Abstract:Advanced implants with simultaneous accelerated osteogenic and angiogenic capacities are of great importance for osteointegration. Much attention has been paid to simultaneously enhancing the osteogenesis and angiogenesis by surface decoration of bioactive molecules or ions on biomaterial surface, but the inherent physical cue of material surface down to the atomic-scale features have always been ignored. In this study, we demonstrate that regulation of surface oxygen vacancies defects of rutile nanorods are able to simultaneous accelerate the osteogenesis and angiogenesis. The concentration of surface oxygen vacancies defects of rutile nanorods can be manipulated by simple redox processing. The osteogenic differentiation of mesenchymal stem cells (MSCs), angiogenic differentiation and vessel-like tube structures of human umbilical vein endothelial cells (HUVECs) on oxygen vacancies rich surface are significantly up-regulated. This work therefore emphasizes the critical role of the inherent material atomic-scale features and provides a novel strategy to accelerate the osteogenesis and angiogenesis of Ti-based implant.
Evaluation of Sustained Drug Release Performance and Osteoinduction of Magnetron-Sputtered Tantalum-Coated Titanium Dioxide Nanotubes.

Abstract:Modifying the drug-release capacity of titanium implants is essential for maintaining their long-term functioning. Titanium dioxide nanotube (TNT) arrays, owing to their drug release capacity, are commonly used in the biomaterial sphere. Their unique half open structure and arrangement in rows increase the drug release capacity. However, their rapid drug release ability not only reduces drug efficiency but also produces excessive local and systemic deposition of antibiotics. In this study, we designed a tantalum-coated TNT system for drug-release optimization. A decreased nanotube size caused by the tantalum nanocoating was observed through SEM and analyzed (TNT: 110 nm, TNT-Ta1: 80 nm, TNT-Ta3: 40 nm, TNT-Ta5: 20 nm, TNT-Ta7: <5 nm). XPS analysis revealed the distribution of the chemical components, especially that of the tantalum element. In vitro experiments showed that the tantalum nanocoating enhanced cell proliferation; in particular, TNT-Ta5 possessed the best cell viability (about 1.18 of TNT groups at 7d). It also showed that the tantalum nanocoating had a positive effect on osteogenesis (especially TNT-Ta5 and TNT-Ta7). Additionally, hydrophilic/hydrophobic drug (vancomycin/raloxifene) release results indicated that the TNT-Ta5 group possessed the most desirable sustained release capacity. Moreover, in this drug release system, the hydrophobic drug showed more sustained release capacity than the hydrophilic drug (vancomycin: sustained release for more than 48 h, raloxifene: sustained release for more than 168 h). More importantly, TNT-Ta5 is proved to be an appropriate drug release system, which possesses cytocompatibility, osteogenic capacity, and sustained drug release capacity.

Abstract:The bio-inertness of titanium (Ti) and the accompanying severe inflammation can extremely inhibit the coupled osteogenesis-angiogenesis regulation and result in the undesirable osseointegration of peri-implant during the implantation process. Herein, three types of TiO 2 nanotubes (TNT) were prepared on native Ti substrates using the anodic oxidation approach, including TNT with a diameter of 30 nm (TNT-30), 70 nm (TNT-70), and 110 nm (TNT-110). Among the four substrates, TNT-70 was the most prominent to induce the differentiation of macrophages to the pro-healing M2 phenotype via significant down-regulation of pro-inflammation genes (CD86, iNOS, CD11c) and cytokines (TNF-α, IL-6), as well as up-regulation of anti-inflammation genes (CD206, Arg-1, IL-10) and cytokines (TGF-β, IL-10). Furthermore, TNT-70 could stimulate the osteogenic potential of mesenchymal stem cells (MSCs) and the angiogenic ability of human umbilical vein endothelial cells (HUVECs) through multiple paracrine signaling. In vivo experiments suggested that TNT-70 significantly relieved early inflammatory response around the implant and facilitated coupled osteogenesis and angiogenesis, and effectively promoted the osseointegration of peri-implant. This study provided a reference for fabricating surface topography on Ti implants for improving osseointegration and relieving the associated excessive inflammatory responses.

