Abstract:Objective Analysis of data on monetary compensation and estimating the rate of injuries related to agricultural activities in the state of Punjab. Methods The primary data were collected from 22 districts of Punjab state for 2012–2015 through Punjab State Agricultural Marketing Board, Mohali. Data were analysed for the nature of health hazards, type of injuries, possible causes and factors of injuries and their gender. Results During the four consecutive years (2012–2015) an overall 5888 hazards cases related to agricultural activities were reported from the state, out of which 1993 (33.85%) were fatal and 3895 (66.15%) were non-fatal injuries. Among the fatal cases, the leading causes reported were due to electric motors (34.47%), poisoning through snake bites (21.48%), poisoning through pesticide applications with sprayers (19.62%) and crushed under/falling from the tractors and related equipment (13.50%). The annual fatality rate of the state of Punjab was estimated as 14.14 per 100 000 agricultural workers, while the overall injury incidence rate was observed as 39.57 per 100 000 agricultural workers. Conclusions Majority of agricultural work-related health hazards in Punjab were mainly due to negligence, lack of knowledge/experience, restlessness/sleeplessness, inadequate safety measures, over speed and wrong practices employed by the operator. There is a need for better ergonomic controls, work environments and practices for the prevention of injuries and health hazards related to agricultural activities.

Estimating patterns in the under-reporting of suicide deaths in India: comparison of administrative data and Global Burden of Disease Study estimates, 2005–2015

Abstract:Background It has been proposed that the National Crime Records Bureau (NCRB), which reports suicides in India, differentially underenumerates suicides by geographic and demographic factors. We assessed the extent of potential underenumeration by comparing suicides recorded in NCRB data with recent estimates of Indian suicides developed by the Global Burden of Disease (GBD) initiative. Methods Age-standardised suicide rates were calculated for both data sources by sex, age and state, and rate ratios of NCRB to GBD estimates by corresponding strata were compared to ascertain the relative under-reporting in the NCRB report. Results The GBD Study reported an additional 802 684 deaths by suicide (333 558 male and 469 126 female suicide deaths) compared with the NCRB report between 2005 and 2015. Among males, the average under-reporting was 27% (range 21%–31%) per year, and among females, the average under-reporting was 50% (range 47%–54%) per year. Under-reporting was more evident among younger (15–29 years) and older age groups (≥60 years) compared with middle age groups. Indian states belonging to low Socio-Demographic Index (SDI) generally had greater underenumeration compared with middle and high-SDI states. Conclusion NCRB data under-report suicides in India, and differentially by sex, age and geographic area, possibly because of lack of community-level reporting of suicides due to social stigma and legal consequences. While the recent decriminalisation of suicide is expected to improve community-level reporting of suicides, suicide prevention policies should be developed, with a priority to address social stigma attached with suicide and suicidal behaviour, especially among females.

Pharmacological inhibition of EZH2 promotes SESTRIN1 re-expression and it restores its tumor suppressive activity, suggesting the possibility to epigenetically control mTORC1 activity.

Abstract:An effective Frenkel-exciton Hamiltonian for the LH2 photosynthetic complex from Rhodospirillum molischianum is calculated using the collective electronic oscillator (CEO) approach combined with the crystal structure. The absorption spectra of the various bacteriochlorophyll aggregates forming the complex are computed using the CEO. Each electronic transition is further analyzed in terms of its characteristic electron−hole motions in real space. Using a two-dimensional representation of the underlying transition density matrices, we identify localized and delocalized electronic transitions, test the applicability of the exciton model, and compute interchromophore electronic couplings. Forster energy-transfer hopping time scales within B800 and from the B800 to the B850 system, obtained using the computed coupling constants, are in excellent agreement with experiment.
A postmortem study of demographics and severity of thoracoabdominal injuries among patients attending a teaching hospital in northeast India

