Abstract:BACKGROUND AND AIM: Metal exposures are suspected to associate with the risk of chronic kidney disease (CKD). Copper (Cu) is an essential yet toxic trace element in humans. Epidemiological evidence on the association of Cu exposure with CKD remains inconsistent. We aim to investigate the association of blood Cu and its co-exposure to other metals with CKD. METHODS: A cross-sectional study was conducted among 3,286 Chinese elderly adults in Zhejiang, China. The whole blood levels of Cu as well as other 10 metals were measured by inductively coupled plasma mass spectrometry (ICP-MS). CKD events were defined as the presence of albuminuria or <60 mL/min/1.73m2 estimated glomerular filtration rate (eGFR). Multivariable logistic regression and linear regression models were applied to assess the single-metal association of Cu with kidney function. The Bayesian kernel machine regression (BKMR) was used to estimate associations of the metal mixture with CKD. RESULTS: In the logistic models, we observed a positive dose-dependent association of blood Cu with CKD prevalence after adjusting for multiple covariates. Positive linear dose-response associations with blood Cu were also found for CKD and negative for eGFR. The positive association between blood Cu and CKD risk also observed in BKMR model and stronger at lower level of manganese (Mn). CONCLUSIONS: The whole blood level of Cu was remarkably associated with the prevalence of CKD and showed positive dose-response relationships in the elderly Chinese population. The toxicity of Cu on kidney function could be antagonized by the exposure of Mn. These findings have provided new clues of the potential roles of Cu in kidney health, and the needs to develop not only recommended intake but also safe limits for Cu. The Bayesian kernel machine regression (BKMR) was used to examine the association of metal interactions with CKD.Results: In logistic models, we found a dose-dependent positive association between blood Cu and the prevalence of CKD. Compared to the lowest quartile of blood Cu, the odds ratio (OR) of CKD in the highest quartile was 1.65 (95% confidence interval, CI: 1.25, 2.17), and the f3 of eGFR was-1.74 (95% CI:-3.15,-0.34) after adjusting for multiple covariates. The positive linear dose-response association was found between blood Cu and the OR of CKD, and negative linear association was found between Cu and the f3 of eGFR. The positive association between blood Cu and CKD risk was stronger at lower level of manganese (Mn) in the BKMR model. Conclusion: The whole blood level of Cu was remarkably associated with the risk of CKD and showed positive dose-response relationships in the older Chinese population. The toxicity of Cu on kidney function could be antagonized by the exposure of Mn. These findings shed new light on the potential role of Cu in kidney health. Abstract:Background Chronic Kidney Disease of unknown cause (CKDu) a disease of exclusion, and remains unexplained in various parts of the world, including India. Previous studies have reported mixed findings about the role of heavy metals or agrochemicals in CKDu. These studies compared CKDu with healthy controls but lacked subjects with CKD as controls. The purpose of this study was to test the hypothesis whether heavy metals, i.e. Arsenic (As), Cadmium (Cd), Lead (Pb), and Chromium (Cr) are associated with CKDu, in central India. Methods The study was conducted in a case-control manner at a tertiary care hospital. CKDu cases ( n = 60) were compared with CKD ( n = 62) and healthy subjects ( n = 54). Blood and urine levels of As, Cd, Pb, and Cr were measured by Inductively Coupled Plasma- Optical Emission Spectrometry. Pesticide use, painkillers, smoking, and alcohol addiction were also evaluated. The median blood and urine metal levels were compared among the groups by the Kruskal-Wallis rank sum test. Results CKDu had significantly higher pesticide and surface water usage as a source of drinking water. Blood As levels (median, IQR) were significantly higher in CKDu 91.97 (1.3–132.7) μg/L compared to CKD 4.5 (0.0–58.8) μg/L and healthy subjects 39.01 (4.8–67.4) μg/L ( p < 0.001) On multinominal regression age and sex adjusted blood As was independently associated with CKDu[ OR 1.013 (95%CI 1.003–1.024) P < .05].Blood and urinary Cd, Pb, and Cr were higher in CKD compared to CKDu ( p > .05). Urinary Cd, Pb and Cr were undetectable in healthy subjects and were significantly higher in CKDu and CKD compared to healthy subjects ( P = < 0.001). There was a significant correlation of Cd, Pb and Cr in blood and urine with each other in CKDu and CKD subjects as compared to healthy subjects. Surface water use also associated with CKDu [OR 3.178 (95%CI 1.029–9.818) p < .05). Conclusion The study showed an independent association of age and sex adjusted blood As with CKDu in this Indian cohort. Subjects with renal dysfunction (CKDu and CKD) were found to have significantly higher metal burden of Pb, Cd, As, and Cr as compared to healthy controls. CKDu subjects had significantly higher pesticide and surface water usage, which may be the source of differential As exposure in these subjects.

