Abstract:Abstract This case report investigates the radiation dose received by a paediatric patient with a ventricular assist device who underwent four non-contrast brain computed tomography (CT) scans, two brain perfusion CT scans and two head angiographic CT scans. The total estimated absorbed dose to the lens of the eye is above the 500 mGy radiation-induced cataract threshold. It is recommended that this patient and those with similar imaging histories have routine follow-up with an ophthalmologist. It is also recommended that radiation dose tracking and an electronic medical alert program be implemented to allow the identification of patients who may exceed tissue reaction thresholds.

Abstract:Radiation dose management of medical staff has become increasingly important. Particularly, based on the statement by the International Commission on Radiological Protection (ICRP) in 2011, a new lower equivalent dose limit for the eye lens is being established in each country. Although many reports have discussed the occupational radiation dose in interventional radiology (IR), few studies have examined the dose during computed tomography (CT) examinations. This study investigated the radiation dose exposure to medical staff present in the CT room during irradiation, with particular focus on the exposure to eye lens. The radiation dose exposure to those who assist patients during head, chest and upper abdomen CT examination was measured in a phantom study. The radiation dose exposure with scattered radiation was never negligible (i.e., high); Hp(3) was the highest in head CT examination, at 0.44 mSv per examination. Furthermore, the shielding effect of radiation protection glasses was large, and radiation protection glasses are useful tools for the medical staff who are involved in CT examinations. The justification and optimisation should be carefully considered in assistant actions.

Abstract:The ability of modern multidetector CT scanners with submillimeter resolution, subsecond rotation time, and large volume imaging has resulted in widespread utilization of cardiovascular computed tomographic angiography (CCTA) [1]. However, the widespread use of CCTA has also raised concerns about the radiation dose to the patients. The National Council on Radiation Protection, NCRP Report No. 160, reported that the radiation exposure to the United States population due to medical sources increased more than 7 times in the 20 years between 1986 and 2006 [2]. Although in 2006 CT constituted about 10% of the diagnostic examinations that utilize X-rays, it contributed to nearly 50% of the population dose [2–4]. Based on our current knowledge of radiation biology, the deleterious effects of radiation are cumulative and medical radiation is increasingly a significant contributor to the amount of radiation accumulated in a person’s lifetime [1, 2, 5]. The risk of cancer from radiation exposure is especially worrisome in children and young women who received multiple CT examinations early in their lives. For example, studies found that one CT examination of the female chest gives as much radiation as 10 mammograms to each breast [6]. Therefore, the practitioners of CT must be constantly aware of the risks of radiation and strive toward applying the lowest dose to the patient consistent with the clinical study.

Abstract:The primary objective of this investigation is to comprehensively evaluate potential strategies to mitigate the risk of radiation-induced ailments stemming from the excessive exposure of radiosensitive organs such as the eyes, thyroid, breast, and gonads during CT scans, without compromising diagnostic image quality. In the methodology, a thorough examination and synthesis of existing literature were conducted, encompassing various studies and their respective findings. By scrutinizing the results and conclusions of these studies, the aim was to identify optimal approaches for minimizing the risk of radiation-related diseases associated with the overexposure of radiosensitive organs. The culmination of this analysis revealed a range of methods tailored to protect specific radiosensitive organs. Notably, for safeguarding the eye lens, gantry tilting emerged as the most efficacious technique. However, circumstances may arise where gantry tilting is impractical; in such cases, the utilization of silicon rubber shielding combined with tube current modulation was deemed viable. Furthermore, for other radiosensitive organs, such as the thyroid, breast, and gonads, the implementation of tube current modulation, supplemented by ADMIRE 3, was identified as a significantly effective measure. In conclusion, this study underscores the importance of adopting targeted strategies to mitigate radiation risks while preserving diagnostic image quality during CT scans. By leveraging techniques such as gantry tilting, silicon rubber shielding, and tube current modulation with ADMIRE 3, healthcare practitioners can enhance patient safety and minimize the likelihood of radiation- induced health complications. Additionally, ensuring the authenticity of these findings, plagiarism was rigorously checked to maintain the integrity of the research.

