Abstract:Objective To evaluate the security and effectiveness of targeted percutaneous transforaminal endoscopic discectomy(PTED) in treating lumbar disc herniation.Methods A total of 58 patients with single segment lumbar disc herniation who were treated with PTED from May 2011 to February 2012 were retrospectively analyzed.All patients underwent targeted paracentesis and PTED after local anesthesia.The clinical outcomes were evaluated by visual analog scale(VAS) scores,Oswestry disability index(ODI) and modified Macnab criteria pre-and postoperative or at the final follow-up.Results All patientswere followed up for 12-22 months(mean 18.5 months).Compared with the preoperative VAS scores,the postoperative scores of back and leg pain were significantly decreased,and the difference was statistically significant(P < 0.01).ODI before and 12 months after operation were 72.4% and 19.7% respectively.There were significant differences compared with preoperative score(P < 0.01).According to the modified Macnab criteria,the excellent and good rate was 93.1%.Conclusion Targeted PTED is safe and effective in the treatment of lumbar disc herniation,which has little trauma and few complications.Meanwhile,targeted puncture is the significant premise and key process to perform a successful operation.

Abstract:Discogenic low back pain (DLBP) is a common cause of low back pain and is often overlooked. When the conservative treatment such as physical therapy/drug therapy is ineffective, and the indications for surgery are not strong, the minimally invasive interventional therapy including epidural hormone injection, percutaneous disc chemical dissolution, and percutaneous low-temperature plasma ablation, are better choices for patients with DLBP. This article reviewed the research progresses of minimally invasive interventional therapy for DLBP with recent literatures.
The Strategy and Early Clinical Outcome of Percutaneous Full-Endoscopic Interlaminar or Extraforaminal Approach for Treatment of Lumbar Disc Herniation

Abstract:Objective is to analyze the surgical strategy, safety, and clinical results of percutaneous full-endoscopic discectomy through interlaminar or extraforaminal puncture technique for LDH. Preoperative CT and MRI were analyzed, which were based on the main location of the herniated disc and its relationship with compressed nerve root. Sixty-two patients satisfied the inclusion criteria during the period from August 2012 to March 2014. We use percutaneous full-endoscopic discectomy through different puncture technique to remove the protrusive NP for LDH. Sixty patients completed the full-endoscopic operation successfully. Their removed disc tissue volume ranged from 1.5 mL to 3.8 mL each time. Postoperative ODI and VAS of low back and sciatica pain were significantly decreased in each time point compared to preoperative ones. No nerve root injury, infection, and other complications occurred. The other two patients were shifted to open surgery. No secondary surgery was required and 91.6% of excellent-to-good ratio was achieved on the basis of Macnab criteria at postoperative 12 months. Acquired benefits are fewer complications, rapid recovery, complete NP removal, effective nerve root decompression, and satisfactory cosmetic effect as well. This is a safe, effective, and rational minimally invasive spine-surgical technology with excellent clinical outcome.

Surgical Strategy and Outcomes of Full Endoscopic Lumbar Discectomy for Recurrent Lumbar Disk Herniation Following a Previous Full Endoscopic Lumbar Discectomy

