Abstract:Background and Objective: Accurate retinal vessel segmentation is often considered to be a reliable biomarker of diagnosis and screening of various diseases, including cardiovascular diseases, diabetic, and ophthalmologic diseases. Recently, deep learning (DL) algorithms have demonstrated high performance in segmenting retinal images that may enable fast and lifesaving diagnoses. To our knowledge, there is no systematic review of the current work in this research area. Therefore, we performed a systematic review with a meta-analysis of relevant studies to quantify the performance of the DL algorithms in retinal vessel segmentation. Methods: A systematic search on EMBASE, PubMed, Google Scholar, Scopus, and Web of Science was conducted for studies that were published between 1 January 2000 and 15 January 2020. We followed the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) procedure. The DL-based study design was mandatory for a study’s inclusion. Two authors independently screened all titles and abstracts against predefined inclusion and exclusion criteria. We used the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool for assessing the risk of bias and applicability. Results: Thirty-one studies were included in the systematic review; however, only 23 studies met the inclusion criteria for the meta-analysis. DL showed high performance for four publicly available databases, achieving an average area under the ROC of 0.96, 0.97, 0.96, and 0.94 on the DRIVE, STARE, CHASE_DB1, and HRF databases, respectively. The pooled sensitivity for the DRIVE, STARE, CHASE_DB1, and HRF databases was 0.77, 0.79, 0.78, and 0.81, respectively. Moreover, the pooled specificity of the DRIVE, STARE, CHASE_DB1, and HRF databases was 0.97, 0.97, 0.97, and 0.92, respectively. Conclusion: The findings of our study showed the DL algorithms had high sensitivity and specificity for segmenting the retinal vessels from digital fundus images. The future role of DL algorithms in retinal vessel segmentation is promising, especially for those countries with limited access to healthcare. More compressive studies and global efforts are mandatory for evaluating the cost-effectiveness of DL-based tools for retinal disease screening worldwide.

Progress in Retinal and Eye Research

Abstract:The advent of computer graphic processing units, improvement in mathematical models and availability of big data has allowed artificial intelligence (AI) using machine learning (ML) and deep learning (DL) techniques to achieve robust performance for broad applications in social-media, the internet of things, the automotive industry and healthcare. DL systems in particular provide improved capability in image, speech and motion recognition as well as in natural language processing. In medicine, significant progress of AI and DL systems has been demonstrated in image-centric specialties such as radiology, dermatology, pathology and ophthalmology. New studies, including pre-registered prospective clinical trials, have shown DL systems are accurate and effective in detecting diabetic retinopathy (DR), glaucoma, age-related macular degeneration (AMD), retinopathy of prematurity, refractive error and in identifying cardiovascular risk factors and diseases, from digital fundus photographs. There is also increasing attention on the use of AI and DL systems in identifying disease features, progression and treatment response for retinal diseases such as neovascular AMD and diabetic macular edema using optical coherence tomography (OCT). Additionally, the application of ML to visual fields may be useful in detecting glaucoma progression. There are limited studies that incorporate clinical data including electronic health records, in AL and DL algorithms, and no prospective studies to demonstrate that AI and DL algorithms can predict the development of clinical eye disease. This article describes global eye disease burden, unmet needs and common conditions of public health importance for which AI and DL systems may be applicable. Technical and clinical aspects to build a DL system to address those needs, and the potential challenges for clinical adoption are discussed. AI, ML and DL will likely play a crucial role in clinical ophthalmology practice, with implications for screening, diagnosis and follow up of the major causes of vision impairment in the setting of ageing populations globally.

Abstract:Artificial intelligence (AI) based on deep learning (DL) has sparked tremendous global interest in recent years. DL has been widely adopted in image recognition, speech recognition and natural language processing, but is only beginning to impact on healthcare. In ophthalmology, DL has been applied to fundus photographs, optical coherence tomography and visual fields, achieving robust classification performance in the detection of diabetic retinopathy and retinopathy of prematurity, the glaucoma-like disc, macular oedema and age-related macular degeneration. DL in ocular imaging may be used in conjunction with telemedicine as a possible solution to screen, diagnose and monitor major eye diseases for patients in primary care and community settings. Nonetheless, there are also potential challenges with DL application in ophthalmology, including clinical and technical challenges, explainability of the algorithm results, medicolegal issues, and physician and patient acceptance of the AI ‘black-box’ algorithms. DL could potentially revolutionise how ophthalmology is practised in the future. This review provides a summary of the state-of-the-art DL systems described for ophthalmic applications, potential challenges in clinical deployment and the path forward.

