Abstract:BACKGROUND:Collateralized intra- and extra-hepatic routes in patients with Budd-Chiari syndrome (BCS) were important. This study aimed to investigate the feasibility and clinical outcomes of the staged management of BCS based on the degree of compensation provided by intra- or extra-hepatic collateral circulations.METHODS:A total of 103 adult patients with BCS caused by co-obstruction of the inferior vena cava (IVC) and main hepatic veins (MHVs) between March 2001 and October 2009 were enrolled in this study. Based on the pathological classification and degree of hemodynamic compensation by collateral circulations, treatment priority for IVC hypertension was determined in the first-stage treatment. Patients were deemed eligible for second-stage treatment when the first-stage treatment failed to relieve.RESULTS:Imaging results revealed that most patients had collateral circulations to different extents. Based on the degree of compensation provided by these collateral circulations, 74 patients underwent single-stage treatment for IVC hypertension, i.e., radiologic intervention (RI) for 61 patients and surgical procedures (SPs) for 13. One patient was treated for portal hypertension. Twenty-nine patients underwent second-stage treatment (25 underwent RI and SP, and 4 only SP). The general morbidity and mortality after all procedures were 8.3% and 1.5%, respectively. After a median follow-up of 35 months, 4 patients underwent second-stage treatment and 7 underwent recanalization of the IVC/MHVs. Two patients died of hepatocellular carcinoma and 1 died of graft obstruction.CONCLUSION:Staged management produces excellent outcomes for patients with BCS caused by co-obstruction of the IVC and MHVs.

Abstract:Introduction: Liver stiffness measurement (LSM) and spleen stiffness measurement (SSM) are increased in patients with cirrhosis and portal hypertension (PH). However, increased SSM values are associated with presence of esophageal varices. Currently, the gold standard for risk stratification in patients with compensated advanced chronic liver disease (cACLD) is by measurement of portal pressure through assessment of hepatic vein or porto-systemic pressure gradient (HVPG and PSPG). However, these procedures are invasive, and either LSM or SSM may be a non-invasive alternative. Aim: We aimed to investigate the relationship between LSM or SSM and HVPG/PSPG and examined the diagnostic accuracy for identification of clinically significant PH (≥10 mm Hg) or severe PH (≥12 mm Hg). Methods: A retrospective study (IRB Protocol #19230) allowed us to identify patients who had who undergone portal pressure measurements (HVPG or PSPG). A total of 36 patients also had SSM values. Of these, 29 patients carried a hepatic cause of PH, while the remaining had pre-hepatic (n=4) and post-hepatic (n=3) etiology. Results: The median age was 58 years (range: 25-71) with 52% male and 83% with cirrhosis. The median LSM and SSM were 35.7 kPa (range:7.4-75.0, normal ≤7.0 kPa) and 43.8 kPa (range: 20.7-100, normal ≤ 21.0 kPa). Both LSM (r = 0.42, P-value = 0.02) and SSM (r = 0.45, P-value: 0.01) correlated significantly with HVPG/PSPG. The diagnostic accuracy for both LSM and SSM for clinically significant PH was good for LSM (AUROC 0.76, 95% CI:0.58-0.94) and excellent for SSM (AUROC: 0.81, 95% CI:0.65-0.98). The diagnostic accuracy of LSM and SSM was lower for severe PH [AUROC 0.71 (P-value = 0.06) and 0.67 (P-value = 0.1) for LSM and SSM, respectively]. Conclusion: Both LSM and SSM correlate with portal pressure measurements and provide diagnostic accuracy to identify patients with clinically significant, but not severe, PH.