Abstract:Objective: Recurrent lumbar disc herniation (RLDH) cannot be prevented after full endoscopic lumbar discectomy (FELD), and the optimal surgical treatment for RLDH after FELD remains controversial. The aim of the study was to suggest a surgical strategy for FELD to treat RLDH following a previous FELD and to present surgical outcomes. Methods: Between February 2015 and March 2022, 68 patients with RLDH were surgically treated with FELD retrospectively. An original approach was suggested for the treatment of early recurrence (24 h-2 weeks). The full endoscopic transforaminal technique (FETD) was considered for patients requiring local anesthesia, and in RLDH with FETD indications or FEID technological difficulties. The full endoscopic interlaminar technique (FEID) was chosen in RLDH with FEID indications. Both FEID and FETD were suitable if no FEID or FETD technological difficulties existed. Clinical efficacy was evaluated using the visual analog scale (VAS) score, Oswestry disability index (ODI), and modified MacNab criteria. Postoperative follow-up data at 24 h, 3 months, and final-follow-up were recorded. Operation time and clinical outcomes were assessed with t test. p-value < 0.05 was considered statistically significant. Results: All 68 patients had an average follow-up time of 23.8 months (range, 6-76 months). In the 13 cases of early recurrence, the operation time (32.30 ± 9.67 vs. 58.00 ± 6.16 min) in the original surgical approach group was shorter than that in the changing surgical approach group (p < 0.05). In the 28 cases of L4/5 FETD recurrence (>2 weeks), the operation time (66.17 ± 12.18 vs. 53.60 ± 5.45 min) in the FETD group was more than that in the FEID group (p < 0.05). In the 22 cases of L5S1 recurrence (>2 weeks), the operation time (55.75 ± 8.79 vs. 79.33 ± 6.65 min) in the FEID group was shorter than in the FETD group (p < 0.05). The postoperative VAS and ODI scores at 24 h, 3 months, and final-follow up were all significantly improved, compared with their preoperative counterparts (p < 0.05). Based on the modified MacNab criteria, 88.23% of patients showed excellent or good results. Re-recurrence occurred in three patients at 3 months. No surgical complications were observed in any of the 68 cases. Conclusion: FELD is a safe and effective treatment option for RLDH with previous FELD. For early recurrence, the original approach was suggested. Both FEID and FETD were effective and safe for recurrence (>2 weeks), but FEID seemed more efficient for L4/5 RLDH after previous FETD and for L5S1 RLDH.
The Early Clinical Outcomes of a Percutaneous Full-Endoscopic Interlaminar Approach via a Surrounding Nerve Root Discectomy Operative Route for the Treatment of Ventral-Type Lumbar Disc Herniation

Abstract:The objective of this study is to introduce a method using a percutaneous full-endoscopic interlaminar approach via a surrounding nerve root discectomy (SNRD) operative route that involves removing the protrusive disc via both the shoulder and the axilla of the corresponding nerve root for the treatment of ventral-type lumbar disc herniation (VLDH) and its early clinical symptoms. Twenty-two patients with VLDH satisfied the inclusion criteria and underwent the full-endoscopic interlaminar approach operation via a SNRD successfully during the period from November 2014 to June 2016. All operations were completed without conversion to other surgical techniques. The average operation time was 78.64 ± 25.97 min (50–145 min). The average removed disc tissue volume was 2.87 ± 0.48 ml (2–3.6 ml). No nerve root injury, infection, or other complications occurred. The postoperative ODI and VAS values of low back and sciatic pain were significantly decreased at each time point compared to preoperative measurements P < 0.05 . The MacNab scores at the 12-month follow-up included 15 excellent and 7 good scores. In summary, a percutaneous full-endoscopic interlaminar approach through SNRD is a safe and effective treatment option for patients with VLDH.

Abstract:AIM: Percutaneous endoscopic transforaminal discectomy (PELD) is a new minimally invasive spine surgery for patients with lumbar disc herniation (LDH). Based on the 3-year follow-up data, the effect of PELD on the clinical outcomes of patients with LDH through a retrospective cohort study was analyzed in this article, so as to provide guidance for clinical selection of surgical options. METHODS: The clinical data of 150 patients with LDH admitted to our hospital from January 2019 to October 2020 were retrospectively analyzed. According to the surgical methods recorded in the medical record system, the patients were divided into the open lumbar microdiscectomy (OLM) group (n = 50) and the PELD group (n = 100). The surgical and postoperative recovery indicators of the two groups were compared after matching. These included incision length, intraoperative blood loss, operation time, postoperative ambulation time and hospital stays, recovery rate, short-term complication rate, Lumbar visual analogue scale (VAS) score, and Oswestry Disability Index (ODI) score. RESULTS: Compared with the OLM group, the PELD group had shorter incision length, shorter operation time, shorter postoperative ambulation time, shorter hospital stays, less intraoperative blood loss, lower short-term complication rate, lower lumbar pain and dysfunction scores at 3 months, 6 months, and 1 year after operation, higher short-term excellent-and-good recovery rate, and higher quality-of-life scores at 3 years after operation (p < 0.05). CONCLUSIONS: Compared with OLM, PELD in the treatment of LDH patients can reduce the operation time, blood loss, and length of hospital stays, suggesting a short-term postoperative recovery effect. Compared with OLM, PELD can also reduce the incidence of short-term complications, enhance the effect of pain control and improvement of dysfunction in the medium term, and improve the long-term quality of life.

