Abstract:AIM: To evaluate the effects of long-term, moderate level noise exposure during crucial periods of rat infants on stereological parameters of medial geniculate body (MGB) and auditory cortex.MATERIALS AND METHODS: Twenty-four male offspring of 12 pregnant rats were divided into four groups: fetal-to-critical period group, which were exposed to noise from the last 10 days of fetal life till postnatal day (PND) 29; fetal period group that exposed to noise during the last 10 days of fetal life; critical period group, exposed to noise from PND 15 till PND 29, and control group. White noise at 90 dB for 2 h per day was used.STATISTICAL ANALYSIS USED: Variance for variables was performed using Proc GLM followed by mean comparison by Duncan's multiple range test.RESULTS: Numerical density of neurons in MGB of fetal-to-critical period group was lower than control group. Similar results were seen in numerical density of neurons in layers IV and VI of auditory cortex. Furthermore, no significant difference was observed in the volume of auditory cortex among groups, and only MGB volume in fetal-to-critical period group was higher than other groups. Estimated total number of neurons in MGB was not significantly different among groups.CONCLUSION: It seems necessary to prevent long-term moderate level noise exposure during fetal-to-critical neonatal period.

THE RESPONSES OF TERM FETUSES TO DIFFERENT ACOUSTIC STIMULATIONS

Abstract:Objective To explore the characteristics of fetal movement and FHR variation elicited by acoustic stimulation and whether acoustic stimulation can replace vibroacoustic stimulation. Methods Ninety-four and seventy-six normal pregnant women for antenatal visit were recruited from the Woman's Hospital, School of Medicine, Zhejiang University from April 2002 to February 2003. All subjects were divided into 5 groups to be exposed to five different intensities of acoustic stimulations at 95,100,105,110,115 dB respectively and self-control of blank and vibroacoustic stimulation were designed. The fetal movements and FHR were recorded during the study. Results (1) The percentage of fetal movement evoked by 95,100,105,110,115 dB airborne sound was 15% , 89%, 83% , 83% , 95% respectively. The total percentage of fetal movement evoked by vibroacoustic stimulation was 99% for all subjects. The percentages of evoked fetal movement by 100,105,110,115 dB airborne sound were not significantly different from those by vibroacoustic stimulation. (2)The percentages of FHR acceleration ≥15 bpm in 100,105,110 and 115 dB airborne sound groups were 39%, 61%, 56% and 85%, respectively, while 92% for all cases evoked by vibroacoustic stimulation was significantly higher than those evoked by 100,105 and 110 dB airborne sound group but with no significant difference to 115 dB airborne sound group. (3)The peak value in FHR evoked by 95,100,105, 110 and 115 dB airborne sound were -4. 5 bpm, 12 bpm, 17 bpm, 14 bpm and 20. 5 bpm, respectively. The peak FHR acceleration evoked by vibroacoustic stimulation was 23 bpm which was significantly higher than those by 100,105,110 dB airborne sound and no significant difference was detected between 115 dB airborne sound and vibroacoustic stimulation group. (4)Compared with 115 dB airborne sound, vibroacoustic stimulation evoked significantly longer duration of FHR tachycardia (42. 5 s vs 5 s, P = 0. 011) and fetal movement (270 s vs 100 s, P = 0. 000). Conclusions Acoustic stimulation at 115 dB is able to elicit efficient fetal movement and FHR acceleration without prolonged tachycardia, fetal behavioral disorganization or excessive fetal movement and is reasonable to replace vibroacoustic stimulation for awaking fetuses combined with NST.

