Abstract:As a response to the severe air quality problems in China, the Chinese Ministry of Environmental Protection in 2012 issued an updated Ambient Air Quality Standard (GB3095-2012), which set a stricter grade II PM10 standard. A successful implementation of this standard will have an impact on human health through reduction of exposure to air pollution in the population. Using the methods of adjusted human capital and value of statistical life, the authors in this article estimate the economic impact of a successful implementation of the updated Ambient Air Quality Standard on human health in China. The key results are: 1) The human health benefits from reaching the updated standard for PM10 would equal CNY 51.1 billion using adjusted human capital, accounting for 18.5% of total human health costs in China, and CNY 83.5 billion using the value of a statistical life; 2) the benefit of reaching the updated standard for PM10 is highest in the east coastal areas and gradually declines for more Western provinces; and 3) the marginal benefit of public health increases as the environmental quality standard PM10 improves. If the annual concentration of PM10 were to be reduced from the target number in the original grade II standard to that in the updated standard, the mortality rate of long-term exposure would be reduced by 6.5% due to reduced chronic exposure. In addition, if the annual concentration were to be reduced further from updated grade II to grade I standard, the mortality rate for long-term exposure would be lowered by 32.8%.

Abstract:INTRODUCTION Toremifene(TOR)is a selective estrogen receptor modulator(SERM). A high dose of 120 mg TOR(HD-TOR) has been used for recurrent breast cancer in Japan, but there is still insufficient evidence regarding the efficacy of HD-TOR. PATIENTS AND METHODS HD-TOR was administered for recurrent or metastatic breast cancer between January 2003 and May 2012. The primary end point of the study was the tumor response rate. Bone metastasis cases were excluded from the efficacy analysis, but were included in the safety population. RESULTS A total of 21 patients registered in the study and the 2 patients with bone metastasis only were excluded from the efficacy analysis. The median follow-up period was 8. 3 months. None of the patients in the study had a CR, 4 had a PR(21. 1%), 9 had SD(47. 4%), and 6 had PD(31. 6%). Eight of the 9 SD cases had a long-term SD. The ORR was 21. 1% and the CB rate was 63. 2%. The median TTP of CB cases was 18. 3 months. None of the patients discontinued treatment because of a grade 3 or grade 4 adverse effects. CONCLUSION In summary, the current study showed that HD-TOR may lead to a CB for recurrent breast cancer in first- or second-line treatment rather than thirdline. In particular, HD-TOR may give a benefit in highly endocrine-sensitive cases.

Abstract:Traffic management strategies not only affects public safety and convenience, but also can affect air quality significantly and thus may affect public health, as traffic is one of the most important air pollution sources in urban areas. In this study, based on the traffic conditions of 2013 in Dublin, Ireland, the impact of a change in transport infrastructure, a traffic regulation change, speed limit changes and fleet composition changes on air quality and air pollution related public health were assessed. Two pollutants are considered in this study: NO2 and PM2.5. A traffic model, emissions model, dispersion model and a health impact model were adopted. The construction of new infrastructure and changes in traffic management were predicted to have contrasting effects on different parts of the city, bringing little benefit to the city as a whole. The mortality incidence change brought about by changes in NO2 concentration due to reductions in speed limits were predicted to be an increase of 22.4 deaths in 2013. A fleet composition change from diesel to petrol vehicles was predicted to reduce the PM2.5 and NO2 concentration and relevant mortality incidence for the whole city. The study highlighted the importance of the consideration of all possible affected areas within a city. It also highlighted the balance of the safety issues and the environmental health impact, when assessing the impact of traffic management strategies.

