Abstract:With increasing volume of cardiac implantable electronic devices in the last decade, the indications for device extraction have increased. Multidisciplinary collaboration between cardiothoracic surgeons, cardiac anesthesiologists, and cardiac electrophysiologists has been recognized as an essential pre-requisite in pre-procedural planning to limit complications from this inherently risky procedure. Fortunately, the tools and techniques have continued to evolve to make extraction safer and more effective. This article discusses traditional and non-traditional techniques for transvenous lead extraction in addition to retrieval of leadless pacemakers.
Preliminary Clinical Experience of Transveneous Extraction in Patients with Cardiovascular Implantable Electronic Device Infection

Abstract:Objective To summarize our clinical experience in transvenous lead extraction.Methods From Jan 2010 to Dec 2014,patients hospitalized in Nanjing Drum Tower Hospital with cardiovasicular implantable electrical device(CIED) infection were enrolled.Transvenous lead extraction were performed to compare clinical effects of different tools.Results A total of 187 patients were enrolled and 318 leads were removed.Averaged implantation duration was (82.0±26.4) months.After bacteriological examination,Pocket local infection in 142 cases,blood infection in 33 cases and infective endocarditis in 12 cases were confirmed.locking stylet、mechanical sheaths、Evolution mechanical dilator sheath and snare were used during operation.Three hundred and nine leads were removed successfully.Major complication in 6 cases,including pericardial tamponade,pulmonary embolism,pneumothorax and serious ventricular arrhythmia.No patientdied.Reimplantation were performed in 171 patients,168 cases in contralateral side.Conclusion The transvenoues lead extraction is effective in management of CIED infection.Reasonable tools choosing will increase the safety and efficacy.

Abstract:Objective To find out the risk factors of major complications with transvenous lead extraction involving cardiovascular implantable electronic devices.Methods All patients who underwent transvenous lead extraction in heart center of People's Hospital between Jan 1st 2012 and Dec 31st 2017 were included in this study.The patients were divided into complication group and control group.All medical records were reviewed and data pertaining to patient demographics,type of devices,dates from original device implantation,number of all total leads extracted,number of right atrial leads extracted,number of right ventricular leads extracted,number of implantable cardioverter defibrillator (ICD) leads extracted,left ventricular leads extracted,and extraction methods were recorded.Univariate and multivariate logistic regression analysis were used to identify,the risk factors for severe complications during transvenous extraction procedures.Results A total of 1 832 leads were removed from 922 patients during studying period.Among them,751 right atrial leads,917 right ventricular leads,74 left ventricular leads,and 90 ICD leads were removed.Sixteen severe complications occurred,including 12 tamponade,3 acute tricuspid valve regurgitation requiring emergency open-heart surgery,and 1 pulmonary embolism needing embolectomy.Both univariate and multivariable logistic regression analysis showed that the number of right atrial leads removed and the duration of initial device implantation were risk factors for serious complications (OR=3.056,95％CI 1.174-7.958,P=0.022;OR=1.102,95％CI 1.029-1.180,P=0.005,respectively).Conclusions Tamponade is the commonest major complication with lead extraction procedures.Number of right atrial leads and time after leads implantation are risk factors for severe perioperative complication of lead extraction.

Abstract:Objective Transveneous lead extraction is the basic therapy in the management of cardiovascular implantable electrical device (CIED) infection.In this report,we presented our initial experience in transvenous lead extraction and the method of tools-choosing.Methods From January 2007 to March 2014,the study was divided into two periods:period one (January 2007-Maych 2013),period two (June 2013-December 2014).1 032 leads in 501 patients were removed.The indications for extraction were based on the recommendations of the heart rhythm society.The success rate,complication,lead types and special tools,etc were compared.Results During the period one(Jan 2007-May 2013),509 leads in 252 patients were removed,in which atrial lead 189(37.1％),right ventricular lead 272(53.4％),left ventricular lead 24(4.7％),defibrillator coil 24(4.7％).Implantation duration was 27 (0.3-360.0)months,Success rate 98.9％,major complication in 6 cases,2 death (1.7％).During the period two(June 2013-December 2014),523 leads in 249 patients were extracted,in which atrial leads 201(38.4％),right ventricular leads 240(45.9％),left ventricular leads 35(6.7％),defibrillator coils 47(9.0％).Implantation duration was 38 (0.4-490.0)months.Success rate was 100％,major complication in 1 case,0 deaths.In period two,77 leads were extracted by Evolution mechanical dilator sheath(Cook Medical),major complication 1 case(1.3％).Four hundreds leads were extracted by Needle's Eye Snare(Cook Medical),major complication in 1 case (0.25％).Conclusion The transvenoues lead extraction is effective in management of CIED infection.It companied the risk of major complication.The reasonable tools-choosing will increase the safety and efficacy.Needle Eye Snare could be the primary choice in lead extraction.

