Abstract:Background: There is a significant need for soft-tissue replacements in the field of reconstructive surgery. Decellularized adipose tissues were heparin crosslinked and loaded with basic fibroblast growth factor (bFGF). This injectable system was evaluated for its adipogenic and angiogenic capabilities for in vivo adipose tissue regeneration. Methods: Decellularized adipose tissues were harvested from the inguinal fat pads of C57BL/6J mice, minced, and heparinized before being loaded with bFGF. Decellularized adipose tissues without bFGF served as a control. In vivo adipose neotissue formation, neovascularization, and volume stability were evaluated over a period of 12 weeks. After 6 or 12 weeks, mice were killed and the newly formed adipose tissues, together with the contralateral endogenous adipose tissues, were harvested for gross, volumetric, histologic, and immunohistochemical analysis. Results: Decellularized adipose tissues that were heparinized and loaded with bFGF induced significant de novo adipose neotissue formation, with progressive tissue growth and neovascularization from 6 to 12 weeks. The adipose neotissues exhibited mature adipose morphology and extracellular matrix that closely resembled that of the endogenous adipose tissue. In contrast, decellularized adipose tissues without bFGF induced limited adipose neotissue formation and were completely resorbed by the end of 12 weeks. Conclusion: This study demonstrates the high efficiency of heparinized decellularized adipose tissue matrix loaded with bFGF in promoting adipose neotissue formation and neovascularization with long-term volume stability.
Regional-Controlled Tissue Expanders Increase Skin Expansion and Thickness Compared to Standard Tissue Expanders in a Rat Model

Abstract:Background: Skin and soft-tissue expansion are widely used. However, the existing methods cannot expand targeted areas on the top flap. Thus, the authors developed a new expander with a partially thickened top. The authors hypothesized that pressure differences would lead to higher growth near nonthickened regions and lower growth near thickened regions, allowing targeted expansion. Methods: Eighteen male Sprague Dawley rats (8 weeks old) were used; 20-ml rectangular regional-controlled expanders ( n = 12) and ordinary expanders ( n = 6) were implanted. Flaps on regional-controlled expanders were divided into nonthickened and thickened regions and tattooed. Discontinuous inflation began 14 days postoperatively, 3 ml every 3 days, until the volume reached 50 ml. Tattooed skin area and thickness were measured. Immunofluorescence staining detected cell proliferation (proliferating cell nuclear antigen-positive) and vascular density (CD31 + ). Growth factors (transforming growth factor-β, epidermal growth factor, vascular endothelial growth factor, and basic fibroblast growth factor) were assessed by enzyme-linked immunosorbent assay. Results: The expanded skin area of regional-controlled expansion nonthickened regions (396.2 ± 41.4 mm 2 ) was 33.8 ± 10.0 percent larger than that of thickened regions (297.8 ± 38.9 mm 2 ). Thickened regions had a 28.9 ± 14.6 percent thicker dermal layer (942.4 ± 55.5 µm) than nonthickened regions (737.1 ± 64.5 µm). Nonthickened regions had 295.0 ± 145.0 percent more proliferating cell nuclear antigen-positive cells (92.4 ± 16.2/mm 2 ) than thickened regions (25.6 ± 7.4/mm 2 ). The vascular density was 133.0 ± 61.7 percent higher in thickened regions (24.8 ± 4.7/mm 2 ) than in nonthickened regions (11.1 ± 2.7/mm 2 ) (all above, p < 0.05). Conclusions: Regional-controlled expansion specifically expands only the targeted area, causing thicker skin flaps with abundant vessels for defect repair. Although this technique has great clinical potential, it should be further validated with large animals and humans. ( Plast. Reconstr. Surg. 150: 1273, 2022.). Clinical relevance statement: This study presents the newly developed regional-controlled expansion technique that realized the targeted expansion. It is suitable for repairing defects and would contribute to shortening the expansion process and reducing complication rates.