Abstract:Introduction: The global estimate reflects 4.4 million injury-related deaths every year and the thoracoabdominal region is frequently injured since it is a wide area. Methods: An autopsy-based cross-sectional study was conducted using a convenient sampling technique for three years. A Chi-square test was conducted for categorical variables. A P value of less than 0.01 was considered statistically significant. Results: Out of 80 cadavers, the male:female ratio was 1:0.31. The commonest age group was 20-29 years. Victims of road traffic accidents were 54 (67.5%) followed by falls from height 10 (12.5%). External thoracic injuries were commoner than internal and vice versa in abdominal injuries. Liver injuries were major in number. Combined thoracic-abdominal and associated injuries were observed in 67 (83.75%) victims. The commonest cause of death was craniocerebral injury combined with hemorrhagic shock (36.25%). Eighteen (22.5%) victims died on the spot having an Injury Severity Score (ISS) of 73.37 followed by a survival time of 1-7 days in 17 (21.25%) cases. A significant correlation was found between ISS and survival period. Conclusion: All thoracic and abdominal injuries represent a possible increase in morbidity and mortality; hence, working toward their prevention and timely intervention is necessary.
Epidemiology of cervical spinal cord injury in eastern India: an autopsy-based study

Abstract:Aims: Cervical spinal cord injury is a major cause of fatality among trauma victims. Unlike developed countries, national spinal cord injury database is not maintained in India. Paucity of data hampers the understanding of epidemiology of the event in India. So, this study was conducted to ascertain the epidemiological pattern of death due to cervical spinal cord injury and to identify the risk factors; also to find the association of vertebral injuries with various epidemiological parameters. Methods: Among autopsies conducted in Burdwan Medical College and Hospital from 2000 to 2010, cases with autopsy report mentioning "death due to cervical spinal cord injury" either primary or secondary were included in the study. It was a retrospective observational study based on autopsy report. Results: 536 cases met the inclusion criteria. Among them 89.4% were male and 10.6% were female; 63.8% victims were young adults (20-39 years). Mean (plus or minus S.D.) age was 33.5 years (plus or minus 12.8). All, apart from 5.4%, suffered from single segment injury with commonest site being C3-C4 (37.3%). Highest number of trauma occurred between 6 am to 9 am, 56.6% of the victims died before hospitalisation. Commonest mode of injury was road traffic accident (52.2%) followed by fall from height (25.0%); 53.7% cases had vertebral fracture with dislocation, 34.3% had only vertebral fracture. Only dislocation was significantly high up to 19 years age (p<0.001) and among females (p=0.029). All elderly suffered from fracture. Conclusions: This study revealed an unusually high male:female ratio with much representation of the younger age groups in the injuries in comparison to other parts of world. In absence of national database, this result can be used as a surrogate data.

Abstract:INTRODUCTION: Expansion in road network, motorization, and urbanization in the country has been accompanied by a rise in road accidents leading to road traffic injuries (RTIs). Today RTIs are one of the leading causes of deaths, disabilities, and hospitalizations with severe socioeconomic costs across the world.OBJECTIVES: The following study analyses the: Age and sex distribution of injured in road traffic accidents (RTAs).Circumstances leading to RTA.Pattern and severity of injuries sustained in RTAs cases.DESIGN: Retrospective record-based study.MATERIALS AND METHODS: The aim of this study was to audit retrospectively the circumstances, severity, and pattern of injury sustained by vehicle occupants presenting to the Saraswathi Institute of Medical Sciences (SIMS) hospital Hapur, for a period of one year. Data were collected using the case sheets of 347 patients from the medical records section of hospital and analyzed using SPSS computer software version 16.0. Results are interpreted in terms of percentage, mean, chi-square, and z-test.RESULTS: The pattern and severity of injuries sustained by 347 vehicle occupants admitted to the emergency department of SIMS, Hapur were retrospectively documented. Male victims 258 (74.35%) were more commonly involved than females 89 (25.65%) and majority of victims 141 (40.63%) were in age group of 20-30 years. Urban victims 222 (64.00%) outnumbered rural. The most frequently injured body regions were the extremities 499 (53.54%), followed by maxillofacial180(19.31%).. Out of total 802 external injuries, the most common type of injury was lacerations 307 (38.28%), abrasions 306 (38.15%)and followed by bruises 154 (19.20%). Multiple external injuries were more common on upper limb 216 (26.93%), lower limbs 210 (26.18%) and face 170 (21.20%), while crush injuries were more predominently seen in both the limbs. While laceration were common on face 120 (38.83%). Injuries to the chest 19 (2.36%), abdomen13 (1.61%), and spine 11 (1.36%) were seen in roughy equal proprotion of victims. The bones on right side 55 (55.55%) were more commonly fractured which is statistically significant. Skull injuries were mostly found on frontal 77 (47.53%), followed by parietal bone 33 (20.37%), mostly on right side.CONCLUSION: RTAs constitute a major public health problem in our setting. Urgent preventive measures targeting at reducing the occurrence of RTAs are necessary to reduce the morbidity and mortality resulting from these injuries.
Magnitude and Risk Factors of Injuries in a Glass Bottle Manufacturing Plant

