Abstract:INTRODUCTION: Cardiac patients frequently experience elevated levels of anxiety, which may influence clinical outcomes. Perceived social support is a precious resource with which to minimise anxiety. The aim of the study was to explore levels of perceived social support, levels of anxiety (state and trait), and the associated factors as well as the impact of patents' characteristics and social support on anxiety.MATERIAL AND METHODS: The study sample consisted of 82 island inhabitant patients with cardiac problems (49 men and 33 women). Data collected by the completion of the State-trait anxiety inventory (STAI) and the Multidimensional scale of perceived social support questionnaire (MSPSS). The statistical significance level was set at <i>p</i> < 0.05.RESULTS: Regarding state and trait anxiety, half of the patients scored below 37 and 38, respectively, (medians) while 25% of participants scored more than 50 and 48, respectively. These values indicate moderate levels of anxiety. Regarding social support, at least 50% of patients scored over 27, 27, and 20 (median) on support from significant ones, family, and friends, respectively. In addition, 25% of patients scored above 28, 28, and 26, respectively. These values indicate high levels of social support. The association between anxiety and social support was found to be linearly negative, meaning that the more support a patient felt, the less anxiety they also experienced.CONCLUSIONS: Enhancing perceived social support may be a key-element in alleviating anxiety among cardiac patients.

Abstract:Objective: This study aimed to determine the correlation between the intensive care experience and the state anxiety of patients hospitalized in the coronary intensive care unit. Methods: This descriptive cross-sectional study included 192 patients from the coronary intensive care unit of a university hospital. Data were collected using a Patient Information Form, the Intensive Care Experience Scale (ICE), and the Spielberger State Anxiety Inventory (STAI-I). The analysis was conducted using SPSS software version 25.0. Descriptive statistics were reported as counts, percentages, means, and standard deviations. A p-value of less than 0.05 was considered to indicate statistical significance. Results: Among the participants, 38% were aged 65 and older, 76.6% were male, 93.2% were married, 63% were admitted to the intensive care unit from the emergency room, and 88.5% had prior experience in an intensive care unit. The total and sub-dimension scores of the ICE and STAI-I were moderate. We found a weak positive correlation between the STAI-I total score and the ICE total score (r= 0.320). There was a very strong positive correlation between pessimistic experiences in the intensive care unit and the STAI-I total score (r=0.907). Additionally, there was a moderate negative correlation between satisfaction with care received in the intensive care unit and the sub-dimensions of memory of experiences (Awareness of Surroundings, Satisfaction with the Care, Recollection of Experiences) (r=0.252, r=-0.489, r=-0.496). Conclusion: The study found that pessimistic experiences in the intensive care unit can significantly impact patients' state anxiety.

Abstract:Background Following critical illness, family members are often required to adopt caregiving responsibilities. Anxiety and depression are common long term problems for both patients and caregivers. However, at present, it is not known how the trajectories of these symptoms compare between patients and caregivers. Objectives The aim of this study was to investigate and compare the trajectories of anxiety and depression in patients and caregivers in the first year following critical illness. Methods This study analyses data from a prospective multicentre cohort study of patients and caregivers who underwent a complex recovery intervention following critical illness. Paired patients and caregivers were recruited. The Hospital Anxiety and Depression Scale was used to evaluate symptoms of anxiety and depression at three timepoints: baseline; 3 months; and 12 months in both patient and caregivers. A linear mixed-effects regression model was used to evaluate the trajectories of these symptoms over the first year following critical illness. Results 115 paired patients and caregivers, who received the complex recovery intervention, were recruited. There was no significant difference in the relative trajectory of depressive symptoms between patients and caregivers in the first 12 months following critical illness (p = 0.08). There was, however, a significant difference in the trajectory of anxiety symptoms between patients and caregivers during this time period (p = 0.04), with caregivers seeing reduced resolution of symptoms in comparison to patients. Conclusions Following critical illness, symptoms of anxiety and depression are common in both patients and caregivers. The trajectory of symptoms of depression was similar between caregivers and patients; however, there was a significantly different recovery trajectory in symptoms of anxiety. Further research is required to understand the recovery pathway of caregivers in order to design effective interventions.

