Abstract:For assessing the acute effect of air particulate matter (PM10) pollution on the crowd death of cardiovascular diseases(CVD); using time-stratified case-crossover design, the relation of air absorbable PM10 daily average concentration short-time increase with the CVD daily mortality of 2002～2004 years in Hangzhou City was analyzed, meanwhile, the acute health effect of other gaseous pollutants (SO2 and NO2) was analyzed. After being adjusted for meteorological factors, the single-pollutant models showed that an increase of 10μg/m3 in PM10, SO2 and NO2 2d before was associated with 0.61%(95% CI: 0.28%～0.94%), 1.73%(95% CI: 0.68%～2.79%) and 2.02%(95% CI: 0.85%～3.20%) increase in the CVD mortality respectively. In multi-pollutant models, PM10 did remain the short-term effect though the estimate seemed to be less after being adjusted for SO2, while other co-pollutants had no stastistically significant effect on cardiovascular deaths.
A Time-Series Study for Effects of Ozone on Respiratory Mortality and Cardiovascular Mortality in Nanchang, Jiangxi Province, China

Abstract:Objective: Most evidence comes from studies show that ambient ozone(O 3 ) pollution has become a big issue in China. Few studies have investigated the impact of ozone spatiotemporal patterns on respiratory mortality and cardiovascular mortality in Nanchang city. Thus, this study aimed to explore the health effect of ozone exposure on respiratory mortality and cardiovascular mortality in Nanchang, Jiangxi Province. Methods: Using the daily mortality data, atmospheric routine monitoring data and meteorological data in Nanchang from 2014 to 2020, we performed a generalized additive model (GAM) based on the poisson distribution in which time-series analysis to calculate the risk correlation between respiratory mortality and cardiovascular mortality and ozone exposure level (8h average ozone concentration, O 3 -8h). Besides, analyses were also stratified by season, age and sex. Results: In the single-pollutant model, for every 10 μg/m 3 increase in ozone, respiratory mortality increased 1.04% with 95% confidence interval (CI) between 0.04 and 1.68%, and cardiovascular mortality increased 1.26% (95%CI: 0.68 ~ 1.83%). In the multi-day moving average lag model, the mortality of respiratory diseases and cardiovascular diseases reached a relative risk peak on the cumulative lag5 (1.77%,95%CI: 0.99 ~ 2.57%) and the cumulative lag3 (1.68%,95%CI: 0.93 ~ 2.45%), respectively. The differences were statistically significant ( P < 0.05). Results of the stratified analyses showed the effect value of respiratory mortality in people aged ≥65 years was higher than aged <65 years, whereas the greatest effect of cardiovascular mortality in people aged <65 years than aged ≥65 years. Ozone had a more profound impact on females than males in respiratory diseases and cardiovascular diseases. In winter and spring, ozone had a obvious impact on respiratory mortality, and effects of ozone pollution on cardiovascular mortality were stronger in summer and winter. There was a statistically significant difference of respiratory mortality in winter and spring and of cardiovascular mortality in summer and winter ( P < 0.05). Conclusions: In the long run, the more extreme the pollution of ozone exposure, the higher the health risk of residents' respiratory mortality and cardiovascular mortality. Therefore, the government should play an important role in the prevention and control ways of decreasing and eliminating the ozone pollution to protect the resident's health. The findings provide valuable data for further scientific research and improving environmental policies in Nanchang city.

Association Between Short-Term Exposure to Ambient Air Pollution and Daily Mortality: a Time-Series Study in Eastern China.

Abstract:Few studies were conducted to evaluate health effects of acute exposure to PM2.5 and daily mortality in Asian countries due to lack of large-scale PM2.5 monitoring data. We conducted a time-series study to examine the associations of short-term exposure to four common air pollutants (PM2.5, PM10, NO2, and SO2) and daily mortality in Ningbo, Zhejiang, China. We used generalized addictive model (GAM) to estimate relative risks (RRs) and 95% confidence intervals (CIs) for the association of these four air pollutants with daily mortality. The study included 9365 people in the 2-year study period from 2014 to 2015. SO2 were significantly associated with risk of NAD, RD, and CD mortality with RRs of 1.034 (95% CI 1.004, 1.064), 1.067 (95% CI 1.010, 1.127), and 1.049 (95% CI 1.001, 1.098), respectively.PM2.5 and PM10 were significantly associated with risk of death from NAD mortality in warm season. Similar associations were observed for PM10 (RR = 1.056, 95% CI 1.004, 1.111) and risk of CD mortality. The study provides further evidence that short-term exposure to PM2.5, PM10, NO2, and SO2 are associated with increased risk of daily mortality.

