Abstract:Background. Pulpotomy is a vital pulp treatment procedure frequently used in primary molars to preserve pulp vitality and function until tooth exfoliation. There is currently no pulp medicament with all the features of the ideal pulpotomy material. The present study compared the radiographic success of sodium hypochlorite with ferric sulfate (FS) when used for pulpotomy in primary molars. Methods. A retrospective analysis was performed to evaluate the success rates of primary molars pulpotomized using sodium hypochlorite or FS according to radiographic findings. Healthy 4‒10-year-old children who had pulpotomy procedures on primary molars between 2018 and 2021 at the pediatric dental clinic and had a control radiograph at least 12 months later were enrolled in the study. The chi-squared test was used to determine the differences in success between these two materials. Results. A total of 142 teeth, including 85 (59.9%) first primary molars and 57 (40.1%) second primary molars, in 98 healthy children were evaluated. The mean follow-up period of the teeth included in the study was 585.1±249.4 days. Radiographic success rates for NaOCl and FS groups were 73.8% and 71.0%, respectively, with no statistically significant difference (P>0.05). Internal root resorption (IRR) was the most common cause of radiographic failure in both groups. Conclusion. Radiographic success rates of both materials were similar, and using these materials in primary molar pulpotomy procedures can be recommended in clinical practice.
Primary molar pulpotomies with different hemorrhage control agents and base materials: A randomized clinical trial

Abstract:Objective: To evaluate the clinical and radiographical success of primary molar pulpotomies which used 15.5% ferric sulfate (FS) or 1.25% sodium hypochlorite (NaOCl) for hemostasis and zinc oxide-eugenol (ZOE) and calcium hydroxide (CH) pastes as base materials. Methods: In 29 healthy children, 80 primary molars were randomly allocated to one of the study groups: Group 1: FS-ZOE, Group 2: FS-CH, Group 3: NaOCl-ZOE, and Group 4: NaOCl-CH. After hemostasis with the respective solutions, pulp stumps and floor of the pulp chambers were covered with either ZOE or CH pastes. All teeth were restored with stainless steel crowns. Follow-up examinations were carried out at 1, 3, 6, and 12 months. Results: One tooth in Group 1 and two teeth in Group 4 were extracted because of pain and periapial pathosis at sixth month. After 12 months, clinical success rates of pulpotomies in Groups 1-4 were 95%, 100%, 100%, and 89.5%, respectively. The differences were not significant (P = 0.548). Radiographic success rates for Groups 1-4 were 80%, 88.9%, 78.9%, and 84.2%, respectively. No statistically significant difference was found (P = 0.968). Pain on percussion was the most observed clinical finding. However, internal root resorption was the most common radiological finding and it was observed significantly more in mandibular primary molars (P < 0.05). Conclusion: Both ZOE and CH can be preferred as base materials after hemostasis achieved by the use of 15.5% FS or 1.25% NaOCl in primary tooth pulpotomy.
Clinical and Radiographic Evaluation of Diode and Er,Cr:YSGG Lasers as an Alternative to Formocresol and Sodium Hypochlorite for Pulpotomy Techniques in Primary Molars: A Randomized Controlled Clinical Trial

Abstract:Background Pulpotomy treatment is one of the vital pulp therapies that can play a major role in the preservation of primary teeth until their natural exfoliation. The objective of this current clinical trial was to assess the clinical and radiographical success of diode and Er,Cr:YSGG lasers as a viable alternative to formocresol (FC) and sodium hypochlorite in the primary molar pulpotomies. Materials and methods Sixty primary molars were selected and randomly allocated to four groups. All treatment groups followed the same clinical protocol, except for the techniques used for hemostasis of the pulpotomy sites. In group A, hemostasis was achieved by applying a 1:5 dilution of FC solution, whereas in group B, 3% sodium hypochlorite was applied to achieve hemostasis. In group C, exposure to a diode laser of 940 nm was performed, whereas for group D, erbium laser irradiation with Er,Cr:YSGG laser of 2,780 nm was employed to achieve hemostasis. Radicular pulp stamps were covered with a 2 mm layer of mineral trioxide aggregate (MTA) paste. Stainless steel crowns were utilized for the final restorations of the primary teeth. The clinical and radiographic outcomes were evaluated at the six- and 12-month follow-up intervals. The investigation was registered with the ClinicalTrials.gov Protocol and Registration System (ID: NCT06002646). Results The overall clinical and radiographic success rates of pulpotomy were 92.3% for FC, 89% for sodium hypochlorite, 98.3% for a diode laser, and 98.7% for Er,Cr:YSGG lasers. There were no statistically significant differences among the four groups (p > 0.05). Conclusions Both the diode and Er,Cr:YSGG lasers showed outcomes comparable to those of FC and sodium hypochlorite. Therefore, they could be promising alternatives to primary tooth pulpotomies.

