Abstract:Abstract Cell cycle dysregulation is a hallmark of cancer and represents tremendous opportunities for clinical blockage in cancer therapy. Currently, trilaciclib, palbociclib, ribociclib, and abemaciclib that bear dual specificities against CDK4 and CDK6 have been approved for clinical usage, and more CDK4/6 targeted agents are actively under clinical evaluations, among which, TY-302, a novel CDK4/6 inhibitor being developed by TYK Medicines, is under Phase II trial (NCT04433494). To further drive the potential of pharmacological regulation of cancer cell cycle, here we report a novel CDK7 kinase inhibitor TY-2699a towards clinical investigation in 2023. CDK7 is a kinase at the core of transcription and also functions in regulating cell cycle progression. CDK7 overexpression has been identified in a wide spectrum of tumor tissues including triple negative breast cancer (TNBC), ovarian carcinoma (OC), small cell lung cancer (SCLC), and pancreatic cancer, and has been correlated to poor prognosis in the diseases. These malignant pathological profiles make CDK7 a pivotal target for the development of novel cancer therapy. Several CDK7 targeted agents, such as SY-5609 and Samuraciclib (CT7001), are under development. TYK Medicines is also committed to provide novel yet safe CDK7 kinase inhibitor. Our data show that TY-2699a potently inhibits the kinase activity of CDK7 in the form of CDK7/Cyclin H/MAT1 kinase complex (IC50 9.5 nM) with high selectivity compared to that with CDK1/Cyclin A2, CDK2/Cyclin E1, CDK4/Cyclin D1, and CDK6/Cyclin D1. The screening of a panel of cancer cell lines revealed strong anti-cell proliferation activities of TY-2699a compared to that of the reference compound, and the phenotypic findings are underscored by TY-2699a-dependent cell cycle disruption. Our data demonstrate that TY-2699a triggers G2 cell cycle arrest, and induces apoptosis in tested cancer cells (HCC70, and MDA-MB-468), but not in hTERT-immortalized normal cell (RPE-hTERT). In vivo studies show that TY-2699a confers significant efficacies in tested CDX mouse models of HCC70 (TNBC), OVCAR3 (OC), and MV-4-11(AML) in a dose-dependent manner. To further validate the anti-tumor activity of the agent, BR5010, a TNBC PDX model was employed to assess the response to TY-2699a treatment. Our data show that the efficacy of TY-2699a at 3 mg/kg, bid × 21 days was similar to that of the reference compound CT7001 at 100 mg/kg, qd × 21 days, and the efficacy of TY-2699a at 6 mg/kg, bid × 21 days was significantly better than that of CT7001 at 100 mg/kg, qd × 21 days in BR5010 mouse model. In summary, we report that TY-2699a is a highly selective and potent CDK7 kinase inhibitor with an acceptable toxicity profile within the therapeutic window. Abstract:Abstract The CDK4/6 inhibitor, palbociclib (PAL), significantly improves progression-free survival and overall survival in HR positive and HER2-negative breast cancer in combination either with letrozole or fulvestrant. However, resistance to the CDK4/6 inhibition inevitably develops. Preclinical models and clinical transcriptome specimen analysis disclosed that abnormal activation of Cyclin E/CDK2 due to CCNE gene amplification was one of the major reasons to CDK4/6 inhibition resistance. And inhibition of CDK2 either by gene knockdown of CCNE or by pharmaceutic approach rescued the sensitivity of resistant tumor cells to CDK4/6 inhibition. To overcome the resistance, a novel and selective CDK2/4/6 inhibitor, SCR-8079 was developed. In biochemical assay, SCR-8079 was demonstrated high potency on inhibiting CDK2/4/6 with IC50 of 3 nM, 1 nM and 2 nM, respectively. And SCR-8079 showed good selectivity over CDK1/9 isoforms (>60 selectivity folds). SCR-8079 demonstrated closed inhibitory activities both on CDK4/6 sensitive and resistant lines. In MCF-7, a breast cancer cell line sensitive to PAL, SCR-8079 potently inhibited Rb phosphorylation (IC50, 53 nM) and cell proliferation (IC50, 28 nM). Meanwhile, SCR-8079 showed high potency on inhibiting on the growth of OVCAR3, an ovarian cancer cell line with elevated CDK2 activation and resistance to PAL. SCR-8079 inhibited Rb phosphorylation (IC50, 41 nM) and cell proliferation (IC50, 21 nM). In order to further mimic CDK4/6 resistance, a PAL resistant line (MCF-7R) was generated via induction of MCF-7 by gradient increased PAL. SCR-8079 potently inhibited MCF-7R cells growth (IC50, 3 nM) but PAL didn’t (IC50 >1 µM). Moreover, SCR-8079 showed high potency on growth inhibition in a broad of breast cancer cell lines, including HR-positive, Her2-positive and triple negative lines. SCR-8079 displayed good oral bioavailability in multiple pre-clinical species and robust anti-tumor activities both in OVCAR3 and MCF7 xenograft models. In summary, SCR-8079 