndrome of autism at a rate of 12-17% in adolescents and young adults with autism spectrum disorders [1,2] and with other intellectual disabilities [3,4]. Mutism, stereotypic speech, echolalia, stereotypic or repetitive behaviors, posturing, grimacing, rigidity, mannerisms and purposeless agitation are catatonic signs and feature prominently in autism. Some patients, often adolescents, experience a sharp increase of these symptoms and qualify for a diagnosis of catatonia [2,5,6].

Abstract:Periodontal disease is one of the most common oral infectious diseases and closely related to many systemic diseases which is in urgent need of novel and effective drugs. In particular, the problems of deep lesion location and synchronization of antibacterial and anti-inflammatory needs to be addressed. Herein, near infrared (NIR) light triggered nanoplatform with antibacterial and anti-inflammatory dual-functions by antibacterial photodynamic therapy (aPDT), photothermal therapy (PTT) and gas therapy was realized by loading S-nitrosothiols (SNO) and ICG into mesoporous silica-coated gold nanorods. Using a single 808 nm NIR light, the excellent antibacterial efficacy can be achieved not only by aPDT-induced bacteria killing but also by PTT-triggered biofilm dispersal. Furthermore, SNO molecules would also be thermally activated to release nitric oxide (NO) gas molecules at the site of periodontal disease, exerting inflammation inhibition by suppressing the assembly of pro-inflammatory factors and NLRP3 inflammasome. Additionally, such nanoplatform could validly combat pathogenic bacterial biofilm, and modulate the inflammatory immunity, leading to favorable therapeutics on animal models with periodontal diseases. Hence, this design provides an effective solution to solve the problems of deep tissue treatment along with simultaneously antibacterial/anti-inflammatory capabilities, which provides a bright prospect for clinical treatment of periodontal diseases, even extended to other refractory infections in deep sites.

Abstract:Given the limitations of clinical curettage for periodontitis, the thorough removal of dental plaque at deep periodontal pockets and periodontal tissue repair require improvement with the assistance of appropriate medical therapy. Despite the significant developments of various local drug delivery systems (LDDSs) for adjuvant periodontitis therapy, several issues exist, such as the relatively low anti-biofilm effect and the tissue damage caused by excessive reactive oxygen species (ROS). To address the issues, we fabricated the versatile nanoplatforms (PPM NPs) via coating polydopamine (PDA) on the surface of monodispersed Prussian blue nanoparticles (PB NPs) through the facile yet efficient self-polymerization of dopamine (PP NPs), and then loading minocycline (Mino). Notedly, these integrative nanocomposites-based PB NPs as photothermal agents enhanced the therapeutic effect of plaque biofilms in vitro utilizing the antibacterial photothermal therapy (PTT) under irradiation of NIR laser. Further, the designed nanocomposites efficiently scavenged ROS over oxidative-stress Raw267.4 cells and human periodontal ligament cells, attributing to the enzyme-like activity of PB nanozymes and the reducibility of catechol in PDA. In vitro and in vivo consequences demonstrate the significantly augmented antibacterial and anti-inflammatory effects from the live/dead staining of biofilms and western blot results of inflammatory factors compared with free Mino solution in periodontitis. Our fabricated nanoplatforms-based PB NPs conveniently modified with PDA not only significantly improved the antibacterial effect via the combinational therapies but also efficiently scavenged cellular ROS utilizing the enzyme-like activity of nanozymes contributing to the treatment of periodontitis.

Abstract:Using nanomaterials with photodynamic therapy (PDT) to combat bacterial infections including periodontal infections has attracted the attention of researchers because of their high efficiency, simplicity and low susceptibility to drug resistance. However, the therapeutic effectiveness is threatened by the endogenous accumulation of hydrogen peroxide and insufficient amounts of the substrate oxygen. Here, we report a microporous Zr porphyrin carboxylate metal-organic framework (MOF) PCN-222 doped with platinum nanozymes, Pt@PCN-222, to reshape the microenvironment that is not conducive to the recovery of infected tissue and further promote recovery after periodontal tissue infection. This MOFdoped with platinum nanozymes can remove excessive endogenous hydrogen peroxide and produce dissolved oxygen simultaneously. This framework can effectively alleviate anaerobic bacterial infection in periodontal tissue, including efficiently killing the bacteria and reducing the level of inflammation in tissue. This work provides a new strategy to solve the long-term clinical challenge of anaerobic bacterial infections in periodontal tissue and proposes an innovative approach to improve treatment.