Abstract:Introduction: A cancer diagnosis is often met with fear and distress in the patient and their family. Cancer also presents many psychosocial challenges for the patient and family caregiver. We aimed to investigate the relationships between health anxiety and intolerance of uncertainty regarding the quality of life in caregivers of cancer patients.Methods: A total of 83 cancer patients’ caregivers were included in the study. A sociodemographic information form, Health Anxiety Scale (HAS), Quality of Life Scale – Family Version (QoL-FV), and Intolerance of Uncertainty Scale (IUS-12), were given to participants to respond. Correlations between the study scale and sub-scale scores were evaluated. Two groups were formed as those with HAS scores greater than and less than the median. It was evaluated whether these groups differed in terms of IUS-12, QoL-FV-Total and QoL-FV subscale scores.Results: Median (IQR) scores of HAS, QoL-FV-Total and IUS-12 were 18.0 (10.0-25.0), 173.0 (145.0-206.0), and 29.0 (23.0-42.0), respectively. There was a significant positive correlation (r=0.469, p<0.001) between HAS scores and IUS-12 scores and a significant negative correlation (r=-0.328, p=0.002) between QoL-FV-Total scores. There was no significant correlation (r=-0.084, p=0.452) between QoL-FV-Total and IUS-12 scores. The IUS-12 score (median (IQR); 24.0 (20.0-27.0) vs 39.5 (31.5-45.0), p<0.001) was significantly lower and the QoL-FV-total score (median (IQR); 206.0 (157.5-263.5) vs 157.5 (142.25-176.0), p<0.001) and QoL-FV subscale scores were significantly higher in the HAS-low group than in the HAS-high group.Conclusions: Health anxiety may be one of the negative factors for quality of life in caregivers of cancer patients. More research is needed to explore the factors that predict and protect against health anxiety, which is thought to affect the quality of life of caregivers of cancer patients. Also, psychological intervention studies aiming at the relationship between health anxiety and the quality of life in caregivers of cancer patients are needed.Keywords: Caregivers, Uncertainty, Health Behavior, Anxiety, Quality of Life
Understanding Intensive Care Unit Family Caregivers' Vulnerability to Post-Traumatic Stress Disorder: The Impact of Neurotic Personality Traits, Emotional Suppression, and Perceptions of Unexpected Death

Abstract:Rationale: Family members of patients who die in an intensive care unit (ICU) are at heightened risk of post-traumatic stress disorder (PTSD) symptoms. Not all surrogates develop these symptoms and heterogeneity exists in PTSD symptom clusters. Objectives: The study tested a personality model of PTSD symptoms in bereaved family caregivers of ICU patients. It was hypothesized that family members endorsing higher levels of neurotic personality traits would report higher levels of PTSD symptom clusters and that these associations would be explained by tendencies to view the patient's death as unexpected and to cope by suppressing their emotions. Methods: Participants were family members of ICU patients at an urban, academic medical center who either died or were discharged to an inpatient hospice unit. Participants were contacted for participation at least six months after the patient's death. Participants completed measures of personality, emotion regulation, and PTSD symptoms. Data were analyzed using path analysis. Results: Approximately one-third (35%) of the sample of 162 caregivers endorsed at-risk levels of PTSD. Individuals reporting higher levels of neurotic personality traits reported more PTSD symptoms (r = 0.53, p < 0.001). Perceptions that the patient death was unexpected partially explained the association of neuroticism with the intrusive re-experiencing PTSD symptoms (B = 0.45, p = 0.031). In contrast, emotional suppression partially explained the association of neuroticism with avoidance (B = 0.70, p = 0.010) and hyperarousal symptom clusters (B = 0.37, p = 0.041). Conclusions: Family vulnerability to PTSD after an ICU death can be conceptualized based on neurotic personality traits. Tendencies to view the patient's death as unexpected may contribute to intrusive thoughts and memories of the ICU experience. Individuals may avoid reminders and remain at heightened levels of arousal when they suppress their emotional experiences. Key Message: Family members who experience the death of a loved one following ICU treatment are vulnerable to PTSD symptoms. These reactions to ICU-related losses may be shaped by family members' personalities, expectations, and emotion regulation styles. Understanding these characteristics could inform family screening and intervention efforts in the ICU.
The Indirect Effect of the Frailty of Dependent Elderly People Needing Hospitalization on Their Informal Caregivers' Anxiety

