Abstract:Blockchain technology, with its unique features, provides an ideal platform for developing a large‐scale electronic health record system. HealthChain, a blockchain‐based secure Electronic Health Record (EHR) System, is introduced. The experimental results demonstrate several significant improvements over traditional EHR systems. Currently, there is no unified Electronic Health Record (EHR) system connecting major healthcare organizations such as hospitals, medical centers, and specialists. Blockchain technology, with its unique features, provides an ideal platform for developing a large‐scale electronic health record system. In this article, the authors introduce HealthChain, a novel blockchain‐based secure EHR system that integrates advanced encryption techniques, a robust consent management system, cross‐platform interoperability, and enhanced scalability. Unlike existing EHR systems, HealthChain allows patients to have comprehensive control over their health data, ensuring that access is strictly regulated according to their preferences. The experimental results demonstrate several significant improvements over traditional EHR systems. HealthChain reduces data access times by 30%, and its interoperability rate with various healthcare systems is 40% higher than that of other blockchain‐based EHR solutions. Security is greatly enhanced, with HealthChain experiencing 50% fewer data breaches due to its advanced encryption and smart contract‐based access controls. Moreover, patient satisfaction has increased by 35% as a result of better control and access to their health records. These findings highlight HealthChain as not only a feasible and effective solution for managing health records but also a significant advancement over existing systems.

Abstract:The privacy of Electronic Health Records (EHRs) is facing a major hurdle with outsourcing private health data in the cloud as there exists danger of leaking health information to unauthorized parties. In fact, EHRs are stored on centralized databases that increases the security risk footprint and requires trust in a single authority which cannot effectively protect data from internal attacks. This research focuses on ensuring the patient privacy and data security while sharing the sensitive data across same or different organisations as well as healthcare providers in a distributed environment. This research develops a privacy-preserving framework viz Healthchain based on Blockchain technology that maintains security, privacy, scalability and integrity of the e-health data. The Blockchain is built on Hyperledger fabric, a permissioned distributed ledger solutions by using Hyperledger composer and stores EHRs by utilizing InterPlanetary File System (IPFS) to build this healthchain framework. Moreover, the data stored in the IPFS is encrypted by using a unique cryptographic public key encryption algorithm to create a robust blockchain solution for electronic health data. The objective of the research is to provide a foundation for developing security solutions against cyber-attacks by exploiting the inherent features of the blockchain, and thus contribute to the robustness of healthcare information sharing environments. Through the results, the proposed model shows that the healthcare records are not traceable to unauthorized access as the model stores only the encrypted hash of the records that proves effectiveness in terms of data security, enhanced data privacy, improved data scalability, interoperability and data integrity while sharing and accessing medical records among stakeholders across the healthchain network.

Abstract:Although the blockchain technology was first introduced through Bitcoin, extending its usage to non-financial applications, such as managing electronic medical records, is an attractive mission for recent research to balance the needs for increasing data privacy and the regular interaction among patients and health providers. Various systems that adopts the blockchain in managing medical records have been proposed. However, there is a need for more work to better characterize, understand and evaluate the employment of blockchain technology in the healthcare industry. In this paper, a design of blockchain based system, namely MedChain, for managing medical records is proposed. MedChain is designed to improve the current systems as it provides interoperable, secure, and effective access for medical records by patients, health care providers, and other third parties, while keeping the patients' privacy. MedChain employs timed-based smart contracts for governing transactions and controlling accesses to electronic medical records. It adopts advanced encryption techniques for providing further security. This work proposes a new incentive mechanism that leverages the degree of health providers regarding their efforts on maintaining medical records and creating new blocks. Extensive experiments are conducted to evaluate the MedChain performance, and results indicate the efficiency of our proposal in handling a large dataset at low latency.

