Abstract:BackgroundDietary self-care is a key element of self-management in type 2 diabetes. It is also the most difficult aspect of diabetes self-management. Adhering to long-term dietary goals and resisting immediate food desires requires top-down inhibitory control over subcortical impulsive and emotional responses to food. Practising simple neurocognitive tasks can improve inhibitory control and health behaviours that depend on inhibitory control, such as resisting alcohol consumption. It is yet to be investigated, however, whether neurocognitive training can improve dietary self-care in people with type 2 diabetes. The aim of this randomised controlled trial is to investigate whether web-based neurocognitive training can improve the ability of people with type 2 diabetes to resist tempting foods and better adhere to a healthy dietary regime.Methods/designIn a double-blind multicentre parallel-group randomised controlled trial, 48 patients (based on power analysis results) with type 2 diabetes recruited from secondary health care services in Birmingham and London, will be randomly allocated to either 25 online sessions of active or control working memory training. Selection criteria include being overweight/obese, having poor diabetes control and reporting to have difficulty following a healthy diet, but having good general health otherwise. Before, immediately after and 3 months after the training, assessment sessions will be conducted. Primary outcome measures include changes in working memory capacity, lab-based food intake and a 24-h guided food recall. Secondary outcome measures include changes in glycaemic control (HbA1c) and lipids. Participants’ experiences of the training will be assessed qualitatively with semi-structured interviews post-training.DiscussionThis is the first trial investigating whether working memory training can improve dietary self-care in people with type 2 diabetes. Selling Real-World Health Care Research to Reluctant Buyers-Evidence-Based Education or Marketing a Defective Product?

Abstract:In December 2004, the editors of BMJ announced a new policy requiring submission of a priori research protocols with manuscripts reporting the results of randomized controlled trials (RCTs). Under the new policy, RCTs would have to be registered at their outset with “a suitable trial registry,” and manuscripts lacking a registered protocol would no longer be sent to peer review. The editors explained that this decision had been made because their “experience of chasing authors for trial protocols, when we have suspected deviation in the protocol or found it hard to fathom what the authors set out to do,” had been “miserable.”1 As occasionally miserable editors, we too are familiar with the experience of seeking information from an author who is unable or unwilling to provide it; and evidence is mounting that we are not alone. As we and others have observed previously, the practices of selective reporting of study findings, publication planning, and other forms of misconduct are, sadly, reportedly endemic in health care research.2,3 Studies that use observational or “real-world” data, particularly pharmacoeconomic modeling and retrospective analyses of administrative databases, are particularly vulnerable to manipulation; it is especially easy to make post hoc changes to a planned protocol behind closed doors when only claims data and hypothetical patient populations, not prospectively studied human subjects, are involved.4 Thus, to the extant problem presented by Brixner et al. in their commentary on use of real-world data in this issue of JMCP5— that decision makers are sometimes reluctant to rely on analyses of real-world data—one reasonable response is that the most reluctant “buyers” of research may well be the best informed. After all, public denunciations of the “scandal of poor epidemiological research” by von Elm and Egger in 2004,6 and the “scandal of poor medical research” by Altman in 1994 and again in 20027,8 are well-known to anyone who has been following health care research even peripherally, and have spawned dozens of publications on how to improve a demonstrably inadequate pool of knowledge about the economic and clinical outcomes of health care interventions.2 Yet, we also know that many—perhaps even most—researchers are “playing by the rules,” endeavoring to provide accurate information, and producing high-quality work. So for a typical decision maker, the question becomes how to distinguish between accurate and inaccurate information. This is essentially the question raised by Brixner et al., and it is an important one. The proposals offered by the March 2008 participants in a roundtable discussion of real-world data, whose views are reported in the Brixner et al. commentary, merit consideration. Nonetheless, currently these ideas appear to generate more new questions than specific guidance. More troubling is the possibility that the effort to promote use of real-world data by decision makers may detract from ongoing efforts to improve the quality of information provided to them.
An Intervention to Change Illness Representations and Self-Care of Individuals With Type 2 Diabetes: A Randomized Controlled Trial

