Abstract:Skeletal class III malocclusion with severe skeletal disharmonies and arch discrepancies is usually treated via the conventional orthodontic-surgical approach. However, when associated with tooth impaction and periodontal risks, the treatment is more challenging and complex. The esthetic, occlusal, and periodontal stability of the treatment outcome is more difficult to obtain. The 16-year-old female patient in this case was diagnosed with dental and skeletal Class III malocclusion, bilateral impacted maxillary canines, and scalloped thin gingiva. The multidisciplinary management included a segmental arch technique, extracting two premolars, a subepithelial connective tissue graft surgery, and orthognathic surgery. The esthetic facial profile, pleasant smile, appropriate occlusion, and functional treatment results were obtained and maintained in 8-year follow-up.
Successful Multidisciplinary Management of an Endodontic-Periodontal Lesion Associated With a Palato-Radicular Groove: A Case Report

Abstract:Introduction: This case report describes the collaborative management of an extensive combined endodontic-periodontal lesion related to a long palato-radicular groove (PRG) on a maxillary lateral incisor. Cases with similar severity have been reported minimally in the endodontic journals but even less in the periodontal journals. This case report illustrates the result of multidisciplinary treatment of the combined lesions associated with PRG. Case presentation: A 63-year-old patient presented with a periapical radiolucency on tooth #10. After evaluation, the patient was diagnosed with an endodontic-periodontal lesion associated with PRG. After being informed of a guarded prognosis, the patient consented to a surgical procedure in an effort to retain the tooth. Management of the case involved a combination of endodontic therapy, odontoplasty under dental operating microscopy to attempt to eliminate the root anomaly, and periodontal regenerative procedures with allografts and a resorbable barrier membrane. Clinical examination and the cone-beam computed tomography scan at a 2-year postoperative visit revealed a substantial reduction in probing depth and significant bone fill of the defect. Conclusions: In the past, a long PRG in combination with a periapical lesion often resulted in extraction of the tooth. With accurate assessment of the etiology of the defect, patient education, and a multidisciplinary approach, teeth with a PRG may be retained with a stable outcome for years.

Abstract:A palatogingival groove of the maxillary lateral incisor is an anatomic malformation, which always predisposes the tooth to pulpal and periodontal disease. The diagnosis and treatment planning become complicated, with uncertain prognosis. Herein, we present an effective interdisciplinary management of a case of combined periodontal-endodontic lesions caused by palatogingival grooves. A series of treatment modalities were undertaken to preserve the two teeth, including root canal treatment, periodontal initial therapy, splinting the mobile teeth, occlusal adjustment, apical microsurgery, grinding and sealing grooves, and guided tissue regeneration. An apparent healing of the lesions was visible after 12 months. Therefore, interdisciplinary management of combined periodontal-endodontic lesions with palatogingival grooves of the maxillary lateral incisors is necessary for a favourable long-term outcome.

Abstract:BACKGROUND:Patients with severe periodontitis often require multidisciplinary treatment to achieve healthy periodontal tissue, normal occlusion, and optimal aesthetics. In the present study, we aimed to evaluate the efficacy of multidisciplinary non-surgical treatment in a patient with stage IV/ grade C periodontitis, malocclusion, and dentition defects.CASE SUMMARY:A 47-year-old woman visited our periodontology department due to teeth mobility problems and difficulties chewing food. The patient had no history of drug allergies or systemic disease. Initial therapy involved scaling and root planning with a Gracey scaler and periodontal ultrasonic therapeutic apparatus using a periodontal endoscope (Perioscopy, Zest Dental Solutions, United States) to control the periodontal inflammation prior to treatment. Five months later, orthodontic treatment was then performed to treat occlusion and overall aesthetics. After completion, a Maryland bridge was used to restore Nos. 22, 31, and 41 teeth. Florida probing (Florida probe, United States) was performed every 2-3 mo to evaluate the periodontal condition throughout treatment. Overall, multidisciplinary non-surgical treatment resulted in satisfactory aesthetic results with healthy periodontal tissue and stable occlusion.CONCLUSION:In some patients with stage IV/grade C periodontitis, systematic and sequential non-surgical treatment can provide excellent therapeutic results.
Successful Management of an Extensive Class V Carious Lesion With Dehiscence and Loss of the Attachment Apparatus: A Case Report With Six-Month Follow-Up

