Abstract:Objective Noisy galvanic vestibular stimulation (GVS) using weak random noise waveforms enhances postural stability by modulating vestibular-related neural networks. This study aimed to investigate the neural interference mechanisms of noisy GVS on lateral vestibulospinal tract (LVST) excitability. Methods Twenty-six healthy volunteers were randomly divided into two groups: balance training combined with noisy GVS and sham GVS. Participants performed 10-minute balance training while standing on a soft foam surface with their eyes closed while adapting to each electrical stimulus. LVST excitability was assessed by measuring the soleus H-reflex following square-wave pulse GVS. Postural stability was measured by assessing the center of foot pressure sway while standing on a foam surface with eyes closed. Results The noisy GVS group showed significantly increased post-intervention H-reflex amplitude. The sham GVS group showed no significant difference in H-reflex amplitude pre- and post-intervention. The average sway velocity in the noisy and sham GVS groups significantly decreased in the medial-lateral direction of the center of foot pressure. Conclusions Noisy GVS may enhance LVST excitability and decrease body sway via vestibular system interference during holding upright, which relies heavily on vestibular sensations. Significance These findings may help understand the neural mechanisms of noisy GVS in neurorehabilitation.

Abstract:Noisy galvanic vestibular stimulation (nGVS) is an emerging non-invasive brain stimulation technique. It involves applying alternating currents of different frequencies and amplitudes presented in a random, or noisy, manner through electrodes on the mastoid bones behind the ears. Because it directly activates vestibular hair cells and afferents and has an indirect effect on a variety of brain regions, it has the potential to impact many different functions. The objective of this review is twofold: (1) to review how nGVS affects motor, sensory, and cognitive performance in healthy adults; and (2) to discuss potential clinical applications of nGVS. First, we introduce the technique. We then describe the regions receiving and processing vestibular information. Next, we discuss the effects of nGVS on motor, sensory, and cognitive function in healthy adults. Subsequently, we outline its potential clinical applications. Finally, we highlight other electrical stimulation technologies and discuss why nGVS offers an alternative or complementary approach. Overall, nGVS appears promising for optimizing human performance and as an assistive technology, though further research is required.
Effect of noisy galvanic vestibular stimulation on dynamic posture sway under visual deprivation in patients with bilateral vestibular hypofunction

Abstract:Abstract A single-blind study to investigate the effects of noisy galvanic vestibular stimulation (nGVS) in straight walking and 2 Hz head yaw walking for healthy and bilateral vestibular hypofunction (BVH) participants in light and dark conditions. The optimal stimulation intensity for each participant was determined by calculating standing stability on a force plate while randomly applying six graded nGVS intensities (0–1000 μA). The chest–pelvic (C/P) ratio and lateral deviation of the center of mass (COM) were measured by motion capture during straight and 2 Hz head yaw walking in light and dark conditions. Participants were blinded to nGVS served randomly and imperceivably. Ten BVH patients and 16 healthy participants completed all trials. In the light condition, the COM lateral deviation significantly decreased only in straight walking ( p = 0.037) with nGVS for the BVH. In the dark condition, both healthy ( p = 0.026) and BVH ( p = 0.017) exhibited decreased lateral deviation during nGVS. The C/P ratio decreased significantly in BVH for 2 Hz head yaw walking with nGVS ( p = 0.005) in light conditions. This study demonstrated that nGVS effectively reduced walking deviations, especially in visual deprived condition for the BVH. Applying nGVS with different head rotation frequencies and light exposure levels may accelerate the rehabilitation process for patients with BVH. Noise-Enhanced Vestibular Input Improves Dynamic Walking Stability in Healthy Subjects