Abstract:Background: In China, the increasing concentration of ozone (O3) has emerged as a significant air pollution issue, leading to adverse effects on public health, particularly the respiratory system. Despite the progress made in managing air pollution in China, it is crucial to address the problem of environmental O3 pollution at present. Methods: The connection between O3 exposure and respiratory mortality in Shenyang, China, from 2014 to 2018 was analyzed by a time-series generalized additive regression model (GAM) with quasi-Poisson regression. Additionally, the potential combined effects of fine particulate matter (PM2.5) and O3 were investigated using the synergy index (SI). Results: Our findings indicate that each 10 μg/m3 increase in O3 at lag 2 days was associated with a maximum relative risk (RR) of 1.0150 (95% CI: 1.0098–1.0202) for respiratory mortality in the total population. For individuals aged ≥55 years, unmarried individuals, those engaged in indoor occupations, and those with low educational attainment, each 10 μg/m3 increase in O3 at lag 07 days was linked to RR values of 1.0301 (95% CI: 1.0187–1.0417), 1.0437 (95% CI: 1.0266–1.0610), 1.0317 (95% CI: 1.0186–1.0450), and 1.0346 (95% CI: 1.0222–1.0471), respectively. Importantly, we discovered a synergistic effect of PM2.5 and O3, resulting in an SI of 2.372 on the occurrence of respiratory mortality. Conclusions: This study confirmed a positive association between O3 exposure and respiratory mortality. Furthermore, it highlighted the interaction between O3 and PM2.5 in exacerbating respiratory deaths.

Abstract:BackgroundEvidence for the association between long-term exposure to ozone (O3) and cause-specific cardiovascular disease (CVD) mortality is inconclusive, and this association has rarely been evaluated at high O3 concentrations.ObjectivesWe aim to evaluate the associations between long-term O3 exposure and cause-specific CVD mortality in a Chinese population.MethodsFrom 2009 to 2018, 744,882 subjects (median follow-up of 7.72 years) were included in the CHinese Electronic health Records Research in Yinzhou (CHERRY) study. The annual average concentrations of O3 and fine particulate matter (PM2.5), which were estimated using grids with a resolution up to 1 × 1 km, were assigned to the community address for each subject. The outcomes were deaths from CVD, ischemic heart disease (IHD), myocardial infarction (MI), stroke, and hemorrhagic/ischemic stroke. Time-varying Cox model adjusted for PM2.5 and individual-level covariates was used.ResultsThe mean of annual average O3 concentrations was 68.05 μg/m3. The adjusted hazard ratio per 10 μg/m3 O3 increase was 1.22 (95% confidence interval [CI]: 1.13–1.33) for overall CVD mortality, 1.08 (0.91–1.29) for IHD, 1.21 (0.90–1.63) for MI, 1.28 (1.15–1.43) for overall stroke, 1.39 (1.16–1.67) for hemorrhagic stroke and 1.22 (1.00–1.49) for ischemic stroke, respectively. The study showed that subjects without hypertension had a higher risk for CVD mortality associated with long-term O3 exposure (1.66 vs. 1.15, p = 0.01).ConclusionsWe observed the association between long-term exposure to high O3 concentrations and cause-specific CVD mortality in China, independent of PM2.5 and other CVD risk factors. This suggested an urgent need to control O3 pollution, especially in developing countries.

