Abstract:The molecular subtyping of diffuse gliomas is important. The aim of this study was to establish predictive models based on preoperative multiparametric MRI. A total of 1016 diffuse glioma patients were retrospectively collected from Beijing Tiantan Hospital. Patients were randomly divided into the training (n = 780) and validation (n = 236) sets. According to the 2016 WHO classification, diffuse gliomas can be classified into four binary classification tasks (tasks I–IV). Predictive models based on radiomics and deep convolutional neural network (DCNN) were developed respectively, and their performances were compared with receiver operating characteristic (ROC) curves. Additionally, the radiomics and DCNN features were visualized and compared with the t-distributed stochastic neighbor embedding technique and Spearman’s correlation test. In the training set, areas under the curves (AUCs) of the DCNN models (ranging from 0.99 to 1.00) outperformed the radiomics models in all tasks, and the accuracies of the DCNN models (ranging from 0.90 to 0.94) outperformed the radiomics models in tasks I, II, and III. In the independent validation set, the accuracies of the DCNN models outperformed the radiomics models in all tasks (0.74–0.83), and the AUCs of the DCNN models (0.85–0.89) outperformed the radiomics models in tasks I, II, and III. DCNN features demonstrated more superior discriminative capability than the radiomics features in feature visualization analysis, and their general correlations were weak. Both the radiomics and DCNN models could preoperatively predict the molecular subtypes of diffuse gliomas, and the latter performed better in most circumstances. • The molecular subtypes of diffuse gliomas could be predicted with MRI. • Deep learning features tend to outperform radiomics features in large cohorts. • The correlation between the radiomics features and DCNN features was low.
Prediction of overall survival and molecular markers in gliomas via analysis of digital pathology images using deep learning

Abstract:Cancer histology reveals disease progression and associated molecular processes, and contains rich phenotypic information that is predictive of outcome. In this paper, we developed a computational approach based on deep learning to predict the overall survival and molecular subtypes of glioma patients from microscopic images of tissue biopsies, reflecting measures of microvascular proliferation, mitotic activity, nuclear atypia, and the presence of necrosis. Whole-slide images from 663 unique patients [IDH: 333 IDH-wildtype, 330 IDH-mutants, 1p/19q: 201 1p/19q non-codeleted, 129 1p/19q codeleted] were obtained from TCGA. Sub-images that were free of artifacts and that contained viable tumor with descriptive histologic characteristics were extracted, which were further used for training and testing a deep neural network. The output layer of the network was configured in two different ways: (i) a final Cox model layer to output a prediction of patient risk, and (ii) a final layer with sigmoid activation function, and stochastic gradient decent based optimization with binary cross-entropy loss. Both survival prediction and molecular subtype classification produced promising results using our model. The c-statistic was estimated to be 0.82 (p-value=4.8x10-5) between the risk scores of the proposed deep learning model and overall survival, while accuracies of 88% (area under the curve [AUC]=0.86) were achieved in the detection of IDH mutational status and 1p/19q codeletion. These findings suggest that the deep learning techniques can be applied to microscopic images for objective, accurate, and integrated prediction of outcome for glioma patients. The proposed marker may contribute to (i) stratification of patients into clinical trials, (ii) patient selection for targeted therapy, and (iii) personalized treatment planning.

Abstract:Accurate grading of gliomas is critical to guide therapy and predict prognosis. The presence of microvascular proliferation is a hallmark feature of high grade gliomas which traditionally requires targeted surgical biopsy of representative tissue. Superb microvascular imaging (SMI) is a novel high resolution Doppler ultrasound technique which can uniquely define the microvascular architecture of whole tumours. We examined both qualitative and quantitative vascular features of gliomas captured with SMI, analysing flow signal density, vessel number, branching points, curvature, vessel angle deviation, fractal dimension, and entropy. Results indicate that high-grade gliomas exhibit significantly greater vascular complexity and disorganisation, with increased fractal dimension and entropy, correlating with known histopathological markers of aggressive angiogenesis. The integrated ROC model achieved high accuracy (AUC = 0.95), highlighting SMI's potential as a non-invasive diagnostic and prognostic tool. While further validation with larger datasets is required, this study opens avenues for SMI in glioma management, supporting intraoperative decision-making and informing future prognosis.
Deep learning-based vessel extraction in 3D confocal microscope images of cleared human glioma tissues