Abstract:Hepatic vein outflow obstruction causes congestion of the liver, leading to necrosis, fibrosis, and portal hypertension (PH). A transjugular intrahepatic portosystemic shunt (TIPS) reduces congestion and PH by providing artificial outflow. The aim of the study was to investigate fibrosis progression in patients with Budd–Chiari syndrome (BCS) and TIPS using transient elastography (TE). From 2010 to 2022, 25 patients received 80 TEs using FibroScan®, Echosens, Paris, France (3.2 ± 2.1 per patient). TIPS function was assessed via Doppler ultrasound or radiological intervention. At the time of TE examination, 21 patients had patent shunts. Four patients had occluded shunts but normal pressure gradients during the intervention. The first TE measurement performed 9.8 ± 6.8 years after the BCS diagnosis showed stiffness values of 24.6 ± 11.5 kPa. A second or last measurement performed 7.0 ± 2.9 years after the first measurement showed similar stiffness values of 24.1 ± 15.7 kPa (p = 0.943). Except for three patients, the liver stiffness was always >12 kPa, indicating advanced fibrosis. Stiffness values obtained 5 years after the BCS diagnosis (24.9 ± 12.7 kPa) did not differ (p = 0.907). In addition, stiffness was not related to the interval between BCS and TIPS implantation (p = 0.999). One patient received liver transplantation, and two patients died from non-hepatic causes. Most patients developed mild to moderate cirrhosis, possibly during the early phase of the disease. Timing of TIPS did not influence fibrosis progression. This and the release of portal hypertension may argue in favor of a generous TIPS implantation practice in patients with BCS.

Orientation of Movements and Habitat Selection in a Spatially Structured Population of Marbled Salamanders (Ambystoma opacum)

Abstract:Abstract Most studies on orientation of movements of pond-breeding salamanders have considered only a single local population (or breeding pond) during ≥1 yr, or multiple populations during a single year. We quantified migratory patterns of Marbled Salamanders at nine breeding ponds during 5 yr in western Massachusetts. Based on captures at drift fences, movements were nonuniform at all breeding ponds. In addition, the direction of orientation differed among breeding ponds and changed slightly across years. Within ponds, orientation of adults and juveniles differed significantly in 52% of comparisons, and adult movements were more directionally concentrated than those of juveniles. In addition, migrating salamanders shifted slightly the orientation of their movements as they traveled into uplands, suggesting that migration routes are spatially complex and that determination of migration “corridors” based on concentrated captures at the pond periphery may be misleading. Although salamanders used migration routes with higher canopy cover, our models did not explain a large portion of the variation in orientation, and protecting areas of high canopy cover alone may not be sufficient as a protection strategy. Our results suggest that movement routes, though perhaps concentrated in the short term, are unpredictable in the long term. Thus, we can offer little evidence that distinct corridors can be identified and protected that would be used consistently over time by migrating or dispersing Marbled Salamanders. Consequently, until we better understand mechanisms governing movements in this species, a conservative conservation strategy would require protecting broad terrestrial areas around breeding sites.

Abstract:Several guidelines have indicated that liver stiffness (LS) assessed by means of shear wave elastography (SWE) can safely replace liver biopsy in several clinical scenarios, particularly in patients with chronic viral hepatitis. However, an increase of LS may be due to some other clinical conditions not related to fibrosis, such as liver inflammation, acute hepatitis, obstructive cholestasis, liver congestion, infiltrative liver diseases. This review analyzes the role that SWE can play in cases of liver congestion due to right-sided heart failure, congenital heart diseases or valvular diseases. In patients with heart failure LS seems directly influenced by central venous pressure and can be used as a prognostic marker to predict cardiac events. The potential role of LS in evaluating liver disease beyond the stage of liver fibrosis has been investigated also in the hepatic sinusoidal obstruction syndrome (SOS) and in the Budd-Chiari syndrome. In the hepatic SOS, an increase of LS is observed some days before the clinical manifestations; therefore, it could allow an early diagnosis to timely start an effective treatment. Moreover, it has been reported that patients that were successfully treated showed a LS decrease, that reached pre-transplantation value within two to four weeks. It has been reported that, in patients with Budd-Chiari syndrome, LS values can be used to monitor short and long-term outcome after angioplasty.