Abstract:OBJECTIVETo estimate the modification effects of ozone on the relationship between PM10 and daily mortality.METHODSDaily total non-accidental, cardiovascular and respiratory motality data in Beijing, Shanghai and Suzhou from 2001 to 2008 were collected from the Center for Disease Control and Prevention of each city. The concentration of PM10 and ozone and other Meteorology data during the study period were abtained from the national air quality monitoring network. A generalized additive model (GAM) were used to estimate the acute effects of air pollution on daily mortality in the three cities. The time-series method were applied to analyze the association between air pollution and daily mortality. In order to analyze the interaction effect between PM10 and ozone, ozone were stratified into three levels (≤5% . as low from 5% - 95% . as moderate, >95%, as high) and then analyzed the effect of PM10. A Meta analysis was used to describe the effect of PM10 in all the cities.RESULTSWith the increment of ozone, PM10 corresponded to an increase of total, cardiovascular and respiratory mortality in Beijing, Shanghai and Suzhou. The result of the Meta analyses showed that on the days of "low" ozone, a 10 µg/m3 increment in PM10 corresponded to a 0.47% (95% CI - 0. 74%, 1. 67%) decrease in total mortality, 0. 36% (95% CI - 0. 59%, 1. 31%) decrease in cardiovascular mortality, and 0. 69% (95% CI - 0. 52%, 1. 90%) decrease in respiratory mortality. When it was moderate, the increment was 0.45% (95% CI 0. 26%, 0. 63%), 0. 16% (95% CI - 2. 26%, 1. 95%) and 0. 57% (95% CI 0. 13%, 1. 02%). The increment was 0. 82% (95% CI 0. 24% , 1. 39%), 0. 37% (95% CI 0. 07%, 0. 66% ) and 1. 13% (95% CI - 0. 67% , 2. 92%) while the.zone was high.CONCLUSIONOzone could modify the effect of PM10 on the daily mortality.

Abstract:Background: Few large multicity studies have been conducted in developing countries to address the acute health effects of atmospheric ozone pollution. Objective: We explored the associations between ozone and daily cause-specific mortality in China. Methods: We performed a nationwide time-series analysis in 272 representative Chinese cities between 2013 and 2015. We used distributed lag models and over-dispersed generalized linear models to estimate the cumulative effects of ozone (lagged over 0–3 d) on mortality in each city, and we used hierarchical Bayesian models to combine the city-specific estimates. Regional, seasonal, and demographic heterogeneity were evaluated by meta-regression. Results: At the national-average level, a 10-μg/m3 increase in 8-h maximum ozone concentration was associated with 0.24% [95% posterior interval (PI): 0.13%, 0.35%], 0.27% (95% PI: 0.10%, 0.44%), 0.60% (95% PI: 0.08%, 1.11%), 0.24% (95% PI: 0.02%, 0.46%), and 0.29% (95% PI: 0.07%, 0.50%) higher daily mortality from all nonaccidental causes, cardiovascular diseases, hypertension, coronary diseases, and stroke, respectively. Associations between ozone and daily mortality due to respiratory and chronic obstructive pulmonary disease specifically were positive but imprecise and nonsignificant. There were no statistically significant differences in associations between ozone and nonaccidental mortality according to region, season, age, sex, or educational attainment. Conclusions: Our findings provide robust evidence of higher nonaccidental and cardiovascular mortality in association with short-term exposure to ambient ozone in China. https://doi.org/10.1289/EHP1849

