Abstract:Background: In patients with chronic obstructive pulmonary disease (COPD), early detection of noninvasive ventilation (NIV) failure is a promising technique for decreasing mortality. The objective of this study was to evaluate the efficacy of heart rate, acidosis, consciousness, oxygenation, and respiratory rate (HACOR) score in predicting NIV outcome in COPD-associated respiratory failure. Methodology: A prospective observational study was conducted on 100 COPD patients with acute respiratory failure who were initiated on NIV. HACOR score was calculated at the start of NIV and after 1-2, 12, and 24 hours. NIV failure was defined as progression to invasive mechanical ventilation or death. NIV success was defined as liberation from NIV prior to hospital day seven and not meeting criteria for failure. Results: In this study, 100 patients with COPD and respiratory failure were enrolled. Their mean age was 65.34 years [standard deviation (SD) 8.19]. Male patients were predominant (n = 81). Eighty-nine percent of patients were smokers, and the remaining had exposure to biomass fuel. At the initiation of NIV, the median HACOR score was 3 (interquartile: 2, 4). In 13% of patients, there was NIV failure. There were 17 (17%) patients whose HACOR score at initiation was ≥5. In patients with a HACOR score ≥5, the NIV failure rate was 76.4% and mortality was 41.1%. The area under the curve (AUC) for prediction of NIV failure by HACOR score at initiation was 0.980 (p-value < 0.05). Conclusion: The HACOR score had high sensitivity as well as specificity at initiation in the prediction of NIV failure. A higher HACOR score predicts a high chance of NIV failure. Obtaining the HACOR score at the bedside makes it convenient for assessing the efficacy of NIV in patients with COPD.

Abstract:Background: Non-invasive ventilation (NIV) is a method of oxygenation supply that eliminates the need for an endotracheal airway. Non-invasive ventilation failure is defined as the necessity for endotracheal intubation or death during the NIV trial. Objectives: This study aimed to identify the predictors and associated factors of NIV failure in coronavirus disease 2019 (COVID-19) patients admitted to an intensive care unit (ICU). Methods: This retrospective, longitudinal cohort study utilized electronic medical records of COVID-19 patients admitted to the ICU. A total of 150 patients were included in the study. Patient demographics, medical history, laboratory tests, partial pressure of carbon dioxide (PCO2), oxygen saturation (SpO2), heart rate, acidosis, consciousness, oxygenation, and respiratory rate (HACOR score), and the ratio of oxygen saturation (ROX) index (the SpO2/fraction of inspired oxygen [FIO2] to respiratory rate [SF] ratio) were recorded. Non-invasive ventilation failure was determined based on the need for endotracheal intubation or cardiac-respiratory arrest while on NIV. Results: Of 150 patients, 55.3% were male (mean age: 55.9 years), with an NIV failure rate of 67.3%, a mortality rate of 66.7%, and 3.3% of patients requiring tracheostomy after NIV failure. The ROX index consistently decreased over time, and an increase in the HACOR score and PCO2 after 6 hours of commencing NIV were the predictors of NIV failure. Additionally, higher levels of lactate dehydrogenase, lower SF ratios, and higher APACHE scores upon ICU admission were significantly associated with NIV failure. Notably, the erythrocyte sedimentation rate (ESR) as an inflammatory index, SF ratio upon ICU admission, HACOR score, ROX index, and PCO2 after 12 hours were significant predictors of in-hospital mortality in patients receiving NIV. Conclusions: The ROX index, HACOR scale, and PCO2 are significant predictors of both NIV failure and in-hospital mortality.

Abstract:Aims: This study aimed to investigate the efficacy of NIV in patients with pneumonia-associated acute hypoxemic respiratory failure (AHRF) and to identify factors that predict NIV failure. Methods: A total of 50 very elderly patients (aged 85 and older) with pneumonia-associated AHRF were retrospectively evaluated from January 2021 to August 2022. Failure of NIV was defined was characterized by a transition to mechanical ventilation (either through tracheal intubation or tracheostomy), discharge under "hopeless" conditions while still on NIV, or death during NIV. Results: The mean age of the patients was 87.4±2.4 years, median SOFA score was 5, median duration of NIV was 4 days, and median length of stay in the ICU for the patients was 7 days. It was determined that 48% of the patients experienced NIV failure due to worsening clinical conditions. The SOFA score, levels of procalcitonin, CRP, heart rate, and respiratory rate were higher in the NIV failure group compared to the NIV successful group. An increased SOFA score (OR= 1.51, 95% CI= 1.11-2.06, p= 0.009), elevated CRP levels (OR= 1.03, 95% CI= 1.01-1.05, p= 0.003), and increased respiratory rate (OR= 1.18, 95% CI= 1.01-1.38, p= 0.039) were identified as independent predictors of NIV failure. Conclusion: In very elderly patients with AHRF due to pneumonia, the use of NIV may offer a significant survival benefit if patients are closely monitored and potential risk factors such as systemic inflammation parameters, SOFA score, and respiratory rate are considered.
Outcome of different modes of non-invasive ventilation in chronic obstructive pulmonary disease patients with type II respiratory failure

