Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 5, May 2024. Background:Nearly 8 million high school students in the United States participate in sports each year. With the lack of recent population data, an update to previous studies on US high school athletes is needed.Purpose:To update the epidemiology of sports injuries in high school athletes in the United States.Study Design:Descriptive epidemiology study.Methods:The data from the National Health School Sports-Related Injury Surveillance Study, including 100 nationally representative high schools, were obtained for 9 high school sports (boys' football, soccer, basketball, wrestling, and baseball and girls' soccer, basketball, volleyball, and softball) between the 2015 and 2019 academic years. Injury rates were calculated as the ratio of injuries per 1000 athlete exposures (AEs), defined as 1 athlete participating in 1 practice or competition. Data on injured body area, injury type (sprains/strains, concussions, contusions, and fractures), time loss, and need for surgery were also obtained. Rate ratios (RRs) with 95% CIs and P values were calculated.Results:Athletic trainers reported 15,531 injuries during 6,778,209 AEs, with an overall rate of 2.29 injuries per 1000 AEs. Injury rates were highest in football (3.96), girls' soccer (2.65), and boys' wrestling (2.36). The overall injury rate was lower in girls' sports (1.86) compared with boys' sports (2.52) (RR, 0.74 [95% CI, 0.71-0.76]; P < .001) and was higher in competition compared with practice (RR, 3.39 [95% CI, 3.28-3.49]; P < .001). The most commonly injured body areas were the head/face (24.2%), ankle (17.6%), and knee (14.1%). Sprains/strains (36.8%) and concussions (21.6%) were the most common diagnoses. Overall, 39.2% and 34% of injuries resulted in a time loss of <1 week and 1 to 3 weeks, respectively. Surgery was required in 6.3% of injuries, with wrestling (9.6%), girls' basketball (7.6%), and boys' baseball (7.4%) being the sports with the highest proportion of injuries needing surgery.Conclusion:Study findings demonstrated that boys' football, girls' soccer, and boys' wrestling had the highest injury rates, with boys' sports overall having higher injury rates than girls' sports. Sprains/strains and concussions were the most common diagnoses. Few injuries required surgery.

Abstract:Sir, We were presented with a baby born to a mother taking sodium valproate who wished to breast feed. There are no data on breast milk levels of sodium valproate; it is presumed from collateral evidence that some of the drug would pass across from maternal serum into the milk. Studies in animals given modest doses of sodium valproate during pregnancy (30-90 mg/kg) showed no significant teratogenic effects. When a much higher dose was used (about 600 mg/kg) there was a dose-related increase in the rate of intrauterine death and major fetal abnormalities, most of which involved the kidneys or the lower lumbar spine (Whittle, 1976). Information on the outcome of the pregnancies where sodium valproate has been taken as the main anticonvulsant is very scanty; in most reports two or more drugs have been used or, in the case of an abnormality, the family history has suggested a strong genetic element (W. Reckitt-Labaz, 1978, personal communication). Case 1, a 22-year-old primipara, was epileptic and changed to sodium valproate in November 1975. She was well controlled taking 1600 mg/day in divided doses. She had had irregular periods and did not realise that she was pregnant until she was about 14 weeks. She consequently booked at the hospital clinic at about 18 weeks. Her anticonvulsant drug was not changed. She was admitted in labour at term in July 1978 and delivered a healthy boy weighing 3010 g; Apgar score was 9 at one minute, and no abnormalities were observed. The baby breast fed well and was discharged home on the 6th day, slightly under his birthweight. Mother and baby were seen at 29 days after delivery. The baby was still breast feeding well, gaining weight, and beginning to smile. The sodium valproate level was measured in the mother's serum, as shown in the Figure. The level fell slightly at term without any change in the oral dose and then rose to the previous level by 29 days. The valproate level in the serum of the neonate was of the same order as that of the mother's at delivery, but fell to insignificant levels by 5 days and was undetectable at 29 days (Figure). The mean level of valproate in breast milk at 5 days was 50 mmol/l; it had fallen to 21 mmol/l by 29 days. The neonatal valproate level indicates that valproate crosses the placenta freely and maternal serum levels will reflect fetal serum levels. Sodium valproate crosses through from serum into breast milk and will be found at a level between 5 and 10% ofthat in the mother's serum. The absorption of valproate from breast milk by the neonate appears to be insignificant. In this instance there was no evidence of any abnormality associated with the use of this drug. It may be concluded that breast feeding while on theraneutic doses of sodium valproate is safe and will not Valproate