Health benefits of decreases in on-road transportation emissions in the United States from 2008 to 2017

Abstract:Decades of air pollution regulation have yielded enormous benefits in the United States, but vehicle emissions remain a climate and public health issue. Studies have quantified the vehicle-related fine particulate matter (PM2.5)-attributable mortality but lack the combination of proper counterfactual scenarios, latest epidemiological evidence, and detailed spatial resolution; all needed to assess the benefits of recent emission reductions. We use this combination to assess PM2.5-attributable health benefits and also assess the climate benefits of on-road emission reductions between 2008 and 2017. We estimate total benefits of $270 (190 to 480) billion in 2017. Vehicle-related PM2.5-attributable deaths decreased from 27,700 in 2008 to 19,800 in 2017; however, had per-mile emission factors remained at 2008 levels, 48,200 deaths would have occurred in 2017. The 74% increase from 27,700 to 48,200 PM2.5-attributable deaths with the same emission factors is due to lower baseline PM2.5 concentrations (+26%), more vehicle miles and fleet composition changes (+22%), higher baseline mortality (+13%), and interactions among these (+12%). Climate benefits were small (3 to 19% of the total). The percent reductions in emissions and PM2.5-attributable deaths were similar despite an opportunity to achieve disproportionately large health benefits by reducing high-impact emissions of passenger light-duty vehicles in urban areas. Increasingly large vehicles and an aging population, increasing mortality, suggest large health benefits in urban areas require more stringent policies. Local policies can be effective because high-impact primary PM2.5 and NH3 emissions disperse little outside metropolitan areas. Complementary national-level policies for NOx are merited because of its substantial impacts-with little spatial variability-and dispersion across states and metropolitan areas.

Abstract:BackgroundOn-road vehicles are an important source of fine particulate matter (PM2.5) in cities, but spatially varying traffic emissions and vulnerable populations make it difficult to assess impacts to inform policy and the public.MethodsWe estimated PM2.5-attributable mortality and morbidity from on-road vehicle generated air pollution in the New York City (NYC) region using high-spatial-resolution emissions estimates, air quality modeling, and local health incidence data to evaluate variations in impacts by vehicle class, neighborhood, and area socioeconomic status. We developed multiple ‘zero-out’ emission scenarios focused on regional and local cars, trucks, and buses in the NYC region. We simulated PM2.5 concentrations using the Community Multi-scale Air Quality Model at a 1-km spatial resolution over NYC and combined modeled estimates with monitored data from 2010 to 2012. We applied health impact functions and local health data to quantify the PM2.5-attributable health burden on NYC residents within 42 city neighborhoods.ResultsWe estimate that all on-road mobile sources in the NYC region contribute to 320 (95 % Confidence Interval (CI): 220–420) deaths and 870 (95 % CI: 440–1280) hospitalizations and emergency department visits annually within NYC due to PM2.5 exposures, accounting for 5850 (95 % CI: 4020–7620) years of life lost. Trucks and buses within NYC accounted for the largest share of on-road mobile-attributable ambient PM2.5, contributing up to 14.9 % of annual average levels across 1-km grid cells, and were associated with 170 (95 % CI: 110–220) PM2.5-attributable deaths each year. These contributions were not evenly distributed, with high poverty neighborhoods experiencing a larger share of the exposure and health burden than low poverty neighborhoods.ConclusionReducing motor vehicle emissions, especially from trucks and buses, could produce significant health benefits and reduce disparities in impacts. Our high-spatial-resolution modeling approach could improve assessment of on-road vehicle health impacts in other cities.

Abstract:Higher concentrations of air pollutants, such as nitrogen dioxide (NO ) and particulate matter (PM), are observed in streets compared to the urban background. These concentrations could be predominantly attributed to road traffic, which remains an important source despite relentless efforts. In this study, performed over the Paris area, the impacts of the evolution of the fleet composition on urban air quality down to the street scale is assessed with two scenarios assuming the introduction of very-low-emission vehicles. Exhaust emission factors for these vehicles are based on the improvement of engine and after-treatment technologies, leading to factors lower than those proposed for the European emission standard Euro 7. Using the year 2014 as the baseline, very-low-emission vehicles are introduced up to the year 2030 for the Paris region. NO emissions are thus reduced by 68 % in streets, and concentrations by 53 %. However, PM concentration reduction is limited to 18 % in streets as non-exhaust emissions from tyre and brake wear and road abrasion are preponderant. PM emissions from non-exhaust sources represent 59 % of the total road-traffic emission of PM in 2014 and 89 % in 2030. Non-regulated pollutant concentrations are also reduced, by 42 % for black carbon and 30 % for organic matter. Considering only very-low-emission and electric vehicles in the fleet further reduce NO emissions and concentrations by 99.5 % and 80 % respectively. PM concentrations are only reduced by 22 %. This study highlights the high reduction potential of NO concentrations with very-low-emission and electric vehicles, because of reduction of exhaust emissions. However, difficulties remain to reduce PM concentrations in urban areas, with the large majority of PM traffic emissions coming from non-exhaust sources.

