Abstract:A three-dimensional motion model has been developed that estimates loads on the lumbar spine under laboratory conditions that simulate manual materials handling conditions. Eleven subjects experienced spinal loading during an experiment in which conditions of trunk velocity, trunk torque output, and trunk asymmetric posture were varied in a series of isokinetic velocity trunk extensions. The electromyographic activity of 10 trunk muscles, subject anthropometry, and trunk kinetics were used as input to a biomechanical simulation model described in Part I of this study. The model calculated estimates of compression, shear, and torsion loading in the lumbar spine, as well as the torque production of the trunk, continuously throughout the exertion. Trunk torque estimates derived from this model were compared with measured trunk torque. The effects of trunk motion, posture, and torque level on spine loading as estimated by the model are discussed. It was concluded that this approach provides a straightforward means of assessing loading of the spine attributable to laboratory simulations of workplace conditions.

Abstract:The kinetic demands of the spine can be assessed using a top-down (TD) or bottom-up (BU) approach, which start calculations from the either the hands or from the feet, respectively. Biomechanists have traditionally favored a BU approach, though existing modeling approaches encourage a TD approach. Regardless of the approach the demands should be similar, provided the external forces and linked segment parameters are equivalently measured and modeled. Demonstrating a level of agreement between the two approaches can help evaluate a model. Further, having both approaches can be advantageous when data is inaccurate or unavailable for one. The purpose of this study was to compare the internal moments and forces at multiple lumbar and thoracic intervertebral joint (IVJ) levels during lifting tasks from an established OpenSim thoracolumbar spine model that applies a TD approach and a similar model modified to adopt a BU approach. Kinematics and external forces were recorded from twelve participants during sagittal and lateral lifts of different lifting speeds and crate masses. For both approaches IVJ kinetics were estimated using a standard OpenSim modeling pipeline. The BU and TD approach IVJ joint moments generally agreed both temporally (R 2 = .94 ± .17) and in magnitude (RMSE=6.2 ± 3.5 Nm) of the primary planes of movement. There were however some temporal fit exceptions for off axes moments with low magnitudes (i.e., < 10 Nm). Bland-Altman plots also indicated acceptable agreement for IVJ peak forces (BU-TD difference of 12 ± 111 and 8 ± 31 N in compression and resultant shear, respectfully). These results support the application of the BU approach and the assigned linked segment parameters of the model. Musculoskeletal models determine the effect of a soft active exosuit on muscle activations and forces during lifting and lowering tasks

Abstract:Exosuits have the potential to mitigate musculoskeletal stress and prevent back injuries during industrial tasks. This study aimed to 1) validate the implementation of a soft active exosuit into a musculoskeletal model of the spine by comparing model predicted muscle activations versus corresponding surface EMG measurements, and 2) evaluate the effect of the exosuit on peak back and hip muscle forces. Fourteen healthy participants performed squat and stoop lift and lower tasks with boxes of 6 and 10 kg, with and without wearing a 2.7 kg soft active exosuit. Participant-specific musculoskeletal models, which included the exosuit, were created in OpenSim. Model validation focused on the back and hip extensors, where temporal agreement between EMG and model estimated muscle activity was generally strong to excellent (average cross-correlation coefficients ranging from 0.84 to 0.98). Root mean square errors of muscle activity (0.05-0.10) were similar with and without the exosuit, and compared well to prior model validation studies without the exosuit (average root mean square errors ranging from 0.05 to 0.19). In terms of performance, the exosuit reduced the estimated peak erector spinae forces during lifting and lowering phases across all lifting tasks but reduced peak hip extensor muscles forces only in a squat lift task of 10 kg. These reductions in total peak muscle forces were approximately 1.7-4.2 times greater than the corresponding exosuit assistance force, which were 146 ± 19 N and 102 ± 14 N at the times of peak erector spinae forces in lifting and lowering, respectively. Overall, the results support the hypothesis that exosuits reduce soft tissue loading, and thereby potentially reduce fatigue and injury risk during manual materials handling tasks. Incorporating exosuits into musculoskeletal models is a valid approach to understand the impact of exosuit assistance on muscle activity and forces.

