Abstract:BACKGROUND:Precisely localizing the seizure onset zone (SOZ) is critical for focal epilepsy surgery. Existing methods mainly focus on high-frequency activities in stereo-electroencephalography, but often fail when seizures are not driven by high-frequency activities. Recognized as biomarkers of epileptic seizures, ictal spikes in SOZ induce epileptiform discharges in other brain regions. Based on this understanding, we aim to develop a universal algorithm to localize SOZ and investigate how ictal spikes within the SOZ induce seizures. METHODS:We proposed a novel metric called standard deviation of spike amplitude (SDSA) and utilized channel-averaged SDSA to describe seizure processes and detect seizures. By integrating SDSA values in specific intervals, the score for each channel located within SOZ was calculated. Channels with high SOZ scores were clustered as SOZ. The localization accuracy was asserted using area under the receiver operating characteristic (ROC) curve. Further, we analyzed early ictal signals from SOZ channels and investigated factors influencing their duration to reveal the seizure inducing conditions. RESULTS:We analyzed data from 15 patients with focal epilepsy. The channel-averaged SDSA successfully detected all 28 seizures without false alarms. Using SDSA integration, we achieved precise SOZ localization with an average area under ROC curve (AUC) of 0.96, significantly outperforming previous methods based on high-frequency activities. Further, we discovered that energy of ictal spikes in SOZ was concentrated at a specific frequency distributed in [6, 12 Hz]. Additionally, we found that the higher the energy per second in this frequency band, the faster ictal spikes could induce seizures. CONCLUSION:The SDSA metric offered precise SOZ localization with robustness and low computational cost, making it suitable for clinical practice. By studying the propagation patterns of ictal spikes between the SOZ and non-SOZ, we suggest that ictal spikes from SOZ need to accumulate energy at a specific central frequency to induce epileptic spikes in non-SOZ, which may have significant implications for understanding the seizure onset pattern.
Localization of the epileptogenic zone based on ictal stereo-electroencephalogram: Brain network and single-channel signal feature analysis

Abstract:Accurate localization of the epileptogenic zone (EZ) is crucial for refractory focal epilepsy patients to achieve freedom from seizures following epilepsy surgery. In this study, ictal stereo-electroencephalography data from 35 patients with refractory focal epilepsy were analyzed. Effective networks based on partial directed coherence were analyzed, and a gray level co-occurrence matrix was applied to extract the time-varying features of the in-degree. These features, combined with the single-channel signal time-frequency features, including approximate entropy and line length, were used to localize the EZ based on a cluster algorithm. For all seizure-free patients (n = 23), the proposed method was effective in identifying the clinical-EZ-contacts and clinical-EZ-blocks, with an F1-score of 62.47 % and 72.18 %, respectively. The sensitivity was 96.00 % for the clinical-EZ-block identification, which provided the information for the decision-making of clinicians, prompting clinicians to focus on the identified EZ-blocks and their nearby contacts. The agreement between the EZ identified by the proposed method and the clinical-EZ was worse for non-seizure-free patients (n = 12) than for seizure-free patients. Furthermore, our method provided better results than using only brain network or single-channel signal features. This suggests that combining these complementary features can facilitate more accurate localization of the EZ.
Transient seizure onset network for localization of epileptogenic zone: effective connectivity and graph theory-based analyses of ECoG data in temporal lobe epilepsy