Impact of lowering fine particulate matter from major emission sources on mortality in Canada: A nationwide causal analysis

Abstract:Emissions of fine particulate matter (PM 2.5 ) from human activities have been linked to substantial disease burdens, but evidence regarding how reducing PM 2.5 at its sources would improve public health is sparse. We followed a population-based cohort of 2.7 million adults across Canada from 2007 through 2016. For each participant, we estimated annual mean concentrations of PM 2.5 and the fractional contributions to PM 2.5 from the five leading anthropogenic sources at their residential address using satellite observations in combination with a global atmospheric chemistry transport model. For each source, we estimated the causal effects of six hypothetical interventions on 10-y nonaccidental mortality risk using the parametric g-formula, a structural causal model. We conducted stratified analyses by age, sex, and income. This cohort would have experienced tangible health gains had contributions to PM 2.5 from any of the five sources been reduced. Compared with no intervention, a 10% annual reduction in PM 2.5 contributions from transportation and power generation, Canada's largest and fifth-largest anthropogenic sources, would have prevented approximately 175 (95%CI: 123–226) and 90 (95%CI: 63–117) deaths per million by 2016, respectively. A more intensive 50% reduction per year in PM 2.5 contributions from the two sources would have averted 360 and 185 deaths per million, respectively, by 2016. The potential health benefits were greater among men, older adults, and low-income earners. In Canada, where PM 2.5 levels are among the lowest worldwide, reducing PM 2.5 contributions from anthropogenic sources by as little as 10% annually would yield meaningful health gains.

The Impact of Individual Anthropogenic Emissions Sectors on the Global Burden of Human Mortality due to Ambient Air Pollution

Abstract:Background: Exposure to ozone and fine particulate matter (PM2.5) can cause adverse health effects, including premature mortality due to cardiopulmonary diseases and lung cancer. Recent studies quantify global air pollution mortality but not the contribution of different emissions sectors, or they focus on a specific sector. Objectives: We estimated the global mortality burden of anthropogenic ozone and PM2.5, and the impact of five emissions sectors, using a global chemical transport model at a finer horizontal resolution (0.67° × 0.5°) than previous studies. Methods: We performed simulations for 2005 using the Model for Ozone and Related Chemical Tracers, version 4 (MOZART-4), zeroing out all anthropogenic emissions and emissions from specific sectors (All Transportation, Land Transportation, Energy, Industry, and Residential and Commercial). We estimated premature mortality using a log-linear concentration-response function for ozone and an integrated exposure-response model for PM2.5. Results: We estimated 2.23 (95% CI: 1.04, 3.33) million deaths/year related to anthropogenic PM2.5, with the highest mortality in East Asia (48%). The Residential and Commercial sector had the greatest impact globally-675 (95% CI: 428, 899) thousand deaths/year-and in most regions. Land Transportation dominated in North America (32% of total anthropogenic PM2.5 mortality), and it had nearly the same impact (24%) as Residential and Commercial (27%) in Europe. Anthropogenic ozone was associated with 493 (95% CI: 122, 989) thousand deaths/year, with the Land Transportation sector having the greatest impact globally (16%). Conclusions: The contributions of emissions sectors to ambient air pollution-related mortality differ among regions, suggesting region-specific air pollution control strategies. Global sector-specific actions targeting Land Transportation (ozone) and Residential and Commercial (PM2.5) sectors would particularly benefit human health. Abstract:Background: Health effects linked to exposure to high air pollutant levels have been described in depth, and many recent epidemiologic studies have also consistently reported positive associations between exposure to air pollutants at low concentrations (particularly PM(2.5)) and adverse health outcomes. Objective: To estimate the number of avoidable deaths associated with reducing PM(2.5) levels in Spain. Materials and methods: For exposure assessment, we used the US Environmental Protection Agency's Community Multiscale Air Quality model to simulate air pollution levels with a spatial resolution of 18×18 km(2). Two different scenarios were compared, namely, a baseline 2004 scenario based on Spain's National Emissions Inventory and a projected 2011 scenario in which a reduction in PM(2.5) was estimated on the basis of the benefits that might be attained if specific air quality policies were implemented. Using an 18×18 km(2) grid, air pollution data were estimated for the entire Iberian Peninsula, the Balearic Islands, Ceuta and Melilla. For these strata, crude all-cause mortality rates (ICD-10: A00-Y98) were then calculated for the over-30 and 25-74 age groups, taking into account the 2004 population figures corresponding to these same age groups, selected in accordance with the concentration-response functions (Pope CA 3rd, Burnett RT, Thun MJ, Calle EE, Krewski D, Ito K et al. Lung cancer, cardiopulmonary mortality, and long-term exposure to fine particulate air pollution. JAMA 2002; 287:1132-41; Laden F, Schwartz J, Speizer FE, Dockery DW. Health impacts were assessed using the Environmental Benefits Mapping and Analysis Program (BenMAP). Results: Air quality improvement was defined as an average annual reduction of 0.7 μg/m(3) in PM(2.5) levels. Using long-term health impact assessment analysis, we estimated that 1720 (673-2760) all-cause deaths (6 per 100,000 population) in the over-30 age group and 1450 (780-2108) all-cause deaths (5 per 100,000 population) in the 25-74 age group could be prevented annually. Conclusions: The results showed the potential benefits in general mortality which could be expected if pollution control policies were successfully implemented by 2011. A specifically adapted BenMAP could be used as a tool for estimating health impacts associated with changes in air pollution in Spain.