Abstract:Importance Pregnant women are exposed to both occupational and environmental noise during their pregnancy. The association between noise and adverse health outcomes is well known. Less is known about the relationship between noise and its effects on the embryo/fetus and pregnancy. Objectives The purpose of the study is to review what is known about the effect(s) of environment and occupational noise during pregnancy on maternal and perinatal outcomes. Evidence Acquisition Electronic databases (PubMed, CINAHL, and Embase) were searched from 1995–2023 with the only limitation being that the articles were in English. Studies were selected that examined associations between environmental and occupational noise and pregnancy outcome, maternal outcome, or perinatal outcome. Results There were 233 articles identified. After reviewing all abstracts and selected full texts, 25 publications were used as the basis of this review. Multiple studies have been undertaken evaluating the effects of noise on embryonal/fetal growth, fetal development, maternal hypertension, gestational diabetes, and maternal anxiety and depression. The overall effects of occupational and environmental exposure on both fetal and maternal outcomes remain uncertain. Conclusions Further high-quality studies are needed to determine the association between noise and pregnancy outcomes. Relevance Even though this review suggests a relationship between noise and maternal/fetal outcomes, confirmation will require well designed future studies. Target Audience Obstetricians and gynecologists, family physicians. Learning Objectives After completing this learning activity, the participant should be able to explain how sound is measured, the thresholds of sound in children and adults, and exposure levels that result in hearing loss; describe the effects of exposure to noise during pregnancy and its effects on adverse pregnancy outcomes; and discuss the effects of environmental and occupational noise on the developing embryo/fetus.

Sound and the developing infant in the NICU: conclusions and recommendations for care

Abstract:Objective: To present the conclusions and associated recommendations for care developed by the Physical and Developmental Environment of the High-Risk Infant Center, Study Group on Neonatal Intensive Care Unit (NICU) Sound, and the Expert Review Panel. Study design: A multidisciplinary group of clinicians and researchers reviewed the literature regarding the effect of sound on the fetus, newborn, and preterm infant and developed recommendations based on the best evidence. An Expert Review Panel reviewed the data and conclusions. Results: The following recommendations are developed from the review of the literature and are clarified in the body of the article. (1) Women should avoid prolonged exposure to low-frequency sound levels (< 250 Hz) above 65 dB(A) during pregnancy. (2) Earphones or other devices for sound production should not be used directly attached to the pregnant woman's abdomen. (3) The voice of the mother during normal daily activities, along with the sounds produced by her body and those present in her usual surroundings, is sufficient for normal fetal auditory development. The fetus does not require supplemental stimulation. Programs to supplement the fetal auditory experience cannot be recommended. (4) Infant intensive care units should incorporate a system of regular noise assessment. (5) Sound limit recommendations are to maintain a nursery with an hourly Leq of 50 dB(A), an hourly L10 of 55 dB(A) and a 1-second Lmax of 70 dB(A), all A-weighted, slow response scale. (6) Infant intensive care units should develop and maintain a program of noise control and abatement in order to operate within the recommended permissible noise criteria. (7) Care practices must provide ample opportunity for the infant to hear parent voices live in interaction between parent and infant at the bedside. (8) Earphones and other devices attached to the infant's ears for sound transmission should not be used at any time. (9) There is little evidence to support the use of recorded music or speech in the environment of the high-risk infant. Audio recordings should not be used routinely or left unattended in the environment of the high-risk infant. Conclusion: The recommendations, if followed, should provide an environment that will protect sleep, support stable vital signs, improve speech intelligibility for the infant, and reduce potential adverse effects on auditory development.
Influence of Prenatal Noise Exposure on the Auditory Object Exploration Behavior of Adult Rats

Abstract:Environmental noise has been considered as one of the important factors to influence health,however,few studies have been conducted regarding the prenatal noise exposure on the auditory behavior of adult.The present study compares the auditory object exploration behavior determined in water maze apparatus among three different groups of rats,i.e.,prenatal noise exposure group,adult noise exposure group,and normal control group.Rats were trained to perform auditory object exploration task from the 11th week after birth and the test lasted for 17 consecutive days.The auditory object exploration behavior of the three groups of rats was compared by the following indexes:the latency and the rate of success for finding the platform,as well as the category of movement tracks.The results indicate that noise exposure can deficit the auditory object exploration behavior of rats,and noise exposure during prenatal period has stronger influence on the adult auditory behavior than noise exposure during adult period does.The results suggest that it is necessary for human and animals to be away from hazardous noise during pregnancy.
Auditory Effects of Acoustic Noise From 3‐T Brain MRI in Neonates With Hearing Protection