Abstract:Objective: Suboptimal self-care by individuals with diabetes mellitus (DM) is a significant public health concern. The common-sense model (CSM) proposes that illness representations are associated with coping and health outcomes across various conditions. The present study examined the efficacy of a CSM-based intervention in improving illness representations, self-care, self-care self-efficacy, use of adaptive coping strategies, and glycated hemoglobin among individuals with type 2 DM (T2DM). Methods: A two-arm randomized controlled trial was used. A total of 455 T2DM patients were recruited from an outpatient DM clinic and randomized to an intervention group that consisted of five weekly group-based education sessions or a control group that received five weekly educational booklets. Evaluation was conducted at baseline and at 1- and 6-month follow-up. Results: The 2 × 3 linear mixed-model analysis using a modified intention to treat revealed a significant time by condition interaction effect on level of self-care (F(2,840) = 7.78, p < .001), self-care self-efficacy (F(1.89,794.57) = 14.40, p < .001), and use of adaptive coping strategies (F(1.94,812.93) = 4.75, p = .010) in which participants in the intervention group reported greater improvement in such aspects compared with those in the control group. A significant time effect was observed in some dimensions of illness representations. No significant effect was found in glycated hemoglobin. Participants reported positive feedback to the intervention and perceived improvement in various domains. Conclusions: The CSM-based intervention was effective in improving self-care and coping among DM patients. The intervention also demonstrated high feasibility and acceptability. Findings provided important insights in improving health-related outcomes for patients with T2DM using the CSM framework.

Abstract:Introduction and Objectives: Diabetes Self-Management Education and Support (DSMES) is a recommended component of diabetes care. Despite proven benefits, patient engagement with DSMES needs improvement. The DM-BOOST program, based on principles of Self-Determination Theory and Behavioral Economics and co-designed with patients, was developed to increase patient engagement with DSMES. DM-BOOST included peer-written text messages and digital health training to improve patient portal and telehealth use. The purpose of this study is to explore the feasibility of implementing DM-BOOST in an established DSMES program and its impact on engagement and experiences among individuals with type 2 diabetes (T2D). Methods: A feasibility pilot was conducted from November 2021-November 2022. Upon conclusion, semi-structured phone interviews were conducted with 36 individuals with T2D. The interviews were audio recorded and transcribed verbatim. An inductive thematic analysis was conducted by two independent coders using MAXQDA24 software. The data collection, analysis, and interpretation of findings were guided by the Unified Theory of Acceptance and Use of Technology (UTAUT). Results: Participants (n=36) were mean (SD) 45.9 (8.9) years old, 61% female, with a mean baseline HbA1c of 10.1% (1.8). The racial/ethnic distribution was 64.9% White, 11.1% Black, 5.6% Asian, 8.3% more than one race and 11.1% preferred not to answer (with 27.8% overall Hispanic/Latino). Four themes emerged: (1) empowering autonomy through accessible and personalized DSMES, (2) perceived transformative impact on self-management competency and health outcomes, (3) empathetic experience with diabetes educators and research team, and (4) timely, additional support that was coordinated with their care providers. Abstract:Background: Approximately 11.3% of the US population has diabetes. The burden of diabetes is higher in older adults and African Americans (AAs), such that 40% of adults aged 50 years and older have diabetes; African Americans are 60% more likely to be diagnosed with diabetes compared to non-Hispanic Whites (NHWs). Structural racism has resulted in fewer economic and educational opportunities for AAs, higher social risks, and unmet basic needs, which result in financial instability, housing instability, food insecurity, and a lack of transportation compared to NHWs. The presence of these unmet basic needs is a driver of poor adherence to diabetes self-management in older AAs. Aim: To test the preliminary efficacy of a nurse case-manager, telephone-delivered intervention that provides foundational diabetes self-management education and skills training, while also addressing and resolving the unmet social needs of older AAs with poorly controlled type 2 diabetes mellitus (T2DM). The primary hypothesis is that older AAs with T2DM randomized to the DM Social Needs intervention will have significantly lower hemoglobin A1C (HbA1C), blood pressure, and LDL levels compared to the usual care arm at 6 months post randomization. Methods: This is a three-year prospective, randomized clinical trial that will enroll 100 AAs aged 50 and older with type 2 diabetes (T2DM) and HbA1C ≥ 8% into one of the following two groups: (1) a nurse case-manager, telephone-delivered intervention that provides foundational diabetes self-management education and skills training, but also addresses and resolves unmet social needs; or (2) an enhanced usual care group that will receive mailed diabetes education materials. Participants will be followed for 6 months to determine the effect of the intervention on HbA1C, blood pressure, and low-density lipoprotein (LDL) cholesterol levels. Results: Baseline characteristics will be presented by study group, and within- and between-group changes in primary outcomes from baseline to 6 months will be reported. Conclusion: The results from this study will provide insights into the efficacy of a combined diabetes education and skills training and social needs resolution intervention for older African Americans with poorly controlled type 2 diabetes and will inform strategies to improve diabetes outcomes for this vulnerable population.