Abstract:<p>Comprehensive visualization and accurate extraction of tumor vasculature are essential to study the nature of glioma. Nowadays, tissue clearing technology enables 3D visualization of human glioma vasculature at micron resolution, but current vessel extraction schemes cannot well cope with the extraction of complex tumor vessels with high disruption and irregularity under realistic conditions. Here, we developed a framework, FineVess, based on deep learning to automatically extract glioma vessels in confocal microscope images of cleared human tumor tissues. In the framework, a customized deep learning network, named 3D ResCBAM nnU-Net, was designed to segment the vessels, and a novel pipeline based on preprocessing and post-processing was developed to refine the segmentation results automatically. On the basis of its application to a practical dataset, we showed that the FineVess enabled extraction of variable and incomplete vessels with high accuracy in challenging 3D images, better than other traditional and state-of-the-art schemes. For the extracted vessels, we calculated vascular morphological features including fractal dimension and vascular wall integrity of different tumor grades, and verified the vascular heterogeneity through quantitative analysis.</p>

Abstract:Abstract Gliomas are primary brain tumors that arise from neural stem cells or glial precursors. Diagnosis of glioma is based on histological evaluation of pathological cell features and molecular markers. Gliomas are infiltrated by myeloid cells that accumulate preferentially in malignant tumors and their abundance inversely correlates with survival, which is of interest for cancer immunotherapies. To avoid time-consuming and laborious manual examination of the images, a deep learning approach for automatic multiclass classification of tumor grades was proposed. Importantly, as an alternative way of investigating characteristics of brain tumor grades, we implemented a protocol for learning, discovering, and quantifying tumor microenvironment elements on our glioma dataset. Using only single-stained biopsies we derived characteristic differentiating tumor microenvironment phenotypic neighborhoods. A challenge of the study was given by a small sample size of human leukocyte antigen stained on glioma tissue microarrays dataset - 203 images from 5 classes - and imbalanced data distribution. This has been addressed by image augmentation of the underrepresented classes. For this glioma multiclass classification task, a residual neural network architecture has been adapted. On the validation set the average accuracy was 0.72 when the model was trained from scratch, and 0.85 with the pre-trained model. Moreover, the tumor microenvironment analysis suggested a relevant role of the myeloid cells and their accumulation to characterize glioma grades. This promising approach can be used as an additional diagnostic tool to improve assessment during intra-operative examination or sub-typing tissues for treatment selection, despite the challenges caused by the difficult dataset. We present here the distributions and visualizations of extracted tumor inter-dependencies. Graphical abstract Highlights Research highlight 1: We demonstrate that the ResNet-18 architecture with simple data augmentation trained in 10-fold cross-validation performs the multiclass classification relatively well even with a small imbalanced dataset with a high degree of similarities between classes. Research highlight 2: After supervised subtyping of the tumor, we investigated the usefulness of the protocol for discovery and learning tumor microenvironment elements for the same task. The protocol designed for deriving new biomarkers based on multiplex stained histological samples proved the ability to detect features characteristic of malignant tumors based only on single target stained tissue microarrays. We propose further studies on this topic can help in formulating specific criteria for improvements in diagnosis of gliomas, allowing to avoid the necessity of conducting advanced histopathological analysis or complementing genetic testing of tumor samples.
CNN-based glioma detection in MRI: A deep learning approach

Abstract:BACKGROUND: More than a million people are affected by brain tumors each year; high-grade gliomas (HGGs) and low-grade gliomas (LGGs) present serious diagnostic and treatment hurdles, resulting in shortened life expectancies. Glioma segmentation is still a significant difficulty in clinical settings, despite improvements in Magnetic Resonance Imaging (MRI) and diagnostic tools. Convolutional neural networks (CNNs) have seen recent advancements that offer promise for increasing segmentation accuracy, addressing the pressing need for improved diagnostic and therapeutic approaches. OBJECTIVE: The study intended to develop an automated glioma segmentation algorithm using CNN to accurately identify tumor components in MRI images. The goal was to match the accuracy of experienced radiologists with commercial instruments, hence improving diagnostic precision and quantification. METHODS: 285 MRI scans of high-grade gliomas (HGGs) and low-grade gliomas (LGGs) were analyzed in the study. T1-weighted sequences were utilised for segmentation both pre-and post-contrast agent administration, along with T2-weighted sequences (with and without Fluid Attenuation by Inversion Recovery [FAIRE]). The segmentation performance was assessed with a U-Net network, renowned for its efficacy in medical image segmentation. DICE coefficients were computed for the tumour core with contrast enhancement, the entire tumour, and the tumour nucleus without contrast enhancement. RESULTS: The U-Net network produced DICE values of 0.7331 for the tumour core with contrast enhancement, 0.8624 for the total tumour, and 0.7267 for the tumour nucleus without contrast enhancement. The results align with previous studies, demonstrating segmentation accuracy on par with professional radiologists and commercially accessible segmentation tools. CONCLUSION: The study developed a CNN-based automated segmentation system for gliomas, achieving high accuracy in recognising glioma components in MRI images. The results confirm the ability of CNNs to enhance the accuracy of brain tumour diagnoses, suggesting a promising avenue for future research in medical imaging and diagnostics. This advancement is expected to improve diagnostic processes for clinicians and patients by providing more precise and quantitative results.