Abstract:The last few years have witnessed exponential rise in online digital data for real-time communication, storage, and query-driven processing. It has broadened the horizon for decentralized and ad-hoc digital data used for swift decision making. Nowadays, Electronic healthcare (e-Healthcare) is one of the most demanding industries for telemedicine and Electronic Healthcare Record (EHR) communication purposes. However, ensuring data-security over uncertain channels has remained challenge. To alleviate data-security challenges and enable preserved and immutable data exchange, blockchain technology has been found potential. Despite such significances, most of the existing EHR -oriented blockchain frameworks are developed permissionless with hospital as central unit with almost all access rights. This as a result has demotivated patients due to private data breaches and fake-insurance claims. Considering this limitation developing a patient-centric secure blockchain framework was inevitable. Recently, securing EHR data is a hot research topic and the existing works used varied schemes to enhance the security and privacy of EHR with minimum cost. But the existing studies faces several critical issues like computational complexity, higher processing time, data leakage, sensitive to different attacks, scalability problem etc. The aforementioned issues in the existing studies leads the system as reduced confidentiality and data insecurity. Thus, to overcome such issues, this paper proposes a highly robust pre-transaction signature verification assisted patient-centric multi-stakeholder blockchain model. The proposed blockchain framework focused on multi-stakeholder scalability as well as interoperability aspects while guaranteeing attack-resilience. It was designed over Ethereum armored with blockchain manager (BCM) and smart contract components, where cloud-based interplanetary file system was developed to support decentralized data-storage. It employed a gateway component to perform user's signature verification to thwart away intruders. Once verifying the signature, BCM triggers user's request to the smart contract that verifies user details and allied role, right and access (RRA) policies. In sync with RRA policies for the requesting node, BCM allows user (patients and doctors) to perform data upload, view and sharing. To share the patient data, the proposed method triggers a signature request phase in which the doctor requests intended patient for signature and receiving the signature-acknowledgement it performs unabstracted data view and sharing and alleviates any probability of single-point-failure. The proposed blockchain framework provides a unified solution with improved flexibility, availability, decentralized access, identity management, user authentication, patient centric permissioned control and data security that makes it suitable for real-time purposes.

Abstract:Healthcare data is crucial and sensitive, as it contains absolute information about a patient's medical history, treatments, and actions; this information gets shared among the stakeholders on a routine basis. Patients' information is vital and should be kept accurate, up-to-date, and secret; it should be available only to authorized users. Most of the existing systems are centralized and may breach data privacy. This study mainly focuses on protecting the privacy and security of sensitive healthcare data while sharing with multiple stakeholders. This work presents a privacy-preserving and access-control blockchain-based framework that uses consensus-driven decentralized data management on top of peer-to-peer distributed computing platforms to ensure the privacy, security, accessibility, and integrity of healthcare data. Blockchain technology helps to protect transactions from manipulation due to its features of irreversibility and immutability. Additionally, we thoroughly examine the security requirements afforded by blockchain-enabled systems by incorporating stakeholders like patients, doctors, chemists, and pathology labs as system entities; they can only share information through a proper channel. The proposed framework has been implemented and evaluated using Hyperledger Fabric. We observe that the proposed framework reveals promising benefits in security, regulation compliance, reliability, flexibility, and accuracy.

A blockchain-enabled sharing platform for personal health records

Abstract:Background: Longitudinal personal health record (PHR) provides a foundation for managing patients' health care, but we do not have such a system in the U.S. except for the patients in the Department of Veterans Affairs. Such a gap exists mainly in the rest of the U.S. by the fact that patients' electronic health records are scattered across multiple health care facilities and often not shared due to privacy, security, and business interests concerns from both patients and health care organizations. In addition, patients have ethical concerns related to consent. To patients, data security, privacy, and consent are based on trustfulness, rather than patients' engagement in ensuring only authorized people can view their PHRs with patient-managed granularity. Resolving these challenges is an important step in making longitudinal PHR useful for patient care. Objective: This research aims to design and implement a blockchain-enabled sharing platform prototype for PHR with desired patient-controlled data security, privacy, and consent granularity. Methods: Built upon our prior work of a blockchain-enabled access control (BAC) model, we design a blockchain-enabled sharing platform for PHR with patient-controlled security, privacy, and consent granularity. We further implement the construct by building a prototypical platform among a patient and two typical health care organizations. Health organizations that hold the patient's electronic health records can join the platform with trust based on the validation from the patient. The mutual trust can be established through a rigorous validation process by both the patient and the built-in Hyperledger Fabric blockchain consensus mechanism. Results: We proposed a system trusted by patients and health care providers and constructed a Web-based PHR sharing platform with patient-controlled security, privacy, and consent granularity. We analyzed the system scalability in three aspects and showed millisecond range of performance when simultaneously changing access permissions on hundreds of PHRs. Consent, security and privacy of the model are ensured by the merits of the BAC model. We discovered the current blockchain model limits the system scalability due to using a non-graphical database. A new graphical database is suggested for future improvements. Conclusions: In this research, we report a solution to electronically sharing and managing patients' electronic health records originating from multiple organizations, focusing on privacy, security, and granularity control of consent in the U.S. Specifically, the system protects data security and privacy, and provides auditability, scalability, distributedness, patient consent autonomy, and zero-trust capabilities. The prototypical instantiation of the designed model suggested the feasibility of combining emerging blockchain technology with next generation access control model to tackle a longstanding longitudinal PHR problem.