Abstract:Purpose: To investigate the eye lens dose reduction by CT scan with organ based tube current modulation (OBTCM) using GPU Monte Carlo code ARCHER-CT. Methods: 36 X-ray sources and bowtie filters were placed around the patient head with the projection angle interval of 10° for one rotation of CT scan, each projection was simulated respectively. The voxel eye models with high resolution(0.1mm*0.1mm*0.1mm) were used in the simulation and different tube voltage including 80kVp, 100kVp, 120kVp and 140kVp were taken into consideration. Results: The radiation doses to the eye lens increased with the tube voltage raised from 80kVp to 140kVp, and the dose results from 0° (AP) direction are much higher than those from 180° (PA) direction for all the 4 different tube voltage investigated. This 360° projection dose characteristic enables organ based TCM, which can reduce the eye lens dose by more than 55%. Conclusion: As the eye lens belongs to superficial tissues, its radiation dose to external exposure like CT is direction sensitive, and this characteristic feature makes organ based TCM to be an effective way to reduce the eye lens dose, so more clinical use of this technique were recommended. Abstract:"When one admits that nothing is certain one must, I think, also admit that some things are much more nearly certain than others." Bertrand Russell (1872-1970) Computed tomography (CT) is one of the largest contributors to man-made radiation doses in medical populations. CT currently accounts for over 60 million examinations in the United States, and its use continues to grow rapidly. The principal concern regarding radiation exposure is that the subject may develop malignancies. For this systematic review we searched journal publications in MEDLINE (1966-2006) using the terms "CT," "ionizing radiation," "cancer risks," "MRI," and "patient safety." We also searched major reports issued from governmental U.S. and world health-related agencies. Many studies have shown that organ doses associated with routine diagnostic CT scans are similar to the low-dose range of radiation received by atomic-bomb survivors. The FDA estimates that a CT examination with an effective dose of 10 mSv may be associated with an increased chance of developing fatal cancer for approximately one patient in 2000, whereas the BEIR VII lifetime risk model predicts that with the same low-dose radiation, approximately one individual in 1000 will develop cancer. There are uncertainties in the current radiation risk estimates, especially at the lower dose levels encountered in CT. To address what should be done to ensure patient safety, in this review we discuss the "as low as reasonably achievable" (ALARA) principle, and the use of MRI as an alternative to CT.

Abstract:Epidemiological studies of patient populations have shown that high doses of radiation increase risks of cardiovascular disease (CVD). Results from a recent meta-analysis of 93 epidemiological studies covering a wide range of doses provided evidence of a causal association between radiation exposure and CVD, and indicated excess relative risk per Gy for maximum dose below 500 mGy or delivered at low dose rates. These doses cover the range of organ doses expected from multiple diagnostic computed tomography (CT) scans. Dose-effect factors for the excess absolute risk of mortality from CVD following radiation exposure were derived from the meta-analysis. The present study uses these factors to estimate excess risks of mortality for various types of CVD, including cerebrovascular disease (CeVD), from CT scans of the body and head, assuming that the meta-analytic factors were accurate and represented a causal relationship. Estimates are based on cumulative doses to the heart and brain from CT scans performed on 105,574 patients on 12 CT scanners over a period of 51⁄2 years. The results suggest that the excess number of deaths from CeVD could be 7 or 26 per 100,000 patients depending whether threshold brain doses of 200 mGy or 50 mGy, respectively are assumed. These results could have implications for head CT scans. However, the results rely on the validity of risk factors derived in the meta-analysis informing this assessment and which include significant uncertainties. Further incidence studies should provide better information on risk factors and dose thresholds, particularly for CeVD following head CT scans.