Abstract:Glioblastoma presents characteristically with an exuberant, poorly functional vasculature that causes malperfusion, hypoxia and necrosis. Despite limited clinical efficacy, anti-angiogenesis resulting in vascular normalization remains a promising therapeutic approach. Yet, fundamental questions concerning anti-angiogenic therapy remain unanswered, partly due to the scale and resolution gap between microscopy and clinical imaging and a lack of quantitative data readouts. To what extend does treatment lead to vessel regression or vessel normalization and does it ameliorate or aggravate hypoxia? Clearly, a better understanding of the underlying mechanisms would greatly benefit the development of desperately needed improved treatment regimens. Here, using orthotopic transplantation of Gli36 cells, a widely used murine glioma model, we present a mesoscopic approach based on light sheet fluorescence microscopic imaging of wholemount stained tumors. Deep learning-based segmentation followed by automated feature extraction allowed quantitative analyses of the entire tumor vasculature and oxygenation statuses. Unexpectedly in this model, the response to both cytotoxic and anti-angiogenic therapy was dominated by vessel normalization with little evidence for vessel regression. Equally surprising, only cytotoxic therapy resulted in a significant alleviation of hypoxia. Taken together, we provide and evaluate a quantitative workflow that addresses some of the most urgent mechanistic questions in anti-angiogenic therapy.

Abstract:Glioma is a systemic disease that can induce micro and macro alternations of whole brain. Isocitrate dehydrogenase and vascular endothelial growth factor are proven prognostic markers and antiangiogenic therapy targets in glioma. The aim of this study was to determine the ability of whole brain morphologic features and radiomics to predict isocitrate dehydrogenase status and vascular endothelial growth factor expression levels. This study recruited 80 glioma patients with isocitrate dehydrogenase wildtype and high vascular endothelial growth factor expression levels, and 102 patients with isocitrate dehydrogenase mutation and low vascular endothelial growth factor expression levels. Virtual brain grafting, combined with Freesurfer, was used to compute morphologic features including cortical thickness, LGI, and subcortical volume in glioma patient. Radiomics features were extracted from multiregional tumor. Pycaret was used to construct the machine learning pipeline. Among the radiomics models, the whole tumor model achieved the best performance (accuracy 0.80, Area Under the Curve 0.86), while, after incorporating whole brain morphologic features, the model had a superior predictive performance (accuracy 0.82, Area Under the Curve 0.88). The features contributed most in predicting model including the right caudate volume, left middle temporal cortical thickness, first-order statistics, shape, and gray-level cooccurrence matrix. Pycaret, based on morphologic features, combined with radiomics, yielded highest accuracy in predicting isocitrate dehydrogenase mutation and vascular endothelial growth factor levels, indicating that morphologic abnormalities induced by glioma were associated with tumor biology.