Abstract:ObjectiveAbnormal and dynamic epileptogenic networks cause difficulties for clinical epileptologists in the localization of the seizure onset zone (SOZ) and the epileptogenic zone (EZ) in preoperative assessments of patients with refractory epilepsy. The aim of this study is to investigate the characteristics of time-varying effective connectivity networks in various non-seizure and seizure periods, and to propose a quantitative approach for accurate localization of SOZ and EZ.MethodsWe used electrocorticogram recordings in the temporal lobe and hippocampus from seven patients with temporal lobe epilepsy to characterize the effective connectivity dynamics at a high temporal resolution using the full-frequency adaptive directed transfer function (ffADTF) measure and five graph metrics, i.e., the out-degree (OD), closeness centrality (CC), betweenness centrality (BC), clustering coefficient (C), and local efficiency (LE). The ffADTF effective connectivity network was calculated and described in five frequency bands (δ, θ, α, β, and γ) and five seizure periods (pre-seizure, early seizure, mid-seizure, late seizure, and post-seizure). The cortical areas with high values of graph metrics in the transient seizure onset network were compared with the SOZ and EZ identified by clinical epileptologists and the results of epilepsy resection surgeries.ResultsOrigination and propagation of epileptic activity were observed in the high time resolution ffADTF effective connectivity network throughout the entire seizure period. The seizure-specific transient seizure onset ffADTF network that emerged at seizure onset time remained for approximately 20–50 ms with strong connections generated from both SOZ and EZ. The values of graph metrics in the SOZ and EZ were significantly larger than that in the other cortical areas. More cortical areas with the highest mean of graph metrics were the same as the clinically determined SOZ in the low-frequency δ and θ bands and in Engel Class I patients than in higher frequency α, β, and γ bands and in Engel Class II and III patients. The OD and C were more likely to localize the SOZ and EZ than CC, BC, and LE in the transient seizure onset network.ConclusionThe high temporal resolution ffADTF effective connectivity analysis combined with the graph theoretical analysis helps us to understand how epileptic activity is generated and propagated during the seizure period. The newly discovered seizure-specific transient seizure onset network could be an important biomarker and a promising tool for more precise localization of the SOZ and EZ in preoperative evaluations.

Abstract:Objective To localize epileptogenic zone and to analyze its network coherence using high frequency intracranial EEG during seizure.Methods First, a normalized high frequency energy index (HFEI) was calculated using intracranial EEG.Then, the index was used to quantify the epileptogenicity of each electrode.Finally, the wavelet coherence was estimated between epileptogenic and non-epileptogenic electrodes.Results The HFEI of intracranial EEG (1252 contacts) from 8 patients with good postoperative outcome were extracted and those electrodes with HFEI above the threshold were identified as epileptogenic electrodes.Compared with physician's localization results, the sensitivity of our method was 29.57%, and specificity was 96.67%.Average coherence between epileptic electrodes of the 8 patients selected by HFEI was significantly higher than that between non epileptic electrodes.Conclusions As a nonparametric statistical method, the HFEI index we propose is successfully used to localize epileptogenic network, with higher specificity than previous studies, which meets the need of identifying the key epileptic electrodes during pre-surgical planning.

Abstract:OBJECTIVES: Stereoelectroencephalography (SEEG) can define the epileptogenic zone (EZ). However, SEEG is susceptible to the sampling bias, where no SEEG recording is taken within a circumscribed EZ.METHODS: Nine patients with medically refractory epilepsy underwent SEEG recording, and brain resection got positive outcomes. Ictal neuronal currents were estimated by distributed source modeling using the SEEG data and individual's anatomical magnetic resonance imaging. Using a retrospective leave-one-out data sub-sampling, we evaluated the sensitivity and specificity of the current estimates using MRI after surgical resection or radio-frequency ablation.RESULTS: The sensitivity and specificity in detecting the EZ were indistinguishable from either the data from all electrodes or the sub-sampled data (rank sum test: rank sum = 23719, p = 0.13) when at least one remaining electrode contact was no more than 20 mm away.CONCLUSIONS: The distributed neuronal current estimates of ictal SEEG data can mitigate the challenge of delineating the boundary of the EZ in cases of missing an electrode implanted within the EZ and a required second SEEG exploration.SIGNIFICANCE: Distributed source modeling can be a tool for clinicians to infer the EZ by allowing for more flexible planning of the electrode implantation route and minimizing the number of electrodes.

Abstract:Surgical resection of the epileptogenic zone (EZ) is an effective method for treating drug-resistant epilepsy. At present, the accuracy of EZ localization needs to be further improved. The characteristics of graph theory based on partial directed coherence networks have been applied to the localization of EZ, but the application of network control theory to effective networks to locate EZ is rarely reported. In this study, the method of partial directed coherence analysis was utilized to construct the time-varying effective brain networks of stereo-electroencephalography (SEEG) signals from 20 seizures in 12 patients. Combined with graph theory and network control theory, the differences in network characteristics between epileptogenic and non-epileptogenic zones during seizures were analyzed. We also used dung beetle optimized support vector machine classification model to evaluate the localization effect of EZ based on brain network characteristics of graph theory and controllability. The results showed that the classification of the average controllability feature was the best, and the area under the receiver operating characteristic (ROC) curve (AUC) was 0.9505, which is 1.32 % and 1.97 % higher than the traditional methods. The AUC value increased to 0.9607 after integrating the average controllability with other features. This study proved the effectiveness of controllability characteristic in identifying the EZ and provided a theoretical basis for the clinical application of network controllability in the EZ.