Abstract:Background Neonates with immature auditory function (eg, weak/absent middle ear muscle reflex) could conceivably be vulnerable to noise‐induced hearing loss; however, it is unclear if neonates show evidence of hearing loss following MRI acoustic noise exposure. Purpose To explore the auditory effects of MRI acoustic noise in neonates. Study Type Prospective. Subjects Two independent cohorts of neonates (N = 19 and N = 18; mean gestational‐age, 38.75 ± 2.18 and 39.01 ± 1.83 weeks). Field Strength/Sequence T1‐weighted three‐dimensional gradient‐echo sequence, T2‐weighted fast spin‐echo sequence, single‐shot echo‐planar imaging‐based diffusion‐tensor imaging, single‐shot echo‐planar imaging‐based diffusion‐kurtosis imaging and T2‐weighted fluid‐attenuated inversion recovery sequence at 3.0 T. Assessment All neonates wore ear protection during scan protocols lasted ~40 minutes. Equivalent sound pressure levels (SPLs) were measured for both cohorts. In cohort1, left‐ and right‐ear auditory brainstem response (ABR) was measured before (baseline) and after (follow‐up) MRI, included assessment of ABR threshold, wave I, III and V latencies and interpeak interval to determine the functional status of auditory nerve and brainstem. In cohort2, baseline and follow‐up left‐ and right‐ear distortion product otoacoustic emission (DPOAE) amplitudes were assessed at 1.2 to 7.0 kHz to determine cochlear function. Statistical Test Wilcoxon signed‐rank or paired t‐tests with Bonferroni's correction were used to compare the differences between baseline and follow‐up ABR and DPOAE measures. Results Equivalent SPLs ranged from 103.5 to 113.6 dBA. No significant differences between baseline and follow‐up were detected in left‐ or right‐ear ABR measures (P > 0.999, Bonferroni corrected) in cohort1, or in DPOAE levels at 1.2 to 7.0 kHz in cohort2 (all P > 0.999 Bonferroni corrected except for left‐ear levels at 3.5 and 7.0 kHz with corrected P = 0.138 and P = 0.533). Data Conclusion A single 40‐minute 3‐T MRI with equivalent SPLs of 103.5–113.6 dBA did not result in significant transient disruption of auditory function, as measured by ABR and DPOAE, in neonates with adequate hearing protection. Evidence Level 2. Technical Efficacy Stage 5.

Abstract:Background: Early sensory experiences have a significant impact on the later life of preterm infants. The NICU soundscape is profoundly influenced by various modalities of respiratory support or ventilation, which are often mandatory early in the care. The incubator, believed to shield from external noise, is less effective against noise originating inside. The objective of this study was to evaluate the sound levels and characteristics of frequently used respiratory support and ventilation modalities, taking into consideration the developing auditory system of premature infants. Methods: To evaluate sound dynamics inside and outside an incubator during respiratory support/ventilation, experimental recordings were conducted at the Center for Pediatric Simulation Training of the Medical University Vienna. The ventilator used was a FABIAN HFOI®. Results: Jet CPAP (Continuous positive airway pressure), whether administered via mask or prongs, generates significantly higher sound levels compared to High-flow nasal cannula (HFNC) and to High-frequency oscillatory ventilation (HFOV) delivered through an endotracheal tube. Upon evaluating the sound spectrum of jet CPAP support, a spectral peak is observed within the frequency range of 4 to 8 kHz. Notably, this frequency band aligns with the range where the hearing threshold of preterm infants is at its most sensitive. Conclusion: Non-invasive HFNC and invasive HFOV generate lower sound levels compared to those produced by jet CPAP systems delivered via masks or prongs. Moreover, HFNC and HFOV show a reduced acoustic presence within the frequency range where the preterm infant's hearing is highly sensitive. Therefore, it is reasonable to speculate that the potential for auditory impairment might be more pronounced in preterm infants who require prolonged use of jet CPAP therapy during their time in the incubator.