Abstract:BACKGROUND: Diabetes mellitus is a metabolic illness that happens whenever blood glucose levels increase. Investigations have proven that Type 2 diabetes is associated with higher risk of cognitive impairment and functional limitations. The extent and severity of cognitive dysfunctions depending upon the duration of diabetes and glycemic control, but it is difficult to do a prediction on who will be at a higher risk for developing cognitive dysfunctions. Cognitive impairment can put up to functional limitations and lead to functionally dependence. Recent researches have shown that cognition and motor performance are interdependent and integration of these two functions are required for the successful performance of daily life activities. Purpose: To investigate the effects of Resistance training on Cognition in people with type 2 diabetes mellitus having mild cognitive impairments, Materials and method: Thirty type 2 diabetes mellitus subjects between the age group of 45 -60 years were selected purposively and were randomly divided into two groups, Group A and Group B, with 15 in each group. Group A received resistance training along with conventional physiotherapy for 3 days a week for 4 months. Group B received conventional physiotherapy alone. Montreal Cognitive Assessment scale (MoCA) was used to assess cognition. Pre score was taken before the first session and post score was taken after 4 months, using Montreal cognitive assessment scale (MoCA). Analysis: Follow up values was taken after four months. The results from outcome measure suggest that Group A (p<0.05) has greater effects when compared to Group B. The pre and post values of MoCA was analysed using Paired t-test and Wilcoxon signed rank test for within the group analysis and the between group analysis was done using Mann Whitney U Test. The corresponding p value of less than 0.05 was considered significant for each outcome. RESULTS: Comparison of Group A and Group B using MoCA score at the end of study in both group was found to statistically significant (p<0.05).The Group A shows greater statistical significance when compared with Group B. CONCLUSION: The study reveals that the addition of resistance training along with conventional exercises enhances cognitive function in subjects having type 2 diabetes mellitus with mild cognitive impairment. A mixed-methods approach to understanding the perspectives, experiences, and attitudes of a culturally tailored cognitive behavioral therapy/motivational interviewing intervention for African American patients with type 2 diabetes: a randomized parallel design pilot study

