Abstract:Familial hyperalphalipoproteinemia Is determined by a major gene and is characterized by high levels of high density lipoprotein cholesterol and longevity. To describe the plasma llpoprotelns in this condition more completely, a kindred consisting of the proband, her affected father, and her two affected brothers was studied. Fasting plasmas were analyzed for lipoprotein lipids by combined preparative ultracentrlfugal and precipitation methods. Levels of apollpoprotein A-l and apolipoproteln A-ll, the major apoproteins of high density lipoproteins, were assayed by radlolmmunoassay. The flotation properties of very low density, low density and high density lipoprotein were determined by zonal ultracentrlfugatlon, and the Isolated high density lipoprotein subtractions were characterized according to their Ilpid and apoprotein compositions. Total cholesterol of all subjects was normal, but trlglycerides were elevated (above the 90th percentile) In the two brothers. High density lipoprotein cholesterol ranged from 150 to 165 mg/dl In the proband and from 72 to 89 mg/dl In her relatives. Apollpoprotein A-l was 318 mg/dl in the proband and 156 to 169 mg/dl In the siblings; respective apolipoproteln A-ll values were 67 and 82 to 83 mg/dl. The high density lipoprotein cholesterol and apollpoprotein A-l and A-ll values for the relatives are above the 95th percentile for sex and age, while the levels of the proband are the highest recorded in our laboratories. On zonal ultracentrlfugatlon, very low density lipoprotein and low density lipoprotein had normal flotation properties. High density lipoprotein was divisible Into three populations, HDL2 HDL3L, and HDL3D (St(1.2t.26°C), 5.9,2.9, and 1.7 respectively), each of which floated in its usual position. But HDL2 was grossly elevated, accounting for most of the rise In the high density lipoprotein fraction. The compositions of the high density lipoprotein fractions with respect to the major Ilpid and apoprotein classes did not differ from normal. Thus, In this kindred, high density lipoprotein was quantitatively rather than qualitatively unusual. It Is of interest that hypertrlglyceridemla and hyperalphalipoproteinemia coexisted in the siblings. These concurrent elevations differ from the expected reciprocal relationship between high density lipoprotein and very low density lipoprotein levels In plasma, and suggests that In some subjects the two abnormalities may be Independently transmitted.
Interaction levels between comfort and discomfort in aircraft seats

Abstract:Background: Although comfort and discomfort on seats have been widely investigated, their nature is still not well described by literature and it is not known exactly how the interaction between these two phenomena happens. Objective: To investigate the multidimensional nature of comfort and discomfort in aircraft seats as well as their levels of interaction. Methods: A comfort-discomfort questionnaire has been given to 1500 passengers in Brazil. Results: The results brought evidence that comfort and discomfort are different phenomena with a strong negative correlation. At extreme levels, the presence of comfort implies the absence of discomfort; however at the other levels of comfort (minimum to moderate) there is the presence of discomfort. Estimating the scores given by the passengers has allowed them to be placed at scales of comfort and discomfort with their different levels. However, no passenger has achieved the levels of maximum comfort or of maximum discomfort. Conclusions: It is possible to infer that comfort, as much as discomfort are multidimensional phenomena and must be evaluated through scales with different levels, since at the extreme levels there is no interaction between the phenomena, but at the intermediate levels the passengers could perceive comfort and discomfort at the same time.
Design and analysis of comparative experiments to assess the (dis-)comfort of aircraft seating

Abstract:This paper focuses on the comparative assessment of comfort and discomfort (hereafter, (dis-)comfort) for aircraft seating. Subjective and objective data of seating (dis-)comfort were collected during an experiment involving 20 volunteers who tested 3 aircraft double-seats in upright and reclined position. In order to minimize experimental uncertainty due to well-known noise factors (i.e. patterns of discomfort during the work week and during the work day, order of evaluation, inter-individual differences), experimental trials were performed according to a crossover design. Statistical data analysis aimed mainly at investigating (dis-)comfort differences across seat conditions; gender-based differences in perceived discomfort on different body parts; effect of sitting duration on perceived discomfort on different body parts. The experimental results show that differences across seat conditions impacted differently on perceived discomfort depending on gender, body parts and sitting duration. No significant differences in perceived discomfort across gender were evident for the lightweight seat in both upright and reclined positions. On the contrary, for both baseline configurations, perceived discomfort at head and neck areas was higher for males than for females. For all seat conditions, participants experienced a significant worsening of perceived comfort over time at shoulders, back, sacrum and thighs and, in addition, at upper body area (i.e. neck, arm and forearm) and knees only for seats in reclined position.