Abstract:This study examined the biocompatibility and expansion volume of tissue expanders utilizing rabbits and beagles as experimental models. The self-inflatable expander was provided using a Tissue balloon ® (Neobiotech Co., Seoul, Korea). In 9 adult rabbits, a Tissue balloon ® was placed under the lateral mandibular periosteum via an extraoral approach. After 2, 3, and 4 weeks (n=3), the expanders were removed, and soft tissue and bone samples were collected for analysis. Histomorphometric analysis and measurements of expander characteristics were performed. In 5 Beagles, all premolars were extracted. Three months after extraction, tissue expanders were placed in the maxilla and mandible, adjacent to dental extraction sites, and removed after 3 weeks. Gingival parameters were measured before and after expansion. Blood perfusion in the oral mucosa was assessed using a laser Doppler flowmeter at different time points. After three weeks, dogs were euthanized, and tissue samples were collected for histologic analysis, focusing on inflammatory response, bone formation, and gingival thickness changes. In the rabbit study, measurements of size, weight, and volume showed significant increases over 4 weeks. Swelling peaked at 2 weeks postimplantation and then gradually stabilized. Histologic examination revealed no signs of inflammation, and expanders were covered by collagen-rich capsules. Some bone resorption was noted due to pressure from the expanders, but the resorption was not significant. In the beagle study, twenty tissue expanders were implanted; 1 expander was lost, and 9 wounds dehisced during soft tissue healing. However, no signs of infection were noted. Histologic evaluation showed bone atrophy, attributed to swelling pressure, at the recipient site. Gingival thickness was not decreased. Perfusion measurements indicated that expansion did not affect microvascular circulation adversely. Overall, both studies suggest that tissue expanders demonstrate biocompatibility and successful expansion volume in vivo, with minimal adverse effects on surrounding tissues and microvascular circulation.

Abstract:The structural and circulatory dynamics of expanded skin in connection with expander fillings were assessed in pigs following 7, 14, and 21 days of expansion by measuring the pressure of te fluid in the expander (Pexp) and the dermal blood flow. The results differed consistently on thoracic and abdominal sites. It was suggested that the stimulus to expansion be defined as the relative increase in extensible surface area of the expander. When plotting this value against Pexp, as measured at the end of injection, there was a good fit to an exponential curve. The area/Pexp relation was used to assess the connective tissue reaction around the expander; the reaction was most pronounced at 21 days. The decrease in Pexp after injection of fluid to 100 mm Hg showed that the tissue adapted rapidly during the first hour. Blood flow in the skin covering the expander was lower than on adjacent normal skin, and there was no increase during the three weeks of expansion. The decreases in blood flow were moderate even after injection of fluid until Pexp reached 100 mm Hg. It was suggested that the connective tissue capsule might protect the dermis from flow decrease.

Abstract:Background: Biological cover over tissue expander prostheses has been introduced to provide soft-tissue support for tissue expanders during breast reconstruction. However, its impact on mechanically induced skin growth remains unknown. This study investigates the hypothesis that covering the tissue expander with acellular dermal matrix (ADM) affects mechanotransduction without compromising the efficacy of tissue expansion. Methods: Tissue expansion, with and without use of ADM, was performed on a porcine model. The tissue expanders were inflated twice with 45 mL of saline, and the full-thickness skin biopsy specimens were harvested from expanded and control unexpanded skin 1 week and 8 weeks after the final inflation. Histologic evaluation, immunohistochemistry staining, and gene expression analysis were performed. Skin growth and total deformation were evaluated using isogeometric analysis. Results: The authors’ results demonstrate that use of ADM as a biological cover during tissue expansion does not impede mechanotransduction that leads to skin growth and blood vessel formation. Isogeometric analysis revealed similar total deformation and growth of expanded skin with and without a biological cover, confirming that its use does not inhibit mechanically induced skin growth. In addition, the authors found that use of an ADM cover results in more uniform distribution of mechanical forces applied by the tissue expander. Conclusions: These results suggest that ADM improves mechanically induced skin growth during tissue expansion by facilitating a more uniform distribution of mechanical forces applied by the tissue expander. Therefore, the use of a biological cover has potential to improve outcomes in tissue expansion–based reconstruction.

Abstract:Ten-fold serial dilutions of the H52 vaccine strain of infectious bronchitis virus were inoculated intravenously into 3-week-old and 10-week-old chickens. The severity of the histological lesions in the kidney was graded and described. The younger birds were much more resistant to the nephritogenic effects of the virus. Although very small doses of virus caused nephritis, no marked dose-related response was demonstrated in either age group.