Abstract:Biofilm infection is the major concern of medical implants, which often causes progressive destruction of surrounding tissues and systemic diseases. However, biofilm resistance significantly increased the difficulty of curing the infectious diseases of medical implants. Herein, we designed an antibacterial phototherapeutic system by combining polydopamine (PDA)-black phosphorus nanosheets (BP NSs)/ZnO nanowires (NWs) on titanium (Ti) substrates to address the infectious concerns. Assisted by the co-efficiency of BP NSs and PDA, remarkable photothermal therapy (PTT) capability was obtained with near-infrared (NIR) irradiation. Note that the photothermal effect not only dissipates the biofilm, but also enhances the Zn2+ release ability of ZnO, greatly improving antibacterial capability. Therefore, with the "dissipating-killing" synergetic strategy, the localized PTT can potently disperse the bacterial biofilms (Gram-positive Staphylococcus aureus and Gram-negative Escherichia coli) and then ZnO NWs can thoroughly kill bacteria in biofilms in vitro with the enhanced sterilization ability. The combination of PTT and antibacterial ZnO NWs achieves an 99.5 % eradication ratio of biofilm in vivo, which is much better than that of PTT or ZnO NWs alone. Importantly, the NIR-triggered synergistic antibacterial strategy has excellent biosafety, which can effectively control the inflammatory response with no damage to normal tissues.

Abstract:Periodontitis is a chronic inflammatory disease caused by the local microbiome and the host immune response, resulting in periodontal structure damage and even tooth loss. Scaling and root planning combined with antibiotics are the conventional means of nonsurgical treatment of periodontitis, but they are insufficient to fully heal periodontitis due to intractable bacterial attachment and drug resistance. Novel and effective therapeutic options in clinical drug therapy remain scarce. Nanotherapeutics achieve stable cell targeting, oral retention and smart release by great flexibility in changing the chemical composition or physical characteristics of nanoparticles. Meanwhile, the protectiveness and high surface area to volume ratio of nanoparticles enable high drug loading, ensuring a remarkable therapeutic efficacy. Currently, the combination of advanced nanoparticles and novel therapeutic strategies is the most active research area in periodontitis treatment. In this review, we first introduce the pathogenesis of periodontitis, and then summarize the state-of-the-art nanotherapeutic strategies based on the triple concerto of antibacterial activity, immunomodulation and periodontium regeneration, particularly focusing on the therapeutic mechanism and ingenious design of nanomedicines. Finally, the challenges and prospects of nano therapy for periodontitis are discussed from the perspective of current treatment problems and future development trends.

Nanoparticles having amphiphilic silane containing Chlorin e6 with strong anti-biofilm activity against periodontitis-related pathogens