Abstract:Purpose Percutaneous endoscopic lumbar discectomy has been increasingly used in the treatment of lumbar disc herniation. However, there is no consensus on which method would be more effective between the transforaminal and interlaminar approach. Objective To compare clinical outcomes, patient satisfaction rate, reoperation rate, and residual symptoms between percutaneous endoscopic transforaminal discectomy (PETD) and percutaneous endoscopic interlaminar discectomy (PEID). Study design A comparative, retrospective, controlled trial. Setting The study was conducted at the Department of Orthopaedics at a local hospital. Methods From January 2015 to September 2020, consecutive patients who underwent PETD or PEID treatment for lumbar disk herniation (LDH) at the L5/S1 level in our department were retrospectively collected. Baseline data including age, gender, body mass index (BMI), smoking status, alcohol drinking, clinical symptoms, physical examination, and radiographic characteristics were documented. During the two to three year follow-up periods, patients were evaluated clinically, including clinical outcomes assessed by the visual analog score (VAS), patient satisfaction rates assessed by the North American Spine Society patient satisfaction index (PSI), recurrent rate, and residual symptoms. Results A total of 113 patients with PELD in our department were included in the current study, with 65 patients in the PETD group and 48 in the PEID group. Demographic characteristics including age, gender, height, BMI, cigarette smoking, alcohol consumption status, and diabetes did not show any significant difference between the PETD and PEID groups. The VAS scores of the two groups were similar preoperatively, but the postoperative VAS score of the PEID group was lower than that of the PETD group. There were 90.8% of patients in the PETD group who were satisfied with the operation compared to 97.9% in the PEID group. The recurrence rate did not differ between groups, with three patients in both groups. Regarding residual symptoms, there were more patients in the PETD group who reported low back pain during the follow-up periods. Limitation The main limitations are that all patients were operated by the same surgical team from the same site, and there was a lack of multicenter data. Conclusion Both PETD and PEID have satisfactory patient-reported outcomes for treating LDH. The PEID procedure results in fewer low back pain residual symptoms than the PELD procedure.

Methods: We retrospectively analyzed young patients with single-segment discogenic lumbago from July 2018 to June 2020 in our department who underwent percutaneous endoscopic surgery according to the inclusion and exclusion criteria. We finally enrolled 20 patients. The follow-up time was 6– 30 months. In all patients, we recorded the visual analog scale (VAS) score for waist pain and the Oswestry Disability Index (ODI) preoperatively, immediately postoperatively and at the last follow-up. We used the modified MacNab criteria to assess the curative effect at the last follow-up. Results: All 20 patients underwent successful operations without complications. No recurrence was observed during follow-up. The VAS score of low back pain was 5.05± 1.19 points before surgery, 1.50± 051 points immediately after surgery, and 1.10± 0.72 points at the last follow-up (P < 0.05 preoperative vs both postoperative). At the last follow-up, the VAS scores of all 20 patients were ≤ 2, and 4 patients had no pain. The ODI was 46.66± 7.03% before surgery, 9.78± 4.05% immediately after surgery, and 4.11± 3.18% at the last (P < 0.05, preoperative vs both postoperative). According to the evaluation under the modified MacNab standard, the good–excellent rate of clinical efficacy at the last follow-up was 95%. Conclusion: Percutaneous endoscopic spinal surgery can significantly improve the symptoms and dysfunction of young patients with DLBP and has little effect on the biomechanical stability of the lumbar spine. Discogenic low back pain (DLBP) is a kind of chronic nonspecific low back pain with a high incidence. 1,2 It is mainly resting low back pain caused by structural and functional disorders of the disc due to intervertebral disc degeneration. Long-term chronic pain has a great impact on quality of life, causing a great burden on the social economy and other aspects of life, and it can even cause disability. 1–3 In essence, DLBP is caused by a series of pathological changes caused by disc degeneration. Due to the long-term bad posture of the young patients, the intervertebral disc showed a series of degenerative reactions, such as dehydration of the nucleus pulposus and "black", the formation of annulus fissure, the formation of pannus mediated by inflammatory factors, and the entry of nerve endings into annulus fibrosus with inflammatory granulation tissue. Percutaneous Intradiscal Radiofrequency Thermocoagulation Combined with Sinuvertebral Nerve Ablation for the Treatment of Discogenic Low Back Pain