Abstract:Context: The advent of Web-based sports injury surveillance via programs such as the High School Reporting Information Online system and the National Collegiate Athletic Association Injury Surveillance Program has aided the acquisition of girls' and women's basketball injury data. Objective: To describe the epidemiology of injuries sustained in high school girls' basketball in the 2005-2006 through 2013-2014 academic years and collegiate women's basketball in the 2004-2005 through 2013-2014 academic years using Web-based sports injury surveillance. Design: Descriptive epidemiology study. Setting: Online injury surveillance from basketball teams in high school girls (annual average = 100) and collegiate women (annual average = 57). Patients or other participants: Girls' and women's basketball players who participated in practices and competitions during the 2005-2006 through 2013-2014 academic years in high school or the 2004-2005 through 2013-2014 academic years in college. Main outcome measure(s): Certified athletic trainers collected time-loss (≥24 hours) injury and exposure data. Injury rates per 1000 athlete-exposures (AEs) were calculated. Injury rate ratios (IRRs) with 95% confidence intervals (CIs) were used to compare injury rates by school size or division, time in season, event type, and competition level. Results: The High School Reporting Information Online system documented 2930 time-loss injuries during 1 609 733 AEs; the National Collegiate Athletic Association Injury Surveillance Program documented 3887 time-loss injuries during 783 600 AEs. The injury rate was higher in college than in high school (4.96 versus 1.82/1000 AEs; IRR = 2.73; 95% CI = 2.60, 2.86). The injury rate was higher in competitions than in practices for both high school (IRR = 3.03; 95% CI = 2.82, 3.26) and collegiate (IRR = 1.99; 95% CI = 1.86, 2.12) players. The most common injuries at both levels were ligament sprains, concussions, and muscle/tendon strains; the majority of injuries affected the ankle, knee, and head/face. These injuries were often caused by contact with another player or a noncontact mechanism. Conclusions: Injury rates were higher in collegiate than in high school athletes and in competitions than in practices. Similarities in distributions of injuries by body parts, specific diagnoses, and mechanisms of injury suggest that both levels may benefit from similar injury-prevention strategies.

Abstract:TOTHEEDITOR: Calcium-channel antagonists have been proposed as agents to prevent neuronal damage in ischemic stroke.' One study has shown that early treatment with nimodipine is effective in ischemic stroke.! However, follow-up studies have failed to substantiate these findings.' Nimodipineis effective in delayed cerebral ischemiafollowing subarachnoid hemorrhage when given prior to the ischemic event.' To see if calcium-channel antagonists might be effective when given prior to stroke, the followingretrospective study was done. Using InternationalClassification of Diseases, 9th Revision, Clinical Modification codes for ischemic stroke, a retrospective study of all patients with this diagnosis admitted in the first six months of 1991 was done. Patients were divided into two groups. Group I patients (n= 17) were receiving a calcium-channel antagonist for another medical indication when they were admitted for a stroke.Group II patients (n=29) were not receivinga calcium-channel antagonist on admission for stroke. The Barthel index and PULSE scores were evaluated on days 3 and 7 after stroke and at discharge.' Length of stay and discharge destination were also compared. Group I patients were receiving diltiazem (n=8), nifedipine (n=7), and verapamil (n=2). The mean ±SO length of stay was significantly shorter for group I patients (group I 8.5 ± 3.5 d; group II 20.7 ± 18.3 d; p=O.Ol). Significant differences in Barthel and PULSE scores were noted at days 3 and 7, with group I patients demonstrating better neurologic function. No difference was seen in discharge scores. There was a greater percentageof men among group I patients, but an analysis of covariance did not indicate gender to be a factor. In addition, significantly greater numbers in group I were discharged to home (group I 58 percent; group II 32 percent) and fewer group I patients required discharge to long-termcare (group I 6 percent;group II 24 percent). As a follow-up to this study, 89 patients with a diagnosis of stroke from North Dakota Medicaid records were reviewed.Twelve patientsreceived a calcium-channel antagonist prior to the ischemic stroke. Calcium-channel antagonist use patterns were similar to that of the previous study.The lengthof stay was different between the two groups (calciumchannel antagonist lOA ± 7.9 d; no calcium-channel antagonist 15.6 ± 16.2d; p=O.088), but did not reach statisticalsignificance. Although limited, these data appear to indicate that pretreatment with a calcium-channelantagonist may be associated with better outcomes in ischemic stroke. Since these studies are small and retrospective in design, controlled, prospective trials should be initiated to further evaluate this potentialeffect of calcium-channelantagonists.
Epidemiology of American Football-Related Fractures in the United States 2002-2021