Abstract:The present work investigates the contact forces between sitters and seat as well as their correlations with perceived discomfort. Twelve different economy class aeroplane seat configurations were simulated using a multi-adjustable experimental seat by varying seat pan and backrest angles, as well as seat pan compressed surface. Eighteen males and 18 females, selected by their body mass index and stature, tested these configurations for two sitting postures. Perceived discomfort was significantly affected by seat parameters and posture and correlated both with normal force distribution on the seat-pan surface and with normal forces at the lumbar and head supports. Lower discomfort ratings were obtained for more evenly distributed normal forces on the seat pan. Shear force at the seat pan surface was at its lowest when sitters were allowed to self-select their seat-pan angle, supporting that a shear force should be reduced but not zeroed to improve seating comfort. Practitioner Summary: The effects of seat-pan and backrest angle, anthropometric dimensions and sitting posture on contact forces and perceived discomfort were investigated using a multi-adjustable experimental seat. In addition to preferred seat profile parameters, the present work provides quantitative guidelines on contact force requirement for improving seating comfort.

Abstract:Perilymphatic fistula (PLF) is considered to be a most challenging otologic issue. There are no currently agreed upon objective tests for the diagnosis of PLF. In an effort to improve diagnostic accuracy in patients with suspected PLF, a novel diagnostic test involving magnetic resonance (MR) contrast imaging was designed. An experimental PLF was created in the cochlear round window membrane of healthy adult cats. Since cochlear perilymph is thought to be an ultrafiltrate of cerebral spinal fluid, gadodiamide (gadolinium-diethylenetriamine penta-acetic acid [DTPA] bismethylamide), a nonionic paramagnetic contrast agent, was injected intrathecally in an attempt to enhance imaging of the created fistula. Post-contrast images of the fistualized cochlea demonstrate a significant increase in the signal intensity of the cochlear perilymph with pooling of enhanced perilymph observed in the ipsilateral mastoid bulla. Magnetic resonance contrast imaging may prove to be a valuable technique in human studies involving perilymphatic fistula.
The influence of activities and duration on comfort and discomfort development in time of aircraft passengers

Abstract:Background: Discomfort increases when duration of sitting is longer. Aircraft passengers are subjected to prolonged sitting in restricted postures, which can cause discomfort. Objective: To examine the relationship between duration of sitting and development of discomfort, as well as the influence of different passenger activities. Methods: Participants (n = 18) performed four different activities in three conditions during an aircraft seat test. Discomfort was measured every 15 min on an 11-point scale. A short online survey asked passengers (n = 114) when they felt most refreshed during their last flight. Results: Discomfort was significantly lower during the 'food' activity for condition 2 (hot meal), compared to conditions 1 and 3 (drinks, snack). The largest decrease in discomfort was during the 15 min break between two conditions, in which participants could walk around. Respondents from the online survey indicated they felt most refreshed after food (34.8%), sleeping (27.0%) and walking through the plane (25.2%). Walking was mentioned more often as most refreshing activity by long-haul passengers (>6 h) compared to short-haul passengers (<2 h). Conclusions: Passengers can be distracted from feeling discomfort by providing food and drinks. Airlines and seat manufacturers can increase comfort by stimulating passengers to move in their seat.

Abstract:Sitting still for extended periods of time can lead to physical discomfort and even serious health risks. Due to safety regulations, reducing passenger' sitting time in aircrafts is not feasible. This paper presents the results of a laboratory study, in where an interactive airplane seat was compared with a current economy class seat. Participants used both seats for 3.5 h, and performed significantly more in-seat movements when using the interactive seating system. Furthermore, this interactive seat predominantly lead to significantly better comfort experiences and reduced discomfort experiences, however no significant differences have been found in self-reported localized musculoskeletal discomfort. Passengers indicated that they would prefer this interactive seat over a standard aircraft seat.