Abstract:BACKGROUND: Controversy remains regarding the relationship between ambient ozone and mortality worldwide. In mainland China, the largest developing country, there has been no prior study investigating the acute effect of O-3 on death risk. Given the changes in types of air pollution from conventional coal combustion to the mixed coal combustion/motor vehicle emissions in China's large cities, it is worthwhile to investigate the acute effect of O-3 on mortality outcomes in the country.OBJECTIVES: We conducted a time-series study to investigate the relation between O-3 and daily mortality in Shanghai using 4 years of daily data (2001-2004). METHODS: We used the generalized additive model with penalized splines to analyze mortality, O-3 pollution, and covariate data in warm and cold seasons. We considered daily counts of all-cause mortality and several causc-specific subcategories (respiratory and cardiovascular). We also examined these associations among several subpopulations based on age and sex.RESULTS: O-3 was significantly associated with total and cardiovascular mortality in the cold season but not in the warm season. In the whole-year analysis, an increase of 10 pg/m(3) of 2-day average (lag01) O-3 corresponds to 0.45% [95% confidence interval (CI), 0.16-0.73%], 0.53% (95% CI, 0.10-0.96%), and 0.35% (95% CI, -0.40 to 1.09%) increase of total nonaccidental, cardiovascular, and respiratory mortality, respectively. In the cold season, the estimates increased to 1.38% (95% CI , 0.68-2.07%), 1.53% (95% CI ,0.54-2.52%), and 0.95% (95% CI, -0.71 to 2.60%), respectively. In the warm season, we did not observe significant associations for both total and causespecific mortality. The results were generally insensitive to model specifications such as lag structure of O-3 concentrations and degree of freedom for time trend. Multipoflutant models indicate that the effect of O-3 was not confounded by particulate matter <= 10 mu m in diameter (PM10) or by sulfur dioxide; however, after adding nitrogen dioxide into the model, the association of O-3 with total and cardiovascular mortality became statistically insignificant.CONCLUSIONS: O-3 pollution has stronger health effects in the cold than in the warm season in Shanghai. Our analyses also.strengthen the rationale for further limiting levels of O-3 pollution in outdoor air in the city.
Short-Term Interaction Effects of PM2.5 and O3 on Daily Mortality: A Time-Series Study of Multiple Cities in China

Abstract:In recent years, PM2.5 and O3 have been the two main pollutants affecting public health in China, but the interaction of the two pollutants on human health remains unclear. A two-stage analytical approach was used to investigate the relationships of PM2.5-O3 co-pollution with nonaccidental, cardiovascular, and respiratory mortality levels across 14 cities in China. We first utilized a generalized additive model (GAM) to determine the city-specific associations of PM2.5 and O3 with daily mortality. The associations were then combined at the national and regional levels using meta-analysis. To investigate the potential interactions between the two pollutants and cause-specific mortality, we performed stratified analyses by co-pollutant exposure levels and the synergy index (SI) (SI > 1 indicates a synergistic interaction). The effect of changes in the two pollutants' concentrations (in 10 μg/m3 increases) on mortality was assessed. The stratification analysis results suggested that each 10 μg/m3 increase in PM2.5 at lag0-1 (lag01) in the low, moderate, and high strata of the O3 concentrations increased nonaccidental mortality by 0.07% (95% confidence interval: -0.03%, 0.17%), 0.33% (0.13%, 0.53%), and 0.68% (0.30%, 1.06%), respectively, with significant between-group differences (p < 0.001). Moreover, each 10 μg/m3 increase in O3 (lag01) in the low, moderate, and high strata of the PM2.5 concentrations increased nonaccidental mortality by 0.15% (-0.06%, 0.36%), 0.53% (0.19%, 0.87%), and 0.75% (0.14%, 1.36%), respectively, with significant between-group differences (p < 0.001). We also found substantial synergistic interactions between the two pollutants and nonaccidental, cardiovascular, and respiratory mortality levels, with SI values of 1.48, 1.51, and 1.33, respectively. Additionally, a subgroup analysis revealed that the interaction of these two pollutants on nonaccidental mortality were greater in South China compared to elsewhere, and during the warm season compared to during the cold season. Our findings suggested that the simultaneous control of PM2.5 and O3 within the context of combined air pollution could significantly decrease the disease risk, especially in southern China and during the warm season.