Abstract:Background: African American (AA) adults are 60% more likely to be diagnosed with diabetes mellitus (DM) and experience more complications than non-Hispanic White adults. Cognitive behavioral therapy (CBT) has shown to be an effective modality for helping patients improve health behaviors and regulate emotional states. Motivational interviewing (MI) addresses participant engagement and motivation. Therefore, MI was combined with CBT as an approach to the process of learning using CBT skills to promote healthy lifestyle choices. We aimed to assess the effects of a culturally tailored CBT/MI intervention on glycemic control in AA participants and understand their perspectives, attitudes, and experiences while participating in this intervention. Methods: Using a randomized, parallel design pilot study (web-based group vs in-person group), 20 participants aged ≥ 18 years, identifying as AA and having a glycosylated hemoglobin (HbA1c) > 8%, were recruited. A CBT/MI intervention was administered in six sessions over 3 months. Participants completed baseline and follow-up assessments on measures for diabetes control (HbA1c), self-efficacy, generalized anxiety, depression, perceived stress, health-related quality of life, and cognitive ability. Post-CBT/MI intervention focus groups were conducted to determine patient perspectives regarding the intervention. Results: Fourteen participants completed the study, their mean HbA1c improved from 10.0 to 8.9% (t(26) = 0.5, p-value = 0.06). The Diabetes Distress Scale demonstrated decreased distress overall (t(26) = 2.6; p-value = 0.02). The Generalized Anxiety Disorder Scale demonstrated decreased generalized anxiety for all participants (t(26) = 2.2; p = 0.04). Themes identified in focus groups included (1) intervention group social support through information sharing, (2) mental health and personal identities in diabetes understanding and management, and (3) receptivity to CBT/MI intervention positively impacts self-efficacy through improved health literacy. Conclusion: This group-based, culturally tailored CBT/MI intervention for type 2 DM care was positively received by AA participants and helped improve diabetes control, as demonstrated by the change in HbA1c. There were additional benefits of social support through group interactions and a stronger sense of self-efficacy due to health education. A comprehensive treatment plan using a CBT/MI intervention may be useful in promoting healthy diabetes self-management. Trial registration: ClinicalTrials.gov , NCT03562767 . Abstract:Aim This study examined the preliminary effects of a nurse‐led self‐management education and support programme on the self‐management behaviours and quality of life among people with type 2 diabetes in Western Ethiopia. Methods A pilot randomized controlled trial was conducted between January and August 2021. Participants were recruited in the hospital and randomly assigned to the control arm to continue usual care (n = 38) or the intervention arm to receive usual care and the diabetes self‐management education and support programme (n = 38) in the community. Self‐management behaviours and quality of life were assessed using a 10‐item summary of diabetes self‐care activity (expanded) scale and a 34‐item diabetes quality of life measure, respectively, at baseline and 2 months after follow‐up. Generalized estimating equation models were used to examine the preliminary effects of the programme on the outcomes. Results Preliminary results indicated that the programme outperformed usual care in self‐management practise, with large effect sizes immediately postintervention and at 2 months after the intervention, and quality of life at 2 months after the intervention. Conclusion A nurse‐led diabetes self‐management education and support intervention, including the families of people with diabetes, may be an option to boost the self‐management practise and quality of life of patients with diabetes.

Feasibility and Acceptability of a Self-Guided Digital Family Skills Management Intervention for Children Newly Diagnosed With Type 1 Diabetes: Pilot Randomized Controlled Trial

Abstract:Background: Family dynamics play an important role in determining the glycemic outcomes of type 1 diabetes (T1D) in children. The time interval immediately following T1D diagnosis is particularly stressful for families, and interventions to support families in adjusting their family practices to support adjustment to and management of T1D in the months following diagnosis may improve glycemic outcomes. Self-guided digital interventions offer a sustainable model for interventions to support caregivers in learning evidence-based family management skills for adjustment to and management of T1D. Objective: We hypothesized that a self-guided, web-based, family skills management program (addressing caregiver social support as well as family problem-solving, communication, and supportive behavior change strategies) initiated at the time of T1D diagnosis would improve glycemic outcomes in children with T1D. In this study, we report on the feasibility and acceptability of this program. Methods: We prospectively evaluated a sample of 37 children newly diagnosed with T1D recruited from a pediatric endocrinology clinic. Parent participants were asked to complete web-based modules addressing social support, family problem-solving, communication, and supportive behavior change strategies. Module completion was analyzed for percentage completion, patterns of completion, and differences in completion rates by coparenting status. Qualitative open-ended feedback was collected at the completion of each module. Results: A total of 31 (84%) of the 37 participants initiated the web-based program. Of those 31 participants, 25 (81%) completed some content and 15 (48%) completed all 5 modules. Completion rates were higher when coparenting partners engaged in the intervention together (P=.04). Of the 18 participants given a choice about the spacing of content delivery, 15 (83%) chose to have all sessions delivered at once and 3 (17%) chose to space sessions out at 2-week intervals. Qualitative feedback supported the acceptability of the program for delivery soon after T1D diagnosis. Families reported on positive benefits, including requesting future access to the program and describing helpful changes in personal or family processes for managing T1D. Conclusions: In this study, we found that a self-guided digital family support intervention initiated at the time of a child's T1D diagnosis was largely feasible and acceptable. Overall, rates of participation and module completion were similar to or higher than other self-guided digital prevention interventions for mental and physical health outcomes. Self-guided digital programs addressing family management skills may help prevent challenges common with T1D management and can decrease cost, increase access, and add flexibility compared to traditional interventions. Abstract:In recent years, great emphasis has been placed on the role of nonpharmacological self-management in the care of patients with diabetes. Studies have reported that nurses, compared to other healthcare professionals, are more likely to promote preventive healthcare seeking behaviors. The aim of this study was to investigate the effectiveness of a nurse-led diabetes self-management education on glycosylated hemoglobin. A two-arm parallel-group randomized controlled trial with the blinded outcome assessors was designed. One hundred forty-two adults with type 2 diabetes were randomized to receive either usual diabetes care (control group) or usual care plus a nurse-led diabetes self-management education (intervention group). Duration of the intervention was 12 weeks. The primary outcome was glycosylated hemoglobin (HbA1c values). Secondary outcomes were changes in blood pressure, body weight, lipid profiles, self-efficacy (efficacy expectation and outcome expectation), self-management behaviors, quality of life, social support, and depression. Outcome measures were assessed at baseline and at 12-week and 24-week postrandomizations. Patients in the intervention group showed significant improvement in HbA1c, blood pressure, body weight, efficacy expectation, outcome expectation, and diabetes self-management behaviors. The beneficial effect of a nurse-led intervention continued to accrue beyond the end of the trial resulting in sustained improvements in clinical, lifestyle, and psychosocial outcomes. The Effectiveness of Cognitive-Focused Interventions for Adults With Diabetes: A Systematic Review