Abstract:Transvenous lead extraction (TLE) is regarded as the first-line strategy for the management of complications associated with cardiac implantable electronic devices (CIEDs), when lead removal is mandatory. The decision to perform a lead extraction should take into consideration not only the strength of the clinical indication for the procedure but also many other factors such as risks versus benefits, extractor and team experience, and even patient preference. TLE is a procedure with a possible high risk of complications. In this paper, we present three clinical cases of patients who presented different indications of TLE and explain how the procedures were successfully performed. In the first clinical case, TLE was necessary because of device extravasation and suspicion of CIED pocket infection. In the second clinical case, TLE was necessary because occlusion of the left subclavian vein was found when an upgrade to cardiac resynchronization therapy was performed. In the last clinical case, TLE was necessary in order to remove magnetic resonance (MR) non-conditional leads, so the patient could undergo an MRI examination for the management of a brain tumor.

Abstract:Background: Cardiac implantable electronic device (CIED) infections associated with large, mobile vegetation adds to the complexity of lead extraction and is associated with significant patient morbidity and mortality. Objective: To show the feasibility of concomitant cardiovascular implantable electronic device extraction and vacuum-assisted removal of lead-related vegetations. Methods: This is a single-center retrospective case series of consecutive patients with persistent bacteremia, sepsis, or endocarditis despite medical therapy who have vegetations >2 cm and subsequently underwent immediate CIED lead extraction after debulking with vacuum-assisted suction. Results: Eight patients underwent successful removal of 17 leads immediately after debulking of vegetations with vacuum-assisted device suction. Debulking procedure was not successful in 1 patient due to inability to direct the vacuum suction device into proper position. There were no intraprocedure complications related to the vacuum-assisted debulking. One patient required open sternotomy for tear of the coronary sinus ostium related to extraction of a left ventricular pacing electrode. There was no mortality within 30 days of the procedure. Conclusions: Based upon these clinical results, it is feasible for patients with infected CIED systems that have large right-sided vegetations to undergo vacuum-assisted debulking then immediately followed by percutaneous CIED removal in whom surgical removal is considered high risk.

Abstract:Percutaneous closure of atrial septal defects in adults has emerged as an alternative to surgery. We report a sequela of such closure in a 16-year-old boy: embolization of the atrial septal defect occluder into the main pulmonary artery when the patient experienced an episode of intense coughing immediately after device deployment. We removed the device surgically and closed the atrial septal defect in a standard manner, with an autologous pericardial patch.