Abstract:Background: White noise galvanic vestibular stimulation (GVS) is thought to enhance the sensitivity of vestibular organs. Objective: To examine the effects of noise-enhanced vestibular input on the walking performance in healthy subjects walking with eyes closed. Methods: Walking performance of 17 healthy subjects (mean age 28.8 ± 1.7 years) at slow, preferred, and fast speeds was examined during three different conditions: (1) walking with eyes open (EO) as baseline condition, (2) walking with eyes closed and sham noisy GVS (EC), and (3) walking with eyes closed and non-zero amplitude noisy GVS set to 80% of the individual sensory threshold for GVS (EC-GVS). Ten gait parameters were examined: stride time, stride length, base of support, swing time percentage, double support time percentage as well as gait asymmetry, bilateral phase coordination and the coefficient of variation (CV) of stride time, stride length and base of support. Results: Noisy GVS improved stride time CV by 36% (p < 0.034), stride length CV by 31% (p < 0.037), base of support CV by 14% (p < 0.009), and bilateral phase coordination by 23% (p < 0.034). The ameliorating effects of noisy GVS on locomotion function were primarily observable during slow walking speeds. Conclusion: Noise-enhanced vestibular input is effective in improving locomotion function and is accompanied by a subjectively felt improvement of walking balance. It predominantly targets the variability and bilateral coordination characteristics of the walking pattern, which are critically linked to dynamic walking stability. Noisy GVS might present an effective treatment option to improve walking performance in patients with bilateral vestibular dysfunction.
No evidence for effects of low-intensity vestibular noise stimulation on mild-to-moderate gait impairments in patients with Parkinson's disease

Abstract:Background: Gait impairment is a key feature in later stages of Parkinson's disease (PD), which often responds poorly to pharmacological therapies. Neuromodulatory treatment by low-intensity noisy galvanic vestibular stimulation (nGVS) has indicated positive effects on postural instability in PD, which may possibly be conveyed to improvement of dynamic gait dysfunction. Objective: To investigate the effects of individually tuned nGVS on normal and cognitively challenged walking in PD patients with mild-to-moderate gait dysfunction. Methods: Effects of nGVS of varying intensities (0-0.7 mA) on body sway were examined in 32 patients with PD (ON medication state, Hoehn and Yahr: 2.3 ± 0.5), who were standing with eyes closed on a posturographic force plate. Treatment response and optimal nGVS stimulation intensity were determined on an individual patient level. In a second step, the effects of optimal nGVS vs. sham treatment on walking with preferred speed and with a cognitive dual task were investigated by assessment of spatiotemporal gait parameters on a pressure-sensitive gait carpet. Results: Evaluation of individual balance responses yielded that 59% of patients displayed a beneficial balance response to nGVS treatment with an average optimal improvement of 23%. However, optimal nGVS had no effects on gait parameters neither for the normal nor the cognitively challenged walking condition compared to sham stimulation irrespective of the nGVS responder status. Conclusions: Low-intensity nGVS seems to have differential treatment effects on static postural imbalance and continuous gait dysfunction in PD, which could be explained by a selective modulation of midbrain-thalamic circuits of balance control.

Abstract:Holding and processing actions in visual working memory (VWM) is crucial for daily functioning, but it is capacity-limited. The vestibular system, which is activated by both vestibular signals and visual gravitational motion, may be involved in this cognitive process. Past research indicates that galvanic vestibular stimulation (GVS) is a non-invasive technique that can enhance motor functions and various cognitive functions. However, the impact of GVS on VWM for actions has not been explored. This study addresses this gap by investigating whether GVS can improve VWM capacity for both upright and inverted actions. Using a virtual reality platform, we assessed VWM capacity in a change detection task, applying either STIM-current (0.8 mA) or SHAM-current (0 mA) GVS. The results showed no significant effect of GVS on VWM for either upright or inverted actions, at least for non-noisy GVS with stimulation intensity below cutaneous threshold. Despite the absence of significant effects, this study provides valuable insights into the interactions between the vestibular system and cognitive processes, laying the groundwork for future research on GVS applications in cognitive enhancement.

Mechanisms underlying treatment effects of vestibular noise stimulation on postural instability in patients with bilateral vestibulopathy