Abstract:Background: Chronic obstructive pulmonary disease (COPD) is a progressive inflammatory airway disorder often leading to type II respiratory failure (RF). Non-invasive ventilation (NIV) is crucial in COPD management. This study compares T (Timed), ST (Spontaneous-Timed), and iVAPS (Intelligent volume assured pressure support) modes. Method: A prospective, experimental, comparative study spanned two years. One hundred COPD patients with Type 2 RF were randomized into three groups: T mode (n=33), ST mode (n=33), and iVAPS mode (n=34). Outcome assessment included clinical, hematological parameters, and intubation rate. Result: The study revealed varying degrees of success among the three modes of NIV. iVAPS mode demonstrated the highest success rate, with 79.4% of cases achieving positive outcomes. Noteworthy improvements were observed in respiratory rate (RR) and oxygen saturation (SpO2) in the iVAPS group after 12 hours of NIV. Both the ST mode and iVAPS mode showed significant increases in pH levels, with a more pronounced improvement seen in the iVAPS group. Additionally, a substantial reduction in pCO2 levels after BiPAP was noted in the iVAPS group. The intubation rate was lowest in the iVAPS group, though the difference did not reach statistical significance. Conclusions: iVAPS mode demonstrated superior outcomes, including significant improvements in RR, SpO2, pH, and pCO2. While the intubation rate was lowest in the iVAPS group, statistical significance was not achieved. iVAPS emerges as a promising alternative, potentially averting the need for invasive ventilation. Larger, diverse studies are needed to validate these findings.
Patient Outcome after Chronic Obstructive Pulmonary Disease Exacerbations Requiring Non-invasive Ventilation during Hospitalization

Abstract:Introduction: Noninvasive ventilation (NIV) during hospitalization for acute hypercapnic exacerbations of chronic obstructive pulmonary disease (COPD) has been shown to be effective, but data on the prognosis of such patients is limited. The aim of this study was to investigate in-hospital and long-term outcome in patients with COPD exacerbations requiring NIV treatment during hospitalization. Methods: Between 2011 and 2013, hospitalized subjects with hypercapnic COPD exacerbations were included in this retrospective single-center cohort study. Subjects' clinical and laboratory data and survival status after a median of 27 months were recorded. The predictive factors of in-hospital and long-term mortality were analyzed. Results: A total of 574 patients (357 men, mean age 68±11 years) were recorded. During hospitalization, 24 (4.1%) patients died. In-hospital mortality was negatively affected by lower baseline values of hematocrit, albumin, and pH, and by higher baseline leucocytes and higher 24h PaCO2. Median survival of the cohort was 27 months. Mortality at 3 and 6 months, and 1 year were 14.5%, 19.5%, and 30%, respectively. In the univariate analysis, reduction in long-term survival was found to be related to older age, higher Charlson score, lower baseline levels of hematocrit and albumin, and lower pH level after 24h. In the multivariate analysis, older age and lower albumin were identified as the strongest predictors of mortality. Conclusion: Life expectancy after a COPD exacerbation requiring NIV treatment is short. Baseline blood counts and day 2 arterial blood gases levels may predict in-hospital mortality. The strongest indicators of poorer long-term outcome were advanced age and lower albumin. Such patients may need closer follow-up.