Abstract:A study was conducted in a glass bottle manufacturing plant in Pondicherry, India, to assess the magnitude and identify the risk factors of work-related injuries between January and December 1998. Three hundred and seventy-seven injuries were reported among 341 permanent workers followed up for one year (incidence=1,105.5/1,000 workers/yr). A higher load of injuries was noted in the first half of the night shifts and the second half of the other three shifts. Injuries were higher in the second half of the week and during the first half of the year. Hands and wrists were the most common sites of injury (40.6%), whereas the eye, foot, ankles and other body parts had 30%, 14.6%, 10.6% and 4.2% of injuries respectively. The commonest type of injury was cuts and lacerations (50.1%); injuries to the eye (due to foreign bodies, chemicals and welding sparks) accounted for 30%, sprains 8% and burns 7.1% of the injuries. A cohort of 75 workers chosen from the 341 permanent workers were followed up for the one year for identification of risk factors. Significant risk factors were age (less than 30 yr) and experience (less than 2 yr). Technical factors responsible for injury were a hazardous worksite in 37 (38.5%) cases, inadequate protection with safety wear in 32 (33%) cases and proximity to machines in 14 (14.6%) cases. Human factors identified were non-use of protective wear in 43 (45%), overconfidence in 18 (18.7%) and timing error while working with machines in 11 (11.4%) episodes.

Validity of a surveillance system for childhood injuries in a rural block of tamilnadu

Abstract:Background: Childhood injuries are increasingly getting the attention of public health experts following WHO's report on global burden of diseases. Surveillance is an important component of control of any disease and effectiveness of the surveillance system depends upon completeness of the information about occurrence of the health related events to the public health authorities. Aims: This study aimed to set up a surveillance system for childhood injuries and validate it by a survey and thereafter estimate the incidence of childhood injuries using capture recapture method. Settings and design: Observational study design. Materials and methods: Passive surveillance system for childhood injuries was created for 26,811 children of less than fourteen years of Kaniyambadi block and it was validated by cross sectional study at the end of surveillance period. Using these two independent information systems, capture recapture method was applied to find out the possible incidence of injuries in the given population at a given period of time. Statistics: Chi square, Lincoln Peterson formula for capture re-capture method. Results: Surveillance and survey for childhood injuries identified 13.59/1000 child-years (CI: 11.86 - 15.32) and 341.89/1000 child-years (CI: 254.46-429.33) of injury rates, respectively. Conclusion: Passive surveillance system underreports childhood injuries markedly but it does identify childhood injuries of serious nature.