Abstract:AimsTo study the association between short-term exposure to air pollutants and mortality of Chronic Ischemic Heart Disease (CIHD).MethodsUsing a case-crossover design, we investigated 148,443 CIHD deaths from 2015 to 2020 in Jiangsu Province, China. Exposure to six ambient pollutants, including PM10, PM2.5, NO2, CO, SO2, and O3, was assessed by extracting daily concentrations from validated 10 km × 10 km pollutant grids at each subject’s residential address. A conditional logistic regression approach was used to explore the exposure-response relationship with adjustment for temperature and relative humidity. We calculated the Population Attributable Fractions (PAFs) and the attributable deaths number of CIHD.ResultsAn increase of 10 μg/m3 in PM10 and PM2.5 exposure was associated with a 1.16% (95% CI: 0.85–1.48%) and 1.80% (1.36–2.24%) increase in CIHD mortality, respectively. A threshold value of 123 μg/m3 was identified for the association between O3 exposure and CIHD mortality. Controlling for PM2.5, each increase of 10 μg/m3 in O3 (>threshold) was statistically significantly associated with a 0.94% (0.19–1.71%) increase in CIHD mortality, however there was no association between NO2, SO2, CO exposure and CIHD mortality. Reducing PM2.5, PM10 and O3 to the WHO air quality guidelines would prevent 6.16% (95% CI: 4.70–7.58%), 4.30% (3.18–5.43%) and 1.29% (0.48–4.20%) of CIHD deaths, respectively. During the warm season, mortality and PAFs of CIHD associated with PM2.5, PM10, and O3 were significantly higher.ConclusionsShort-term exposure to ambient PM2.5, PM10, and O3 might trigger deaths from CIHD. These findings indicate that the premature deaths of CIHD patients can be alleviated by reducing exposure to polluted air.

Association between ambient ozone pollution and mortality from a spectrum of causes in Guangzhou, China

Abstract:Ambient ozone (O3) has emerged as an important public health issue worldwide. Previous studies found an association between O3 and cardiorespiratory mortality. However, evidence was limited regarding the risk of O3 on mortality from other diseases. In this study, we aimed to estimate the association between O3 and mortality from a broad spectrum of diseases in Guangzhou, China, which has experienced a rapid increase in O3 concentration over the past decades. Daily data were obtained on cause-specific mortality, air pollutant concentrations and weather conditions during 2013–2018. A generalized additive model with quasi-Poisson regression was applied to examine the association between O3 and mortality from 10 broad causes and 26 refined subcategories, with adjustment of long-term and seasonal trends, weather conditions, public holidays and days of the week. We found that the threshold concentrations of O3 were 40 μg/m3 for all-cause, non-accidental, cardiovascular and respiratory mortality. Mortality risk increased monotonically with O3 concentrations above the threshold. Per 10 μg/m3 increase of O3 at lag 0–3 days was associated with 0.54% (95%CI: 0.34–0.74%), 0.56% (95%CI: 0.36–0.76%), 0.59% (95%CI: 0.30–0.88%), 0.78% (95%CI: 0.33–1.24%) and 0.52% (95%CI: 0.21–0.83%) elevated risk of death from all causes, non-accidental causes, cardiovascular diseases, respiratory diseases and neoplasms, respectively. Among the subcategories, the largest effect estimate was observed in people with chronic obstructive pulmonary disease. The elderly suffered from a higher mortality risk from O3. Stringent emission control strategies and multi-sectoral collaborations are needed to reduce the detrimental impact of O3 on vulnerable populations.

Spline, subset, and threshold models were applied to further evaluate whether a safe threshold level exists.Results: This study found strong evidence that short-term ozone exposure is significantly associated with premature total mortality. A 10 mu g/m(3) increase in the average of the current and previous days' maximum 8-h average ozone concentration was associated with a 0.55% (95% posterior interval: 0.34%, 0.76%) increase of total mortality. This finding is robust when considering the confounding effect of PM2.5, PM10, NO2, and SO2. No consistent evidence was found for a threshold in the ozone-mortality concentration-response relationship down to concentrations well below the current Chinese Ambient Air Quality Standard (CAAQS) level 2 standard (160 mu g/m(3)).Conclusions: Our findings suggest that ozone concentrations below the current CAAQS level 2 standard could still induce increased mortality risks in Jiangsu Province, China. Continuous air pollution control measures could yield important health benefits in Jiangsu Province, China, even in cities that meet the current CAAQS level 2 standard.
Associations between short-term ambient ozone exposure and cause-specific mortality in rural and urban areas of Jiangsu, China