Abstract:Titanium has been widely employed in the fields of orthopaedics and dentistry, attributed to its superior mechanical and biological properties. The mechanical stimulation induced by the titanium dioxide (TiO2) nanotubes (TNTs) morphology resulting from surface modification has been demonstrated to enhance the osteogenic differentiation of bone marrow-derived mesenchymal stem cells (BMSCs). Kindlin-2, a pivotal focal adhesion protein, is involved in mechanical signaling processes through the regulation of stress fibril filament assembly. Additional research is needed to clarify the involvement of Kindlin-2 in the mechanism of TNTs-induced osteogenic differentiation. This study systematically investigated the impact of Kindlin-2 on TNTs-induced osteogenesis and mechanotransduction. TiO2 nanotubes with diameters of approximately 30 nm (TNT-30) and 100 nm (TNT-100) were fabricated and characterized using anodic oxidation. The results showed that TNT-100 significantly increased the expression of Kindlin-2 and enhanced osteogenic differentiation compared to polished titanium (PT) and TNT-30. Additionally, Kindlin-2 promotes cytoskeleton assembly by regulating the integrin β1/FAK/RhoA signaling pathway, impacting osteogenic gene expression and BMSC differentiation in a Yes-Associated Protein (YAP)-dependent manner. Therefore, these findings contribute to a more comprehensive understanding of the fate of BMSCs on TNTs morphologies and provide a novel theoretical foundation for the development of advanced bone repair biomaterials.

Abstract:The immunomodulatory role of monocytes is essential for tissue healing and can influence the osseointegration of implanted materials. Properties such as the surface structure, hydrophilicity, and roughness of the implanted materials can modulate monocyte–macrophage function. In this study, we characterized material-hydrogenated TiO2 nanotubes (H-TNT) with superhydrophilic surfaces to investigate the effect of H-TNT on macrophage polarization and osseointegration. H-TNT were prepared by anodic oxidation and hydrogenation and used in the experimental group, while TNT and smooth pure Ti were employed in the control groups. RAW264.7 cells were selected to observe the immunomodulatory effect of these samples. The cell morphology was observed via scanning electron microscopy, and cytokine expression was detected using an enzyme-linked immunosorbent assay and immunofluorescence staining. After 24 hr of cultivation, the macrophage-conditioned medium was collected and used for indirect coculture with MC3T3-E1 cells. The morphology of MC3T3-E1 cells was observed using fluorescence staining. Cell adhesion and proliferation were measured using the Cell Counting Kit-8 assay. Alkaline phosphatase activity measurement, alizarin red staining, calcium quantification, and reverse transcription polymerase chain reaction were performed to assess the osteogenic differentiation of MC3T3-E1 cells. The results showed that H-TNT promoted the M2-type polarization of macrophages, which in turn influenced the adhesion, proliferation, and osteogenic differentiation of MC3T3-E1 cells. These materials can serve as useful candidates for bone implants because they activate macrophages to produce a favorable osteoimmunomodulatory microenvironment.

Abstract:Titania nanotubes (TNT) generated on titanium implant are emerged as important modification technique to facilitate bone regeneration. Mesenchymal stem cells (MSCs)-derived exosomes are membrane bound extracellular vesicles (EVs), which play an important role in tissue regeneration. The objective of this study was to generate an EVs hybrid TNT aiming at regulating inflammation, MSCs recruitment and osteogenesis. We isolated EVs from MSCs (MSCs EVs) and 3-day osteogenically differentiated MSCs (3d EVs). MSC EVs and 3d EVs exhibited round morphology under TEM, which also showed robust internalization by human bone marrow derived MSCs (hBMSCs). Next, we fabricated 3d EVs/MSC EVs hybrid TNT. When inflammatory macrophages were co-cultured with EVs hybrid TNT, the gene and protein expression of inflammatory cytokine were significantly reduced. Macrophage morphology was also examined by confocal laser scanning microscopy (CLSM) and scanning electron microscopy (SEM). Further migratory ability study using hBMSCs indicated significant enhancement of MSCs migration in EVs hybrid TNT. In addition, we further demonstrated significant increase of osteogenic differentiation of hBMSCs in EVs hybrid TNT. This study suggests that EVs hybrid TNT may serve as a viable therapeutic approach to enhance osteogenesis and bone regeneration.

