Abstract:To compare liver stiffness measurements obtained from MR elastography with liver T1 relaxation times obtained from T1 mapping and T2 relaxation times obtained from T2 mapping for detection and staging of liver fibrosis. 223 patients with known or suspected liver disease underwent MRI of the liver with T1 mapping (Look-Locker sequence) and 2D SE-EPI MR elastography (MRE) sequences. 139 of these patients also underwent T2 mapping with radial T2 TSE sequence. Two readers (R1 & R2) measured liver stiffness, T1 relaxation times and T2 relaxation times. T1 and T2 times were correlated with stiffness measurements. ROC analysis was used to compare the performance of both techniques in discriminating fibrosis stage in 23 patients who underwent liver biopsy. For each reader there was significant moderate positive correlation between liver MRE and liver T1 mapping (r = 0.49 and 0.36). There was significant moderate positive correlation between liver T2 mapping and each of MRE and T1 mapping for one of the readers (r = 0.40 and 0.27). AUC for differentiating early (F0–F2) from advanced (F3–F4) fibrosis in biopsied patients was 0.975 (R1) and 0.925 (R2) for MRE, 0.671 (R1) and 0.642 (R2) for T1 mapping and 0.671 (R1) and 0.743 (R2) for T2 mapping. Inter-reader agreement was good for MRE (ICC = 0.84) substantial for T1 mapping (0.94) and T2 mapping (0.96). Liver T1 and T2 mapping showed moderate positive correlation with MR elastography. Accuracy of MRE is however superior to T1 and T2 mapping in the subset of patients who underwent liver biopsy. Accuracy of combination of MRE and T1 mapping/T2 mapping was not superior to MRE alone.
Application Values of Gadolinium-ethoxybenzyl Diethylenetriaminepentaacetic Acid Enhanced Magnetic Resonance Imaging-based Radiomics in the Quantitative Assessment of Liver Reserve Function of Patients with Liver Cirrhosis

Abstract: To evaluate the correlation between the radiomics signature of hepatobiliary phase imaging of gadolinium-ethoxybenzyl diethylenetriaminepentaacetic acid(Gd-EOB-DTPA)enhanced magnetic resonance imaging(MRI)and Child-Pugh of liver cirrhosis,establish nomogram prediction model,and assess the predictive value of quantitative assessment of liver reserve function of patients with liver cirrhosis. One hundred patients with liver cirrhosis who met the inclusion criteria were divided into 52 patients with Child-Pugh grade A and 48 patients with Child-Pugh grade B+C according to Child-Pugh classification criteria,and were randomly divided into training set and test set at a proportion of 7∶3.The AK software was used to extract the imaging features of the Gd-EOB-DTPA-enhanced MRI hepatobiliary images of the patients in the training set,and the least absolute shrinkage and selection operator feature selection algorithm was used to reduce the dimension of the data,select the features,and construct the radiomics tags.According to the radiomics label Rad-score,a line chart(nomogram)prediction model was established to predict the Child-Pugh B+C level of liver reserve function.The model was applied to the training set and test set respectively,and the diagnostic efficiency was quantitatively evaluated by receiver operating characteristic(ROC)curve. After dimension reduction and screening of 396 texture feature parameters extracted by AK software,7 image feature parameters were obtained.According to the above characteristics,the radiomics tag Rad-score was constructed and the nomogram prediction model was created.The differences of Rad-score scores between Child-Pugh A and Child-Pugh B+C groups in training set and test set were statistically analyzed by Wilcoxon rank sum test(=0.000, =0.001).The diagnostic efficacy of nomogram prediction model for predicting Child-Pugh B+C grade of liver reserve function in the ROC curve of training set and test set was 0.88 and 0.86 respectively. The nomogram prediction model created according to the radiomics tag Rad-score of patients with liver cirrhosis with different liver reserve functions can be used as a more accurate and reliable auxiliary detection tool for liver reserve function.It provides a new means for clinicians to evaluate liver reserve function more accurately.
MR Elastography, T1 and T2 Relaxometry of Liver: Role in Noninvasive Assessment of Liver Function and Portal Hypertension