Abstract:Minute-by-minute heart rate (HR) recordings over a period of 24 h were obtained once for 30 elderly subjects diagnosed as having senile dementia of Alzheimer type (SDAT). Twenty-one of the subjects were studied in a hospital ward setting, and nine were studied in their own homes. Twelve were men and 18 were women. Eleven took some form of sedative medication; 10 took no medication. Thirty-minute mean values were unmasked to take account of the effects of activity and sleep on HR. Results indicate that the masked HR circadian rhythm of SDAT may be more often unimodal than that of normal subjects of similar age, and that phase shift of the endogenous, clock-mediated component of the rhythm (with higher HR at night) is to be expected in a proportion of individuals with SDAT.
Comparative efficacy of medicaments or techniques for pulpotomy of primary molars: a network meta-analysis

Abstract:Objectives: We performed this network meta-analysis to determine the comparative efficacy of formocresol (FC), ferric sulfate (FS), sodium hypochlorite (NaOCl), calcium hydroxide (CH), mineral trioxide aggregate (MTA), biodentine, and laser for pulpotomy of molar teeth. Materials and methods: An updated search was conducted in PubMed, Embase, and the Cochrane Library to identify relevant randomized controlled trials (RCTs) published before October 30, 2022, after screening previous meta-analyses. The Cochrane risk of bias assessment tool was used to appraise the methodological quality of included studies. Clinical and radiographic success rates were assessed as outcomes. Random network meta-analysis was performed by using STATA software (version 14.0) with "network" command. Results: A total of 43 RCTs were included. Network meta-analysis indicated that CH was inferior to other medicaments and techniques in all outcomes, and MTA and biodentine was better than FC, FS, and NaOCl in terms of clinical and radiographic success rates. Results of ranking probabilities suggested that MTA ranked first in all outcomes except for clinical success at both 6 months. Conclusions: Our results suggested that MTA was associated with significant improvement in both clinical and radiographic success than other pulpotomy medicaments and techniques, with the highest probability of being the optimal option. Clinical relevance: The current network meta-analysis determined the comparative efficacy and safety of 7 common pulpotomy medicaments in molar pulpotomy, including FC, FS, NaOCl, CH, MTA, biodentine, and laser, and the pooled results revealed comparable efficacy in clinical and radiographic success rates at 6 and 12 months between FC, FS, and NaOCl in primary molars pulpotomies. However, MTA, biodentine and laser may have more advantages than other pulpotomy medicaments for clinical and radiographic success. Therefore, in clinical practice, practitioners should select MTA, biodentine, or laser as pulpotomy medicaments in molar pulpotomy.
[Two-Year Outcomes of Pulpotomy in Primary Molars Using Mineral Trioxide Aggregate: a Retrospective Study].