Abstract:For patients with advanced chronic liver disease who are in a compensated state, the development of portal hypertension (PHT) can lead to a heightened risk of hepatic decompensation and mortality. This underscores the importance of timely and appropriate treatment to manage the condition and prevent further complications. The current gold standard procedure for determining PHT is the hepatic venous pressure gradient, but its invasiveness limits its usage in clinical practice and larger trials of novel agents. The current clinical demand for accurate, validated and non-invasive methods to assess the severity of PHT remains unmet. One potential non-invasive option is tissue elastography, which examines the elastic behaviour of tissue after a force has been applied. This method involves quantifying alterations in the biomechanical properties of the liver or spleen in patients with cirrhosis. Available methods are various, including transient elastography, shear wave elastography, acoustic radiation force impulse and magnetic resonance elastography. Importantly, the measurement of spleen stiffness appears to outperform liver stiffness as a direct and dynamic indicator of portal pressure, offering the potential to monitor PHT and evaluate improvements in PHT as a marker for clinical outcomes.

Abstract:Budd-Chiari syndrome (BCS) is a relatively rare disease, most commonly found in middle-aged females, and is caused by hepatic vein blockage, except in Asian countries where it is more commonly found in males with inferior vena cava (IVC) obstruction.[1] The condition is classified as either primary or secondary depending on the source of the obstruction and whether it originates from within the venous system, such as in thrombosis, or external to the venous system, such as in compression by tumors of adjacent organs or polycystic kidney disease. Primary BCS is the most common manifestation and is most prevalent in those with underlying thrombogenic disorders such as primary myeloproliferative disease, essential thrombocythemia, and myelofibrosis.[2] Conditions that result in hypercoagulability such as pregnancy, oral contraceptive use, factor V mutation, antithrombin deficiency, protein C deficiency, and paroxysmal nocturnal hemoglobinuria can also cause primary BCS.[3] However, in most cases, the local cause of the thrombus formation cannot be identified. BCS generally becomes clinically evident after the occlusion of two or more major hepatic veins ([Figs. 1] and [2]). This causes a build-up of sinusoidal pressure and increased filtration of interstitial fluid into the liver capsule after the capacity of lymphatic drainage capacity has been exceeded. Resulting portal hypertension causes reduced hepatic blood flow and hepatocyte damage.[2] BCS and its progression can range in severity from asymptomatic, unobtrusive disease to acute, debilitating disease with significant morbidity.[4] [5] When symptomatic, BCS can present with abdominal pain and/or distention, ascites, hepatomegaly, jaundice, upper gastrointestinal bleeding, hepatic encephalopathy, and edema.[4] In chronic cases, patients can also present with renal impairment.[1] Article published online: 14 March 2024 © 2024. Clinical Application of Liver Stiffness Measurement in Patients with Cavernous Transformation of Portal Vein

Abstract:The clinical outcomes differ between patients with cavernous transformation of the portal vein (CTPV) with and without cirrhosis. Therefore, invasive liver biopsy may be needed for the differential diagnosis of patients with CTPV with or without cirrhosis. The present study aimed to investigate the diagnostic efficacy of liver stiffness measurements (LSM) for the prediction of cirrhosis in patients with CTPV. A total of 20 patients with CTPV, 34 with chronic hepatitis B (CHB)-related cirrhosis and 20 healthy volunteers, were retrospectively recruited in the study. CTPV was diagnosed with contrast-enhanced computed tomography (CT) and ultrasound for the abdomen. LSM values were detected for each patient, while liver biopsy was performed in each patient in the CTPV and cirrhosis groups. The results demonstrated that LSM values were significantly lower in the CTPV group (12.5 kPa; range, 6.8-21.5 kPa) compared with the CHB-related cirrhosis group (21.0 kPa; range, 15.5-27.2 kPa; P=0.017). However, this was still higher compared with healthy volunteers (4.9 kPa; range 4.0-5.8 kPa; P<0.001). In addition, CTPV patients with cirrhosis (17.7 kPa; range, 13.9-30.8 kPa) exhibited significantly increased LSM values compared with those without cirrhosis (6.4 kPa; range, 5.7-7.8 kPa; P<0.001). Furthermore, LSM values in CTPV patients without cirrhosis were slightly higher compared with those of healthy volunteers (P=0.003), while no statistically significant difference was observed in LSM between CTPV patients with cirrhosis and CHB-related cirrhosis group. These findings indicated that LSM values could be used for the differential diagnosis of CTPV patients with or without cirrhosis. However, further validation studies are needed.