Abstract:Background: Ambient air pollution, including particulate matter (such as PM10 and PM2·5) and nitrogen dioxide (NO2), has been linked to increases in mortality. Whether populations' vulnerability to these pollutants has changed over time is unclear, and studies on this topic do not include multicountry analysis. We evaluated whether changes in exposure to air pollutants were associated with changes in mortality effect estimates over time. Methods: We extracted cause-specific mortality and air pollution data collected between 1995 and 2016 from the Multi-Country Multi-City (MCC) Collaborative Research Network database. We applied a two-stage approach to analyse the short-term effects of NO2, PM10, and PM2·5 on cause-specific mortality using city-specific time series regression analyses and multilevel random-effects meta-analysis. We assessed changes over time using a longitudinal meta-regression with time as a linear fixed term and explored potential sources of heterogeneity and two-pollutant models. Findings: Over 21·6 million cardiovascular and 7·7 million respiratory deaths in 380 cities across 24 countries over the study period were included in the analysis. All three air pollutants showed decreasing concentrations over time. The pooled results suggested no significant temporal change in the effect estimates per unit exposure of PM10, PM2·5, or NO2 and mortality. However, the risk of cardiovascular mortality increased from 0·37% (95% CI -0·05 to 0·80) in 1998 to 0·85% (0·55 to 1·16) in 2012 with a 10 μg/m3 increase in PM2·5. Two-pollutant models generally showed similar results to single-pollutant models for PM fractions and indicated temporal differences for NO2. Interpretation: Although air pollution levels decreased during the study period, the effect sizes per unit increase in air pollution concentration have not changed. This observation might be due to the composition, toxicity, and sources of air pollution, as well as other factors, such as socioeconomic determinants or changes in population distribution and susceptibility. Abstract:The adverse health effects of short-term exposure to ambient air pollution are well documented. 1-3 Particulate matter (PM), especially, arouses public health concerns because of its toxicity and the widespread human exposure to this pollutant. PM, which includes inhalable particles with an aerodynamic diameter of 10 μm or less (PM 10 ) and fine particles with an aerodynamic diameter of 2.5 μm or less (PM 2.5 ), is emitted from combustion sources or formed through atmospheric chemical transformation. Given the extensive evidence regarding their effects of health, the daily and annual mean concentrations of PM 10 and PM 2.5 are regulated according to the World Health Organization (WHO) Air Quality Guidelines 4 and standards in major countries.