Abstract:The determination of toxic elements in the biological samples of human beings is an important clinical screening procedure. The aim of this work was to determine total content of toxic elements-aluminum (Al), cadmium (Cd), and lead (Pb)-in whole blood and urine samples of male chronic renal failure patients (CRFPs) on maintenance hemodialysis from 2006 to 2007. The study included 100 CRFPs, plus 150 healthy volunteers in the control group. The concentration of toxic elements (TEs) were determined in blood sample before and after hemodialysis, while urine sample was determined once, before dialysis. Toxic elements were analyzed by electrothermal atomic absorption spectrometer, prior to microwave-induced acid digestion. The accuracy of the total Al, Cd, and Pb measurements was tested by simultaneously analyzing certified reference materials. No significant differences were established between the analytical results and the certified values (paired t-test at p > 0.05). The levels of TEs in blood samples of patients before dialysis were found to be higher than blood samples after dialysis session. In the control group, the blood levels of Al, Cd, and Pb were significantly lower than the chronic renal failure patients. Moreover, the study shows that analyzing levels of Al, Cd, and Pb may be useful in hemodialysis patients in evaluating TEs status.

Abstract:Sampling Blood collected for patients with renal failure of the dialysis group and the non-dialysis group for randomly for (40-60) age. And conduct tests CBC. The oxidation enzymes and some of the sample heavy metal (Ai, Cd, Cr, Pb and As) after digestion and measured in atomic spectrophotometer the result show They found a clear relationship between kidney failure and the effect of heavy metals. Where it affected the blood rate and the number of white blood cells and oxidation enzymes. Where were the values after conducting the tests (12.8 ± 0.45) (9.57 ± 0.62) (9.43 ± 0.47) respectively for control, hemodialysis and non-hemodialysis for Hp. Tests for WBCs (7.8 ± 0.32) (8.07 ± 0.75) (8.37 ± 0.50) respectively for control, hemodialysis and non-hemodialysis. The results of the current study showed in the age group (40_60) that there was a decrease in significant at (P< 0.05) in SOD, GSH and CAT for hemodialysis patients compared with control (0.336±0.035 Mg\dl), (0.4086±0.0041 Mg\dl) and (0.191±0.0051 Mg\dl) Consecutively, according to the table indicated (1-2) and That's agreement with previous studies. The results of the current study showed in the age group (40_60) that there was a decrease in significant at (P< 0.05) in SOD, GSH and CAT for non-hemodialysis patients compared with control (0.748±0.071 Mg\dl), (0.838±0.019Mg\dl) and (0.335±0.0023 Mg\dl) Consecutively, according to the table indicated (1-2) and That's agreement with previous studies

Abstract:A known case of hypertension and recent onset diabetes presented to our neurological clinic with symptoms of ataxia, rigidity, and tremors. His symptoms were of relatively recent onset. He gave no history of any renal disease in past. The magnetic resonance imaging of the brain done by the neurologist was suggestive of demyelinating pathology. His renal functions showed progressive deterioration (Cr 1.4 mg/dl about 3 months back to 2.2 mg/dl at present) along with the onset of his neurological illness. An extensive work up for autoimmune encephalitis and paraneoplastic syndrome was noncontributory. A toxicology screen revealed high levels of aluminum in the blood. A renal biopsy showed features of interstitial nephritis and predominant vacuolar injury of the proximal tubule (suggestive of toxic injury.) On further questioning, the patient gave history of using an over the counter native medication. The medication was stopped and weekly desferrioxime chelation advised. A short course of steroids (0.5 mg/kg/day tapering dose for 6 weeks) was also given. The creatinine stabilized to 1.3 mg/dl on follow-up after 3 months. The neurological symptoms also resolved completely.