Materials and methods 223 patients with known or suspected liver disease underwent MRI of the liver with T1 mapping (Look-Locker sequence) and 2D SE-EPI MR elastography (MRE) sequences. 139 of these patients also underwent T2 mapping with radial T2 FS sequence. Two readers measured liver stiffness, T1 relaxation times and T2 relaxation times, and assessed qualitative features such as presence or absence of cirrhosis, ascites, spleen length, and varices on conventional MRI images. A third reader collected the clinical data (MELD score, MELD-Na Score, ALBI grade, and results of endoscopy in 78 patients). Results Significant moderate correlation was found between MELD score and all three imaging techniques for both readers (MRE,r = 0.35 and 0.28; T1 relaxometry,r = 0.30 and 0.29; T2 relaxometry,r = 0.45, and 0.37 for reader 1 and reader 2 respectively). Correlation with MELD-Na score was even higher (MRE,r = 0.49 and 0.40; T1,r = 0.45 and 0.41; T2,r = 0.47 and 0.35 for reader 1 and reader 2 respectively). Correlations between MRE and ALBI grade was significant and moderate for both readers:r = 0.39 and 0.37, higher than T1 relaxometry (r = 0.22 and 0.20) and T2 relaxometry (r = 0.17, andr = 0.24). Significant moderate correlations were found for both readers between MRE and the presence of varices on endoscopy (r = 0.28 and 0.30). MRE and T1 relaxometry were significant predictors of varices at endoscopy for both readers (MRE AUC 0.923 and 0.873; T1 relaxometry AUC = 0.711 and 0.675 for reader 1 and reader 2 respectively). Cirrhotic morphology (AUC = 0.654), spleen length (AUC = 0.610) and presence of varices in the upper abdomen on MRI (AUC of 0.693 and 0.595) were all significant predictors of endoscopic varices. Multivariable logistic regression model identified that spleen length and liver MRE were significant independent predictors of endoscopic varices for both readers. Conclusion MR elastography, T1 and T2 relaxometry demonstrated moderate positive correlation with the MELD score and MELD-Na Score. Correlation between MRE and ALBI grade was superior to T1 and T2 relaxometry methods. MRE performed better than T1 and T2 relaxometry to predict the presence of varices at endoscopy. On multivariate analyses, spleen length and MRE were the only two significant independent predictors of endoscopic varices.
Diffusion-weighted Imaging and Variable Flip Angle T1 Mapping: a Supplement for Image-Guided Biopsy in Follow-Up Analysis of Liver Fibrosis

Abstract:Purpose To evaluate the performance of diffusion-weighted imaging (DWI) and variable flip angle (VFA) T1 mapping as a supplement to image-guided biopsy in follow-up analysis of liver fibrosis.Materials and Methods This prospective study was approved by the institution's committee on human research,and written informed consent was provided from the enrolled patients.We investigated five MRI parameters of DWI and VFA T1 mapping,collected from 11 patients who underwent serial ultrasound image-guided biopsy with follow-up MRI within 1.5 years after treatment for liver fibrosis/cirrhosis.For each patient,four consecutive MRI examinations were conducted,including baseline MRI before treatment and three follow-up MRI examinations after treatment at each 0.5-year interval.ADC values at four b values and T1 relaxation times were correlated to pathology-confirmed liver fibrosis stages,which were subsequently divided into two groups,stages F2-3 and F4.The receiver operating characteristic (ROC) analysis and repeated measurement analysis of variance were used for statistical analysis.Results Among these ADC parameters,ADC value (b =500 s/mm2) was the most consistent in differentiating between stage F2-3 and F4 liver fibrosis.Repeated measurement analysis showed that the intra-group and inter-group differences were 0.447 and 0.024,respectively.T1 relaxation time could not consistently differentiate between the F2-3 and F4 groups;however,it was repeatable,and the intra-group and inter-group differences were 0.410 and 0.042,respectively.Conclusion MRI-ADC value at a b value of 500 s/mm2 can be a promising biomarker for differentiating stages F2-3 and F4 liver fibrosis.A combination of this biomarker with repeatable T1 relaxation time may function as a non-invasive tool for follow-up liver fibrosis in patients who reject repeated image-guided biopsy.