Purpose: This study aimed to evaluate the osteogenic potential of 110 nm TNTs (TNT110) compared to 30 nm TNTs (TNT30) in an aging rat model exhibiting OS, and to investigate the mechanisms involved. Methods: Surface properties of TNTs were characterized, and in vitro and in vivo experiments were conducted to assess their osteoinductive effects under OS. Transcriptomic, proteomic analyses, and Western blotting were performed to investigate the protective mechanisms of TNT110 on osteoblasts. Protein adsorption studies focused on the roles of fibronectin (FN) and albumin (BSA) in modulating osteoblast behavior on TNT110. Results: In both in vitro and in vivo experiments, TNT110 significantly improved new bone formation and supported osteoblast survival under OS conditions. Subsequent ribonucleic acid sequencing results indicated that TNT110 tended to attenuate inflammatory responses and reactive oxygen species (ROS) expression while promoting endoplasmic reticulum (ER) stress and extracellular matrix receptor interactions, all of which are crucial for osteoblast survival and functionality. Further confirmation indicated that the cellular behavior changes of osteoblasts in the TNT110 group could only occur in the presence of serum. Moreover, proteomic analysis under OS conditions revealed the pivotal roles of FN and BSA in augmenting TNT110's resistance to OS. Surface pretreatment of TNT110 with FN/BSA alone could beneficially influence the early adhesion, spreading, ER activity, and ROS expression of osteoblasts, a trend not observed with TNT30. Conclusion: TNT110 effectively protects osteoblast function in the OS microenvironment by modulating protein adsorption, with FN and BSA synergistically enhancing osteogenesis. These findings suggest TNT110's potential for use in implants for elderly patients. Currently, Ti-based implants display effective osseointegration and high success rates. However, compared to the middle-aged and young population, the time required for early osseointegration in elderly implant patients is significantly prolonged. 2 Increasing evidence suggests a close link between the production of reactive oxygen species (ROS) and aging. 3 In the context of implantation, successful osseointegration is crucial for maintaining enduring implant functionality and ensuring the overall success of oral implant procedures. 4 Yet, in elderly patients, the bone microenvironment undergoes abnormal changes, notably marked by increased oxidative stress (OS). This rise in OS is primarily due to elevated levels of ROS, which inflict direct damage on osteoblasts, leading to their deterioration, aging, and apoptosis. 5–7 This, in turn, impedes osteoblast differentiation while simultaneously promoting the proliferation of osteoclasts. Abstract:Imagine a Samaritan living kidney donor, who some time ago has anonymously donated one of his kidneys to a patient on top of the waiting list. He now contacts the transplantation centre once again, to donate part of his liver. The Centre, startled by this idea, refers him to the regular screening procedure for all Samaritan donations. It turns out that his wish is well‐informed, voluntarily made and that he is competent to decide. We acknowledge that a donor’s wish should not be followed in all cases, even though this wish is a clear expression of his own free will. However, a refusal must be based on sound moral reasons and it is less clear what reasons these might be. We outline the most common arguments for refusal, assess these arguments in terms of strengths and weaknesses, and show which arguments, if any at all, are most promising. We conclude, firstly, that we should only assess risks (which include motivations), not judge relationships, and secondly, that it is not a transplant centre’s mission to carry out a donor’s life project.
Ag2O‐TiO2‐NTs enhance osteogenic activity in vitro by modulating TNF‐α/β‐catenin signaling in bone marrow‐derived mesenchymal stem cells