Abstract:Background: American football is the most popular sport in the United States, with over 5.6 million people >6 years old playing the sport. Fractures in American football athletes are significant, as they compromise an athlete's performance and can also lead to prolonged recovery periods, affecting team dynamics and player careers. Analyzing these injuries is critical to evaluate preventive measures and tailor rehabilitation strategies to ensure the well-being and sustained peak performance of football athletes on and off the field. Purpose: To analyze the trend of American football fractures by body site, sex, and age in amateur athletes over a 20-year period between 2002 and 2021. Study design: Descriptive epidemiology study. Methods: The National Electronic Injury Surveillance System (NEISS) database was queried to characterize football-related fractures and injuries from 2002 to 2021 in patients aged 0-99 years old. National injury estimates were calculated using sample weights. Chi-square analysis and one-way ANOVA were performed to compare categorical variables. Results: Of 56,809 cases of American football-related fractures over 20 years, patients aged 10 to 14 years had the highest incidence of fractures, composing 41.2% of all fractures (n = 23,389), and patients aged <18 years represented 88.8% of all fractures (n = 50,457). The median age of players when fractures occurred was 13 years. Upper extremity fractures (n = 41,863 [73.7%]) were the most common fracture reported compared with lower extremity and head, neck, and trunk fractures. Among upper extremity fractures, finger fractures had the highest prevalence (35.6%), followed by lower arm fractures (19.9%) and wrist fractures (13%). While most cases resulted in release after treatment (93.1%), 5.9% resulted in treatment and hospitalization. When analyzing trends in American football-related fractures over time, we observed the largest number of fractures in 2006 (n = 3664), while the smallest number of fractures occurred in 2020 (n = 1313). Also, a downward trend was found in American football-related fractures since 2006, with a mean of 101 fewer fractures each year. Conclusion: Our analysis showed that American football-related fractures diagnosed in the emergency department in the United States were most likely to occur in pediatric patients (10-18 years old). In addition, patients experiencing American football-related fractures were most likely to have an upper extremity fracture and not require hospitalization. The trend in American football-related fractures has been decreasing since 2006.

Abstract:Background: Although data exist on injuries in youth football leagues, there are limited recent data on injury incidence in middle school football. Updated injury incidence estimates can help drive the development of injury prevention strategies. Purpose: Describe the epidemiology of injuries in middle school football during school years 2015-2016 to 2017-2018. Study Design: Descriptive epidemiology study. Methods: Data originated from 9 public middle schools in Virginia during school years 2015-2016 to 2017-2018. Certified athletic trainers collected injury and athlete-exposure (AE) data from school-sanctioned games and practices in boys’, football. Injury counts and rates per 1000 AEs were calculated. Injury rate ratios with 95% CIs compared rates between games and practices. Results: Overall, 664 middle school boys’, football injuries were reported, leading to an overall injury rate of 20.54 per 1000 AEs (95% CI, 18.98-22.11). The time loss injury rate (inclusive of injuries with participation restriction time ≥24 hours) was 9.28 per 1000 AEs (95% CI, 8.23-10.33). The injury rate was higher in competition than practice (36.19 vs 17.97 per 1000 AEs; injury rate ratio, 2.01; 95% CI, 1.69-2.40). Most injuries were to the head/face (competition, 20.6%; practice, 15.8%) and hand/wrist (competition, 18.8%; practice, 16.4%) and were diagnosed as contusions (competition, 30.9%; practice, 25.9%) and sprains (competition, 19.4%; practice, 12.6%). Competitions also had a large proportion of concussions (10.3%). Overall, 80.0% and 66.9% of injuries were due to contact in competition and practice, respectively; of these contact-related injuries, 62.1% and 41.6% were specifically player contact. Conclusion: Injury distributions parallel those found in previous research from middle school and other sport settings. Injury rates in middle school football were higher than those reported in previous findings in high school and college. However, caution must be taken when interpreting findings in relation to other surveillance systems with varying methodologies. Still, the findings highlight the need for injury prevention strategies within middle school football, particularly as related to contact-related mechanisms.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Recent evidence suggests that for completely displaced midshaft clavicular fractures, surgery offers no clear benefit over nonoperative treatment in a general adolescent population from 10 to 18 years of age. However, the comparative outcomes of comminuted and/or severely shortened clavicular fractures specifically in older adolescent athletes have not been explored in a focused, methodologically rigorous fashion.Hypothesis:The study hypothesis was that outcomes would be superior in older adolescent athletes who underwent operative treatment compared with nonoperative treatment for comminuted and/or severely shortened clavicular fractures.Study Design:Cohort study; Level of evidence, 2.Methods:A level 2, multicenter, prospective cohort study investigating the outcomes of midshaft fractures in adolescents between 2013 and 2017 was filtered to analyze the subcohorts of athletes 14 to 18 years of age with either fracture comminution or fracture shortening of ≥25 mm or both. Patient characteristics, injury mechanisms, fracture characteristics, and treatments were compared. Complications, rates, timing of return to sports (RTS), and patient-reported outcomes (PROs) were analyzed.Results:The 2 treatment groups, which included 136 older adolescent athletes (69 nonoperative, 67 operative), showed similar distributions of primary sport type, competition level, comminution, shortening, and 2-year PRO response rate (n = 99; 73%). The operative group demonstrated 3 mm–greater mean superior displacement, which was therefore statistically controlled for as a confounder in the comparative PRO analysis. No 2-year differences in nonunion, delayed union, symptomatic malunion, refracture, clinically significant complications, or rates of RTS were detected between treatment groups. The difference in timing of RTS (operative, 10.3 weeks; nonoperative, 13.5 weeks) was statistically significant. After controlling for the minor difference in superior displacement, regression analysis and matched comparison cohorts demonstrated no differences between the nonoperative and operative groups in mean or dichotomized PRO scores.Conclusion:In this prospective, multicenter cohort study investigating older adolescent athletes with comminuted and/or severely shortened clavicular fractures, contrary to the study hypothesis, there were no differences in complications, RTS, or PROs between nonoperatively and operatively treated patients at 2 years. Comparably excellent outcomes of severe clavicular fractures in adolescent athletes can be achieved with nonoperative treatment.