Abstract:PURPOSE:To investigate lens dose reduction with organ based tube current modulation (TCM) using the Monte Carlo method.METHODS:To calculate lens dose with organ based TCM, 36 pairs of X-ray sources with bowtie filters were placed around the patient head using a projection angle interval of 10° for one rotation of Computed Tomography (CT). Each projection was simulated respectively. Both voxelized and stylized eye models and Chinese reference male phantoms were used in the simulation, and tube voltages 80, 100, 120 and 140 kVp were used.RESULTS:Dose differences between two eye models were less than 20%, but large variations were observed among dose results from different projections of all tube voltages investigated. Dose results from 0° (AP) directions were 60 times greater than those from 180° (PA) directions, which enables organ based TCM reduce lens doses by more than 47%.CONCLUSIONS:Organ based TCM may be used to reduce lens doses. Stylized eye models are more anatomically realistic compared with voxelized eye models and are more reliable for dose evaluation.
Evaluation of radiation dose and radiation-induced cancer risk associated with routine CT scan examinations

Abstract:Using patient-specific data, the present study aimed to identify organ and effective dose to estimate the lifetime attributable risks (LARs) of cancer incidence and mortality associated with routine CT procedures. A cross-sectional investigation was conducted on 515 patients who underwent CT scan. The effective dose was computed using two methods: first, the ImPACT software and relevant conversion factors. Second, the effective dose was determined using the scanner-derived DLP and AAPM TG 23's conversion factor. Using the biological effects of ionizing radiation (BEIR VII) report, LAR of cancer incidence and mortality were computed. The greatest mean organ doses for abdomen-pelvis, routine chest, chest HRCT, brain, and sinus examinations were 20.64 ± 3.27 mGy (kidneys), 21.55 ± 5.47 mGy (thymus and esophagus), 10.51 ± 1.31 mGy (thymus and esophagus), 22.72 ± 3.01 mGy (lens), and 18.56 ± 1.25 mGy (lens) respectively. Using ImPACT dosimetry software, the mean effective dose for routine chest, abdomen-pelvis, HRCT chest, brain, and sinus examinations was 8.46 mSv, 10.13 mSv, 4.19 mSv, 1 mSv, and 0.68 mSv, respectively. Using the DLP to effective dose conversion factor, the mean effective dose was lower. The mean cancer incidence risks per 100,000 people were 63.33, 54.98, 27.82, 6.31, and 5.22 for abdomen-pelvis, routine chest, HRCT, brain, and sinus examinations, respectively. The mean LAR of cancer mortality for all cancer was 38.22, 33.17, 16.72, 3.87, 2.93 for abdomen-pelvis, routine chest, HRCT, brain, and sinus examinations, respectively, per 100,000 people. Thus, CT scan exams are connected with a non-negligible risk of cancer, and patient radiation dose must be optimized to limit radiation risk.

Abstract:Computed tomography (CT) multi-detector array has been heavily utilized over the past decade. While transforming an individual's diagnosis, the risk of developing pathogenesis as a result remains a concern. The main aim of this institutional cumulative effective dose (CED) review is to highlight the number of adult individuals with a record of CED ≥ 100 mSv over a time span of 5 years. Further, we aim to roughly estimate both incidence and mortality life-attributable risks (LARs) for the shortlisted individuals. CT studies performed over one year, in one dedicated trauma and emergency facility, were retrospectively retrieved and analyzed. Individuals with historical radiological CED ≥ 100 mSv were short-listed. LARs were defined and established based on organ, age and gender. Out of the 4406 CT studies reviewed, 22 individuals were found with CED ≥ 100 mSv. CED varied amongst the short-listed individuals, with the highest CED registered being 223.0 mSv, for a 57-year-old male, cumulated over an average study interval of 46.3 days. The highest median mortality risk was for females, 214 per 100,000 registered for the age group 51-60 years. While certain clinical indications and diseases require close follow-up using radiological examinations, the benefit-to-risk ratio should be carefully considered, particularly when CT is requested.