Abstract:Currently available musculoskeletal inverse-dynamics thoracolumbar spine models are entirely based on data from adults and might therefore not be applicable for simulations in children and adolescents. In addition, these models lack lower extremities, which are required for comprehensive evaluations of functional activities or therapeutic exercises. We therefore created OpenSim-based musculoskeletal full-body models including a detailed thoracolumbar spine for children and adolescents aged 6-18 years and validated by comparing model predictions to in vivo data. After combining our recently developed adult thoracolumbar spine model with a lower extremity model, children and adolescent models were created for each year of age by adjusting segmental length and mass distribution, center of mass positions and moments of inertia of the major body segments as well as sagittal pelvis and spine alignment based on literature data. Similarly, muscle strength properties were adjusted based on CT-derived cross-sectional area measurements. Simulations were conducted from in vivo studies reported in the literature involving children and adolescents evaluating maximum trunk muscle strength (MTMS), lumbar disc compressibility (LDC), intradiscal pressure (IDP) and trunk muscle activity (MA). Model predictions correlated highly with in vivo data (MTMS: r>=0.82, p<=0.03; LDC: r=0.77, p<0.001; IDP: r>=0.78, p<0.001; MA: r>=0.90, p<0.001), indicating suitability for the reasonably accurate prediction of maximal trunk muscle strength, segmental loading and trunk muscle activity in children and adolescents. When aiming at investigating children or adolescents with pathologies such as idiopathic scoliosis, our models can serve as a basis for the creation of deformed spine models and for comparative purposes.

Abstract:The aim of this study was to append a passive soft back exoskeleton to a validated musculoskeletal model and assess its effectiveness in reducing lumbar loads. Fifteen participants lifted a box, with and without wearing a CORFOR ® exoskeleton. A full body OpenSim model was used to estimate lumbar joint moments and reaction forces, as well as low back muscles forces. Wearing the exoskeleton reduced the peak flexion moment, muscles forces, as well as peak compressive and shear forces. This musculoskeletal modelling study shows that wearing the exoskeleton may reduce lumbar spine loads and may contribute to prevent low back disorders.

Abstract:Due to lack of reference validation data, the common strategy in characterizing adolescent idiopathic scoliosis (AIS) by musculoskeletal modelling approach consists in adapting structure and parameters of validated body models of adult individuals with physiological alignments. Until now, only static postures have been replicated and investigated in AIS subjects. When aiming to simulate trunk motion, two critical factors need consideration: how distributing movement along the vertebral motion levels (lumbar spine rhythm), and if neglecting or accounting for the contribution of the stiffness of the motion segments (disc stiffness). The present study investigates the effect of three different lumbar spine rhythms and absence/presence of disc stiffness on trunk muscle imbalance in the lumbar region and on intervertebral lateral shear at different levels of the thoracolumbar/lumbar scoliotic curve, during simulated trunk motions in the three anatomical planes (flexion/extension, lateral bending, and axial rotation). A spine model with articulated ribcage previously developed in AnyBody software and adapted to replicate the spinal alignment in AIS subjects is employed. An existing dataset of 100 subjects with mild and moderate scoliosis is exploited. The results pointed out the significant impact of lumbar spine rhythm configuration and disc stiffness on changes in the evaluated outputs, as well as a relationship with scoliosis severity. Unfortunately, no optimal settings can be identified due to lack of reference validation data. According to that, extreme caution is recommended when aiming to adapt models of adult individuals with physiological alignments to adolescent subjects with scoliotic deformity.

Abstract:In computational biomechanics, two separate types of models have been used predominantly to enhance the understanding of the mechanisms of action of the lumbosacral spine (LSS): Finite element (FE) and musculoskeletal multibody (MB) models. To combine advantages of both models, hybrid FE-MB models are an increasingly used alternative. The aim of this paper is to develop, calibrate, and validate a novel passive hybrid FE-MB open-access simulation model of a ligamentous LSS using ArtiSynth. Based on anatomical data from the Male Visible Human Project, the LSS model is constructed from the L1-S1 rigid vertebrae interconnected with hyperelastic fiber-reinforced FE intervertebral discs, ligaments, and facet joints. A mesh convergence study, sensitivity analyses, and systematic calibration were conducted with the hybrid functional spinal unit (FSU) L4/5. The predicted mechanical responses of the FSU L4/5, the lumbar spine (L1-L5), and the LSS were validated against literature data from in vivo and in vitro measurements and in silico models. Spinal mechanical responses considered when loaded with pure moments and combined loading modes were total and intervertebral range of motions, instantaneous axes and centers of rotation, facet joint contact forces, intradiscal pressures, disc bulges, and stiffnesses. Undesirable correlations with the FE mesh were minimized, the number of crisscrossed collagen fiber rings was reduced to five, and the individual influences of specific anatomical structures were adjusted to in vitro range of motions. Including intervertebral motion couplings for axial rotation and nonlinear stiffening under increasing axial compression, the predicted kinematic and structural mechanics responses were consistent with the comparative data. The results demonstrate that the hybrid simulation model is robust and efficient in reproducing valid mechanical responses to provide a starting point for upcoming optimizations and extensions, such as with active skeletal muscles.