Abstract:Background: During skin expansion, subcutaneous adipose tissue undergoes the greatest change. The adipose layer appears to gradually thin or even disappear in long-term expansion. The response and contribution of adipose tissue to skin expansion remain to be elucidated. Methods: The authors established a novel expansion model by transplanting luciferase-transgenic adipose tissue into the rat dorsum, followed by integrated expansion, to trace the dynamic changes in subcutaneous adipose tissue during expansion and the migration of adipose tissue-derived cells. In vivo luminescent imaging was performed to continuously track the adipose tissue changes. Histologic analysis and immunohistochemical staining evaluated the regeneration and vascularization of the expanded skin. Growth factor expression in expanded skin with or without adipose tissue was determined to evaluate the paracrine effect of adipose tissue. Adipose tissue-derived cells were traced in vitro by anti-luciferase staining, and their fate was determined by costaining for PDGFRα, DLK1, and CD31. Results: In vivo bioimaging showed that cells in adipose tissue were alive during expansion. After expansion, the adipose tissue exhibited fibrotic-like structures, with more DLK1 + preadipocytes. Skin expanded with adipose tissue was significantly thicker than that without adipose tissue, with more blood vessels and cell proliferation. Vascular endothelial growth factor, epidermal growth factor, and basic fibroblast growth factor expression was higher in adipose tissue than in skin, indicating paracrine support from adipose tissue. Luciferase-positive adipose tissue-derived cells were observed in expanded skin, indicating direct participation in skin regeneration. Conclusion: Adipose tissue transplantation can effectively promote long-term skin expansion by contributing to vascularization and cell proliferation by means of various mechanisms. Clinical relevance statement: The authors' findings suggest that it would be better if the expander pocket is dissected over the superficial fascia to preserve a layer of adipose tissue with skin. In addition, their findings support the treatment of fat grafting when expanded skin presents with thinning.

Abstract:Pyrene is a common organic luminescent material. To improve the fluorescence properties of pyrene, we have designed a pyrene-2,7-diyl bridged macrocage in which the pyrene moiety is sterically protected by the outside alkyl chains. The macrocage shows intense fluorescence from a monomeric excited state without excimer fluorescence even in saturated solutions, although the parent pyrene shows excimer fluorescence in highly concentrated solutions. These results indicate that the steric shielding by the cage prevents the formation of the excimer. Intensities of florescence in the presence of nitrobenzene were investigated to clarify the cage effects on fluorescence quenching. Lower efficiency of the fluorescence quenching caused by intermolecular collision between the caged pyrene (fluorophore) and nitrobenzene (quencher) was revealed by the analysis of the bimolecular quenching constants kq.
The Effects of Negative Pressure by External Tissue Expansion Device on Epithelial Cell Proliferation, Neo-Vascularization and Hair Growth in a Porcine Model

Abstract:While pre-treating a fat transplant recipient site with negative pressure has shown promise for increasing the fat survival rate, the underlying mechanisms have not been investigated, partly due to challenges related to immobilization of vacuum domes on large animal subjects. The aim of this study was to examine the effect of negative pressure treatment by External Tissue Expansion Device (ETED) on fat grating recipient sites in a porcine model. The ETED was designed to provide negative pressure on the dorsum of swine. Pressure treatment (-70 mmHg) was applied for 1 or 3 hours every other day for 10 and 20 treatments. The treated areas (3.5 cm in diameter) were harvested and examined for histological changes, vessel density, cell proliferation (Ki67) and growth factor expression (FGF-1, VEGF and PDGB-bb). The application of the ETED increased epidermis thickness even after 1-hour treatments repeated 10 times. The results of Ki67 analysis suggested that the increasing thickness was due to cell proliferation in the epidermis. There was a more than two-fold increase in the vessel density, indicating that the ETED promotes vascularization. Unexpectedly, the treatment also increased the number of hair follicles. Negative pressure provided by the ETED increases the thickness of epidermis section of tissue, cell proliferation and vessel density. The porcine model provides a better representation of the effect of the ETED on skin tissue compared to small animal models and provides an environment for studying the mechanisms underlying the clinical benefits of negative pressure treatment.