Automated Machine-Learning Framework Integrating Histopathological and Radiological Information for Predicting IDH1 Mutation Status in Glioma

Abstract:Diffuse gliomas are the most common malignant primary brain tumors. Identification of isocitrate dehydrogenase 1 (IDH1) mutations aids the diagnostic classification of these tumors and the prediction of their clinical outcomes. While histology continues to play a key role in frozen section diagnosis, as a diagnostic reference and as a method for monitoring disease progression, recent research has demonstrated the ability of multi-parametric magnetic resonance imaging (MRI) sequences for predicting IDH genotypes. In this paper, we aim to improve the prediction accuracy of IDH1 genotypes by integrating multi-modal imaging information from digitized histopathological data derived from routine histological slide scans and the MRI sequences including T1-contrast (T1) and Fluid-attenuated inversion recovery imaging (T2-FLAIR). In this research, we have established an automated framework to process, analyze and integrate the histopathological and radiological information from high-resolution pathology slides and multi-sequence MRI scans. Our machine-learning framework comprehensively computed multi-level information including molecular level, cellular level, and texture level information to reflect predictive IDH genotypes. Firstly, an automated pre-processing was developed to select the regions of interest (ROIs) from pathology slides. Secondly, to interactively fuse the multimodal complementary information, comprehensive feature information was extracted from the pathology ROIs and segmented tumor regions (enhanced tumor, edema and non-enhanced tumor) from MRI sequences. Thirdly, a Random Forest (RF)-based algorithm was employed to identify and quantitatively characterize histopathological and radiological imaging origins, respectively. Finally, we integrated multi-modal imaging features with a machine-learning algorithm and tested the performance of the framework for IDH1 genotyping, we also provided visual and statistical explanation to support the understanding on prediction outcomes. The training and testing experiments on 217 pathologically verified IDH1 genotyped glioma cases from multi-resource validated that our fully automated machine-learning model predicted IDH1 genotypes with greater accuracy and reliability than models that were based on radiological imaging data only. The accuracy of IDH1 genotype prediction was 0.90 compared to 0.82 for radiomic result. Thus, the integration of multi-parametric imaging features for automated analysis of cross-modal biomedical data improved the prediction accuracy of glioma IDH1 genotypes.

Abstract:Glioblastoma (GBM) is a highly angiogenesis malignant tumor with a significant heterogeneity of vascular morphology. The vascular patterns could be served as an indicator of vascular morphological complexity that associated with postoperative prognosis of patients with GBM. Here, multiphoton microscopy (MPM), based on second harmonic generation (SHG) and two-photon excited fluorescence (TPEF), is applied for detecting the architectural feature of blood vessels from ex vivo, unfixed, and unstained human GBM specimens. Combined with the image processing algorithms, the morphology, location, orientation, alignment, and density of vascular collagen are qualitatively and quantitatively visualized, revealing the presented method not only contribute to differentiate GBM from normal brain tissue, but also provide important indicators of different vascular patterns, which may serve as a reference to the prognosis more accurately. Therefore, these results can provide prognosis-related histopathological diagnosis of GBM, holding a translational potential to be used as a label-free, quick, and on-site imaging tool for clinicians to determine the extent of excision involving brain functional areas.

Insight into deep learning for glioma IDH medical image analysis: A systematic review

Abstract:Glioma is the most prevalent primary malignant brain tumor in adults. The World Health Organization currently classifies gliomas (grade I-IV) through a combination of histological observation and genetic molecular detection (including IDH, 1p/19q, and ATRX). [ 1–3 ] Regarding gliomas, a patient's prognosis, diagnosis, and course of therapy are significantly influenced by their molecular genetic status. Certain genetic variations, such as isocitrate dehydrogenase (IDH) and 1p/19q, respond differently to targeted treatment. [ 4 ] Mutations of isocitrate dehydrogenase gene IDH1 and IDH2 are common in diffuse and anaplastic astrocytic tumors, oligodendrocytic tumors, and secondary glioblastoma. [ 5 ] Research indicates that patients with IDH mutations generally experience a more favorable prognosis compared to those with IDH wild type. [ 6 ] Furthermore, knowledge of IDH status may be linked to how well anti-IDH medications and vaccines work in advance. [ 7 ] IDH serves as a crucial biomarker for the diagnosis, treatment, and prognosis of glioma. At present, the gold standard for IDH mutation detection is immunohistochemical (IHC) and high-throughput molecular sequencing. These methods usually require additional reagents and specialized equipment, with professionals required to explain the test results. [ 8 ] These factors significantly impede how quickly doctors can diagnose patients, particularly in places where there is a scarcity of medical personnel. In contrast to histopathological examination, molecular biology detection offers greater objectivity and reliability; Nevertheless, its cost is frequently considerable, and its availability may be limited in less developed economies.

