Abstract:Objectives: This review presents a comprehensive overview of the rapidly evolving digital healthcare industry, aiming to provide a broad understanding of the recent landscape and directions for the future of digital healthcare. Methods: This review examines the key trends in sectors of the digital healthcare industry, which can be divided into four main categories: digital hardware, software solutions, platforms, and enablers. We discuss electroceuticals, wearables, standalone medical software, non-medical health management services, telehealth, decentralized clinical trials, and infrastructural systems such as health data systems. The review covers both global and domestic perspectives, addressing definitions, significance, revenue trends, major companies, regulations, and socioenvironmental factors. Results: Diverse growth patterns are evident across digital healthcare sectors. The applications of electroceuticals are expanding. Wearables are becoming more ubiquitous, facilitating continuous health monitoring and data collection. Artificial intelligence in standalone medical software is demonstrating clinical efficacy, with regulatory frameworks adapting to support commercialization. Non-medical health management services are expanding their scope to address chronic conditions under professional guidance. Telemedicine and decentralized clinical trials are gaining traction, driven by the need for flexible healthcare solutions post-pandemic. Abstract:Background: Digital therapeutics, evidence-based software applications, offer preventive measures for various diseases and enhance healthcare accessibility and change. This article explores the current status and future trajectory of these devices.Current Concepts: Digital therapeutics prioritize a patient-centered approach. Historically, healthcare relied heavily on prescribed treatments during hospital visits. However, these devices provide objective data on patient symptoms through real-time digital monitoring. They also foster advancements in biotechnology and information technology, leading to improved patient care through technologies like artificial intelligence and virtual reality apps. Rigorous clinical trials are crucial for these devices, and integrating digital technology is proving increasingly efficient, particularly for decentralized clinical trials. Despite initial promise, challenges abound. While an international leader in this sector gained U.S. Food and Drug Administration approval, it recently filed for bankruptcy. Similarly, companies employing gaming techniques for attention deficit hyperactivity disorder treatment experienced lackluster performance. In Korea, the domestic digital therapeutics ecosystem stagnates with limited new approvals. High dropout rates and low user engagement remain major hurdles. Addressing these issues requires a design focused on user experience and motivation.Discussion and Conclusion: Digital therapeutic devices, coupled with digital monitoring, are poised to revolutionize healthcare into a patient-centered ecosystem. Establishing this ecosystem demands collaboration among patients, medical professionals, developers, and regulatory agencies. Continuous regulatory innovation, such as shortening the medical device evaluation process from 390 days to 80 days, is vital for progress.
Status of MyHealthWay and Suggestions for Widespread Implementation, Emphasizing the Utilization and Practical Use of Personal Medical Data

Abstract:Objectives: In the Fourth Industrial Revolution, there is a focus on managing diverse medical data to improve healthcare and prevent disease. The challenges include tracking detailed medical records across multiple institutions and the necessity of linking domestic public medical entities for efficient data sharing. This study explores MyHealthWay, a Korean healthcare platform designed to facilitate the integration and transfer of medical data from various sources, examining its development, importance, and legal implications. Methods: To evaluate the management status and utilization of MyHealthWay, we analyzed data types, security, legal issues, domestic versus international issues, and infrastructure. Additionally, we discussed challenges such as resource and infrastructure constraints, regulatory hurdles, and future considerations for data management. Results: The secure sharing of medical information via MyHealthWay can reduce the distance between patients and healthcare facilities, fostering personalized care and self-management of health. However, this approach faces legal challenges, particularly relating to data standardization and access to personal health information. Legal challenges in data standardization and access, particularly for secondary uses such as research, necessitate improved regulations. There is a crucial need for detailed governmental guidelines and clear data ownership standards at institutional levels. Conclusions: This report highlights the role of Korea's MyHealthWay, which was launched in 2023, in transforming healthcare through systematic data integration. Challenges include data privacy and legal complexities, and there is a need for data standardization and individual empowerment in health data management within a systematic medical big data framework.