is a selective CDK2/4/6 inhibitor, which overcomes the CCNE-mediated resistance to CDK4/6 inhibition. Abstract:1594 The cyclin-dependent kinases (CDKs) are a family of serine/threonine kinases that play a critical role in regulation of the eukaryotic cell cycle. A hallmark of human tumors is their rapid progression through the cell cycle comparing to normal tissues. Inhibition of CDK1/CDK2 in tumors is predicted to provoke cell cycle arrest and apoptosis. Hence, inhibition CDK2 and CDK1 represents an attractive therapeutic strategy for human cancers. SCH 727965 is a potent inhibitor of CDK1, CDK2 and CDK9 activities in vitro with IC50 of 1-4 nM with cell-based IC50 at 10-20 nM. SCH 727965 has been shown to be effective as a single agent against a wide-spectrum of human tumor xenograft models including lung, prostate, breast, ovarian and multiple myeloma, with tumor growth inhibition from 12% to 150% (regression) at 5 mpk and 20 mpk once a dose schedule. Treatment with SCH 727965 in vivo results in decreased Rb phosphorylation accompanied by reduction of proliferation marker Ki-67. In addition, we have demonstrated that combination of SCH 727965 with various cytotoxics and targeted anti-cancer agent results in significantly enhanced anti-tumor efficacy in vivo. SCH 727965 is a potent anti-tumor agent in preclinical models and is currently undergoing evaluation in clinical trials.
Identification of a Novel Oral Small-Molecule JAK1/2 Inhibitor and Its Potent Synergistic Drug Combinations Against Cervical Cancer

Abstract:Background: Although JAK (Janus Kinase) inhibitors have been widely applied to many types of aggressive tumors with continuously activated JAK/STAT signaling pathways, efficacious compounds against cervical cancer (CC) are still lacking, and chronic exposure to them inevitably results in drug resistance that often arises from compensatory activation of alternative signaling and survival pathways. Therefore, developing novel JAK inhibitors and identifying their partner compounds for combination therapy against CC, remains an unmet clinical need.Methods: In this study, by virtual screening the SPECS database, we selected out multiple JAK inhibitor candidates, validated their kinase activities and antitumor drug efficacies in various biochemical, cellular and in vivo xenograft models. Next, we screened a chemical library containing 2094 small molecules to identify compounds that act synergistically with the best JAK inhibitor obtained above in dampening CC cell propagation. Finally, the molecular mechanism of such synergistic effect was explored.Findings: Twenty potential small-molecule JAK inhibitors were identified by virtual screening, and the top 9 ranking compounds were validated by biochemical assays. AH057, the best JAK inhibitor identified, can effectively block JAK/STAT pathways by directly inhibiting JAK1/2 activities, and exhibits potent antitumor effects including compromised cell proliferation and invasion, increased apoptosis, arrested cell cycles and impaired tumor progression in a xenograft model. Among all cell lines examined, CC cell lines were the most sensitive to AH057. Further, based on the calculated Bliss synergy scores, SGI-1027, a DNMT1 inhibitor, was identified as the best compound that works in high synergy with AH057. The combination of AH057 and SGI-1027 potently induced cell cycle arrest at G1 phase partially through Cyclin D1 and Cyclin A2, and apoptotic cell death that is linked to PARP, Caspase-3, Bcl-xL and Survivin.Interpretation: A novel oral small-molecule JAK1/2 inhibitor AH057 with definitive antitumor efficacy against CC is identified in this study. The combination of AH057 with the DNMT1 inhibitor SGI-1027 synergistically impairs cancer cell propagation via induced apoptosis and cell cycle arrest, likely by acting on a same group of downstream effector molecules that are dually controlled by JAK1/2 and DNMT1. Abstract:Emerging clinical evidence indicates that selective CDK9 inhibition may provide clinical benefits in the management of certain cancers. Many CDK9 selective inhibitors have entered clinical developments, and are being investigated. No clear winner has emerged because of unforeseen toxicity often observed in clinic with these agents. Therefore, a novel agent with differentiated profiles is still desirable. Herein, we report our design, syntheses of a novel azaindole series of selective CDK9 inhibitors. SAR studies led to a preclinical candidate YK-2168. YK2168 exhibited improved CDK9 selectivity over AZD4573 and BAY1251152; also showed differentiated intravenous PK profile and remarkable solid tumor efficacy in a mouse gastric cancer SNU16 CDX model in preclinical studies. YK-2168 is currently in clinical development in China (CTR20212900).