Abstract:Artificial intelligence (AI), also known as machine intelligence, is a branch of science that empowers machines using human intelligence. AI refers to the technology of rendering human intelligence through computer programs. From healthcare to the precise prevention, diagnosis, and management of diseases, AI is progressing rapidly in various interdisciplinary fields, including ophthalmology. Ophthalmology is at the forefront of AI in medicine because the diagnosis of ocular diseases heavy reliance on imaging. Recently, deep learning-based AI screening and prediction models have been applied to the most common visual impairment and blindness diseases, including glaucoma, cataract, age-related macular degeneration (ARMD), and diabetic retinopathy (DR). The success of AI in medicine is primarily attributed to the development of deep learning algorithms, which are computational models composed of multiple layers of simulated neurons. These models can learn the representations of data at multiple levels of abstraction. The Inception-v3 algorithm and transfer learning concept have been applied in DR and ARMD to reuse fundus image features learned from natural images (non-medical images) to train an AI system with a fraction of the commonly used training data (<1%). The trained AI system achieved performance comparable to that of human experts in classifying ARMD and diabetic macular edema on optical coherence tomography images. In this study, we highlight the fundamental concepts of AI and its application in these four major ocular diseases and further discuss the current challenges, as well as the prospects in ophthalmology.

Abstract:Background and objectives Optical coherence tomography (OCT) has ushered in a transformative era in the domain of ophthalmology, offering non-invasive imaging with high resolution for ocular disease detection. OCT, which is frequently used in diagnosing fundamental ocular pathologies, such as glaucoma and age-related macular degeneration (AMD), plays an important role in the widespread adoption of this technology. Apart from glaucoma and AMD, we will also investigate pertinent pathologies, such as epiretinal membrane (ERM), macular hole (MH), macular dystrophy (MD), vitreomacular traction (VMT), diabetic maculopathy (DMP), cystoid macular edema (CME), central serous chorioretinopathy (CSC), diabetic macular edema (DME), diabetic retinopathy (DR), drusen, glaucomatous optic neuropathy (GON), neovascular AMD (nAMD), myopia macular degeneration (MMD) and choroidal neovascularization (CNV ) diseases. This comprehensive review examines the role that OCT-derived images play in detecting, characterizing, and monitoring eye diseases. Method The 2020 PRISMA guideline was used to structure a systematic review of research on various eye conditions using machine learning (ML) or deep learning (DL) techniques. A thorough search across IEEE, PubMed, Web of Science, and Scopus databases yielded 1787 publications, of which 1136 remained after removing duplicates. Subsequent exclusion of conference papers, review papers, and non-open-access articles reduced the selection to 511 articles. Further scrutiny led to the exclusion of 435 more articles due to lower-quality indexing or irrelevance, resulting in 76 journal articles for the review. Results During our investigation, we found that a major challenge for ML-based decision support is the abundance of features and the determination of their significance. In contrast, DL-based decision support is characterized by a plug-and-play nature rather than relying on a trial-and-error approach. Furthermore, we observed that pre-trained networks are practical and especially useful when working on complex images such as OCT. Consequently, pre-trained deep networks were frequently utilized for classification tasks. Currently, medical decision support aims to reduce the workload of ophthalmologists and retina specialists during routine tasks. In the future, it might be possible to create continuous learning systems that can predict ocular pathologies by identifying subtle changes in OCT images.

Abstract:The advent of artificial intelligence (AI), especially the state-of-the-art deep learning frameworks, has begun a silent revolution in all medical subfields, including ophthalmology. Due to their specific microvascular and neural structures, the eyes are anatomically associated with the rest of the body. Hence, ocular image-based AI technology may be a useful alternative or additional screening strategy for systemic diseases, especially where resources are scarce. This review summarizes the current applications of AI related to the prediction of systemic diseases from multimodal ocular images, including cardiovascular diseases, dementia, chronic kidney diseases, and anemia. Finally, we also discuss the current predicaments and future directions of these applications.