Abstract:Dynamic contrast-enhanced (DCE) CT studies are increasingly used in both medical care and clinical trials to improve diagnosis and therapy management of the most common life-threatening diseases: stroke, coronary artery disease and cancer. It is thus the aim of this review to briefly summarize the current knowledge on deterministic and stochastic radiation effects relevant for patient protection, to present the essential concepts for determining radiation doses and risks associated with DCE-CT studies as well as representative results, and to discuss relevant aspects to be considered in the process of justification and optimization of these studies. For three default DCE-CT protocols implemented at a latest-generation CT system for cerebral, myocardial and cancer perfusion imaging, absorbed doses were measured by thermoluminescent dosimeters at an anthropomorphic body phantom and compared with thresholds for harmful (deterministic) tissue reactions. To characterize stochastic radiation risks of patients from these studies, life-time attributable cancer risks (LAR) were estimated using sex-, age-, and organ-specific risk models based on the hypothesis of a linear non-threshold dose-response relationship. For the brain, heart and pelvic cancer studies considered, local absorbed doses in the imaging field were about 100-190 mGy (total CTDI(vol), 200 mGy), 15-30 mGy (16 mGy) and 80-270 mGy (140 mGy), respectively. According to a recent publication of the International Commission on Radiological Protection (ICRP Publication 118, 2012), harmful tissue reactions of the cerebro- and cardiovascular systems as well as of the lenses of the eye become increasingly important at radiation doses of more than 0.5 Gy. The LARs estimated for the investigated cerebral and myocardial DCE-CT scenarios are less than 0.07% for males and 0.1% for females at an age of exposure of 40 years. For the considered tumor location and protocol, the corresponding LARs are more than 6 times as high. Stochastic radiation risks decrease substantially with age and are markedly higher for females than for males. To balance the diagnostic needs and patient protection, DCE-CT studies have to be strictly justified and carefully optimized in due consideration of the various aspects discussed in some detail in this review.

Abstract:Workers occupationally exposed to ionizing radiation, including nuclear medicine workers, are one of the groups at higher risk of a radiation cataract. Reducing the lens dose limit from 150 mSv/year to 20 mSv (averaged over defined periods of 5 yrs, or 50 mSv in 1 yr) makes other risk factors less obvious. The presented work is a review of the literature not only in the scope of the most obvious risk factor in this occupational group - ionizing radiation, with special attention given to issues specific to nuclear medicine (e.g. the variety of radionuclides or manual performing of procedures), as well as the difficulties related to the routine monitoring of eye lens exposure. Moreover, the paper presents several other risk factors that may play a key role in modulating damage to the eye lens. These include age, exposure to UV radiation, oxidative stress, genetic factors and dietary nutrient intake. An overview of all known molecular and biophysical mechanisms that imply the modulation of the cataract-related mechanism most often focus on radiation protection, has been given. The prepared review is the basis for the possibility of developing new studies aimed to introduce new protocols based on the intake of specific nutrients that could reduce the effects of the dose absorbed by the lens and, consequently, reduce the onset of cataracts in an employee involved in nuclear medicine practices.

Abstract:In 2011, the International Commission on Radiological Protection issued a statement on tissue reactions (formerly termed non-stochastic or deterministic effects) to recommend lowering the threshold for cataracts and the occupational equivalent dose limit for the crystalline lens of the eye. Furthermore, this statement was the first to list circulatory disease (cardiovascular and cerebrovascular disease) as a health hazard of radiation exposure and to assign its threshold for the heart and brain. These changes have stimulated various discussions and may have impacts on some radiation workers, such as those in the medical sector. This paper considers emerging issues associated with cataracts and cardiovascular disease. For cataracts, topics dealt with herein include (i) the progressive nature, stochastic nature, target cells and trigger events of lens opacification, (ii) roles of lens protein denaturation, oxidative stress, calcium ions, tumor suppressors and DNA repair factors in cataractogenesis, (iii) dose rate effect, radiation weighting factor, and classification systems for cataracts, and (iv) estimation of the lens dose in clinical settings. Topics for cardiovascular disease include experimental animal models, relevant surrogate markers, latency period, target tissues, and roles of inflammation and cellular senescence. Future research needs are also discussed.