Abstract:Abstract Objectives This study: (i) quantified the typical noise levels in an Irish neonatal intensive care unit (NICU) and compared the values to recommendations by the American Academy of Paediatrics (AAP) and the European Standards for Care for Newborn Health (EFCNI) and to occupational exposure limit value and exposure action values; and (ii) qualified the perception of noise levels and the sources of noise across the various stakeholders within a typical NICU. Methods A noise survey was conducted in an Irish NICU. Observations identified practices and behaviours in the NICU that potentially had an impact on noise levels. Noise levels were compared to occupational exposure limits and AAP and EFCNI standards. A noise perception survey was conducted to identify noise sources and awareness of noise levels in the NICU. Results were analysed using SPSS Statistics to determine statistical significance. Results Noise levels recorded were consistent with previous similar studies and in all cases, the average noise levels recorded exceeded the 45 dBA as recommended by the AAP and EFCNI. There was a statistically significant difference (P < 0.01) between noise levels recorded on the day shift compared to the night shift. The perception of noise levels reported by nurses versus parents was found to be statistically significant (P = 0.001). 38.3% of all respondents reported having received no information or training with regard to noise in the NICU. There was a statistically significant difference in the perception of who is most likely to be affected by noise in the NICU, with nurses reporting those most likely to be affected by noise were patients, and parents reporting those most likely to be affected were staff (P = 0.003). Conclusions This study supports the hypothesis that noise levels within the NICU are of concern and require regular assessment and monitoring. Training and awareness programmes are an important component to ensuring all persons in the NICU recognise their potential impact on noise levels in the NICU and in reducing the risk for patients and staff.

Abstract:Background: In the last years, a significant body of scientific literature was dedicated to the noisy environment preterm-born infants experience during their admission to Neonatal Intensive Care Units (NICUs). Nonetheless, specific data on sound characteristics within and outside the incubator are missing. Therefore, this study aimed to shed light on noise level and sound characteristics within the incubator, considering the following domain: environmental noise, incubator handling, and respiratory support. Methods: The study was performed at the Pediatric Simulation Center at the Medical University of Vienna. Evaluation of noise levels inside and outside the incubator was performed using current signal analysis libraries and toolboxes, and differences between dBA and dBSPL values for the same acoustic noises were investigated. Noise level results were furthermore classed within previously reported sound levels derived from a literature survey. In addition, sound characteristics were evaluated by means of more than 70 temporal, spectral, and modulatory timbre features. Results: Our results show high noise levels related to various real-life situations within the NICU environment. Differences have been observed between A weighted (dBA) and unweighted (dBSPL) values for the same acoustic stimulus. Sonically, the incubator showed a dampening effect on sounds (less high frequency components, less brightness/sharpness, less roughness, and noisiness). However, a strong tonal booming component was noticeable, caused by the resonance inside the incubator cavity. Measurements and a numerical model identified a resonance of the incubator at 97 Hz and a reinforcement of the sound components in this range of up to 28 dB. Conclusion: Sound characteristics, the strong low-frequency incubator resonance, and levels in dBSPL should be at the forefront of both the development and promotion of incubators when helping to preserve the hearing of premature infants.

