Abstract:Animal health care is being delivered by an increasing number of professions and occupations. This article takes an inward look at the veterinary team, focusing on the day-to-day life of veterinarians and those with whom they work, such as veterinary technicians/nurses, physical therapists, and farriers. The evolution of the work of the veterinarian should be explored alongside the evolution of closely related occupations, as the current working practice of veterinarians is affected by the practice of these other occupations. An example is given of UK veterinary nurses (similar to veterinary technicians in North America) who are currently undergoing professionalization. Seminal implementations have included a register (2007), disciplinary procedures (2011), a declaration of professional responsibilities (2012), and required continuing professional development for registered veterinary nurses (RVNs). These implementations result in veterinary nurses who are now accountable for their actions. There are many potential benefits to good inter-professional practice for the practice itself, individual team members, clients, and patients, including better results produced by the whole team rather than the sum of the parts, financial benefits to using individuals in lower-paid occupations for shared roles, and greater client satisfaction regarding increased options for treatment. There are, however, many challenges to inter-professional working that center on the interlinked themes of hierarchy (power, status, and the understanding/appreciation of professional roles) and communication (lack of/poor). Inter-professional education (IPE) is suggested as a potential means to overcome these challenges; however, research into IPE exclusively related to the veterinary team is lacking.

Abstract:While teaching medical professionalism has been an important aspect of medical education over the past two decades, the recent emergence of professional identity formation as an important concept has led to a reexamination of how best to ensure that medical graduates come to "think, act, and feel like a physician." If the recommendation that professional identity formation as an educational objective becomes a reality, curricular change to support this objective is required and the principles that guided programs designed to teach professionalism must be reexamined. It is proposed that the social learning theory communities of practice serve as the theoretical basis of the curricular revision as the theory is strongly linked to identity formation. Curricular changes that support professional identity formation include: the necessity to establish identity formation as an educational objective, include a cognitive base on the subject in the formal curriculum, to engage students in the development of their own identities, provide a welcoming community that facilitates their entry, and offer faculty development to ensure that all understand the educational objective and the means chosen to achieve it. Finally, there is a need to assist students as they chart progress towards becoming a professional.
Communication identity in veterinary medicine: a grounded theory approach

Abstract:It is well established that communication skills are a core competence in veterinary medicine. Most studies in the field of veterinary medicine have focused on communication as an interpersonal skill. Much less is known about communication in the context of professional identity formation. Semistructured interviews with practitioners from companion and farm animal practice, veterinary industry, veterinary research and government service were conducted in Germany in 2016. A grounded theory approach as described by Strauss and Corbin was used to identify characteristics associated with professional communication identity formation in veterinarians. According to the current study, the identity formation process occurs in three steps: existing personal communication identity, socialisation and professional communication identity. Essentials of interpersonal communication, communication interaction experiences at work, acquisition of communication skills and subjective clarification of veterinary communication ability and skills are the key factors associated with this formation process. Since communication skills are of uttermost importance for all fields of veterinary medicine, communication education, and supporting undergraduate students in the process of communication identity formation, should be an important part of the veterinary curriculum. Furthermore, integrating communication skills training in continuing education courses could foster professionalism in veterinary medicine.
Effects of β-2 Agonist on Hepatic Glycogen Metabolism in the Fetal Lamb

Abstract:ABSTRACT: To determine the effects of fetal β-2 agonist exposure on fetal hepatic glycogen metabolism, we infused ritodrine at a rate of 1.3 ± 0.4 μg/kg/min (mean ± SD) for 24 h into six chronically catheterized twin fetal lambs starting between 128 and 134 days gestation. The control twins received 0.9% saline at 1.2 ± 0.12 ml/kg/h. In addition, 15 uncatheterized fetuses were killed between 115 and 148 days gestation as unoperated controls. Ritodrine infusion produced a 1.7-fold elevation in fetal serum glucose level, from 23 ± 5 to 42 ± 15 mg/dl, and a 2-fold elevation in serum insulin level, from 16 ± 5 to 34 ± 8 mg/ml, p < 0.01. Hepatic glycogen content increased 7-fold in the uncatheterized controls between 115 and 148 days gestation (r = 0.9, p < 0.001). Ritodrine infusion reduced hepatic glycogen content by 50% from 179 ± 19 μg/mg in twin controls to 90 ± 25 μg/mg in the ritodrine-infused twins, p < 0.001. Hepatic glycogen phosphorylase kinase activity was elevated 1.3-fold from 0.149 ± 0.100 mU/mg protein in control twins to 0.186 ± 0.007 mU/mg protein in the ritodrine infused twins, p < 0.001. Glycogen phosphorylase a activity was also increased 1.4-fold from 8.60 ± 0.76 nM NADPH/min/mg protein in control twins to 11.85 ± 0.68 nM NADPH/min/mg protein in the ritodrine infused twins, p < 0.001. There were no significant differences in hepatic total glycogen phosphorylase (a + b) activity, active (I) or total I + D) glycogen synthase activity, protein, or DNA content between the ritodrine treated and control twins or between the control twins and the uncatheterized control fetuses. We conclude that β-2 agonist depletes hepatic glycogen in fetal lambs through activation of glycogen phosphorylase by glycogen phosphorylase kinase. Depletion of hepatic glycogen may be an unrecognized side effect of fetal β-2 agonist exposure.