Abstract:Western Journal of Nursing Research, Ahead of Print. Background:Adults with diabetes and impaired memory and executive functions are more likely to experience difficulties in diabetes self-management and achieving glycemic targets. The purpose of this systematic review was to evaluate the evidence of the effects of cognitive-focused interventions on cognitive ability, diabetes self-management, and management of glycemic levels for middle-aged adults and older adults with diabetes.Methods:A systematic review of randomized controlled/clinical trials published in English between 2012 and 2022 was conducted. A search was performed using 5 databases (PubMed, CINAHL, Embase, Web of Science, and Scopus) in addition to hand-searching. The search terms included diabetes, adults, cognitive-focused intervention, cognition, self-management, and hemoglobin A1C (HbA1C).Results:Eleven studies met the inclusion criteria. Cognitive ability and diabetes self-management were assessed using different measurements, and glycemic levels were measured with HbA1C. Nine studies applied cognitive training, one provided working memory training, and one used occupational therapy. Eight studies combined cognitive training with a co-intervention, including self-efficacy, lifestyle management, physical training, chronic disease self-management program, square-stepping exercise, psychoeducational intervention, and empowerment. Eight studies showed statistically significant improvements in at least one cognitive domain.Conclusions:Cognitive-focused interventions have a positive effect on improving memory and executive function. However, the evidence of cognitive-focused interventions on diabetes self-management and glycemic levels has not been established. Future studies to improve cognition using effective strategies to improve cognitive function enhancing diabetes self-management behaviors and managing glycemic levels are warranted.

The Science of Diabetes Self-Management and Care

Abstract:Purpose: The purpose of this study was to assess the feasibility of delivering the Diabetes Tune-Up Group (DTU), a cognitive-behavioral-therapy-based (CBT) multidisciplinary intervention for adults with diabetes distress and elevated A1C using a group in-person delivery format. Methods: The DTU intervention consisted of 6 weekly group sessions (90 minutes in duration per session). The groups were cofacilitated by a diabetes care and education specialist (DCES) and a master’s-level clinical psychology trainee. The intervention integrated CBT with patient-centered diabetes education. Using a pre/post study design, participants completed assessments at baseline, post-intervention, and 3 months following the intervention. Results: The sample consisted of 29 adults with type 1 diabetes (N = 8) or type 2 diabetes (N = 21) who were predominantly female (79%), White (59%), and educated (56% with a college degree or greater). Participants attended 131 total sessions out of 174 possible sessions, for an overall attendance rate of 75.3%. At 3-month follow-up, significant improvements were observed in A1C values (mean decrease = 0.39%). Diabetes distress improved significantly from baseline (mean = 3.44, SD = 0.68) to post-intervention (mean = 2.94, SD = 0.68), and 3-month follow-up (mean = 2.55, SD = 0.75). Significant improvements were also observed in diabetes self-efficacy from baseline to post-intervention and at 3-month follow-up. Conclusions: This group-based, multidisciplinary intervention resulted in improvements in A1C, diabetes distress, and patient self-efficacy in caring for diabetes. Future studies to validate this intervention approach across settings and delivery platforms are needed.