Abstract:Background A high prevalence of anxiety symptoms has been identified among the caregivers of disabled older people. The aim of the study was to explore the relationships between objective burden (intensity of care and burdensome characteristics of the care recipient, like frailty status), caregiver characteristics, subjective burden, and anxiety in a sample of informal caregivers caring for hospitalized elderly patients. Methods In this cross-sectional study, patients' and their informal caregivers' characteristics were recorded for 311 patient-caregiver dyads. Subjective caregiver burden and caregivers' anxiety were assessed by using the Zarit Burden Interview and the Hospital Anxiety and Depression Scale (HADS), respectively. Correlation coefficients and path analysis were used to examine the relationship between variables. Caregivers' anxiety was considered as the outcome variable. Caregivers' subjective burden was entered as a mediator between caregiver characteristics-objective burden and anxiety. An objective burden was measured based on the care needs of the dependent elderly (frailty status, cognitive impairment, comorbidity, independence in activities of daily living, behavioral problems, hours spent on caregiving, and duration of caregiving). Results Abnormal anxiety symptoms (HADS score 11-21) were reported by 92 caregivers (29.6%). Borderline cases (HADS score 8-10) were 66 caregivers (21.2%). A mild, moderate, or severe subjective burden was recorded for 113 (36.3%), 100 (32.2%), and 26 (8.4%) caregivers, respectively. The female gender of the caregiver, the spousal relationship with the patient, and the subjective burden were directly related to higher levels of caregivers' anxiety. A subjective burden was found to be a significant mediator in the relationship between duration of caregiving, patients' frailty status, caregiver gender, patients' comorbidity, and caregivers' anxiety. Conclusion Among the risk factors for caregivers' anxiety, the frailty status of the patient is probably the only modifiable factor via interventions targeting frailty reversion or reduction.
Association between care burden, depression and personality traits in Alzheimer’s caregiver: A pilot study

Abstract:Introduction In this study correlations between care burden, depression, and personality at intrapsychic level in caregivers of Alzheimer’s disease patients were evaluated. Materials and methods Caregivers: n.40. Tests: Social-schedule; CBI; BDI; SASB-Structural-Analysis of Social Behaviours- Form-A- intrapsychic behaviours (8 Cluster); ECOG. Patients:MMSE. Statistical analysis: Chi-squared test; Anova one way F test; Pearson’s R coefficient. Results Correlations: CBI-total and NPI(p < .001); CBI-total—ECOG (p = .042); CBI-total—BDI(p< .001); CBI- total-SASB-Cl7(p = .014); SASB-CL8(p<0.000); BDI and SASB-Cl 2 (p = .018), SASB-Cl 3 (p = .004), SASB-Cl7(p < .000), SASB-CL8 (p < .000). High CBI is correlated with high depression, neuropsychiatry symptoms, low cognitive patient’s functions. Caregivers have the following intrapsychic behaviors: poor self-care, poor ability to take care of themselves; they exercise control over themselves and do not consider and/or ignore their basic needs at emotional and physical levels. These intrapsychic behaviours are indicators of depression (SASB Model) and are correlated with high care burden–CBI and high depression-CDQ. Discussion Care burden is closely related to the depression and individual personality (intrapsychic experience) of the caregiver. This may reveal a source of strength and may suggest areas of multidimensional and psychotherapeutic interventions.