Abstract:Musculoskeletal symptoms are a major occupational health problem in workers, and these can affect all professional occupations. Previous studies have proposed metrics capable of evaluating the musculoskeletal discomfort experienced by workers. However, no study has developed a metric that considers professional groups. Thus, this study aimed to develop a scale for musculoskeletal discomfort in the lower limbs to compare self-reported symptoms among education, health, and industry professionals. The sample included 159 teachers, 167 health professionals, and 401 industrial operators who relayed their symptoms using a diagram of the hips, thighs, knees, lower legs, and feet. Factor and multigroup item response theory analyses were used to construct a musculoskeletal discomfort scale consisting of seven levels and to assess and compare the identified symptoms. The results showed that the progressive evolution of discomfort differed for each profession, demonstrating that each context and work environment affects workers differently, which may explain the different patterns of symptom responses among professional groups.

Abstract:OBJECTIVE This study was designed to explore the relationship between intramuscular cysts and rotator cuff tendon tears. CONCLUSION Intramuscular cysts are strongly associated with rotator cuff tendon tears. Identification of such a cyst should prompt a search for a rotator cuff tear. Findings on MR arthrography and surgery suggest that a delaminating component of the rotator cuff tear may lead to the development of these cysts and may explain the occasional discrepancy between location of tears and location of cysts.
Airport service quality perception and flight delays: examining the influence of psychosituational latent traits of respondents in passenger satisfaction surveys

Abstract:The service quality of a passenger transport operator can be measured through face-to-face surveys at the terminals or on board. However, the resulting responses may suffer from the influence of the intrinsic aspects of the respondent's personality and emotional context at the time of the interview. This study proposes a methodology to generate and select control variables for these latent psychosituational traits, thus mitigating the risk of omitted variable bias. We developed an econometric model of the determinants of passenger satisfaction in a survey conducted at the largest airport in Latin America, São Paulo GRU Airport. Our focus was on the role of flight delays in the perception of quality. The results of this study confirm the existence of a relationship between flight delays and the global satisfaction of passengers with airports. In addition, favorable evaluations regarding airports' food/beverage concessions and Wi-Fi services, but not their retail options, have a relevant moderating effect on that relationship. Furthermore, dissatisfaction arising from passengers' interaction with the airline can have negative spillover effects on their satisfaction with the airport. We also found evidence of blame-attribution behavior, in which only delays of internal origin, such as failures in flight management, are significant, indicating that passengers overlook weather-related flight delays. Finally, the results suggest that an empirical specification that does not consider the latent psychosituational traits of passengers produces a relevant overestimation of the absolute effect of flight delays on passenger satisfaction.

Abstract:Background Anxiety is a frequent comorbidity in patients with Chronic Obstructive Pulmonary Disease (COPD) and with important repercussions, such as functional impairment, decreased quality of life and adherence to treatment, and increased risk of hospitalization. In this context, the Anxiety Inventory for Respiratory Diseases (AIR), the only instrument specifically developed to investigate anxiety symptoms in this population, was cross-culturally adapted for use in Brazil. Still, the measurement properties of the face-to-face version have not yet been investigated. Objectives To investigate the internal consistency, test-retest and inter-rater reliability, convergent and divergent validity of the face-to-face AIR in patients with COPD. Methods This is a cross-sectional methodological study. On the first day, after signing the Free and Informed Consent Form, the Mini-Mental State Examination, the sociodemographic and clinical questionnaire were applied, and anthropometric data were collected, followed by spirometry. Subsequently, patients answered the Modified Medical Research Council (mMRC), the COPD Assessment Test (CAT), the AIR, the Hospital Anxiety and Depression Scale (HADS), the Beck Anxiety Inventory (BAI), and the London Chest Activity of Daily Living (LCADL). To determine the test-retest and inter-rater reliability of the IAR, seven days after the first application of the questionnaire, the leading researcher reapplied the scale with a control form via telephone contact, and a second rater contacted patients 48 hours later. The internal consistency of the AIR was measured using Cronbach's alpha and reliability using the Intraclass Correlation Coefficient (ICC). Spearman's correlation test (rho) was used to determine validity (p<0.05). Results Twelve individuals [7 female, age 60.5 (min. 53.7-max.70.8) years] participated in the study; nine were included in the test-retest reliability analyses and eight in the inter-rater reliability. For internal consistency, Cronbach's α was 0.94. For test-retest reliability, the ICC was 0.73; for inter-rater reliability, it was 0.88. Regarding convergent validity, significant correlations of high magnitude were observed between the AIR and the HADS anxiety domain (rho = 0.82), depression domain (rho = 0.87), and the total score (rho = 0.79), and of moderate magnitude between the AIR and the BAI (rho = 0.70). For divergent validity, non-significant correlations of weak magnitude were observed between AIR and LCADL (rho = 0.39), AIR and CAT (rho = 0.10), and AIR and MRC (rho = 0. 17). Conclusion The study's preliminary results indicate that the AIR demonstrates adequate measurement properties for assessing anxiety symptoms in patients with COPD. The study will be continued to expand the sample number. Implications This study will provide a measurement instrument for investigating anxiety symptoms in patients with COPD with appropriate measurement properties, which may facilitate early identification and proper treatment.