Abstract:Introduction: Patients with bronchiectasis can develop hypercapnic ventilatory failure in the later stages of their disease. The role of non-invasive positive pressure ventilation (NIV) in this context is unclear. There are benefits of domiciliary NIV on mortality and hospital admission in ventilatory failure due to chronic obstructive pulmonary disease (COPD). There is some evidence to suggest patients with COPD and non-CF bronchiectasis can have improvements in pCO 2 , exacerbation frequency and number of hospital admissions. We aimed to assess the benefit of NIV in hypercapnic respiratory failure in patients with bronchiectasis in a single centre cohort. Methods: A retrospective review of 42 patients with bronchiectasis as primary or secondary diagnoses initiated on domiciliary NIV took place. Data was collected on lung function, admissions, antibiotic use and respiratory microbiology, pre and post NIV. Results: Average age was 68 yrs, 52% males, 67% current or ex-smokers. Over a 6 year period, 8 (19%) patients had a reduction in the number of admissions to hospital after starting NIV. Seven had objective evidence of COPD, with an average FEV1 of 0.88. There was an overall 11% reduction in bacterial cultures following commencement of NIV (Haemophilus 26% reduction and Pneumococcus 9%). Conclusions: NIV may be useful in non-CF bronchiectasis to help with sputum clearance. This is likely to be most useful in patients with the severe airways disease and compromised respiratory muscle function as this usually results in chronic respiratory infection with reduced sputum clearance. A larger multi-centre cohort of predominantly bronchiectasis patients would allow further exploration.

Risk factors for non-invasive ventilation failure in influenza infection with acute respiratory failure in emergency department

Abstract:Objective: Non-invasive ventilation (NIV) has been widely used in hypoxemic acute respiratory failure (ARF) due to influenza pneumonia in the emergency department (ED). However, NIV used in influenza-associated acute respiratory failure had a variable rate of failure. Previous studies have reported that prolonged use of NIV was associated with increased mortality. Our study aimed to identify risk factors for NIV failure in influenza infection with acute respiratory failure in ED. Method: We performed a retrospective cohort observational study. Enrolled patients were older than 18 years who used NIV due to influenza infection with ARF between 1 January 2017 to 31 December 2018 in Ramathibodi Emergency Department. Patients characteristics, comorbidity, clinical, laboratory outcome, chest imaging, initial NIV setting, and parameters were collected in ED setting. Sequential organ failure assessment (SOFA) score and PaO2/FiO2 (PF) ratio were calculated from the first arterial blood gas in ED. We followed the outcome success or failure of the NIV used. Results: A total of 162 patients were enrolled and 72 (44%) suffered NIV failure in influenza infection with ARF. We used univariate and multivariate logistic analyses to assess risk factors for NIV failure. The ability of risk factor to predict NIV failure was analyzed using the area under the receiver operating characteristic (AUROC). Risk factors of NIV failure included SOFA score (P = 0.001), PF ratio (P = 0.001) and quadrant infiltrations in chest x-rays (CXR) (P = 0.001). SOFA score, PF ratio, and number quadrant infiltrations in chest radiography have good ability to predict NIV failure, AUROC 0.894 (95%CI 0.839-0.948), 0.828 (95%CI 0.764-0.892), and 0.792 (95%CI 0.721-0.863), respectively and no significant difference in the ability to predict NIV failure among three parameters. The use of PF ratio plus number quadrant infiltrations in chest radiography demonstrated a higher predictive ability for NIV failure in influenza infection with ARF. Conclusions: SOFA score, PF ratio, and quadrant infiltrations in chest radiography were good predictors of NIV failure in influenza infection with ARF.

Abstract:Objective: To study the effect of noninvasive positive-pressure ventilation (NIPPV) in treatment of chronic hypercapnic respiratory failure caused by chronic obstructive pulmonary disease,obesity-hypoventilation syndrome and bronchiectasis. Methods:From 2002 to 2005,there were 116 patients with obstructive pulmonary disorders ,19 patients with obesity-hypoventilation syndrome and 46 patients with bronchiectasis were treated in shanghai No . 6 Hopital because of hupercapic respiratory failure, their clinical data were analyzed respectively. All patients were not treated by NIPPV firstly in terms of chronic hyper-capnic respiratory failure; But at the second time these inpatients were treated by NIPPV. Given pulmonary function , arterial blood gas and calculated the hospitalization dates. Results: (1)Patients had the high probability of pursuing NIPPV, the compliance rate was high (compliance rate. 91 % ). (2)The date of arterial blood gas levels were improved. The arterial PaCO2 decrease from 62±10 mmHg to 53±6 mmHg in chronic obstructive pulmonary disease( P0.01);PaCO2 from 52±10 mmHg to 42±7 mmHg in obesity-hypoventilation syndrome( P0. 001); PaCO2 from 56±6 mmHg to 44±10 mmHg in bronchiectasis(P0.005). Meanwhile the arterial PaO2 increase from 61±13 mmHg,60±12 mmHg,58±11 mmHg to 71±15 mmHg,71±11 mmHg,75±18 mmHg in the three groups respectively( all P0.001). (3)Hospitalization dates decreased in all groups after initiating NIPPV, when compared with the year before NIPPV (P0.05). Conclusion: Noninvasive ventilation has partly effect on chronic obstructive pulmonary disease, obesity-hypoventilation syndrome and bronchiectasis .