Abstract:Introduction: Exposure to maternal speech during the prenatal period shapes speech perception and linguistic preferences, allowing neonates to recognize stories heard frequently in utero and demonstrating an enhanced preference for their mother's voice and native language. Yet, with a high prevalence of bilingualism worldwide, it remains an open question whether monolingual or bilingual maternal speech during pregnancy influence differently the fetus' neural mechanisms underlying speech sound encoding. Methods: In the present study, the frequency-following response (FFR), an auditory evoked potential that reflects the complex spectrotemporal dynamics of speech sounds, was recorded to a two-vowel /oa/ stimulus in a sample of 129 healthy term neonates within 1 to 3 days after birth. Newborns were divided into two groups according to maternal language usage during the last trimester of gestation (monolingual; bilingual). Spectral amplitudes and spectral signal-to-noise ratios (SNR) at the stimulus fundamental (F 0 ) and first formant (F 1 ) frequencies of each vowel were, respectively, taken as measures of pitch and formant structure neural encoding. Results: Our results reveal that while spectral amplitudes at F0 did not differ between groups, neonates from bilingual mothers exhibited a lower spectral SNR. Additionally, monolingually exposed neonates exhibited a higher spectral amplitude and SNR at F 1 frequencies. Discussion: We interpret our results under the consideration that bilingual maternal speech, as compared to monolingual, is characterized by a greater complexity in the speech sound signal, rendering newborns from bilingual mothers more sensitive to a wider range of speech frequencies without generating a particularly strong response at any of them. Our results contribute to an expanding body of research indicating the influence of prenatal experiences on language acquisition and underscore the necessity of including prenatal language exposure in developmental studies on language acquisition, a variable often overlooked yet capable of influencing research outcomes.

Defining the Concept of Acoustic Neuroprotection in the Neonate: A Concept Analysis

Abstract:Background: It has long been understood and acknowledged that the Neonatal Intensive Care Unit (NICU) environment and the transport environments are extremely loud, with both long- and short-term sequelae to the neonate, being well over the recommended amount of noise by the American Academy of Pediatrics (AAP). This problem has yet to be properly addressed. The purpose of this manuscript is to define and explain the concept of acoustic neuroprotection. While we cannot change the internal structures of the neonates' auditory system, we could change the acoustics of the environment to be support neuroprotection of these sensitive patients. Evidence acquisition: Walker and Avant's concept analysis steps were followed to create and define the idea of acoustic neuroprotection, as it has not had a definition before. A total of 45 articles from multiple search engines were chosen. A combination of 2 concepts were used: acoustic protection and neurodevelopmental protection/support. The search was expanded past 20 years for lack of research and importance of seminal works. Results: To achieve acoustic neuroprotection, a neonate should not be exposed to sound greater than 45 decibels (dBa) for longer than 10 s, and exposure to sound above 80 dBa should never occur. Appropriate interventions need to include supporting the neurodevelopment of the neonate through therapeutic sound, while decreasing the amount of toxic noise exposure to safe levels. Implications for practice and research: By further understanding and having a quantifiable goal of acoustic neuroprotection for neonates, neonatal clinicians can work together to create new interventions for how to better protect and support the care of our tiniest patients.

Abstract:Auditory experience during the postnatal critical period is essential for the normal maturation of auditory function. Previous studies have shown that rearing infant rat pups under conditions of continuous moderate-level noise delayed the emergence of adult-like topographic representational order and the refinement of response selectivity in the primary auditory cortex (A1) beyond normal developmental benchmarks and indefinitely blocked the closure of a brief, critical-period window. To gain insight into the molecular mechanisms of these physiological changes after noise rearing, we studied expression of the AMPA receptor subunit GluR2 and GABA(A) receptor subunit beta 3 in the auditory cortex after noise rearing. Our results show that continuous moderate-level noise rearing during the early stages of development decreases the expression levels of GluR2 and GABA(A)beta 3. Furthermore, noise rearing also induced a significant decrease in the level of GABA(A) receptors relative to AMPA receptors. However, in adult rats, noise rearing did not have significant effects on GluR2 and GABA(A)beta 3 expression or the ratio between the two units. These changes could have a role in the cellular mechanisms involved in the delayed maturation of auditory receptive field structure and topographic organization of A1 after noise rearing. Abstract:The paper presents evidence that the intrauterine auditory environment plays a key role in shaping later auditory development. The acoustic environment in utero begins to shape the auditory system much earlier than sensory systems that are not exposed to input until after birth. The effects of prenatal auditory experience can be observed both among fetus through different paradigms and in the new-borns within few hours or days after birth. This manuscript collects a comprehensive snapshot of the work in this research area presenting evidence of a consistent number of papers published in this topic of study. Furthermore, the potential function of learning prenatally is explored in terms of its relevance for perinatal development. So, we describe growing evidence that externally generated sounds and music influence the developing foetus, and argue that such prenatal auditory experience may also set the trajectory for the development.

