Abstract:Purpose: Periprosthetic infection after total knee arthroplasty (TKA) is a devastating complication, with a two-stage revision currently the 'gold standard' treatment for chronic infections. There is, however, a lack of information about mechanical complications during this treatment. The purpose of this study was to determine: (1) the rate and type of mechanical complications encountered during a two-stage exchange revision for periprosthetic infection of the knee and (2) possible factors of influence. Methods: Between 2000 and 2011, 133 patients received an antibiotic-laden cement spacer as part of a two-stage protocol. The overall frequency and types of complication were recorded (fissure/fracture of the tibia or femur, spacer fracture, subluxation of the patella, peroneus affection, wound healing disorder and mobilization under anaesthesia based on a constricted ROM). Also analysed were potential influencing factors (BMI, ASA classification, length of the interval with the enclosed spacer, revision needed after explantation, revision needed after reimplantation, complications after primary TKA, service life of the primary prosthesis) in terms of the overall outcome (possibility of reimplantation, complications during the two-stage protocol). Results: The mean age at the time of the first stage operation was 70.1 ± 9.9 years. Overall, 20 of 133 patients suffered one of the complications mentioned above (15 %). Fracture/fissure of the tibia occurred in nine cases (6.8 %) and fracture/fissure of the femur in three (2.3 %). There were also three mobilizations under anaesthesia after TKA reimplantation, two affections of the peroneus nerve, one spacer fracture, one subluxation of the patella and one wound healing disorder. The influencing factors on the overall outcome were revision after reimplantation (reinfection, p = 0.002), revision after explantation (reinfection, p = 0.044), prior aseptic revision after primary TKA (reimplantation, p = 0.019), and prior two-stage revision (reimplantation, p = 0.002). Conclusion: A two-stage revision arthroplasty using a static cement spacer is an effective therapy for infected TKAs. The complication rate of 15 % (including restricted ROM after reimplantation) is acceptable. Influencing factors (revision needed after reimplantation, revision needed after explantation) can be demonstrated and should be avoided during the two-stage protocol.

Abstract:Aims: Periprosthetic joint infection (PJI) is a challenging complication of any arthroplasty procedure. We reviewed our use of static antibiotic-loaded cement spacers (ABLCSs) for staged management of PJI where segmental bone loss, ligamentous instability, or soft-tissue defects necessitate a static construct. We reviewed factors contributing to their failure and techniques to avoid these complications when using ABLCSs in this context. Methods: A retrospective analysis was conducted of 94 patients undergoing first-stage revision of an infected knee prosthesis between September 2007 and January 2020 at a single institution. Radiographs and clinical records were used to assess and classify the incidence and causes of static spacer failure. Of the 94 cases, there were 19 primary total knee arthroplasties (TKAs), ten revision TKAs (varus-valgus constraint), 20 hinged TKAs, one arthrodesis (nail), one failed spacer (performed elsewhere), 21 distal femoral endoprosthetic arthroplasties, and 22 proximal tibial arthroplasties. Results: A total of 35/94 patients (37.2%) had spacer-related complications, of which 26/35 complications (74.3%) were because of mechanical failure of the spacer construct, while 9/35 (25.7%) were due to recurrence of infection. Risk factors for internal failure were a construct where the total intramedullary spacer length was less than twice the length of the central osseous defect (p = 0.009), where proximal or distal intraosseous spacer contact was < 10%, and after tibial tubercle osteotomy (p = 0.005). The incidence of spacer complications significantly increased the time to second stage: mean 157 days (42 to 458) in those without complications versus 227 days (11 to 528) with complications (p = 0.014). Conclusion: The failure rate of static antibiotic-loaded cement spacers is much higher than anticipated. Complications of the spacer significantly increased the time to second-stage revision. The risk of mechanical failure is significantly increased if the spacer is less than double the size of the segmental defect, or if inadequate reinforcement is inserted into the residual bone. These findings serve as a guide for surgeons to avoid mechanical complications with static spacers.
Use of Hinged Implants for Multi-Stage Revision Knee Arthroplasty for Severe Periprosthetic Joint Infection: Remission Rate and Outcomes After a Minimum Follow-Up of 5 Years