Abstract:This study assessed the accuracy of L5/S1 moment estimates calculated with an Inertial Motion Capture (IMC) system during an asymmetrical and variable height lifting task. The effects of load weight, asymmetry, and lifting height on estimates of lumbar moment have not been comprehensively considered in studies using IMC systems. Thirty-six participants engaged in tasks involving three loads, lifting heights, and trunk rotation angles. Lumbar moments were calculated using bottom-up and top-down biomechanical models. Gold-standard Optical Motion Capture (OMC) and Force Plates (FP) were used as the reference. A randomized block partially confounded design was used to compare the root mean square errors (RMSE) between the IMC and OMC-based reference estimates. The IMC system's estimated peak moments were 12%–13% lower than those estimated using the gold standard OMC-BU inverse dynamics, while the RMSE varied between 19 and 21 Nm. A Load*Height interaction was found; a trend was identified where the RMSE values increased as both the load and height levels increased. The angle did not show a significant effect on any of the tested scenarios. A close correspondence between the IMC and OMC-based moment estimates was established, with the load being the main factor affecting the differences between systems. The IMC system shows potential for use in occupational settings to capture data on the lumbar moments of workers, which could be utilized to assess ergonomic risk.

Abstract:Musculoskeletal (MSK) models offer great potential for predicting the muscle forces required to inform more detailed simulations of vertebral endplate loading in adolescent idiopathic scoliosis (AIS). In this work, simulations based on static optimization were compared with in vivo measurements in two AIS patients to determine whether computational approaches alone are sufficient for accurate prediction of paraspinal muscle activity during functional activities. We used biplanar radiographs and marker-based motion capture, ground reaction force, and electromyography (EMG) data from two patients with mild and moderate thoracolumbar AIS (Cobb angles: 21° and 45°, respecitvely) during standing while holding two weights in front (reference position), walking, running, and object lifting. Using a fully automated approach, 3D spinal shape was extracted from the radiographs. Geometrically personalized OpenSim-based MSK models were created by deforming the spine of pre-scaled full-body models of children/adolescents. Simulations were performed using an experimentally controlled backward approach. Differences between model predictions and EMG measurements of paraspinal muscle activity (both expressed as a percentage of the reference position values) at three different locations around the scoliotic main curve were quantified by root mean square error (RMSE) and cross-correlation (XCorr). Predicted and measured muscle activity correlated best for mild AIS during object lifting (XCorr's ≥ 0.97), with relatively low RMSE values. For moderate AIS as well as the walking and running activities, agreement was lower, with XCorr reaching values of 0.51 and comparably high RMSE values. This study demonstrates that static optimization alone seems not appropriate for predicting muscle activity in AIS patients, particularly in those with more than mild deformations as well as when performing upright activities such as walking and running.

Estimating Compressive and Shear Forces at L5-S1: Exploring the Effects of Load Weight, Asymmetry, and Height Using Optical and Inertial Motion Capture Systems

Abstract:This study assesses the agreement of compressive and shear force estimates at the L5-S1 joint using inertial motion capture (IMC) within a musculoskeletal simulation model during manual lifting tasks, compared against a top-down optical motion capture (OMC)-based model. Thirty-six participants completed lifting and lowering tasks while wearing a modified Plug-in Gait marker set for the OMC and a full-body IMC set-up consisting of 17 sensors. The study focused on tasks with variable load weights, lifting heights, and trunk rotation angles. It was found that the IMC system consistently underestimated the compressive forces by an average of 34% (975.16 N) and the shear forces by 30% (291.77 N) compared with the OMC system. A critical observation was the discrepancy in joint angle measurements, particularly in trunk flexion, where the IMC-based model underestimated the angles by 10.92–11.19 degrees on average, with the extremes reaching up to 28 degrees. This underestimation was more pronounced in tasks involving greater flexion, notably impacting the force estimates. Additionally, this study highlights significant differences in the distance from the spine to the box during these tasks. On average, the IMC system showed an 8 cm shorter distance on the X axis and a 12–13 cm shorter distance on the Z axis during lifting and lowering, respectively, indicating a consistent underestimation of the segment length compared with the OMC system. These discrepancies in the joint angles and distances suggest potential limitations of the IMC system's sensor placement and model scaling. The load weight emerged as the most significant factor affecting force estimates, particularly at lower lifting heights, which involved more pronounced flexion movements. This study concludes that while the IMC system offers utility in ergonomic assessments, sensor placement and anthropometric modeling accuracy enhancements are imperative for more reliable force and kinematic estimations in occupational settings.