Abstract:Aluminum accumulation is a well-described complication in dialysis patients. Improvements in hemodialysis technology have possibly eliminated the occurrence of aluminum overload. Limited evidence suggests that aluminum overload may decline in the era of aluminum removal from dialysis fluids, even with the use of aluminum binders. We examined the data from January 2014 to June 1, 2020, identified through our electronic records, to evaluate the desferrioxamine (DFO) test results for aluminum overload. The presentation and treatment of aluminum overload were recorded. Ninety-nine dialysis patients were enrolled for the DFO test. Forty-seven patients (47.5%) were identified as DFO test positive for aluminum overload, of which 14 (14/47) patients had symptoms, including one patient with an unexplained fracture, eight patients with unexplained anemia despite high-dose erythropoiesis-stimulating agents, and five patients with hypercalcemia (serum calcium >11 mg dL -1 ). None of the patients with aluminum overload developed encephalopathy. Only four of the 47 patients had microcytic anemia. Patients requiring longer treatments (>10 months versus <10 months) had similar basal serum aluminum ( p = 0.219) but had an increase in serum aluminum after DFO ( p = 0.041). Furthermore, the treatments decreased erythropoietin doses in the aluminum overload group, with serum total alkaline phosphatase levels <60 U L -1 ( p = 0.028). We concluded that aluminum overload existed in the reverse osmosis dialysis era. In light of non-obvious symptoms, such as anemia and bone turnover change, serum aluminum in dialysis patients should be monitored in countries using aluminum-based phosphate binders, despite reverse osmosis dialysis.

Assessing Cross-Sectional Association of Uremic Pruritus with Serum Heavy Metal Levels: A Single-Center Study

Abstract:(1) Background: Uremic pruritus (UP) is a common and taxing symptom in patients on maintenance hemodialysis (MHD). We have previously shown that blood lead levels (BLLs) and blood aluminum levels (BALs) were separately positively associated with UP in MHD patients. We also found that blood cadmium levels (BCLs) were positively associated with all-cause mortality and cardiovascular-related mortality in MHD patients. We wondered whether there is any correlation between BCLs and UP after adjusting for BLLs and BALs. (2) Methods: Patients enrolled in this study were all from three hemodialysis (HD) centers at Chang Gung Memorial Hospital, Lin-Kou Medical Center, including both the Taipei and Taoyuan branches. Correlations between UP and BLLs, BALs, BCLs, and other clinical data were analyzed. (3) Results: Eight hundred and fifty-three patients were recruited. Univariate logistic regressions showed that diabetes mellitus, hepatitis B virus infection, hepatitis C virus infection, HD duration, hemodiafiltration, dialysis clearance of urea, normalized protein catabolic rate, non-anuria, serum albumin levels, log (intact-parathyroid hormone levels), total serum cholesterol levels, serum low-density lipoprotein levels, log (blood aluminum levels), and log (blood lead levels) were associated with UP. Although log BCLs were not significantly associated with UP (p = 0.136) in univariate analysis, we still included log BCLs in multivariate logistic regression to verify their effect on UP given that our aim in this study was to verify associations between serum heavy metals and UP. Multivariate logistic regressions showed that log BLLs (OR: 27.556, 95% CI: 10.912–69.587, p < 0.001) and log BALs (OR: 5.485, 95% CI: 2.985–10.079, p < 0.001) were positively associated with UP. The other logistic regression, which stratified BLLs and BALs into high and low BLLs and BALs, respectively, showed that high BLLs or high BALs (low BLLs and low BALs as reference) (OR: 3.760, 95% CI: 2.554–5.535, p < 0.001) and high BLLs and high BALs combined (low BLLs and low BALs as reference) (OR: 10.838, 95% CI: 5.381–21.828, p < 0.001) were positively correlated with UP. (4) Conclusions: BLLs and BALs were positively correlated with UP. BCLs were not correlated with UP. Clinicians should pay more attention to the environmental sources of lead and aluminum to prevent UP.