Abstract:Background: Previous studies indicate that imbalance in patients with bilateral vestibulopathy (BVP) may be reduced by treatment with low-intensity noisy galvanic vestibular stimulation (nGVS). Objective: To elucidate the potential mechanisms underlying this therapeutic effect. In particular, we determined whether nGVS-induced balance improvements in patients are compatible with stochastic resonance (SR)-a mechanism by which weak noise stimulation can paradoxically enhance sensory signal processing. Methods: Effects of nGVS of varying intensities (0-0.7 mA) on body sway were examined in 19 patients with BVP standing with eye closed on a posturographic force plate. We assumed a bell-shaped response curve with maximal sway reductions at intermediate nGVS intensities to be indicative of SR. An established SR curve model was fitted on individual patient outcomes, and three experienced human raters had to judge whether responses to nGVS were consistent with the exhibition of SR. Results: nGVS-induced reductions of body sway compatible with SR were found in 12 patients (63%) with optimal improvements of 31 ± 21%. In 10 patients (53%), nGVS-induced sway reductions exceeded the minimally important clinical difference (optimal improvement: 35 ± 21%), indicative of strong SR. This beneficial effect was more likely in patients with severe vestibular loss (i.e. lower video head impulse test gain; R = 0.663; p = 0.002) and considerable postural imbalance (baseline body sway; R = 0.616; p = 0.005). Conclusions: More than half of the assessed patients showed robust improvements in postural balance compatible with SR when treated with nGVS. In particular, patients with a higher burden of disease may benefit from the non-invasive and well-tolerated treatment with nGVS.

Abstract:Objective: To examine the effects of imperceptible levels of white noise galvanic vestibular stimulation (nGVS) on dynamic walking stability in patients with bilateral vestibulopathy (BVP). Methods: Walking performance of 13 patients with confirmed BVP (mean age 50.1 ± 5.5 years) at slow, preferred, and fast speeds was examined during walking with zero-amplitude nGVS (sham trial) and nonzero-amplitude nGVS set to 80% of the individual cutaneous threshold for GVS (nGVS trial). Eight standard gait measures were analyzed: stride time, stride length, base of support, double support time percentage as well as the bilateral phase coordination index, and the coefficient of variation (CV) of stride time, stride length, and base of support. Results: Compared to the sham trial, nGVS improved stride time CV by 26.0% ± 8.4% (p < 0.041), stride length CV by 26.0% ± 7.7% (p < 0.029), base of support CV by 27.8% ± 2.9% (p < 0.037), and phase coordination index by 8.4% ± 8.8% (p < 0.013). The nGVS effects on walking performance were correlated with subjective ratings of walking balance (ρ = 0.79, p < 0.001). Effect of nGVS on walking stability was most pronounced during slow walking. Conclusions: In patients with BVP, nGVS is effective in improving impaired gait performance, predominantly during slower walking speeds. It primarily targets the variability and bilateral coordination characteristics of the walking pattern, which are linked to dynamic walking stability. nGVS might present an effective treatment option to immediately improve walking performance and reduce the incidence of falls in patients with BVP. Classification of evidence: This study provides Class IV evidence that in patients with BVP, an imperceptible level of nGVS improves dynamic walking stability.

Abstract:Background: Parkinson's disease (PD) is a common neurodegenerative disease characterized by dopaminergic neuronal loss. The underlying cause of PD is unknown.Objective: To assess the clinical relevance of vestibular-ocular reflex (VOR) gain in patients with PD, especially those in the early stages.Methods: Sixty-three PD patients and 56 control healthy individuals were enrolled in this study between Mar 2015 and Aug 2015. VOR gains were determined by video head impulse test (vHIT) device. Statistical analysis was performed to assess the difference in VOR gains between PD patients and normal people. The relationship of VOR gain with age, duration and severity of disease was also assessed.Results: In the control group, average VOR gain was 0.98 +/- 0.09 on the left side and 0.99 +/- 0.16 on the right side. No statistically significant difference was observed between the two sides in the control group (P>0.05). In the PD group, average VOR gain was 1.20 +/- 0.22 on the left side and 1.23 +/- 0.23 on the right side. No statistically significant difference was observed between the two sides in the PD group (P>0.05). There was a significant difference in VOR gain between the PD (both in early and mid-late stages) and the control group (P < 0.05). A weak correlation was observed between VOR gain and the motor Unified Parkinson Disease Rating Scale score. No correlation of VOR gain with age, duration of disease or the Hoehn and Yahr Scale score was observed. VOR gains in PD patients were found to be higher than normal, especially in the early stages of the disease.Conclusion: vHIT is a potential tool to determine the VOR gain in PD patients and may help detect PD at an early stage. Abstract:We investigated whether noisy galvanic vestibular stimulation (nGVS) modulates the vestibulo-ocular reflex (VOR) and whether this effect is correlated with the effect of nGVS on body sway. Thirty healthy young adults participated. The video head impulse test (vHIT) was used to estimate the ratio of eye motion velocity/head motion velocity to VOR-gain. The gain 60 ms after the start of head motion (VOR-gain-60 ms) and regression slope (RS) (i.e., gain in eye and head motion; VOR-gain-RS) were calculated. The total path length of the foot center of pressure (COP-TL) during upright standing was calculated to estimate body sway. Noisy Galvanic Vestibular Stimulation at 0.2, 0.6, 1.2 mA, or sham stimulation (direct current: 0 mA) was delivered to the bilateral mastoid process in random order during vHIT and COP measurements. Application of nGVS at 0.2 mA significantly reduced VOR-gain-RS, while application of nGVS at 0.6 mA significantly increased COP-TL. Vestibulo-ocular reflex-gain-60 ms differed significantly between 0.2 and 1.2 mA. There was no significant correlation between COP-TL and VOR-related parameters. These findings suggest that nGVS at 0.2 mA inhibits the VOR, while nGVS at 0.6 mA increases body sway during upright standing, although there may be no relationship between the respective effects in healthy individuals.