Lumbar spine finite element model for healthy subjects: development and validation

Abstract:Finite element (FE) method is a proven powerful and efficient tool to study the biomechanics of the human lumbar spine. However, due to the large inter-subject variability of geometries and material properties in human lumbar spines, concerns existed on the accuracy and predictive power of one single deterministic FE model with one set of spinal geometry and material properties. It was confirmed that the combined predictions (median or mean value) of several distinct FE models can be used as an improved prediction of behavior of human lumbar spine under identical loading and boundary conditions. In light of this improved prediction, five FE models (L1-L5 spinal levels) of the human lumbar spine were developed based on five healthy living subjects with identical modeling method. The five models were extensively validated through experimental and computational results in the literature. Mesh convergence and material sensitivity analysis were also conducted. We have shown that the results from the five FE models developed in this paper were consistent with the experimental data and simulation results from the existing literature. The validated modeling method introduced in this study can be used in modeling dysfunctional lumber spines such as disc degeneration and scoliosis in future work.

An artificial neural network for full-body posture prediction in dynamic lifting activities and effects of its prediction errors on model-estimated spinal loads

Abstract:Musculoskeletal models have indispensable applications in occupational risk assessment/management and clinical treatment/rehabilitation programs. To estimate muscle forces and joint loads, these models require body posture during the activity under consideration. Posture is usually measured via video-camera motion tracking approaches that are time-consuming, costly, and/or limited to laboratories. Alternatively, posture-prediction tools based on artificial intelligence can be trained using measured postures of several subjects performing many activities. We aimed to use our previous posture-prediction artificial neural network (ANN), developed based on many measured static postures, to predict posture during dynamic lifting activities. Moreover, effects of the ANN posture-prediction errors on dynamic spinal loads were investigated using subject-specific musculoskeletal models. Seven individuals each performed twenty-five lifting tasks while their full-body three-dimensional posture was measured by a 10-camera Vicon system and also predicted by the ANN as functions of the hand-load positions during the lifting activities. The measured and predicted postures (i.e., coordinates of 39 skin markers) and their model-estimated L5-S1 loads were compared. The overall root-mean-squared-error (RMSE) and normalized (by the range of measured values) RMSE (nRMSE) between the predicted and measured postures for all markers/tasks/subjects was equal to 7.4 cm and 4.1 %, respectively (R2 = 0.98 and p < 0.05). The model-estimated L5-S1 loads based on the predicted and measured postures were generally in close agreements as also confirmed by the Bland-Altman analyses; the nRMSE for all subjects/tasks was < 10 % (R2 > 0.7 and p > 0.05). In conclusion, the easy-to-use ANN can accurately predict posture in dynamic lifting activities and its predicted posture can drive musculoskeletal models.

Abstract:Low back pain is one of the most common musculoskeletal disorder. To reduce its incidences, some back exoskeletons have been designed and already commercialized. However, there is a gap between the phases of device and testing on subjects. In fact, the main unsolved problem is the lack of realistic simulation of human-device interaction. Consequently, the objective of this paper is to design a 3D multibody model of the human body that includes the full thoracic and lumbar spine combined with a low back exoskeleton, enabling to analyze the interactions between them. The results highlight that the use of the exoskeleton reduces the net torque in the lower lumbar spine but creates normal forces transmitted through the thighs and the pelvis, which should also be considered for low back pain. As a perspective, this model would enable to redesign low back exoskeletons reducing both torques and forces in the human joints in a realistic dynamic context.
Towards Wearable Electromyography for Personalized Musculoskeletal Trunk Models using an Inverse Synergy-based Approach