Abstract:Background: In the era of big data, artificial intelligence (AI), and the Internet of Things (IoT), digital data have become essential for our everyday functioning and in health care services. The sensitive nature of health care data presents several crucial issues such as privacy, security, interoperability, and reliability that must be addressed in any health care data management system. However, most of the current health care systems are still facing major obstacles and are lacking in some of these areas. This is where decentralized, secure, and scalable databases, most notably blockchains, play critical roles in addressing these requirements without compromising security, thereby attracting considerable interest within the health care community. A blockchain can be maintained and widely distributed using a large network of nodes, mostly computers, each of which stores a full replica of the data. A blockchain protocol is a set of predefined rules or procedures that govern how the nodes interact with the network, view, verify, and add data to the ledger. Objective: In this article, we aim to explore blockchain technology, its framework, current applications, and integration with other innovations, as well as opportunities in diverse areas of health care and clinical research, in addition to clarifying its future impact on the health care ecosystem. We also elucidate 2 case studies to instantiate the potential role of blockchains in health care. Methods: To identify related existing work, terms based on Medical Subject Headings were used. We included studies focusing mainly on health care and clinical research and developed a functional framework for implementation and testing with data. The literature sources for this systematic review were PubMed, Medline, and the Cochrane library, in addition to a preliminary search of IEEE Xplore. Results: The included studies demonstrated multiple framework designs and various implementations in health care including chronic disease diagnosis, management, monitoring, and evaluation. We found that blockchains exhibit many promising applications in clinical trial management such as smart-contract application, participant-controlled data access, trustless protocols, and data validity. Electronic health records (EHRs), patient-centered interoperability, remote patient monitoring, and clinical trial data management were found to be major areas for blockchain usage, which can become a key catalyst for health care innovations. Conclusions: The potential benefits of blockchains are limitless; however, concrete data on long-term clinical outcomes based on blockchains powered and supplemented by AI and IoT are yet to be obtained. Nonetheless, implementing blockchains as a novel way to integrate EHRs nationwide and manage common clinical problems in an algorithmic fashion has the potential for improving patient outcomes, health care experiences, as well as the overall health and well-being of individuals.

Abstract:Electronic health records (EHRs) are increasingly replacing traditional paper-based medical records due to their speed, security, and ability to eliminate redundant data. However, challenges such as EHR interoperability and privacy concerns remain unresolved. Blockchain, a distributed ledger technology comprising connected, encrypted data blocks, presents a promising solution. This study explores how blockchain technology can revolutionize hospital EHR management. Our proposed solution securely transfers medical records between patients and doctors using the InterPlanetary File System (IPFS) and the Ethereum platform. Utilizing smart contracts automates data transfers, ensuring patient anonymity and reducing computational complexity while securely storing patient data on the network. Patient records are stored locally on the Ganache server, with the front end managed using HTML, CSS, ReactJS, and JavaScript, and the backend developed in Solidity. Blockchain technologies combined with Role- Based access control instead of attribute -based access control. The system's throughput increases linearly with the number of users and requests, enhancing the framework's efficiency and scalability. The minimum recorded latency is 14 ms.