Abstract:PurposeTo evaluate the possibility of using a variable flip angle (VFA) T1 mapping technique to diagnose liver fibrosis.Materials and MethodsLiver fibrosis was induced in rabbits by repetitive administration of carbon tetrachloride (CCl4). T1‐weighted magnetic resonance imaging (MRI) was performed in 29 animals (liver fibrosis, n = 18; control, n = 11) using a series of nonenhanced liver acquisition volume acceleration (LAVA) with VFAs at 3.0T. Hepatic T1 relaxation times were measured via regions of interest, which were correlated with subsequent histologic confirmation. The results of T1 mapping in assessment of liver fibrosis were compared with that of apparent diffusion coefficient (ADC) values.ResultsThe mean T1 relaxation time of the control group was the lowest (250.07 ± 88.12 msec), followed by the nonadvanced fibrosis group (387.83 ± 166.58 msec) and the advanced fibrosis group (496.90 ± 291.24 msec). T1 relaxation time measurements differed significantly between the liver fibrosis group and control group (P < 0.05), with a trend of increased mean T1 relaxation times as the fibrotic stage increased. Statistically significant differences were observed between the control group and the nonadvanced fibrosis group (P < 0.05), however with much overlap between the less severe stages. In discriminating between the control group and liver fibrosis group, stage F0‐1 (control and stage F1) and stage F2‐3, stage F0‐2 (control and stage F1‐2) and stage F3, area under the receiver operating characteristic (ROC) curves were 0.803 (cutoff value 273.01 msec), 0.712 (cutoff value 371.54 msec), and 0.696 (cutoff value 276.99 msec), respectively. No difference was found between T1 relaxation times and ADC values in assessment of liver fibrosis in our study.ConclusionVFA T1 mapping may become a noninvasive imaging tool for the diagnosis of liver fibrosis. Assessment of Liver Fibrosis Using Pharmacokinetic Parameters of Dynamic Contrast-Enhanced Magnetic Resonance Imaging.

Abstract:Purpose(:) To evaluate the pharmacokinetic parameters of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in diagnosing and staging liver fibrosis in rabbits.Materials and Methods: DCE-MRI with gadodiamide (Gd-DTPA-BMA) was performed on a 3.0 Tesla, 60cm bore MR scanner for rabbits with CCl4-induced liver fibrosis, and an untreated control group. Fibrosis was staged according to the METAVIR system: control (F0; n=13), nonadvanced fibrosis (F1-2; n=15), and advanced fibrosis (F3-4; n=12). The DCE-MRI parameters K-trans, k(ep), V-e, and v(p) were measured with a dual-input extended Tofts model. Receiver operating characteristic analyses were performed to assess the diagnostic performance of K-trans, V-e, and v(p) in staging liver fibrosis.Results: Both K-trans and V-e decreased with increasing fibrosis stage. K-trans of the control group was significantly different from that of the overall fibrosis group, nonadvanced group, and advanced group (P < 0.001 for all). Significant differences were found between V-e of the control group and that of the overall fibrosis and advanced groups (P=0.019 and P=0.009, respectively). For K-trans, the areas under the receiver operating characteristic curve (AUROCs) for discriminating the control group from the overall fibrosis and advanced fibrosis groups were 0.909 (95% confidence interval [CI], 0.809-1.000), and 0.936 (95% CI, 0.847-1.000), respectively. For discriminating between the control and nonadvanced fibrosis groups, the AUROC of K-trans was 0.887 (95% CI, 0.762-1.000). The AUROCs of K-trans were higher than those of V-e and v(p) for discriminating between the control and overall fibrosis groups, the control and nonadvanced fibrosis groups, and the control and advanced fibrosis groups. Pharmacokinetic parameters were negatively correlated with fibrosis stage (K-trans, rho=-0.668, P < 0.001; V-e, rho=-0.438, P=0.005; v(p), rho=-0.360, P=0.023).Conclusion: Among pharmacokinetic parameters of DCE-MRI in our study, K-trans was an excellent predictor for differentiating fibrotic livers from normal livers, and differentiating normal livers from nonadvanced or advanced fibrosis livers.