Abstract:OBJECTIVE:To evaluate the two-year outcomes of primary molars pulpotomy using mineral trioxided aggregate (MTA) and to find out the potential influence factor, with the help of electronic medical record database.METHODS:Children who received primary molars pulpotomy in the Department of Pediatric Dentistry in Peking University School and Hospital of Stomatology from May, 2014 to November, 2015 were searched in the Electronic Medical Record Database of Peking University School and Hospital of Stomatology, via the database for scientific research provided by the corporation of Kaientai. The children who were healthy, no more than 8 years old and followed up over 1.5 years were selected as the subjects of this study. At the same time, those children who didn't have complete medical records and X-rays before and after treatment were removed. Basic information, the relevant medical records and radiographic records of those children were collected. All molars were examined clinically and radiographically, and classified into 1 of 5 outcomes: N, H, P0, PX, PY. Molars classified into P0, PX and PY were regarded as failed. Survival analysis was applied. The survival rate and survival time of the deciduous teeth were calculated.Multivariate analysis was performed by using Cox proportional hazard model.RESULTS:One hundred and fifteen children were finally included,aged from 2.6 to 8.2 years, with the mean age of (4.5±1.1) years. 211 primary molars were included, and the average follow up time was (880±154) days. A good level of agreement between the raters was found for molars with five outcomes(κ=0.913). Intrarater reliability was good for molars with five outcomes (κ=0.916). Forty-nine molars failed by September, 2017. Forty-three molars had abnormal radiographic manifestation. Six molars suffered premature loss. Only fourteen molars had an associated gingival swelling or parulis, or pathologic mobility upon clinical examination. The cumulative survival probability of half a year, one year, one year and a half, two years, two year and a half for the pulpotomy was 100%, 98.5%, 92.9%, 90.5%,73.8% through the Kaplan-Meier method, respectively. Through the analysis of Cox proportional hazard model, the survival probability was significantly higher when the age was younger. When sodium hypochlorite and the saline were used as the irrigation at the same time, the survival probability was not improved for those teeth with more bleeding in the operation compared with that used saline only. The teeth with preformed metal crown (PMC) gained longer median survival time than those restored with resin composite and others, but the difference was not significant, either. What's more, gender, the restoration time of PMC, the restoration of the opposite teeth were not found to be associated with the success of the treatment.CONCLUSION:The cumulative survival probability of two years after the primary pulpotomy was 90.5%, complying the current instructions in our department. The survival probability of the molars after the treatment was significantly affected by the age.

Abstract:AimThe aim of this study was to evaluate the scientific evidence of pulpotomy in primary teeth comparing mineral troxide aggregate (MTA), calcium hydroxide, ferric sulphate, and electrosurgery with formocresol.MethodsA systematic search using key words was conducted using seven databases up to December 10, 2013. Clinical articles in English, Portuguese and Spanish were selected, which were in accordance with the inclusion and exclusion criteria and the research objective of comparing whether pulpotomy performed with formocresol in primary teeth is more effective than other medicaments or techniques.ResultsOut of the 12,515 publication initially identified, 30 clinical articles were included in the systematic review and analysed by four meta-analyses. The success rate of MTA (94.6 %) was higher than that of formocresol (87.4 %), with a statistically significant difference (OR = 0.39; 95 % CI = 0.25–0.62). Formocresol pulpotomy success was not statistically different from ferric sulphate or electrosurgery.ConclusionMTA was clinically and radiographically superior to formocresol for pulpotomy of primary teeth. The other alternatives to formocresol such as electrosurgery and ferric sulphate can be used instead of formocresol since they showed success similar to formocresol. In addition, there is no evidence to support calcium hydroxide for pulpotomies in primary teeth.
Cytotoxic Effect of Two Different Concentrations of Sodium Hypochlorite: An In-Vitro Study

Abstract:Introduction The endodontic treatment of primary teeth presents considerable complications due to their distinct anatomical properties. In order to achieve a successful endodontic treatment, certain factors must be assessed. These factors include a precise diagnosis, thorough cleaning, and a reliable disinfection protocol. Although sodium hypochlorite (NaOCl) has been effective as an irrigation agent in primary teeth, it is important to recognize that higher concentrations of NaOCl might possibly inflict toxic harm on the periapical environment if they penetrate the tooth's apical foramina. Since primary teeth are important, pediatric dentists must choose an appropriate NaOCl concentration for root canal irrigation, as higher concentrations can be toxic. Thus, the current investigation examined the cytotoxicity of two different NaOCl concentrations at various volumes. Methods To evaluate the cytotoxicity potential, a culture of nauplii (brine shrimp) was prepared and subjected to testing. For the test, 5, 10, 20, and 40 µL of 1% and 3% NaOCl were added to the brine shrimp culture at different concentrations, and saline was used as a control. After a span of 24 hours, the total number of alive nauplii was duly noted. Results After 24 hours, nauplii showed no mortality in the control group. For 1% NaOCl, mortality ranged from 10% to 20% across volumes, with no significant differences (p = 0.193). In contrast, 3% NaOCl caused significantly higher mortality: 20% at 5 µL, 30% at 10 and 20 µL, and 60% at 40 µL (p = 0.007). Tukey's analysis revealed no significant differences for 1% NaOCl (p > 0.05) but significant differences for 3% NaOCl at 40 µL (p < 0.05). Conclusion Based on the results of the present study, it was observed that a 1% NaOCl solution exhibited a lower level of toxicity in comparison to a 3% NaOCl solution. These findings highlight the importance of using lower concentrations of NaOCl for endodontic irrigation in pediatric dentistry to reduce the risk of tissue damage and ensure safer outcomes for young patients.
Comparative evaluation of the efficacy of 1% and 3% Sodium hypochlorite in reducing the microbial counts in primary teeth root canals using Bioluminometer - A randomized clinical trial