Abstract:Blockchains as a technology emerged to facilitate money exchange transactions and eliminate the need for a trusted third party to notarize and verify such transactions as well as protect data security and privacy. New structures of Blockchains have been designed to accommodate the need for this technology in other fields such as e-health, tourism and energy. This paper is concerned with the use of Blockchains in managing and sharing electronic health and medical records to allow patients, hospitals, clinics, and other medical stakeholder to share data amongst themselves, and increase interoperability. The selection of the Blockchains used architecture depends on the entities participating in the constructed chain network. Although the use of Blockchains may reduce redundancy and provide caregivers with consistent records about their patients, it still comes with few challenges which could infringe patients' privacy, or potentially compromise the whole network of stakeholders. In this paper, we investigate different Blockchains structures, look at existing challenges and provide possible solutions. We focus on challenges that may expose patients' privacy and the resiliency of Blockchains to possible attacks.

Abstract:Electronic Health Records (EHRs) have undergone numerous technical improvements in recent years, including the incorporation of mobile devices with the cloud computing technologies to facilitate medical data exchanges between patients and the healthcare professionals. This cutting-edge architecture enables cyber physical systems housed in the cloud to provide healthcare services with minimal operational costs, high flexibility, security, and EHR accessibility. If patient health information is stored in the hospital database, there will always be a risk of intrusion, i.e., unauthorized file access and information modification by attackers. To address this concern, we propose a decentralized EHR system based on Blockchain technology. To facilitate secure EHR exchange across various patients and medical providers, we develop a reliable access control method based on smart contracts. We incorporate Cryptocurrency, specifically Ethereum, in the suggested system to protect sensitive health information from potential attackers. In our suggested approach, both physicians and patients are required to be authenticated. Patients can register, and a block with a unique hash value will be generated. Once the patient discusses the disease with the physician, the physician can check the patient's condition and offer drugs. For experimental findings, we employ the public Block chain Ganache and solidity remix-based smart contracts to protect privacy. Ethers are used as the crypto currencies.

Abstract:The rapid development of wearable sensors and the 5G network empowers traditional medical treatment with the ability to collect patients’ information remotely for monitoring and diagnosing purposes. Meanwhile, the health-related mobile apps and devices also generate a large amount of medical data, which is critical for promoting disease research and diagnosis. However, medical data is too sensitive to share, which is also a common issue for IoT (Internet of Things) data. The traditional centralized cloud-based medical data sharing schemes have to rely on a single trusted third party. Therefore, the schemes suffer from single-point failure and lack of privacy protection and access control for the data. Blockchain is an emerging technique to provide an approach for managing data in a decentralized manner. Especially, the blockchain-based smart contract technique enables the programmability for participants to access the data. All the interactions are authenticated and recorded by the other participants of the blockchain network, which is tamper resistant. In this paper, we leverage the K-anonymity and searchable encryption techniques and propose a blockchain-based privacy-preserving scheme for medical data sharing among medical institutions and data users. To be specific, the consortium blockchain, Hyperledger Fabric, is adopted to allow data users to search for encrypted medical data records. The smart contract, i.e., the chaincode, implements the attribute-based access control mechanisms to guarantee that the data can only be accessed by the user with proper attributes. The K-anonymity and searchable encryption ensure that the medical data is shared without privacy leaking, i.e., figuring out an individual patient from queries. We implement a prototype system using the chaincode of Hyperledger Fabric. From the functional perspective, security analysis shows that the proposed scheme satisfies security goals and precedes others. From the performance perspective, we conduct experiments by simulating different numbers of medical institutions. The experimental results demonstrate that the scalability and performance of our scheme are practical.

Abstract:A blockchain is a system for storing and sharing information that is secure because of its transparency. Each block in the chain is both its own independent unit containing its own information, and a dependent link in the collective chain, and this duality creates a network regulated by participants who store and share the information, rather than a third party. Blockchain has many applications in healthcare, and can improve mobile health applications, monitoring devices, sharing and storing of electronic medical records, clinical trial data, and insurance information storage. Research about blockchain and healthcare is currently limited, but blockchain is on the brink of transforming the healthcare system; through its decentralized principles, blockchain can improve accessibility and security of patient information, and can therefore overturn the healthcare hierarchy and build a new system in which patients manage their own care.