Abstract:Background: In severe periprosthetic joint infection (PJI) after total knee arthroplasty (TKA), multi-stage procedures are indicated for ongoing signs of infection after implant removal during the spacer interval of an intended two-stage exchange. In these cases, several additional debridement and spacer exchange surgeries may be necessary. Herein, we analyzed the complications, remission rate, and functional outcome after multi-stage revision arthroplasty using hinged TKAs. Methods: Patients (n = 79) treated with multi-stage revision arthroplasty after chronic PJI of the knee were included (2010 to 2018). During the prosthesis-free interval, a static spacer containing antibiotic-loaded bone cement was implanted. The mean number of surgeries, including implant removal and revision arthroplasty, was 3.8 (range, 3 to 8). The mean duration from implant removal to revision arthroplasty was 83 days (range, 49 to 318). Complications, remission, and mortality were analyzed after a minimum follow-up of 5 years. Outcomes were assessed based on the Knee Society Score (KSS) and the Western Ontario McMasters University Osteoarthritis Index (WOMAC). Results: During follow-up, 24 (30.4%) patients underwent revision surgery, with a mean time to surgical revision of 99 weeks (range, 1 to 261). After follow-up, the infection-free remission rate and overall mortality were 87.3 and 11.4%, respectively. The mean KSS was 74.3 (range, 24 to 99); the KSS Function Score was 60.8 (range, 5 to 100); and the WOMAC, 30.2 (range, 5 to 83). Conclusion: In difficult-to-treat cases, multi-stage revision arthroplasty showed high remission rates and low mortality after a follow-up of 5 years. The overall revision rate was comparably high, accounting for early and late reinfections most of the time. In cases of implant survival, functional outcomes comparable to those of revision hinge TKA reported in the literature can be achieved. Therefore, multi-stage procedures with additional debridement steps should be performed in cases of ongoing infections in intended two-stage procedures.

Abstract:Two-stage revision surgery for infected total knee replacements remains the gold standard treatment. Articulating spacers are preferred to static spacers for improved functional outcome. Articulating spacers made of cement can be prone to fracture, may not be suitable for full weight bearing, create abrasion debris and necessitate second-stage revision surgery. An alternative is the use of primary knee replacement implants as temporary spacers. With this technique, implants are loosely cemented into place at time of revision, allow the patient reasonable mobility and an ability to fully weight bear and can obviate the need for second-stage surgery. A retrospective review of all patients undergoing revision for infection over two years was conducted. Patients were clinically assigned to single- or two-stage revision. Patients who had a temporary knee replacement, that is, a primary knee replacement used as an articulating spacer, were identified and contacted to complete an Oxford Knee Score. Time to second stage and recurrence was identified from the notes 23 patients received temporary knee replacements. Of these, one patient died, 13 proceeded to a second-stage revision and nine remain in situ. Median time to second-stage revision was 19 weeks [range 11–27]. No patients had re-infection. Median follow-up for ongoing temporary knee replacements was 43 weeks [range 24–90]. Four temporary implants had survived for longer than 1 year. Median Oxford Knee Score was 26 [23–32] and satisfaction score was 8 out of 10 [8–8]. These early results show that knee replacement implants used as spacers provide a good alternative to cement-based articulating spacers with low re-infection rates. Their additional cost when compared with cement spacers is offset by the fact that many patients achieve adequate function and frail patients can avoid a revision procedure.Level of evidence Case series, Level IV.
Articulating spacer in the two-stage revision for severe infected knee arthroplasty