Abstract:Background: Most epilepsy research is based on interictal or ictal functional connectivity. However, prolonged electrode implantation may affect patients' health and the accuracy of epileptic zone identification. Brief resting-state SEEG recordings reduce the observation of epileptic discharges by reducing electrode implantation and other seizure-inducing interventions. New method: The location coordinates of SEEG in the brain were identified using CT and MRI. Based on undirected brain network connectivity, five functional connectivity measures and data feature vector centrality were calculated. Network connectivity was calculated from multiple perspectives of linear correlation, information theory, phase, and frequency, and the relative influence of nodes on network connectivity was considered. We investigated the potential value of resting-state SEEG for epileptic zone identification by comparing the differences between epileptic and non-epileptic zones, as well as the differences between patients with different surgical outcomes. Results: By comparing the centrality of brain network connectivity between epileptic and non-epileptic zones, we found significant differences in the distribution of brain networks between the two zones. There was a significant difference in brain network between patients with good surgical outcomes and those with poor surgical outcomes (p < 0.01). By combining support vector machines with static node importance, we predicted an AUC of 0.94 ± 0.08 for the epilepsy zone. Conclusions and significance: The results illustrated that nodes in epileptic zones are distinct from those in non-epileptic zones. Analysis of resting-state SEEG data and the importance of nodes in the brain network may contribute to identifying the epileptic zone and predicting the outcome.

Abstract:Objective To propose a novel stereo-electroencephalography(SEEG) quantitative measure analyzing ictal high frequency (60-90 Hz) and calculating high frequency epileptogenicity index (HFEI) to localize epileptogenic zone and evaluate epileptogenic network. Methods The clinical presurgical evaluation and SEEG data of 15 patients who were performed SEEG electrodes implantation from April 2015 to March 2016 were analyzed retrospectively. Post-implantation head CT images and 3D MRI data were fused for accurately identifying and locating each electrode contact. Ictal SEEG quantitative measure HFEI was calculated and threshold was set. The epileptogenic network was divided into focal, regional, multiple regional and bilateral ones and the results were compared with the pathological results.Results The epileptogenic network was focal for four patients, regional for four patients, multiple regional for six patients and bilateral for one patient (7/15). In terms of the pathology,two cases with hippocampal sclerosis both showed regional network. In four cases with cerebral malacia, two cases showed multiple regional network and the other two cases showed focal network. In six cases with cortical malformation, three cases showed multiple regional network, the other three cases showed focal, regional and bilateral networks respectively. Conclusions We explored a novel SEEG quantitative measure based on the high frequency power analysis,which is objective and could localize epileptogenic zone and evaluate the epileptic network.

Abstract:Background: Positive correlations between low- and high-frequency spectra from stereotactic electroencephalogram (SEEG) recordings have been implicated in pathological brain activity interictally and have been used for ictal detection in both focal and network models. Objective: We evaluated SEEG signals in patients who ultimately underwent temporal lobectomy to evaluate their utility in seizure localization and prediction of seizure freedom post-resection. Methods: We retrospectively analyzed cross-frequency correlations between beta and high gamma (HG) interictal SEEG signals from 22 patients. We compared signals based on temporal versus extra-temporal locations, seizure-free (SF) versus non-seizure-free (NSF) outcomes, and mesial (M) versus mesial temporal-plus (M+) onset. Results: Positive cross-correlations were increased in temporal areas. NSF patients showed a higher proportion of positive electrodes in temporal areas. SF patients had a greater proportion of significant channels in mesial versus lateral temporal areas. HG/Beta correlations in mesial versus lateral temporal areas predicted seizure freedom better than ictal SEEG seizure onset localization to M or M+ locations. Conclusions: We present preliminary data that local HG/Beta correlations may predict epilepsy focus and surgical outcome and may have utility as adjunct methods to conventional SEEG analysis. Further studies are needed to determine strategies for prospective studies and clinical use.