Abstract:No doubt that medical data sharing is very crucial but important in realizing cross-hospital diagnosis and improving research development. In traditional mechanisms, there always exists a tradeoff between correctness of medical data and patient privacy. Here we are going to introduce the concept of blockchain and smart contract to build up an electronic medical record sharing mechanism: medical blockchain. Specifically, we have implemented the medical record sharing through cryptography design. Simulations have demonstrated that the correctness of medical data and privacy of patient can be guaranteed through the adoption of blockchain, while the integrity of a specific patient can be achieved via the smart contract control. Furthermore, the proposed medical blockchain can resist the potential Internet attack simulated by the formal verification. Thus, no leakage of patient identity occurs, and the tradeoff could be eliminated effectively without the modification of current hospital devices according to the simulation results.

Abstract:In an era driven by information exchange, transparency and security hold crucial importance, particularly within the healthcare industry, where data integrity and confidentiality are paramount. This paper investigates the integration of blockchain technology in healthcare, focusing on its potential to revolutionize Electronic Health Records (EHR) management and data sharing. By leveraging Ethereum-based blockchain implementations and smart contracts, we propose a novel system that empowers patients to securely store and manage their medical data. Our research addresses critical challenges in implementing blockchain in healthcare, including scalability, user privacy, and regulatory compliance. We propose a solution that combines digital signatures, Role-Based Access Control, and a multi-layered architecture to enhance security and ensure controlled access. The system's key functions, including user registration, data append, and data retrieval, are facilitated through smart contracts, providing a secure and efficient mechanism for managing health information. To validate our approach, we developed a decentralized application (dApp) that demonstrates the practical implementation of our blockchain-based healthcare solution. The dApp incorporates user-friendly interfaces for patients, doctors, and administrators, showcasing the system's potential to streamline healthcare processes while maintaining data security and integrity. Abstract:Background: With increased specialization of health care services and high levels of patient mobility, accessing health care services across multiple hospitals or clinics has become very common for diagnosis and treatment, particularly for patients with chronic diseases such as cancer. With informed knowledge of a patient's history, physicians can make prompt clinical decisions for smarter, safer, and more efficient care. However, due to the privacy and high sensitivity of electronic health records (EHR), most EHR data sharing still happens through fax or mail due to the lack of systematic infrastructure support for secure, trustable health data sharing, which can also cause major delays in patient care. Objective: Our goal was to develop a system that will facilitate secure, trustable management, sharing, and aggregation of EHR data. Our patient-centric system allows patients to manage their own health records across multiple hospitals. Each hospital will provide a blockchain node integrated with its own EHR system to form the blockchain network. A web-based interface will be used for patients and doctors to initiate EHR sharing transactions. We take a hybrid data management approach, where only management metadata will be stored on the chain. Actual EHR data, on the other hand, will be encrypted and stored off-chain in Health Insurance Portability and Accountability Act-compliant cloud-based storage. The system uses public key infrastructure-based asymmetric encryption and digital signatures to secure shared EHR data. Results: In collaboration with Stony Brook University Hospital, we developed ACTION-EHR, a system for patient-centric, blockchain-based EHR data sharing and management for patient care, in particular radiation treatment for cancer. The prototype was built on Hyperledger Fabric, an open-source, permissioned blockchain framework. Data sharing transactions were implemented using chaincode and exposed as representational state transfer application programming interfaces used for the web portal for patients and users. The HL7 Fast Healthcare Interoperability Resources standard was adopted to represent shared EHR data, making it easy to interface with hospital EHR systems and integrate a patient's EHR data. We tested the system in a distributed environment at Stony Brook University using deidentified patient data. Conclusions: We studied and developed the critical technology components to enable patient-centric, blockchain-based EHR sharing to support cancer care. The prototype demonstrated the feasibility of our approach as well as some of the major challenges. The next step will be a pilot study with health care providers in both the United States and Switzerland. Our work provides an exemplar testbed to build next-generation EHR sharing infrastructures.
An Architecture and Management Platform for Blockchain-Based Personal Health Record Exchange: Development and Usability Study