Abstract:The ability of human fetuses to recognize their own mother's voice was examined. Sixty term fetuses were assigned to one of two conditions during which they were exposed to a tape recording of their mother or a female stranger reading a passage. Voice stimuli were delivered through a loudspeaker held approximately 10 cm above the maternal abdomen and played at an average of 95 dB SPL. Each condition consisted of three 2-min periods: no stimulus, voice (mother or stranger), and no stimulus. Fetal heart rate increased in response to the mother's voice and decreased in response to the stranger's; both responses were sustained for 4 min. The finding of differential behavior in response to a familiar versus a novel voice provides evidence that experience influences fetal voice processing. It supports an epigenetic model of speech perception, presuming an interaction between genetic expression of neural development and species-specific experience.

Abstract:Background: Intrauterine inflammation and the requirement for mechanical ventilation independently increase the risk of perinatal brain injury and adverse neurodevelopmental outcomes. We aimed to investigate the effects of mechanical ventilation for 24 h, with and without prior exposure to intrauterine inflammation, on markers of brain inflammation and injury in the preterm sheep brain. Methods: Chronically instrumented fetal sheep at ~115 days of gestation were randomly allocated to receive a single intratracheal dose of 1 mg lipopolysaccharide (LPS) or isovolumetric saline, then further randomly allocated 1 h after to receive mechanical ventilation with room air or no mechanical ventilation (unventilated control + saline [UVC, n = 7]; in utero mechanical ventilation + saline [VENT, n = 8], unventilated control + intratracheal LPS [UVC + LPS, n = 7]; in utero ventilation + intratracheal LPS [VENT + LPS, n = 7]). Serial fetal blood and plasma samples were collected throughout the experimental protocol for assessment of blood biochemistry and plasma interleukin (IL)-6 levels. After 24 h of mechanical ventilation, fetal brains were collected for RT-qPCR and immunohistochemical analyses. Results: LPS exposure increased numbers of microglia and upregulated pro-inflammatory related genes within the cortical gray matter (GM) and subcortical white matter (SCWM) ( p LPS < 0.05). Mechanical ventilation alone increased astrocytic cell density in the periventricular white matter (PVWM) ( p VENT = 0.03) but had no effect on pro-inflammatory gene expression. The combination of ventilation and LPS increased plasma IL-6 levels ( p < 0.02 vs. UVC and VENT groups), and exacerbated expression of pro-inflammatory-related genes ( IL1β , TLR4 , PTGS2 , CXCL10 ) and microglial density ( p < 0.05 vs. VENT). Conclusion: This study demonstrates that 24 h of mechanical ventilation after exposure to intrauterine inflammation increased markers of systemic and brain inflammation and led to the upregulation of pro-inflammatory genes in the white matter. We conclude that 24 h of mechanical ventilation following intrauterine inflammation may precondition the preterm brain toward being more susceptible to inflammation-induced injury.