Abstract:This work presents a novel brain network analysis to identify the seizure onset zone and predict seizure outcome with an accuracy of around 90% using short‐time resting‐state stereotactic‐electroencephalography (SEEG) data, suggesting that brief resting‐state SEEG data has tremendous clinical values in diagnosis and aiding surgical planning in drug‐resistant epilepsy without requiring long‐term ictal recordings. Localization of epileptogenic zone currently requires prolonged intracranial recordings to capture seizure, which may take days to weeks. The authors developed a novel method to identify the seizure onset zone (SOZ) and predict seizure outcome using short‐time resting‐state stereotacticelectroencephalography (SEEG) data. In a cohort of 27 drug‐resistant epilepsy patients, the authors estimated the information flow via directional connectivity and inferred the excitation‐inhibition ratio from the 1/f power slope. They hypothesized that the antagonism of information flow at multiple frequencies between SOZ and non‐SOZ underlying the relatively stable epilepsy resting state could be related to the disrupted excitation‐inhibition balance. They found flatter 1/f power slope in non‐SOZ regions compared to the SOZ, with dominant information flow from non‐SOZ to SOZ regions. Greater differences in resting‐state information flow between SOZ and non‐SOZ regions are associated with favorable seizure outcome. By integrating a balanced random forest model with resting‐state connectivity, their method localized the SOZ with an accuracy of 88% and predicted the seizure outcome with an accuracy of 92% using clinically determined SOZ. Overall, this study suggests that brief resting‐state SEEG data can significantly facilitate the identification of SOZ and may eventually predict seizure outcomes without requiring long‐term ictal recordings.

Abstract:Seizure types and characteristics may vary with time in a patient with distinct mechanisms underlying the propagation of ictal activity. Similarly, we found that both focal and generalized seizures coexist in some pilocarpine-induced chronic temporal lobe epilepsy (TLE) rats. In different seizure patterns, mapping complex networks and analyzing epileptic characteristics involved in seizure propagation are likely to reflect seizure propagation mechanisms, and indicate the establishment of stimulation strategy for epilepsy treatment, especially on the selection of stimulation targets. In our study, we used Granger causality method to track the time-variant epileptic brain functional connectivity in focal and generalized seizures from multi-site local field potentials (LFPs). Results showed that these two major types of seizures had different propagation patterns during ictal period. When comparing them, generalized seizures involved in a network with more complex relationships and spread to more extensive brain regions than in local seizures at mid-ictal stage. Moreover, we observed that focal seizures had a focused causal hub with strong interactions, while generalized seizures had relative distributed causal hubs to drive the development of seizure during seizure-onset stage. These findings suggest that stimulation strategy might need to be adapted to different seizure types thus allowing for retuning abnormal epileptic brain network and obtaining better treatment effect on seizure suppression.
Epileptic Discharge Related Functional Connectivity Within and Between Networks in Benign Epilepsy with Centrotemporal Spikes

Abstract:Benign epilepsy with centrotemporal spikes (BECTS) is a common childhood epilepsy syndrome associated with abnormalities in neurocognitive domains, particularly during interictal epileptiform discharges (IEDs). Here, we investigated the effects of IEDs on brain’s intrinsic connectivity networks in 43 BECTS patients and 28 matched healthy controls (HCs). Patients were further divided into IED and non-IED subgroups based on simultaneous EEG-fMRI recordings. Functional connectivity within and between five networks, corresponding to seizure origination and cognitive processes, were analyzed to measure IED effects. We found that patients exhibited increased connectivity within the auditory network (AN) and the somato-motor network (SMN), and decreased connectivity within the basal ganglia network and the dorsal attention network, suggesting that both transient and chronic seizure activity may disturb normal network organization. The IED group showed decreased functional connectivity within the default mode network (DMN) compared with the non-IED group and HCs, implying that the DMN was selectively impaired during epileptiform discharges associated with altered self-referential cognitive functions. Moreover, the IED group exhibited increased positive correlations between the AN and the SMN, which suggests a possible excessive influence of centrotemporal spiking on information processing in the auditory system. The association between epileptic activity and network dysfunctions highlights their importance in investigating the pathological mechanism underlying BECTS.