Abstract:Background: Hospitals are complex places with a large number of employees, patients, furniture, equipment, etc. Healthcare workers (HCWs), patients, or the general public are vulnerable to injuries and illness due to unseen hazards at the workplace. This study aims to identify the hazards and assess the risks at a hospital to ensure safety for HCWs, patients, and the public and generate awareness about the same. It helps in reducing the financial obligation of the institution due to the treatment of illnesses of staff, absenteeism, and service disruption and slows down manpower turnover. Hazard Identification and Risk Assessment (HIRA) helps reduce human errors and promote safe behavior. Objective: This study aims to identify and study the hazards in a hospital, assess the risks associated with the hazards, and recommend methods to reduce or eliminate the hazards based on the outcomes of the study. Methodology: An observational study was conducted at a 1000-bed tertiary-level teaching public sector hospital in eastern India. A checklist was used for direct observation, conducting staff interviews, and document reviews. A risk scoring tool was used, and hazards were ranked as per the risk score. Results: Thirty-eight hazards were identified in the study and classified under the categories of natural, physical, chemical, biological, ergonomic, psychological, and safety. The fire risk and occurrence of cyclones had the highest risk scores. Conclusions: The study identified hazards through direct observations, record reviews, and staff interviews. These findings can guide the prioritization of areas requiring necessary action in risk reduction, ensuring a safe workplace for healthcare workers (HCWs), patients, and the public. They can also help the institution shift from a reactive approach to a proactive method for HCW safety.
Population Pharmacokinetics and Exposure‐Response of a Fixed‐Dose Combination of Azilsartan Medoxomil and Chlorthalidone in Patients With Stage 2 Hypertension

Abstract:Population pharmacokinetic and exposure‐response models for azilsartan medoxomil (AZL‐M) and chlorthalidone (CLD) were developed using data from an 8‐week placebo‐controlled phase 3, factorial study of 20, 40, and 80 mg AZL‐M every day (QD) and 12.5 and 25 mg CLD QD in fixed‐dose combination (FDC) in subjects with moderate to severe essential hypertension. A 2‐compartment model with first‐order absorption and elimination was developed to describe pharmacokinetics. An Emax model for exposure‐response analysis evaluated AZL‐M/CLD effects on ambulatory systolic blood pressure (SBP). Estimated oral clearance and apparent volume of distribution (central compartment) were 1.47 L/h and 3.98 L for AZL, and 4.13 L/h and 62.1 L for CLD. Age as a covariate had the largest effect on AZL and CLD exposure (±20% change). Predicted maximal SBP responses (Emax) were –15.6 and –23.9 mm Hg for AZL and CLD. Subgroup analysis identified statistically significant Emax differences for black vs nonblack subjects, whereby the reduced AZL response in black subjects was offset by greater response to CLD. The estimated Emax for AZL and CLD was generally greater in subjects with higher baseline BP. In conclusion, no dose adjustments to AZL‐M or CLD are warranted based on identified covariates, and antihypertensive efficacy of AZL‐M/CLD combination therapy is comparable in black and nonblack subjects.
Injury prevalence and causality in developing nations: Results from a countrywide population-based survey in Nepal

Abstract:Background: Traumatic injury affects nearly 5.8 million people annually and causes 10% of the world's deaths. In this study we aimed to estimate injury prevalence, to describe risk-factors and mechanisms of injury, and to estimate the number of injury-related deaths in Nepal, a low-income South Asian country. Methods: A cluster randomized, cross-sectional nationwide survey using the Surgeons OverSeas Assessment of Surgical Need tool was conducted in Nepal in 2014. Questions were structured anatomically and designed around a representative spectrum of operative conditions. Two-stage cluster sampling was performed: 15 of 75 districts were chosen randomly proportional to population; within each district, after stratification for urban and rural populations, 3 clusters were randomly chosen. Injury-related results were analyzed. Results: A total of 1,350 households and 2,695 individuals were surveyed verbally, with a response rate of 97%. A total of 379 injuries were reported in 354 individuals (13.1%, 95% confidence interval 11.9-14.5%), mean age of 32.6. The most common mechanism of injury was falls (37.5%), road traffic injuries (19.8%), and burns (14.2%). The most commonly affected anatomic site was the upper extremity (42.0%). Of the deaths reported in the previous year, 16.3% were injury-related; 10% of total deaths may have been averted with access to operative care. Conclusion: This study provides baseline data on the epidemiology of traumatic injuries in Nepal and is the first household-based countrywide assessment of injuries in Nepal. These data provide valuable information to help advise policymakers and government officials for allocation of resources toward trauma care.
Global, regional and national burdens of occupational injuries, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019