Abstract:Anxiety is common in patients experiencing neurocritical illness and their family caregivers. Resilience factors like mindfulness and coping skills may be protective against symptoms of emotional distress, including anxiety. Less is known about the interplay of anxiety symptoms and resilience factors between patients and caregivers. The purpose of this study is to examine the trajectory of anxiety symptoms among dyads of neurocritical care patients without major cognitive impairment and their family caregivers and to elucidate the relationship between resiliency (e.g., mindfulness and coping) and anxiety in these dyads.

Abstract:Objective:To explore the relationship between early trauma experience,state-trait anxiety and coronary heart disease(CHD).Method:fifty one patients with coronary heart disease(CHD group),23 patients with chest pain instead of coronary heart disease(non-CHD group) and 60 normal people(normal control group) were assessed with early trauma inventory-short form(ETI-SF) and state-trait anxiety inventory(STAI).The relationship between coronary artery stenosis and ETI-SF and STAI score was analyzed.Results:The differences of ETI-SF score and the common trauma,physical trauma scores among the 3 groups were statistically significant(F=24.625,F=12.125,F=7.098;all P<0.01).ETI-SF score of CHD group was significantly higher than non-CHD group and normal control group(F=0.565,F=-0.823;P<0.01).General trauma score,physical trauma score and emotional abuse score of CHD group were significantly higher than normal control group(F=0.588,F=0.469,F=0.436;P<0.01).The differences of trait anxiety score in STAI among the 3 groups were statistically significant(F=4.730,P<0.01).The scores of state anxiety and trait anxiety in CHD group were significantly higher than those in normal control group(F=4.021,F=4.932;P<0.05 or P<0.01).ETI-SF score was positively correlated with the degree of coronary stenosis(r=0.387,P<0.01).Conclusion:Patients with coronary heart disease have obvious anxiety tendency.The early trauma experience is related to the degree of coronary artery stenosis.
Relationship between anxiety and poor sleep quality in family caregivers of psychiatric patients based on history of sharp object injuries: a case-control study in Bitlis, Turkey

Abstract:Objectives This study aims to investigate the relationship between anxiety levels and poor sleep quality among family caregivers of psychiatric patients, based on the history of sharp object injuries (SOI) inflicted by the patients. Design A case-control study. Setting and data The data were collected through face-to-face interviews with family caregivers of patients at the Bitlis Province Community Mental Health Center and Bitlis State Hospital Psychiatry Service between December 2021 and May 2022. Participants A total of 111 family caregivers participated in the study. Outcome measures The sleep quality of family caregivers was evaluated using the Pittsburgh Sleep Quality Index, while anxiety levels were assessed using the Beck Anxiety Inventory (BAI). Results Among family caregivers, 52.3% had poor sleep quality, 24.3% experienced moderate anxiety and 31.5% experienced severe anxiety. An increase of one point on the anxiety scale (measured by BAI) raised the odds of poor sleep by 14%. The anxiety-poor sleep relationship was stronger in SOI-exposed caregivers (r=0.699) than in non-exposed ones (r=0.607). When age was controlled, the relationship strengthened among SOI-exposed individuals (r=0.722) but remained unchanged for non-exposed ones (r=0.608). Conclusions This study shows that each point increase on the anxiety scale is associated with a 14% increase in the odds of poor sleep among family caregivers of psychiatric patients. The relationship between anxiety level and poor sleep quality was further elevated among individuals exposed to SOI. Providing family caregivers of psychiatric patients, especially those exposed to SOI, with coping strategies and sleep hygiene training can improve their anxiety and sleep quality and assist in managing the care process.