Comparison of Volleyball-Related Injuries at US Emergency Departments Between High School and Collegiate Athletes: A 10-Year NEISS Database Study

Abstract:Background: There is a paucity of current data regarding the sport-specific injury patterns and epidemiological trends associated with volleyball. Purpose: To provide an updated, comparative assessment of the epidemiology of volleyball-related injuries among female high school- and college-aged athletes and to characterize the burden of these injuries on emergency departments (EDs) across the United States. Study design: Descriptive epidemiological study. Methods: Data were obtained from the National Electronic Injury Surveillance System for volleyball-related injuries between 2012 and 2021 in high school-aged (14-18 years) and college-aged (19-23 years) patients. Incidence, injury characteristics, incident locales, and dispositions were analyzed with weighted population statistics based on National Federation of State High School Associations and National Collegiate Athletic Association data as well as incidence rates and incidence rate ratios (IRRs) and 95% CIs. Results: In total, an estimated 214,302 female athletes aged 14 to 23 years were evaluated in EDs across the United States with volleyball-related injuries between 2012 and 2021. Female college-aged athletes were nearly 3 times more likely to be evaluated with these injuries than their high school-aged counterparts, with incidence rates of 12.8 per 100 at-risk individuals among college-aged athletes and 4.3 per 100 at-risk individuals in high school-aged athletes (IRR, 0.338; 95% CI, 0.333-0.342). The ankle, head, and knee were most frequently injured, often involving strains/sprains, contusions, fractures, and concussions. The IRRs of nearly all injuries were higher among collegiate athletes, especially among knee (IRR, 4.56; 95% CI, 4.40-4.72) and shoulder (IRR, 5.07; 95% CI, 4.81-5.35) injuries. Conclusion: Among volleyball-related injuries evaluated in EDs between 2012 and 2021, the incidence rates of injuries in college-aged athletes far surpassed those of their high school-aged peers irrespective of injury type or bodily location. While sprains and strains were the most frequent injuries, head injuries accounted for the second most common diagnosis in both groups, suggesting that clinicians should maintain a high index of suspicion for concussion when evaluating players.

Abstract:Chronic/persistent pain due to osteoarthritis is one of the most common pain conditions affecting Americans today. Inadequate pain relief or dissatisfaction with current treatments is a source of frustration and suffering for patients with chronic/persistent pain. The requirement of multiple doses of commonly used analgesics to maintain adequate pain relief is inconvenient for many patients with osteoarthritis-related chronic pain. The several extended-release opioid analgesics that have been developed may provide an opportunity for improved patient convenience; however, clinicians must consider adverse event profiles, pharmacokinetics, abuse potential, and controlled substance-scheduling status of extended-release opioid analgesics. The purpose of this review is to highlight the efficacy and safety of extended-release opioid analgesics utilized in the management plan of chronic pain due to osteoarthritis.