Abstract:Objective To investigate the efficiency and safety of articulating spacer for severe infected knee arthroplasty in patients with medical comorbidities and local sinus tracts. Methods Ten consecutive patients with medical comorbidities (rheumatoid arthritis, diabetes mellitus, etc) or local sinus tracts,who were complicated with late infected TKA, were included in the study. All the patients underwent twostage revision using articulating spacers. All of the patients were debridement thoroughly and followed by implantation of an antibiotic-loaded cement articulated spacer. Two-stage revisions were not followed untill the infection were controlled. The hospital for special surgery (HSS) knee scoring system and range of motion were used to evaluate the outcomes. Results One patient underwent knee fusion because the infection was not controlled after first-stage surgery. The other 9 patients had no evidence of infection. The mean follow-up was 50 months (range, 24-90 months), no recurrent infection developed for these 9 patients. The mean modified HSS score had improved from 48 points (range, 32-63) before the resection surgery to 79 points (range,62-91) at the end of the spacer period. At the latest follow-up, the modified HSS score averaged 89 points (range, 74-95). The good and excellent rate was 0, 80% and 100%, respectively. The average range of motion had increased from 13°-70° preoperatively to 8°-93° prior to the revision. And at the latest follow-up, the range of motion averaged 3° to 110°. Conclusion The delayed two-stage revision using an articulating spacer is effective in the treatment of chronically infected TKA characterized by simple, good reproducible, high rate of infection control, better joint function after surgeries
Two-stage knee arthrodesis with a modular intramedullary nail due to septic failure of revision total knee arthroplasty with extensor mechanism deficiency

Abstract:Background: Periprosthetic joint infection is a serious complication and reconstruction after failed revision total knee arthroplasty with significant bone loss and compromised soft-tissues can be challenging. Objective of this study was to assess clinical and functional results, implant survival and infection recurrence rates in patients treated with two-stage arthrodesis after failed revision TKA with extensor mechanism deficiencies due to PJI, and to identify the factors that affect outcomes after surgery. Methods: Thirty seven patients with PJI treated within a two-stage exchange and reimplantation of an arthrodesis nail between 2008 and 2014 were included. Systemic and local risk factors were graded preoperatively according to McPherson et al. All patients were treated according to a structured treatment algorithm. Clinical and functional evaluation was performed using the Oxford Knee Score and the Visual Analogue Scale. Results: Thirty two of 37 patients (86.5%) were graded as free of infection. Five patients (13.5%) had recurrent infection after arthrodesis with the need of revision surgery. Mean leg-length discrepancy was 2.2cm. The mean VAS for pain was three, the mean Oxford Knee Score was 38±9. Total implant survival at a 74month follow-up was 74.3% (95% CI: 45.4 to 91.1%), as determined by Kaplan-Meier survival curves. Local McPherson Score, as well as number of revisions was found to be of significant influence to the survival rate. Conclusions: Septic failure of revision knee arthroplasty can be effectively treated with two-stage arthrodesis using a modular intramedullary nail, providing a stable and painless limb with satisfactory functional results and acceptable infection eradication rates.

Abstract:Objective To present our experience for the management of the infected hip and knee arthroplasty with two-staged revision.Methods Between June 2008 and Aug 2010 the replacement of the infected arthroplasty in two stages was performed in 14 patients.The mean follow-up time after revision was 16.6 months.Results One of 14 patients had a recurrence of the infection.One case had femoral fracture.The revision increased the Harris Hip Score from 48.9 points preoperative to 82.9(44～95) points postoperative,and the postoperative AKS averaged 84.25 points and increased about 41.75 points more than preoperative.Conclusions Two-stage reconstruction of the infected hip and knee is useful and effective because of higher rate of eradication of the infection and good postoperative functional results in treating infected THA or TKA.

A static knee spacer has been used to deliver high doses of antibiotics locally, to minimize contractures of collateral ligaments and to preserve joint space for subsequent prosthesis re implantation.ConclusionReinforced static spacer is an option for massive bone loss during periprosthetic joint infection treatment in two-stage procedure. Patients with massive bone loss, lack of soft tissue or ligamentous integrity may be the ideal candidate for its use. Reinforced static spacer provides stable fixation across the knee, local antibiotic delivery, maintenance of the joint space, preservation of soft-tissue tension around the joint and length maintenance

The average range of motion had improved to 8° to 93° at the end of the spacer period, and 3° to 110° at the last follow-up, compared to 13° to 70° preoperatively.Delayed two-stage reimplantation using an articulating spacer is effective for treating infected TKA in patients with medical comorbidities or local sinuses.
The Use of Metal Augmentation for Uncontained Bone Defects in Revision Total Knee Arthroplasty