Abstract:With the development of the digital healthcare sector, the software itself has now become a medical device. Digital Therapeutics(DTx) are defined as “providing evidence-based treatment interventions driven by high-quality software programs to prevent, manage, or treat medical disorders or diseases.” With the spread of digital therapeutics(DTx), major countries are expanding government-level support and investment, and overhauling related laws. This study examined the current status of domestic and foreign digital therapeutics and identified related laws and regulatory trends. Based on this, domestic legal and institutional improvement measures were proposed. According to the results of the study, major countries, including Korea, defined medical devices based on the Medical Device Act and regulated medical devices by classifying them according to the degree of risk. In the case of digital therapeutics, they are classified into low-risk grades and stipulate processes such as licensing and test evaluation of devices. In addition, a separate fast track was applied or the digital therapeutics was supported with individual guidelines. However, for the safe development of digital therapeutics in the future, it is necessary to systematically reorganize the regulations of digital therapeutics by reflecting the core contents of the guidelines in the law or enacting separate laws. In addition, the compensation method and level for the application of the health insurance system should be improved. In other words, doctor fees and patient compensation methods should be considered by reflecting the characteristics of digital therapeutics. Various discussions are needed in consideration of health insurance finance issues. Finally, due to the nature of digital therapeutics, it is necessary to strengthen data security measures and establish a management system for this.

Abstract:Background: As the need for digital health care based on mobile devices is increasing, with the rapid development of digital technologies, especially in the face of the COVID-19 pandemic, gaining a better understanding of the industrial structure is needed to activate the use of digital health care. Objective: The aim of this study was to suggest measures to revitalize the digital health care industry by deriving the stakeholders and major issues with respect to the ecosystem of the industry. Methods: A total of 1822 newspaper articles were collected using Big Kings, a big data system for news, for a limited period from 2016 to August 2021, when the mobile health care project was promoted in Korea centered on public health centers. The R and NetMiner programs were used for network analysis. Results: The Korean government and the Ministry of Health and Welfare showed the highest centrality and appeared as major stakeholders, and their common major issues were "reviewing the introduction of telemedicine," "concerns about bankruptcy of local clinics," and "building an integrated platform for precision medicine." In addition, the major stakeholders of medical institutions and companies were Seoul National University Hospital, Kangbuk Samsung Hospital, Ajou University Hospital, Samsung, and Vuno Inc. Conclusions: This analysis confirmed that the issues related to digital health care are largely composed of telemedicine, data, and health care business. For digital health care to develop as a national innovative growth engine and to be institutionalized, the development of a digital health care fee model that can improve the regulatory system and the cost-effectiveness of patient care, centering on the Ministry of Health and Welfare as a key stakeholder, is essential.

Genetics of scoliosis in chickens.

Abstract:The genetics of an inherited form of scoliosis in chickens was studied to estimate the number of genes involved, whether they are autosomal or sex-linked, their degree of dominance and penetrance, and the heritability of this trait in this population. Expression of scoliosis and in the progeny was analyzed by radiographs of birds 12 weeks of age or older. Crosses between an inbred line selected for scoliosis expression (incidence of scoliosis - 89 percent) and a highly inbred line displaying normal spinal development provided data for genetic analyses. The incidence of expression of scoliotic parent line implicates three major autosomal, recessive genes. The significantly higher incidence of severe scoliosis found in the homogametic male sex is ascribed to a sex-influenced, on the scoliosis trait rather than to sex-linkage. Variation of expression observed in the scoliotic line is attributed to incomplete penetrance of the major genes, additive effects of minor modifying genes, and primarily to environmental effects. Because of the similarities in the expression of this disease in chickens and humans, the inheritance pattern determined for chickens may provide useful insights into that operating for so-called adolescent idiopathic scoliosis in humans.

Abstract:Objectives: The need for interoperability at the national level was highlighted in Korea, leading to a consensus on the importance of establishing national standards that align with international technological standards and reflect contemporary needs. This article aims to share insights into the background of the recent national health data standardization policy, the activities of the Health Data Standardization Taskforce, and the future direction of health data standardization in Korea.Methods: To ensure health data interoperability, the Health Data Standardization Taskforce was jointly organized by the public and private sectors in December 2022. The taskforce operated three working groups. It reviewed international trends in interoperability standardization, assessed the current status of health data standardization, discussed its vision, mission, and strategies, engaged in short-term standardization activities, and established a governance system for standardization.Results: On September 15, 2023, the notice of “Health Data Terminology and Transmission Standards” in Korea was thoroughly revised to improve the exchange of health information between information systems and ensure interoperability. This notice includes the Korea Core Data for Interoperability (KR CDI) and the Korea Core Data Transmission Standard (HL7 FHIR KR Core), which are outcomes of the taskforce’s efforts. Additionally, to reinforce the standardized governance system, the Health-Data Standardization Promotion Committee was established.Conclusions: Active interest and support from medical informatics experts are needed for the development and widespread adoption of health data standards in Korea.