Abstract:Targeting cyclin-dependent kinase 7 (CDK7) has emerged as a highly sought-after therapeutic strategy in oncology due to its duality of function in regulating biological processes, including cell cycle progression and transcriptional control. Herein, we describe the design, optimization and characterization of a series of thieno[3,2- d ]pyrimidine derivatives as potent CDK7 inhibitors. The involvement of thiophene as core structure plays critical role in leading to the remarkable selectivity and incorporation of a fluorine atom into the piperidine ring enhances metabolic stability. Structure-activity relationship (SAR) study generated compound 36 as lead compound with potent inhibitory activity against CDK7 and good kinome selectivity in vitro . Compound 36 demonstrated strong efficacy against a triple negative breast cancer (TNBC) cell line-derived xenograft (CDX) mouse model upon oral administration at 5 mg/kg once daily. Therefore, it exhibits immense potential as a lead candidate for further exploration in the development of cancer therapy.

Abstract:Purpose: Cyclin-dependent kinase 9 (CDK9) is a transcriptional regulator and potential therapeutic target for many cancers. Multiple nonselective CDK9 inhibitors have progressed clinically but were limited by a narrow therapeutic window. This work describes a novel, potent, and highly selective CDK9 inhibitor, AZD4573. Experimental Design: The antitumor activity of AZD4573 was determined across broad cancer cell line panels in vitro as well as cell line- and patient-derived xenograft models in vivo . Multiple approaches, including integrated transcriptomic and proteomic analyses, loss-of-function pathway interrogation, and pharmacologic comparisons, were employed to further understand the major mechanism driving AZD4573 activity and to establish an exposure/effect relationship. Results: AZD4573 is a highly selective and potent CDK9 inhibitor. It demonstrated rapid induction of apoptosis and subsequent cell death broadly across hematologic cancer models in vitro , and MCL-1 depletion in a dose- and time-dependent manner was identified as a major mechanism through which AZD4573 induces cell death in tumor cells. This pharmacodynamic (PD) response was also observed in vivo , which led to regressions in both subcutaneous tumor xenografts and disseminated models at tolerated doses both as monotherapy or in combination with venetoclax. This understanding of the mechanism, exposure, and antitumor activity of AZD4573 facilitated development of a robust pharmacokinetic/PD/efficacy model used to inform the clinical trial design. Conclusions: Selective targeting of CDK9 enables the indirect inhibition of MCL-1, providing a therapeutic option for MCL-1–dependent diseases. Accordingly, AZD4573 is currently being evaluated in a phase I clinical trial for patients with hematologic malignancies (clinicaltrials.gov identifier: NCT03263637). Synergistic Anti-Tumor Effect of Combining Selective CDK7 and BRD4 Inhibition in Neuroblastoma

Abstract:Purpose Cyclin-dependent kinases (CDKs) that have critical roles in RNA polymerase II (Pol II)-mediated gene transcription are emerging as therapeutic targets in cancer. We have previously shown that THZ1, a covalent inhibitor of CDKs 7/12/13, leads to cytotoxicity in MYCN -amplified neuroblastoma through the downregulation of super-enhancer-associated transcriptional upregulation. Here we determined the effects of YKL-5-124, a novel covalent inhibitor with greater selectivity for CDK7 in neuroblastoma cells. Experimental Design We tested YKL-5-124 in MYCN -amplified and nonamplified neuroblastoma cells individually and in combination with other inhibitors in cell line and animal models. Cell viability, target validation, effects on cell cycle and transcription were analyzed. Results CDK7 inhibition with YKL-5-124 did not lead to significant cell death, but resulted in aberrant cell cycle progression especially in MYCN -amplified cells. Unlike THZ1, YKL-5-124 had minimal effects on Pol II C-terminal domain phosphorylation, but significantly inhibited that of the CDK1 and CDK2 cell cycle kinases. Combining YKL-5-124 with the BRD4 inhibitor JQ1 resulted in synergistic cytotoxicity. A distinct MYCN -gene expression signature associated with resistance to BRD4 inhibition was suppressed with the combination. The synergy between YKL-5-124 and JQ1 translated into significant tumor regression in cell line and patient-derived xenograft mouse models of neuroblastoma. Conclusions The combination of CDK7 and BRD4 inhibition provides a therapeutic option for neuroblastoma and suggests that the addition of YKL-5-124 could improve the therapeutic efficacy of JQ1 and delay resistance to BRD4 inhibition.