An Investigation of Key Factors Influencing Aircraft Comfort Experience

Abstract:Comfort has been an increasingly important issue in air travel. Passenger comfort is clearly a key variable in research on user acceptance of transportation systems, and it is related to passenger’s satisfaction and the willingness to fly again. After realizing its importance in capturing and retaining customers, a growing number of airlines are focusing on comfort. How to improve the experience of passenger comfort has become a major strategic goal for the airline management. This paper regards the passenger comfort in the aircraft cabin during the flight as an experience shaped by interaction between individual and the flight contextual situation. The aim of this study is prioritizing the factors affecting the passenger comfort experience, and sorts these factors into different categories. Firstly, the paper undertakes a systematic literature review to distinguish key critical factors affecting the aircraft passenger comfort, then classifies the factors into five categories by Kano model. At last, the managerial implication is given based on the result. The findings of this research provide important implications for the designers and airline managers and help them better understand the passenger comfort experience. The result can be used as a reference of symmetrically designing the aircraft interior and reasonably allocating the human and material resource.
Translation, Transcultural Adaptation, and Validation of the Empathy, Spirituality, and Wellness in Medicine Scale to the Brazilian Portuguese Language

Abstract:Construct: The Empathy, Spirituality, and Wellness in Medicine Scale (ESWIM) is a 43-item multidimensional scale developed to investigate different dimensions of physicians and medical students. Background: Medical education research requires the use of several different instruments with dozens of items that evaluate each construct separately, making their application slow and increasing the likelihood of students providing a large number of incomplete or missing responses. To provide an alternative measure, this study aims to translate, adapt, and validate the multidimensional ESWIM instrument for Brazilian medical students. This is a very promising instrument because it is multidimensional, relatively short, and cost free; it evaluates important constructs; and it has been explicitly designed for use in the medical context. Approach: The English-language instrument was translated and adapted into the Brazilian Portuguese language using standard procedures: translation, transcultural adaptation, and back-translation. ESWIM was administered to students in all years of the medical curriculum. A retest was given 45 days later to evaluate reliability. To assess validity, the questionnaire also included sociodemographic data, the Duke Religion Index, the Empathy Inventory, the brief version of the World Health Organization Quality of Life (WHOQOL-Bref), and the Oldenburg Burnout Inventory. Results: A total of 776 medical students (M age = 22.34 years, SD = 3.11) were assessed. The Brazilian Portuguese version of ESWIM showed good internal consistency for the factor of Empathy (α = 0.79-0.81) and borderline internal consistency for the other factors: Openness to Spirituality (α = 0.61-0.66), Wellness (α = 0.57-0.68), and Tolerance (α = 0.56-0.65). The principal component analysis revealed a four-factor structure; however, the confirmatory factor analysis showed a better fit for a three-factor structure. We found a significant positive correlation between ESWIM empathy and empathy measured by the Empathy Inventory (r = .444, p < .01), as well as negative correlations between ESWIM empathy and burnout (r = -.145 to -.224, p < .01). ESWIM openness to spirituality was also significantly correlated with different subscales of religiosity (r = .301-.417, p < .01), and ESWIM wellness was significantly correlated with the WHOQOL-Bref factors (r = .390-.673, p < .01). The test-retest reliability (applied to 83 students) was high for all factors except Tolerance. Conclusion: This study provides supportive evidence regarding the reliability and validity of ESWIM empathy scores. The ESWIM scale opens a new field of research in relation to openness to spirituality by introducing a scale that measures this openness attitude. Despite borderline internal consistency, ESWIM wellness was strongly associated with quality of life and had good test-retest reliability. Thus, ESWIM appears to be a valid option for evaluating these constructs in medical students.