Abstract:Objectives: The objectives of this study were to: (1) develop the multifunctional nanoparticles containing Chlorin e6 (Ce6), Coumarin 6 (C6) and Fe3O4 nanoparticles (NPs); and (2) investigate the inhibitory effects of the nanoparticles via antibacterial photodynamic therapy (aPDT) against three species of periodontitis-related pathogens for the first time. Materials and methods: Ce6 and C6 were co-loaded into the Fe3O4-silane core-shell structure to form multifunctional nanoparticles (denoted "Fe3O4-silane@Ce6/C6 MNPs"). The physical and chemical properties of nanoparticles were characterized. Biofilm properties of Streptococcus sanguinis, Porphyromonas gingivalis and Fusobacterium nucleatum were tested. Colony-forming units (CFU), live/dead assay, and metabolic activity of biofilms were determined to evaluate the aPDT function mediated by the Fe3O4-silane@Ce6/C6 MNPs. Fluorescence imaging and the targeted antibacterial effects were also investigated. Results: Fe3O4-silane@Ce6/C6 MNPs showed superparamagnetic properties, chemical stability and water-solubility, with no cytotoxicity. Fe3O4 NPs did not compromise the emission peaks of C6 and Ce6. The Fe3O4-silane@Ce6/C6-mediated aPDT had much greater reduction in biofilms than the control groups (p < 0.05). Biofilm CFU was reduced by about 4-5 orders of magnitude via Fe3O4-silane@Ce6/C6-mediated aPDT. The co-loading of Ce6 and C6 enabled the real-time aPDT monitoring by ratio emissions with the same wavelength. Fe3O4 with magnetic field enabled the targeting of infection sites by killing bacteria via magnetic field. Conclusion: The multifunctional nanoparticles exerted strong anti-biofilm activity against periodontitis-related pathogens, with excellent biocompatibility, real-time monitoring, and magnetically-targeting capacities. The multifunctional nanoparticles have great potential in antibacterial applications to inhibit the occurrence and progression of periodontitis.
Branched AuAg nanoparticles coated by metal–phenolic networks for treating bacteria-induced periodontitis via photothermal antibacterial and immunotherapy

Abstract:Periodontitis is an inflammatory disease caused by bacteria. The prolonged and repeated occurrence of this disease results from immune system disorders and leads to the destruction of surrounding tissues. Research on new antibacterial materials that can regulate autoimmunity and promote repair can effectively treat periodontal inflammation. Hence, metal–phenolic networks (MPNs) were loaded onto the surface of branched AuAg nanoparticles (NPs), denoted as The procyanidin (PC)-Fe network not only enhanced the photothermal properties of AuAg NPs to achieve effective photothermal antibacterial activity against periodontal pathogens but also alleviated oxidative stress and excessive inflammation. The mechanism of action is as follows: promotes the polarisation of alternatively activated macrophages by activating the phosphoinositide 3-kinase/protein kinase B signaling pathway and upregulates nuclear factor erythroid 2-related factor 2, scavenging reactive oxygen species and subsequently inhibiting the nuclear factor kappa-B signaling pathway to regulate immunity. The ability of periodontal inflammatory tissue to repair was improved in vivo. This design provides new ideas for applying MPNs to photothermal therapy and immunotherapy. It presents a new therapeutic platform for treating periodontitis and other infectious diseases.

Abstract:The efficacious delivery of antimicrobial drugs to intractable oral biofilms remains a challenge due to inadequate biofilm penetration and lack of pathogen targeting. Herein, we have developed a microenvironment-activated poly(ethylene glycol) (PEG)-sheddable nanoplatform to mediate targeted delivery of drugs into oral biofilms for the efficient prevention of dental caries. The PEGylated nanoplatform with enhanced biofilm penetration is capable of deshielding the PEG layer under slightly acidic conditions in a PEG chain length-dependent manner to re-expose the bacteria-targeting ligands, thereby facilitating targeted codelivery of ciprofloxacin (CIP) and IR780 to the bacteria after accumulation within biofilms. The nanoplatform tends to induce bacterial agglomeration and suffers from degradation in the acidic oral biofilm microenvironment, triggering rapid drug release on demand around bacterial cells. The self-modulating nanoplatform under near-infrared (NIR) irradiation accordingly displays greatly augmented potency in oral biofilm penetration and disruption compared with drugs alone. Topical oral treatment with nanoplatforms involving synergetic pharmacological and photothermal/photodynamic trinary therapy results in robust biofilm dispersion and efficacious suppression of severe tooth decay in rats. This versatile nanoplatform can promote local accumulation and specific drug transport into biofilms and represents a new paradigm for targeted drug delivery for the management of oral biofilm-associated infections.