Abstract:Objective To explore the clinical results of the use of metal augmentation for uncon-tained bone defects in revision total knee arthroplasty. Methods A retrospective study was carried out on 62 patients with the use of metal augmentation and intramedullary stem for uncontained bone defects (AORI type Ⅱ) in 227 revision total knee arthroplasties from 1992 to 2001. There were 28 male and 34 female with a mean age of 67.8 years (range, 42 to 87 years). The diagnosis at the time of primary total knee arthroplasty was osteoarthritis in 51 (82.2%), rheumatoid arthritis in 8 (12.9%), lupus arthritis in 1 (1.6%) and posttrau-matic arthritis in 2 (3.2%). Revision was performed for aseptic loosening of femoral or tibial component in 30 (48.3%), reimplantation post- infection in 19 (30.6%), polyethylene wear with osteolysis in 3 (4.8%), malpo-sition or malalignment and instability in 10 (16.1%). Results Sixty-two patients were reviewed at a mean of 60.8 months (range, 13 to 132 months). Clinical evaluation revealed that the mean knee score of the knee society rating system had improved from 25 points preoperatively to 76 points postoperatively, the mean functional score from 40 points to 62 points at the latest follow-up and the mean range of motion ( had in-creased from a flexion arc of 78° to 87°. Radiolucent lines at the cement bone interface beneath the posteri-or femoral augmentation were present in 11 knees, beneath the medial tibial augmentation in 4 knees. None of radiolucent lines were progressive. All knees were asymptomatic. Two cases were failed due to recurrence of infection. One patient underwent for arthrodesis because of high risk for reinfection, another patient pre-ferred for resection arthroplasty and fused reimplantation. Post-operative manipulation under anesthesia for poor motion was carried out in 5 knees. Conclusion Metal augmentation for uncontained bone defects combined with intramedullary stem is feasible to accomplish, it provides immediate secure fixation to host bone, and improves success rate of revision knee arthroplasty, especially in elder patients.
Functional Outcome after Reimplantation in Patients Treated with and without an Antibiotic-Loaded Cement Spacers for Hip Prosthetic Joint Infections

Abstract:Purpose: A staged revision with placement of a temporary antibiotic-loaded cement spacer after removal of the implant is the "gold standard" for treatment of chronic prosthetic joint infection (PJI). It enables local delivery of antibiotics, maintenance of limb-length and mobility, easier reimplantation. However, bacterial colonization of spacers and mechanical complications can also occur. The aim of this study is to evaluate functional results and infection control in two-stage treatment of total hip arthroplasty (THA) PJI with and without a spacer.Materials and Methods: A retrospective review of 64 consecutive patients was conducted: 34 underwent two-stage revision using a cement spacer (group A), 30 underwent two-stage revision without a spacer (group B). At the final follow-up, functional evaluation of patients with a THA in site, without PJI recurrence, was performed using the Harris hip score (HHS). Measurement of limb-length and off-set discrepancies was performed using anteroposterior pelvic X-rays.Results: Most patients in group B were older with more comorbidities preoperatively. Thirty-three patients (97.1%) in group A underwent THA reimplantation versus 22 patients (73.3%) in group B (P<0.001). No significant differences in limb-length and off-set were observed. The results of functional evaluation performed during the final follow-up (mean, 41 months) showed better function in patients in group A (mean HHS, 76.3 vs. 55.9; P<0.001).Conclusion: The use of antibiotic-loaded cement spacer seems superior in terms of functional outcomes and reimplantation rate. Resection arthroplasty might be reserved as a first-stage procedure in patients who are unfit, who might benefit from a definitive procedure.
Acute Delayed or Late Infection of Revision Total Hip Arthroplasty Treated with Debridement/Antibiotic-loaded Cement Beads and Retention of the Prosthesis