Abstract:Background: One of the most common diseases in the world is kidney failure, which can lead to the death of patients. Hemodialysis is a treatment for patients whose kidneys are failing. The water used to perform dialysis must be healthy, safe, and clean. This study aimed to investigate the concentration of heavy metals in hemodialysis water in one of the Hospitals in Iran and compare it with European Pharmacopeia (EPH) and Association for the Advancement of Medical Instrumentation (AAMI) standards. Methods: The present study is a descriptive-analytical study conducted on the inlet water of hemodialysis machines in hospital. The samples were collected for 3 months from June to September 2021, Which was examined in terms of free residual chlorine, electrical conductivity, pH, and calcium, magnesium, sodium, aluminum, zinc, copper, and lead concentration. Metals concentration in hemodialysis water was measured by Inductively Coupled Plasma Mass Spectrometry (ICP-MS) technique. Results: The average value of parameters such as electrical conductivity, pH, residual free chlorine, sodium, calcium, magnesium, zinc, copper and lead in the hemodialysis water was less than the AAMI and EPH standards limits. There was a significant difference at the 95% confidence level with the standard limits, but the aluminum concentration was higher than the standard limits. Also, by examining the medical files of dialysis patients, the most observed problems were anemia and bone diseases, which are probably caused by exposure to high concentrations of aluminum in hemodialysis water. Conclusion: In present study the aluminum concentration is higher than the standard limits. Considering that the higher aluminum concentration can cause diseases such as anemia, bone diseases, nervous deterioration, and death in hemodialysis patients, therefore, it is recommended to continuously evaluate and monitor the quality of hemodialysis water and the performance of its treatment system.
The association between urinary aluminum and lung function among an urban adult population: A repeated-measure longitudinal study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>To investigate the cross-sectional and longitudinal associations between aluminum exposure and lung function and the risk of chronic obstructive pulmonary disease (COPD).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The repeated-measure study was developed with 3917 adults from the Wuhan-Zhuhai cohort and they were followed-up after 3 years and 6 years. Urinary aluminum and lung function were measured at each period. Linear mixed models were used to estimate the exposure–response relationship between urinary aluminum and lung function. COX regression models were used to evaluate the association of urinary aluminum with the risk of COPD.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 6996 observations including 2251 (32.2%) males with a mean age of 54.8 years were included. In the cross-sectional analyses, each 1-unit increase in log-transformed urinary aluminum was associated with a –33.34 mL (95% confidence interval (CI) –45.71 to –20.96) change in forced vital capacity (FVC) and a –17.89 mL (–27.80 to –7.97) change in forced expiratory volume in 1 second (FEV1). The follow-up analyses detected a negative association between urinary aluminum and the annual change of FVC (–6.73 mL/year, 95% CI –10.92 to –2.54), while the association of annual decline of FEV1 with urinary aluminum was statistically insignificant (–2.26 mL/year, –5.76 to 1.23). In the adjusted COX regression model, each 1-unit increase in log-transformed urinary aluminum was associated with a 29% increase in the incident risk of COPD (hazard ratio 1.29, 95% CI 1.04-1.62).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Inclusion</h3><p>Increased urinary aluminum was associated with lung function reduction and the increased risk of COPD in a general urban population.</p>

Abstract:Objectives: To examine reasons behind the failure of the Federal Trade Commission (FTC) to preserve puff count information from standard cigarette testing and to elucidate the importance of puff count to overall tar yields. Methods: We reviewed industry documents on origins of the FTC test and datasets provided by the Tobacco Institute Testing Laboratory to the tobacco industry and FTC for reporting purposes. Results: The majority of the tobacco industry argued for “dual reporting” of tar yields—both per cigarette and per puff. Despite a request from the Tobacco Institute in 1967 that puff count information be preserved, documents and recent communications with the FTC indicate that puff number data have not been maintained by the government. In contrast, for the cigarette industry, puff count data are a fundamental and routine part of testing and important to cigarette design. A sample of puff counts for cigarettes tested in 1996 (n = 471) shows that on average 100 mm cigarettes have 18% more puffs taken on them than do 85 mm cigarettes in standard tests (7.66 vs 9.03; p<0.01). The 10th percentile puff count is 6.8 and the 90th percentile is 8.8 for king size; the 10th percentile puff count is 8.2 and the 90th percentile is 10.0 for 100 mm cigarettes, indicating that puff counts can vary substantially among brands. Conclusions: The FTC has failed to seek or preserve puff count information that the industry finds important. Any standard test of tar and nicotine yields should at minimum preserve puff count information.