Galvanic Vestibular Stimulation Improves Subnetwork Interactions in Parkinson’s Disease

Abstract:Background. Activating vestibular afferents via galvanic vestibular stimulation (GVS) has been recently shown to have a number of complex motor effects in Parkinson’s disease (PD), but the basis of these improvements is unclear. The evaluation of network-level connectivity changes may provide us with greater insights into the mechanisms of GVS efficacy. Objective. To test the effects of different GVS stimuli on brain subnetwork interactions in both health control (HC) and PD groups using fMRI. Methods. FMRI data were collected for all participants at baseline (resting state) and under noisy, 1 Hz sinusoidal, and 70-200 Hz multisine GVS. All stimuli were given below sensory threshold, blinding subjects to stimulation. The subnetworks of 15 healthy controls and 27 PD subjects (on medication) were identified in their native space, and their subnetwork interactions were estimated by nonnegative canonical correlation analysis. We then determined if the inferred subnetwork interaction changes were affected by disease and stimulus type and if the stimulus-dependent GVS effects were influenced by demographic features. Results. At baseline, interactions with the visual-cerebellar network were significantly decreased in the PD group. Sinusoidal and multisine GVS improved (i.e., made values approaching those seen in HC) subnetwork interactions more effectively than noisy GVS stimuli overall. Worsening disease severity, apathy, depression, impaired cognitive function, and increasing age all limited the beneficial effects of GVS. Conclusions. Vestibular stimulation has widespread system-level brain influences and can improve subnetwork interactions in PD in a stimulus-dependent manner, with the magnitude of such effects associating with demographics and disease status.

Abstract:Background Postural imbalance and falls are an early disabling symptom in patients with progressive supranuclear palsy (PSP) of multifactorial origin that may involve abnormal vestibulospinal reflexes. Low-intensity noisy galvanic vestibular stimulation (nGVS) is a non-invasive treatment to normalize deficient vestibular function and attenuate imbalance in Parkinson's disease. The presumed therapeutic mode of nGVS is stochastic resonance (SR), a mechanism by which weak sensory noise stimulation can enhance sensory information processing. Objective To examine potential treatment effects of nGVS on postural instability in 16 patients with PSP with a clinically probable and [ 18 F]PI-2620 tau-PET-positive PSP. Methods Effects of nGVS of varying intensity (0–0.7 mA) on body sway were examined, while patients were standing with eyes closed on a posturographic force plate. We assumed a bell-shaped response curve with maximal sway reductions at intermediate nGVS intensities to be indicative of SR. An established SR-curve model was fitted on individual patient outcomes and three experienced human raters had to judge whether responses to nGVS were consistent with the exhibition of SR. Results We found nGVS-induced reductions of body sway compatible with SR in 9 patients (56%) with optimal improvements of 31 ± 10%. In eight patients (50%), nGVS-induced sway reductions exceeded the minimal clinically important difference (improvement: 34 ± 5%), indicative of strong SR. Conclusion nGVS yielded clinically relevant reductions in body sway compatible with the exhibition of SR in vestibular sensorimotor pathways in at least half of the assessed patients. Non-invasive vestibular noise stimulation may be thus a well-tolerated treatment strategy to ameliorate postural symptoms in PSP.