Abstract:Background Injury characteristics of high school track and field throwing athletes in the United States are not well studied. Understanding epidemiology of injuries is important to identify treatment and prevention strategies. Objective To describe injury rates and patterns in U.S. high school track and field throwing events from a longitudinal national sports injury surveillance system. Design Descriptive epidemiology study. Setting Data were provided by the National High School Sports Related Injury Surveillance System, High School RIO (Reporting Information Online). Methods Athletic trainers reported injury and exposure data through the High School RIO website on a weekly basis. An athlete exposure (AE) was defined as one athlete participating in one school‐sanctioned practice or competition. Throwing events of discus, shot put, and javelin were analyzed in this study. Main Outcome Measures Injury rate, rate ratios (RR), injury proportion ratios (IPR). Participants U.S. high school athletes. Results A total of 267 track and field throwing injuries occurred during 5,486,279 AEs. Overall, the rate of injuries in competition was higher than in practice (RR 1.35, 95% confidence interval [CI] 1.01–1.80). In practice, the rate of injuries was higher for girls than boys (RR 1.53, 95% CI 1.12–2.08). The most frequently injured body part was the shoulder (21.7%), followed by the ankle (16.5%) and knee (12.0%). The most common types of injury were muscle strains (26.14%) and ligament sprains (25%). Recurrent injuries accounted for a higher proportion of chronic injuries compared to new injuries (IPR 1.85, 95% CI 1.16–2.97). Conclusion This study described injury characteristics of high school track and field throwing athletes from 2008 to 2019. Based on our results, injury prevention may be particularly important for female throwers with prior injury.

Background:The exact incidence of shoulder dislocation in the general population of the United States (US) has yet to be well studied.Purpose:To establish the current incidence and patterns of shoulder dislocations in the US, especially regarding sports-related activity.Study Design:Descriptive epidemiology study.Methods:This was a retrospective analysis of shoulder dislocations encountered in emergency departments in the US between 1997 and 2021 as recorded in the National Electronic Injury Surveillance System (NEISS). Data were further analyzed according to patient age, sex, and sports participation. Information from the United States Census Bureau was used to determine the overall incidence of dislocations.Results:A total of 46,855 shoulder dislocations were identified in the NEISS database, representing a national estimate of 1,915,975 dislocations (mean 25.2 per 100,000 person-years). The mean patient age was 35.3 years. More than half of the dislocations (52.5%) were sports-related, and basketball (16.4%), American football (15.6%), and cycling (9%) were the sports most commonly associated with dislocation. Most dislocations (72.1%) occurred in men. This disparity by sex was more significant for sports-related dislocations (86.1% in men) than nonsports-related dislocations (56.7% in men; P < .001). With sports-related dislocations, people 39 years (44.6% vs 14.9%; P < .001), while the opposite distribution was seen with nonsports-related dislocations ( 39 years: 51.7%; P 61 years.Conclusion:Sports-related shoulder dislocations were more common among younger and male individuals than older and female individuals. Contact sports such as basketball and American football were associated with more shoulder dislocations compared with noncontact sports.

Abstract:IN a previous paper on the influence of certain salts upon the act of clotting' a fluid capable of spontaneous coagulation was selected, and to this fluid salts in varying proportions were added. In this way it was sought to demonstrate the important part played by salts as agents favouring or retarding coagulation, and also that more or less antagonism or interference resulted from the simultaneous admixture of certain of these salts. The cooperative influence of salts was suggested by some of the experimental results obtained, according to which a maximum effect (as measured by the rapidity of the act of clotting and the degree of shrinkage of the clot) was witnessed when sodium, potassium and calcium chloride were added in certain proportions. This question was not, however, followed up. The experiments about to be detailed form a natural sequence to those of the above investigation. The coagulation of certain proteids by heat was the process now examined into, the question being:-how the presence of salts might affect this process. Into the differenlce, if any, between the spontaneous act of clotting or setting, e.g. of blood recently shed, of milk to which rennet has been added, and the separation by heat of proteid bodies, we do not propose to enter at all fully; and whether Gerinnung be one thing and Abscheidung another, this we must leave undecided. Neither have we in the first instance attempted to determine what might be the essentials, if one may use the expression, of the act of heat coagulation, we have simply taken that act and noted its modification by the presence of certain salts in certain proportions, and in respect of this same act we have examined the mutual reaction of these salts upon one another when present together. Secondly, we have touched upon the question of the essential factors of heat coagulation, but this really represents a side issue.