Abstract:Most previous studies on the acute health effects of ozone are limited to urban areas, largely due to the paucity of air pollutant measurements in rural areas. We here estimated the county-specific daily maximum 8-h average ozone concentration in Jiangsu Province, China during 2015-2018, using a recently developed spatiotemporal machine learning model at a spatial resolution of 0.1° × 0.1° (∼11 × 11 km). Counties were equally divided into urban and rural groups based on the median of the percentage of urban residents across Jiangsu counties obtained from the National Population Census in 2010. We first conducted time-series analyses to estimate the county-specific effect of ozone using generalized linear models, then pooled the effect estimates by random-effects modeling. A 10 μg/m3 increase in the 4-day moving average (lag 0-3) of ambient ozone exposure was associated with increases of 0.66% (95% confidence interval [CI] 0.36%-0.95%) in daily nonaccidental mortality in rural areas and 0.42% in urban areas (95% CI, 0.27%-0.56%). Short-term ambient ozone exposure was associated with an increased risk of mortality caused by chronic obstructive pulmonary disease, hypertension, ischemic heart disease, and stroke. Our finding suggests that both urban and rural residents suffer adverse health effects from short-term ozone exposure.

Abstract:Controversy remains regarding the relationship between various metrics of ozone (O3) and mortality. In China, the largest developing country, there have been few studies investigating the acute effect of O3 on death. We used three exposure metrics of O3 (1-hour maximum, maximum 8-hour average and 24-hour average) to examine its short-term association with daily mortality in Suzhou, China. We used a Generalized Additive Model (GAM) with penalized splines to analyze the mortality, O3, and covariate data. We examined the association by season, age group, sex and educational level. We found that the current level of O3 in Suzhou is associated with death rates from all causes and cardiovascular diseases. Among various metrics of O3, maximum 8-hour average and 1-hour maximum concentrations seem to be more strongly associated with increased mortality rate compared to 24-hour average concentrations. Using maximum 8-hour average, an inter-quartile range increase of 2-day average O3 (lag 01) corresponds to 2.15% (95%CI, 0.36 to 3.93), 4.47% (95%CI, 1.43 to 7.51), −1.85% (95%CI, −6.91 to 3.22) increase in all-cause, cardiovascular, and respiratory mortality, respectively. The associations between O3 and daily mortality appeared to be more evident in the cool season than in the warm season. In conclusion, maximum 8-hour average and 1-hour maximum concentrations of O3 are associated with daily mortality in Suzhou. Our analyses strengthen the rationale for further limiting levels of O3 pollution in the city.

Abstract:Abstract Evidence on the effects of ozone on chronic lower respiratory disease (CLRD), acute respiratory infections (ARI) is still not conclusive. This study aimed to investigate the association between them in Hohhot China. Daily death counts, concentrations of PM2.5, PM10, SO2, NO2, and O3, and meteorological data were collected for Hohhot city from January 1, 2018, to December 31, 2019. We performed a time series analysis using generalized additive model (GAM). In single pollutant model, the estimated excess relative risks (EERs) for respiratory and CLRD mortality were 2.815% (95% CI: 0.075–5.630) at lag 04 and 4.409% (95% CI: 1.577–7.320) at lag 01, respectively, for each 10 μg/m3 increase in O3. No significant association was found for ARI and the other respiratory diseases mortality. Subgroup analysis showed that the respiratory diseases for men were more susceptible to O3 than women, while men had a lower association with CLRD than women. Concentrations of O3 were significantly associated with respiratory disease and CLRD for older adults. The significant positive association between O3 and respiratory mortality and CLRD mortality remained largely unchanged in the two-pollutant model and the multi-pollutant model. In conclusion, short-term exposure to O3 is significantly associated respiratory diseases and CLRD mortality, and this association is dominated by gender and age.