Abstract:Background: Percutaneous intervertebral radiofrequency thermocoagulation (PIRFT) and sinuvertebral nerve ablation (SVNA) are commonly used clinical treatments for discogenic low back pain (DLBP). However, they have been reported to have low efficacy rates of approximately 16.5%-26.5%, especially in the medium to long term. Objectives: To investigate whether PIRFT combined with SVNA can reduce pain and improve clinical outcomes in patients with DLBP. Study design: This is a prospective study. Setting: All data were from Honghui Hospital in Xi'an. Methods: Following the inclusion and exclusion criteria, 195 patients were enrolled in this study and randomly divided into 3 groups of 65 patients each and treated with PIRFT+SVNA, PIRFT, or SVNA. Postoperative follow-ups were done at one week, one month, 3 months, 6 months, and 12 months. The demographic characteristics, relevant surgical information, and observed complications of all groups were recorded. The efficacy of the surgeries was evaluated using the visual analog scale (VAS), Oswestry disability index (ODI), and modified Macnab criteria. Results: In total, 167 patients, comprising 81 men and 86 women (aged 28-75 years), were included in this study and completed postoperative follow-ups. There were 54 patients in the combined PIRFT and SVNA (PIRFT+SVNA) group, 58 patients in the PIRFT group, and 55 patients in the SVNA group. All groups were comparable because there were no significant differences in gender, age, disease duration, follow-up time, surgical segments and presence of high-intensity zones of the groups (P > 0.05). In addition, the efficacy of the PIRFT+SVNA group was significantly higher than that of the PIRFT and SVNA groups as assessed by the modified Macnab criteria (P = 0.032). Surgery was successfully completed in all 3 groups, and VAS and ODI improved at all postoperative time points in all 3 groups compared to the preoperative scores. The differences between the VAS and ODI scores preoperation and 12 months postoperation were not statistically significant between all 3 groups. However, at one week, one month, 3 months, and 6 months after surgery, the VAS and ODI scores were lower in the PIRFT+SVNA group compared to the PIRFT and SVNA groups. The difference in VAS scores among the 3 groups was most significant at one week postoperation, and the difference in ODI scores was most significant at one month postoperation. The VAS and ODI improvement rates of the 3 groups showed significant improvement at one week, one month, 3 months, and 6 months postoperation (P < 0.05). There was no significant difference among the 3 groups at 12 months postoperation (P > 0.05). Limitations: This study was limited by its small sample size in a single-center study. Conclusions: In DLBP, the sinuvertebral nerve (SVN) is the main nerve involved in the lumbar disc pain signaling pathway, and compared with PIRFT and SVNA alone, combined PIRFT and SVNA treatment may provide more satisfactory pain relief and functional improvement at an early stage.

Abstract:Lumbar disc herniation (LDH) is a prevalent degenerative spinal disease affecting a significant portion of the population, often causing severe low back pain and radiculopathy. [ 1 ] When conservative treatment proves ineffective, open discectomy with nucleus pulposus removal has traditionally been the preferred surgical intervention. This approach, however, presents several limitations, including extensive tissue dissection, elevated complication risks, and extended recovery periods. [ 2 , 3 ] In recent years, minimally invasive techniques like percutaneous endoscopic lumbar discectomy (PELD) have revolutionized the surgical management of LDH. PELD offers several advantages over open discectomy, including smaller incisions, less tissue trauma, quicker recovery, and lower complication rates. [ 4 , 5 ] Notwithstanding these advantages, both open and endoscopic nucleotomy can lead to annular fissures, which are significant factors in postoperative recurrence and discogenic low back pain. [ 6 ]

Transforaminal Endoscopic Discectomy for Thoracolumbar Disc Herniation: A Retrospective Study and Technical Note.