Abstract:Drawing on extensive evidence and experience in human medicine, this article offers a practical conceptual framework for thinking more precisely about how to teach and learn communication systematically and intentionally in veterinary medicine. The overarching goal is to promote the development of communication programs so as to improve communication in veterinary practice to a professional level of competence. A three-part conceptual framework is presented that first explores the rationale behind teaching and learning communication, including the evidence base regarding the impact of communication on clinician–client interactions and outcomes of care and the research on teaching and learning communication skills in medicine. The second part considers four ways to conceptualize what to teach and learn, as explicated by (a) the domains of communication in veterinary medicine; (b) “first principles” of effective communication; (c) evidence-based goals or outcomes for communication programs; and (d) delineation and definition of the specific individual skills that research evidence supports, as presented in the Calgary-Cambridge Guides. The last part of the conceptual framework examines how to teach communication, including the use of models, a primary focus on skill development as the backbone of communication programs, and the value of other methods supported by the evidence, such as simulated patients, videotape, small groups, and feedback and facilitation skills. Communication impacts the clinician–client interaction and outcomes of care in very significant ways. Communication can and should be taught and learned with as much rigor as other aspects of clinical competence. Veterinary programs at all levels should include the teaching of communication.

Abstract:Introduction: In veterinary education, reflection and reflective portfolio learning aim to enhance professional development. Although reflection and reflective portfolio learning are widely used in teaching and healthcare, their demonstrable impact on veterinary education is unclear. Although the benefits are uncertain, reflection may provide potential for self-development and help to prepare students for clinical practice. The aim of this work was to review research into reflective practice and reflective portfolio learning, to find evidence for its use in teaching, to confirm how it is best implemented to optimise professional development and to identify gaps for future research. Methods: This scoping review was conducted according to the Joanna Briggs Institute guidelines adhering to review methodology. Ten databases were searched and screened for reflection and portfolio learning as a primary source of data. Of 29,933 texts identified by title and/or abstract, 51 final works were screened in full. Of these, 16 papers that gathered evidence on reflection were included. Results: The results confirmed a growing evidence base for reflection in the veterinary field. In total, six works demonstrated a positive impact on professional development. Written reflective assignments were the most common methods of implementation, with 11 papers referencing their use. Other methods included group discussion and facilitated reflection. Conclusions: This review identified numerous gaps for research, including validating the methods of implementation, exploring the efficacy of methods other than written reflection, examining how reflection is used at different stages of the veterinary career and study of different models of reflection to identify which are most useful.

Abstract:This paper presents recent results obtained applying the Model Based Infrared Reflectometry (MBIR) technique as an in-line monitoring technique for high aspect ratio structures. The paper will focus on the description of the metrology development and itsimplementation in a manufacturing environment for the particular case of few microns Deep Trench Isolatio n (DTI) structures. The technique is demonstrated to be robust method for the in-line geometry control of etched structures. Experimental data relating to high aspect ratio Through Silicon Via (TSV) structures will be presented as well. The results confirm that the technique is very sensitive to the geometry even for small Via of diameter down to 3 microns.

Abstract:The taxometric procedures developed by Paul Meehl and his colleagues have been used in a large body of research seeking to distinguish between categorical and continuous models of latent variables. In this article the authors survey taxometric studies of psychopathological, personality, and other variables and draw conclusions about the taxonicity of latent variables in these domains. In addition, the authors review research on the validation and refinement of taxometric methods and make proposals for improving the application of taxometric research. The authors consider questions that remain about the conceptual status of taxometrics and raise new ones. They show that taxometric methodology has made an accelerating number of contributions to psychological research, has resolved several longstanding controversies, and has challenged some entrenched theoretical assumptions in differential psychology. Moreover, they contend that the research possibilities that it affords have yet to be fully exploited.
Beyond NAVMEC: competency-based veterinary education and assessment of the professional competencies