Abstract:Machine learning and especially deep learning techniques are dominating medical image and data analysis. This article reviews machine learning approaches proposed for diagnosing ophthalmic diseases during the last four years. Three diseases are addressed in this survey, namely diabetic retinopathy, age-related macular degeneration, and glaucoma. The review covers over 60 publications and 25 public datasets and challenges related to the detection, grading, and lesion segmentation of the three considered diseases. Each section provides a summary of the public datasets and challenges related to each pathology and the current methods that have been applied to the problem. Furthermore, the recent machine learning approaches used for retinal vessels segmentation, and methods of retinal layers and fluid segmentation are reviewed. Two main imaging modalities are considered in this survey, namely color fundus imaging, and optical coherence tomography. Machine learning approaches that use eye measurements and visual field data for glaucoma detection are also included in the survey. Finally, the authors provide their views, expectations and the limitations of the future of these techniques in the clinical practice.

Abstract:Abstract In clinical ophthalmology, a variety of image-related diagnostic techniques have begun to offer unprecedented insights into eye diseases based on morphological datasets with millions of data points. Artificial intelligence (AI), inspired by the human multilayered neuronal system, has shown astonishing success within some visual and auditory recognition tasks. In these tasks, AI can analyze digital data in a comprehensive, rapid and non-invasive manner. Bioinformatics has become a focus particularly in the field of medical imaging, where it is driven by enhanced computing power and cloud storage, as well as utilization of novel algorithms and generation of data in massive quantities. Machine learning (ML) is an important branch in the field of AI. The overall potential of ML to automatically pinpoint, identify and grade pathological features in ocular diseases will empower ophthalmologists to provide high-quality diagnosis and facilitate personalized health care in the near future. This review offers perspectives on the origin, development, and applications of ML technology, particularly regarding its applications in ophthalmic imaging modalities.

Abstract:We conducted a systematic review of research in artificial intelligence (AI) for retinal fundus photographic images. We highlighted the use of various AI algorithms, including deep learning (DL) models, for application in ophthalmic and non-ophthalmic (i.e., systemic) disorders. We found that the use of AI algorithms for the interpretation of retinal images, compared to clinical data and physician experts, represents an innovative solution with demonstrated superior accuracy in identifying many ophthalmic (e.g., diabetic retinopathy (DR), age-related macular degeneration (AMD), optic nerve disorders), and non-ophthalmic disorders (e.g., dementia, cardiovascular disease). There has been a significant amount of clinical and imaging data for this research, leading to the potential incorporation of AI and DL for automated analysis. AI has the potential to transform healthcare by improving accuracy, speed, and workflow, lowering cost, increasing access, reducing mistakes, and transforming healthcare worker education and training.

