Abstract:Gastric cancer is one of the most common malignant tumors worldwide. The number of gastric cancer cases and related deaths in China accounts for almost half of the global data. The survival prognosis of advanced gastric cancer is poor. With the development of individualized precision therapy, targeted therapy and immunotherapy have become the focus of comprehensive therapy. This paper reviews the latest research progress of chemotherapy, targeted therapy, and immunotherapy for advanced gastric cancer.

Abstract:The incidence and mortality rates of gastric cancer are among the top three cancers in China, which poses great threat to people's lives and health. So far, surgery remains to be the cornerstone of treatment for gastric cancer. With the development of laparoscopic surgery, minimally invasive treatment techniques, together with the deepening of clinical researches, as we review the research progress in 2021, the core controversial issues of gastric cancer surgery have been basically addressed. The series of "minimal-innovation" concepts and technologies represented by single-incision/reduced-port laparoscopic surgeries have been further developed; radiomics and artificial intelligence aided prediction have been applied into the forefront of surgical accurate decision-making; targeted and immune-therapy is about to break through the bottleneck of surgical efficacy of gastric cancer. Currently, molecular imaging and targeted tracer guided precision cancer surgery are being explored, which is expected to revolutionize in key links such as real-time in-vivo determination of tumor margin, tracing of metastatic lymph nodes and visualization of nerves. Looking forward into the future, gastric cancer surgery will break through the century-old ceiling of "gross appearance by naked eye" and "traditional extensive experience", and set off a new round of technological revolutions in molecular visualization intelligent precision minimally invasive surgery.

Abstract:Dysregulation and polymorphism of the related genes play important roles in etiopathogenesis of gastric cancer.Genetic mutations and over-expression of human epidermal growth factor receptor 2(HER2) and vascular endothelial growth factor(VEGF) and abnormal activation of downstream signal transduction pathway have significant correlations with the genesis and metastasis of gastric cancer,which have been used as target spots of molecular target therapy to treat gastric cancer.There is no doubt that gastric cancer is closely related to Helicobacter pylori(H.pylori).Relevant virulence factors of H.pylori include cytotoxin-associated gene A(CagA),vacuolating cytotoxin A(VacA) and outer membrane protein(OMP).With the evidence that tumor is essentially a kind of diseases from stem cells,research of stem cells in gastric cancer has became a hot point.Gastric cancer screening is a very effective method for early detection.Recently,some scholars recommended gastric cancer screening using the ABC method(combined measurement of serum pepsinogen and H.pylori antibody) and endoscopy,which made the diagnostic rate of early gastric cancer increased to 78%.Great progress has been achieved in the techniques of digestive endoscopy and pathology biopsy.A diversity of new endoscopic techniques and methods have been widely used clinically.Endoscopic submucosal dissection(ESD) has been widespread accepted as the main approach to treat early gastric cancer with a high resection rate in en bloc greater than 95%.The research of molecular target therapy in gastric cancer has developed rapidly and many related medicines approved to enter clinical trials are promising.

Abstract:Gastric cancer is a high incidence cancer in China. Surgical treatment is regarded as thecornerstone of gastric cancer treatment. Surgical research has been focusing on endoscopic surgery andlymph node dissection. At the same time, with the continuous innovation of immunotherapeutic and targetedtherapeutic drugs, the mode of perioperative treatment and Ⅳ stage treatment has made great changes andprogress, and the survival benefit of patients has been improved. In addition, with the rapid developmentof cancer biology and next-generation sequencing, accurate stratification of patients' populations andaccurate drug use based on patients' subpopulation have been providing hopes for gastric cancer patients.

Abstract:Gastric cancer is one of the most frequently occurring cancers in China, with an estimated 380,000 new cases each year, accounting for more than 40% of the worldwide annual cancer incidence. There is geographical clustering of the distribution of gastric cancer in China, with most of the high-risk areas being rural. D2 resection is the standard lymphadenectomy for curative resection in China, but more extensive lymphadenectomy is conducted for selected patients. Perioperative chemotherapy, postoperative chemotherapy or chemoradiotherapy can be combined with surgery. It remains uncertain which option is best, but if surgery is insufficient, adjuvant chemoradiotherapy is recommended. In the palliative setting, although there is no standard first-line chemotherapy, regimens based on taxane, oxaliplatin or capecitabine, or the epirubicin, cisplatin, 5-fluorouracil regimen and its modifications are the most common options selected by Chinese oncologists. Several studies to evaluate target therapy are ongoing, but it is too early to draw any conclusions. However, the development of target therapy is likely to become a milestone in the treatment of gastric cancer.