Abstract:Purpose: Sodium hypochlorite (NaOCl) is commonly used to irrigate primary teeth during pulpectomy. Although high concentrations of NaOCl are effective, they pose a risk of toxic damage to periapical tissues if NaOCl penetrates through the apical foramen. Therefore, low concentrations of NaOCl are preferred to mitigate this risk. However, concerns persist regarding the antibacterial efficacy of low concentrations of NaOCl compared to high concentrations. The objective of this study was to assess and compare the efficacy of 1% and 3% NaOCl irrigation in reducing bacterial load within primary teeth root canals. Materials and methods: This clinical study involved forty participants divided into two groups. Group 1 (n = 20) received canal irrigation with 1 % NaOCl solution, while Group 2 (n = 20) received canal irrigation with 3 % NaOCl solution. Microbial samples were collected from the root canal using a paper point before and after irrigation. The samples were aseptically transferred to ultra-snap tubes and then analyzed using a Bioluminometer. The results were recorded. Results: Both groups exhibited a decrease in bacterial count after irrigation. The mean colony count post irrigation for 3 % NaOCl was 258.05 ± 28.61, and for 1 % NaOCl it was 267.60 ± 30.56. However, no statistically significant difference was observed upon intergroup comparison. Conclusion: This study shows that 1% NaOCl is equally effective as 3% NaOCl in reducing bacterial count in root canals. Thus, using 1% NaOCl as an irrigant is appropriate in clinical practice.
Mineral Trioxide Aggregate Versus Calcium Hydroxide For Pulpotomy In Primary Molars

Abstract:Objective: To compare the effects mineral trioxide aggregate (MTA) and calcium hydroxide (CH) for pulpotomy in primary molars.Methods: A randomised, bilateral selkontrolled clinical trial was designed to compare the clinical effect of MTA and CH in pulpotomies in primary molars in 4- to 9-year-old children. Children with two similar-sized cavities' on bilateral primary molar counterparts requiring pulpotomies were included. The two contralateral molars in each patient were randomly assigned to MTA or CH treatment. Clinical and radiographic examinations were performed to evaluate the treatment results at post-treatment recall.Results: Seventeen pairs of self-controlled contralateral teeth were available for follow-up evaluations. The success rate of MTA was 94.1% (16/17), while the success rate of CII was 64.7% (11/17). Internal root resorption was the most frequent reason for failure in the CH group. Crown discolouration was common in the MT/1-treated group.Conclusion: 114Tilwas more successful than CH for pulpotomies in primary molar teeth, and may be a suitable replacement for CH in primary molar pulpotomies.
The success of emergency pulpotomies in primary molars

Abstract:Purpose: The purpose of this retrospective study was to assess the success of pulpotomies performed on an emergency basis and restored with a temporary zinc-oxide eugenol restorative material. Methods: Records were collected from 216 pulpotomies in primary molars performed on an emergency basis by pediatric dental residents at the University of Florida pediatric dental clinic between July 1999 and June 2001. The selection criteria include teeth with a positive history of pain but with absence of clinical and radiographic signs of pulp degeneration. Formocresol was the medicament used and the teeth were restored temporarily with a reinforced zinc-oxide eugenol material. Treatment success or failure was determined, at the definitive restorative appointment, by the presence or absence of clinical and/or radiographic pathology. The time interval between emergency and definitive treatment appointments, tooth-type, arch, age, and gender were the variables analyzed in this study. Chi-square analysis was used to assess the effect of the different variables on the outcome of the treatment. Results: Sixty-four teeth were available for assessment in the two-year period. A 53% success rate was observed, if the tooth was evaluated and restored with the definitive restoration, within the first 90 days post-emergency treatment. A 31% success rate was observed after 365 days. These results were statistically significant (P=0.016). Patients younger than six years old showed statistically significant higher chances for success than older children did (P=0.018). No significant differences were found for gender, type of tooth, or arch. Conclusions: The low success rate found during the first three months post emergency treatment may be attributed to undiagnosed, subclinical inflamed pulp, while long-term failure may be associated with microleakage of the temporary restorative material.