Abstract:Diabetic retinopathy (DR) remains a leading cause of vision loss worldwide, with early detection critical for preventing irreversible damage. This review explores the current landscape and future directions of artificial intelligence (AI)-enhanced detection of DR from fundus images. Recent advances in deep learning and computer vision have enabled AI systems to analyze retinal images with expert-level accuracy, potentially transforming DR screening. Key developments include convolutional neural networks achieving high sensitivity and specificity in detecting referable DR, multi-task learning approaches that can simultaneously detect and grade DR severity, and lightweight models enabling deployment on mobile devices. While these AI systems show promise in improving the efficiency and accessibility of DR screening, several challenges remain. These include ensuring generalizability across diverse populations, standardizing image acquisition and quality, addressing the "black box" nature of complex models, and integrating AI seamlessly into clinical workflows. Future directions in the field encompass explainable AI to enhance transparency, federated learning to leverage decentralized datasets, and the integration of AI with electronic health records and other diagnostic modalities. There is also growing potential for AI to contribute to personalized treatment planning and predictive analytics for disease progression. Abstract:Purpose of review: The purpose of this review is to describe the current status of automated deep learning in healthcare and to explore and detail the development of these models using commercially available platforms. We highlight key studies demonstrating the effectiveness of this technique and discuss current challenges and future directions of automated deep learning. Recent findings: There are several commercially available automated deep learning platforms. Although specific features differ between platforms, they utilise the common approach of supervised learning. Ophthalmology is an exemplar speciality in the area, with a number of recent proof-of-concept studies exploring classification of retinal fundus photographs, optical coherence tomography images and indocyanine green angiography images. Automated deep learning has also demonstrated impressive results in other specialities such as dermatology, radiology and histopathology. Summary: Automated deep learning allows users without coding expertise to develop deep learning algorithms. It is rapidly establishing itself as a valuable tool for those with limited technical experience. Abstract:Purpose: To discuss the worldwide applications and potential impact of artificial intelligence (AI) for the diagnosis, management and analysis of treatment outcomes of common retinal diseases. Methods: We performed an online literature review, using PubMed Central (PMC), of AI applications to evaluate and manage retinal diseases. Search terms included AI for screening, diagnosis, monitoring, management, and treatment outcomes for age-related macular degeneration (AMD), diabetic retinopathy (DR), retinal surgery, retinal vascular disease, retinopathy of prematurity (ROP) and sickle cell retinopathy (SCR). Additional search terms included AI and color fundus photographs, optical coherence tomography (OCT), and OCT angiography (OCTA). We included original research articles and review articles. Results: Research studies have investigated and shown the utility of AI for screening for diseases such as DR, AMD, ROP, and SCR. Research studies using validated and labeled datasets confirmed AI algorithms could predict disease progression and response to treatment. Studies showed AI facilitated rapid and quantitative interpretation of retinal biomarkers seen on OCT and OCTA imaging. Research articles suggest AI may be useful for planning and performing robotic surgery. Studies suggest AI holds the potential to help lessen the impact of socioeconomic disparities on the outcomes of retinal diseases. Conclusions: AI applications for retinal diseases can assist the clinician, not only by disease screening and monitoring for disease recurrence but also in quantitative analysis of treatment outcomes and prediction of treatment response. The public health impact on the prevention of blindness from DR, AMD, and other retinal vascular diseases remains to be determined.

Abstract:Diabetic retinopathy (DR) affects the small vessels of the eye and is the leading cause of blindness in people on reproductive age; however, less than half of patients are aware of their condition; therefore, early detection and treatment is essential to combat it. There are currently multiple technologies for DR detection, some of which are already commercially available. To understand how these technologies work, we must know first some basic concepts about artificial intelligence (AI) such as machine learning (ML) and deep learning (DL). ML is the basic process by which AI incorporates new data using different algorithms and thus creates new knowledge on its base, learns from it, and makes determinations and predictions on some subject based on all that information. AI can be presented at various levels. DL is a specific type of ML, which trains a computer to perform tasks as humans do, such as speech recognition, image identification, or making predictions. DL has shown promising diagnostic performance in image recognition, being widely adopted in many domains, including medicine. For general image analysis, it has achieved strong results in various medical specialties such as radiology dermatology and in particular for ophthalmology. We will review how this technology is constantly evolving which are the available systems and their task in real world as well as the several challenges, such as medicolegal implications, ethics, and clinical deployment model needed to accelerate the translation of these new algorithms technologies into the global health-care environment.

Abstract:Traditional dilated ophthalmoscopy can reveal diseases, such as age-related macular degeneration (AMD), diabetic retinopathy (DR), diabetic macular edema (DME), retinal tear, epiretinal membrane, macular hole, retinal detachment, retinitis pigmentosa, retinal vein occlusion (RVO), and retinal artery occlusion (RAO). Among these diseases, AMD and DR are the major causes of progressive vision loss, while the latter is recognized as a world-wide epidemic. Advances in retinal imaging have improved the diagnosis and management of DR and AMD. In this review article, we focus on the variable imaging modalities for accurate diagnosis, early detection, and staging of both AMD and DR. In addition, the role of artificial intelligence (AI) in providing automated detection, diagnosis, and staging of these diseases will be surveyed. Furthermore, current works are summarized and discussed. Finally, projected future trends are outlined. The work done on this survey indicates the effective role of AI in the early detection, diagnosis, and staging of DR and/or AMD. In the future, more AI solutions will be presented that hold promise for clinical applications.