Abstract:The skin circulation was examined in nine patients during filling of their breast expander prostheses. Despite the high expansion pressure, which was increased above the systolic blood pressure, the overlying skin showed good circulation both clinically and when measured with a laser Doppler flowmeter and a transcutaneous oximeter. We conclude that the expansion was therefore directed to the tissues between the expander and the skin--that is, the fibrous capsule surrounding the prosthesis. Nine subcutaneous tissue expanders located in different anatomical regions were overfilled to a pressure at which all skin circulation ceased. Sufficient saline was then removed to allow skin circulation to return. Oxygenation of the skin ceased at a relatively low pressure when measured by transcutaneous oximetry, whereas when the skin circulation was measured with a laser Doppler flowmeter it did not disappear until the expander pressure had been raised appreciably above that level.

Abstract:Background: In the repair of massive tissue defects using expanded large skin flaps, the incidence of complications increases with the size of the expanded area. Currently, stem cell therapy has limitations to solve this problem. We hypothesized that conditioned medium of adipose-derived stem cells (ADSC-CM) collected following mechanical pretreatment can assist skin expansion. Methods: Rat aortic endothelial cells and fibroblasts were cultured with ADSC-CM collected under 0%, 10%, 12%, and 15% stretching force. Ten-milliliter cylindrical soft tissue expanders were subcutaneously implanted into the backs of 36 Sprague-Dawley rats. The 0% and 10% stretch groups were injected with ADSC-CM collected under 0% and 10% stretching force, respectively, while the control group was not injected. After 3, 7, 14, and 30 days of expansion, expanded skin tissue was harvested for staining and qPCR analyses. Results: Endothelial cells had the best lumen formation and highest migration rate, and fibroblasts secreted the most collagen upon culture with ADSC-CM collected under 10% stretching force. The skin expansion rate was significantly increased in the 10% stretch group. After 7 days of expansion, the number of blood vessels in the expanded area, expression of the angiogenesis-associated proteins vascular endothelial growth factor, basic fibroblast growth factor, and hepatocyte growth factor, and collagen deposition were significantly increased in the 10% stretch group. Conclusions: The optimal mechanical force upregulates specific paracrine proteins in ADSCs to increase angiogenesis and collagen secretion, and thereby promote skin regeneration and expansion. This study provides a new auxiliary method to expand large skin flaps.

Abstract:BACKGROUND: Conventional expansion inadequately restores damaged skin for patients with large areas of skin deficiency or who lack sources of normal skin. These patients require repeated skin expansions, but little is known about the outcomes of this procedure.OBJECTIVE: To evaluate the microscopic changes and biomechanical properties of skin and soft tissue after repeated expansion.MATERIALS AND METHODS: We prepared three groups of six pigs each: a conventional expansion group, a repeated expansion group, and a blank nonsurgical control group. We measured histology, ultrastructure, basic fibroblast growth factor (bFGF), stress-strain, stress relaxation, and stress strength.RESULTS: Skin obtained after conventional expansion and repeated expansion was basically healthy, but the microscopic and biomechanical properties differed from those of nonexpanded skin, especially in the repeated expansion group.CONCLUSION: Repeated skin expansion involves growth under stress, simultaneous injuries, and further repairs, with fibers showing more injury signs than cells. This article describes the microscopic changes and biomechanical properties that occur after repeated expansion.

Abstract:Various factors contribute to a predisposition towards the excessive consumption of alcohol: socio-demographic factors, a disturbed family background or certain neurological disorders in childhood, a family background of heavy drinking or total abstinence, or a genetic predisposition towards the biochemical reactions of excessive drinking. Alcohol dependence is the last of several stages which begin with drinking on "social occasions" during adolescence or even childhood, which may lead to certain people drinking excessively in certain social surroundings; this may easily become a habit if the subject moreover suffers from psychological problems inducing him to consume greater quantities more frequently to overcome inhibitions or to escape. This increased consumption leads in turn to psychological dependence, and toxicomania becomes apparent since if drinking is suddenly stopped, giving rise to neuro-vegetative disorders, these only disappear when further alcohol is consumed. This vicious circle is completed by psychological problems affecting personal relationships and professional activities. Thus for the alcoholic, for whom at the outset drinking was usually merely a social habit, the consumption of alcohol has become an escape from the reality of his situation, often drinking solely to avoid withdrawal symptoms, and therefore the appropriate therapy in these cases is detoxication.
Bone Marrow Mesenchymal Stem Cells Increase Skin Regeneration Efficiency in Skin and Soft Tissue Expansion