Abstract:BACKGROUND: An estimated 30 to 40 million school children participate in sports in the United States; 34% of middle-school participants become injured and seek medical treatment at an annual cost close to $2 billion. The purpose of the current study was to evaluate the injury incidence and rates in female athletes in the middle-school setting during the course of 3 seasons.METHODS: Female basketball, soccer, and volleyball players were recruited from a single county public school district in Kentucky consisting of 5 middle schools. A total of 268 female athletes (162 basketball, 26 soccer, and 80 volleyball) participated. Athletes were monitored for sports-related injury and number of athlete exposures (AEs) by an athletic trainer. Injury rates were calculated for specific types of injuries within each sport. Injury rates for games and practices were also calculated and compared for each sport.RESULTS: A total of 134 injuries were recorded during the 3 sport seasons. The knee was the most commonly injured body part (99 injuries [73.9%]), of which patellofemoral dysfunction (31.3%), Osgood-Schlatter disease (10.4%), and Sinding-Larsen-Johansson/patella tendinosis (9%) had the greatest incidence. The ankle was the second most commonly injured body part, accounting for 16.4% of all injuries. The overall rates of injury by sport were as follows: soccer, 6.66 per 1000 AEs; volleyball, 3.68 per 1000 AEs; and basketball, 2.86 per 1000 AEs.CONCLUSIONS: Female middle-school athletes displayed comparable injury patterns to those seen in their high-school counterparts. Future work is warranted to determine the potential for improved outcomes in female middle-school athletes with access to athletic training services.CLINICAL RELEVANCE: As the participation levels and number of injuries continue to rise, middle-school athletes demonstrate an increasing need for medical services provided by a certified athletic trainer.

Abstract:Despite the worldwide popularity of football, research on epidemiology of high school injury is sparse. To describe high school football injury rates and patterns by type of athletic exposure and gender. This descriptive epidemiologic study used retrospective analysis of high school sports injury data from a multi-year US surveillance program capturing data from a large national cohort of schools. Annually, this prospective surveillance study recruits 100 US high schools with National Athletic Trainers' Association affiliated, athletic trainers to report exposure and injury data for athletes participating in school-sanctioned high school sports. Student football players from these nationally representative high schools from 2005–06 through 2013–14. Independent variables included gender, mechanism of injury, playing position, and field location. Rates and patterns of football-related injuries. Overall 6154 football injuries occurred during 2,985,991 athlete exposures (AEs); injury rate of 2.06 per 1000 AEs. This corresponded to a national estimate of 3,381,189 US high school football-related injuries. Injury rates were higher in girls (2.33) than boys (1.83) (RR=1.27, 95% CI 1.21–1.34). Player-player contact more commonly resulted in competition injuries (injury proportion ratio [IPR]=2.87; 95% CI 2.57–3.21), while noncontact mechanisms were more common among practice injuries (IPR=2.10; 95% CI 1.86–2.38). Most injuries occurred to midfielders (37.6%), followed by forwards (28.9%), and defenders (23.6%).. The most common activities leading to injuries were general play (24%), defending (13%), and chasing a loose ball (11%). Injury patterns were similar between genders with respect to position played and location on the field at the time of injury. High school football injury rates vary by gender and type of exposure while injury patterns are more similar across genders. These findings should drive additional research into development, implementation, and evaluation of targeted injury prevention efforts.