Abstract:Aim To assess the diagnostic accuracy of liver and spleen stiffness measured by 2-D shear-wave elastography (SWE) in evaluation of clinically significant and severe portal hypertension (CSPH and SPH, respectively). Methods Clinical data of 155 hepatitis B-related cirrhosis patients and their liver and spleen stiffness (L-SWE and S-SWE, respectively) were collected. The diagnostic performances of L-SWE, S-SWE, the liver stiffness-spleen diameter to platelet ratio score (LSPS) and portal hypertension risk score were evaluated. Results One hundred and four patients were eligible for analysis. Clinically significant and severe PH were detected in 84 and 74 patients, respectively. Liver and spleen stiffness were significantly correlated with hepatic venous pressure gradient in overall, CSPH, and SPH groups (r(L) = 0.607, 0.554, and 0.412; r(S) = 0.665, 0.566, and 0.467, respectively; all P < 0.05). The area under the receiver operating characteristic curves of L-SWE, S-SWE, LSPS, and PH risk score were 0.72 (95% confidence interval [CI], 0.49-0.95), 0.81 (95% CI, 0.55-0.97), 0.76 (95% CI, 0.51-0.96), and 0.73 (95% CI, 0.55-0.88) for CSPH, and 0.77 (95% CI, 0.51-0.93), 0.85 (95% CI, 0.59-0.96), 0.80 (95% CI, 0.58-0.98), and 0.80 (95% CI, 0.59-0.93) for SPH. The best cut-off of L-SWE for determining CSPH and SPH were 16.1 kPa (sensitivity, 78%; specificity, 72%) and 23.5 kPa (sensitivity, 81%; specificity, 79%). For S-SWE, the best cut-offs were 25.3 kPa (sensitivity, 85%; specificity, 79%) and 33.4 kPa (sensitivity, 74%; specificity, 70%). A cut-off of L-SWE <13.2 kPa or S-SWE <23.2 kPa was able to rule out CSPH, whereas a cut-off of L-SWE >24.9 kPa or S-SWE >34.2 kPa was able to rule in CSPH. Conclusions Liver and spleen stiffness measured by 2-D SWE are reliable and promising non-invasive parameters to assess CSPH and SPH.
Derivation of Splenic Stiffness in Subjects with Normal Liver Stiffness and No Vascular Liver Disease Using Transient Elastography with a Spleen-Dedicated Module: A Large Cross Sectional Study

Abstract:Background: Liver and splenic stiffness measurements (LSM and SSM) are useful to predict varices and clinical decompensation in cirrhosis. SSM values are highly variable and overlapping and no guidelines exists on what constitutes normal SSM, that might limit interpretation of results. Methods: Consecutive subjects with LSM < 6 kPa and reliable SSM (FibroScan630 Expert device with spleen-dedicated module) and no vascular liver disease were analysed for significant correlations of SSM values with age, sex, BMI, portal and splenic vein diameter, splenic diameter, liver fat and diabetes. Based on timeline of SSM, subjects were randomly assigned in 70:30 ratio into derivation [n = 502] and validation subset [n = 214]. Results: Of 7200 subjects with simultaneous reliable LSM and SSM, 715 fulfilled the selection criteria (mean age: 43.8 ± 12.8 years, 67.2% male, mean BMI-26.4 ± 4.5 kg/m2). The mean SSM was 22.6 ± 5.8 kPa (c10-c90 percentile range: 15.2-31.3 kPa) and followed the normal distribution curve. In the derivation subset, mean SSM for males was higher than female (23.06 ± 6.2 vs. 21.78 ± 5.93 kPa; p = 0.028). SSM value correlated with LSM (r = 0.454, p = 0.001). Mean SSM in subjects with LSM 3-4, 4.1-5 and 5.1-6 kPa were 21.7 ± 5.8, 22.27 ± 5.67 and 23.76 ± 5.88 kPa (p value = 0.001). There was no difference in SSM based on age, BMI, diabetes and liver fat on ultrasound. Above results hold true for subjects in validation subset. Conclusion: SSM range in subjects with normal LSM and no vascular liver disease using spleen-dedicated module varies from 15.2 to 31.3 kPa, values being higher in male and not affected by Age, BMI, spleen size, liver fat and diabetes. Our results may serve as reference point in evaluation of SSM in compensated advanced liver disease patients.
Enhancing liver cirrhosis varices and CSPH risk prediction with spleen stiffness measurement using 100-Hz probe