Abstract:PURPOSE: The treatment of infected revision total hip arthroplasty (THA) is very challenging due to retained revision prosthesis, poor bone stock and soft tissue condition derived from previous revision surgeries, and comorbidities. The purpose of this study was to investigate the effectiveness and short-term outcomes of aggressive debridement and use of antibiotic-loaded cement beads with retention of the prosthesis for acute delayed or late infection of revision THAs.MATERIALS AND METHODS: Ten consecutive patients with symptoms or signs of less than one-week evolution and well-fixed prostheses, were treated with this procedure and a postoperative course of organism-specific antibiotics for a minimum of 6 weeks. All hips presented with acute delayed or late infection of revision THAs. Patients with a mean age of 68.1 years (range, 59-78 years) underwent an average of 1.9 previous revision THAs (1-4) before the index surgery. The minimal follow-up was 2 years with a mean of 46.2 months (range, 24-64 months).RESULTS: There were 8 cures (80.0%) and 2 failures with no mortality during the study period. The 2 failures involved the same and resistant bacteria implicated in the primary infection (methicillin-resistant <i>Staphylococcus aureus</i> and <i>Prevotella oralis</i>, respectively). The mean Harris hip score was 65.2 (range, 26-83) and the mean visual analogue scale was 2.6 (range, 1-4) at final follow-up.CONCLUSION: With a favorable success rate and no mortality, our procedure may be considered a safe and effective alternative for the treatment of acute delayed or late infection of revision THAs with well-fixed prostheses.
Successful Two-Stage Exchange Arthroplasty for Periprosthetic Infection Following Total Knee Arthroplasty: The Impact of Timing on Eradication of Infection

Abstract:Background: Periprosthetic joint infection (PJI) represents a serious complication following total knee arthroplasty. In the setting of chronic infections, the two-staged approach has traditionally been the preferred treatment method. The aim of this study was to determine the optimal period of rest between the first and second stage. Furthermore, we analyzed potentially outcome-relevant parameters, such as general and local conditions and the presence of difficult-to-treat or unidentified microorganisms, with regard to their impact on successful treatment of PJI. Patients and Methods: We performed a retrospective analysis of prospectively collected data for all patients treated for PJI at our institution. Seventy-seven patients who had undergone two-stage revision arthroplasty for PJI of the knee were included into the study. Antibiotic-loaded cement spacers were used for all patients. Results: After a median follow-up time of 24.5 months, infection had reoccurred in 14 (18.7%) patients. A prolonged spacer-retention period of more than 83 days was related to a significantly higher proportion of reinfections. Furthermore, significant compromising local conditions of the prosthetic tissue and surrounding skin, as well as repeated spacer-exchanges between first- and second-stage surgery, negatively influenced the outcome. Neither the patients' age nor gender exerted a significant influence on the outcome regarding reinfection rates for patients' age or gender. Conclusions: We observed the best outcome regarding infection control in patients who had undergone second-stage surgery within 12 weeks after first-stage surgery. Nearly 90% of these patients stayed free from infection until the final follow-up. An increased number of performed spacer-exchanges and a bad local extremity grade also had a negative impact on the outcome.

Abstract:BACKGROUND: Infection is one of the most serious complications of total hip arthroplasty (THA) surgery, although there are many ways can treat the infection, secondary revision procedures is recognized as an effect treatment method. OBJECTIVE: To retrospectively study the effect of antibiotic-loaded bone cement spacer in second-stage revision for infected THA. METHODS: Totally 27 hips from 26 cases who infected after THA were treated with two-stage revision using antibiotic-loaded bone cement spacer to instead the position of original prosthesis. During this procedure, a steinmann pin was load in the center of the spacer as scaffold. All the cases were followed up after the surgery and Harris score was evaluated. RESULTS AND CONCLUSION: Due to improper weight-bearing activities after surgery, the spacer of one case was ruptured, after removal of the bone cement spacer and debridement, the joint was spacious placed. The other 25 cases were all followed up for 1 to 7 years. After surgery, the mean Harris hip score reached 90.2 which improved 61.9 as compared with before operation, and the cure rate was 96.3%. It is indicated that infected THA can be successfully treated by two-stage revision surgery with antibiotic-loaded bone cement spacers, and has the advantage of higher infection control rate and joint function recovery.