Abstract:The International Commission on Radiological Protection (ICRP) has sustained interest in radiological protection in computed tomography (CT), and ICRP Publications 87 and 102 focused on the management of patient doses in CT and multi-detector CT (MDCT) respectively. ICRP forecasted and ‘sounded the alarm’ on increasing patient doses in CT, and recommended actions for manufacturers and users. One of the approaches was that safety is best achieved when it is built into the machine, rather than left as a matter of choice for users. In view of upcoming challenges posed by newer systems that use cone beam geometry for CT (CBCT), and their widened usage, often by untrained users, a new ICRP task group has been working on radiological protection issues in CBCT. Some of the issues identified by the task group are: lack of standardisation of dosimetry in CBCT; the false belief within the medical and dental community that CBCT is a ‘light’, low-dose CT whereas mobile CBCT units and newer applications, particularly C-arm CT in interventional procedures, involve higher doses; lack of training in radiological protection among clinical users; and lack of dose information and tracking in many applications. This paper provides a summary of approaches used in CT and MDCT, and preliminary information regarding work just published for radiological protection in CBCT.
Impact of Patient Head Posture on Lens Radiation Exposure During Cerebral Angiography

Abstract:Background and purpose: Cerebral angiography remains crucial for detailed characterization and preoperative assessments for intracranial aneurysm. Despite its diagnostic importance, cerebral angiography poses challenges due to its invasiveness, the risk of neurological complications, and radiation exposure. To investigate the impact of head posture on lens radiation exposure during cerebral angiography, this study focused on the correlation between radiation doses to the eye lens, head flexion angles, and head size. Materials and methods: A retrospective analysis was performed on 20 patients who underwent cerebral angiography for unruptured intracranial aneurysms between October and November 2022. Radiation doses to the lens, which were measured in a prior prospective study using photoluminescent glass dosimeters, were analyzed alongside head flexion angles, anteroposterior (AP) head diameters, and kerma-area product (KAP) to evaluate their correlation with lens radiation exposure. The lateral radiation source is located on the left side of the patients. Results: The cohort consisted of 20 patients (60% female, mean age: 62.3 ± 9.9 years). The radiation dose to the left eye (the eye closer to the x-ray source) was 2.8 times higher than that to the right eye (9.18 ± 3.31 mGy vs. 3.3 ± 0.60 mGy, P < 0.001). A strong positive correlation was observed between the left eye lens dose and head flexion angle (R = 0.815, P < 0.001). While the AP head diameter significantly correlated with the flexion angle, it showed no significant correlation with lens dose. The KAP was inversely correlated with both the left lens dose (R = -0.597, P = 0.005) and the flexion angle (R = -0.689, P < 0.001). Conclusions: Our findings underscore the significant impact of head posture on lens radiation exposure during cerebral angiography. Adjusting head positioning may provide a practical approach to reduce radiation exposure to the lens. Furthermore, it is worth noting that the left lens received more radiation than the right, likely due to the X-ray source being on the left side of the patient. Abstract:Biological effectiveness of ionizing radiation differs with its linear energy transfer (LET) such that high-LET radiation is more effective for various biological endpoints than low-LET radiation. Human exposure to high-LET radiation occurs in cancer patients, nuclear workers, aviators, astronauts and other space travellers. From the radiation protection viewpoint, the ocular lens is among the most radiosensitive tissues in the body, and cataract (a clouding of the normally transparent lens) is classified as tissue reactions (formerly called nonstochastic or deterministic effects) with a threshold below which no effect would occur. To prevent radiation cataracts, the International Commission on Radiological Protection (ICRP) has recommended an equivalent dose limit for the lens according to the threshold for vision-impairing cataracts. ICRP recommended the threshold of >8Gy in 1984 and an occupational dose limit of 150mSv/year in 1980. These remained unchanged until 2011, when ICRP recommended lowering the threshold to 0.5Gy and the dose limit to 20mSv/year (averaged over 5 years with no single year exceeding 50mSv). Although such reduction of the threshold was based on findings from low-LET radiation, the dose limit was recommended in Sv. Historically, the lens is the exceptional tissue for which ICRP had assigned a special factor in addition to a general radiation weighting factor, predicated on a belief that the lens is more vulnerable to high-LET radiation than other tissues. Considering such radiosensitive nature of the lens, a deeper understanding of a cataractogenic potential of high-LET radiation is indispensable. This review is thus designed to provide an update on the current knowledge as to high-LET radiation cataractogenesis. To this end, changes in ICRP recommendations on lenticular radiation protection, epidemiological and biological findings on high-LET cataractogenesis are reviewed, and future research needs are then discussed.
Advanced Computational Methods for Radiation Dose Optimization in CT