Abstract:We recently showed that vestibular stimulation can produce a long-lasting alleviation of motor features in Parkinson's disease. Here we investigated whether components of the motor related cortical response that are commonly compromised in Parkinson's - the Bereitschaftspotential and mu-rhythm event-related desynchronization - are modulated by concurrent, low frequency galvanic vestibular stimulation (GVS) during repetitive limb movement amongst 17 individuals with idiopathic Parkinson's disease. Relative to sham, GVS was favourably associated with higher amplitudes during the late and movement phases of the Bereitschaftspotential and with a more pronounced decrease in spectral power within the mu-rhythm range during finger-tapping. These data increase understanding of how GVS interacts with the preparation and execution of voluntary movement and give added impetus to explore its therapeutic effects on Parkinsonian motor features.
The effect of galvanic vestibular stimulation on postural balance in Parkinson's disease: A systematic review and meta-analysis

Abstract:People with Parkinson's disease (PD) develop postural imbalance and falls. Galvanic Vestibular Stimulation (GVS) may potentially improve postural balance in humans and hence reduce falls in PD. This systematic review and meta-analysis investigate the effects of GVS on postural balance in PD. Six separate databases and research registers were searched for cross-over design trials that evaluated the effects of GVS on postural balance in PD. We used standardized mean difference (Hedges' g) as a measure of effect size in all studies. We screened 223 studies, evaluated 14, of which five qualified for the meta-analysis. Among n = 40 patients in five studies (range n = 5 to 13), using a fixed effects model we found an effect size estimate of g = 0.43 (p < 0.001, 95% CI [0.29,0.57]). However, the test for residual heterogeneity was significant (p < 0.001), thus we used a random effects model and found a pooled effect size estimate of 0.62 (p > 0.05, 95% CI [- 0.17, 1.41], I2 = 96.21%). Egger's test was not significant and thus trim and funnel plot indicated no bias. To reduce heterogeneity, we performed sensitivity analysis and by removing one outlier study (n = 7 patients), we found an effect size estimate of 0.16 (p < 0.05, 95% CI [0.01, 0.31], I2 = 0%). Our meta-analysis found GVS has a favourable effect on postural balance in PD patients, but due to limited literature and inconsistent methodologies, this favourable effect must be interpreted with caution.

Abstract:Background Stochastic resonance (SR) refers to a faint signal being enhanced with the addition of white noise. Previous studies have found that vestibular perceptual thresholds are lowered with noisy galvanic vestibular stimulation (i.e., “in-channel” SR). Auditory white noise has been shown to improve tactile and visual thresholds, suggesting “cross-modal” SR. Objective We investigated galvanic vestibular white noise (nGVS) ( n = 9 subjects) to determine the cross-modal effects on visual and auditory thresholds. Methods We measured auditory and visual perceptual thresholds of human subjects across a swath of different nGVS levels in order to determine if some individual-subject determined best nGVS level elicited a reduction in thresholds as compared the no noise condition (sham). Results We found improvement in visual thresholds (by an average of 18%, p = 0.014). Subjects with higher (worse) visual thresholds with no stimulation (sham) improved more than those with lower thresholds ( p = 0.04). Auditory thresholds were unchanged by vestibular stimulation. Conclusion These results are the first demonstration of cross-modal improvement with galvanic vestibular stimulation, indicating galvanic vestibular white noise can produce cross-modal improvements in some sensory channels, but not all.

The Effect of taVNS at 25 Hz and 100 Hz on Parkinson's Disease Gait-A Randomized Motion Sensor Study