Abstract:Background: Few studies have investigated the value of Gd-EOB-DTPA-enhanced T1 mapping in exact fibrosis staging, especially its correlation with hepatic molecular transporters.Aims: To investigate the diagnostic value of Gd-EOB-DTPA-enhanced T1 mapping in staging liver fibrosis and its relationship with hepatic molecular transporters.Methods: Thirty rats were divided into the carbon tetrachloride-induced fibrosis groups and a control group. Tl-mapping was performed before and 20 min after administration of Gd-EOB-DTPA. The T1 relaxation time and reduction rate (Delta%) were calculated, and their correlations with the degree of fibrosis, necroinflammatory activity, iron load and hepatic molecular transporters were assessed and compared.Results: Hepatobiliary phase T1 relaxation time (HBP) and Delta% were different between each adjacent fibrosis subgroups(P=0.000-0.042). Very strong correlations existed between fibrosis and both HBP and Delta% (r=0.960/-0.952), and multivariate analyses revealed that fibrosis was the only factor independently predicted by HBP (P=0.000) and Delta% (P=0.001), comparing to necroinflammatory activity and iron load. The expression of the organic anion transporting polypeptidelal (Oatplal) was significantly correlated with HBP and Delta% at both mRNA (r=-0.741/0.697) and protein (r=-0.577/0.602) levels. Weaker correlations were found for multidrug resistance associated protein2 (Mrp2). Generally, both transporters showed decreasing levels with increasing degrees of fibrosis.Conclusion: Gd-EOB-DTPA-enhanced T1 mapping may provide a reliable diagnostic tool in staging liver fibrosis, and can be regarded as a useful imaging biomarker of hepatocyte transporter function. Staging liver fibrosis in chronic hepatitis B with T relaxation time index on gadoxetic acid-enhanced MRI: Comparison with aspartate aminotransferase-to-platelet ratio index and FIB-4.

Abstract:PurposeTo assess the accuracy of the T-1 relaxation time index on gadoxetic acid-enhanced magnetic resonance imaging (MRI) for staging liver fibrosis in chronic hepatitis B (CHB), in comparison and combination with the aspartate aminotransferase-to-platelet ratio index (APRI) and fibrosis-4 (FIB-4). Materials and MethodsA retrospective study of gadoxetic acid-enhanced T-1 mapping and serum biochemical tests was performed on 126 CHB patients who underwent gadoxetic acid-enhanced 1.5T MRI, and the histological score used as the gold standard. The reduction rate of T-1 relaxation time before and 20 minutes after gadoxetic acid injection (T-1, R1%), the contrast uptake rate (K-Hep), APRI, and FIB-4 were calculated. The diagnostic efficacy of T-1, R1%, K-Hep, APRI, and FIB-4 for predicting stage 2 or greater (S2), stage 3 or greater (S3), and stage 4 (S4) was compared. ResultsT(1) (r=-0.513, P < 0.001), R1% (r=-0.626, P < 0.001), K-Hep (r=-0.527, P < 0.001), APRI (r=0.519, P < 0.001), and FIB-4 (r=0.476, P < 0.001) correlated significantly with fibrosis stages. Areas under the curves (AUCs) of R1% for detecting S2, S3, and S4 were 0.849, 0.827, and 0.809, which were greater than that of APRI (0.763, 0.745, 0.787) and FIB-4 (0.727, 0.738, 0.772), but significant difference was found only in discriminating S2 between R1% and FIB-4 (P=0.027). The combination of all five indices performed best, with AUC, sensitivity, and specificity of 0.860, 87.21%, and 72.50% for diagnosing S2, 0.878, 82.81%, and 85.48% for S3, and 0.867, 80.00%, and 83.95% for S4. ConclusionThe gadoxetic acid-enhanced T-1 relaxation time index appears to be superior to APRI and FIB-4 for predicting hepatic fibrosis. The combined use of gadoxetic acid-enhanced T-1 mapping, APRI, and FIB-4 may be more reliable for staging liver fibrosis in CHB.
Assessing Liver Function by T1 Maps on Gd-EOB-DTPA-Enhanced MRI for up to 50 Min in Rat Models of Liver Fibrosis: A Longer Hepatobiliary Time Period may Help