Abstract:BACKGROUND:The objective of this study was to evaluate the association between noninvasive ventilation (NIV) compared with invasive ventilation and mortality in subjects with severe acute respiratory infection.METHODS:This was a retrospective multi-center study of subjects with severe acute respiratory infection treated with ventilatory support between September 2012 and June 2018. We compared the 90-d mortality of subjects managed initially with NIV (NIV group) with those managed with invasive ventilation only (invasive ventilation group), adjusting by propensity score.RESULTS:Of 383 subjects, 189 (49%) were in the NIV group and 194 (51%) were in the invasive ventilation group. Of the subjects initially treated with NIV, 117 (62%) were eventually intubated. Crude 90-d mortality was lower in the NIV group versus the invasive ventilation group (42 [22.2%] vs 77 [39.7%]; P < .001). After propensity score adjustment, NIV was associated with lower 90-d mortality than invasive ventilation (odds ratio 0.54, 95% CI 0.38-0.76; P < .001). The association of NIV with mortality compared with invasive ventilation was not different across the studied subgroups.CONCLUSIONS:In subjects with severe acute respiratory infection and acute respiratory failure, NIV was commonly used. NIV was associated with a lower 90-d mortality. The observed high failure rate suggests the need for further research to optimize patient selection and facilitate early recognition of NIV failure.

Incidence and causes of non-invasive mechanical ventilation failure after initial success

Abstract:Background: The rate of failure of non-invasive mechanical ventilation (NIMV) in patients with chronic obstructive pulmonary disease (COPD) with acute respiratory insufficiency ranges from 5% to 40%. Most of the studies report an incidence of "late failure" (after >48 hours of NIMV) of about 10-20%. The recognition of this subset of patients is critical because prolonged application of NIMV may unduly delay the time of intubation. Methods: In this multicentre study the primary aims were to assess the rate of "late NIMV failure" and possible associated predictive factors; secondary aims of the study were evaluation of the best ventilatory strategy in this subset of patients and their outcomes in and out of hospital. The study was performed in two respiratory intensive care units (ICUs) on patients with COPD admitted with an episode of hypercapnic respiratory failure (mean (SD) pH 7.23 (0.07), PaCO(2) 85.3 (15.8) mm Hg). Results: One hundred and thirty seven patients initially responded to NIMV in terms of objective (arterial blood gas tensions) and subjective improvement. After 8.4 (2.8) days of NIMV 31 patients (23%; 95% confidence interval (CI) 18 to 33) experienced a new episode of acute respiratory failure while still ventilated. The occurrence of "late NIMV failure" was significantly associated with functional limitations (ADL scale) before admission to the respiratory ICU, the presence of medical complications (particularly hyperglycaemia), and a lower pH on admission. Depending on their willingness or not to be intubated, the patients received invasive ventilation (n=19) or "more aggressive" (more hours/day) NIMV (n=12). Eleven (92%) of those in this latter subgroup died while in the respiratory ICU compared with 10 (53%) of the patients receiving invasive ventilation. The overall 90 day mortality was 21% and, after discharge from hospital, was similar in the "late NIMV failure" group and in patients who did not experience a second episode of acute respiratory failure. Conclusions: The chance of COPD patients with acute respiratory failure having a second episode of acute respiratory failure after an initial (first 48 hours) successful response to NIMV is about 20%. This event is more likely to occur in patients with more severe functional and clinical disease who have more complications at the time of admission to the ICU. These patients have a very poor in-hospital prognosis, especially if NIMV is continued rather than prompt initiation of invasive ventilation.