Abstract:Cyclin-dependent kinases (CDKs) are promising therapeutic targets for cancer therapy. Herein, we describe our efforts toward the discovery of a series of 5-chloro-N4-phenyl-N2-(pyridin-2-yl)pyrimidine-2,4-diamine derivatives as dual CDK6 and 9 inhibitors. Intensive structural modifications lead to the identification of compound 66 as the most active dual CDK6/9 inhibitor with balancing potency against these two targets and good selectivity over CDK2. Further biological studies revealed that compound 66 was directly bound to CDK6/9, resulting in suppression of their downstream signaling pathway and inhibition of cell proliferation by blocking cell cycle progression and inducing cellular apoptosis. More importantly, compound 66 significantly inhibited tumor growth in a xenograft mouse model with no obvious toxicity, indicating the promising therapeutic potential of CDK6/9 dual inhibitors for cancer treatment. Therefore, the above results are of great importance in the development of dual CDK6/9 inhibitors for cancer therapy.

Abstract:Dentures are not always marked. In Sweden legislation now exists to enforce it. This study was undertaken to establish the frequency of marked dentures, the incidence of edentulousness and a temperature resistance test of three possible marking bands. Patients from two long-term units were included in the study (n = 58). Observation of the dental status included absence of teeth, some teeth present and influence of dentures, complete, partial, upper or lower. Marking of the dentures was also recorded. Three different types of steel bands (Jasch; Remanit; ID-band) were exposed to temperature levels of 1100, 1200 and 1300 degrees C. Of a total of 58 patients 64% were edentulous and only 17 of the patients could be identified by means of the denture markings. None of the metal bands had readable markings at 1200 and 1300 degrees C, but at 1100 degrees C the ID-band and the Jasch band were readable, but not the Remanit band.

Abstract:Abstract CDK7 is a well-known regulator of both the cell cycle and transcription and plays a crucial role in cell cycle progression by catalyzing T-loop phosphorylation of other CDKs. CDK7 is also an integral subunit of the general transcription factor, TFIIH and regulates transcription through phosphorylation of RNA Polymerase II (Pol II) at CTD Ser5 and CDK9. We used Q901, a highly selective CDK7 inhibitor to further investigate the molecular function of CDK7 in the transcriptional process. Q901 is currently being evaluated in a phase 1 clinical study for selected advanced solid tumors. Compared to previously reported CDK7 inhibitors, Q901 has increased specificity and efficacy, but the impact on transcription have not been studied in detail. In our study, we combined RNA Pol II (pan, Ser5P, Ser2P) ChIP-seq and PRO-seq techniques in the MCF7 breast cancer cell line. The findings demonstrated that the Q901 effectively altered transcription and the RNA Pol II profile in a subset of genes, leading to a significant reduction in Ser5 phosphorylation levels in RNA Pol II. The downregulated genes were associated with critical pathways including downstream of oncogenic genes (MYC and E2F), DNA damage repair, cell cycle checkpoint, and gene expression. To verify whether these genes were the direct targets of CDK7, we performed CDK7 ChIP-seq analysis. CDK7 binding exclusively to the genes that were downregulated by Q901, without any detectable binding to upregulated genes. Abstract:Developing selective CDK7 inhibitors has emerged as a promising approach for cancer treatment owing to the critical role of CDK7 in cancer progression. Starting from BTX-A51, a CK1α inhibitor that also targets CDK7 and CDK9, we designed and synthesized a series of 2,4-diaminopyrimidine derivatives as potent CDK7 inhibitors. The representative compound, 22, displayed significant enzymatic inhibitory activity and demonstrated a remarkable selectivity profile against a panel of kinases, including seven CDK subtypes. Modeling studies and molecular dynamics simulations revealed that the sulfone group of 22 significantly enhanced the binding affinity, while the acetyl group contributed to the increased selectivity of CDK7 against CDK9. Compound 22 effectively inhibited the phosphorylation of RNA polymerase II and CDK2 and resulted in G1/S phase cell cycle arrest and apoptosis in MV4-11 cells. It appears to be a promising lead compound for the development of a CDK7 inhibitor for cancer therapy.