Abstract:Introduction: In the scenario of metal toxicity, aluminum (Al) stands out as a ubiquitous type of metal that can be combined with other elements and form different compounds. Al is widely used daily as an adjuvant in vaccines, antacids, food additives (as components of AI-containing food additives), skin care products, cosmetics, and kitchenware, and can be an element or contaminant present in our daily life. Objective: To present a review of the main deleterious effects of Al on human health. Methods: The search was carried out from September 2022 to February 2023 in the Scopus, PubMed, Science Direct, Scielo, and Google Scholar databases, using scientific articles from 2012 to 2023. The quality of the studies was based on the GRADE instrument, and the risk of bias was analyzed according to the Cochrane instrument. Results and Conclusions: A total of 115 files were search returned. Further, 95 articles were evaluated, and 44 were included in this review. Based on the results, measuring Al's relevance to health is essential in medicine. Several studies have demonstrated clinical outcomes and metabolic alterations with Al exposure. The tolerable weekly intake established by the European Food Safety Authority (EFSA) of 1 mg Al/kg body weight can be achieved through dietary exposure alone. Proven neurotoxicity in humans is the critical adverse effect of Al. A carcinogenic effect of Al has not been proven so far. Preventive medicine advocates that exposure to Al should be kept as low as possible. Chelating agents, such as calcium disodium ethylene diamine tetraacetic acid and deferoxamine, are options for acute poisoning, and monomethysilanetriol supplementation may be a long-term strategy with chelation potential. Further studies are needed to assess the impacts of Al on human health.

Abstract:The development of the studies on the aluminum intoxication in the pathology of Alzhmer's disease, Parkinson's disease, dialysis encephalopathy, osteomalacia and anaemia and on the drugs unloading aluminum in human body in recent decades have been reviewed in this paper. The etiologies and pathophysiology of the interested diseases are discussed. Association between aluminium exposure and cognitive functions: A systematic review and meta-analysis

Abstract:INTRODUCTION: Chronic Aluminium (Al) exposure is reported to be linked with neuro-cognitive impairment. However, there is limited synthesized information on the role of chronic Al exposure on individual cognitive domains. This knowledge gap is explored here by systematic review and meta-analysis of the published literature.METHODS: Observational studies that reported the association between Al exposure and cognitive functions were systematically searched in PubMed, Scopus and Embase databases since inception to June 2019 and updated on September 2020. PRISMA guidelines were adhered in this study. Meta-analysis was performed using a random-effect model if the included studies exhibited heterogeneity, in the absence of heterogeneity fixed effect model was used. Heterogeneity was assessed using Cochran-Q test and I<sup>2</sup> statistic. Risk of bias was assessed using the risk of bias in non-randomized studies of exposures. Sub-group analysis and meta regression analysis were explored.RESULTS: Twenty-three studies including 1781 Al exposed and 1186 unexposed were part of the final results. The pooled standardized mean difference of global cognitive scores (-0.65, 95%CI: -1.09 to -0.22, I<sup>2</sup> = 91.09%), memory (-0.45, 95% CI: -0.69 to -0.21, I<sup>2</sup> = 81.67%), working memory (-0.3, 95%CI: -0.45 to -0.15, I<sup>2</sup> = 0%) and processing speed domains were significantly inferior among Al exposed as compared to unexposed. The other evaluated cognitive domains, such as cognitive flexibility, visuospatial abilities and psychomotor functions did not significantly differ between the two groups. We observed a serious risk of bias in most of the included studies.CONCLUSION: Current pieces of evidence suggest an association between chronic Al exposure and impaired cognitive function in majority of domains including memory, processing speed and working memory while no significant influence in other cognitive domains. However, considering high heterogeneity and low quality of primary evidence, further high-quality studies are necessary for conclusive evidence in this regard.