Abstract:Type 2 diabetes mellitus (T2DM) is a chronic disease that affects a person's general well-being. Current evidence sets an association between psychological well-being and controlled metabolic parameters. People newly diagnosed with T2DM show higher prevalence of depression and anxiety symptoms. Cognitive behavioral therapy (CBT) has effectively improved psychological adjustment, but most studies do not specifically address recently diagnosed people nor usually include long-term follow-up measures. We sought to assess changes in psychological variables in people with newly diagnosed diabetes who received a cognitive–behavioral intervention, within a comprehensive care program. 1208 adults with T2DM (≤5 years) who attended a national health institute in Mexico received a cognitive–behavioral intervention aimed at improving quality of life and reducing emotional distress that often interferes with diabetes control, as well as evaluating cognitive and emotional resources and social support. Measures of quality of life, diabetes-related distress, anxiety and depression questionnaires were compared at pre-test, post-test and follow up using Friedman's ANOVAs. Multiple logistic regression models evaluated glycosylated hemoglobin (HbA1c) and triglycerides control at post-test and follow up. Questionnaire measures and metabolic variables significantly decreased symptomatology at post-test and these changes maintained at follow-up. Significant associations were found between quality-of-life scores and HbA1c and triglycerides levels in post-test and follow-up. Diabetes-related distress scores increased the odds of having adequate HbA1c control at post-test. This study contributes to the evidence on the importance of considering psychological factors as part of comprehensive diabetes care to improve quality of life and emotional burden and facilitate the achievement of metabolic goals.
Improving Self-management of Type 2 Diabetes in Latinx Patients: Protocol for a Sequential Multiple Assignment Randomized Trial Involving Community Health Workers, Registered Nurses, and Family Members

Abstract:Background: The rate of Type 2 diabetes mellitus (T2DM) among Mexican American individuals is 16.3%, about twice that of non-Hispanic White individuals. While a number of education approaches have been developed and shown to improve diabetes self-management behaviors and glycemic control for Spanish-speaking Latinx patients with T2DM, there is little research to guide health practitioners regarding which interventions to apply and when so that resources are used efficiently, and treatment outcomes are maximized. Objective: This study aimed to describe an adaptive intervention that integrates community mental health workers, diabetes nurse educators, family members, and patients as partners in care while promoting diabetes self-management for Mexican American individuals with T2DM. The project incorporates four evidence-based, culturally tailored treatments to determine what sequence of intervention strategies work most efficiently and for whom. Given the increasing prevalence of T2DM, achieving better control of diabetes and lowering the associated medical complications experienced disproportionally by Mexican American individuals is a public health priority. Methods: Funded by the National Institute of Nursing Research (National Institutes of Health grant R01 NR015809), this project used a sequential multiple assignment randomized trial and included 330 Spanish-speaking Latinx patients with T2DM. In the first phase of the study, subjects were randomly assigned to an evidence-based diabetes self-management educational program called Tomando Control delivered in a group format for 6, biweekly 1.5-hour sessions, led either by a community health worker or a diabetes nurse educator. In the second phase of the study, those subjects who did not improve their diabetes self-management behaviors were rerandomized to receive either an augmented version of Tomando Control or a multifamily group treatment focused on problem-solving. The primary outcome measure was the "Summary of Diabetes Self-Care Activities." Evaluations were made at baseline and at 3, 6, and 12 months. Results: This study was funded in June 2016 for a period of 5 years. Institutional review board approval was obtained in November 2016. Between March 2017 and September 2020, a total of 330 patients were recruited from the outpatient primary care clinics of Olive View-UCLA Medical Center, with a brief hiatus between May 2020 and July 2020 due to COVID-19 restrictions. The study interventions were completed in December 2020. Data collection began in March 2017 and was completed in December 2021. Data analysis is expected to be completed in Spring 2023, and results will be published in Fall 2023. Conclusions: The results of this trial should help practitioners in selecting the optimal approach for improving diabetes self-management in Spanish-speaking, Latinx patients with T2DM. Abstract:The epidemic of type 2 diabetes (T2DM) is spreading around the globe and challenging the unprecedented success of health sciences in increasing longevity. T2DM has been linked to accelerated brain aging, functional decline in older adults and dementia. Brain insulin resistance and glycemic variability are potential mechanisms underlying T2DM-related brain damage and cognitive decline. Intranasal insulin therapy has emerged as a potential new treatment for T2DM-related cognitive impairment. Wearable technologies now allow better monitoring of behaviors and glycemic levels over several days and deliver real time feedback that can be used to improve self-management and lead to new prevention strategies and therapies for T2DM complications.
Nurse-in-the-Loop Artificial Intelligence for Precision Management of Type 2 Diabetes in a Clinical Trial Utilizing Transfer-Learned Predictive Digital Twin