Abstract:BACKGROUND:Thoracolumbar disc herniation (TLDH) is a rare disorder with unique characteristics that can result in undesirable surgical outcomes after traditional discectomy. In view of the widespread use of transforaminal endoscopic discectomy for lower lumbar disc herniation, we investigated treatment of TLDH by this procedure. The purpose of this study was to evaluate the clinical efficacy of transforaminal endoscopic discectomy for treating TLDH and share our technical experience.METHODS:We retrospectively evaluated the clinical data of 19 patients who had undergone transforaminal endoscopic discectomy for TLDH in our institution between April 2018 and July 2021. Operation time, follow-up time, blood loss, postoperative hospital stay, visual analog scale scores for low-back and leg pain, and Japanese Orthopedic Association (JOA) scores were evaluated.RESULTS:The differences between preoperative and postoperative JOA and visual analog scale scores were significant (P < 0.05). According to the JOA scores, 14 of the 19 patients had excellent improvement, 3 had good improvement, and 2 had fair improvement; thus, the rate of satisfactory improvement was 89.5%.CONCLUSIONS:Operation time, blood loss, postoperative hospital stay, and surgical outcomes were favorable. Transforaminal endoscopic discectomy is an ideal surgical procedure for treating TLDH.
Efficacy of Percutaneous Transforaminal Endoscopic Discectomy for the Treatment of Patients Undergoing Lumbar Disc Herniation With Positive Nerve Root Sedimentation Sign: a Retrospective Case-Control Study

Abstract:Abstract Background: To investigate the efficacy of percutaneous transforaminal endoscopic discectomy (PTED) in the treatment of patients who were diagnosed with lumbar disc herniation (LDH) with positive nerve root sedimentation sign (NRSS). Methods: A total of 86 patients who underwent LDH were recruited and divided into NRSS-positive group (n=49) and NRSS-negative group(n=37). The visual analog scale (VAS), Japanese Orthopaedic Association (JOA), and Oswestry Disability Index (ODI) were used to evaluate the low back pain and functional recovery and were compared between the two groups. Results: There were no significant differences in the demographic parameters between the two groups. The average area of the dural sac compression in the NRSS-positive group was significantly higher than that in the NRSS-negative group. Patients with a positive NRSS showed a better low back pain relief than those with a negative NRSS at 1 week and 1 month after surgery. The JOA and ODI in the NRSS-positive group were better than those in the NRSS-negative group at 3 months and 6 months postoperatively. The apparent efficiency of JOA and the excellent and good rate of ODI in the negative group was lower than that in the positive group at 6 months after surgery. Conclusions: Patients undergoing LDH with a positive NRSS showed better pain relief and functional recovery than those with a negative NRSS. The present study suggested that NRSS might be a valuable sign and associated with better clinical outcomes in patients undergoing LDH with the treatment of PTED.

Abstract:Background To investigate the clinical efficacy of percutaneous endoscopic transforaminal discectomy (PETD) in the treatment of upper lumbar disc herniation (LDH). Methods Twenty-two patients, 14 males and 8 females with ages ranging from 23 to 76 years, who had upper LDH and were treated with PETD from April 2015 to April 2020 in the Department of Neurosurgery of Xuanwu Hospital, were selected to evaluate the surgical efficacy by the visual analog scale (VAS) and Oswestry Disability Index (ODI). Results All patients underwent successful completion of PETD surgery. The operation time was 80.4 ± 18.0 min; intraoperative fluoroscopy was used 17.1 ± 8.7 times; and the hospital stay was 3.2 ± 0.6 days. The VAS scores were 7.9 ± 1.2, 2.3 ± 1.5, 2.2 ± 1.3, and 2.1 ± 1.0 before the operation, 1 day and 3 months after the operation, and during the last follow-up, respectively. The postoperative VAS score was significantly lower than that before the operation ( P < 0.01). The ODI scores before and 3 months after the operation were 59.8 ± 16.8 and 15.3 ± 8.2, respectively; thus, the postoperative score was decreased ( P < 0.01). Conclusion Upper lumbar discs have unique anatomical structures, and PETD is a safe and effective surgical method for the treatment of upper LDH.