Abstract:Background Not all hypercapnic COPD patients benefit from home noninvasive ventilation (NIV), and mechanisms through which NIV improves clinical outcomes remain uncertain. We aimed to identify "responders" to home NIV, denoted by a beneficial effect of NIV on arterial partial pressure of carbon dioxide ( P aCO 2 ), health-related quality of life (HRQoL) and survival, and investigated whether NIV achieves its beneficial effect through an improved P aCO 2 . Methods We used individual patient data from previous published trials collated for a systematic review. Linear mixed-effect models were conducted to compare the effect of NIV on P aCO 2 , HRQoL and survival, within subgroups defined by patient and treatment characteristics. Secondly, we conducted a causal mediation analysis to investigate whether the effect of NIV is mediated by a change in P aCO 2 . Findings Data of 1142 participants from 16 studies were used. Participants treated with lower pressure support (<14 versus ≥14 cmH 2 O) and with lower adherence (<5 versus ≥5 h·day –1 ) had less improvement in P aCO 2 (mean difference (MD) –0.30 kPa, p<0.001 and –0.29 kPa, p<0.001, respectively) and HRQoL (standardised MD 0.10, p=0.002 and 0.11, p=0.02, respectively), but this effect did not persist to survival. P aCO 2 improved more in patients with severe dyspnoea (MD –0.30, p=0.02), and HRQoL improved only in participants with fewer than three exacerbations (standardised MD 0.52, p=0.03). The results of the mediation analysis showed that the effect on HRQoL is mediated partially (23%) by a change in P aCO 2 . Interpretation With greater pressure support and better daily NIV usage, a larger improvement in P aCO 2 and HRQoL is achieved. Importantly, we demonstrated that the beneficial effect of home NIV on HRQoL is only partially mediated through a reduction in diurnal P aCO 2 .

Abstract:Abstract Background The utilization of noninvasive positive pressure ventilation (NPPV) is becoming more and more common, especially in patients with acute or chronic respiratory failure. The purpose of our study is to analyze the factors that influence the efficacy of NPPV in the treatment of respiratory failure caused by a variety of etiology. Methods From May 2011 to April 2020, patients treated with NPPV during hospitalization in the First Affiliated Hospital of Soochow University were enrolled. According to the clinical outcome of NPPV treatment and whether converted to invasive mechanical ventilation, patients were divided into the success group and the failure group. The clinical data and the characteristics of NPPV application were compared between the two groups. Results A total of 3312 patients were enrolled, including 2025 patients in the success group and 1287 patients in the failure group. Univariate analysis suggested that there were no statistical differences in patients' age, gender, use of analgesia and/or sedation, complicated with barotrauma, inspiratory positive airway pressure and expiratory positive airway pressure between the success and failure groups ( P > 0.05). However, there were statistically significant differences in serum albumin levels, Ca 2+ concentration, blood glucose levels, duration of NPPV treatment and length of hospital stay between the success and failure groups ( P < 0.05). Multivariate logistic regression analysis indicated that serum albumin levels and duration of NPPV treatment had statistical significance on the therapeutic effect of NPPV ( P < 0.05). Conclusion Serum albumin levels and duration of NPPV treatment were independent risk factors for the efficacy of NPPV treatment in respiratory failure.

Abstract:Objectives: To determine the clinical profile and outcome of children requiring noninvasive ventilation (NIV) in a PICU. Methods: This prospective observational study was conducted in the PICU at Dayanand Medical College and Hospital, Punjab. Children (1 mo-18 y) with moderate-to-severe respiratory distress who received NIV during one-year period were included. Failure was defined as the need for endotracheal intubation. The patients received bilevel positive airway pressure (BiPAP) with inspiratory and expiratory positive airway pressure (8-18 cm H2O and 4-8 cm), respectively and indigenous continuous positive airway pressure (CPAP) were included. Vital signs (heart rate, respiratory rate) and gasometric parameters (pH, HCO3, pCO2, pO2) were recorded. Results: Out of total 115 patients, 81.7% were successfully treated by NIV whereas 18.3% constituted NIV failure group. Two types of NIV were used, 65.2% were started on BiPAP and 34.8% on indigenous bubble CPAP. Most common diagnosis was tropical fever (24.3%), bronchopneumonia (20%), and sepsis with multiple organ dysfunction syndrome (MODS) (7.8%). Commonest indication of NIV was respiratory distress (70.4%) and prevention of postextubation respiratory failure (20.8%). Seven patients (6.9%) died during the study. NIV failure is higher in children with sepsis with MODS, abnormal blood gas (acidosis), and moderate-to-severe acute respiratory distress syndrome (ARDS). Conclusions: This study demonstrates that NIV is an effective form of respiratory support for children with acute respiratory distress/failure. Sepsis with MODS, acidosis and ARDS (moderate to severe) were predictors of NIV failure. Careful patient selection may help in judicious use of NIV in PICU.