Abstract:Background: Transcutaneous electrostimulation of the auricular branch of the vagal nerve (taVNS) has the propensity to reach diffuse neuromodulatory networks, which are dysfunctional in Parkinson's disease (PD). Previous studies support the use of taVNS as an add-on treatment for gait in PD. Objectives: We assessed the effect of taVNS at 25 Hz (taVNS25), taVNS at 100 Hz (taVNS100), and sham earlobe stimulation (sVNS) on levodopa responsive (arm swing velocity, arm range of motion, stride length, gait speed) and non-responsive gait characteristics (arm range of motion asymmetry, anticipatory postural adjustment [APA] duration, APA first step duration, APA first step range of motion), and turns (first turn duration, double 360° turn duration, steps per turn) in advanced PD. Methods: In our double blind sham controlled within-subject randomized trial, we included 30 PD patients (modified Hoehn and Yahr stage, 2.5-4) to assess the effect of taVNS25, taVNS100, and sVNS on gait characteristics measured with inertial motion sensors during the instrumented stand and walk test and a double 360° turn. Separate generalized mixed models were built for each gait characteristic. Results: During taVNS100 compared to sVNS arm swing velocity (P = 0.030) and stride length increased (P = 0.027), and APA duration decreased (P = 0.050). During taVNS25 compared to sVNS stride length (P = 0.024) and gait speed (P = 0.021) increased and double 360° turn duration decreased (P = 0.039). Conclusions: We have found that taVNS has a frequency specific propensity to improve stride length, arm swing velocity, and gait speed and double 360° turn duration in PD patients. © 2024 The Authors. Abstract:Low-intensity noisy galvanic vestibular stimulation (nGVS) is a promising non-invasive treatment for enhancing vestibular perceptual performance and postural control in patients with chronic vestibular hypofunction. However, this approach has so far been studied mainly under laboratory conditions. Evidence indicates that continuous application of nGVS in daily life is necessary for it to be effective. To address this need, we have developed a mobile nGVS stimulator and conducted a series of pilot studies to evaluate its safety, tolerability, functionality, and therapeutic effects. The device is a lightweight, compact, and portable AC stimulator featuring a user-friendly interface for the individualized adjustment of nGVS parameters. It includes an integrated motion sensor that automatically activates stimulation during body movement and deactivates it during inactivity, optimizing its practical use in real-world settings. The stimulator adheres to strict safety standards and, in initial long-term use, has exhibited only mild side effects (e.g., skin irritation and headaches), likely attributable to the current electrode placement, which requires further optimization. As expected, the device consistently elicits known vestibular sensorimotor reflex responses in healthy individuals. Importantly, further pilot studies in healthy participants demonstrate that the device can reliably replicate known facilitating effects on vestibular perception and postural control. Together, these findings suggest that this mobile stimulation device can facilitate the translation of nGVS into therapeutic everyday use.

Abstract:Background Perceptible galvanic vestibular stimulation (GVS) causes nystagmus and postural sway deterioration. Conversely, imperceptible GVS improves postural stability, suggesting the presence of stochastic resonance. Research question Similar to GVS, strong magnetic vestibular stimulation of 7 T induces nystagmus and increases body sway. Thus, a relatively small magnetic stimulation may improve postural stability. In this study, we measured the effect of a relatively small magnetic field on postural sway. Methods Posturography was performed in eight healthy participants using a stabilometer with foam rubber on board. The center of pressure (COP) trajectories were recorded in both the anterior–posterior and medial–lateral directions for 60 s with the eye closed. Neodymium magnets (0.4 T) or aluminum disks of similar size (0 T) were placed bilaterally over the mastoid processes. Results Both the trajectory length and envelopment area of the COP movement with 0.4 T were significantly smaller than those with 0 T. Significance The relatively smaller magnetic vestibular stimulation decreased postural sway. This method may be useful for improving the vestibular function and related reflexes.

Abstract:A rapidly aging population worldwide has spurred interest in developing new strategies to cope with neural declines and neurodegenerative disorders. Noninvasive brain stimulation (NIBS) is increasingly being used to explore functional mechanisms of the brain and induce the therapeutic modulation of behavior, cognition, and emotion. Galvanic vestibular stimulation (GVS), a safe and well-tolerated NIBS technique, is capable of modulating activity in various cortical and subcortical areas involved in vestibular and multisensory processing. A key facet of GVS is that the resultant effects may, in part, be a function of the individual being treated and the stimulus waveform that is delivered. Yet, most GVS studies have utilized the same generic stimulus, chosen from a reduced repertoire of candidates, across all subjects. The future use and, ultimately, clinical adoption of this technology will rely on contributions from the signal processing community to customize stimuli that are optimized for their effect and to exert maximum influence on brain imaging biomarkers. We provide a signal processing-focused overview of the current GVS state of the art in neurorehabilitation, including general stimulation design, concurrent analysis with neuroimaging data, and suggestions for future directions.