Abstract:Digital health interventions refer to the use of digital technology and connected devices to improve health outcomes and healthcare delivery. This includes telemedicine, electronic health records, wearable devices, mobile health applications, and other forms of digital health technology. To this end, several research and developmental activities in various fields are gaining momentum. For instance, in the medical devices sector, several smart biomedical materials and medical devices that are digitally enabled are rapidly being developed and introduced into clinical settings. In the pharma and allied sectors, digital health-focused technologies are widely being used through various stages of drug development, viz. computer-aided drug design, computational modeling for predictive toxicology, and big data analytics for clinical trial management. In the biotechnology and bioengineering fields, investigations are rapidly growing focus on digital health, such as omics biology, synthetic biology, systems biology, big data and personalized medicine. Though digital health-focused innovations are expanding the horizons of health in diverse ways, here the development in the fields of medical devices, pharmaceutical technologies and biotech sectors, with emphasis on trends, opportunities and challenges are reviewed. Preparedness, Challenges, and Opportunities for Digital Intervention for Chronic Disease Management: A Qualitative Study in Rural Areas of South Korea

Abstract:Motivated by the prevalence of an aging population and the associated increase in chronic diseases, coupled with rising medical expenditure, the Korean government initiated a pilot project in Pyeongchang-gun, Gangwon-do, a rural area, to implement a "smart online-to-offline (O2O) digital health care model" aimed at managing and preventing chronic diseases. However, there is limited understanding regarding perspectives and levels of preparedness for digital health among stakeholders at various levels. In-depth focus group interviews were conducted with elderly and non-elderly community members, health care providers, and staff members at Pyeongchang Health and Medical Center. The study found the presence of both positive and negative perceptions and a lack of preparedness across different levels. At the end-user level, it was observed that community members, especially the elderly, have low levels of health and digital literacy, compounded by limited access to social support. At the health care provider level, there was uncertainty about the acceptance of the digital health program. At the area level, the need to bolster health staff members and enhance their capacity was observed. Recommendations include: customizing the design of the online and offline service components by considering end-user factors (such as age, occupation, and household type) that may contribute to disparities in health; establishing a platform for providers to share their experiences to facilitate the effective incorporation of digital health into their practices; and preparing an appropriate provider payment mechanism.
A Review of Current Digital Mental Health Care Applications for Anxiety Symptoms and Future Prospects

Abstract:Objective: Since the impact of the coronavirus disease-2019 pandemic, the need for efficiency in medical services has become more urgent than ever. The digital treatment market is rapidly growing worldwide and digital therapeutics (DTx), a major part of the digital medical services, is also emerging as a new paradigm for treatment, with its industry growing rapidly as well. Increasing research is done on the effectiveness of mobile DTx in improving mental health conditions such as insomnia, panic, and depression. Methods: This review paper investigates 1) the functions and characteristics of mobile digital mental health care applications for the treatment of anxiety symptoms, 2) extracts common attributes of the applications, and 3) compares them with existing traditional treatment mechanisms. Results: Among the 20,000 mental health management applications that have been developed so far, 8 applications that are relatively widely used were selected and reviewed. Check-in, self-help tips, quick relief, journal, courses for practice are common features of the digital mental health care applications for anxiety and are also widely used feature in the cognitive behavioral therapy. Conclusion: Based on this review, we have proposed the essential elements and directions for the development of a Korean digital mental health care applications for anxiety disorders.

Abstract:With rapid developments in genomic and digital technologies, genomic data sharing has become a key issue for the achievement of precision medicine in South Korea. The legal and administrative framework for data sharing and protection in this country is currently under intense scrutiny from national and international stakeholders. Policymakers are assessing the relevance of specific restrictions in national laws and guidelines for better alignment with international approaches. This manuscript will consider key issues in international genome data sharing in South Korea, including consent, privacy, security measures, compatible adequacy and oversight, and map out an approach to genomic data sharing that recognizes the importance of patient engagement and responsible use of data in South Korea.