Abstract:This century has seen significant advances in the treatment and research of gastric cancer in China. Chinese scholars have made a series of key technological breakthroughs in minimally invasive surgery, perioperative treatment and artificial intelligence diagnosis. These world-leading clinical researches have improved treatment outcomes and reduced surgical trauma. Global surveillance of trends in cancer survival 2000-14 reported that survival of gastric cancer in China has significantly improved during the last 20 years. This paper reviews the research history of surgical oncology for gastric cancer in China, summarises the experience and attempts to explore the future direction.

Abstract:Gastric cancer (GC) is one of the most common malignancies worldwide. Most patients are diagnosed at advanced stages due to the subtle symptoms of earlier disease and the low rate of regular screening. Systemic therapies for GC, including chemotherapy, targeted therapy and immunotherapy, have evolved significantly in the past few years. For resectable GC, perioperative chemotherapy has become the standard treatment. Ongoing investigations are exploring the potential benefits of targeted therapy or immunotherapy in the perioperative or adjuvant setting. For metastatic disease, there have been notable advancements in immunotherapy and biomarker-directed therapies recently. Classification based on molecular biomarkers, such as programmed cell death ligand 1 (PD-L1), microsatellite instability (MSI), and human epidermal growth factor receptor 2 (HER2), provides an opportunity to differentiate patients who may benefit from immunotherapy or targeted therapy. Molecular diagnostic techniques have facilitated the characterization of GC genetic profiles and the identification of new potential molecular targets. This review systematically summarizes the main research progress in systemic treatment for GC, discusses current individualized strategies and presents future perspectives.

Abstract:With the approaches of artificial intelligence and big data, the development of cancer genomics and updating of imaging technology, gastric cancer surgery is facing great challenges and opportunities. The main focus is on laparoscopic surgery technology, enhanced recovery after surgery, multidisciplinary comprehensive treatment, and precision medicine. Considering the common de-mand for reduced complication rate among doctors and patients, laparoscopic surgery has become widely popular owing to its advan-tages of small incision and rapid recovery. Furthermore, the development of artificial intelligence and big data has raised a new chal-lenge in routine diagnosis and treatment. As a result, we encourage multicenter cooperation, and data standardization and sharing. At present, completion of the transition from empirical medicine to evidence-based medicine and promotion of the individualization and standardization of gastric cancer treatment are needed.

Abstract:China has high prevalence of gastric cancer, with the morbidity and mortality leading the list of malignancy. The conditions of diagnosis and treatment of gastric cancer in China fail to meet the need of a large number of patients because of restriction in re-gional development. Therefore, we are facing a very serious situation of fighting against gastric cancer. Multidiscipline, individualiza-tion, and standardization are the development tendency of diagnosis and treatment of gastric cancer. Numerous topics about gastric cancer are currently controversial and solutions to these problems depend on the development of evidence-based medicine. This re-view summarized the recent progress in the clinical methods used for stomach cancer, laparoscopic surgical techniques, diagnosis and treatment of early gastric cancer, conversion therapy of advanced gastric cancer, therapeutic strategy of esophageal-gastric junction cancer, enhanced recovery after surgery, and translational research and clinical trials of gastric cancer. Moreover, the foci and prob-lems of the diagnosis and treatment of gastric cancer were discussed to provide reference for further studies.

Abstract:INTRODUCTION:Gastric cancer (GC) is a highly aggressive cancer and a major cause of cancer-related deaths worldwide. Approximately half of the world's GC cases and deaths occur in china. GC presents challenges in early diagnosis and effective therapy due to a lack of understanding of the underlying molecular biology. The primary goals of this review are to outline current GC research in china and describe future trends in this field. Areas covered: This review mainly focuses on a series of GC-related advances China has achieved. Considerable progress has been made in understanding the role of H. pylori in GC by a series of population-based studies in well-established high-risk areas; A few germline and somatic alterations have been identified by 'omics' studies; Studies on the mechanisms of malignant phenotypes have helped us to form an in-depth understanding of GC and advance drug discovery. Moreover, identification of potential biomarkers and targeted therapies have facilitated the diagnosis and treatment of GC. However, many challenges remain. Expert commentary: To combat GC, sufficient funding is important. More attention should be paid on early diagnosis and the discovery of novel efficient biomarkers and the development of biomarker-based or targeted therapeutics in GC.