Abstract:BMSCs were isolated from the femurs of SD rats aged 7–8 weeks and identified by flow cytometry. BMSCs were inoculated into Ag2O‐TiO2‐NTs and added into stem cell osteogenic induction differentiation culture medium. Extracellular LDH enzyme activity was detected at 7 and 14 days of induction, respectively, and osteogenic genes in BMSCs were detected by qRT‐PCR. The cell mineralization ability and ALP activity were observed by alizarin red staining and ALP staining. The protein levels of TNF‐α and β‐catenin in BMSCs of each group were observed by western blot. Our results suggest that Ag2O‐TiO2‐NTs coatings enhance the osteogenic activity of BMSCs by regulating TNF‐α/β‐catenin signaling. The toxic effects of nanoparticles‐silver oxide (Ag2O) limited its use. However, loading Ag2O nanoparticles into titanium dioxide (TiO2) nanotubes (Ag2O‐TiO2‐NTs) has more efficient biological activity and safety. The aim of this study was to observe the effect of Ag2O‐TiO2‐NTs on osteogenic differentiation of bone marrow‐derived mesenchymal stem cells (BMSCs) and its mechanism. The enzyme activity of lactate dehydrogenase (LDH) and the expression of RUNX family transcription factor 2 (Runx2), OPN, OCN in BMSCs were detected by quantitative real time polymerase chain reaction. At 14 days of induction, the mineralization ability and alkaline phosphatase (ALP) activity of cells in each group were observed by Alizarin Red S staining and ALP staining. In addition, the protein levels of tumor necrosis factor‐α (TNF‐α) and β‐catenin in BMSCs of each group were observed by western blot. After 14 days of the induction, the mineralization ability and ALP activity of BMSCs in the Ag2O‐TiO2‐NTs group were significantly enhanced compared with those in the Ag2O and TiO2 groups. Western blot analysis showed that the BMSCs in the Ag2O‐TiO2‐NTs group exhibited much lower protein level of TNF‐α and higher protein level of β‐catenin than those in the Ag2O and TiO2 groups.Ag2O‐TiO2‐NTs enhance the osteogenic activity of BMSCs by modulating TNF‐α/β‐catenin signaling.

Abstract:With the introduction of a new interdisciplinary field, osteoimmunology, today, it is well acknowledged that biomaterial-induced inflammation is modulated by immune cells, primarily macrophages, and can be controlled by nanotopographical cues. Recent studies have investigated the effect of surface properties in modulating the immune reaction, and literature data indicate that various surface cues can dictate both the immune response and bone tissue repair. In this context, the purpose of the present study was to investigate the effects of titanium dioxide nanotube (TNT) interspacing on the response of the macrophage-like cell line RAW 264.7. The cells were maintained in contact with the surfaces of flat titanium (Ti) and anodic TNTs with an intertube spacing of 20 nm (TNT20) and 80 nm (TNT80), under standard or pro-inflammatory conditions. The results revealed that nanotube interspacing can influence macrophage response in terms of cell survival and proliferation, cellular morphology and polarization, cytokine/chemokine expression, and foreign body reaction. While the nanostructured topography did not tune the macrophages' differentiation into osteoclasts, this behavior was significantly reduced as compared to flat Ti surface. Overall, this study provides a new insight into how nanotubes' morphological features, particularly intertube spacing, could affect macrophage behavior.

Abstract:Controlling macrophage response to biomaterials is critical for the reduction of inflammation after implantation. Here we designed a sustained release system from TiO2 nanotubes (TNTs) to improve osteogenesis on titanium implants with anti-inflammatory properties. TNTs (around 70 nm diameter) were first fabricated on titanium surfaces by anodization, directly filled with the anti-inflammatory drug, dexamethasone (DEX) and then covered by chitosan (CHI) multilayer films. Primary osteoblast and macrophage (RAW 264.7) cells were cultured on untreated and treated titanium surfaces in vitro. Osteoblasts grown on CHI-coated Dex-filled TNTs surfaces displayed higher alkaline phosphatase (ALP) and mineralization, which was consistent with qRT-PCR analysis of osteoblastic genes including collagen type I (Col I), osteocalcin (OCN), osteopontin (OPN) and runt related transcription factor 2 (Runx2). In contrast, protein levels of nitric oxide (NO) and proinflammatory cytokines (TNF-α and IL-1β) from macrophages on Dex-filled TNTs, CHI-coated TNTs and CHI-coated Dex-filled TNTs were significantly lower, especially on CHI-coated Dex-filled TNTs surfaces compared to levels on titanium and TNTs. These results indicate that CHI-coated Dex-filled TNTs enhanced osteoblast differentiation and decreased the inflammatory response of macrophages. The approach presented here provides new insight into the modification of TNTs for the development of titanium-based implants.