Factors affecting perceived caregiver burden: caregivers of trauma patients discharged from the intensive care unit

Abstract:Background: Caregivers to intensive care unit survivors are vulnerable to caregiver burden, which has been demonstrated to have consequences to patients' and caregivers' mental and physical health. Potentially modifiable factors should be explored so that interventions may be developed. Methods: Ninety-one anticipated caregivers were prospectively assessed both at a southwestern trauma intensive care unit during their patient's admission and at home via remote interview 3 months after intensive care unit admission. Caregiver depression, posttraumatic stress symptoms, experiences, and perceived burden were assessed. Results: At follow up, there was a significant negative correlation between caregiver burden and understanding of their patient's condition (P < 0.001) and intensive care unit events (P = 0.008). Caregivers with lower understanding of their patient's injury/illness had significantly greater depressive symptoms (P = 0.04). Conclusions: Education and knowledge of intensive care unit events regarding their patient may influence perceived burden among caregivers. Interventions to orient caregivers to intensive care unit events are suggested to reduce caregiver and healthcare system burden.
Prevalence and Correlates of Caregiver Anxiety in Family Caregivers of Patients With Advanced Cancer: a Cross-Sectional Study in a Palliative Care Unit in Hong Kong

Abstract:Objectives: To assess the prevalence of caregiver anxiety and to identify correlates of caregiver anxiety in family caregivers of patients with advanced cancer in a palliative care unit in Hong Kong. Methods: Family caregivers of patients admitted to the palliative care ward for the first time between July 2019 and September 2019 were screened for recruitment. Caregiver demographics (age, sex, marital status, occupation, education level, housing condition, relationship with the patient, known diagnosis of anxiety or depression, and Charlson comorbidity index) were collected, as were caregiver understanding of patient's diagnosis and prognosis, perceived patient's distress, frequency of witnessing confusion or delirium of patients in past month, perceived adequacy of psychological support (other than general social support), and perceived anxiety towards patient's approaching death. Caregiver anxiety was assessed using the 7-item Generalised Anxiety Disorder scale (GAD-7). Caregiver depression status was assessed using the 9-item Patient Health Questionnaire (PHQ-9). Caregiver psychological well-being was assessed using the 11-item Psychological Well-being Scale for Family Caregivers (PWS-C). Caregiver overall burden was assessed using the Chinese version of the 13-item Modified Caregiver Strain Index (C-M-CSI). Results: 70 caregivers (of 66 patients) were included, with a response rate of 98.6%. 16 (22.9%) caregivers had moderate to severe anxiety (GAD-7 score of ≥10). The GAD-7 score was higher in female caregivers than in male caregivers (mean rank: 39.26 vs 27.83, p = 0.026). 26 (37.1%) caregivers had depression (PHQ-9 score of ≥7). The GAD-7 score of caregivers was associated with the PHQ-9 score (rs = 0.834, p < 0.01), the four subscales of PWS-C: social support (rs = -0.308, p = 0.01), life meaning (rs = -3.30, p < 0.01), emotional distress (rs = 0.615, p < 0.01), and caregiver inadequacy (rs = 0.41, p < 0.01), and the C-M-CSI score for caregiver overall burden (rs = 0.332, p < 0.01). In multiple linear regression, predictors for GAD-7 score were the PHQ-9 score, caregiver's feeling of anxiety towards patient's approaching death, and caregiver sex. Conclusions: 22.9% of family caregivers of patients with advanced cancer have moderate to severe anxiety. Anxiety of caregivers was associated with depression, caregiver's feeling of anxiety towards patient's approaching death, psychological support, and life meaning. All these are potentially amendable by interventions. A structured screening of anxiety in family caregivers of patients with advanced cancer should be considered as routine practice.
Delivery of Hand Care to Patients With High Anxiety Burden