Abstract:Objectives To investigate whether a longer time period of gadolinium ethoxybenzyl diethylenetriaminepen-taacetic acid (Gd-EOB-DTPA)-enhanced T1 mapping scanning, as well as dynamic contrast-enhanced (DCE) and multiple hepatobiliary phase magnetic resonance imaging (MRI) have the potential to provide information about liver function in rats with liver fibrosis. Methods Forty rats were divided into the carbon tetrachloride-induced hepatic injury groups [carbon tetrachloride for four (n=14), eight (n=8), or twelve (n=8) weeks] and the control group (n=10). Gd-EOB-DTPA-enhanced MRI was performed including T1-mapping (delayed to 50 min), DCE, and multiple hepatobiliary phases. Indocyanine green retention rate at 15 min (ICG-R15) was determined. Parameters such as T1 reduction rate (ΔT1), elimination half-life of ΔT1 (TΔT1 1/2), relative enhancement (RE), time to maximum RE (Tmax), and perfusion parameters were calculated. Pearson correlation analysis was used for correlation analysis between ICG-R15 and each MRI indices. Results ΔT1 at 30, 40, and 50 min showed significant positive correlations with ICG-R15 ( r=0.784, 0.653, 0.757, P=0.007, 0.041, 0.030). TΔT1 1/2 showed a significant positive correlation with ICG-R15 (r=0.685, P=0.029). Tmaxshowed a significant positive correlation with ICG-R15 (r=0.532, P=0.019). Conclusions ΔT1 in the late hepatobiliary phase and T ΔT1 1/2 exhibited moderate correlations with liver function. The longer time period of Gd-EOB-DTPA-enhanced T1 mapping scanning, as well as DCE and multiple hepatobiliary phases, may be of some value for estimating liver function in rats with liver fibrosis.

Abstract:The accurate diagnosis and staging of hepatic fibrosis is crucial for treatment and prognosis of liver disease. The current gold standard is liver biopsy, but it cannot be used in population-based screening, and has well known drawbacks if used for monitoring of disease progression or treatment results. Functional MR, as a non-invasive method, is increasingly used in hepatic fibrosis and became the current hot spot. Most recently available functional MR imaging techniques including diffusion weighted imaging, perfusion weighted imaging and MR spectroscopy can detect cirrhosis or fibrosis reasonably accurately. However, to date only MR elastography has been able to stage fibrosis or diagnose mild disease. MR diffusion weighted appears next most promising.
The hepatocyte phase of Gd-EOB-DTPA-enhanced MRI in the evaluation of hepatic fibrosis and early liver cirrhosis in a rat model: An experimental study

Abstract:Aims To evaluate the hepatocyte phase of Gd-EOB-DTPA-enhanced MRI in the early diagnosis of hepatic fibrosis and cirrhosis and assessment of liver function in a rat model. Main methods In 2 groups of SD rats, liver fibrosis was induced in experimental animals by repetitive carbon tetrachloride injections, while the control group received saline injections. Five experimental rats and 2 control rats were randomly selected at weeks 4, 8, 12. One week after carbon tetrachloride administration, MRI (FIRM T1WI) scan was performed. Gd-EOB-DTPA (0.08mL) was injected into the rat's tail vein and hepatocyte phase images were obtained after 20min. The pre-enhanced phase and hepatocyte phase signal intensities (SI) were measured, and the relative contrast enhancement index (RCEI) was calculated. ANOVA analysis (LSD) of RCEI values in controls (n=6), hepatic fibrosis (n=7), and histopathologically-determined early cirrhosis group (n=6) was performed. Key findings RECI values showed a decreasing trend in the control group, hepatic fibrosis and early cirrhosis groups (1.11±0.43, 0.96±0.22, and 0.57±0.33, respectively). While the difference between the control and early cirrhosis groups was statistically significant (p=0.013), there was no significant difference in the hepatic fibrosis group vs the control (p=0.416) and the hepatic fibrosis group vs the early cirrhosis group (p=0.054). Significance Hepatocyte phase RCEI values obtained with Gd-EOB-DTPA-enhanced MRI scan indicate liver injury in hepatic fibrosis and early cirrhosis. RCEI values are helpful for early diagnosis of liver cirrhosis.