Abstract:In the universe of entomology with its close to one million described and an estimated ten million undescribed species, Diabrotica virgifera virgifera LeConte (Coleoptera: Chrysometidae), the Western corn rootworm (WCR), is one of the economically most important pests. It causes annual treatment costs and damages of a billion dollars in the US (Metcalf 1986). Similar costs are predicted for Europe which the beetle invaded 15 years ago. Due to lack of natural enemies it is now expanding its territory at a rapid rate. With prior experience gained in Illinois, USA, and subsequent largely unsuccessful efforts to stop the pest in Europe, eradication efforts, although attempted in France and in the Veneto region of northern Italy, are not a viable sustainable strategy for the future nor a long term solution. Crop rotation, so far one of the best cultural management options of practicing entomologists, is beginning to show weaknesses specificly on Glycine max (Shaw et al. 1978, Levine et al. 2002, Spencer et al. 2005, Kiss et al. 2005, Tollefson and Prasifka 2006). WCR also was discovered on alternative hosts such as Cucurbita pepo in Slovenia (Hummel et al. 2007a, 2007b), C.maxima in Hungary (Moeser and Vidal 2001), and Serbia (Baca 1993, Baca and Berger 1994, Baca unpublished 1998, Baca 2007). WCR thus turns out to remain a challenge to general entomolog'sts, agricultural and ecological entomologists, geneticists, epidemiologists, crop protection engineers, phytosanitary services and economists alike. WCR and H. sapiens today are reaching a labile ecological equilibrium of coexistence, with new surprises on both sides in the "arms race" just waiting around the corner. Most experts will agree that WCR is in Europe to stay and is readily expanding from here to Asia and Africa if given the slightest chance.- The really unsolved question remains how to manage WCR sensibly, economically, and sustainably, and how and where to train a new generation of capable, reliable, all round entomological pest managers needed for the future.
Comparison of dynamic and static spacers for the treatment of infections following total knee replacement: a systematic review and meta-analysis

Abstract:Background: Revision surgery is the most common treatment for patients who develop infection after total knee arthroplasty (TKA). Two types of spacers are often used in revision surgery: dynamic spacers and static spacers. The comparative efficacy of these two types of spacers on knee prosthesis infections is not well established. Therefore, we carried out a systematic evaluation and meta-analysis with the aim of comparing the difference in efficacy between dynamic and static spacers. Methods: We conducted the literature search in PubMed, Web of Science, Cochrane Library, and Embase databases. The articles searched were clinical study comparing the difference in efficacy between dynamic spacers and static spacers for the treatment of prosthetic infections occurring after total knee arthroplasty. Results: We conducted a literature search and screening based on the principles of PICOS. Ultimately, 14 relevant clinical studies were included in our current study. We use infection control rate as the primary evaluation indicator. The KSS knee scores (KSSs), KSS functional scores, bone loss and range of motion (ROM) are secondary indicators of evaluation. Thirteen of these included studies reported the infection control rates, with no significant difference between dynamic and static shims (RR: 1.03; 95% Cl 0.98, 1.09; P = 0.179 > 0.05). The KSSs were reported in 10 articles (RR: 5.98; 95% CI 0.52, 11.43; P = 0.032 < 0.05). Six articles reported the KSS functional scores (RR: 13.90; 95% CI 4.95, 22.85; P = 0.02 < 0.05). Twelve articles reported the ROM (RR: 17.23. 95% CI 10.18, 24.27; P < 0.0001). Six articles reported the bone loss (RR: 2.04; 95% CI 1.11, 3.77; P = 0.022 < 0.05). Conclusion: Current evidence demonstrates that dynamic spacers are comparable to static spacers in controlling prosthetic joint infection. In terms of improving the functional prognosis of the knee joint, dynamic spacers are more effective than static spacers.
Evolving trends in liver transplantation: an outcome and charge analysis.

Abstract:BACKGROUND Due to the limited supply and increased demand for donor livers, waiting times are progressively lengthening, which may lead to transplantation at more advanced and less cost-effective stages of disease. The purpose of this study was to evaluate the outcomes and hospital charges of liver transplantation during two recent eras to identify areas for providing more cost-effective care. METHODS A total of 144 primary liver allografts were performed from 1991 to 1996. Patient characteristics, outcome measures, and hospital charges were compared for patients receiving allografts between 1991 and 1993 (group A) versus those receiving grafts between 1994 and 1996 (group B) using unpaired Student t tests for continuous data and chi-squared tests for categorical data. RESULTS In comparing groups A and B, no significant differences in patient demographics, relative contraindications, or indication for transplantation existed; median waiting time from date of listing until transplant increased from 88 days to 159 days; and a shift in UNOS priority status at time of transplantation occurred, as the percentage of patients requiring inpatient care increased from 58% to 75% (P=0.034). Despite this, patient hospital and 1-year survival significantly improved from 75.0% to 90.3% (P=0.016), and from 68.1% to 88.9% (P=0.002), respectively. Total hospital charges, without correction for inflation, were $174,908+/-16,388 in A and $193,525+/-14,444 in B (P=0.288). The increased charges were associated with longer inpatient length of stay (LOS) before transplant, resulting in increased pretransplant charges from $24,088+/-4134 (A) to $39,490+/-6,196 (B) (P=0.011). Room and service (54%) was the largest pretransplant contributor to charges, while blood products (23%), room and service (21%), organ acquisition (13%), and operating room charges (11%) contributed the most after transplant. CONCLUSIONS Longer waiting times resulting in transplantation at later stages of disease have occurred, leading to longer pretransplant LOS and its associated charges. Despite more advanced disease, patient survival rates have significantly improved with fewer infection-related deaths. LOS pretransplant, blood products, and operating room services represent potential areas for providing more cost-effective care.