Abstract:Titanium-based orthopedic implants are gaining popularity in recent years due to their excellent biocompatibility, superior corrosion resistance and lightweight properties. However, these implants often fail to perform effectively due to poor osseointegration. Nanosurface modification approaches may help to resolve this problem. In this work, TiO2 nanotube (NT) arrays were fabricated on commercially available pure titanium (Ti) surface by anodization and annealing. Then, zinc (Zn) and strontium (Sr), important for cell signaling, were doped on the NT surface by hydrothermal treatment. This very simple method of Zn and Sr doping takes less time and energy compared to other complicated techniques. Different surface characterization tools such as scanning electron microscopy (SEM), X-ray photoelectron spectroscopy (XPS), energy-dispersive X-ray spectroscopy (EDS), static water contact angle, X-ray diffraction (XRD) and nanoindentation techniques were used to evaluate the modified surfaces. Then, adipose derived stem cells (ADSCs) were cultured with the surfaces to evaluate cell adhesion, proliferation, and growth on the surfaces. After that, the cells were differentiated towards osteogenic lineage to evaluate alkaline phosphatase (ALP) activity, osteocalcin expression, and calcium phosphate mineralization. Results indicate that NT surfaces doped with Zn and Sr had significantly enhanced ADSC adhesion, proliferation, growth, and osteogenic differentiation compared to unmodified surface, thus confirming the enhanced performance of these surfaces.

Abstract:Titanium (Ti) alloys, particularly Ti6 Al4 V, are the most commonly used biomedical implant material. Ti alloys are biologically inert, so there have been continuous efforts to improve their osteogenic properties and clinical performance. Since TiO2 nanotubes (NT) appear to be excellent drug platforms, and strontium reportedly enhances osteogenesis, we constructed a TiO2 nanotube coating on the surface of Ti6 Al4 V and immersed it in Sr (OH)2 solution in order to incorporate Sr into TiO2 nanotubes (NT-Sr). The results of field emission scanning electron microscope and X-ray diffraction analysis verified the fabrication of NT-Sr. We next added polydopamine (PDA) and cyclo- (arginine-glycine-aspartic acid-phenylalanine-cysteine) [c(RGDfC)] peptides to further promote biocompatibility of the implant. Fourier transform infrared spectroscopy and X-ray photoelectron spectroscopy confirmed the existence of PDA and c(RGDfC). Mesenchymal stem cells (MSCs) were planted on Ti, NT, NT-Sr, NT-Sr/PDA, and NT-Sr/PDA-RGD surfaces. The adhesion and differentiation of MSCs on different surfaces were evaluated. The mRNA expression of alkaline phosphatase, runt-related transcription factor 2 (Runx2) and type I collagen (Col I) of different groups were also tested. Finally, we observed that the NT-Sr/PDA-RGD group showed significantly better performance than other groups in terms of the differentiation and osteogenesis-related gene expression of MSCs. Thus, the NT-Sr/PDA-RGD complex may be an important modification strategy for Ti, as it shows excellent osteogenic potential.

Abstract:Titanium (Ti) and its alloys have been widely used as percutaneous and subcutaneous implants due to their excellent mechanical properties and biocompatibility. However, the aggregation of reactive oxygen species (ROS) and persistent inflammatory responses at the implant site severely affect the soft tissue integration of Ti implants, causing a series of biological complications. To address this issue, in this study, Fe-nitrogen-doped carbon single-atom nanozymes (Fe-NC nanozymes) loaded titanium oxide nanotube arrays (Fe-NC@TNT) were constructed by anodic oxidation and solvothermal method on medical titanium surfaces. The surface morphology, physical composition, enzyme-like catalytic activity, inflammatory response, and soft tissue compatibility of Fe-NC@TNT were investigated. The unique nanotube array fully exposes the catalytic active sites of Fe-NC nanozymes and significantly enhances their enzyme-like catalytic performance to eliminate superoxide anion, hydrogen peroxide, and more toxic hydroxyl radicals, which could effectively reduce the intracellular ROS levels of macrophages and fibroblasts, thereby inhibiting the inflammatory responses of macrophages and promoting the functional expression of fibroblasts. In vivo animal experiments have further demonstrated that Fe-NC@TNT can effectively regulate the immune response and promote the integration between the implant and the surrounding soft tissues.
Magnesium-doped Nanostructured Titanium Surface Modulates Macrophage-mediated Inflammatory Response for Ameliorative Osseointegration