Abstract:Glioblastoma displays morphological heterogeneity on routine H&E whole slide images, with conflicting evidence on the reliability of morphological features in predicting tumor behavior and treatment response. However, current standards for assessing morphology and disease status rely on nonquantitative manual evaluation by pathologists, which are prone to inherent variation. We sought to (1) expand the tools available for quantitative assessment and (2) evaluate the utility of this approach in identifying valuable biomarkers. We trained a deep learning-based algorithm using DenseNet on HALO AI platform for automated segmentation of six tissue classes (heavy infiltration, mild infiltration, mesenchymal tissue, geographic necrosis, palisading necrosis, and hemorrhage) on 3232 labeled annotations from 100 whole slide images. Manual pathologists' estimations from clinical records showed strong correlation with corresponding digital quantifications (p<0.001), and further pixel-wise validation on 828 separate annotations showed high overlap between predicted and ground truth segmentations (precision=0.88, recall= 0.85, F1-score=0.87). We then identified a longitudinal cohort of 115 newly diagnosed glioblastoma tissue paired with the recurrence post-treatment (RT/TMZ), including all 827 H&E whole slide images scanned at 40x. The quantification of longitudinally paired tissue showed a relative increase in geographic necrosis (+62%) and mild infiltration (+110%) and a relative decrease in palisading necrosis (-54%) and heavy infiltration (-35%) between the two timepoints. Patients with rapid clinical progression had more areas of large geographic necrosis (p=0.002), and mesenchymal tissue increase was associated with worse overall survival (p=0.016). Digitally quantified heavy infiltration as a predominant tissue on recurrence outperformed corresponding pathologist estimation of tumor as a predominant tissue on recurrence in stratifying overall-survival (p=0.005 vs p=0.640). Collectively, our AI-powered classifier enables an automated objective quantification of glioblastoma tissue, identifies markers of clinical response, and offers a potential adjunct to enhance routine pathologic reporting of glioblastoma.

Abstract:PURPOSE: Glioblastoma (GBM) is one of the deadliest cancers with no cure. While conventional MRI has been widely adopted to examine GBM clinically, accurate neuroimaging assessment of tumor histopathology for improved diagnosis, surgical planning, and treatment evaluation remains an unmet need in the clinical management of GBMs.EXPERIMENTAL DESIGN: We employ a novel diffusion histology imaging (DHI) approach, combining diffusion basis spectrum imaging (DBSI) and machine learning, to detect, differentiate, and quantify areas of high cellularity, tumor necrosis, and tumor infiltration in GBM.RESULTS: Gadolinium-enhanced T1-weighted or hyperintense fluid-attenuated inversion recovery failed to reflect the morphologic complexity underlying tumor in patients with GBM. Contrary to the conventional wisdom that apparent diffusion coefficient (ADC) negatively correlates with increased tumor cellularity, we demonstrate disagreement between ADC and histologically confirmed tumor cellularity in GBM specimens, whereas DBSI-derived restricted isotropic diffusion fraction positively correlated with tumor cellularity in the same specimens. By incorporating DBSI metrics as classifiers for a supervised machine learning algorithm, we accurately predicted high tumor cellularity, tumor necrosis, and tumor infiltration with 87.5%, 89.0%, and 93.4% accuracy, respectively.CONCLUSIONS: Our results suggest that DHI could serve as a favorable alternative to current neuroimaging techniques in guiding biopsy or surgery as well as monitoring therapeutic response in the treatment of GBM.

Abstract:INTRODUCTION: 5-Aminolevulinic acid (5-ALA) is the most widely used fluorophore in image-guided glioma surgery, and previous work at our institution demonstrated that 5-ALA highlights tumor-associated macrophages (TAMs) in two-photon microscopy images of brain tumor tissue. Using a unique, paired dataset of stimulated Raman histology (SRH) and two-photon images that share one-to-one spatial resolution, we propose a deep-learning approach to identify macrophages from intraoperative SRH images without requiring fluorescent labels. METHODS: We compiled a dataset of 9,554 non-overlapping, 300-by-300 pixel fields of view from paired SRH and two-photon images representing 40 cases of high-grade glioma. A deep generative adversarial network (pix2pix), consisting of a U-Net generator and PatchGAN discriminator, was trained to generate synthetic two-photon images from each SRH patch. The model was trained for 200 epochs, and similarity between the synthetic and real distributions was assessed qualitatively while training and quantitatively using FrÉchet inception distance (FID). RESULTS: Our model was successfully trained to generate synthetic two-photon images nearly indistinguishable from ground truth examples. The FID between a held-out test set of real and synthetic images was 8.58, compared to a mean FID of 9.26 ± 0.24 between randomly sampled sets of real images. Our model consistently inferred the location of brightly fluorescing TAMs in held-out, previously unseen test images. CONCLUSIONS: We used deep learning to visualize TAMs in label-free SRH images. Analysis of TAM infiltration has the potential to identify patients most likely to benefit from immunotherapy clinical trials. Ongoing work will leverage additional deep-learning approaches to automatically identify and quantify TAM infiltration with SRH, enabling rapid patient-specific analysis of the glioma immune microenvironment.

