Abstract:A 71-year-old woman, who was treated with calcmm heparm 5000 units given subcutaneously twice daily, developed complamts of redness and itchmg at the sites of the Injections after I week. Later, large erythematous plaques with central bullae appeared at these sites, followed by a generalized Itchy erythematopapular eruption with fever up to 39°C. The medicament was stopped and the patient was g:tven an antih1stamme. After that, the skm eruption gradually disappeared m about a week. Inqmry revealed that the patient had had a Similar reaction to hepann 5 years earlier, but at that time, the symptoms had not started until after I month of treatment. Skm tests with hepann were performed 3 months later. The results of a patch test with the undiluted preparation were negative, but an mtradermal test with 0.05 ml of vanous dilutiOns (up to 25 U /ml) of a calcmm hepann solution gave an Immediate-type positive reaction, charactenzed by erythema and edema, after 20 mm. Moreover, a delayed reaction to the same dilution, consisting of erythema arid papules, became manifest after 48 h. 10 controls showed negative reactions to the same test.