Abstract:Electromyography (EMG)-driven musculoskeletal models (EMS) of the trunk are used for estimating lumbosacral joint moments and compressive loads during lifting tasks. These models provide personalized estimates of the parameters using information from many sensors. However, to advance technology from labs to workplaces, there is a need for sensor reduction to improve wearability and applicability. Therefore we introduce an EMG sensor reduction approach based on inverse synergy extrapolation, to reconstruct unmeasured EMG signals for different box-lifting techniques. 12 participants performed an array of tasks (squat, stoop, unilateral twist and bilateral twist) with different weights (0 kg, 7.5 kg and 15 kg). We found that two synergies were sufficient to explain the different lifting tasks (median variance accounted for of 0.91). Building upon this, we used two sensors at optimal subject-specific muscle locations to reconstruct the EMG of four unmeasured channels. Evaluation of the reconstructed and reference EMG showed median coefficients of determination (R ) between 0.70 and 0.86, with median root mean squared errors (RMSE) ranging from 0.02 to 0.04 relative to maximal voluntary contraction. This indicates that our proposed method shows promise for sensor reduction for driving a trunk EMS for ambulatory biomechanical risk assessment in occupational settings and exoskeleton control.

Abstract:This study aimed to establish an ambulatory field-friendly system based on miniaturised wireless flexible sensors for studying the biomechanics of human-exoskeleton interactions. Twelve healthy adults performed symmetric lifting with and without a passive low-back exoskeleton, while their movements were tracked using both a flexible sensor system and a conventional motion capture (MoCap) system synchronously. Novel algorithms were developed to convert the raw acceleration, gyroscope, and biopotential signals from the flexible sensors into kinematic and dynamic measures. Results showed that these measures were highly correlated with those obtained from the MoCap system and discerned the effects of the exoskeleton, including increased peak lumbar flexion, decreased peak hip flexion, and decreased lumbar flexion moment and back muscle activities. The study demonstrated the promise of an integrated flexible sensor-based system for biomechanics and ergonomics field studies as well as the efficacy of exoskeleton in relieving the low-back stress associated with manual lifting. This study established and tested a flexible sensor-based ambulatory system for biomechanical evaluation of human-exoskeleton interactions and as a promising new tool for field ergonomics studies in practical or naturalistic settings. Twente Spine Model: A thorough investigation of the spinal loads in a complete and coherent musculoskeletal model of the human spine

Abstract:Although in vivospinal loads have been previously measured, existing data are limited to certain lumbar and thoracic levels. A detailed investigation of spinal loads would assist with injury prevention and implant design but is unavailable. In this study, we developed a complete and coherent musculoskeletal model of the entire human spine and studied the intervertebral disc compression forces for physiological movements on three anatomical planes. This model incorporates the individual vertebrae at the cervical, thoracic, and lumbar regions, a flexible ribcage, and complete muscle anatomy. Intradiscal pressures were estimated from predicted compressive forces, and these were generally in close agreement with previously measured data. We found that compressive forces at the trunk discs increased during trunk lateral bending and axial rotation of the trunk. During flexion, compressive forces increased in the thoracolumbar and lumbar regions and slightly decreased at the middle thoracic discs. In extension, the forces generally decreased at the thoracolumbar and lumbar discs whereas they slightly increased at the upper and middle thoracic discs. Furthermore, similar to a previous biomechanical model of the cervical spine, our model predicted increased compression forces in neck flexion, lateral bending, and axial rotation, and decreased forces in neck extension.
Trunk Hybrid Passive-Active Musculoskeletal Modeling to Determine the Detailed T12-S1 Response Under In Vivo Loads

Abstract:Biomechanical models of the spine either simplify intervertebral joints (using spherical joints or deformable beams) in musculoskeletal (MS) or overlook musculature in geometrically-detailed passive finite element (FE) models. These distinct active and passive models therefore fail to determine in vivo stresses and strains within and load-sharing among the joint structures (discs, ligaments, and facets). A novel hybrid active-passive spine model is therefore developed in which estimated trunk muscle forces from a MS model for in vivo activities drive a mechanically-equivalent passive FE model to quantify in vivo T12-S1 compression/shear loads, intradiscal pressures (IDP), centers of rotation (CoR), ligament/facet forces, and annulus fiber strains. The predicted and in vivo L4-L5 IDP and L1-S1 CoRs showed satisfactory agreements. The FE model under commonly-used in vitro loading (pure moments and follower loads) predicted different kinetics from those of the hybrid model under in vivo loads (muscle exertions and gravity loads) contributing to suggest the inadequacy of such in vitro loads when simulating in vivo tasks. For an improved assessment of the injury risk, evaluation of the internal loads, and design of implants, such hybrid models should therefore be used.