Abstract:Cyclin-dependent kinases (CDKs) are an attractive class of cancer targets due to their role in cell-cycle regulation. CDK2 is a Ser/Thr kinase activated via interaction with its cyclin partners CCNE and CCNA, driving G1/S progression of the cell cycle. CDK2 inhibition has the potential to address multiple resistance mechanisms to CDK4/6 inhibitors in breast cancer. In addition, CCNE1 is frequently amplified and over-expressed in cancers including ovarian, with pre-clinical data linking CDK2 inhibition sensitivity to high CCNE1. First-generation inhibitors targeting CDK2 suffered from poor tolerability in the clinic likely due to off-target activities across both the CDK family and across the protein kinome. As well as achieving CDK family selectivity in cells versus key off-targets (CDK1, CDK4/6, CDK9), it's noteworthy that AZD8421 had no significant kinase inhibition outside the CDK family. X-ray co-crystal structures of AZD8421 with CDK2 provided the basis for the observed selectivity among the CDK-family. For example, hydrogen bonding interaction of AZD8421 with Lys89 (a CDK2-specific residue) near the solvent region of the ATP-binding pocket was the key requisite for achieving >300-fold selectivity over CDK9. In nanoBRET assays measuring cellular target engagement, AZD8421 had an IC50 against CDK2 of 9nM with selectivity over CDK1, CDK4 and CDK6. Further kinetic characterization of AZD8421 binding revealed a long residence time at CDK2 (2 hours) but not CDK1 (7 minutes). These kinetic studies showed that AZD8421 has a low Kd,app for CDK2 and thus even higher selectivity vs CDK1 than was measured by single timepoint assays. In a CCNE1 amplified cell line, AZD8421 potently inhibited cell proliferation (69nM, OVCAR3), correlated with inhibition of pRB, arrest in G1/S phase of the cell cycle and induction of senescence. In vitro combination assays in CDK4/6 inhibitor resistant breast cancer cell lines showed combination benefit with AZD8421 plus approved CDK4/6 inhibitors. In vivo, AZD8421 potently suppressed phosphorylation of Rb, and demonstrated robust monotherapy and CDK4/6i combination activity in breast and ovarian in vivo models. In vivo PD marker suppression and efficacy was demonstrated in CDK4/6 inhibitor resistant breast PDXs in combination with palbociclib (CDK4/6i). AZD8421 showed robust monotherapy activity in a CCNE1 amplified ovarian model OVCAR3 with regressions seen with monotherapy and in combination with palbociclib. Abstract:Abstract Background: Cyclin-dependent kinase 9 (CDK9) is a transcriptional regulator that mediates expression and downstream activity of oncogenic transcription factors (TFs) including MYC and chimeric TFs. Inhibiting CDK9 presents a promising approach to treat transcriptionally addicted cancers. KB-0742 is a potent, selective, orally bioavailable inhibitor of CDK9 that is currently being studied in a phase 1/2 dose escalation in solid tumors and NHL and cohort expansion for patients with transcription-dependent tumors (NCT04718675). Methods: KB-0742 is administered orally once daily for 3 consecutive days with 4 days off, weekly, in 28-day cycles until toxicity or disease progression. Eligibility criteria include age > 18 years, relapsed or refractory solid tumors or NHL, acceptable organ function and ECOG PS < 2. Study objectives include evaluation of safety, tolerability, PK, PD, and identification of KB-0742 MTD and RP2D using a modified Continuous Reassessment Method (mCRM). PK was measured from patient plasma and PD was assessed in peripheral blood mononuclear cells (PBMCs). PD measurements included analysis of phosphorylation of the CDK9 substrate serine 2 of the RNA Polymerase II C-terminal domain (pSER2), and changes in gene expression of prospectively defined CDK9 responsive genes. Results: As of 1 June 2023, 28 patients enrolled in dose escalation up to 60 mg. Patients had received a median of 4 (2-11) prior lines of therapy and remained on KB-0742 for a median of 86 days (10-311+). Twelve of 28 patients received at least 4 cycles of KB-0742. The most common tumor types enrolled were colorectal (5), chordoma (4), sarcoma (4), and breast (3). Treatment-emergent adverse