Abstract:The present case report presents a rare instance of a Class V carious lesion combined with alveolar dehiscence in a non-vital maxillary central incisor without gingival recession. A 25-year-old male exhibited symptoms, including a blackened tooth (#21), pus discharge, and tenderness, with clinical examination revealing a Class V carious lesion and partial dehiscence of the labial cortical plate, confirmed by radiographic and cone-beam computed tomography (CBCT) imaging. Treatment involved a two-phase approach: endodontic therapy with root canal instrumentation and obturation using a bioceramic sealer, followed by surgical intervention that included carious lesion removal, restoration with resin-modified glass ionomer cement (RMGIC), and application of a platelet-rich fibrin (PRF) membrane. PRF was selected for its regenerative properties, promoting bone healing and tissue repair, and was particularly beneficial in extensive lesions. Post-treatment follow-up at six months demonstrated complete soft tissue healing, reduced probing depth, and significant bone regeneration. This case illustrates the effective management of a complex dental condition through a combined endodontic and periodontal approach enhanced by PRF therapy, yielding favorable clinical and radiographic outcomes.
Clinical implications of large vegetations in infectious endocarditis.

Abstract:Thirty-four patients with clinical infectious endocarditis were examined by M-mode and two-dimensional (2D) echocardiography. Vegetations were identified in 16 patients (47%) by M-mode and in 27 patients (87%) by 2D echocardiography. Vegetations identified by 2D echocardiography were categorized as small (less than 5 mm), medium (5 to 9 mm), or large (greater than or equal to 10 mm). Large vegetations were caused by a variety of organisms, had a higher incidence of surgery (44% v 0%), and had no increased incidence of stroke or death. The larger the vegetation, the more detectable it was by M-mode. Aortic valve vegetations were associated with a higher incidence of congestive heart failure (CHF) (67% v 14%) and stroke (44% v 9%). Four patients with large aortic valve vegetations had the highest complication rate; CHF developed in all four, two had valve replacement, one had a stroke, and two died.

Abstract:Abstract Endodontic microsurgery (EMS) presents a great treatment option for unresolved periradicular pathologies, typically involving the removal of the apical root portion and adjacent inflamed tissues. This case report describes the management of persistent periapical lesions in the lower anterior teeth of a 32-year-old male. Root canal treatment was completed 4 years before presentation. The patient presented to the clinic with tooth mobility and mild discomfort and large radiographical periapical radiolucency. The case was diagnosed with previously treated with asymptomatic apical periodontitis. EMS was performed in conjunction with periodontal-guided tissue regeneration that includes bone graft and resorbable membrane and the teeth showed favorable resolution of the symptoms. Histopathological analysis showed the presence of a radicular cyst. After 3 years of follow-up, cone-beam computed tomography and periapical radiographs demonstrated substantial lesion regression and favorable outcome. This case report offering promising insights into the synergistic potential of combined endodontic and regenerative periodontal therapy in cases with unresolved periapical lesion with extensive bone destruction.

Sometimes the situation becomes more complicated by pulp necrosis and development of a combined endodontic-periodontal lesion which is a diagnostic and treatment challenge for the clinician. This report presents the successful management of an endodontic-periodontal lesion in a maxillary lateral incisor that has a developmental palatal radicular groove using a combination of nonsurgical endodontic therapy and periodontal regenerative techniques. Conclusion: The combination of nonsurgical endodontic and periodontal regenerative treatment is a predictable method in treating combined endodontic-periodontal lesions caused by palato-gingival groove.