Abstract:Purpose: To determine whether patients presenting with Patient-Reported Outcome Measurement Information System (PROMIS) anxiety scores at >95th percentile of the US population undergo elective hand surgery at rates different from patients with less anxiety. Secondarily, we aimed to assess surgeon notation of these patients' emotional states and incidences of postoperative complications. Methods: This single-center retrospective cohort study analyzed data from new adult patients presenting for hand care between January 2019 and December 2020. Patients with initial PROMIS anxiety scores ≥70 were identified as the high anxiety burden (HAB) group. For each patient with HAB presenting with carpal tunnel syndrome, trigger finger, and distal radius fractures (n = 45), three controls were matched (n = 135). After matching, bivariate statistical analysis compared outcome variables of interest between patient groups. Sentiment analysis was used to explore if patient anxiety was realized and considered in surgical decision making. Results: After matching, patients with HAB averaged baseline PROMIS scores in every assessed domain that were ≥1 SD worse than unaffected patients. Patients with HAB and control patients were offered surgery at a comparable rate (58% vs 47%). Among those offered surgery, patients with HAB were less likely to undergo surgery (73% vs 92%). Postoperative complications occurred more frequently in the patients with HAB (32% vs 8%). Sentiment analysis of office notes revealed that the patients with HAB had their emotional state explicitly noted more often (40% vs 24%), but the majority of patients with HAB did not have this addressed in records. Conclusions: Patients with HAB both underwent surgery offered less frequently and when undergoing surgery, experienced more complications. Surgeons are likely to miss opportunities to positively influence extreme patient anxiety as most patients with HAB were treated without mention of their emotional state. Future investigations should explore whether preoperative anxiety alleviation could diminish these disparities. Type of study/level of evidence: Symptom prevalence III.

Abstract:Introduction: Psychological distress is prevalent among the primary family caregivers of patients with stroke. Objective: This study evaluated the effects of acupressure on anxiety, stress, and depression among the primary family caregivers of Patient With Stroke. Materials and Methods: In this sham-controlled randomized clinical trial, 96 family caregivers of Patient with Stroke were selected and allocated to the acupressure (n=40) and sham (n=42) groups. The study data were collected using demographic questionnaires for the patients and caregivers, the Barthel index (for assessing independence in daily living activities,) and the Persian version of the Depression Anxiety Stress Scale (DASS). Participants in the acupressure group received training about acupressure on the Yin Tang and HT7 points, while participants in the sham group received training about acupressure on the CV24 and TB5 sham points. The acupressure intervention lasted for 30 consecutive days. Statistical tests, including the Kolmogorov-Smirnov test, the independent t test, the Chi-square test, the Fisher exact test, the Wilcoxon, Mann-Whitney U test, and the analysis of covariance (ANCOVA) were applied to analyze the data. Results: The mean ages of the participants were 46.1±12.1 years in the acupressure group and 42.7±11.8 years in the sham group. The mean ages of the patients in these groups were 75.1±8.1 and 76.8±9.7 years; respectively. Regarding the demographic characteristics, no significant differences were observed between the acupressure and the sham groups. Regarding the pre-test mean scores of anxieties, stress, and depression, there were no significant differences between the acupressure and the sham groups. Anxiety and stress significantly reduced after the intervention only in the acupressure group (P<0.05). However, depression did not change substantially during the study.The ANCOVA indicated significant differences between the two groups after the intervention regarding anxiety and stress. In other words, in the acupressure group, anxiety and stress significantly improved compared to the sham group (P<0.05). Conclusion: It seems that the acupressure on the Yin Tang and HT7 points can help manage anxiety and stress among the primary family caregivers of Patient With Stroke.
The Association Between Spirituality, Self-Esteem, Anxiety and Depression: A Comparative Exploratory Study Between People with a Spinal Cord Injury and Healthy Subjects in Greece

Abstract:The purpose of this study was to evaluate spirituality, religiosity, self-esteem, depression, and stress in people with spinal cord injuries. The Royal Free Questionnaire for Spiritual and Religious Beliefs, the Rosenberg Self-Esteem Scale, the Center for Epidemiologic Studies Depression Scale, and the Spielberger State-Trait Anxiety Scale were all designed for the current study to collect demographic and injury-related information. In the study, 88 patients with traumatic SCI and 88 healthy subjects were included. Half of the participants and healthy subjects were women, while the mean age of the healthy subjects was 42.8 years and that of the subjects was 42.4 years. The majority of participants were Greek (97.2%), while 44.1% were married or with a partner and 43.2% were single. The mean depression and stress score was higher in patients than in healthy subjects, and the mean score of self-esteem was lower in patients than healthy subjects. Multivariable linear regression model showed that patients who believed more that a spiritual force or power outside of ourselves can influence what happens in everyday life had less depression and stress. People with spinal cord injuries experience extreme stress, anxiety, and depression. Spirituality and religiosity can play a significant role in their recuperation, rehabilitation, and adaptation.
The relationships between body image, self-esteem and quality of life in adults with trauma-related limb loss sustained in the Syrian war