Abstract:Gastric cancer (GC) is defined as the primary epithelial malignancy derived from the stomach, and it is a complicated and heterogeneous disease with multiple risk factors. Despite its overall declining trend of incidence and mortality in various countries over the past few decades, GC remains the fifth most common malignancy and the fourth leading cause of cancer-related death globally. Although the global burden of GC has shown a significant downward trend, it remains severe in certain areas, such as Asia. GC ranks third in incidence and mortality among all cancer types in China, and it accounts for nearly 44.0% and 48.6% of new GC cases and GC-related deaths in the world, respectively. The regional differences in GC incidence and mortality are obvious, and annual new cases and deaths are increasing rapidly in some developing regions. Therefore, early preventive and screening strategies for GC are urgently needed. The clinical efficacies of conventional treatments for GC are limited, and the developing understanding of GC pathogenesis has increased the demand for new therapeutic regimens, including immune checkpoint inhibitors, cell immunotherapy and cancer vaccines. The present review describes the epidemiology of GC worldwide, especially in China, summarizes its risk and prognostic factors, and focuses on novel immunotherapies to develop therapeutic strategies for the management of GC patients.

Abstract:Gastric cancer is the second highest morbidity and mortality malignant tumor in China. Approximately 70% of the patients are in advanced stage with limited curative effect and poor prognosis. As the result of several important researches been published, the basic understanding of gastric cancer is more and more profound with the improvement of clinical treatment ability. At the same time with the coming of era of molecular and precision medicine, exploring the molecular characteristics of gastric cancer to screening the therapeutic target is becoming the hotpot of the experimental study. We will set forth recent progress of experimental study of gastric surgery in terms of basic and clinical study and look in the future.

Abstract:Gastric cancer is a heterogeneous disease with large variations across geographical regions. Although the global incidence of gastric cancer is declining, it remains highly prevalent in Asia as compared to the West. China is one of the countries with the highest incidence of gastric cancer, and accounts for over 40% of all new gastric cancer cases in the world. Gastric cancer is the third leading cause of cancer mortality in China. Gastric cancer in Chinese patients is different from that occurring in the West, and is a significant health burden. Moreover, there is currently no internationally accepted standard treatment regimen and clinical practice varies widely across countries. With the development of medical technology and wide application of more and more novel technologies, evidence-based approaches in combination with the strengths of various treatments will be the key to multidisciplinary management of gastric cancer for ultimately improving the outcomes and quality of life of these patients.

Abstract:Gastric cancer (GC) is one of the most common malignant tumors worldwide. Its incidence ranks the 5th among all malignant tumors globally, and it is the 3rd leading cause of death among patients with cancer. Surgical treatment is the first choice in clinical practice. However, targeted therapy, immunotherapy, and other treatment methods have also become research hotspots at home and abroad with the development of individualized precision therapy in recent years, besides traditional radiotherapy and chemotherapy. At present, targeted therapy and immunotherapy are methods used for treating GC, and they have important clinical application value and prospects. This study aimed to review the research progress of targeted therapy and immunotherapy for GC, focusing on its mechanism of action and related important clinical trials, hoping to provide references for the clinical treatment of GC.

Current Status and Challenges of Gastrointestinal Cancer Treatment in China

Abstract:Gastrointestinal cancer has a high incidence in China, especially upper gastrointestinal cancer (esophageal and gastric cancer). Although the incidence has been declining in recent years, the annual incidence of new cases accounts for more than 50% of global cases, and the incidence of colorectal cancer in big cities like Beijing and Shanghai is increasing annually but the mortality rate has not yet increased significantly. Gradual implementation and promotion of multidisciplinary team (MDT) reduced the mortality of gastrointestinal tumors, but this change is not obvious. Successful clinical studies of immune checkpoint inhibitors [programmed death-1 (PD-1), programmed death ligand 1 (PD-L1)] for esophageal and gastric cancer have changed clinical treatment strategies. The overall survival of advanced patients has improved but the comprehensive treatment of locally advanced patients warrants further research. In addition to the microsatellite instability high (MSI-H) population, few drugs have changed the clinical outcome of colorectal cancer in the past decade, which makes it difficult to affect the prognosis of patients. Clinical research on MDT is difficult to conduct, resulting in less data accumulation in this field. The heterogeneity of gastrointestinal tumors is relatively strong and precise treatment should be the direction of future development. In this regard, several studies have been conducted and developed rapidly in recent years in China, including those on MSI-H, human epidermal growth factor receptor 2 (HER2), c-mesenchymal-epithelial transition factor (c-MET), B-Raf proto-oncogene serine/threonine kinase (BRAF), rat sarcoma virus (RAS), fibroblast growth factor receptors (FGFR) and polymerases epsilon (POLE). The selection of immunotherapy markers, as well as the discovery of efficacy and prognostic markers, are also clinicians' concerns. Several articles presented in this issue reflect the research carried out by Chinese researchers based on clinical problems. These concerns of clinicians would impact the clinical practice. However, due to the limitation of time and level, the findings cannot fully reflect the current level of gastrointestinal cancer research in China which is expected to develop rapidly in the future.