Abstract:Rationale: The associations between ambient coarse particulate matter (PM2.5-10) and daily mortality are not fully understood on a global scale. Objectives: To evaluate the short-term associations between PM2.5-10 and total, cardiovascular, and respiratory mortality across multiple countries/regions worldwide. Methods: We collected daily mortality (total, cardiovascular, and respiratory) and air pollution data from 205 cities in 20 countries/regions. Concentrations of PM2.5-10 were computed as the difference between inhalable and fine PM. A two-stage time-series analytic approach was applied, with overdispersed generalized linear models and multilevel meta-analysis. We fitted two-pollutant models to test the independent effect of PM2.5-10 from copollutants (fine PM, nitrogen dioxide, sulfur dioxide, ozone, and carbon monoxide). Exposure-response relationship curves were pooled, and regional analyses were conducted. Measurements and Main Results: A 10 μg/m3 increase in PM2.5-10 concentration on lag 0-1 day was associated with increments of 0.51% (95% confidence interval [CI], 0.18%-0.84%), 0.43% (95% CI, 0.15%-0.71%), and 0.41% (95% CI, 0.06%-0.77%) in total, cardiovascular, and respiratory mortality, respectively. The associations varied by country and region. These associations were robust to adjustment by all copollutants in two-pollutant models, especially for PM2.5. The exposure-response curves for total, cardiovascular, and respiratory mortality were positive, with steeper slopes at lower exposure ranges and without discernible thresholds. Conclusions: This study provides novel global evidence on the robust and independent associations between short-term exposure to ambient PM2.5-10 and total, cardiovascular, and respiratory mortality, suggesting the need to establish a unique guideline or regulatory limit for daily concentrations of PM2.5-10.
Twenty five year mortality and air pollution: results from the French PAARC survey

Abstract:Aims and methods: Long term effects of air pollution on mortality were studied in 14,284 adults who resided in 24 areas from seven French cities when enrolled in the PAARC survey (air pollution and chronic respiratory diseases) in 1974. Daily measurements of sulphur dioxide, total suspended particles, black smoke, nitrogen dioxide, and nitric oxide were made in 24 areas for three years (1974-76). Cox proportional hazards models controlling for individual confounders (smoking, educational level, body mass index, occupational exposure) were applied, and frailty models used to take into account spatial correlation. Indicators of air pollution were the mean concentration. Results: Models were run before and after exclusion of six area monitors influenced by local traffic (NO/NO2 >3 in ppb). After exclusion of these areas, analyses showed that adjusted risk ratios (95% CI) for TSP, BS, NO2, and NO for non-accidental mortality were 1.05 (1.02 to 1.08), 1.07 (1.03 to 1.10), 1.14 (1.03 to 1.25), and 1.11 (1.05 to 1.17) for 10 microg/m3 respectively. Consistent patterns for lung cancer and cardiopulmonary causes were observed. Conclusions: Urban air pollution assessed in the 1970s was associated with increased mortality over 25 years in France.

Abstract:Background: Model-estimated air pollution exposure products have been widely used in epidemiological studies to assess the health risks of particulate matter with diameters of ≤2.5 µm (PM2.5). However, few studies have assessed the disparities in health effects between model-estimated and station-observed PM2.5 exposures. Methods: We collected daily all-cause, respiratory and cardiovascular mortality data in 347 cities across 15 countries and regions worldwide based on the Multi-City Multi-Country collaborative research network. The station-observed PM2.5 data were obtained from official monitoring stations. The model-estimated global PM2.5 product was developed using a machine-learning approach. The associations between daily exposure to PM2.5 and mortality were evaluated using a two-stage analytical approach. Results: We included 15.8 million all-cause, 1.5 million respiratory and 4.5 million cardiovascular deaths from 2000 to 2018. Short-term exposure to PM2.5 was associated with a relative risk increase (RRI) of mortality from both station-observed and model-estimated exposures. Every 10-μg/m3 increase in the 2-day moving average PM2.5 was associated with overall RRIs of 0.67% (95% CI: 0.49 to 0.85), 0.68% (95% CI: -0.03 to 1.39) and 0.45% (95% CI: 0.08 to 0.82) for all-cause, respiratory, and cardiovascular mortality based on station-observed PM2.5 and RRIs of 0.87% (95% CI: 0.68 to 1.06), 0.81% (95% CI: 0.08 to 1.55) and 0.71% (95% CI: 0.32 to 1.09) based on model-estimated exposure, respectively. Conclusions: Mortality risks associated with daily PM2.5 exposure were consistent for both station-observed and model-estimated exposures, suggesting the reliability and potential applicability of the global PM2.5 product in epidemiological studies.