Abstract:Purpose: In this prospective study, we aimed to test the feasibility of dynamic Gd-EOB-DTPA-enhanced MRI with a free-breathing T1 mapping technique in a single scan and examine the influence of functional future liver remnant(FLR) on the incidence of post-hepatectomy liver failure(PHLF) in patients who underwent major liver resection for suspected bile duct cancer(BTC) after bile drainage. Method: Patients were included for suspected biliary tract tumor with a plan for hepatectomy in this prospective study between 2020 and 2022. Patients were excluded if artifacts existed on images or with a history of radiotherapy or chemotherapy before surgery. PHLF was defined and graded according to the International Study Group of Liver Surgery(ISGLS) criterion. All the scans were performed on a 3.0 T MR scanner (Philips Healthcare) and functional liver indexes were calculated accordingly. GOAL-SNAP sequence was optimized for quantifying T1 of the liver including an inversion recovery (IR) preparation pulse followed by a series of 3D golden angle radial acquisition. Results: Total 49 patients met the inclusion criteria and completed preoperative routine MRI sequence, among whom 33 patients completed additional free-breathing T1 mapping sequence. Perihilar cholangiocarcinoma was the main clinical diagnosis on admission in 46 patients. The other diagnoses were carcinoma of common bile duct and gallbladder carcinoma(n=3). △T1, △T1%, HEF(n=33) and UR(n=32) of the whole liver have statistical significance correlations with preoperative liver biochemical indexes and ICG test(Fig. 1). In this study, 39 patients finally underwent major liver resection and thus were included for PHLF analysis. According to ISGLS criterion, 16 patients happened PHLF which 10 patients were diagnosed in grade B and C.There were no difference between PHLF and NO PHLF groups in surgical characteristics. Univariable and multivariable analysises for PHLF were shown in Table 1(Results with P<0.05). Conclusion: In this prospective study, the correlation results have demonstrated MRI parameters' clinical feasibility for liver function estimation in patients with BTC. Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>This study aimed to assess the use of hepatocyte fraction in gadoxetic acid-enhanced magnetic resonance imaging (MRI) for quantitatively evaluating the liver function in comparison with T1 relaxation‐based indices.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This retrospective study included 79 patients with chronic liver disease, who were divided into 2 groups based on the results of the indocyanine green retention test (ICG). All patients underwent a gadoxetic acid-enhanced MRI of the liver. Pre- and post-contrast Look-Locker sequences were used 20 min after gadoxetic acid administration to acquire T1 mapping. Two readers independently identified and measured the MRI parameters [five T1 relaxation‐based indices (T1pre, T1post, rrT1, R1post/R1pre and ΔR1) and two hepatocyte fraction indices ( HeF and<em>K</em><sub>Hep</sub>)]. An Independent-samples <em>t</em> test was used to compare each parameter for the two groups. Pearson correlation analysis was used to analyze the correction in each parameter and 15-minute ICG retention rate (ICG-R15). Receiver operating characteristic analyses were performed to differentiate the diagnostic performance of each parameter in ICG-R15 ≤ 20% and ICG-R15 > 20% groups.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>T1pre and T1post were significantly lower in the ICG-R15 ≤ 20% group than in the ICG-R15 > 20% group (<em>P</em> < 0.05). rrT1, R1post/R1pre, ΔR1, HeF, and <em>K</em><sub>Hep</sub> were significantly higher in the ICG-R15 ≤ 20% group than in the ICG-R15 > 20% group (<em>P</em> < 0.05). The correction coefficients between T1pre, T1post, rrT1, R1post/R1pre, ΔR1, HeF, <em>K</em><sub>Hep</sub>, and ICG-R15 were 0.343, 0.783, –0.833, –0.781, –0.803, –0.819, and –0.832, respectively. The area under the curves (AUCs) of T1pre, T1post, rrT1, R1post/R1pre, ΔR1, HeF, and <em>K</em><sub>Hep</sub> in assessing the ICG-R15>20% groups were 0.761, 0.945, 0.912, 0.912, 0.948, 0.945, and 0.950, respectively. <em>K</em><sub>Hep</sub> had the highest AUC, sensitivity, and specificity.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Hepatocyte fraction based on gadoxetic acid-enhanced T1-mapping MRI is an efficient diagnostic tool for the quantitative evaluation of liver function.</p>