Abstract:BACKGROUND: Next generation of coating materials on the surface of implants is designed with a paradigm shift from an inert material to an osteoimmunomodulatory material. Regulating immune response to biomedical implants through influencing the polarization of macrophage has been proven to be an effective strategy.METHODS: Through anodization and hydrothermal treatment, magnesium ion incorporated TiO2 nanotube array (MgN) coating was fabricated on the surface of titanium and it is hypothesized that it has osteoimmunomodulatory properties. To verify this assumption, systematic studies were carried out by in vitro and in vivo experiments.RESULTS: Mg ion release behavior results showed that MgN coating was successfully fabricated on the surface of titanium using anodization and hydrothermal technology. Scanning electron microscopy (SEM) images showed the morphology of the MgN coating on the titanium. The expression of inflammation-related genes (IL-6, IL-1β, TNF-α) was downregulated in MgN group compared with TiO2 nanotube (NT) and blank Ti groups, but anti-inflammatory genes (IL-10 and IL-1ra) were remarkably upregulated in the MgN group. The in vitro and in vivo results demonstrated that MgN coating influenced macrophage polarization toward the M2 phenotype compared with NT and blank-Ti groups, which enhanced osteogenic differentiation of rat bone mesenchymal stem cells rBMSCs in conditioned media (CM) generated by macrophages.CONCLUSION: MgN coating on the titanium endowed the surface with immune-regulatory features and exerted an advantageous effect on osteogenesis, thereby providing excellent strategies for the surface modification of biomedical implants.

Abstract:In this study, titania nanotubes (TNTs) incorporating silicon (Si) were formed on Ti disks using anodization and electron beam evaporation (EBE) technology to improve the osteogenic activity. The amount of Si was exquisitely adjusted by controlling the duration of EBE to optimize the biofunctionality. As the Si was incorporated, the samples exhibited hydrophilic surfaces. Long lasting and controllable Si release was observed from the EBE-modified samples without cytotoxicity. Moreover, initial cell adhesion, spreading, proliferation and osteogenic differentiation of MC3T3-E1 cells were evaluated. The results showed a notable enhancement of spreading, osteogenesis and differentiation of cells on silicon-coated TNTs (Si-TNTs). In particular, samples with highest amount of silicon (∼5.93% Si) displayed greatest augmentation of ALP activity, osteogenic-related gene expression and mineralization compared to the others in the present study. It was indicated that the modification with TNTs and appropriated Si content resulted in enhanced osteoblastic spreading, proliferation and differentiation, and therefore has the potential for future applications in the field of orthopedics.
Nanoporous titanium implant surface promotes osteogenesis by suppressing osteoclastogenesis via integrin β1/FAKpY397/MAPK pathway

Abstract:Macrophages and osteoclasts are both derived from monocyte/macrophage lineage, which plays as the osteoclastic part of bone metabolism. Although they are regulated by bone implant surface nanoarchitecture and involved in osseointegration, the beneath mechanism has not been simultaneously analyzed in a given surface model and their communication with osteoblasts is also blurring. Here, the effect of implant surface topography on monocyte/macrophage lineage osteoclastogenesis and the subsequent effect on osteogenesis are systematically investigated. The nanoporous surface is fabricated on titanium implant by etching and anodizing to get the nanotubes structure. The early bone formation around implant is significantly accelerated by the nanoporous surface in vivo. Meanwhile, the macrophage recruitment and osteoclast formation are increased and decreased respectively. Mechanistically, the integrin mediated FAK phosphorylation and its downstream MAPK pathway (p-p38) are significantly downregulated by the nanoporous surface, which account for the inhibition of osteoclastogenesis. In addition, the nanoporous surface can alleviate the inhibition of osteoclasts on osteogenesis by changing the secretion of clastokines, and accelerate bone regeneration by macrophage cytokine profiles. In conclusion, these data indicate that physical topography of implant surface is a critical factor modulating monocyte/macrophage lineage commitment, which provides theoretical guidance and mechanism basis for promoting osseointegration by coupling the osteogenesis and osteoclastogenesis.