Abstract:The sequential use of non-invasive ventilation (NIV) for weaning in hypercapnic respiratory failure patients is a recommended practice. However, the effectiveness of weaning on high-flow nasal cannula (HFNC) is unclear. Chronic obstructive pulmonary disease patients with hypercapnic respiratory failure who received invasive ventilation were screened for enrollment. This study was a single-center, prospective, randomized comparative study. The primary outcome was treatment failure within 72 hours after extubation. Patients who were screened positive for extubation were enrolled in the study and randomized into the HFNC group and the NIV group using a computer-generated simple randomization chart. Treatment failure was defined as a return to invasive mechanical ventilation or a switch in respiratory support modality (i.e., changing from HFNC to NIV or from NIV to HFNC). The study included 62 of the 72 patients. Treatment failure occurred in 8 patients (26.67%) in the HFNC group and 8 patients in the NIV group (25%) (p=0.881). The mean duration of intensive care unit stay in the HFNC group was 5.47±2.26 days and 6.56±3.39 in the NIV group (p=0.376). In the current study, HFNC was non-inferior to NIV in preventing post-extubation respiratory failure in chronic obstructive pulmonary disease patients, while HFNC had better treatment tolerance.
Dose-response characteristics of noninvasive ventilation in acute respiratory failure

Abstract:Acute noninvasive ventilation (NIV) is a well-established therapy for acute respiratory failure but the dose-response characteristics of this therapy have not been defined. The aim of this study was to define this dose-response relationship. This study was a retrospective review of patients receiving NIV for acute respiratory failure in a tertiary hospital respiratory high-dependency unit between July 2012 and June 2017. Mask-on time (rather than the period that NIV was in use) as the "dose" was compared with hospital survival as the "response". 654 patients were included, 594 (91%) with hypercapnic respiratory failure (HCRF). NIV was used for a median (interquartile range (IQR)) duration of 2.74 (1.51-4.73) days and median (IQR) mask-on time was 34 (18-60) h (56.1% (41.2-69.5%) of treatment time). There was evidence of a dose-response relationship in the HCRF group up to a ceiling of 24 h mask-on time, but not in the hypoxaemic respiratory failure (HRF) group. There was a difference in survival with as little as 2 h mask-on time (92% compared with 73%; p<0.001). Patients requiring NIV for 80-100% of therapy time had lower survival. We conclude that there is evidence of a dose-response relationship between cumulative NIV usage (mask-on time) and survival from as little as 2 h to a ceiling of ∼24 h in HCRF, but not in HRF.

Abstract:OBJECTIVE The Quebec Plan d'action en santé mentale (PASM) (Mental Health Action Plan) reform, a major transformation of the province's mental health care system, has put primary care rather than hospital-based care at the forefront of mental health service delivery. This study documents perceptions of changes in child and youth mental health (CYMH) services following the reform, as well as facilitators and obstacles to collaboration and partnership in CYMH services, and the specific challenges related to collaboration and partnership when servicing multi-ethnic populations. METHODS This qualitative participatory research study collected data using semi-structured individual interviews, focus groups and participant observation in community-based health and social service institutions. Thematic analysis was performed. RESULTS The reform process encountered challenges in building a common culture of care within and between institutions, while collaboration and partnership evolved in a positive direction throughout the study. Study results highlighted the importance of fostering communication at all levels. Collaboration and partnership was facilitated by opportunities for clinical discussions, dialogue on models of care, harmonizing administrative and clinical priorities, and involving key actors and structures. The results revealed difficulties in implementing multidisciplinary work and in negotiating partners' responsibilities. Quality of partnership and collaboration appeared particularly crucial in providing optimal care to vulnerable families, including migrants. CONCLUSION The PASM reform involved a major and challenging transformation in CYMH services. Continuous dialogue through time and leadership sharing appeared promising to foster this transformation.