Abstract:INTRODUCTION: New advances in liver magnetic resonance imaging (MRI) may enable diagnosis of unseen pathologies by conventional techniques. Normal T1 (550-620 ms for 1.5 T and 700-850 ms for 3 T), T2, T2* (>20 ms), T1rho (40-50 ms) mapping, proton density fat fraction (PDFF) (≤5%) and stiffness (2-3kPa) values can enable differentiation of a normal liver from chronic liver and diffuse diseases. Gd-EOB-DTPA can enable assessment of liver function by using postcontrast hepatobiliary phase or T1 reduction rate (normally above 60%). T1 mapping can be important for the assessment of fibrosis, amyloidosis and copper overload. T1rho mapping is promising for the assessment of liver collagen deposition. PDFF can allow objective treatment assessment in NAFLD and NASH patients. T2 and T2* are used for iron overload determination. MR fingerprinting may enable single slice acquisition and easy implementation of multiparametric MRI and follow-up of patients. Areas covered: T1, T2, T2*, PDFF and stiffness, diffusion weighted imaging, intravoxel incoherent motion imaging (ADC, D, D* and f values) and function analysis are reviewed. Expert commentary: Multiparametric MRI can enable biopsyless diagnosis and more objective staging of diffuse liver disease, cirrhosis and predisposing diseases. A comprehensive approach is needed to understand and overcome the effects of iron, fat, fibrosis, edema, inflammation and copper on MR relaxometry values in diffuse liver disease.

Abstract:Purpose: To evaluate the value of T-2, T(1)rho, and diffusion metrics in assessment of liver fibrosis in rats. Materials and Methods: Liver fibrosis in a rat model (n=72) was induced by injection of carbon tetrachloride (CCl4) at 3T. T-2, T(1)rho, and diffusion parameters (apparent diffusion coefficient (ADC), D-true) via spin echo (SE) diffusion-weighted imaging (DWI) and stimulated echo acquisition mode (STEAM) DWI with three diffusion times (DT: 80, 106, 186 msec) were obtained in surviving rats with hepatic fibrosis (n=52) and controls (n = 8). Liver fibrosis stage (F0-F6) was identified based on pathological results using the traditional liver fibrosis staging method for rodents. Nonparametric statistical methods and receiver operating characteristic (ROC) curve analysis were employed to determine the diagnostic accuracy. Results: Mean T-2, T(1)rho, ADC, and D-true with DT=186 msec correlated with the severity of fibrosis with r=0.73, 0.83, -0.83, and -0.85 (all P < 0.001), respectively. The average areas under the ROC curve at different stages for T(1)rho and diffusion parameters (DT=186 msec) were larger than those of T-2 and SE DWI (0.92, 0.92, and 0.92 vs. 0.86, 0.82, and 0.83). The corresponding average sensitivity and specificity for T(1)rho and diffusion parameters with a long DT were larger (89.35 and 88.90, 88.36 and 89.97, 90.16 and 87.13) than T-2 and SE DWI (90.28 and 79.93, 85.30 and 77.64, 78.21 and 82.41). The performances of T(1)rho and D-true (DT=186 msec) were comparable (average AUC: 0.92 and 0.92). Conclusion: Among the evaluated sequences, T(1)rho and STEAM DWI with a long DT may serve as superior imaging biomarkers for assessing liver fibrosis and monitoring disease severity.

Abstract:Purpose: We investigated the capability and imaging criteria of T1 mapping and arterial spin labeling (ASL) MRI to identify renal injury in patients with liver cirrhosis. Methods: We recruited 27 patients with cirrhosis and normal renal function (cirrhosis-NR), 10 with cirrhosis and renal dysfunction (cirrhosis-RD) and 23 normal controls (NCs). All participants were examined via renal T1 mapping and ASL imaging. Renal blood flow (RBF) derived from ASL was measured from the renal cortex, and T1 values were measured from the renal parenchyma (cortex and medulla). MRI parameters were compared between groups. Diagnostic performances for detecting renal impairment were statistically analyzed. Results: Cortical T1 (cT1) and medullary T1 (mT1) were significantly lower in the NCs than in the cirrhosis-NR group. The cortical RBF showed no significant changes between the NCs and cirrhosis-NR group but was markedly decreased in the cirrhosis-RD group. The areas under the curve (AUCs) for discriminating cirrhosis-NR from NCs were 0.883 and 0.826 by cT1 and mT1, respectively. Cortical RBF identified cirrhosis-RD with AUC of 0.978, and correlated with serum creatinine (r = -0.334) and the estimated glomerular filtration rate (r = 0.483). A classification and regression tree based on cortical RBF and cT1 achieved 85% accuracy in detecting renal impairment in the cirrhosis. Conclusion: Renal T1 values might be sensitive predictors of early renal impairment in patients with cirrhosis-NR. RBF enabled quantifying renal perfusion impairment in patients with cirrhosis-RD. The diagnostic algorithm based on cortical RBF and T1 values allowed detecting renal injury during cirrhosis.