Abstract:Objective To discuss the clinical effects of the percutaneous endoscopic discectomy of L_5S_1 disc herniation through different approaches: postlateral transforaminal and interlaminar approach. Methods From June 2003 to October 2008, 57 patients with L_5S_1 disc herniation were included in the study. There were 32 males and 25 females, with a mean age of 42, ranging from 17 to 79.22 patients un-derwent percutaneous endoscopic diseectomy through postlateral transforaminal approach while the other 35 patients through interlaminar approach. Oswestry Function Index was used to evaluate preoperative and post-operative lumbar vertebrae, function. The clinical outcome, indications and the risk of secondary operation of two approaches were analyzed. Results All of the patients had followed up, with an average period of 13.4 months, ranging from 12 to 64 months. There is no difference in age and blood loss during operation in two groups. One revision were needed in each groups. One ease of postoperatve infection was found in each group. When far lateral disc herniations were identitied postlateral transforaminal approach was applied, the high iliac crest had to be considered. When intracanalicular disc herniations at the L_5S_1 level were found, Interlaminar approach was suitable. For postlateral transforaminal approach , the average OFI had improved from 74.36 to 13.91. For interlaminar approach, the average OFI had improved from 77.45 to 12.56. There is no difference in OFI between the two groups According to MacNab's criteria, Overall excellent-to-good re-sults were 86% and 89% in the patients underwent postlateral transforaminal approach and those underwent interlaminar approach. Conclusion Both postlateral transforaminal approach and interlaminar approach are safe, effective, and minimally invasive procedure for the treatment of disc herniations at the L_5S_1 level in properly selected cases.

Abstract:Objective: To observe the clinical effect percutaneous transforaminal endoscopic discectomy (PTED) in the treatment of single lumbar disc herniation. Methods: From August 2017 to June 2019, 42 patients with low lumbar single segment lumbar disc herniation were treated with percutaneous transforaminal endoscopic discectomy surgery in our hospital. The operation time, incision size, bleeding volume and hospitalization time were recorded respectively. The patients were evaluated before operation, 1 month and 6 months after operation. Visual analogue scale (VAS) and assessment were used to evaluate the lumbocrural pain. The JOA score and the Oswestry disability index (ODI) were used to evaluate the lumbar function, and the modified macnab score was used to evaluate the clinical effect in the last follow-up. Results: All the 42 patients successfully completed the operation without any other operation. There were no severe complications such as dural injury and nerve root injury. The operation time was (76.98 ± 8.58) min, the incision size was (8.45 ± 1.2) mm, the bleeding volume was (20.14 ± 2.93) ml, and the hospitalization time was (4.55 ± 1.13) d. One month and six months after the operation, the visual analogue scale (VAS), the evaluation of lumbar function (Oswestry) and the disability index (ODI) were significantly improved compared with those before the operation (P < 0.05). The last follow-up evaluation of the clinical effect of modified macnab: excellent in 30 cases, good in 8 cases, fair in 3 cases and poor in 1 case. The excellent rate was 90.47%. One patient developed back pain and discomfort. One case recurred. Conclusion: The treatment of low lumbar but segmental lumbar disc herniation with percutaneous intervertebral foramen, with small incision, less bleeding and quick recovery, can improve the pain and dysfunction of patients.