Abstract:Background: Personal health record (PHR) security, correctness, and protection are essential for health and medical services. Blockchain architecture can provide efficient data retrieval and security requirements. Exchangeable PHRs and the self-management of patient health can offer many benefits to traditional medical services by allowing people to manage their own health records for disease prevention, prediction, and control while reducing resource burdens on the health care infrastructure and improving population health and quality of life. Objective: This study aimed to build a blockchain-based architecture for an international health record exchange platform to ensure health record confidentiality, integrity, and availability for health management and used Health Level 7 Fast Healthcare Interoperability Resource international standards as the data format that could allow international, cross-institutional, and patient/doctor exchanges of PHRs. Methods: The PHR architecture in this study comprised 2 main components. The first component was the PHR management platform, on which users could upload PHRs, view their record content, authorize PHR exchanges with doctors or other medical health care providers, and check their block information. When a PHR was uploaded, the hash value of the PHR would be calculated by the SHA-256 algorithm and the PHR would be encrypted by the Rivest-Shamir-Adleman encryption mechanism before being transferred to a secure database. The second component was the blockchain exchange architecture, which was based on Ethereum to create a private chain. Proof of authority, which delivers transactions through a consensus mechanism based on identity, was used for consensus. The hash value was calculated based on the previous hash value, block content, and timestamp by a hash function. Results: The PHR blockchain architecture constructed in this study is an effective method for the management and utilization of PHRs. The platform has been deployed in Southeast Asian countries via the Asia eHealth Information Network (AeHIN) and has become the first PHR management platform for cross-region medical data exchange. Conclusions: Some systems have shown that blockchain technology has great potential for electronic health record applications. This study combined different types of data storage modes to effectively solve the problems of PHR data security, storage, and transmission and proposed a hybrid blockchain and data security approach to enable effective international PHR exchange. By partnering with the AeHIN and making use of the network's regional reach and expert pool, the platform could be deployed and promoted successfully. In the future, the PHR platform could be utilized for the purpose of precision and individual medicine in a cross-country manner because of the platform's provision of a secure and efficient PHR sharing and management architecture, making it a reasonable base for future data collection sources and the data analytics needed for precision medicine.

Abstract:The healthcare industry has been transitioning from paper-based medical records to electronic health records (EHRs) in most healthcare facilities. However, the current EHR frameworks face challenges in secure data storage, credibility, and management. Interoperability and user control of personal data are also significant concerns in the healthcare sector. Although block chain technology has emerged as a powerful solution that can offer the properties of immutability, security, and user control on stored records, its potential application in EHR frameworks is not yet fully understood. To address this gap in knowledge, this research aims to provide an interoperable blockchain-based EHR framework that can fulfill the requirements defined by various national and international EHR standards such as HIPAA and HL7. The research method employed is a systematic literature review to explore the current state of the art in the field of EHRs, including blockchain-based implementations of EHRs. The study defines the interoperability issues in the existing blockchain-based EHR frameworks, reviews various national and international standards of EHR, and further defines the interoperability requirements based on these standards. The proposed framework can offer safer methods to interchange health information for the healthcare sector and can provide the properties of immutability, security, and user control on stored records without the need for centralized storage. The contributions of this work include enhancing the understanding of the potential application of blockchain technology in EHR frameworks and proposing an interoperable blockchain-based EHR framework that can fulfill the requirements defined by various national and international EHR standards. Overall, this study has significant implications for the healthcare sector, as it can enhance the secure sharing and storage of electronic health data while ensuring the confidentiality, privacy, and integrity of medical records.

Abstract:Many researchers in recent years have shown a keen interest in finding a feasible and appropriate solution for storing big medical data, mainly on cloud-based storage using centralized architectures. But, there are some challenges, like ownership of data, interoperability with other platforms, fault tolerance, and security and privacy issues in implementing a cloud-based storage architecture. So, it is necessary to design a reliable architecture to overcome all these constraints and limitations. This paper proposes a patient-centric framework called HealthRec-Chain to store and share susceptible medical reports and images. The proposed HealthRec-Chain framework uses distributed file system technology called Interplanetary file system (IPFS) to store automated encrypted health records through Java-enabled GPG encryption to provide high-level security and privacy. It demonstrates the layered approach for HealthRec-Chains through an experimental setup in a given simulation environment. HealthRec-Chain justifies its feasibility by uniquely building a personalized Ethereum dashboard to monitor performance. It also measures a network hash rate to find the security attack's difficulty. It proposes manual benchmarking for measuring its performance parameters. Also, the proposed work compares the existing framework with different performance metrics like latency, throughput, scalability, ease of use, etc. Hence, it validates the feasibility of HealthRec-Chain in a unified view of the solution's healthcare security, privacy, and scalability.