We further evaluated any statistical differences between the seizure free (SF) and non-seizure free (NSF) groups, and mesial (M) versus mesial temporal-plus (M+) onset location. RESULTS: 1) Clinical recordings from SEEG electrodes can detect cross-frequency correlations between HG/Beta using our previous methodology. 2) Positive correlations are significantly increased in temporal versus other sampled areas. 3) Positive correlations are more predictive of seizure onset during asleep recordings. 4) NSF patients show a significantly higher proportion of positive electrodes in the resected temporal lobe than SF patients. 5) SF patients have a greater proportion of significant contacts in the mesial versus the lateral temporal lobe than NSF patients. 6) Significant HG/Beta correlations in the mesial versus lateral temporal lobe predict seizure freedom better than ictal SEEG localization of seizure onset to M or M+ locations. CONCLUSIONS: We present preliminary data that local HG/Beta correlations may be predictive for epilepsy focus and surgical outcome and may have utility as adjunct methods to conventional SEEG analysis. Abstract:Epileptogenic networks are defined by the brain regions involved in the production and propagation of epileptic activities. In this review we describe the historical, methodologic, and conceptual bases of this model in the analysis of electrophysiologic intracerebral recordings. In the context of epilepsy surgery, the determination of cerebral regions producing seizures (i.e., the "epileptogenic zone") is a crucial objective. In contrast with a traditional focal vision of focal drug-resistant epilepsies, the concept of epileptogenic networks has been progressively introduced as a model better able to describe the complexity of seizure dynamics and realistically describe the distribution of epileptogenic anomalies in the brain. The concept of epileptogenic networks is historically linked to the development of the stereoelectroencephalography (SEEG) method and subsequent introduction of means of quantifying the recorded signals. Seizures, and preictal and interictal discharges produce clear patterns on SEEG. These patterns can be analyzed utilizing signal analysis methods that quantify high-frequency oscillations or changes in functional connectivity. Dramatic changes in SEEG brain connectivity can be described during seizure genesis and propagation within cortical and subcortical regions, associated with the production of different patterns of seizure semiology. The interictal state is characterized by networks generating abnormal activities (interictal spikes) and also by modified functional properties. The introduction of novel approaches to large-scale modeling of these networks offers new methods in the goal of better predicting the effects of epilepsy surgery. The epileptogenic network concept is a key factor in identifying the anatomic distribution of the epileptogenic process, which is particularly important in the context of epilepsy surgery.

Seizure‐onset regions demonstrate high inward directed connectivity during resting‐state: An SEEG study in focal epilepsy

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>In patients with medically refractory focal epilepsy, stereotactic‐electroencephalography (SEEG) can aid in localizing epileptogenic regions for surgical treatment. SEEG, however, requires long hospitalizations to record seizures, and ictal interpretation can be incomplete or inaccurate. Our recent work showed that non‐directed resting‐state analyses may identify brain regions as epileptogenic or uninvolved. Our present objective is to map epileptogenic networks in greater detail and more accurately identify seizure‐onset regions using directed resting‐state SEEG connectivity.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>In 25 patients with focal epilepsy who underwent SEEG, 2 minutes of resting‐state, artifact‐free, SEEG data were selected and functional connectivity was estimated. Using standard clinical interpretation, brain regions were classified into four categories: ictogenic, early propagation, irritative, or uninvolved. Three non‐directed connectivity measures (mutual information [MI] strength, and imaginary coherence between and within regions) and four directed measures (partial directed coherence [PDC] and directed transfer function [DTF], inward and outward strength) were calculated. Logistic regression was used to generate a predictive model of ictogenicity.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Ictogenic regions had the highest and uninvolved regions had the lowest MI strength. Although both PDC and DTF inward strengths were highest in ictogenic regions, outward strengths did not differ among categories. A model incorporating directed and nondirected connectivity measures demonstrated an area under the receiver‐operating characteristic (ROC) curve (AUC) of 0.88 in predicting ictogenicity of individual regions. The AUC of this model was 0.93 when restricted to patients with favorable postsurgical seizure outcomes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Significance</h3><p>Directed connectivity measures may help identify epileptogenic networks without requiring ictal recordings. Greater inward but not outward connectivity in ictogenic regions at rest may represent broad inhibitory input to prevent seizure generation.</p></section>

Individual localization value of resting-state fMRI in epilepsy presurgical evaluation: A combined study with stereo-EEG