Abstract:Background Cardiac resynchronization therapy (CRT) device and coronary sinus (CS) lead extraction are required due to the occurrence of systemic infection, malfunction, or upgrade. Relevant research of CS lead extraction is rare, especially in developing countries because of the high cost and lack of specialized tools. We aimed to evaluate percutaneous extraction of CS leads by modified conventional techniques.Methods Of 200 patients referred for lead extraction from January 2007 to June 2011, 24 (12.0%) involved CS leads (24 CS leads). We prospectively analyzed clinical characteristics, optimized extraction techniques and feasibility of extraction.Results Complete procedural success was achieved in 23 patients (95.8%), and the clinical success in 24 patients (100.0%). The leading indication for CS lead extraction was infection (66.7%). Mean implant duration was (29.5 +/- 20.2) months (range, 3-78 months). Sixteen CS leads (66.6%) were removed with locking stylets plus manual traction by superior transvenous approach. Mechanical dilatation and counter-traction was required to free fibrotic adhesions and extract 4 CS leads (16.7%), which had longer implant duration than other leads ((62.5 +/- 12.3) vs. (22.9 +/- 14.1) months, P<0.05). Another 4 CS (16.7%) leads were removed by modified and innovative snare techniques from femoral vein approach. Median extraction time was 11 minutes (range, 3-61 minutes) per CS lead, which had significant correlation with implant duration (r = 0.8, P<0.001). Sixteen patients (66.6%) were reimplanted with new devices at a median of 7.5 days after extraction. Median followed-up was 23.5 months (range, 8-61 months), three patients died due to sudden cardiac death (26 months), heart failure (45 and 57 months, respectively).Conclusion The modified procedure was proved to be practical for percutaneous extraction of CS leads, especially in developing countries lacking expensive powered sheaths. Abstract:Introduction and objectives: There is an increasing need for endocardial pacing and defibrillators leads to be removed. However, the procedure can be complex and it is not risk-free. We reviewed our experience between April 1989 and June 2006 with the percutaneous extraction of leads. Methods: In total, 314 electrodes were extracted from 187 patients. The leads had been implanted over an average period of 69.16 months (range 0.11-234.6 months, median 60.25 months). Some 115 were atrial leads, 196 were ventricular, and three were in the coronary veins; of these, 78 had been abandoned in the vascular bed. Results: Indications for removal were infection (26.1%), dysfunction (22.9%), erosion (25%), endocarditis (20.7%), and bacteremia (2.7%). Overall, 58.8% of patients were referred from other departments. In 96.8%, the electrodes were completely removed. Simple traction was used in 23.4%, and countertraction techniques (with and without radiofrequency current support) were used in 60.7%. For abandoned leads, a biopsy clamp was used in combination with countertraction (4.3%) or a femoral approach with a snare (10.1%). A sternotomy was required in three of the 10 patients with remaining electrode fragments. The complication rate was 4.6% (with major complications in 2.5%). Complications were associated with age < 60 years (odds ratio [OR]=5.38, 95% confidence interval [CI] 1.07-27.23), the presence of endocarditis (OR=4.97: 95%CI, 1.04-23.70), and right side implantation (OR=17.09; 95% CI, 2.15-135.70). Conclusions: In the majority of cases, pacing and defibrillator leads can be removed without difficulty using modern extraction techniques. However, because there is a risk of complications during extraction, even though it is low, the procedure should be carried out in specialized centers with surgical facilities.

Abstract:Background: Infections of cardiac implantable devices (CIEDI) have poor outcomes despite improvement in lead extraction (TLE) procedures. Methods: To explore the influence of CIEDI on the outcomes of TLE and the differences between patients with systemic (Sy) vs. local (Lo) CIEDI, we performed a sub-analysis of the EORP ELECTRa (European Lead Extraction ConTRolled) Registry. Results: Among 3555 patients enrolled by 73 centers in 19 Countries, the indication for TLE was CIEDI in 1850: 1170 with Lo-CIEDI and 680 with Sy-CIEDI. Patients with CIEDI had a worse in-hospital prognosis in terms of major complications (3.57% vs. 1.71%; p = 0.0007) and mortality (2.27% vs. 0.49%; p < 0.0001). Sy-CIEDI was an independent predictor of in-hospital death (H.R. 2.14; 95%CI 1.06–4.33. p = 0.0345). Patients with Sy-CIEDI more frequently had an initial CIED implant and a higher prevalence of comorbidities, while subjects with Lo-CIEDI had a higher prevalence of previous CIED procedures. Time from signs of CIEDI and TLE was longer for Lo-CIEDI despite a shorter pre-TLE antibiotic treatment. Conclusions: Patients with CIEDI have a worse in-hospital prognosis after TLE, especially for patients with Sy-CIEDI. These results raise the suspicion that in a relevant group of patients CIEDI can be systemic from the beginning without progression from Lo-CIEDI. Future research is needed to characterize this subgroup of patients.