Abstract:Objective: To evaluate the application and clinical results of two-stage total hip arthroplasty in the treatment of the deep hip infection following the internal fixation of hip fractures. Methods: From May 2007 to November 2014, 21 patients with active hip infection secondary to internal fixation of hip fractures were treated with two-stage total hip arthroplasty using a temporary antibiotic-loaded cement spacers. Of 21 cases, 15 were males and 6 were females, aged from 27 to 64 years (mean, 45); there are 18 cases of femoral neck fractures and 3 cases of intertrochanteric fractures. The serologic examination and X-ray were taken at 1 month, 3 months, 6 months, 12 months and annually thereafter post-operatively to evaluate the clinical results and prosthesis status. Harris hip score system was used to evaluate the joint function. Results: All patients were successfully treated with two stage operations under general anesthesia. The operational interval was 12-44 weeks (mean, 21) and 1 spacer breakage. For the arthroplasty, cementless components were used in 20 cases and cemented component was use in 1 case. The patients were followed up 25-102 months ( mean, 55 ) and infections were eradicated in all hips. The Harris hip score was improved from 23.24±11.81 pre-operatively to 90.24±3.92 post-operatively and the difference was statistically significant (P<0.05). According to this scoring criteria, the excellent and good rate reached up to 95%. At latest fellow-up, the location of prosthesis were well and the function of hip joint was satisfied. All cases had no dislocation, periprosthetic fracture, ectopic ossification or any other complications at the latest fellow-up. Conclusion: By means of an antibiotic-loaded cement spacer, two-stage total hip arthroplasty is an effective salvage procedure eradicating infection and providing functional improvement to the infected internal fixation of hip fractures. The early and mid-term clinical effects are satisfied.

Abstract:Background: Periprosthetic joint infection remains a common and serious complication after hip arthroplasty. To improve function and patient comfort after joint removal in two-stage revision, commercially manufactured spacers for the hip joint allow retention of the anatomical joint geometry thereby limiting soft tissue contraction and allow mobilization. Indications: Periprosthetic joint infection of the hip, septic arthritis with severe destruction of the hip cartilage and/or bone requiring arthroplasty. Contraindications: Allergies to polymethylmethacrylate (PMMA) or antibiotics, severe hip dysplasia with insufficient cranial support, incompliant patient, large osseous defect of the acetabulum, insufficient metaphyseal/diaphyseal support of the femoral bone, resistance of the microbiological pathogen to spacer-inert antibiotic medication, inability to perform primary wound closure requiring temporary open-wound therapy. Surgical technique: Preoperative templating on radiograph; removal of joint prosthesis and thorough debridement with removal of all foreign material; trial spacer selection and insertion and trial reduction of the joint, fixing the spacer with PMMA to the proximal femur, final reduction, radiograph and stability test. Results: Data were analyzed from patients treated between 2016 and 2021. In all, 20 patients were treated with preformed spacers and 16 with custom-made spacers. Pathogens were detected in 23 of the 36 cases (64%). Polymicrobial infections were present in 8 of 36 cases (22%). In patients who received preformed spacers, there were 6 cases of spacer-related complications (30%). Of the 36 patients (83%), 30 were reimplanted with a new implant; 3 patients died due to septic or other complications before reimplantation (8%). Average follow-up was 20.2 months after reimplantation. There were no major differences between the two groups of spacers. Patient comfort was not measured.