Abstract:Background: The incidence of anterior cruciate ligament (ACL) injuries among adolescent athletes is steadily increasing. Identification of the highest risk sports for ACL injuries by sex and competitive setting (ie, practice vs match) is important for targeting injury prevention programs. Purpose: To identify the risk of ACL injuries in adolescent athletes by sport, sex, and setting across a variety of common US and international sports. Study Design: Meta-analysis. Methods: Essentially, 3 online databases (PubMed, Embase, and Cochrane Library) were searched for all studies of ACL injuries per athlete-exposure (AE) or hours of exposure in adolescent athletes. Injuries were then pooled and incidence rates (IRs) reported per 1000 AEs or hours of exposure, with the relative risk (RR) of injuries calculated for sex-comparable sports. IRs per competitive setting (match vs practice) were also calculated. Results: A total of 1235 ACL injuries over 17,824,251 AEs were identified (IR, 0.069 [95% CI, 0.065-0.074]), with 586 of these injuries in girls across 6,986,683 AEs (IR, 0.084 [95% CI, 0.077-0.091]) versus 649 injuries in boys over 10,837,568 AEs (IR, 0.060 [95% CI, 0.055-0.065]). Girls had a higher overall rate of ACL injuries (RR, 1.40 [95% CI, 1.25-1.57]), with the most disproportionate risk observed in basketball (RR, 4.14 [95% CI, 2.98-5.76]). The risk of ACL injuries by sex was highest in girls’ soccer (IR, 0.166 [95% CI, 0.146-0.189]) and boys’ football (IR, 0.101 [95% CI, 0.092-0.111]). ACL injuries were over 8 (RR, 8.54 [95% CI, 6.46-11.30]) and 6 (RR, 6.85 [95% CI, 5.52-8.49]) times more likely to occur in a match versus a practice setting for female and male athletes, respectively. Conclusion: The risk of ACL injuries overall approached nearly 1 per 10,000 AEs for female athletes, who were almost 1.5 times as likely as male athletes to suffer an ACL injury across all adolescent sports. A multisport female athlete was estimated to have a nearly 10% risk of ACL injuries over her entire high school or secondary school career. Specifically, male and female adolescents playing soccer, basketball, lacrosse, and football appeared at particular risk of injuries, a finding that can be used to target an injury intervention.

Abstract:Background: There is limited epidemiologic data on pediatric basketball injuries and the comparison of these injuries before and after adolescence and between male and female athletes. Purpose: To assess common sex- and age-based injury patterns in pediatric basketball players. Study Design: Descriptive epidemiology study. Methods: Injury data from the National Electronic Injury Surveillance System (NEISS) and participation data from the National Sporting Goods Association were used to complete this study. Data on pediatric basketball injuries between January 2012 and December 2018 in patients aged 7 to 11 years (childhood) and 12 to 17 years (adolescence) were extracted and used to calculate national injury incidence rates with 95% CIs. Sex and age group patterns were examined utilizing Pearson chi-square tests. Z tests were conducted for the comparison of injury rates between female and male athletes in each age group and overall injury rate across age groups. Results: An average of 9582 basketball injuries were reported annually in the NEISS, which calculated to an annual national estimate of 294,920 injuries. The most common diagnoses were ankle strain/sprain (17.7%), finger strain/sprain/fracture (12.1%), concussion/head injury (9.4%), knee strain/sprain (4.5%), and facial laceration (3.3%). There was a significant increase in injury prevalence in adolescents (12- to 17-year-old category: 238,678 injuries per year) when compared with childhood (7- to 11-year-old category: 56,242 injuries per year) ( P < .0001). Concussions/head injuries occurred at a high rate in childhood, second only to finger strain/sprain/fracture, and at a similar rate in females and males (injuries per 100,000 athlete-days: 4.9 [95% CI, 3.1-6.7] vs. 5.9 [4.3-7.5], respectively; P = .41). From childhood to adolescence, injury prevalence increased for all areas and across both sexes, except for female finger strain/sprain/fracture; however, the rate of increase for concussion/head injuries and knee injuries was significantly higher in female compared with male athletes ( P < .0001 for both). In adolescents, ankle injuries were the most common injury overall. Conclusion: Ankle injuries continue to be the most predominant pediatric basketball injury. However, disproportionate rates of both knee and concussion/head injuries in female athletes during adolescent basketball are of concern and have implications for injury prevention.

Abstract:Objectives: Rugby is a high-impact collision sport with identical competition rules by sex. The aim of this study was to analyze the trend of rugby-related fractures by body site, sex, and age in amateur athletes from 1999 to 2018. Methods: The National Electronic Injury Surveillance System (NEISS) database was queried to characterize rugby-related injuries from 1999 to 2018 in patients aged 14–23 years old. National injury estimates were calculated using sample weights. Chi-square analysis and one-way ANOVA were performed to compare categorical variables. Results: Out of a total of 43,722 weighted cases of rugby-related fractures over the 19-year period, 70.9% were among high school- and college-aged males and females between the ages of 14–23 years (N = 30,996). Males constituted 79.3% of cases whereas females composed 20.7% of the cases. The proportion of upper extremity fractures was similar in both males and females, yet facial fractures were significantly more common among males than females (27.9% vs. 14.6%, P < 0.001). Among facial fractures, nasal fractures represented 74.4% of the sample, yet facial fractures only resulted in hospital admittance in 1.3% of cases. Lower extremity fractures were more likely to be severe, with 11.3% of LE cases being admitted to the hospital.Conclusion: Rugby players in the US diagnosed in the ED with fractures consisted largely of male, high school- and college-aged athletes. Males and females experienced upper and lower-extremity fractures at comparable rates, yet lower extremity injuries were more likely to be admitted to a hospital. Men were significantly more likely to experience a facial fracture in which a majority were nasal fractures.