Abstract:Managing complications of liver cirrhosis such as varices needing treatment (VNT) and clinically significant portal hypertension (CSPH) demands precise and non-invasive diagnostic methods. This study assesses the efficacy of spleen stiffness measurement (SSM) using a 100-Hz probe for predicting VNT and CSPH, aiming to refine diagnostic thresholds. A retrospective analysis was conducted on 257 cirrhotic patients, comparing the diagnostic performance of SSM against traditional criteria, including Baveno VII, for predicting VNT and CSPH. The DeLong test was used for statistical comparisons among predictive models. The success rate of SSM@100 Hz was 94.60%, and factors related to SSM failure were high body mass index and small spleen volume or length. In our cohort, the identified SSM cut-off of 38.9 kPa, which achieved a sensitivity of 92% and a negative predictive value (NPV) of 98% for detecting VNT, is clinically nearly identical to the established Baveno threshold of 40 kPa. The predictive capability of the SSM-based model for VNT was superior to the LSM ± PLT model ( p = 0.017). For CSPH prediction, the SSM model notably outperformed existing non-invasive tests (NITs), with an AUC improvement and significant correlations with HVPG measurements (obtained from 49 patients), highlighting a correlation coefficient of 0.486 ( p < 0.001) between SSM and HVPG. Therefore, incorporating SSM into clinical practice significantly enhances the prediction accuracy for both VNT and CSPH in cirrhosis patients, mainly due to the high correlation between SSM and HVPG. SSM@100 Hz can offer valuable clinical assistance in avoiding unnecessary endoscopy in these patients.

Abstract:Introduction Clinically significant portal hypertension (CSPH) marks a critical step in the natural history of compensated advanced chronic liver disease (cACLD) and may lead to esophageal varices (EV). The Baveno VI criteria suggest liver stiffness measurement (LSM) and platelet count (PLT) for non-invasive identification of cACLD patients not requiring screening gastroscopy. Aim We investigated the accuracy of the novel 100 Hz vibration-controlled transient elastography-based spleen stiffness measurement (SSM) exam for the identification of EV in cACLD patients. Materials and Methods Retrospective study of Mainz, Vienna, Leuven, Rome and Palermo. Patients with cACLD of any etiology (LSM≥10kPa or histological F4 fibrosis), but without previous decompensation (bleeding, encephalopathy, ascites) were included. SSM and LSM were obtained using Fibroscan F630 ≤1 month within screening gastroscopy. Prediction performance between different SSM cut-offs with respect to the Baveno criteria (LSM>20kPa and/or PLT<150 G/L) were compared by logistic regression with 10-fold cross-validation, adjusted for age, gender, BMI, transaminases, INR, albumin, and bilirubin. Backward feature selection based on likelihood ratio test was applied to identify significant confounders. Performance was calculated by balanced accuracy (BA), specificity (SP) and sensitivity (SE). Wilcoxon test was used to evaluate significant performance improvement of SSM cut-offs with respect to Baveno criteria, or to signifcant confounders. Results 343 cACLD patients with a median age of 59 years (60.3% male) and NAFLD as the main etiology (51.3%) were included. 137 had EV with 49 high-risk EV (HR-EV), while median SSM, LSM and PLT were 40.5kPa, 21kPa and 139 G/L, respectively. The figure shows BAs at different SSM cut-offs, compared to Baveno (red line). The best overall performance with all-type EV was at SSM=60 kPa (BA=0.72, SP=0.86, SE=0.58); Baveno: BA=0.66, SP=0.86, SE=0.39. Comparing HR-EV vs. absence of EV, the best cut-off was at 50kPa (BA=0.71, SP=0.95, SE=0.47; Baveno: BA=0.56, SP=0.92, SE=0.29). These SSM thresholds significantly improved BA when significant confounders were considered. Conclusions The novel spleen-dedicated 100 Hz SSM is associated with presence of EV in cACLD patients. In both all-type EV and HR-EV, SSM showed better accuracy than the Baveno LSM-PLT criteria, achieving a better trade-off between SP and SE.