Abstract:Gastric cancer is one of the most common cause of cancer related deaths worldwide which results in malignant tumors in the digestive tract. The only radical treatment option available is surgical resection. Recently, the implementation of neoadjuvant chemotherapy resulted in 5-year survival rates of 95% for early gastric cancer. The main reason of treatment failure is that early diagnosis is minimal, with many patients presenting advanced stages. Hence, the greatest benefit of radical resection is missed. Consequently, the main therapeutic approach for advanced gastric cancer is combined surgery with neoadjuvant chemotherapy, targeted therapy, or immunotherapy. In this review, we will discuss the various treatment options for advanced gastric cancer. Clinical practice and clinical research is the most practical way of reaching new advents in terms of patients' characteristics, optimum drug choice, and better prognosis. With the recent advances in gastric cancer diagnosis, staging, treatment, and prognosis, we are evident that the improvement of survival in this patient population is just a matter of time.

Abstract:As one of the most common forms of solid tumours, gastric carcinoma has been revealed as the third leading cause of death worldwide. The symptom of gastric cancer is usually not obvious and thus difficult to detect at earlier stages. Therefore, gastric cancer is already in the advanced stage once detected in patients, which has a poor prognosis due to ineffective therapies and multiple resistance. Recent advance in understanding the microenvironment of cancer has significantly promoted the development of immunotherapy for advanced gastric cancer. Immunotherapy can induce immune responses in gastric cancer patients thus leads to the destruction of cancer cells. In comparison of traditional therapy, immunotherapy has demonstrated robust efficacy and tolerable toxicity. Therefore, this novel strategy for treatment of advanced gastric cancer has gain increasingly popularity. In this review, we summarize recent progress of immunotherapy in advanced gastric cancer, such as immune check point inhibitors, adoptive cell therapy, VEGF inhibitors, cancer vaccines and CAR-T cell therapy. We highlight immunotherapies involved in clinical applications and discuss the existing challenges of current immunotherapies and promising strategies to overcome these limitations.

Global progress and future prospects of early gastric cancer screening

Abstract:Gastric cancer is a prevalent malignancy that poses a serious threat to global health. Despite advances in medical technologies, screening methods, and public awareness, gastric cancer remains a significant cause of morbidity and mortality worldwide. Early gastric cancer frequently does not present with characteristic symptoms, while advanced stage disease is characterized by a dismal prognosis. As such, early screening in gastric cancer is of great importance. In recent years, advances have been made globally in both clinical and basic research for the screening of early gastric cancer. The current predominant screening methods for early gastric cancer include imaging screening, endoscopic screening and serum biomarker screening. Imaging screening encompasses upper gastrointestinal barium meal, multidimensional spiral computed tomography (MDCT), Magnetic resonance imaging (MRI), and ultrasonography. Endoscopic screening methods include white light endoscopy, chromoendoscopy, computed virtual chromoendoscopy, and other endoscopic techniques like endocytoscopy, confocal laser endomicroscopy, optical coherence tomography and so on. Biomarkers screening involves the assessment of conventional biomarkers such as CEA, CA19-9 and CA72-4 as well as more emerging biomarkers such as peptides (PG, G-17, GCAA, TAAs and others), DNA (cfDNA, DNA methylation, MSI), noncoding RNA (miRNA, lncRNA, circRNA, and tsRNA) and others. Each screening method has its strengths and limitations. This article systematically summarizes worldwide progress and future development of early gastric cancer screening methods to provide new perspectives and approaches for early diagnostic and treatment advancements in gastric cancer worldwide.