Abstract:BACKGROUND:Skin and soft tissue expansion has limitations such as long hospitalization time and flap retraction after expansion. Our previous study suggested that bone marrow-derived stem cells contribute skin regeneration in skin and soft tissue expansion. In this study, the authors explored the feasibility of applying the bone marrow mesenchymal stem cells (BMMSCs) to the treatment of skin and soft tissue expansion and increasing the skin regeneration efficiency.METHODS:Sixty silicone expanders were implanted in the backs of 15 pigs, and allogeneic BMMSCs were transplanted to skin shallow fascia layer (local transplantation, Group A) or via ear vein (systemic transplantation, Group B). Group C was the Sham operation control; and then the expanders were injected with normal saline (N.S.). Skin was obtained at different time points (days 0, 14, 21, 28, 35, and 42). The organizational structure changes of the target skin were observed in the expansion process. The distribution, differentiation, and paracrine function of labeled BMMSCs were detected.RESULTS:Comparing with Group B (25.00 ± 1.98 cm(2)) or Group C (24.00 ± 1.10 cm(2), no transplantation), the expanded skin area of Group A (28.82 ± 1.43 cm(2)) increased, with the morphology of epidermis thickened, and dermis thinned. The BMMSCs differentiated into vascular endothelial cells and dermal fibroblasts. The quantity of newborn cells was proportional to the number of transplanted cells. The gene expression of VEGF, bFGF, EGF, and SDF in Group A was higher than those in Group B or C. The most obvious changes were on day 35.CONCLUSIONS:The local transplanted BMMSCs could increase the skin regeneration efficiency in skin and soft tissue expansion and reduce skin shrinkage effectively after removing the expander. Growth factors, VEGF, bFGF, EGF, and SDF, are favorable to this process.
Changes of biomechanical properties and collages of expansive skin in different maintenance and expansion periods.

Abstract:Objective The study was to investigate a new method of rapid expansion by biomechanical methods. Methods Dogs were set up as animal models. Six areas, 5 cm×8 cm each, were symmetrically designed on a dog. Expanders of 240 ml were implanted in each area. Four groups were divided: A. Injection of 2 weeks. B. Injection of 6 weeks. C. Sham operation group. D. Blank control group. The first two groups were subdivided into three groups respectively according to maintaining times: one week, two weeks and four weeks. The following indexes were measured on the second stage surgery: biomechanical properties of the skin, stress strength, stress strain, stress relaxation and creep, the collagen content and alignment of skin. Results After expansion, maximal stress strength fell off, and stress strain curve moved backward. Relaxation curve fell off as well, as creep curve rose up. With elongation of the maintaining period, viscoelasticity of the skin gradually approximated normal skin, and had less relation with injection time. With the maintaining period increased, the collage content increased distinctly, and the alignment became much order. Conclusion Two week maintaining period is very important for expanded skin to recover the biomechanical properties and the content of collagen and alignment.