Abstract:The complex intercommunication between the pulpal and periodontal tissues may result in the formation of endodontic periodontal lesions. The relationship between the pulpoperiapical and periodontal microflora explains the possibility of infection spreading between the root canal and the periodontal pocket. The associated tooth has periodontal involvement in the form of either pocket, clinical attachment loss, furcation loss, or other infrabony defects and also pulpoperiapical disease. The diagnosis of endodontic periodontal diseases can sometimes be challenging to make an accurate diagnosis. We report a case of a 34 years old female patient with an endo-perio lesion in the right lower second molar # 47 which was previously treated with endodontic therapy followed by prosthetic rehabilitation. We highlight the need for adequate endodontic management and the importance of maintaining proper prosthetic contour for the prevention of endodontic periodontal lesions. In our case, the endodontic periodontal was the result of inadequate endodontic treatment by the dental clinician. Hence with this case, we reiterate the importance of adequate and complete endodontic treatment and also the need to follow the tooth contours during prosthetic rehabilitation as a preventive protocol for endodontic periodontal lesions.
Multidisciplinary Management and Pulp Vitality Preservation of a Tooth With Extensive Iatrogenic Furcal Root Perforation and Biologic Width Violation

Abstract:This article describes the case of a vital molar tooth with a vast furcal iatrogenic root perforation and biologic width violation, which was successfully managed by a multidisciplinary approach aimed at preserving pulp vitality. The root perforation was cleaned and then sealed with mineral trioxide aggregate, which was positioned onto the pulp at the canal orifices. After one month, the patient was not reporting symptoms, and the tooth was positively responding to the thermal test. The tooth was orthodontically extruded, subjected to minimally invasive crown lengthening, and prepared to receive a full-crown restoration. Radiotransparent composite resin was chosen as a permanent restorative material to better monitor possible endodontic complications at the coronal level. The patient's tooth was followed up for eight years uneventfully. The present case is an example of the possibility to subject a root-repaired tooth with fully formed apices to conservative yet complex multidisciplinary treatment while maintaining pulp vitality.

Abstract:The objective of dentists is to preserve teeth affected by either caries, trauma, or any other pathological condition. Teeth with deep carious lesions often require endodontic treatment to preserve their form and function. Sound knowledge of root canal anatomy, endodontic pathology, and proper treatment protocol are vital in endodontic treatment success. Currently, there has been an ongoing trend of case reports that highlight the presence of extra canals, which in turn, cautions the clinician to be more prudent. Any missed canal during root canal therapy is the common cause of treatment failure. This case report emphasizes the fact that proper understanding of pulp anatomy combined with the use of modern diagnostic aids and proper treatment protocol is essential for treatment success. Abstract:A 32-year-old male patient presented with a chief complaint of ongoing endodontic treatment and swelling in the hard palate, specifically in the rugae region. On examination, a soft, non-tender, non-fluctuant swelling was observed between teeth #11 and #12, accompanied by radiographic evidence of periapical radiolucency and perforation. Cone beam computed tomography (CBCT) scans confirmed a well-defined radiolucency in the affected region, indicative of a radicular cyst. Root canal treatment was completed with mineral trioxide aggregate (MTA) obturation during the second visit. An apicoectomy was performed to remove the cystic content, followed by the placement of bone graft material, a platelet-rich fibrin (PRF) membrane, and sutures to facilitate bone regeneration. This comprehensive approach aimed to resolve the periapical pathology and promote tissue healing around the affected tooth.