Abstract:The implementation of competency-based curricula within the health sciences has been an important paradigm shift over the past 30 years. As a result, one of the five strategic goals recommended by the North American Veterinary Medical Education Consortium (NAVMEC) report was to graduate career-ready veterinarians who are proficient in, and have the confidence to use, an agreed-upon set of core competencies. Of the nine competencies identified as essential for veterinary graduates, seven could be classified as professional or non-technical competencies: communication; collaboration; management (self, team, system); lifelong learning, scholarship, value of research; leadership; diversity and multicultural awareness; and adaptation to changing environments. Traditionally, the professional competencies have received less attention in veterinary curricula and their assessment is often sporadic or inconsistent. In contrast, the same or similar competencies are being increasingly recognized in other health professions as essential skills and abilities, and their assessment is being undertaken with enhanced scrutiny and critical appraisal. Several challenges have been associated with the assessment of professional competencies, including agreement as to their definition and therefore their evaluation, the fact that they are frequently complex and require multiple integrative assessments, and the ability and/or desire of faculty to teach and assess these competencies. To provide an improved context for assessment of the seven professional competencies identified in the NAVMEC report, this article describes a broad framework for their evaluation as well as specific examples of how these or similar competencies are currently being measured in medical and veterinary curricula.
Professional Identity Formation in Allied Health: A Systematic Review with Narrative Synthesis

Abstract:Phenomenon: Professional identity formation is a key component of health professional education. Changing expectations of healthcare requires more than competencies, but also an ingrained responsibility to patients, with our values and behaviors aligned with community expectations of patient-centered health professionals. Research into professional identity formation has focused on nursing and medical training, and, although allied health professionals make up one third of the workforce, research is uneven across these disciplines. Health professions educators from these under-researched disciplines have less guidance for how to support students' professional identity, meaning students may graduate with less mature professional identities. This systematic literature review synthesizes the research on professional identity formation across nine similar allied health disciplines. The purpose is to guide health professions educators in the formation of professional identity in allied health students. Approach: We carried out a systematic literature review, registered on Prospero, following the PRIMSA framework, to find, appraise, and synthesize research on professional identity for nine allied health professions. We synthesized extracted data using a narrative synthesis with convergent qualitative meta-integration of qualitative and quantitative data. Results: Our database searches combined with ancestry and forward searching resulted in 70 studies that met inclusion criteria. Studies came from 17 countries, from seven of the included disciplines (art therapy, dietetics, occupational therapy, physiotherapy, podiatry, psychology, and speech pathology) and from undergraduate, postgraduate, and new graduate perspectives. We found professional identity was influenced by the places of learning (curriculum and clinical experiences or placements), interactions with people (relationships with academics, peers, supervisors, patients,and role models), and educational practices engaged (dialogue, reflection, processing challenges), each acting on the individual person (student or new graduate) who also brought unique characteristics and experiences. Insights: This model of influences can inform health professions education to enable students to develop and graduate with a stronger professional identity formation.

Abstract:Introduction Assessing service quality is one of the basic requirements to develop the medical tourism industry. There is no valid and reliable tool to measure service quality of medical tourism. This study aimed to determine the reliability and validity of a Persian version of medical tourism service quality questionnaire for Iranian hospitals. Methods To validate the medical tourism service quality questionnaire (MTSQQ), a cross-sectional study was conducted on 250 Iraqi patients referred to hospitals in Ahvaz (Iran) from 2015. To design a questionnaire and determine its content validity, the Delphi Technique (3 rounds) with the participation of 20 medical tourism experts was used. Construct validity of the questionnaire was assessed through exploratory and confirmatory factor analysis. Reliability was assessed using Cronbach’s alpha coefficient. Data were analyzed by Excel 2007, SPSS version18, and Lisrel l8.0 software. Results The content validity of the questionnaire with CVI=0.775 was confirmed. According to exploratory factor analysis, the MTSQQ included 31 items and 8 dimensions (tangibility, reliability, responsiveness, assurance, empathy, exchange and travel facilities, technical and infrastructure facilities and safety and security). Construct validity of the questionnaire was confirmed, based on the goodness of fit quantities of model (RMSEA=0.032, CFI= 0.98, GFI=0.88). Cronbach’s alpha coefficient was 0.837 and 0.919 for expectation and perception questionnaire. Conclusion The results of the study showed that the medical tourism SERVQUAL questionnaire with 31 items and 8 dimensions was a valid and reliable tool to measure service quality of medical tourism in Iranian hospitals.