Abstract:Introduction: Periodontitis is a chronic inflammatory disease brought on by various bacteria, and effective antibacterial, anti-inflammatory and alveolar bone regeneration are the main goals of treating periodontal disease. Methods: In the current work, we employed Icariin (ICA) into a hydrogel modified with carbon nanofiber (CNF) to create a multifunctional composite nanoplatform. The composite was activated in the near infrared (NIR) to treat periodontitis. Results: The antibacterial results showed that the ICA+CNF@H showed 94.2% and 91.7% clearance of S. aureus and E. coli , respectively, under NIR irradiation. In vitro experiments showed that NIR-irradiated composites suppressed inflammatory factor (IL-6) and ROS expression and up-regulated the performance of anti-inflammatory factor (IL-10) in RAW264.7 cells. At the same time, the composites promoted the production of osteogenic factors in BMSCs, with an approximately 3-fold increase in alkaline phosphatase activity after 7 days and an approximately 2-fold increase in the rate of extracellular matrix mineralization after 21 days. In vivo tests showed that the alveolar bone height was clearly greater in the ICA+CNF@H (NIR) group compared to the periodontitis group. Discussion: In conclusion, ICA+CNF@H under NIR irradiation achieved a synergistic effect of antibacterial, anti-inflammatory, reduction of reactive oxygen species and promotion of osteogenesis, offering a novel approach for treating periodontitis.

Abstract:Antibacterial photodynamic therapy (aPDT) has emerged as an attractive treatment option for efficient removal of pathogenic bacteria. However, aPDT in deep tissue will encounter difficulties such as limited light penetration depth, insufficient oxygen (O2) supply and inability to eliminate inflammation introduced by bacteria, which hinders its clinical application. Herein, the near infrared (NIR) strategy of simultaneously generating O2 and CO was developed for aPDT based antibacterial therapy and mitigation of deep infection inflammation. Methods: We prepared NIR-mediated multifunctional aPDT nanoplatform (POS-UCNPs/ICG) producing therapeutic gas of O2 and CO. The CO, O2 and ROS generation of the nanoplatform were characterized by dye probes, respectively. The antibacterial activity and anti-inflammation of POS-UCNPs/ICG were demonstrated in vitro and in vivo. In addition, the therapeutic effects in vivo were serially analyzed by immunofluorescence staining, Masson's staining, hematoxylin and eosin staining, colony formation units (CFU) and so on. Results: NIR-mediated multifunctional aPDT nanoplatform was realized by combining the up-conversion nanoparticles (UCNPs) and partially oxidized SnS2 (POS) nanosheets (NSs) as well as indocyanine green (ICG). Using a single 808 nm light, aPDT can be achieved via ICG molecules, meanwhile, O2/CO can be generated by POS NSs through upconversion light excitation. During the aPDT process, O2 can enhance aPDT, while CO can regulate inflammation through the PI3K/NF-κB pathway. Therefore, POS-UCNPs/ICG groups had a highest percentage of healing area up to 91.55±1.26% in mouse abscess model. Conclusion: Due to enhanced aPDT and anti-inflammatory collaborative therapy, the POS-UCNPs/ICG composites showed remarkably accelerated recovery in animal abscess models. Such NIR light responsive nanoplatform with optimized antibacterial capacity and immunomodulatory functions is promising for clinical therapeutics of bacteria-induced infections.