Abstract:Background: Type 2 diabetes (T2D) is a prevalent chronic disease with a significant risk of serious health complications and negative impacts on the quality of life. Given the impact of individual characteristics and lifestyle on the treatment plan and patient outcomes, it is crucial to develop precise and personalized management strategies. Artificial intelligence (AI) provides great promise in combining patterns from various data sources with nurses' expertise to achieve optimal care. Methods: This is a 6-month ancillary study among T2D patients (n = 20, age = 57 +- 10). Participants were randomly assigned to an intervention (AI, n=10) group to receive daily AI-generated individualized feedback or a control group without receiving the daily feedback (non-AI, n=10) in the last three months. The study developed an online nurse-in-the-loop predictive control (ONLC) model that utilizes a predictive digital twin (PDT). The PDT was developed using a transfer-learning-based Artificial Neural Network. The PDT was trained on participants self-monitoring data (weight, food logs, physical activity, glucose) from the first three months, and the online control algorithm applied particle swarm optimization to identify impactful behavioral changes for maintaining the patient's glucose and weight levels for the next three months. The ONLC provided the intervention group with individualized feedback and recommendations via text messages. The PDT was re-trained weekly to improve its performance. Findings: The trained ONLC model achieved >=80% prediction accuracy across all patients while the model was tuned online. Participants in the intervention group exhibited a trend of improved daily steps and stable or improved total caloric and total carb intake as recommended.

Abstract:Studies are presented on a 62-yr-old woman with extreme hepatomegaly due to a giant hemangioma with alterations in the clotting system indicating a consumption coagulopathy. There was a fall of hemoglobin, fibrinogen, antithrombin III, and platelet number after arteriography of the truncus celiacus. Furthermore, there was sustained bleeding in the patient's right thigh caused by puncture of the arteria femoralis. Continuous administration of iv heparin corrected the clotting disorder including a rise in platelets from 95,000/microliters to 148,000/microliters, permitting surgical removement of the hematoma. Celiacography is a useful tool for the diagnosis of hepatic hemangiomata, as well as ultrasound, computed tomography, and magnetic resonance imaging. Being an invasive technique, it requires testing for possible consumption coagulopathy if used in patients with hemangiomatosis. Its application should be restricted to cases in which exact diagnosis cannot be established by other means.

Abstract:Objective To investigate the effect of multidisciplinary cognitive intervention in treating type 2 diabetes mellitus patients with mild cognitive impairment.Methods By convenience sampling,80 patients with type 2 diabetes mellitus and mild cognitive impairment admitted to geriatric ward of our hospital from January 2015 to June 2016 were selected and randomly divided into control group and observation group. The control group received routine cognitive intervention,while the observation group received multidisci-plinary cognitive intervention.Results No statistical significance of cognitive function,2h postprandial blood sugar and hemoglobin score was found between before intervention and 1 month after interven-tion(P> 0.05),however statistical significances were found between before intervention and 3 and 6 months after intervention(all P<0.05).There was no statistically significant difference of daily living score between two groups before intervention(P > 0.05),while statistical significances were found in 1 month,3 months and 6 months after intervention(all P<0.05).In addition,The time effect,group effect and their interaction of all indexes were all statistically significant in both groups(all P<0.05).Conclu-sions The cognitive function and blood glucose level of type 2 diabetes mellitus patients with mild cogni-tive impairment could be improved by both multidisciplinary cognitive intervention and conventional cogni-tive intervention,and with the better effect multidisciplinary cognitive intervention.