Abstract:Aluminum (Al), one of the three most prevalent metals in the Earth's crust, adversely impacts all metabolic systems of living organisms due to its extensive utilization by humans. It is known that omega-3 fatty acids (ω-3FA) protect the organism against diseases and have positive effects on the immune system. The aim of the study was to investigate the effect of ω-3FA on 8-OH-2-deoxyguanosine (8-OHdG), glutathione (GSH) levels and adenosine deaminase (ADA), paraoxonase (PON), and catalase (CAT) activities in rats with acute aluminum toxicity. The study also aimed to investigate the antioxidant system, as well as Al, zinc (Zn), and iron (Fe) levels. Forty Sprague-Dawley rats (n = 40) were used in the study and the rats were divided into four equal groups (n = 10). In group I, 0.5 mL of 0.9% saline solution (NaCI) was injected intraperitoneally. Group II was injected with 34 mg/kg aluminum chloride (AlCI3) intraperitoneally. Group III received 400 mg/kg ω-3FA for 7 days and group IV received both AlCI3 and 400 mg/kg ω-3FA for 7 days. At the end of the study, blood samples were obtained by cardiac puncture. The findings showed that Al exposure increased serum 8-OHdG and total oxidant status (TOS) levels, as well as ADA activity, which are markers associated with oxidative damage. Conversely, PON and CAT activities, GSH, and total antioxidant status (TAS) levels decreased compared to the control group. Furthermore, Zn and Fe levels decreased as Al levels increased. In conclusion, Al has the capacity to induce oxidative damage and lipid peroxidation, while ω-3 fatty acids may mitigate this damage through a regulatory mechanism. Moreover, ω-3-FA could be used as a therapeutic agent that reduces Al toxicity.
Plasma levels and dietary intake of minerals in patients with type 2 diabetes and chronic kidney disease: a case-control study

Abstract:Background and aim Diabetic kidney disease (DKD) is the primary cause of chronic kidney disease (CKD) worldwide. Altered mineral levels leading to adverse outcomes are widely reported in diabetes but limited in DKD, in the Indian scenario, hence this study was taken up to address this issue Methods A cross-sectional hospital-based case-control study was taken up with 54 healthy controls (C) and 140 subjects with type 2 diabetes wherein 74 subjects with diabetes, and CKD formed the DKD group, and 66 subjects with diabetes, no CKD formed the diabetic no-chronic kidney disease (DNCKD) group. High-resolution inductively coupled plasma mass spectrometry was used to evaluate the blood levels of minerals (calcium (Ca), vanadium (V), chromium (Cr), manganese (Mn), Iron (Fe), cobalt (Co), Copper (Cu), zinc (Zn), and selenium (Se)), and a raw food-based food frequency questionnaire for dietary intakes. Estimated glomerular filtration rate (eGFR) was calculated using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation (mL/min/1.73 m 2 ) and albuminuria. Spearman's rank correlation was used to evaluate the relationship between the categorical variables. Results The median values of plasma Ca in the DKD group were significantly lower compared with the DNCKD and C groups (10.5 mg/dL vs. 11.0 mg/dL and 11.7 mg/dL, p<0.001). Furthermore, plasma Ca levels lowered with declining kidney function, as evidenced by the eGFR and albuminuria segregation. Dietary intake of minerals did not correlate with the corresponding plasma levels. However, in the DKD group, eGFR correlated positively with the plasma levels of Ca (r= 0.422, p=0.001), Cr (r= 0.351, p=0.008), Mn (r= 0.338, p=0.011), Fe (r= 0.403, p=0.002), Cu (r= 0.274, p=0.041) and negatively with Se (r= -0.486, p<0.001). Conclusion Plasma calcium levels are lower in the DKD group with a strong positive association with eGFR, indicating its role in predicting the onset and progression of kidney function decline.