Abstract:Aiming at the current situation that being short of the research results of aircraft cabin comfort at home,measures the aircraft cabin environment parameters in 14 flights,and conducts comfort investigation by questionnaire among 1 000 passengers.The results show that the passengers are generally satisfied with the cabin environment,but it still needs to be improved.Noise is the most unsatisfactory factor.Among the factors influencing overall satisfaction,air quality,noise,vibration and seat comfort are comparatively important,while air pressure,airflow and temperature are weaker,and humidity is not significant.

Abstract:Abstract Emotional regulation is a recent construct with few adapted instruments in Brazil capable of measuring it. This study aimed to find evidences of content validity, internal consistency and reliability of the Difficulties in Emotion Regulation Scale (DERS) for the Brazilian context. The participants were 402 adults (78.1% women) aged between 18 to 71 years, of which 86.8% resided in Rio Grande do Sul. We revised an adaptation of the referred instrument to facilitate the understanding of the items. In order to test the most appropriate structure to the Brazilian context, we performed a confirmatory factor analysis, comparing the original instrument structure to that validated to Portuguese. The results showed that DERS had good psychometric properties, exhibiting validity and reliability for the Brazilian sample. The factorial structure was equivalent to the original version, containing six dimensions.
The effect of aircraft seat pitch on comfort

Abstract:This study explores the relationship between seat pitch and comfort, and the influencing factors, like space experience and anthropometric measurements. Two hundred ninety-four participants experienced economy class seats in a Boeing 737 with 28-inch, 30-inch, 32-inch and 34-inch seat pitches. Anthropometric measurements of the participants were measured. Participants completed a questionnaire on comfort (10-scale), discomfort (CP-50) and space experience and the results were analysed using SPSS 25. This study showed a significant relationship between seat pitch and comfort as well as discomfort. Additionally, it was found that the mean rank of discomfort of each pitch size for the middle seat was higher than the window and aisle seat, though seat pitch did affect the (dis)comfort more compared with seat location. It was also found that anthropometric sizes significantly affect the (dis)comfort on smaller pitch sizes, and all space experience questions had a correlation to the pitch sizes.

Abstract:Noise and vibration are two physical factors that can reduce the perceived comfort of passengers during a flight. In this application context, vibrations are often measured using accelerometers placed at the seat rail or with a seat pad between the seat surface and the buttock of a sitting person. To develop pleasant aircraft cabin environments, it necessary to know which measurement location best reflects subjective ratings and whether the application of a frequency weighting as proposed in the ISO 2631 standard is beneficial. In this study, 40 participants rated the pleasantness of sinusoidal whole-body vibrations in the vertical direction while seated on a typical aircraft seat bench and listening to a synthetic broadband aircraft cabin noise over headphones. The vibration signals covered seven frequencies and four vibration levels and ratings were made on a verbally anchored scale. The results showed that an increase in vibration level leads to more unpleasant ratings for each of the tested frequencies, independent of the accelerometer position. Although the ISO frequency weighting underpredicts the ratings for high frequencies in both cases, applying it to the seat rail measurements seems to reflect the pleasantness ratings better than the seat pad measurements proposed in the ISO 2631 standard.

Abstract:With the rapid development of civil aviation industry and the continuous improvement in passenger demand, higher requirements have been put forward for safety and comfort in the design process of civil aircraft. In order to develop a special ergonomic simulation experimental system for civil aircraft cabin seats, from the perspective of "human", this paper comprehensively considers the impact on civil aircraft cabin seats environment on human physiology and psychology, and puts forward an evaluation method of civil aircraft cabin ergonomic simulation experimental system based on passengers' perception of key design features. Based on the principles of ergonomics, the connection between key design features of seat comfort and the requirements of user preference is comprehensively considered, a simulation experimental system based on comfort evaluation is constructed using subjective and objective evaluation methods. Moreover, according to the experimental requirements of the simulation experimental system and the characteristics of seat structure, an ergonomic simulation experimental system for civil aircraft cabin is developed. The final experimental results prove the practicability and validity of the simulation experiment system designed.