Abstract:Background/Objectives: Globally, life expectancy has been increasing with South Korea focusing on improving health to enhance quality of life. The COVID-19 pandemic further emphasized the need for digital transformation in healthcare, accelerating digital health adoption. This study explores the digital divide between 'Digital Natives (20–39 Y)' and 'Digital Immigrants (40–69 Y)', focusing on digital device usage and confidence. Methods: This study utilized national survey data from the Digital Health Literacy Survey Results and Policy Implications, focusing on differences in digital device use and confidence between young adults (20–39 Y) and middle-aged adults (40–69 Y). The participants comprised 1000 adults aged 20 to 69 in the Republic of Korea. Respondents were queried about their use of digital health tools, such as wearable devices and mobile apps. Confidence in using digital systems and managing health via digital tools was assessed using a five-point Likert scale. Results: The findings indicated that while young adults have lower rates of using digital devices for healthcare, they exhibit higher confidence in using such devices. In contrast, middle-aged adults, despite having lower confidence, report higher usage of digital devices for healthcare purposes. Conclusions: This study explored differences in digital confidence and healthcare usage between age groups and aimed to propose effective health management strategies based on digital accessibility.

Abstract:OBJECTIVES: The purpose of this study was to review the current telemedicine of the Korea and the telemedicine of the other countries.METHODS: This study reviewed several documents on telemedicine and summarized the documents on the initiation of the telemedicine of the Korea, the recent regulations of the government, the analytical research results, and the telemedicine of foreign countries.RESULTS: One of recent demonstration trials of telemedicine began in July 2015 in the Korea. The plan was to conduct an emergency telemedical treatment trial among the cooperative medical service centers until the end of February 2016. No telemedical services were provided at the level of local primary care clinics, and there was 1.2% provision at the hospital level. The Europe Union and the United States had more active telemedical services in comparison to the Korea.CONCLUSIONS: The introduction and usage of telemedicine in the Korea was behind those of other countries. It is necessary to develop a proactive support policy for telemedicine through a government implemented trial.

Abstract:Digital health technology is utilized in contemporary nursing practice and education. This review explored the scope of digital health applications and major trends in nursing research involving digital health in Korea using topic modeling. Our analysis of data using the Latent Dirichlet Allocation model identified four distinct research topics: nursing education using digital technologies (35.17%), hospital-based nursing practice using digital technologies (19.88%), digital technologies for health education (25.75%), and development of digital technologies to support self-management of chronic conditions (19.20%). Our findings reveal trends, current issues, and gaps in digital health nursing research.

Abstract:Digital health encompasses a wide range of novel digital technologies related to health and medicine. Such technologies rely on recent advances in the collection and analysis of ever increasing amounts of data from both patients and healthy citizens. Along with new opportunities, however, come new ethical and policy challenges. These range from the need to adapt current evidence-based standards, to issues of privacy, oversight, accountability and public trust as well as national and international data governance and management. This review illustrates key issues and challenges facing the rapidly unfolding digital health paradigm and reflects on the impact of big data in medical research and clinical practice both internationally and in Switzerland. It concludes by emphasising five conditions that will be crucial to fulfil in order to foster innovation and fair benefit sharing in digital health.

Current Status of Electronic Medical Record Systems in Hospitals and Clinics in Korea

Abstract:OBJECTIVES: Many healthcare organizations and professionals have had interests in healthcare information and communication technology (ICT). The objective of this study was to investigate the current status of overall healthcare ICT, especially focusing on Electronic Medical Record (EMR) systems in Korea.METHODS: This study used a part of the nationwide survey collected for the OECD benchmarking ICT study. The Health Insurance Review and Assessment Service conducted the survey from November 19, 2013 to January 10, 2014. This study followed the methodological guidelines of the OECD. A total of 2,093 hospitals and clinics, including long-term care hospitals, participated in the survey. Among them, 554 hospitals and 906 clinics were included in this study for the generalization of the results.RESULTS: The adoption rates of EMR systems were 96.3% in hospitals and 95.7% in clinics. Most of the hospitals and clinics had high rates of healthcare information exchange (HIE) within the organization; however, there were extremely low HIE rates among external organizations. Most of the hospitals and clinics had EMR systems with clinical-decision-supporting functionalities. Ninety-six percent of the EMR systems of the hospitals and 89.2% of the clinic systems had checking functions, such as alerts or reminders, on contraindications of drug-drug and drug-age interaction.CONCLUSIONS: Korea has maintained a high healthcare ICT status compared to countries in the European Union. The EMR systems of hospitals and clinics in Korea had sophisticated functionalities; however, their HIE status was extremely low, which indicates the need for healthcare ICT standardization.