Abstract:BACKGROUND Ameloblastoma is a locally aggressive, benign, odontogenic tumor. Reports suggest that the chances of recurrence of this tumor are high if treated with a conservative approach. Concordantly, surgical removal of the lesion along with the affected adjacent tissues and bone structure is recommended to reduce the chances of recurrence. Post-surgical prosthetic rehabilitation is advised to improve speech, mastication, and aesthetic appearance. This case report highlights the treatment and reconstruction challenges that maxillofacial surgeons and their teams face in managing cases of large ameloblastoma. CASE REPORT A 41-year-old Sudani man was referred for the management of a large ameloblastoma associated with the left border of the mandible. Management consisted of surgical removal of the affected mandible along with prosthetically preserving the mandible with grafts and screws. Histopathological, computed tomography, and incisional biopsy evaluation confirmed the presence of ameloblastoma. Postoperatively, no complications were reported. Six months postoperatively, no sign of recurrence was seen. The patient was referred to a surgeon for placement of an endosseous implant. CONCLUSIONS When dealing with large ameloblastoma, an interdisciplinary dental team is essential for improving the treatment results. This case highlights the importance of precise and timely primary care diagnosis and a collaborative approach to treatment. By embracing advancements in digital technologies, surgeons can enhance functional and aesthetic results, improving long-term quality of life.
Multidisciplinary Management Of Congenitally Missing Teeth With Osseointegrated Dental Implants: A Long-Term Report

Abstract:Objective: To describe a multidisciplinary approach to ensure optimal treatment and timing of treatment for these patients.Methods: Ten patients with congenital missing teeth were treated with dental implants with a multidisciplinary approach, including orthodontics, prosthodontics, and/or orthognathic surgery, from 1997 to 2006 in the Department of Implant Dentistry, Peking University Hospital of Stomatology. All patients were followed up over five years. Clinical and radiographic examinations were conducted* all the patients. A thorough diagnostic workup was used and the outline of the treatment planning was given from the prosthodontic point of viewResults: A total of 3l implants were placed and restored for the 10 patients involved. Followup averaged 108.4 months (61 155 months) after implant prosthetic functioning. With the multidisciplinary approach, a satisfactory treatment for these patients was acquired, with follow-up for more than 5 years. The per-implant marginal bone level was stable with hone resorption averaged 1.97 mm. Peri-implantitis was found in one patient with two implants and effectively treated with local anti-infection. Ceramic chips were observed in two cases and the crowns were re-lqbricated.Conclusion: Implant restoration with an interdisciplinary approach provides an alternative way with predictable clinical results for patients with congenitally missing teeth.
3-D diagnosis-assisted management of anomalous mandibular molar

Abstract:This case report describes the successful non-surgical endodontic management of carious exposed three-rooted mandibular molar with four root canals detected on the pre-operative radiograph taken with 20 degrees mesial angulation and confirmed with a 64-slice helical computed tomography scan-assisted 3-D-reconstructed images. Access cavity shape was modified to locate the extra canal with respect to the distolingual root in the left mandibular first molar. Copious irrigation was accomplished with 5.25% sodium hypochlorite and 17% EDTA. Biomechanical preparation was done using protapers. Calcium hydroxide dressing was done for 1 week. The tooth was obturated using gutta percha and AH 26 root canal sealer, and it was permanently restored with composite. Clinical examination on follow-up visits revealed no sensitivity to percussion and palpation in the left mandibular first molar. Thorough knowledge of root canal variations and use of advanced diagnostic modalities lead to successful non-surgical management of the complex cases.