Abstract:Background: The antimicrobial activity of metallic nanoparticles (NPs) has been confirmed to fight a broad spectrum of microorganisms, through antimicrobial effects that are amplified when these particles are irradiated with light of the proper wavelength. This is the first study to use phytoconjugated Zinc oxide (ZnO) NPs containing traces of active biomolecules derived from Emblica officinalis (E. officinalis) plant extract in antimicrobial photocatalysis (PCT) during non-surgical periodontal therapy. Objectives: This study aimed to evaluate the effects of repeated PCT application in the treatment of periodontitis, using a gel containing bio-hydrothermally synthesized ZnO NPs and visible light as an adjunct to scaling and root planing (SRP). Methodology: In total, 16 systemically healthy volunteers with stage 3 grade B generalized periodontitis were recruited for this prospective double blind, randomized placebo-controlled trial. After receiving SRP, the subjects received the following interventions in a split-mouth design at baseline, 1 week and 1 month: Group 1 - Placebo gel + Sham PCT; Group 2 - Nano ZnO gel + Sham PCT; Group 3 - Placebo gel + PCT; and Group 4 - Nano ZnO gel + PCT. The site-specific profile of Porphyromonas gingivalis in the subgingival plaque and clinical parameters (Plaque Index, Gingival Index, Gingival Bleeding Index, Probing pocket Depth and Clinical Attachment Level) were assessed at baseline, 1 month and 3 months. Results: All interventions tested caused participants' clinical and microbiological parameters to generally improve after 3 months. Subjects who received the Nano ZnO gel + PCT combination showed a sustained and progressive improvement in their treatment outcomes, a result that presented statistically significant differences from the outcomes obtained through the remaining interventions, at all time points during the study period. Conclusions: The repeated application of PCT using bio-hydrothermally synthesized ZnO NPs can effectively complement SRP in the non-surgical treatment of Periodontitis.

Abstract:Abstract Background Periodontitis is a chronic inflammatory disease in oral cavity owing to bacterial infection. Photothermal therapy (PTT) and photodynamic therapy (PDT) have many advantages for antibacterial treatment. As an excellent photosensitizer, indocyanine green (ICG) shows prominent photothermal and photodynamic performances. However, it is difficult to pass through the negatively charged bacterial cell membrane, thus limiting its antibacterial application for periodontitis treatment. Results In this work, self-assembled nanoparticles containing ICG and polycationic brush were prepared for synergistic PTT and PDT against periodontitis. First, a star-shaped polycationic brush poly(2-(dimethylamino)ethyl methacrylate) (sPDMA) was synthesized via atom transfer radical polymerization (ATRP) of DMA monomer from bromo-substituted β-cyclodextrin initiator (CD-Br). Next, ICG was assembled with sPDMA to prepare ICG-loaded sPDMA (sPDMA@ICG) nanoparticles (NPs) and the physicochemical properties of these NPs were characterized systematically. In vitro antibacterial effects of sPDMA@ICG NPs were investigated in porphyromonas gingivalis (Pg), one of the recognized periodontitis pathogens. A ligature-induced periodontitis model was established in Sprague–Dawley rats for in vivo evaluation of anti-periodontitis effects of sPDMA@ICG NPs. Benefiting from the unique brush-shaped architecture of sPDMA polycation, sPDMA@ICG NPs significantly promoted the adsorption and penetration of ICG into the bacterial cells and showed excellent PTT and PDT performances. Both in vitro and in vivo, sPDMA@ICG NPs exerted antibacterial and anti-periodontitis actions via synergistic PTT and PDT. Conclusions A self-assembled nanosystem containing ICG and polycationic brush has shown promising clinical application for synergistic PTT and PDT against periodontitis. Graphical Abstract

Abstract:The threat posed by antibiotic-resistant bacteria and the challenge of biofilm formation has highlighted the inadequacies of conventional antibacterial therapies, leading to increased interest in antibacterial photodynamic therapy (aPDT) in recent years. This approach offers advantages such as minimal invasiveness, low systemic toxicity, and notable effectiveness against drug-resistant bacterial strains. Porphyrins and their derivatives, known for their high molar extinction coefficients and singlet oxygen quantum yields, have emerged as crucial photosensitizers in aPDT. However, their practical application is hindered by challenges such as poor water solubility and aggregation-induced quenching. To address these limitations, extensive research has focused on the development of porphyrin-based nanomaterials for aPDT, enhancing the efficacy of photodynamic sterilization and broadening the range of antimicrobial activity. This review provides an overview of various porphyrin-based nanomaterials utilized in aPDT and biofilm eradication in recent years, including porphyrin-loaded inorganic nanoparticles, porphyrin-based polymer assemblies, supramolecular assemblies, metal-organic frameworks (MOFs), and covalent organic frameworks (COFs). Additionally, insights into the prospects of aPDT is offered, highlighting its potential for practical implementation.