Abstract:Background: In planning radiotherapy treatments, computed tomography (CT) has become a crucial tool. CT scans involve exposure to ionizing radiation, which can increase the risk of cancer and other adverse health effects in patients. Ionizing radiation doses for medical exposure must be kept "As Low As Reasonably Achievable". Very few articles on guidelines for radiotherapy-computed tomography scans are available. This paper reviews the current literature on radiation dose optimization based on the effective dose and diagnostic reference level (DRL) for head, neck, and pelvic CT procedures used in radiation therapy planning. This paper explores the strategies used to optimize radiation doses, and high-quality images for diagnosis and treatment planning. Methods: A cross-sectional study was conducted on 300 patients with head, neck, and pelvic region cancer in our institution. The DRL, effective dose, volumetric CT dose index (CTDIvol), and dose-length product (DLP) for the present and optimized protocol were calculated. DRLs were proposed for the DLP using the 75th percentile of the distribution. The DLP is a measure of the radiation dose received by a patient during a CT scan and is calculated by multiplying the CT dose index (CTDI) by the scan length. To calculate a DRL from a DLP, a large dataset of DLP values obtained from a specific imaging procedure must be collected and can be used to determine the median or 75th-percentile DLP value for each imaging procedure. Results: Significant variations were found in the DLP, CTDIvol, and effective dose when we compared both the standard protocol and the optimized protocol. Also, the optimized protocol was compared with other diagnostic and radiotherapy CT scan studies conducted by other centers. As a result, we found that our institution's DRL was significantly low. The optimized dose protocol showed a reduction in the CTDIvol (70% and 63%), DLP (60% and 61%), and effective dose (67% and 62%) for both head, neck, and pelvic scans. Conclusions: Optimized protocol DRLs were proposed for comparison purposes.

Abstract:<span>There are well-known correlations between high and moderate doses (>0.5 Gy) of ionizing radiation exposure and circulatory system damage, also between radiation and posterior subcapsular cataract. At lower dose correlations with circulatory disease are emerging in the Japanese atomic bomb survivors and in some occupationally exposed groups, and are still to some extent controversial. Heterogeneity in excess relative risks per unit dose in epidemiological studies at low (<0.1 Gy) and at low-moderate (>0.1 Gy, <0.5 Gy) doses may result from confounding and other types of bias, and effect modification by established risk factors. There is also accumulating evidence of excess cataract risks at lower dose and low dose rate in various cohorts. Other ocular endpoints, specifically glaucoma and macular degeneration have been little studied. In this paper, we review recent epidemiological findings, and also discuss some of the underlying radiobiology of these conditions. We briefly review some other types of mainly neurological nonmalignant disease in relation to radiation exposure.We document statistically significant excess risk of the major types of circulatory disease, specifically ischemic heart disease and stroke, in moderate- or low-dose exposed groups, with some not altogether consistent evidence suggesting dose-response non-linearity, particularly for stroke. However, the patterns of risk reported are not straightforward. We also document evidence of excess risks at lower doses/dose-rates of posterior subcapsular and cortical cataract in the Chernobyl liquidators, US Radiologic Technologists and Russian Mayak nuclear workers, with fundamentally linear dose-response. Nuclear cataracts are less radiogenic. For other ocular endpoints, specifically glaucoma and macular degeneration there is very little evidence of effects at low doses; radiation-associated glaucoma has been documented only for doses >5 Gy, and so has the characteristics of a tissue reaction. There is some evidence of neurological detriment following low-moderate dose (∼0.1–0.2 Gy) radiation exposure <i>in utero</i> or in early childhood.</span>