Abstract:Growth of the R1 progeny of a tobacco plant (Nicotiana tabacum) transformed with an antisense gene to the small subunit of ribulose-1,5-carboxylase/oxygenase (Rubisco) was analyzed under 330 and 930 [mu]bar of CO2, at an irradiance of 1000 [mu]mol quanta m-2 s-1. Rubisco activity was reduced to 30 to 50% and 13 to 18% of that in the wild type when one and two copies of the antisense gene, respectively, were present in the genome, whereas null plants and wild-type plants had similar phenotypes. At 330 [mu]bar of CO2 all antisense plants were smaller than the wild type. There was no indication that Rubisco is present in excess in the wild type with respect to growth under high light. Raising ambient CO2 pressure to 930 [mu]bar caused plants with one copy of the DNA transferred from plasmid to plant genome to achieve the same size as the wild type at 330 [mu]bar, but plants with two copies remained smaller. Differences in final size were due mostly to early differences in relative rate of leaf area expansion (m2 m-2 d-1) or of biomass accumulation (g g-1 d-1): within less than 2 weeks after germination relative growth rates reached a steady-state value similar for all plants. Plants with greater carboxylation rates were characterized by a higher ratio of leaf carbon to leaf area, and at later stages, they were characterized also by a relatively greater allocation of structural and nonstructural carbon to roots versus leaves. However, these changes per se did not appear to be causing the long-term insensitivity of relative growth rates to variations in carboxylation rate. Nor was this insensitivity due to feedback inhibition of photosynthesis in leaves grown at high partial pressure of CO2 in the air (pa) or with high Rubisco activity, even when the amount of starch approached 40% of leaf dry weight. We propose that other intrinsic rate-limiting processes that are independent of carbohydrate supply were involved. Under plentiful nitrogen supply, reduction in the amount of nitrogen invested in Rubisco was more than compensated for by an increase in leaf nitrate. Nitrogen content of organic matter, excluding Rubisco, was unaffected by the antisense gene. In contrast, it was systematically lower at elevated pa than at normal pa. Combined with the positive effects of pa on growth, this resulted in the single-dose antisense plants growing as fast at 930 [mu]bar of CO2 as the wild-type plants at 330 [mu]bar of CO2 but at a lower organic nitrogen cost.

Abstract:Background: Palato-radicular groove (PRG) is defined as an anomalous formation of teeth. The etiology of PRG remains unclear. The prognosis of a tooth with a PRG is unfavorable. The treatment of combined periodontal-endodontic lesions requires multidisciplinary management to control the progression of bone defects. Some researchers reported cases that had short-term observations. The management of teeth with PRGs is of great clinical significance. However, to date, no case reports have been documented on the use of bone regeneration and prosthodontic treatment for PRGs. Case summary: This case reported the management of a 40-year-old male patient with the chief complaint of slight mobility and abscess in the upper right anterior tooth for 15 d and was diagnosed with type II PRG of tooth 12 with combined endodontic-periodontal lesions. The accumulation of plaque and calculus caused primary periodontitis and a secondary endodontic infection. A multidisciplinary management approach was designed that included root canal therapy, groove sealing, a periodontal regenerative procedure, and prosthodontic treatment. During a 2-year follow-up period, a good prognosis was observed. Conclusion: This report indicates that bone regeneration and prosthodontic treatment may contribute to the long-term favorable prognosis of teeth with PRGs.
Root canal treatment of a six-canal first mandibular molar with extensive periapical lesion: A case report

Abstract:Rationale: There is an increasing tendency for case reports to reveal anatomical aberrances in mandibular first molars, such as the lateral and accessory canals. Thus, clinicians should pay special attention to anatomic variances when dealing with mandibular first molars requiring endodontic treatment to prevent reinfection within the root canal system, which is responsible for the failure of root canal treatment. Patient concerns: This article presents the clinical report and successful root canal treatment of a 24-year-old healthy female patient with an extensive periapical lesion in a 6-canal first mandibular molar. The patient was admitted to the endodontic department because of a periapical abscess found 1 month ago in her left mandibular first molar. Diagnosis: Chronic apical periodontitis was diagnosed based on clinical examination coupled with radiographic and cone-beam computed tomography images. Interventions: The treatment plan was to first perform root canal therapy and then perform clinical observation. Outcomes: During 1-year follow-up period, the treated tooth was asymptomatic, and complete resolution of the extensive apical lesion was eventually achieved, as shown in the postoperative cone-beam computed tomography images and clinical examination. Lessons: The present case emphasizes the importance of a comprehensive understanding of root canal morphology, especially rare anatomical variations, to ensure successful root canal treatment. Additionally, the case report adds to the library of previously reported cases of extensive periapical lesions with a direct connection to the root canal system, which demonstrates the potential clinical advantages of root canal therapy as a conservative nonsurgical approach in these cases.