Abstract:Following the removal of cardiovascular implantable electronic devices (CIEDs), reassessment of the need for a new device is vital. Some patients may have exhibited an improvement in rhythm or cardiac function and may thus no longer meet the guideline requirements for reimplantation. However, the long-term outcomes of non-systematic device reimplantation remain unknown. In the present study, it was hypothesized that the implantation of pacing systems in selected patients following lead extraction is safe. In order to confirm this hypothesis, a total of 854 patients (aged between 28 and 82 years) who underwent the removal of a CIED were enrolled in the present study and they were all reassessed to determine whether a new device following lead extraction was necessary. In order to determine which patients would undergo non-systematic device reimplantation, the standard guidelines, the criteria and the wishes of the patient were all taken into consideration. Patients remained device-free unless an adverse clinical event occurred that required reimplantation. The primary study endpoint was the rate of sudden death or reimplantation. Between January, 2014 and December, 2019, 854 consecutive patients underwent pacing system extraction, of whom 210 patients (24.6%) underwent non-systematic device reimplantation following careful reassessment (the non-reimplantation group). Among the 210 patients, 162 (77.1%) were fitted with pacemakers, 26 (12.4%) underwent cardiac resynchronization therapy or cardiac resynchronization therapy-defibrillator and 22 (10.5%) were implanted with a cardioverter-deﬁbrillator. During a mean follow-up period of 40.4 months, 86 patients reached the primary endpoint of the study, including 54 out of 210 patients (25.7%) who experienced an adverse clinical event that required reimplantation and 32 out of 210 patients (15.2%) who experienced sudden death. Reimplantation of a new device was not required in ~25% of the patients. On the whole, the present study demonstrates that following pacing system removal, non-systematic device reimplantation associated with close surveillance is safe for selected patients.

Abstract:Abstract Background With the expanding use of cardiac implantable electronic device (CIEDs) in older populations with more complicated conditions have bring about higher rates of CIED infections. The recommended treatment of which involves the complete removal of all hardware, followed by antibiotic therapy and re-implant. Application of transoesophageal echocardiography (TEE) helps improving efficacy and safety by capacitating the operators to perform the procedure, guiding them to better plans and to rapidly recognize and manage relative complications. Case presentation : We report four cases of CIED infections from single-centre, who were extracted endocardial leads with intra-op TEE monitoring and guidance. Three of them had three fatal complications as pulmonary embolism, pericardial tamponade and tricuspid trauma. The other one was found a massive vegetation detected by TEE not by pre-op TTE, which avoided intra-op embolism. Conclusions Continuously intra-op TEE should be one of the routine monitoring methods in these high-risk endocardial leads extraction procedures.

Abstract:Background: Complications associated with cardiovascular implantable electronic devices may necessitate device and lead removal. An open approach to removal may be electively chosen in cases with high risk of complications or those requiring additional concomitant cardiac surgery. This study aimed to investigate outcomes of patients who underwent elective open lead extractions (OLE) at two large tertiary care centers. Methods: The records of 29 patients undergoing elective OLE were analyzed through retrospective chart review. Results: 69 total leads were extracted from 29 patients (77% completely, 23% partially). The average age of the oldest leads was 13.3 ± 11.3 years. Infective endocarditis with severe valvular insufficiency requiring valvular intervention (41%)-an infectious etiology, and tricuspid valve intervention to correct RV lead-related severe TR (38%)-a noninfectious etiology, were the most common reasons for OLE. 38% of the patients had additional co-primary or secondary indications for open extraction, such as CABG and pericardiectomies. The rate of major complications and procedural failure was 3% each (1/29). 30-day survival was 100%, and 1-year survival was 92%. The average length of hospital stay was 15 days and higher among those undergoing OLE for infectious indications. Conclusion: Open lead extractions offered a similar clinical success rate (97%) to transvenous extractions in this cohort and may be a viable alternative for those necessitating valvular intervention or when the risk of complications from TLE is considered very high.