Abstract:Exposure to fine particulate matter (PM2.5) is associated with an increased risk of morbidity and mortality. In Europe, residential fuel combustion and road transport emissions contribute significantly to PM2.5. Toxicological studies indicate that PM2.5 from these sources is relatively more hazardous, owing to its high content of black and organic carbon. Here, we study the contribution of the emissions from these sectors to long-term exposure and excess mortality in Europe. We quantified the impact of anthropogenic carbonaceous aerosols on excess mortality and performed a sensitivity analysis assuming that they are twice as toxic as inorganic particles. We find that total PM2.5 from residential combustion leads to 72,000 (95% confidence interval: 48,000–99,000) excess deaths per year, with about 40% attributed to carbonaceous aerosols. Similarly, road transport leads to about 35,000 (CI 23,000–47,000) excess deaths per year, with 6000 (CI 4000–9000) due to carbonaceous particles. Assuming that carbonaceous aerosols are twice as toxic as other PM2.5 components, they contribute 80% and 37%, respectively, to residential fuel combustion and road transport-related deaths. We uncover robust national variations in the contribution of each sector to excess mortality and emphasize the importance of country-specific emission reduction policies based on national characteristics and sectoral shares.

Abstract:TPS 681: Short-term health effects of air pollutants 1, Exhibition Hall, Ground floor, August 26, 2019, 3:00 PM - 4:30 PM Background/Aim: Previous studies showed that short-term associations between ambient particulate air pollution (PM) and mortality are heterogeneous across populations in different areas of the world. However, little is known about the factors responsible for these inter-regional differences. In this contribution, we used data from a multi-country multi-city study to identify contextual factors that modify short-term mortality risks of PM. Methods: We collected daily series of mortality counts and air pollution data (PM10, PM2.5) for 327 cities in 25 countries in the period 1985-2015. City-level standardized measures of demographic, socioeconomic, and environmental indicators were derived from the OECD Regional/Metropolitan Database, in addition to climatological variables from the weather series. We applied a two-stage time-series design. First, for each city we modelled the linear associations of two-day moving average PM10, and PM2.5 with total mortality using quasi-Poisson regression and distributed lag nonlinear models. Second, we used meta-regression models to evaluate the effect modification of each indicator. Results: In preliminary analyses, the pooled percent mortality change for a 10 μg/m3 increase in PM10 and PM2.5 concentrations were 0.20% 95%CI(0.14%; 0.25%), and 0.31% 95%CI(0.21%; 0.41%) respectively. For PM10, significant changes of the figures above, for an IQR increase in each factor, were found for Gini index (0.12% to 0.29%, p=0.015), average temperature (0.13% to 0.27%, p=0.001), and Sprawl index (0.14% to 0.26%, p=0.033). For PM2.5, a tendency to increase the mortality risk was found for GDP (0.24% to 0.38%, p=0.073), while cities with an extended green area have a lower percent mortality change (0.41% to 0.20%, p=0.054). Conclusion: This represents the largest study assessing effect modification of ambient particulate air pollution and mortality relationships, performed by comparing different populations across the world. Preliminary findings suggest larger PM-mortality risks, in urban communities characterised by a higher level of inequalities, warmer climate, and less green area.
Potential Reductions in Premature Mortality Attributable to PM2.5 by Reducing Indoor Pollution: A Model Analysis for Beijing-Tianjin-Hebei of China.