Abstract:Anatomy contact time reductions, in favour of early clinical exposure and professionalism coursework, remain trending in healthcare education. These efforts, have not necessarily improved student classroom to clinic transitions, and may have negative impacts on acquisition of knowledge fit‐for‐practice. In an effort to improve clinical integration and transitions from classroom learning to clinical environments, we evaluated the impact of anatomy education on healthcare students' professional identity and role formation. Using a longitudinal qualitative study approach with Monash University physiotherapy and medicine student participants, we probed students' perceptions regarding the impact of anatomy education on their professional identity development through serial online discussion forums during semester and delved deeper into these discussions by engaging participants in interprofessional focus groups at the end of each semester. Preliminary Framework analysis was undertaken by the research team to identify themes. Preliminary results indicate that there are six themes related to professional identity (PI) and role (PR) implicit in anatomy education: (1) Process, (2) Peer Relationships, (3) Educator Relationships, (4) Ethics, (5) Tolerance of Ambiguity (ToA) and (6) Contexts. Interestingly, some of these themes changed with time and teaching methods (e.g. Ethics), while others appear to be impeded by anatomy education (ToA). Integration of professional development into anatomy education is not only possible, it is an existing (albeit sometimes implicit) component of current teaching practices. Understanding the hidden and informal anatomy curriculum and its impact on PI and PR will not only serve to inform current practices, but it may also help healthcare students improve the ease of transitions (from classroom to clinics to practice). This work will serve as a foundation for builidng a framework which fosters purposeful and explicit integration between anatomy learning and PI/PR development. Together this will foster easier transitions into the clinic by providing students a platform for PI development early in their educational progression. Abstract:The use of small-group collaborative case-based learning methodologies has been growing both in interest and implementation across veterinary college curriculums in recent years. The ability of this pedagogical approach to solidify and deepen learning outcomes is well-established in the broader education literature. However, to achieve this positive impact, students must interact in productive discussions that expand the scope of their understanding. The present study focused on analyzing the ways in which professional veterinary students interact as they work collaboratively through clinical cases, in the context of a Team-Based Learning intensive curriculum. This data was used to draw connections between the questions posed in the clinical case activities and the resulting intragroup collaborative outcomes, which can assist veterinary educators in sparking more robust student discussions through facilitation and instructional design. Fourteen participants formed two student groups that worked on 49 case questions across 5 sessions, providing 98 episodes of collaboration for analysis. The findings of this study revealed how professional veterinary students negotiated perspectives to come to consensus on in-class case-based learning tasks, including eight primary types of statements they made and seven overall patterns of group collaboration. This study highlighted specific elements of instructional design that influenced student collaboration including: allowing for multiple perspectives, sparking disagreement, perceived difficulty, learning outcome level, and the level of consensus required by the question structure. We present specific recommendations for veterinary educators to consider while designing questions for veterinary student groups.

Abstract:Widespread pandemic-related disruption has led to increasingly challenging client interactions. TheBritish Veterinary Association (BVA) found that 57% of veterinary staff surveyed in 2021 reported feeling intimidated by client behaviour during the previous year; a 10% increase from 2019. The psychological impact of consistently difficult or abusive interactions can be significant and contribute to the high incidences of stress, burnout, attrition and potentially suicide increasingly noted in the industry. This understanding further highlights the need to focus on developing individual self-care strategies and leaders offering appropriate support to their team. Historically, veterinary education has overlooked the importance of training in interpersonal skills, such as communication, conflict resolution and emotional intelligence. However, human and veterinary medicine is evolving with increasing recognition of the significance of communication skills training to help prevent and manage of challenging client behaviour. This may help to ensure practitioners are better prepared for the challenges that await them.

Abstract:Educational theory can and should form the basis for teaching in veterinary medicine. Nevertheless, formal training for teachers in veterinary medicine is uncommon and rarely includes study of different educational theories or perspectives, leaving educators to rely on informal or "hidden" educational constructs to guide them in their everyday teaching. Using a modified case-based format, we present critiques of a hypothetical teaching scenario from four different educational viewpoints: behaviorist, cognitive, social learning, and inspired teaching approaches. The importance and utility of formal educational theory in faculty development is discussed.
Measuring Veterinarian Professions' Readiness for Interprofessional Learning in a Pre- and Post-Intervention Study

Abstract:The integration of interprofessional collaboration is becoming increasingly crucial in veterinary care settings, emphasising the need for interprofessional education (IPE) in veterinary programmes. This study explores the readiness for interprofessional learning among German veterinary students, apprentices and related occupations before and after an interprofessional communication course. It assesses the impact of this course on the participants' attitudes using the Readiness for Interprofessional Learning Scale (RIPLS). The course, offered in two iterations, combined asynchronous online modules, live seminars and practical training elements. The RIPLS was administered before and after the course to gauge attitude shifts towards interprofessional learning. Statistical analyses, including McNemar, Cohen's Kappa and exact Fisher tests, were employed to compare pre- and post-test responses. Despite challenges in participant linking, significant findings emerged between the student and apprentice groups in specific areas of the RIPLS, notably in the "Professional Identity" subscale post-course. However, correlations between face-to-face contact and RIPLS ratings were not observed, suggesting a need for more integrated interprofessional learning experiences. While some limitations in sample size and profession distribution hinder generalisability, this study indicates a high receptiveness to interprofessional learning in veterinary education, emphasising the potential for attitude changes with more interactive participation and programme adjustments.