Abstract:Purpose: This study aimed to describe the epidemiology of ACL tears in NCAA men's and women's sports. Methods: Injury and exposure data collected within the NCAA Injury Surveillance Program from 2014/2015 to 2018/2019 were analyzed. ACL tear frequencies, injury rates (IR), and injury proportions were used to describe injury incidence by sport, event type, injury mechanism, and injury history. Injury rate ratios (IRR) were used to examine differential injury rates, and injury proportion ratios (IPR) were used to examine differential distributions. Results: A total of 729 ACL tears were reported from 8,474,401 recorded athlete exposures (AE) during the study period (IR = 0.86 per 10,000 AE), and the competition-related ACL tear rate was higher than the practice-related rate (IRR = 5.52, 95% confidence interval [CI] = 4.75-6.39). Among men's sports, the highest overall ACL tear rate was observed in men's football (IR = 1.44 per 10,000 AE), whereas among women's sports, the highest overall rate was observed in women's soccer (IR = 2.60 per 10,000 AE). Among sex-comparable sports, ACL tear rates were higher in women's basketball, softball, and soccer, as compared with their men's counterparts. ACL tears were more prevalently attributed to player contact mechanisms in men's sports than women's sports (IPR = 1.73, 95% CI = 1.37-2.19), but more prevalently attributed to noncontact mechanisms in women's sports than men's sports (IPR = 1.17, 95% CI = 1.01-1.35). Conclusions: ACL tear risk in women's sports continues to warrant attention and prevention efforts. Given the differential rates by event type, future research efforts may also evaluate initiatives to reduce competition-related injury burden in NCAA sports.
Epidemiological Patterns of Ankle Sprains in Youth, High School, and College Football

Abstract:Background: Variations in ankle injury rates and distributions among competition levels are unclear, but such data may help inform strategies to prevent ankle sprains during American football. Purpose: To describe the epidemiological patterns of ankle sprains in youth, high school (HS), and collegiate American football. Study design: Descriptive epidemiological study. Methods: Data regarding youth, HS, and college football athletes were collected from 3 injury surveillance programs: (1) the Youth Football Safety Study (YFSS), (2) the National Athletic Treatment, Injury and Outcomes Network (NATION), and (3) the National Collegiate Athletic Association (NCAA) Injury Surveillance Program (ISP). During the 2012-2014 seasons, the YFSS, NATION, and NCAA ISP included 310, 184, and 71 football team-seasons, respectively. Athletic trainers (ATs) attended each practice and game and reported injuries and athlete-exposures (AEs) via their preferred injury documentation application. Ankle sprain rates for each type of ankle sprain were calculated overall, by event type (ie, practices and games), and specifically for severe injuries (ie, participation restriction time >21 days) and recurrent injuries (as defined by ATs). Rate ratios (RRs) were used to compare ankle sprain rates by competition level and event type. Injury proportion ratios (IPRs) were used to compare differences in severity, surgical needs, recurrence, injury mechanism, and injury activity by competition level. RRs and IPRs with 95% confidence intervals excluding 1.00 were considered statistically significant. Results: A total of 124, 897, and 643 ankle sprains were reported in youth, HS, and college football, respectively. This led to respective rates of 0.59, 0.73, and 1.19 sprains per 1000 AEs. The ankle sprain rate in college football was higher than the rates in HS (RR = 1.64; 95% CI, 1.48-1.82) and youth (RR = 2.01; 95% CI, 1.65-2.43) football. The proportion of ankle sprains that were recurrent in youth football was higher than the proportions in HS (IPR = 2.73; 95% CI, 1.68-4.50) and college (IPR = 2.19; 95% CI, 1.33-3.61) football. Conclusion: Ankle sprain rates were highest in college athletes. However, level-specific variations in ankle sprain severity and recurrence may highlight the need to develop level-specific policies and prevention strategies to reduce injury incidence.