Abstract:The study of the spine range of motion under given external load has been the object of many studies in literature, finalised to a better understanding of the spine biomechanics, its physiology, eventual pathologic conditions and possible rehabilitation strategies. However, the huge amount of experimental work performed so far cannot be straightforwardly analysed due to significant differences among loading set-ups. This work performs a meta-analysis of various boundary conditions in literature, focusing on the flexion/extension behaviour of the lumbar spine. The comparison among range of motions is performed virtually through a validated multibody model. Results clearly illustrated the effect of various boundary conditions which can be met in literature, so justifying differences of biomechanical behaviours reported by authors implementing different set-up: for example, a higher value of the follower load can indeed result in a stiffer behaviour; the application of force producing spurious moments results in an apparently more deformable behaviour, however the respective effects change at various segments along the spine due to its natural curvature. These outcomes are reported not only in qualitative, but also in quantitative terms. The numerical approach here followed to perform the meta-analysis is original and it proved to be effective thanks to the bypass of the natural variability among specimens which might completely or partially hinder the effect of some boundary conditions. In addition, it can provide very complete information since the behaviour of each functional spinal unit can be recorded. On the whole, the work provided an extensive review of lumbar spine loading in flexion/extension.

Abstract:Trunk biomechanical models play an indispensable role in predicting muscle forces and spinal loads under whole-body vibration (WBV) exposures. Earlier measurements on the force-motion biodynamic response (impedance, apparent mass) at the body-seat interface and vibration transmissibility (seat to head) have led to the development of different mechanical models. Such models could simulate the overall passive response and serve as an important tool for vehicle seat design. They cannot, however, evaluate physiological parameters of interest under the WBV. On the contrary, anatomical models simulating human's physiological characteristics can predict activities in muscles and their dynamic effects on the spine. In this study, a kinematics-driven nonlinear finite element model of the spine, in which the kinematics data are prescribed, is used to analyse the trunk response in seated WBV. Predictions of the active model (i.e., with varying muscle forces) as compared with the passive model (i.e., with no muscle forces) compared satisfactorily with measurements on vertical apparent mass and seat-to-head transmissibility biodynamic responses. Results demonstrated the crucial role of muscle forces in the dynamic response of the trunk. Muscle forces, while maintaining trunk equilibrium, substantially increased the compression and shear forces on the spine and, hence, the risk of tissue injury.
Effect of low back pain on the kinetics and kinematics of the lumbar spine - a combined in vivo and in silico investigation

Abstract:Lifting is a significant risk factor for low back pain (LBP). Different biomechanical factors including spinal loads, kinematics, and muscle electromyography (EMG) activities have previously been investigated during lifting activities in LBP patients and asymptomatic individuals to identify their association with LBP. However, the findings were contradictory and inconclusive. Accurate and subject-specific prediction of spinal loads is crucial for understanding, diagnosing, planning tailored treatments, and preventing recurrent pain in LBP patients. Therefore, the present study aimed to estimate the L5-S1 compressive and resultant shear loads in 19 healthy and 17 non-specific chronic LBP individuals during various static load-holding tasks (holding a 10 kg box at hip, chest, and head height) using full-body and personalized musculoskeletal models driven by subject-specific in vivo kinematic/kinetic, EMG, and physiological cross-sectional areas (PCSAs) data. These biomechanical characteristics were concurrently analyzed to identify potential differences between the two groups. Statistical analyses showed that LBP had almost no significant effect on the range of motion (trunk, lumbar, pelvis), PCSA, and EMG. There were no significant differences (p > 0.05) in the predicted L5-S1 loads. However, as the task became more demanding, by elevating the hand-load from hip to head, LBP patients experienced significant increases in both compressive (33 %, p = 0.00) and shear (25 %, p = 0.02) loads, while asymptomatic individuals showed significant increases only in compressive loads (30 %, p = 0.01). This suggests that engaging in more challenging activities could potentially magnify the effect of LBP on the biomechanical factors and increase their discrimination capacity between LBP and asymptomatic individuals.