Abstract:Introduction: The study was performed to evaluate and compare the clinical and radiographic efficacy of chlorhexidine (CHX) polymer scaffold, 3Mixtatin, and formocresol for vital primary pulp therapy-a randomized clinical study.Materials and methods: A total of 120 primary molars were included from children aged between 6 and 8 years in this randomized clinical study based on inclusion and exclusion criteria and were randomly allocated into three groups (group I-CHX polymer scaffold, group II-3Mixtatin, and group III-formocresol. Pulpotomy was performed in a vital cariously exposed primary tooth with healthy periodontium where their retention is more beneficial than extraction. Subjects were followed up at 1, 3, and 6 months for clinical and radiographic evaluations.Results: At 6 months of follow-up, the overall success rate of pulpotomy in groups I, II, and III was 56.41, 71.05, and 60.52% in each group, respectively. Nonsignificant difference (<i>p</i> > 0.05) was seen during intergroup comparison.Conclusion: However, among the three materials used in this study, 3mixtatin comparatively had better results.How to cite this article: Goel N. Clinical and radiographic success of TheraCal versus Formocresol in primary teeth pulpotomy: A systematic review and meta-analysis

Abstract:Background: Various dressing materials have been evaluated for pulpotomy of primary teeth. However, an ideal pulp dressing material has not been identified yet. This systematic review investigated the effectiveness of TheraCal compared to Formocresol in pulpotomy of primary teeth. Materials and methods: This research was conducted in the form of a secondary study, with a systematic search of texts until 2023 in Scopus, Web of Science, PubMed and Google Scholar databases. The articles were selected based on the inclusion and exclusion criteria and, finally the search results were reported in the PRISMA chart. The quality of the studies was evaluated based on the NIH checklist. The extracted information was entered into Stata17 software. Heterogeneity was evaluated using Cochran's chi-square test and I2 statistics. Egger's tests were used to detect publication bias. Results: After removing duplicate articles and articles that did not meet the inclusion criteria, 4 studies were selected for qualitative analysis. The odds' ratio of success rate between Formocresol and TheraCal pulpotomy for absence of the pain, abscess, Mobility, internal root resorption and bone radiolucency was obtained 1.12 (95 % CI: 0.32, 3.85, P = 0.86), 0.47 (95 % CI: 0.1, 2.14, P = 0.33), 0.82 (95 % CI: 0.21, 3.21, P = 0.78), 0.89 (95 % CI: 0.3, 2.67, P = 0.84), and 1.96 (95 % CI: 0.68, 5.62, P = 0.21) respectively. Conclusion: The study results revealed that there was no significant difference in clinical and radiographic success between pulpotomy with TheraCal and Formocresol.
Longitudinal Study Comparing Pulpectomy and Pulpotomy Treatments for Primary Molars of Alaska Native Children