Abstract:ObjectiveTo examine the individual-patient-level localization value of resting-state functional MRI (rsfMRI) metrics for the seizure onset zone (SOZ) defined by stereo-electroencephalography (SEEG) in patients with medically intractable focal epilepsies.MethodsWe retrospectively included 19 patients who underwent SEEG implantation for epilepsy presurgical evaluation. Voxel-wise whole-brain analysis was performed on 3.0T rsfMRI to generate clusters for amplitude of low-frequency fluctuations (ALFF), regional homogeneity (ReHo) and degree centrality (DC), which were co-registered with the SEEG-defined SOZ to evaluate their spatial overlap. Subgroup and correlation analyses were conducted for different clinical characteristics.ResultsALFF demonstrated concordant clusters with SEEG-defined SOZ in 73.7% of patients, with 93.3% sensitivity and 77.8% PPV. The concordance rate showed no significant difference when subgrouped by lesional/non-lesional MRI, SOZ location, interictal epileptiform discharges on scalp EEG, pathology or seizure outcomes. No significant correlation was seen between ALFF concordance rate and epilepsy duration, seizure-onset age, seizure frequency or number of antiseizure medications. ReHo and DC did not achieve favorable concordance results (10.5% and 15.8%, respectively). All concordant clusters showed regional activation, representing increased neural activities.ConclusionALFF had high concordance rate with SEEG-defined SOZ at individual-patient level.SignificanceALFF activation on rsfMRI can add localizing information for the noninvasive presurgical workup of intractable focal epilepsies.

Abstract:Objective: To demonstrate the capability of utilizing graph feature-based supervised machine learning (ML) algorithm on intracranial electroencephalogram (iEEG) recordings for the identification of seizure onset zones (SOZs) in individuals with drug-resistant epilepsy. Approach: Utilizing three model-free measures of effective connectivity (EC) – directed information (DI), mutual information-guided Granger causality index (MI-GCI), and frequency-domain convergent cross-mapping (FD-CCM) – directed graphs are generated. Graph centrality measures at different sparsity are used as the classifier's features. Main results: The centrality features achieve high accuracies exceeding 90% in distinguishing SOZ electrodes from non-SOZ electrodes. Notably, a sparse graph representation with just ten features and simple machine learning models effectively achieves such performance. The study identifies FD-CCM centrality measures as particularly significant, with a mean AUC of 0.93, outperforming prior literature. The FD-CCM-based graph modeling also highlights elevated centrality measures among SOZ electrodes, emphasizing heightened activity relative to non-SOZ electrodes during ictogenesis. Significance: This research not only underscores the efficacy of automated SOZ identification but also illuminates the potential of specific EC measures in enhancing discriminative power within the context of epilepsy research.

Abstract:The neuronal synchronous discharging may cause an epileptic seizure. Currently, most of the studies conducted to investigate the mechanism of epilepsy are based on EEGs or functional magnetic resonance imaging (fMRI) recorded during the ictal discharging or the resting-state, and few studies have probed into the dynamic patterns during the inter-ictal discharging that are much easier to record in clinical applications. Here, we propose a time-varying network analysis based on adaptive directed transfer function to uncover the dynamic brain network patterns during the inter-ictal discharging. In addition, an algorithm based on the time-varying outflow of information derived from the network analysis is developed to detect the epileptogenic zone. The analysis performed revealed the time-varying network patterns during different stages of inter-ictal discharging; the epileptogenic zone was activated prior to the discharge onset then worked as the source to propagate the activity to other brain regions. Consistence between the epileptogenic zones detected by our proposed approach and the actual epileptogenic zones proved that time-varying network analysis could not only reveal the underlying neural mechanism of epilepsy, but also function as a useful tool in detecting the epileptogenic zone based on the EEGs in the inter-ictal discharging.

Abstract:Electrophysiological recording and metabolic imaging are two complimentary techniques in preoperative evaluation of epilepsy surgery. High frequency electrical brain activity has been increasingly used in localizing seizure onset zone (SOZ) owing to the development of stereo-EEG (SEEG) technique. However, SEEG recording has no capacity of whole brain coverage as functional MRI. In this study, we performed cross-modal validation of epileptogenic network defined by SEEG and that by resting-state fMRI on individual brain of epilepsy patients. Epileptogenicity index (EI) based on abnormal high frequency SEEG signals was employed to define the epileptogenic network in electrophysiology. Meanwhile, taking the SEEG sites with highest EI as the seed, resting-state functional MRI and connectivity analysis were used to define the epileptogenic network in hemodynamics. The spatial consistency of these two networks was measured by ROC, and the average AUC reached 0.87, which proved a good consistency between electrophysiological and hemodynamic networks in epileptic brain. The difference part in resting-state fMRI network may supplement the SEEG electrode coverage, to reveal more information about epileptic network.