Abstract:Objectives Education in biomedical and health informatics is essential for managing complex healthcare systems, bridging the gap between healthcare and information technology, and adapting to the digital requirements of the healthcare industry. This review presents the current status of biomedical and health informatics education domestically and internationally and proposes recommendations for future development. Methods We analyzed evidence from reports and papers to explore global trends and international and domestic examples of education. The challenges and future strategies in Korea were also discussed based on the experts’ opinions. Results This review presents international recommendations for establishing education in biomedical and health informatics, as well as global examples at the undergraduate and graduate levels in medical and nursing education. It provides a thorough examination of the best practices, strategies, and competencies in informatics education. The review also assesses the current state of medical informatics and nursing informatics education in Korea. We highlight the challenges faced by academic institutions and conclude with a call to action for educators to enhance the preparation of professionals to effectively utilize technology in any healthcare setting. Conclusions To adapt to the digitalization of healthcare, systematic and continuous workforce development is essential. Future education should prioritize curriculum innovations and the establishment of integrated education programs, focusing not only on students but also on educators and all healthcare personnel in the field. Addressing these challenges requires collaboration among educational institutions, academic societies, government agencies, and international bodies dedicated to systematic and continuous workforce development.

Abstract:Lecture As most people expect, the COVID-19 pandemic will change our life radically. One of the directions is expected to minimize off-line contacts in daily lives and healthcare is surely not the exception. Big hospitals, which are centralized systems evolved to deliver healthcare most effectively, can become the media of the spread of diseases in some cases and the adoption of a decentralized healthcare system will be a helpful strategy to prevent it. This strategy can be implemented through a smart healthcare system, and it will be one more necessity of a smart healthcare system in our society. Smart healthcare, which will be delivered mainly through online contact, is a very comprehensive concept enabling otherwise not possible healthcare such as health monitoring using life-log data from the wearable device and far beyond the online consultation with a doctor. Although there will be many social, ethical, and technical challenges for smart healthcare, the discussion today will be limited to two challenges, interoperability and protecting the data subjects' rights. Smart healthcare needs to collect and process comprehensive health information from a vast number of data subjects during its development and delivery and the data subjects' rights are highly vulnerable during these processes. Collecting and exchanging health information from various sources such as hospitals, clinics, smartwatches, and so on, inevitably raises the issue of interoperability of the data from different sources. The implementation of smart healthcare requires technical solutions to both challenges. General Data Protection Regulation, recently issued by the EU, deals the data subjects' rights very seriously, such as the right to be informed, the right of access, the right to rectification, the right to erasure, the right to restrict processing, the right to data portability, the right to object, and the rights in relation to automated decision making and profiling. Technical innovations required to protect these data subjects' rights will be discussed here. Compared to traditional healthcare which mostly dealt with medical data generated within the healthcare facility itself, smart healthcare requires health data beyond the healthcare facilities. Not to mention the life-log data from different wearable devices, even the medical data from different hospitals are not interoperable. Although many health information standards have been proposed, such as Systematized Nomenclature of Medicine Clinical Terms (SMOMED CT), Logical Observation Identifier Name Code (LOINC), Health Level 7 Fast Healthcare Interoperability Resources (HL7 FHIR), and so on, the adoption of these standards in hospital information system in Korea has been very slow. This low adoption of health information standards in our hospital information system obstructs the so-called, meaningful use of electronic medical records. The necessity of adopting health information standards will be refreshed today. Smart healthcare requires the active participation of health consumers and active participation of health consumers requires their health literacy. Health literacy of health consumers will be fostered by innovative technical aids such as data visualization, the decision support system for consumers, and more. Health literacy will the most powerful driving force of smart healthcare implementation.