Abstract:Somatostatin (SRIF) inhibits both basal and vasoactive intestinal peptide (VIP)-stimulated hormone secretion by the GH4C1 clonal strain of rat pituitary tumor cells. We have previously shown that SRIF inhibits cAMP accumulation stimulated by VIP but does not alter basal cAMP levels in this cell line. To determine the importance of changes in cAMP accumulation in the mechanism of SRIF action, we have compared the effect of SRIF on hormone release stimulated by VIP and two other secretagogues which increase effective intracellular cAMP concentrations: forskolin and 8-Bromo-cAMP (8-Br-cAMP). VIP stimulated GH and PRL secretion to the same maximal extent (220% of control) with similar ED50 values (0.37 +/- 0.03 and 0.43 +/- 0.08 nM, mean +/- SE, respectively). SRIF (100 nM) reduced maximal VIP-stimulation of both GH and PRL release from 220 to 140% of control; however, it did not significantly change the ED50 values for VIP. The effect of SRIF on VIP-stimulated hormone release parallels its action on VIP-stimulated cAMP accumulation. Furthermore, the concentrations of SRIF required to produce half-maximal inhibition of VIP-stimulated GH and PRL release (0.8 +/- 0.2 nM and 0.7 +/- 0.1 nM, respectively) were similar to its potency to inhibit VIP-stimulated cAMP accumulation (1.2 +/- 0.1 nM). These data indicate that changes in cAMP levels mediate inhibition of VIP-stimulated hormone secretion by SRIF. Forskolin increased cAMP accumulation with an ED50 value of 2.4 +/- 0.5 microM. A maximal concentration of forskolin (100 microM) stimulated cAMP accumulation to a greater extent than 100 nM VIP (34 +/- 4-fold vs. 9 +/- 1-fold). Together, forskolin (100 microM) and VIP (100 nM) stimulated cAMP accumulation by more than 50-fold. However, PRL secretion in response to maximal concentrations of VIP or forskolin individually or together were the same (approximately 200% of control). These results support the conclusion that both compounds stimulate PRL secretion by a cAMP-mediated mechanism which can be fully activated by either one alone.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract:This work proposes a novel method of temporal signal-to-noise ratio (SNR)-guided adaptive acoustic output adjustment and demonstrates this approach during in vivo fetal imaging. Acoustic output adjustment is currently the responsibility of sonographers, but ultrasound safety studies show recommended as low as reasonably achievable (ALARA) practices are inconsistently followed. This study explores an automated ALARA method that adjusts the mechanical index (MI) output, targeting imaging conditions matching the temporal noise perception threshold. A 28-dB threshold SNR is used as the target SNR, following prior work showing relevant noise quantities are imperceptible once this image data quality level is reached. After implementing adaptive output adjustment on a clinical system, the average MI required to achieve 28-dB SNR in an 11-volunteer fetal abdomen imaging test ranged from 0.17 to 0.26. The higher MI levels were required when imaging at higher frequencies. During tests with 20-s MI adjustment imaging periods, the degree of motion impacted the adaptive performance. For stationary imaging views, target SNR levels were maintained in 90% of SNR evaluations. When scanning between targets the imaging conditions were more variable, but the target SNR was still maintained in 71% of the evaluations. Given the relatively low MI recommended when performing MI adjustment and the successful adjustment of MI in response to changing imaging conditions, these results encourage adoption of adaptive acoustic output approaches guided by temporal SNR.

Abstract:There is an increasing interest in noise exposure for critically ill patients given the detrimental cardiac, neurological, and mental effects that might be induced by excessive noise.1 Neonatal ICUs (NICUs) are highly technological and noisy wards. A significant proportion of noise may come from ventilators that may often exceed the recommended levels.2 The situation is more complex during transportation of critically ill patients, where noise can also be produced by movement, sirens, or vibrations, and each noise source can add to the others. Noise-generating devices can be placed inside or outside the incubators, which may act as resonance chambers.2 Noise can have detrimental effects on patient physiology3 and professional communications.4 Finally, workers repeatedly exposed to transportation-induced excessive noise have been found to report annoyance, cognitive disruption, sleep problems, and hearing loss.5 Despite this background, there is a paucity of data regarding noise during newborn transportation. The American College of Occupational and Environmental Medicine recommends that sound during neonatal transport should not exceed 60 dBA.6 This represents an expert opinion and is higher than the threshold suggested for NICUs.7 In contrast to the NICU setting, the contribution of ventilators to the noise generation during transportation is unclear. To clarify this, we conducted a study reproducing various conditions that occur during neonatal transportation applying several respiratory support techniques.