Abstract:Background: Numerous military soldiers have lost limbs as a result of the Syrian War. While there are variations between trauma-related amputations in military and civilian populations, both scenarios result in life-changing injuries. Objective: To explore the relationship between body image, self-esteem, and quality of life (QOL) domains following trauma-related major amputation. It will be the first study in Syria on the correlation between self-esteem and body image. It will help improve our quality of care to meet patient needs and increase well-being, which in turn will help to address body image, self-esteem, and QOL. Method: A cross-sectional study that recruited 235 soldiers with amputations in two centers and two military hospitals in Latakia and Tartous. Patients were given an 81-item questionnaire that included the Amputee Body-Image Scale (ABIS), the Rosenberg Self-esteem (RSE) scale, the WHOQOL-BREF questionnaire, and unidimensional pain measures. The ANOVA test, a student's t-test, multiple linear regression, internal consistency, and test-retest reliability were utilized for statistical analysis. Results: There was a strong relationship between body image, self-esteem, and QOL, with the presence of body image concerns significantly associated with lower self-esteem scores and lower QOL scores (p=0.001). Patients with phantom pain sensation had significantly reduced self-esteem (p =0.001), greater body image concerns (p =0.001), and lower scores in all domains of QOL. We found that body image and self-esteem impacted the psychological, social, and environmental domains. After controlling for pain level and number of co-morbid conditions, body image and self-esteem did not predict WHOQOL-BREF scores, with the exception of the environmental domain, where no pain and low self-esteem predicted better environmental domain scores. Conclusion: Patients' body image and self-esteem were greatly impacted by lower-limb amputations. Additionally, phantom pain further impacted self-esteem, body image, and QOL. The image of the body had a profound effect on psychological, social, and environmental domains, and self-esteem was influenced by almost all aspects of QOL.

Abstract:Previous research has shown that informal caregiver burden can have deleterious effects on patient recovery; however, this relationship has yet to be investigated in patients undergoing coronary artery bypass graft (CABG) surgery. This study aimed to examine the impact of the change of caregiver burden from pre- to post-surgery on patients' mental and physical health after CABG surgery. Ninety patient-caregiver dyads were assessed one month before the surgery, two months and one year after the surgery. Caregivers completed the Oberst Burden Scale to measure caregiver burden. Patients completed the Beck Depression Inventory to measure depressive symptoms, a subscale of the Hospital Anxiety and Depression Scale to measure anxiety symptoms, the Short Form-12 health assessment to measure health-related quality of life, and the Coronary Revascularisation Outcomes Questionnaire to measure surgery symptoms. Hierarchical linear regression analyses were conducted to explore the association between change in caregiver burden and post-surgery patient outcomes. A greater increase in caregiver burden from pre- to two-months post-surgery significantly predicted higher patients' depressive symptoms (β = 0.179, p = 0.010), anxiety symptoms (β = 0.114, p = 0.017), reported post-surgery symptoms (β = 0.335, p = 0.008) and reduced physical health-related quality of life (β = -0.358, p = 0.003), after controlling for a wide range of covariates. These findings were not replicated at a one-year follow-up. These results provide insights into the impact of caregiver burden on patient distress following CABG surgery, supporting the idea of psychological support interventions for caregivers to increase patients' short-term recovery outcomes.