Abstract:The objective of the present study was to review the epidemiological and laboratory evidence on the role of aluminum (Al) exposure in the pathogenesis of cardiovascular diseases. Epidemiological data demonstrated an increased incidence of cardiovascular diseases (CVD), including hypertension and atherosclerosis in occupationally exposed subjects and hemodialysis patients. In addition, Al body burden was found to be elevated in patients with coronary heart disease, hypertension, and dyslipidemia. Laboratory studies demonstrated that Al exposure induced significant ultrastructural damage in the heart, resulting in electrocardiogram alterations in association with cardiomyocyte necrosis and apoptosis, inflammation, oxidative stress, inflammation, and mitochondrial dysfunction. In agreement with the epidemiological findings, laboratory data demonstrated dyslipidemia upon Al exposure, resulting from impaired hepatic lipid catabolism, as well as promotion of low-density lipoprotein oxidation. Al was also shown to inhibit paraoxonase 1 activity and to induce endothelial dysfunction and adhesion molecule expression, further promoting atherogenesis. The role of Al in hypertension was shown to be mediated by up-regulation of NADPH-oxidase, inhibition of nitric oxide bioavailability, and stimulation of renin-angiotensin-aldosterone system. It has been also demonstrated that Al exposure targets cerebral vasculature, which may be considered a link between Al exposure and cerebrovascular diseases. Findings from other tissues lend support that ferroptosis, pyroptosis, endoplasmic reticulum stress, and modulation of gut microbiome and metabolome are involved in the development of CVD upon Al exposure. A better understanding of the role of the cardiovascular system as a target for Al toxicity will be useful for risk assessment and the development of treatment and prevention strategies.

Abstract:Background Chronic kidney disease (CKD) is a global public health problem, resulting in end-stage kidney disease, cardiovascular disease, and premature death. Aim The aim of the study was to determine the profile of essential and toxic trace elements in erythrocytes of patients with end-stage renal disease (ESRD) and their relationship with selected anthropometric and biochemical parameters. Methods The present study compared the profiles of trace elements, including toxic sub-stances, in the erythrocytes of 80 hemodialysis patients with CKD with 40 healthy subjects. All patients had stage 5 CKD. The levels of Cd and Pb were determined by graphite furnace atomic absorption spectrometry and levels of Fe, Mn, Zn, Cu Cr, Ni, and Li by inductively coupled plasma atomic emission spectrometry. Results The ESRD patients demonstrated significantly lower Fe and Zn concentrations and significantly higher Mn and Li and toxic Pb and Cd concentrations in erythrocytes compared to those of the healthy controls. Negative correlations were observed, among others, between the concentrations of Cu, Li, and creatinine; Cu and phosphates; Mn, Pb, and transferrin saturation while positive correlations were noted between Cu, Cr, and transferrin and Pb, Cr, and the normalized protein catabolism rate. Conclusions The higher concentrations of toxic elements present in the erythrocytes of CKD patients might have resulted from the reduced ability of the kidneys to excrete them. Moreover, differences in the concentrations of essential elements (Fe, Mn, Zn) between the two groups indicated that their resorption in the kidneys of CKD patients was impaired. Patients with CKD might benefit from interventions intended to reduce high, toxic concentrations of Pb and Cd and Li and Mn as an alternative supportive treatment. Iron and zinc supplementation should be a component for the treatment of anemia in CKD patients.

Abstract:Background. The kidney is the first target organ of heavy metal toxicity due to its capacity to reabsorb and accumulate divalent metals. Hemodialysis therapy is used to purify the blood of individuals with impaired kidney function. Objective. The aim of the present study was to evaluate the relationship between dialysis fluid quality and the health of hemodialysis patients. Methods. A field sampling program was conducted to collect blood samples from 320 hemodialysis patients (56% males and 44% females) in order to examine the concentrations of heavy metals that typically occur in municipal water in Baghdad (aluminum (Al), copper (Cu), lead (Pb), and zinc (Zn)), and explore associations with the same metals in dialysis fluid collected from four major dialysis centers in Baghdad hospitals for a period of one year (2018). Results. The results showed that the dialysis fluid quality was not in compliance with international standards. The dialysis fluid in 63% of the samples contained high Al concentrations, while Cu and Zn concentrations were within international standards. Lead concentrations were elevated in dialysis fluid in some hospitals as well. Discussion. The average blood levels of biologically important heavy elements were significantly varied in hemodialysis patients when compared with local reference values. Conclusions. Since both deficiency and excess elements are potentially harmful, the hypothesis that heavy element status affects the risk of adverse clinical outcomes is a worthy investigation.