Abstract:(1) Background: Gliomas are the most common primary brain neoplasms accounting for roughly 40–50% of all malignant primary central nervous system tumors. We aim to develop a deep learning-based framework for automated segmentation and prediction of biomarkers and prognosis in patients with gliomas. (2) Methods: In this retrospective two center study, patients were included if they (1) had a diagnosis of glioma with known surgical histopathology and (2) had preoperative MRI with FLAIR sequence. The entire tumor volume including FLAIR hyperintense infiltrative component and necrotic and cystic components was segmented. Deep learning-based U-Net framework was developed based on symmetric architecture from the 512 × 512 segmented maps from FLAIR as the ground truth mask. (3) Results: The final cohort consisted of 208 patients with mean ± standard deviation of age (years) of 56 ± 15 with M/F of 130/78. DSC of the generated mask was 0.93. Prediction for IDH-1 and MGMT status had a performance of AUC 0.88 and 0.62, respectively. Survival prediction of <18 months demonstrated AUC of 0.75. (4) Conclusions: Our deep learning-based framework can detect and segment gliomas with excellent performance for the prediction of IDH-1 biomarker status and survival.

Abstract:Cancer treatment response has traditionally been monitored by measuring tumor volume without considering tumor microvascular heterogeneity (TMH), which is known to profoundly influence treatment outcomes. As TMH presents as distinct vascular subtypes with different functions and structures, we employed super-resolution ultrasound imaging (SRUS) to resolve various tumor vascular subgroups. The feasibility of characterizing tumor vascular subtypes in evaluating anti-angiogenic treatment was also validated. Compared with traditional vascular parameters, the evaluation of multiple vascular subtypes provides a new approach to investigating the complex vascular dynamics induced by antiangiogenic drugs. It is reasonable to believe that tumor microvascular categorization allows the identification of at-risk vascular subgroups with unfavorable functional properties. In summary, this preliminary study of SRUS in the non-invasive characterization of microvascular subtypes sheds light on personalized cancer management.

Abstract:Arachnoid cysts of the spinal canal are relatively common lesions that may be either intra- or extradural. These cysts are usually asymptomatic but may produce symptoms by compressing the spinal cord or nerve roots. We report a case in which an intradural thoracic arachnoid cyst became symptomatic after a routine decompressive lumbar laminectomy for spinal stenosis. Myelography revealed no abnormality, although magnetic resonance imaging and computed tomography after myelography demonstrated a mass within the posterior aspect of the thoracic spinal canal associated with anterior displacement and compression of the spinal cord. A change in the flow dynamics of the cerebrospinal fluid probably allowed the development of spinal cord compression due to one of the following: expansion of the cyst, decreased cerebrospinal fluid buffer between the cord and the cyst, or epidural venous engorgement. A concomitant and more cephalad lesion such as an arachnoid cyst should be considered when myelopathic complications arise after lumbar surgery. Magnetic resonance imaging and computed tomography after myelography are useful to demonstrate the additional pathological processes.
Comprehensive analysis of microglia gene and subpathway signatures for glioma prognosis and drug screening: linking microglia to glioma