Abstract:The objective of this study is to compare the clinical effectiveness of visualization of percutaneous endoscopic lumbar discectomy (VPELD) combined with annulus fibrosus suture technique and simple percutaneous endoscopic lumbar discectomy (PELD) technique in the treatment of lumbar disc herniation. A retrospective analysis was conducted on 106 cases of lumbar disc herniation treated with foraminoscopic technique at our hospital from January 2020 to February 2022. Among them, 33 cases were treated with VPELD combined with annulus fibrosus suture in group A, and 73 cases were treated with PELD in group B. The preoperative and postoperative visual analogue scale (VAS), functional index (Oswestry Disability Index, ODI), healing of the annulus fibrosus, intervertebral space height, and postoperative recurrence were recorded and compared between the two groups. All patients underwent preoperative and postoperative MRI examinations, and the average follow-up period was 12 ± 2 months. Both groups showed significant improvements in postoperative VAS and ODI scores compared to the preoperative scores ( P < 0.05), with no statistically significant difference between the groups during the same period ( P > 0.05). There was no significant decrease in intervertebral space between the two groups after surgery ( P > 0.05). Group A showed significantly lower postoperative recurrence rate and better annulus fibrosus healing compared to group B ( P < 0.05). The VPELD combined with annulus fibrosus suture technique is a safe, feasible, and effective procedure for the treatment of lumbar disc herniation. When the indications are strictly adhered to, this technique can effectively reduce the postoperative recurrence rate and reoperation rate. It offers satisfactory clinical efficacy and can be considered as an alternative treatment option for eligible patients.

An Analysis of the Clinical and Radiological Prognostic Factors Affecting the Outcomes of Lumbar Intradiscal Biacuplasty

Abstract:Purpose: Intradiscal biacuplasty (IDB) has been proven to be effective for treating lumbar degenerative disc disease (DDD). However, there has not been a reported prognostic factor for IDB. The present study meticulously evaluates the general and radiographic features that may serve as markers for predicting the therapeutic outcome of IDB. Methods: A prospective case series study was conducted, following time-series analysis moving averages models, with forty-one patients suffering from chronic discogenic lower back pain for more than six months. These patients subsequently received lumbar cool radiofrequency IDB and were enrolled in the study. Thirty-seven patients completed follow-up questionnaires at 1, 3, 6, and 12 months. The surgical outcomes were reported using visual analogue scale (VAS), Oswestry disability index (ODI), and the consumption of nonsteroidal anti-inflammatory drugs (NSAID). Furthermore, a univariate analysis was performed to identify prognostic factors associated with pain relief from age, gender, body mass index (BMI), and pre-operative lumbar magnetic resonance imaging reading. Results: Significant reductions were found in estimated VAS and ODI at the post-operative period at 1, 3, 6, and 12 months (P < 0.001). The NSAID dosage was significantly decreased at 3-month and 1-year follow-up (P < 0.05). No procedure-related complications were detected. The prognosis of IDB was not related to disc height, Pfirrmann grading or Modic endplate change. However, disc extrusions were associated with promising outcomes (VAS improvement ≥ 50%) on pain relief (P < 0.05). Conclusion: IDB is a good alternative choice for treating lumbar DDD. Patients with a painful extrusion lumbar disc may gain some benefits after receiving IDB following a period of failed conservative treatment. These findings may also add some references for physicians in the decision making when treating lumbar DDD.