Abstract:Purpose: The purpose of this study was to determine which pulp treatment technique, performed at the Alaska Native Medical Center, is most successful by comparing failure rates of primary molars treated with ferric sulfate (FS) pulpotomy, sodium hypochlorite (SH) pulpotomy or pulpectomy between January 2005 and January 2016. Methods: All data were abstracted from the dental records of Alaska Native children aged 2-13 years, retrospectively. Clinical and radiographic failures up to five years after treatment were assessed. Cox proportional hazards models using random effects to account for correlated failure time were adjusted for age, sex, molar tooth position, and visit type. A total of 1,149 procedures in 830 children met the inclusion criteria: 490 pulpectomies, 111 SH Pulpotomies and 548 FS Pulpotomies. Results: Teeth treated with FS pulpotomy had 3.7 times higher risk of radiographic failure (adjusted hazard ratio [aHR]=3.73, 95% confidence interval [CI]= 2.25-6.16), and teeth treated with SH pulpotomy had 2.5 times higher risk of failure (aHR=2.57, 95% CI= 1.17-5.64) than those treated with pulpectomy. Conclusions: The findings from this large cohort study suggest that molar teeth treated with pulpectomies survive significantly longer than those treated with either FS or SH pulpotomies. Materials and methods: In this present study, pulpotomies were performed on 48 primary molars in 30 children aged between 4 and 8 years (12 teeth in each group). Following the pulpotomy procedure, teeth were evaluated clinically and radiographically at 1st, 3rd, 6th, and 9 monthly intervals. Results: After 9 months of follow-up, the clinical success was 91.6% for AMD and C-MTA and 83.3% for DL and FS. Radiographic success was 91.6, 91.6, 75, and 83.3% for AMD, C-MTA, DL, and FS groups, respectively. There is no statistically significant difference between the four groups (p > 0.05). Interpretation and conclusion: Results of our study showed that both AMD and C-MTA were equally successful compared to traditional agents like laser and ferric sulfate as pulpotomy agents. Clinical significance: Amniotic membrane derivative (AMD) and C-MTA are alternative biomimetic pulpotomy agents that can be used in pediatric primary tooth pulpotomies. Abstract:The present study aimed to compare the clinical and radiographic success rates of Formocresol (FC), Portland Cement (PC), and NeoMTA Plus in primary molar pulpotomy. Ninety-six primary molars in ninety-six children aged 3‒9 years were randomly assigned to FC, PC, and NeoMTA Plus groups. Radiographic and clinical follow-up evaluations were performed at six and twelve months. After six and twelve months, clinical success rates were 100% in the three groups. The radiographic success rates in the 6-month follow-up were 96.7 and 96.7% in the FC and PC groups and 100% in the NeoMTA Plus group. At 12 months, radiographic success rate was 93.8, and 93.8% in the FC and PC groups and 96.9% in the NeoMTA Plus group. There were no significant differences between the three groups at all follow-up evaluations. NeoMTA Plus showed highest success rate, clinically and radiographically. PC and NeoMTA Plus can be considered as an alternative to FC.

Abstract:The major nuclear medicine contribution to the pulmonary imaging literature for the past year was the inaugural report from the Prospective Investigation of Pulmonary Embolism Diagnosis study. Investigations focusing on the role of thrombolytic agents for the management of pulmonary thrombosis have advanced to the stage in which different therapeutic regimens are being tested along with new agents that continue to emerge. Ongoing research and clinical trials are evaluating newer radiolabeled aerosols in patients with airways and interstitial lung disease using imaging and clearance measurements. Positron-emission tomography continues to be utilized in a few research centers for tumor localization and characterization.
Chemical Removal of Necrotic Periodontal Ligament on Delayed Replanted Teeth by Sodium Hypochlorite: Morphological Analysis and Microhardness Indentation Test of Cementum

Abstract:AIM:To compare the efficacy of sodium hypochlorite (NaOCl) used at different concentrations and working times for removing necrotic periodontal ligament (PDL) from delayed replanted teeth and to observe the effects of NaOCl on surface structure and microhardness of cementum.METHODOLOGY:A total of 88 healthy premolars with a single root extracted for orthodontic purposes were selected and kept dry at room temperature for 1 h. The teeth were divided into 11 groups: group 1 (control): roots were untreated; group 2: necrotic PDL was removed with gauze; groups 3-11: teeth were immersed in NaOCl at different concentrations (1, 2.5 and 5.25%) and for different working times (5, 10 and 15 min). The specimens in each group were inspected separately for cementum integrity and the presence of PDL remnants by histomorphometric analysis, confocal laser scanning microscopy (CLSM) and scanning electron microscopy (SEM). Another 14 healthy premolars with roots divided into two pieces were selected for Vickers microhardness indentation tests before and after NaOCl treatment. The data were analysed statistically using Wilcoxon signed-rank test of two-related samples (P = 0.05).RESULTS:In teeth treated with 1% NaOCl for 15 min or 5.25% NaOCl for 5 min, the cementum remained morphologically intact without cracks, and PDL remnants were absent. In the 1% NaOCl for 15 min group, the microstructure of cementum was arranged more regularly, as observed ×8000 magnification by SEM. Teeth in each of the other groups displayed cementum damage and/or the presence of PDL remnants. Microhardness tests revealed that treatment with 1% NaOCl for 15 min or 5.25% NaOCl for 5 min significantly decreased microhardness of root cementum (P < 0.05).CONCLUSIONS:Use of either 1% NaOCl for 15 min or 5.25% NaOCl for 5 min was effective at removing necrotic PDL from the delayed replanted teeth whilst having a minimal influence on cementum integrity. However, 1% NaOCl for 15 min was less damaging to cementum.
Comparative Evaluation of Success of NeoPutty Mineral Trioxide Aggregate® with Formocresol as Pulpotomy Medicaments in Primary Molars: A Clinical Study