Abstract:Objectives Occupational injuries pose a significant challenge to global health and development. This study aimed to quantify the international and regional burdens of occupational injuries from 1990 to 2019, stratified by specific causes. Methods We analysed global trends in deaths, disability-adjusted life years (DALYs), age-standardised mortality rates (ASMR) and age-standardised DALY rates (ASDR) related to occupational injuries. Specific injuries, including animal contact, drowning, mechanical forces, falls, fire, heat, hot substances, foreign bodies, poisoning and road injuries, were evaluated. Age-stratified and regional analyses were also performed. Results Globally, the number of deaths, DALYs, ASMR and ASDR related to occupational injuries declined from 1990 to 2019. The middle Socio-demographic Index (SDI) region exhibited the highest burden, whereas the high SDI region showed the least burden. China and India had the highest occupational injury-related death rates in 2019. Males, particularly those aged 25–44 years, were found to be more vulnerable. Road injuries were the leading cause of death in all age groups. Compared with 1990, mortality numbers and rates decreased significantly by 2019, with the highest burdens experienced in East Asia, South Asia and Southeast Asia. Conclusion The global decline in occupational injuries is promising; however, certain regions and demographics remain disproportionately affected. Targeted interventions in high-burden areas are crucial to further reduce the impact of occupational injuries.

Abstract:Background Child injuries significantly impact health and development, shaping the nation’s future. With proper knowledge, parents or primary caregivers can effectively prevent, address, and treat these predictable injuries. Methods Face-to-face interviews were conducted with 373 parents or primary caregivers of children under 14 years old in four randomly selected urban slum areas in Malwani, Mumbai Suburban District. Results Only 30% of respondents had basic first aid knowledge, and merely 10% were aware of unintentional injuries, correlating significantly with parental literacy levels. However, most respondents demonstrated appropriate responses. Conclusions Parental or primary caregiver knowledge and response play a critical role in mitigating unintentional child injuries. Increasing awareness among parents and primary caregivers about injuries, their impact on child development and first aid is imperative.

Abstract:Road traffic injury (RTI) is one of the most common causes of accidental deaths in India. The study investigates the changes in trends in age-standardised RTI mortality rates in India by sex and age groups, using data from the Global Burden of Disease (GBD) Study 2021. The trend segments are estimated from 1990 to 2021 employing a joinpoint regression model. Additionally, the influence of age, time period, and birth cohort on mortality rate trends was assessed using the age-period-cohort model. Over the past 32 years, the RTI mortality rates have experienced multiple trend segments. RTI mortality rate in the ≤ 14-year-old population has declined remarkably, dropping from 5.71 (4.65 to 6.88) per lakh population in 1990 to 3.66 (3.01 to 4.35) per lakh population in 2010, and further declining to 1.98 (1.65 to 2.37) per lakh population in 2021. The study found a positive correlation between RTI mortality rates and age, with rates consistently lower for women compared to men across all age groups. The variation in RTI mortality rates across Indian states has widened over time, with the coefficient of variation increasing from 30.58% in 1990 to 32.36% in 2010, and further to 35.11% in 2021. Despite efforts, Indian states are unlikely to achieve the goal of halving RTI deaths by 2030, based on 2010 levels. To address this, road conditions and road safety policies aimed at preventing the incidence of RTIs should be further intensified.