Abstract:Glioma is the most common malignant tumors in the brain. Previous studies have revealed that, as the innate immune cells in nervous system, microglia cells were involved in glioma pathology. And, the resident microglia displayed its specific biological roles which distinguished with peripheral macrophages. In this study, an integrated analysis was performed based on public resource database to explore specific biological of microglia within glioma. Through comprehensive analysis, the biological characterization underlying two conditions, glioma microglia compared to glioma macrophage (MicT/MacT) as well as glioma microglia compared to normal microglia (MicT/MicN), were revealed. Notably, nine core MicT/MicN genes displayed closely associations with glioma recurrence and prognosis, such as P2RY2, which was analyzed in more than 2800 glioma samples from 25 studies. Furthermore, we applied a random walk based strategy to identify microglia specific subpathways and developed SubP28 signature for glioma prognostic analysis. Multiple validation data sets confirmed the predictive performance of SubP28 and involvement in molecular subtypes. The associations between SuP28 score and microglia M1/M2 polarization were also explored for both GBM and LGG types. Finally, a comprehensive drug-subpathway network was established for screening candidate medicable molecules (drugs) and identifying therapeutic subpathway targets. In conclusions, the comprehensive analysis of microglia related gene and functional signatures in glioma pathobiologic events by large-scale data sets displayed a framework to dissect inner connection between microglia and glioma, and identify robust signature for glioma clinical implications.
Automated Grading of Glioma Based on Density and Atypia Analysis in Whole Slide Images

Abstract:Glioma is the most common malignant brain tumor and classification of low grade glioma (LGG) and high grade glioma (HGG) is an important reference of making decisions on patient treatment options and prognosis. This work is largely done manually by pathologist based on an examination of whole slide image (WSI), which is arduous and heavily dependent on doctors' experience. In the World Health Organization (WHO) criteria, grade of glioma is closely related to hypercellularity, nuclear atypia and necrosis. Inspired by this, this paper designed and extracted cell density and atypia features to classify LGG and HGG. First, regions of interest (ROI) were located by analyzing cell density and global density features were extracted as well. Second, local density and atypia features were extracted in ROI. Third, balanced support vector machine (SVM) classifier was trained and tested using 10 selected features. The area under the curve (AUC) and accuracy (ACC) of 5-fold cross validation were 0.92 ± 0.01 and 0.82 ± 0.01 respectively. The results demonstrate that the proposed method of locating ROI is effective and the designed features of density and atypia can be used to predict glioma grade accurately, which can provide reliable basis for clinical diagnosis.

Abstract:Background Gliomas are the most common primary brain tumours and constitute approximately half of all malignant glioblastomas. Unfortunately, patients diagnosed with malignant glioblastomas typically survive for less than a year. In light of this circumstance, genotyping is an effective means of categorising gliomas. The Ki67 proliferation index, a widely used marker of cellular proliferation in clinical contexts, has demonstrated potential for predicting tumour classification and prognosis. In particular, magnetic resonance imaging (MRI) plays a vital role in the diagnosis of brain tumours. Using MRI to extract glioma-related features and construct a machine learning model offers a viable avenue to classify and predict the level of Ki67 expression. Methods This study retrospectively collected MRI data and postoperative immunohistochemical results from 613 glioma patients from the First Affliated Hospital of Nanjing Medical University. Subsequently, we performed registration and skull stripping on the four MRI modalities: T1-weighted (T1), T2-weighted (T2), T1-weighted with contrast enhancement (T1CE), and Fluid Attenuated Inversion Recovery (FLAIR). Each modality's segmentation yielded three distinct tumour regions. Following segmentation, a comprehensive set of features encompassing texture, first-order, and shape attributes were extracted from these delineated regions. Feature selection was conducted using the least absolute shrinkage and selection operator (LASSO) algorithm with subsequent sorting to identify the most important features. These selected features were further analysed using correlation analysis to finalise the selection for machine learning model development. Eight models: logistic regression (LR), naive bayes, decision tree, gradient boosting tree, and support vector classification (SVM), random forest (RF), XGBoost, and LightGBM were used to objectively classify Ki67 expression. Results In total, 613 patients were enroled in the study, and 24,455 radiomic features were extracted from each patient's MRI. These features were eventually reduced to 36 after LASSO screening, RF importance ranking, and correlation analysis. Among all the tested machine learning models, LR and linear SVM exhibited superior performance. LR achieved the highest area under the curve score of 0.912 ± 0.036, while linear SVM obtained the top accuracy with a score of 0.884 ± 0.031. Conclusion This study introduced a novel approach for classifying Ki67 expression levels using MRI, which has been proven to be highly effective. With the LR model at its core, our method demonstrated its potential in signalling a promising avenue for future research. This innovative approach of predicting Ki67 expression based on MRI features not only enhances our understanding of cell activity but also represents a significant leap forward in brain glioma research. This underscores the potential of integrating machine learning with medical imaging to aid in the diagnosis and prognosis of complex diseases.