This study aimed to describe the specific technology of percutaneous transforaminal endoscopic lumbar foraminotomy (PTELF) as a therapeutic intervention for managing radicular leg pain (RLP) resulting from stable degenerative lumbar isthmic spondylolisthesis (DLIS) and to present the associated clinical results. Methods: From March 2022 and April 2023, 25 patients were diagnosed with single-level stable DLIS with RLP and underwent PTELF. Clinical assessments utilized the visual analog scale (VAS), Oswestry Disability Index (ODI), and modified MacNab criteria. All endoscopic surgery videos were reviewed to interpret the pathology associated with DLIS. Results: The mean age of the cohort was 65.3 ± 11.0 years. The mean preoperative ODI score, VAS score for low back, and VAS score of the leg were 64.1 ± 8.2, 7.0 ± 0.7, and 7.3 ± 0.8, respectively. These scores significantly improved to 16.3 ± 10.4, 2.0 ± 0.6, and 1.7 ± 1.0 at the final follow-up, respectively (P< 0.01). The modified MacNab criteria indicated "good" or "excellent" outcomes in 92.0% of cases. Analysis of 23 surgical videos revealed 15 patients with disc herniation, nine with lower vertebral endplate involvement, consistent presence of uneven bone spurs (at the proximal lamina stump and around the foramen), and accumulated scars. Two patients experienced postoperative dysesthesia, and one encountered a recurrence of RLP. Conclusion: PTELF emerges as a potentially safe and effective procedure for alleviating RLP in patients with stable DLIS. However, additional evidence and extended follow-up periods are imperative to evaluate the feasibility and potential risks associated with PTELF. Keywords: radicular leg pain, lumbar isthmic spondylolisthesis, transforaminal, foraminotomy, local anesthesia Degenerative Lumbar isthmic spondylolisthesis (DLIS) is a form of lumbar spondylolisthesis, characterized by an abnormal isthmus connection leading to the anterior displacement of one vertebral body relative to the next. 1,2 This anterior movement primarily occurs in the early stages of the disease and eventually halts, reaching a stable state irrespective of whether the isthmus defect undergoes bone or fibrous healing. 3,4 At this juncture, conservative measures such as anti-inflammatory drugs and physical therapy may offer relief from DLIS-induced pain. However, a substantial number of patients experience persistent and refractory back or radicular leg pain (RLP) or both, necessitating surgical intervention to alleviate symptoms and restore physical function. 5 The distinctive pathological features of DLIS encompass four key aspects: fibrocartilage formation or scar accumulation at the isthmus defect, irregular and uneven bone spurs (isthmus bone spurs extending from the proximal lamina stump to the extraforaminal exit zone and bone spurs distributed at the edge of the foramen), 6 involvement of the lower vertebral endplate (LVE), and disc herniation (DH). These factors, alone or in combination, contribute to foraminal stenosis (FS), exerting pressure on the exiting nerve root (ENR) and resulting in RLP in patients. 2,6 Concurrently, vertebral spondylolisthesis and diminished disc height can exacerbate this pathological condition. Currently, decompression with or without fusion is regarded as a viable surgical option for treating DLIS. 7 However, the gold standard fusion surgery is conventionally associated with a high incidence of complications and substantial costs. 8 In adult DLIS cases, the degree of slippage seldom increases, and therefore the need for stabilization and fusion is rarely required to prevent progression of the malalignment. 9 In cases where patients exhibit stability without evident deformities associated with DLIS, growing evidence supports decompression without fusion as a viable treatment option. 10 Consequently, decompression alone can serve as an effective intervention for younger patients, delaying the need for fusion surgery or as a means of symptomatic relief in older individuals who may not tolerate spinal fusion. 6 Certain endoscopic surgeries have been employed to address patients with DLIS expe -Abstract Truncated-

Abstract:PurposeThis study reports a new approach for percutaneous endoscopic lumbar iscectomy (PELD), especially for soft, highly down-migrated lumbar disc herniation.MethodsSeven patients with soft, highly down-migrated lumbar disc herniation who underwent PELD via a translaminar approach under local anaesthesia from January 2013 to June 2015, including five patients who underwent failed PELD in other hospitals, were retrospectively analyzed. Clinical outcomes were evaluated according to pre-operative and post-operative visual analogue scale (VAS) scores, Oswestry disability index (ODI) scores and post-operative magnetic resonance imaging (MRI).ResultsThe highly down-migrated lumbar disc herniation was completely removed by PELD via a translaminar approach in seven patients, as confirmed by post-operative MRI. Leg pain was eased after removal of the disc migrations. The mean follow-up duration was 9.8 (range, 6-14) months. The mean pre-operative VAS was 7.6 ± 0.8 (range, 6-9), which decreased to 3.1 ± 1.5 (range, 2-5) at one week post-operatively and to 1.3 ± 0.8 (range, 0-3) by the last follow-up visit. The mean pre-operative ODI was 61.6 (range, 46-84), which decreased to 16.3 (range, 10-28) at the one month post-operative follow-up and to 8.4 (range, 0-14) by the last follow-up visit. No recurrence was observed in any of the seven patients during the follow-up period.ConclusionPELD via a translaminar approach could be a good alternative option for the treatment of soft, highly down-migrated lumbar disc herniation.