Abstract:Background: Ozone pollution is associated with cardiovascular disease mortality, and there is a high correlation between different pollutants. This study aimed to assess the association between ozone and cardiovascular disease deaths and the resulting disease burden in Nanjing, China. Methods: A total of 151,609 deaths from cardiovascular disease were included in Nanjing, China from 2013 to 2021. Daily data on meteorological and air pollution were collected to apply a generalized additional model with multiple pollutants to perform exposure-response analyses, stratification analysis, and evaluation of excess deaths using various standards. Results: In the multi-pollutant model, an increase of 10 μg/m 3 in O 3 was significantly associated with a 0.81% (95%CI: 0.49, 1.12%) increase in cardiovascular disease deaths in lag05. The correlation weakened in both the single-pollutant model and two-pollutant models, but remained more pronounced in females, the older group, and during warm seasons. From 2013 to 2021, the number of excess deaths attributed to ozone exposure in cardiovascular disease continued to rise with an increase in ozone concentration in Nanjing. If the ozone concentration were to be reduced to the WHO standard and the minimum level, the number of deaths would decrease by 1,736 and 10,882, respectively. Conclusion: The risk of death and excess deaths from cardiovascular disease due to ozone exposure increases with higher ozone concentration. Reducing ozone concentration to meet WHO standards or lower can provide greater cardiovascular disease health benefits.

Does methylphenidate normalize the classroom performance of children with attention deficit disorder?

Abstract:The present study examined the degree to which methylphenidate (MPH) normalized the classroom behavior and academic functioning of 31 children with attention deficit disorder (ADD). Subjects with ADD participated in a double-blind, placebo-controlled trial in which children received each of four doses (5, 10, 15, and 20 mg) of MPH and a placebo following baseline measures. Children with ADD were compared with a group of 25 normal control children on teacher ratings of social conduct, direct observations of on-task behavior, and academic efficiency. At a group level of analysis, MPH exerted a significant effect on classroom measures of attention and academic efficiency to a point where they were no longer statistically deviant from scores obtained by normal control children. Nevertheless, when examined at the individual level, 25% of the sample failed to show normalized levels of classroom performance, thus implying the need for ancillary school-based interventions.
Long-term exposure to ozone and cardiovascular mortality in China: a nationwide cohort study

Abstract:BACKGROUND: The evidence for a causal relationship between long-term ozone exposure and cardiovascular mortality is inconclusive, and most published data are from high-income countries. We aimed to investigate the association between long-term exposure to ozone and cardiovascular mortality in China, the most populous middle-income country.METHODS: We did a nationwide cohort study comprising Chinese adults aged 18 years and older from the 2010-11 China Chronic Disease and Risk Factors Surveillance project; participants were followed up until Dec 31, 2018, or the date of death. Data on participants' deaths were obtained through linkage to the Disease Surveillance Point system, a national death registration database. Residential ozone exposure was estimated with a previously developed random forest model. We applied stratified Cox proportional hazards models to estimate the associations of ozone with mortality due to overall cardiovascular diseases, ischaemic heart disease, and stroke. The models were stratified by age and sex and adjusted for a set of individual-level and regional covariates. Warm-season average ozone concentration for the previous 1-3 years was added as a time-varying variable. We also did subgroup analyses by age, sex, level of education, smoking status, urban or rural residence, and geographical region.FINDINGS: Data were analysed for 96 955 participants. The warm-season average ozone concentration during the follow-up period was 89·7 μg/m3 (SD 14·4). In the fully adjusted models, we observed significant and positive associations between ozone and mortality from overall cardiovascular diseases (hazard ratio [HR] 1·093 [95% CI 1·046-1·142] per 10 μg/m3 increase in warm-season ozone concentrations), ischaemic heart disease (1·184 [1·099-1·276] per 10 μg/m3 increase in warm-season ozone concentrations), and stroke (1·063 [1·002- 1·128] per 10 μg/m3 increase in warm-season ozone concentrations). After adjusting for fine particulate matter, the associations with overall cardiovascular disease and ischaemic heart disease mortality were almost unchanged, whereas the association with stroke mortality lost statistical significance. The association of long-term ozone exposure with cardiovascular mortality was more prominent in people aged 65 years and older than in those younger than 65 years. We did not find any effect modification of sex, level of education, smoking status, urban or rural residence, and geographical region. We observed an almost linear exposure-response relationship between ozone and cardiovascular mortality.INTERPRETATION: This study is, to the best of our knowledge, the first nationwide cohort study to show that long-term ozone exposure contributes to elevated risks of cardiovascular mortality, particularly from ischaemic heart disease, in a middle-income setting. The exposure-response function generated from this study could potentially inform future air quality standard revisions and environmental health impact assessments.FUNDING: National Natural Science Foundation of China.