Injury Epidemiology in a Safe Community Health Service Center in Shanghai, China

Abstract:The current study has investigated the injury epidemiology in a community health service center (CHSC) under a Safe Community in Shanghai, China. It was a cross sectional study with data generated from hospital records and Injury Report Card (IRC). Open wounds constituted 571 (50.8%) injuries. Majority of the injuries (99.64%) did not need any hospitalization. Among the injured victims, 59.16% were floating population and occupied in the manufacturing or transportation sector (31.49% of the injury), commercial services and farms. Finger, toes, head and face were most affected part of the body due to injuries. Mechanical objects and falls constituted nearly 95% causes of injuries. During start of working hours (9am) and during Wednesday and Thursday the frequencies of injuries were highest. In a WHO Safe Community program, injury epidemiology has great emphasize as it dwelled with proper scientific evidences of the injury etiologies. The study had identified some important issues within its objected framework. Education and supervision of the floating workers can be effective for reducing injuries.

Abstract:CONTEXT: Trauma is an immediate cause of patients flowing to the emergency department of any hospital. Besides epidemiology, clinical profile and treatment strategy forms an important aspect to reflect the gap in the existing public sector health-care system and the requirement.AIMS: To evaluate the clinicoepidemiological profile of trauma patients admitting to an apex trauma hospital in east India.SETTINGS AND DESIGN: A prospective observational study was performed during the time period of December 2018 to July 2019 on trauma patients admitted to the Trauma and Emergency department.METHODS AND MATERIAL: Patient's demographic profile, injury type, mechanism, the vehicle involved in the accident, and transportation were recorded. Various trauma scores (clinical) and outcome measures were recorded.STATISTICAL ANALYSIS USED: Statistical analysis was done by R version 3.6.1.RESULTS: Male: female ratio was 407:93 with the 21-30 age group predominantly. 2-6 PM was the most common time of injury and ambulance was the predominant mode of transport (58%). Road traffic injury (RTI) accounted for 75% victims; two-wheelers (68%) dominated over others. Thirty percent (drivers 18%, pillion riders 12%) were wearing helmet; 41% were wearing seat belts (drivers 34%, passenger 12%). Twenty-five percent of drivers consumed alcohol. The median ± Interquartile range of injury severity score (ISS), revised trauma and trauma score and injury severity score were 17 (11-26), 7.8 (4.1-7.8), and 98.41 (95.95-99.30), respectively. Extremity injury (54% fractures) and head injury (50%) were the frontrunners in the pattern of injury, with half of the victims were polytrauma (ISS > 15).CONCLUSIONS: The injury was prominently RTI and the trauma victims/patients were young male drivers on two wheelers. The focus should be directed to make use of safety measures among the youth.
Occupational accidental injury deaths in Tokyo and Chiba prefectures, Japan: A 10-year study (2011-2020) of forensic institute evaluations

Abstract:Occupational accidental injury deaths (OAIDs) are a major social problem, and the analysis of individual cases is important for developing injury prevention measures. In this study, OAIDs with autopsies performed at forensic facilities in the metropolitan area of Japan (Tokyo and Chiba prefectures) from 2011 to 2020 were reviewed. The epidemiological characteristics of these OAIDs (n = 136), which accounted for 13.5% of OAIDs reported in the region during the study period, were compared with those of non-occupational accidental injury deaths (non-OAID) cases (n = 3926). Among OAID cases, 134 (98.5%) were men and 13 (9.6%) were foreign-born workers, which was significantly more than in non-OAID cases (p < 0.001, respectively). OAIDs were most frequent in construction (39.0%) followed by the manufacturing category (21.3%). The percentage of OAIDs in workers aged 65 and over showed an increasing trend. Most accidents occurred just after the start of work or just before the workday ended, as well as during the peak months of the year. The most common type of accident was fall/crash from a height (25.0%), and the most common injury site was the chest; none of these cases were confirmed to have been wearing a safety belt properly. Among foreign-born workers, the most common type of accident was caught in/between. As the working population is expected to change in the future, and an increase in the number of older adults and foreign workers is expected, it is necessary to take preventive measures such as improving the work environment based on ergonomics and providing safety education.