Abstract:BACKGROUND:China has one of the highest PM2.5 (particulate matter with an aerodynamic diameter smaller than 2.5 μm) pollution levels in the world. It might still be long before air quality reaches the National Class II standard of 35 μg/m3.OBJECTIVE:We aim to estimate the potential reduction in premature mortality by reducing indoor PM2.5 levels in the Beijing-Tianjin-Hebei (BTH) region and compare it with reducing outdoor levels.METHODS:We combined PM2.5 transport model and the Global Burden of Disease (2016) methodology to estimate potential reductions in premature mortality attributable to PM2.5 by reducing indoor PM2.5 to National Class I standard of 15 μg/m3, and compared with reducing outdoor PM2.5 to Government 2020 Interim target of 64 μg/m3 or National Class II standard of 35 μg/m3.RESULTS:A total of 74,000 (95% confidence interval (CI): 43,000-111,000) premature deaths were attributable to PM2.5 exposure in 2013. Thirty percent, or 22,000 (95% CI: 17,000-32,000) deaths, would have been averted if indoor PM2.5 had reached the National Class I standard. The benefit is greater than that from reaching the Government 2020 Interim target for outdoor PM2.5 [22%, or 16,000 (95% CI: 12,000-23,000), deaths], although still smaller than that from reaching the National Class II standard [42%, or 31,000 (95% CI: 24,000-45,000), deaths].CONCLUSIONS:Reaching the National Class I level of indoor PM2.5 at current outdoor pollution levels could bring considerable health benefits, which are comparable to those from reaching the Government 2020 Interim target for outdoor PM2.5.MAIN FINDINGS:The avertable premature deaths gained from cleaning indoor PM2.5 to National Class I standard level would be greater than reducing outdoor PM2.5 to Government 2020 Interim target.

Abstract:Air quality in the United States has dramatically improved, yet exposure to air pollution is still associated with 100000–200000 deaths annually. Reducing the number of deaths effectively, efficiently, and equitably relies on attributing them to specific emission sources, but so far, this has been done for only highly aggregated groups of sources, or a select few sources of interest. Here, we estimate mortality in the United States attributable to all domestic, human-caused emissions of primary PM2.5 and secondary PM2.5 precursors. We present detailed source-specific attributions in four alternate groupings relevant for identifying promising ways to reduce mortality. We find that nearly half of the deaths can be attributed to just five activities, all in different sectors. Around half of the deaths can be attributed to fossil fuel combustion, with the remainder attributable to combustion of nonfossil fuels, agricultural processes, and other noncombustion processes. Both primary and secondary PM2.5 are important, including PM2.5 from currently unregulated precursor pollutants such as ammonia. We suggest improvements in air quality can be realized by continued reductions of emissions from traditionally important sources and by novel strategies for reducing emissions from sources of emerging relative importance and research focus. Abstract:Objective: Air pollution is a global public health issue, with particulate matter (PM) being the pollutant with the greatest impact on health. The main objective of this article was to estimate the impact of mortality attributable to particulate pollution in the city of Valencia during the period 2015-2017. Methods: The Health Impact Assessment (HIA) methodology from the Aphekom project was used. Scenarios of a 5 µg/m3 reduction in the annual mean concentration of PM10 and PM2.5 were employed, along with the assumption of meeting the World Health Organization (WHO) recommendations in effect during the study period, to estimate both short- and long-term impacts. Results: The estimated average concentrations for 2015-2017 were 18.4 µg/m3 for PM10 and 12.3 µg/m3 for PM2.5. The short-term HIA, assuming a reduction of 5 µg/m3 in the averages, resulted in a total of 65.4 premature deaths that could be postponed during that period (21.8 annually), corresponding to a rate of 2.8 deaths per 100,000 inhabitants. In the long term, if PM2.5 concentrations had been reduced by 5 µg/m3, 124 premature deaths could have been postponed annually. Conclusions: The annual average concentrations of these pollutants meet the limits set by European regulations. However, compared to WHO recommendations, PM2.5 levels are higher by 2.3 µg/m3. An air quality scenario in line with WHO recommendations would have resulted in a reduction of 122 premature deaths annually.