Abstract:OBJECTIVE: Not only tumor patients suffer enormously from their disease, also the caregivers are massively affected by the disease of their relatives. In this study, we investigate the psychological burden in caregivers of outpatient malignant brain tumor patients.PATIENTS AND METHODS: Fifty caregivers of patients with primary malignant brain tumors were included in our study. Study participants filled in a form with demographic details, a self-established questionnaire concerning general well-being and three established psychological questionnaires to assess anxiety, depression, stress and social support: The "Hospital Anxiety and Depression Scale" (HADS), the "Perceived Stress Scale" (PSS-10) and the "Social Support Questionnaire" (F-SozU).RESULTS: Caregivers of patients with primary malignant brain tumors showed in the HADS clinically relevant anxiety in 49% and depression in 20% of the cases. The stress level of the caregivers was increased (mean: 18 points) compared to the general population mean: 13 points), although they felt well supported by their social environment (mean: 4.25 points; general population 3.99 points). There was a significant positive correlation between anxiety and depression (p < 0.001). Female caregivers suffered significantly more from anxiety (p = 0.017) and stress (p = 0.012) than their male counterparts. No correlation was found between tumor grade, age of relatives and patients or the state of living together with the patient and anxiety or depression.CONCLUSIONS: Although the caregivers felt well supported by their social environment, stress, anxiety and depression are common phenomena in caregivers of patients with malignant brain tumors. Especially female ones have an increased risk for developing these comorbidities.

Abstract:Study objective To investigate the association between family cancer caregivers’ unmet daily needs and emotional states of depression, anxiety and stress across their care recipient’s treatment phases. Method A cross-sectional study design and self-report questionnaires were used. Family caregivers (N = 237) of cancer patients in ambulatory cancer clinics were recruited from May to December 2017, and completed a sociodemographic and medical questionnaire, the Depression Anxiety Stress Scale and Needs Assessment of Family Caregivers-Cancer Scale. Hierarchical linear regression was conducted to examine the influence of each predictor (sociodemographic variables, unmet personal care and role management needs, cancer treatment phase) on the Depression Anxiety Stress Scale total score, depression subscale, anxiety subscale, and the stress subscale. Results Family caregivers’ unmet daily activity needs, in particular higher unmet personal care needs, during the intermediate phase (6–9 months), were significantly associated ( ps <0.05) with overall distress ( b = 4.93) and stress ( b = 2.26). In the chronic treatment phase (>9 months), the significant association of unmet personal care needs was with overall distress ( b = 5.91), anxiety ( b = 1.97) and stress ( b = 2.53). After completing treatment, unmet role management needs were only significantly associated with stress ( b = -1 . 59 ). Caregivers’ higher depression was also associated with greater unmet role management needs, regardless of treatment phases. Conclusions Intermediate and chronic cancer treatment phases were identified as having greatest effect on caregivers’ unmet daily activity needs and emotions. Unmet personal care needs played the major effect on overall negative emotional states in the intermediate treatment phase and stress in the chronic treatment phase. Close attention to caregivers needs in intermediate and chronic treatment phases, would be highly beneficial in alleviating negative emotional disturbances.

Abstract:Introduction: Family caregivers play a large supportive role in the care of patients suffering from special conditions, such as trauma and stroke. This makes them to be at risks of variety of mental disorders such as anxiety and depression. Effective communication and nursing consulting can play an important role in managing anxiety and depression in family caregivers. The aim of this study was to investigate the effect of tele-counseling on the improvement of depression and anxiety in family caregivers of patients with trauma and stroke. Methods: This is a clinical trial study with 80 caregivers of stroke survivors discharged from Hospital. Inclusion criteria included being the primary caregiver, over 15 years of age, lack of cognitive impairment (dementia and delirium), lack of mourning experience for the past 6 months, and patient care for at least 2 months at home. Study tools included Beck Anxiety and Depression Inventory. The data were analyzed using t test, Mann-Whitney U, Wilcoxon and ANOVA tests in SPSS software. Results: In the experimental group, there was a significant decrease in levels of depression and anxiety of family caregivers of traumatic patients, but no significant difference was observed in the control group before and after the intervention. Conclusion: Nursing telephone consulting reduced the level of depression and anxiety of family caregivers of traumatic patients. Therefore, the development of desirable protocols for tele-counseling to support acute and chronic disease caregivers is suggested.