Abstract:Background: Tissue expansion is a powerful tool for reconstruction of pediatric soft-tissue pathologies, but complication rates for children have been reported as high as 40%. Infection and implant extrusion lead to premature removal and delays in reconstruction. Expanding the head and neck is uniquely challenging because the confluence of facial aesthetic units must be respected. These challenges have prompted the senior author’s creation of an aesthetic-unit based algorithm. Methods: A retrospective study of pediatric patients who underwent cervicofacial tissue expander placement by the senior author (R.J.R) was performed over a 17-year period. Predictor variables included age, sex, race, indication, number of expanders placed at each operation, serial expansion, expander type, expander size, home versus clinic inflation, and prophylactic antibiotics. Univariate and multivariate analyses were performed to identify risk factors for complications. Results: An aesthetic-unit based reconstructive algorithm is proposed. Forty-eight pediatric patients had 111 cervicofacial tissue expanders placed. Twenty expanders were associated with complications (18%) for surgical site-infection (12.6%), extrusion (4.5%), and expander deflation (6.3%). Expanders placed for congenital nevi (p=0.042) and use of textured expander (p=0.027) were significantly associated with decreased complications. When controlling for covariates, serial expansion of the same site was associated with increased rates of readmission (p=0.027) after having just one prior expander. Iatrogenic ectropion occurred in 13.5% of the study population; expanders with at least one complication during tissue expansion were significantly associated with incidence of iatrogenic ectropion (p=0.026). Conclusion: By using an aesthetic-unit based algorithm, reconstructive outcomes can be optimized for pediatric cervicofacial tissue expansion.

Abstract:This study investigated the changes in biorheological characteristics of skin after expansion. Expanders were implanted into the back of eight adult dogs, and after an eight-week expansion the expanded and the non-expanded control skins were transferred to an adjacent site. The expanded specimens and their controls were obtained at 3, 6, 12 and 24 weeks after transfer, and their biorheological characteristics and histological changes were studied. The characteristics of the stress-relaxation, the stress-strain relationship and tensile strength of the expanded and control specimens were measured. The tests demonstrated that during the initial stage after transfer, the biorheological properties of experimental specimens differed significantly from those of their controls. However, the differences between expanded and control specimens began to lessen gradually with increasing recovery time. When the recovery time reached 24 weeks or longer, the experimental skin exhibited the same mechanical properties as the control skin. Histological examination of expanded specimens showed thickened epidermis and thinned dermis. But there were no significant histological differences between expanded skins and their controls after 24 weeks.

Abstract:Background: Tissue expansion for treating giant congenital melanocytic nevi (GCMN) is a commonly employed surgical method. However, the procedure’s efficacy is often hindered by anatomical and histological characteristics as well as blood supply, particularly in the extremities and trunk. Enhancing expansion efficiency while reducing complications is thus a topic to be investigated, especially for pediatric patients undergoing rapid physical and psychological development with higher risks of non-compliance to medical instructions. Object: To explore the effectiveness of expansion in extremities and trunk by immobilizing the acellular dermal matrix (ADM) in the gravitational force zone of inflating expanders. Methods: All patients involved in this research underwent ADM-assisted tissue expansion in either the extremities or trunk. ADM was fully flattened, securely fixed to the lower pole of the expander, and subsequently attached to the inner surface of the expanding flap. Results: From 2021 to 2023, a total of nine pediatric patients with GCMN underwent the ADM-assisted tissue expansion. All patients achieved the desired expanding volume without experiencing petechiae, ecchymosis, or skin ulceration in the ADM-covered area. The process was well tolerated by all patients, with no reports of itching, pain, allergic reaction, or fever. During the flap transfer, the ADM was observed to be firmly adhered to the expanding flap with discernible capillary network. Conclusion: ADM-assisted tissue expansion demonstrates promise in augmenting expansion efficiency and reducing the time needed for surgical intervention in the extremities and trunk, thereby presenting significant clinical value for pediatric patients afflicted with GCMN.

Abstract:Microstructures of the early external callus after diaphyseal fractures of human long bone were investigated by using scanning electron microscopy, X-ray diffraction, and transmission electron microscopy. It was found that the main structural framework of the human early callus consists of disordered, mineralized collagen fibrils with a small fraction of regions of ordered collagen fibrils. X-ray diffraction analyses show that hydroxyapatite containing some carbonate impurity has been the dominant crystalline phase in the human early callus. In addition, a small amount of brushite phase was detected. Selected area diffraction analyses indicated that hydroxyapatite microcrystals were embedded in microfibrils with a diameter of 4.5 nm and well-banded fibrils, whereas brushite particles of 15-20 nm in an irregular shape were located in the noncollagenous organic matter around the nonmineralized, ordered collagen fibrils. The spatial distribution of the brushite particles in the human early callus was for the first time determined. The brushite particles probably serve as the reservoir of calcium and phosphate ions for subsequent mineralized periods rather than the precursor of hydroxyapatite.