events (TEAEs) occurring in >20% of patients included nausea, vomiting, anemia, fatigue, nervous system disorders, and peripheral edema. The most common reasons for treatment discontinuation were progressive disease, TEAEs, and withdrawal of consent. Across 4 dose levels, AUC and Cmax of KB-0742 increased linearly with a terminal half-life of 24 hours. At 60 mg, evidence of target engagement was observed by pSER2 reduction and proportional changes to CDK9 responsive genes. In the escalation phase 60 mg cohort, three patients, (2 colorectal and 1 PTCL) had MYC over-expressing tumors and achieved SD. Of the two patients with myxoid liposarcoma, both exhibited radiographic regression of their target lesions. One patient achieved 26% reduction in the sum of partial diameters lasting > 5 mos. The second patient achieved a RECIST 1.1 partial response and remains on treatment at > 12 mos. Consistent with the therapeutic hypothesis, both patients had TF fusions. Conclusions: KB-0742 treatment was well tolerated with manageable toxicity and no evidence of neutropenia. CDK9 inhibition was observed at the 60 mg dose level, and expansion cohorts in tumor types with a high prevalence of MYC amplification or overexpression and other transcriptionally addicted tumors are accruing. The MTD has not been reached and further dose escalation is continuing. Abstract:Abstract CDK7 plays a pivotal role in cell proliferation, serving as both a cell-cycle CDK regulator and a transcription machinery component. Clinical trials are currently exploring CDK7 inhibitors (CDK7i) as a second-line treatment for hormone receptor-positive (HR+) breast cancer post-CDK4/6 inhibitor (CDK4/6i) therapy progression and as a first-line treatment in triple-negative breast cancer (TNBC). This study evaluates the impact of CDK7i on transcription and CDKs activities and the synergistic potential of CDK7i in combination with CDK4/6i. Kinetic analyses indicate that CDK7i preferentially target global transcription over CDKs activities. Furthermore, a combination of CDK7i and CDK4/6i effectively suppresses cell proliferation in drug-naïve cells, albeit without the same efficacy in CDK4/6i-resistant cells. Our findings provide valuable insights into the evolving field of CDK inhibition strategies and provide critical guidance on the potential therapeutic application of CDK7i in breast cancer treatment. Abstract:CDK4/6 inhibitors have proven effective in treating HR+/HER2- breast cancer, but their limited therapeutic indications and the emergence of acquired resistance are concerning. Recent studies suggest that hematological toxicity linked to CDK4/6 dual inhibitors may be due to strong CDK6 inhibition. To address these challenges, small molecules targeting different CDK subfamily members could provide clinical advantages in specific patient subgroups and expand CDK inhibitor indications beyond breast cancer. However, the high conservation of CDK's binding pocket makes designing molecules selectively targeting different CDKs challenging. In this study, we use in-depth in silico modeling and structural biology analysis of co-crystal data to analyze subtle variations in several CDKs' key binding pockets. Our findings indicate that a sequential difference of a αD-helix motif between CDK4 and CDK6 represents an ideal "sweet spot" that influences binding selectivity. To achieve this selectivity, we designed small molecules with a 1,4-trans-cyclohexanediamine sidechain that specifically promotes interactions with the αD-helix motif in CDK4 but not in CDK6. Interestingly, the same αD-helix motif in CDK9 presented an opportunity to develop a CDK4/9 inhibitor, potentially improving acquired resistance to CDK4/6 inhibitors. Our rational approach identified a series of analogs exhibiting distinct in vitro selectivity among CDK4, CDK6, and CDK9. The lead compound (29) was further evaluated for in vivo pharmacokinetics. This work revealed key molecular insights for developing next-generation CDK inhibitors with unique selectivity profiles. This approach could help overcome the challenges of developing selective CDK inhibitors and broaden their therapeutic indications beyond breast cancer.