Abstract:Objective Magnetic resonance (MR) relaxometry is an absolute and reproducible quantitative method, compared with signal intensity for the evaluation of liver biliary function. This is obtainable by the T1 reduction rate (T1RR), as it carries a smaller systematic error than the pre/post contrast agent T1 measurement. We aimed to develop and test an MR T1 relaxometry tool tailored for the evaluation of liver T1RR after gadolinium ethoxybenzyl-diethylenetriaminepentaacetic acid administration on 1.5T MR. Methods In vitro/vivo (liver) T1RR values with two 3D FLASH variable-flip-angle sequences were calculated by a MATLAB algorithm. In vitro measurements were done by 2 physicists, in consensus. The prospective in vivo study was approved by the local ethical committee and performed on 13 normal/26 cirrhotic livers. A supplemental test in 5 normal/5 cirrhotic livers, out of the studied series, was done to compare the results of our method (without B1 inhomogeneity correction) and those of a standardized commercial tool (with B1 inhomogeneity correction). All in vivo evaluations were performed by 2 radiologists with 7 years of experience in abdominal imaging. Open-source Java-based software ImageJ was used to draw the free-hand regions of interest on liver section and for the measurement of hepatic T1RR values. The T1RR values of each group of patients were compared to assess statistically significant differences. All statistical analyses were performed with IBM-SPSS Statistics. In vivo evaluations, the intrareader and interreader reliability was assessed by intraclass correlation coefficient. Results Our method showed good accuracy in evaluating in vitro T1RR with a maximum percentage error of 9% (constant at various time points) with T1 values in the 200- to 1400-millisecond range. In vivo, a high concordance between the T1RR evaluated with the proposed method and that calculated from the standardized commercial software was verified ( P < 0.05). The median T1RRs were 74.8, 67.9, and 52.1 for the normal liver, Child-Pugh A, and Child-Pugh B cirrhotic groups, respectively. A very good agreement was found, both within intrareader and interreader reliability, with intraclass correlation coefficient values ranging from 0.88 to 0.95 and from 0.85 to 0.90, respectively. Conclusions The proposed method allowed accurate reliable in vitro/vivo T1RR assessment evaluation of the liver biliary function after gadolinium ethoxybenzyl-diethylenetriaminepentaacetic acid administration.

Abstract:PurposeTo investigate the utility of dynamic contrast-enhanced MRI (DCE-MRI) with Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA) for detecting liver fibrosis induced by carbon tetrachloride (CCl4) in rats.MethodsThis study was approved by the institutional animal care and use committee. Liver fibrosis in rats was induced by intraperitoneal injection of 1 mL/kg 50% CCl4 twice a week for 4-13 weeks. Control rats were injected with saline. Liver fibrosis was graded using the Metaviar score: no fibrosis (F0), mild fibrosis (F1-F2) and advanced fibrosis (F3-F4). DCE-MRI with Gd-EOB-DTPA was performed for all rats. K-trans, K-ep, V-e and iAUC of the liver parenchyma were measured. Relative enhancement (RE) value of the liver was calculated on T-1-weighted images at 15, 20 and 25 min after Gd-EOB-DTPA administration.ResultsThirty-five rats were included: no fibrosis (n=13), mild fibrosis (n=11) and advanced fibrosis (n=11). K-trans and iAUC values were highest in advanced fibrosis group and lowest in no fibrosis group (P<0.05). The area under the receiver operating characteristic curve (AUROC) for fibrosis (stages F1 and greater) were 0.773 and 0.882 for K-trans and iAUC, respectively. AUROC for advanced fibrosis were 0.835 and 0.867 for K-trans and iAUC, respectively. K-ep and RE values were not able to differentiate fibrosis stages (all P>0.05).ConclusionK-trans and iAUC obtained from DCE-MRI with Gd-EOB-DTPA are useful for the detection and staging of rat liver fibrosis induced by CCl4.