Abstract:Objective: This clinical study was aimed to investigate the clinical and radiographic outcomes of NeoPutty MTA® (Nusmile), a novel mineral trioxide aggregate (MTA) pulpotomy material, in primary teeth of children aged 4-6 years. Methods: An in vivo study was done in children aged 4-6 years old. A total of 50 primary second molar teeth were included in the study, with 25 of them undergoing treatment with formocresol (FC) and 25 receiving treatment with NeoPutty MTA®. The subjects were divided into two groups for treatment: the first group received a treatment involving a 1:5 dilution of Buckley's FC. The second group, however, underwent treatment with NeoPutty MTA®. This study employs a meticulous approach to evaluate the impact of different treatments on a significant number of molar teeth, using FC and NeoPutty MTA® as the respective treatment methods. The demographics of the subject pool are well-documented, comprising a balance of male and female participants. The technique employed for pulp removal and the subsequent management of bleeding are outlined, underscoring the thoroughness of the process. Results: At 12 months, the NeoPutty MTA® pulpotomy group showed a significantly higher success rate clinically (96%) compared to the FC group (72%) (P < 0.05), but radiographically although NeoPutty MTA® was better (92%) than FC (82%), it was not statistically significant. NeoPutty®-treated teeth exhibited no signs of pain or discomfort and demonstrated better pulp vitality compared to the FC group. Conclusion: NeoPutty MTA® demonstrated superior clinical success rates and better radiographic outcomes compared to traditional FC pulpotomy.

Abstract:Aim: To evaluate the 6-year outcome of root canal treatment irrigated with 0.5% or 3% sodium hypochlorite (NaOCl). Methodology: The baseline trial was designed as a quasi-randomized clinical trial. Patients referred for root canal treatment to an endodontic specialist clinic were recruited to the study (n = 298). The concentration of NaOCl was allocated quasi-randomized to 271 subjects (0.5% [n = 139], 3% [n = 132]). Bacterial sampling was performed immediately before root canal filling. Samples were cultured and evaluated as growth or no growth. Patients were invited to a clinical and radiological follow-up >5 years postoperatively. The clinical outcome measurements were tooth survival, cumulative incidence of endodontic retreatments, patients' assessment of pain, clinical findings and radiological signs of apical periodontitis (AP). Results: Tooth survival was 85.6% in the 0.5% NaOCl group and 81.1% in the 3% NaOCl group (p = .45). There was no record of retreatment in 94.4% in the 0.5% NaOCl group and in 92.2% in the 3% NaOCl group (p = .76). The percentage of asymptomatic cases were 87.8% in the 0.5% group and 85.3% in the 3% NaOCl group (p = .81). Absence of clinical signs of AP was seen in 86.6% in the 0.5% NaOCl group and in 83.6% in the 3% NaOCl group (p = .80). Absence of radiological signs of AP was seen in 74.0% in the 0.5% NaOCl group and 64.1% in the 3% NaOCl group (p = .20). Subjects with positive culture before root filling reported subjective pain with a significantly higher frequency as compared to negative-culture subjects (p = .014). Conclusions: The use of 0.5% or 3% NaOCl for irrigation during root canal treatment resulted in similar clinical outcomes 5-7 years postoperatively. Persisting bacteria immediately before root filling may predict future episodes of subjective pain.