Abstract:Introduction and Objective The gold standard for the diagnosis of clinicallysignificant portal hypertension isrepresented by HVPG and Upper Gastrointestinalendoscopy is able to indirectlyassess the degree of portalhypertension through the evaluation of esophageal and/or gastric varices. Over the last 20 years, the introduction of non invasive tests (NITs) such asFibroscan has made it possibleto select patients with significantportal hypertension and therefore identify patients eligiblefor endoscopy. The BAVENO VII consensus identifies patientswho could benefit from endoscopy as those patientswith LSM ≥ 20 kPa assessed by TE and platelet count ≤ 150 × 109L. Moreover identifiesvalues of spleen stiffness to rulein and rule out portalhypertension such as SSM >50 kPa and SSM <21 kPa,respectively. Point-shear waveelastography and 2D-shear waveelastography have beenproposed as possible tools to non-invasively assess portalhypertension, but validation of the best cut-off is needed. Aim of this study is the evaluation of the role of 2D-Shear wave in non invasive diagnosis of portalhypertension. Methods From April 2022 to October 2023 we evaluated one hundred patients with chronichepatitis and cirrhosis (50 chronic hepatitis and 50 cirrhosis) whose baseline characteristics are presented in TABLE 1. All patients underwentabdominal ultrasound, liver and spleen stiffness (LS and SS) and liver and spleen 2D-shear wave(L2D-S and S2D-S); FIB-4 and the APRI score were alsocalculated. All cirrhotic patientsunderwent endoscopy to evaluate portal hypertension. Results The prevalence of varices in cirrhotic patients was50%. We found a statisticallysignificant correlation betwen LS and SS (p<0,001) and L2D-S and S2D-S (p<0,001) and the presence of varices, this was true also for APRI and FIB-4 (p<0,001), independently fromage, sex and BMI. We evaluated also the diagnostic performance for each individual test (Table 2), with an optimal cut-off 13.1 kPa and 12.5 kPa for LS and L2D-S (sensitivity 0.83 and 0.79, specificity 0.72 and 0.77, PPV 0.50 and 0.54, NPV 0.93 and 0.92 and AUROC 0.84 and 0.83 respectively), 45 kPa and 25 kPa for SS and S2D-S (sensitivity 0.77 and 0.91, specificity 0.89 and 0.66, PPV 0.69 and 0.47, NPV 0.92 and 0.96 and AUROC 0.88 and 0.80 respectively), 2.82 for FIB-4 (sensitivity 0.84, specificity 0.74, PPV 0.52, NPV 0.93 AUROC 0.80), 0.64 for APRI (sensitivity 0.72, specificity 0.70, PPV 0.44, NPV 0.88 AUROC 0.74). Each single test showed a low PPV for the prediction of oesophagealvarices (OV), however, when we combined the tests together wefound that the combination of LS and S2D-S showed a higher PPV for predicting OV (PPV 0.73; NPV 0.99). Conclusion We foundthat the combination of twotests (Liver stiffness and Spleen stiffness 2D-Shear wave) increases the probability of findingoesophageal varices and therefore can be useful for the non-invasive identification of patients with clinicallysignificant portalhypertension.

Abstract:Portal hypertension (PH) is a major complication of liver cirrhosis, as it predisposes to the development of serious clinical manifestations such as ascites, hepatic encephalopathy and variceal bleeding, aggravating the prognosis of patients. Hepatic vein pressure gradient (HVPG) is considered the reference method for the estimation of the presence and severity of PH, but this procedure is available only in specialized centers. Alternatively, many non-invasive methods have been proposed in order to substitute HVPG. Among them, liver stiffness measurement (LSM) has been widely used, as it has been shown to correlate well with HVPG, though this relationship seems to weaken in values of HVPG higher than 12 mmHg, the threshold of serious complications development. Several studies supported the use of spleen stiffness measurement (SSM) instead of LSM, anticipating to a more adequate assessment of this advanced stage of PH. The aim of this paper is to critically appraise and summarize the literature about the role of SSM as a predictive tool of liver decompensation and prognosis, highlighting the strengths and the potential limitations of the studies published so far.