Abstract:Cyclin Dependent Kinase 9 (CDK9) is one of the most important transcription regulatory members of the CDK family. In conjunction with its main cyclin partner—Cyclin T1, it forms the Positive Transcription Elongation Factor b (P-TEFb) whose primary function in eukaryotic cells is to mediate the positive transcription elongation of nascent mRNA strands, by phosphorylating the S2 residues of the YSPTSPS tandem repeats at the C-terminus domain (CTD) of RNA Polymerase II (RNAP II). To aid in this process, P-TEFb also simultaneously phosphorylates and inactivates a number of negative transcription regulators like 5,6-dichloro-1-β-D-ribofuranosylbenzimidazole (DRB) Sensitivity-Inducing Factor (DSIF) and Negative Elongation Factor (NELF). Significantly enhanced activity of CDK9 is observed in multiple cancer types, which is universally associated with significantly shortened Overall Survival (OS) of the patients. In these cancer types, CDK9 regulates a plethora of cellular functions including proliferation, survival, cell cycle regulation, DNA damage repair and metastasis. Due to the extremely critical role of CDK9 in cancer cells, inhibiting its functions has been the subject of intense research, resulting the development of multiple, increasingly specific small-molecule inhibitors, some of which are presently in clinical trials. The search for newer generation CDK9 inhibitors with higher specificity and lower potential toxicities and suitable combination therapies continues. In fact, the Phase I clinical trials of the latest, highly specific CDK9 inhibitor BAY1251152, against different solid tumors have shown good anti-tumor and on-target activities and pharmacokinetics, combined with manageable safety profile while the phase I and II clinical trials of another inhibitor AT-7519 have been undertaken or are undergoing. To enhance the effectiveness and target diversity and reduce potential drug-resistance, the future of CDK9 inhibition would likely involve combining CDK9 inhibitors with inhibitors like those against BRD4, SEC, MYC, MCL-1 and HSP90.

The CDK2 inhibitor field has historically suffered from high attrition in Phase I due to off-target toxicity. AZD8421 is a uniquely selective CDK2 inhibitor, that offers potential to show improved therapeutic index and combinability. As well as achieving CDK family selectivity in cells versus key off-targets (CDK1, CDK4/6, CDK9), AZD8421 had no significant kinase inhibition outside the CDK family. Only 7 distinct targets showed greater than 50% inhibition in a panel of 403 kinases when screened with 1μM of AZD8421. In nanoBRET assays measuring cellular target engagement, AZD8421 had an IC50 against CDK2 of 9nM with selectivity over CDK1, CDK4 and CDK6. In vitro cellular assays demonstrated that AZD8421 inhibits endogenous phospho-substrate at 58nM with >327-fold selectivity over CDK9 phospho-substrate pSer2 of RNAPII (>19.2 μM). In a CCNE1 amplified cell line, AZD8421 potently inhibited cell proliferation (69nM, OVCAR3), correlated with inhibition of pRB, arrest in G1/S phase of the cell cycle and induction of senescence, compared to a CCNE1 non-amplified cell line (SKOV3) with IC50 of 2.05 μM. In vitro combination assays in CDK4/6 inhibitor resistant breast cancer cell lines showed combination benefit with AZD8421 plus approved CDK4/6 inhibitors. In vivo, AZD8421 potently suppressed phosphorylation of Rb, and demonstrated robust monotherapy and CDK4/6i combination activity in breast and ovarian in vivo models. In vivo PD marker suppression and efficacy was demonstrated in CDK4/6 inhibitor resistant breast PDXs in combination with palbociclib (CDK4/6i).