Abstract:Non-selective β-blocker (NSBB) therapy is the treatment of choice for primary prophylaxis of cirrhotic patients with high-bleeding risk esophageal varices (HRV). The hemodynamic response to NSBB is assessed by the measurement of the hepatic venous pressure gradient (HVPG). Recently, liver and spleen stiffness measurements (LSM and SSM) were proposed as non-invasive surrogates of HVPG. We aimed to evaluate LSM and SSM changes for assessing hemodynamic response in these patients.

Abstract:Introduction: Liver stiffness measurement (LSM) using vibration-controlled transient elastography (VCTE) is being increasingly used as a screening tool to predict varices. Our aim was to test the utility of Baveno VII criteria and other combinations of LSM, platelet count (PC), and splenic stiffness measurement (SSM) to predict the presence of varices in a cohort of Sri Lankan patients with compensated advanced liver cell disease (cALCD). Methods: Consecutive patients with newly diagnosed Child-Pugh class A cALCD (non-viral, BMI<30) were recruited prospectively. They underwent gastroscopy. LSM and SSM were taken using vibration-controlled transient elastography (VCTE) (Echosens FibroScan 502 Touch; Echosens SA, Paris, France) by a single operator who was unaware of endoscopy findings. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of different Baveno VII criteria to predict the varices and different combinations of LSM, SSM, and PC were also explored. Results: One hundred and seventy-four individuals were recruited. The mean age was 61.4 ((95% CI: 59.7-62.8) years. A total of 110 individuals were males, and 106 had varices. Our results indicated that the three Baveno VII criteria had sensitivities of 61%, 63%, and 42%, and specificities of 79%, 77%, and 87% to predict varices. SSM>30kPa alone and in combination with LSM>15kPa had sensitivities of 81 and 75%, specificities of 72 and 83%, PPVs of 82 and 87%, NPVs of 71% and 67%, and accuracies of 78 and 78%, respectively, to predict varices. Conclusion: Baveno VII criteria had a low sensitivity but high specificity in predicting the presence of varices. However, SSM>30kPa alone or in combination with LSM>15kPa had better sensitivity, specificity, PPV, NPV, and accuracy in predicting varices.

Abstract:Objective: To assess the diagnostic accuracy of liver and spleen stiffness measured by two dimensional shear wave elastography (2D-SWE) in hepatitis B-related cirrhosis. Methods: The clinical data of fifty-eight hepatitis B-related cirrhosis patients were collected in Zhongshan Hospital, Fudan University from September 2017 to April 2018. Pearson's correlation analyses were used to assess the relationship between liver/spleen stiffness (L-SWE and S-SWE) and hepatic venous pressure gradient (HVPG), as well as the comparison with serological model. The SWE diagnostic performances of Liver (L-SWE), Spleen (S-SWE) were also evaluated. Results: Of all 58 patients, 47 were found HVPG ≥10 mmHg, diagnosed as clinically significant portal hypertension (CSPH) and severe portal hypertension (SPH), which patients are at increased risk of developing complications. Thirty-four patients were found HVPG≥12 mmHg, diagnosed as SPH, which patients were at increased risk of variceal bleeding. Moderate positive correlation was found between L-SWE and HVPG (r=0.42, P<0.01), and S-SWE were significantly correlated with HVPG (r=0.68, P<0.01), while serological models and HVPG were slightly correlated (r=0.36 and 0.28, all P<0.01). The area under the receiver operating characteristic curves of L-SWE, S-SWE and the combination for CSPH were 0.78, 0.88 and 0.89. When L-SWE was>12.86 kPa or S-SWE was>35.73 kPa, patients were at increased risk of developing complications. The area under the receiver operating characteristic curves for SPH were 0.68, 0.81 and 0.77 and the S-SWE had the highest specificity, so when S-SWE was>41.5 kPa, patients were at increased risk of variceal bleeding. Conclusion: L-SWE and S-SWE are reliable and promising non-invasive parameters to assess CSPH and SPH.
