Abstract:Purpose This study aimed to develop a method to calculate the mattress indentation for further estimating spinal alignment. Design/methodology/approach A universal indentation calculation model is derived based on the system theory, and the deformation characteristics of each component are analyzed by the finite element (FE) model of a partial air-spring mattress under the initial air pressure of 0.01–0.025 MPa. Finally, the calculation error of the model is verified. Findings The results indicate that the indentation calculation model could describe the stain of a mattress given the load and the constitutive model of each element. In addition, the FE model of a partial air-spring mattress can be used for further simulation analysis with an error of 1.47–3.42 mm. Furthermore, the deformation of the series system is mainly contributed by the air spring and the components directly in contact with it, while the top component is mainly deflection deformation. In addition, the error of the calculation model is 2.17–5.59 mm on the condition of 0.01–0.025 MPa, satisfying the engineering application. Finally, the supine spinal alignment is successfully extracted from the mattress indentation. Research limitations/implications The limitation of this study is that it needs to verify the practicality of the indentation calculation model for the Bonnier spiral spring mattress. The main feature of the Bonnier spring mattress is that all springs are connected, so the mattress deflection and neighborhood effect are more significant than those of the air-spring mattress. Therefore, the applicability of the model needs to be tested. Moreover, it is worth further research to reduce the deformation error of each component. Practical implications As part of the series of studies on the intelligent air-spring mattress, the indentation-based evaluation method of spinal alignment in sleep postures will be studied for hardness and intelligent regulation based on this study. Social implications The results of this research are ultimately used for the intelligent adjustment of air-spring mattresses, which automatically adjusts the hardness according to the user's sleep postures and spinal alignment, thus maintaining optimal spinal biomechanics. The successful application of this result could improve the sleep health of the general public. Originality/value Based on the series system theory, an indentation calculation model for mattresses with arbitrary structure is proposed, overcoming the dependence of parameters on materials and their combinations when fitting the Burgers model. Further, the spinal alignment in supine posture is extracted from the indentation, laying a theoretical foundation for further recognition and adjustment of the spinal alignment of the intelligent mattress.

Abstract:Sleep quality is an important issue of public concern. This study, combined with sensor application, aims to explore the determinants of perceived comfort when using smart bedding to provide empirical evidence for improving sleep quality. This study was conducted in a standard sleep laboratory in Quanzhou, China, from March to April of 2023. Perceived comfort was evaluated using the Subjective Lying Comfort Evaluation on a seven-point rating scale, and body pressure distribution was measured using a pressure sensor. Correlation analysis was employed to analyze the relationship between perceived comfort and body pressure, and multiple linear regression was used to identify the factors of perceived comfort. The results showed that body pressure was partially correlated with perceived comfort, and sleep posture significantly influenced perceived comfort. In addition, height, weight, and body mass index are common factors that influence comfort. The findings highlight the importance of optimizing the angular range of boards based on their comfort performance to adjust sleeping posture and equalize pressure distribution. Future research should consider aspects related to the special needs of different populations (such as height and weight), as well as whether users are elderly and whether they have particular diseases. The design optimization of the bed board division and mattress softness, based on traditional smart bedding, can improve comfort and its effectiveness in reducing health risks and enhancing health status.

A Feasibility Study on Smart Mattresses to Improve Sleep Quality

Abstract:Good sleep quality is essential, especially for clinical users. Sleep disorders not only impair the success rate of treatment but also delay recovery. They can seriously interfere with treatment outcomes and even endanger a user's life. In this study, we created a smart mattress containing 10 × 18 air packs and control units. Each air pack contains a set of pressure and height sensors and two air valves. Each row control unit can detect and adjust the pressure and height of each air bag in the row. When the bed body is turned on, it automatically initializes, adjusts the state of each air bag to the same height and pressure, and enters a slow scanning state. When perceived objects or people are lying on the bed, the bed automatically perceives the human body structure and body pressure matrix, increases the scanning speed for more timely and accurate measurements of the digital matrix and forming pressure by matrix-normalized processing, and then uses local pressure variance detection to automatically adjust to the sleeping position of the human body and thus achieve a uniform force distribution and a comfortable state. Finally, pressure matrix binarization was used to match sleeping position templates to identify the best template for automatic recognition of the sleeping position. The experimental results show that the sleeping position recognition method has high accuracy, recall, and precision. Our mattress is designed with interfaces for external devices. In future research, the smart mattress can connect to an auxiliary part of a smart ecosystem consisting of a smart pill box, a smart lighting system, and a microclimate system, which is expected to yield a more comprehensive intelligent ward to explore the possibility of improving sleep quality.
The Influence of Mattress Stiffness on Spinal Curvature and Intervertebral Disc Stress—An Experimental and Computational Study

Abstract:Sleeping support systems can influence spinal curvature, and the misalignment of the spinal curvature can lead to musculoskeletal problems. Previous sleep studies on craniocervical support focused on pillow variants, but the mattress supporting the pillow has rarely been considered. This study used a cervical pillow and three mattresses of different stiffnesses, namely soft, medium, and hard, with an indentation load deflection of 20, 42, and 120 lbs, respectively. A novel electronic curvature measurement device was adopted to measure the spinal curvature, whereby the intervertebral disc loading was computed using the finite element method. Compared with the medium mattress, the head distance increased by 30.5 ± 15.9 mm, the cervical lordosis distance increased by 26.7 ± 14.9 mm, and intervertebral disc peak loading increased by 49% in the soft mattress environment. Considering that the pillow support may increase when using a soft mattress, a softer or thinner pillow is recommended. The head distance and cervical lordosis distance in the hard mattress environment were close to the medium mattress, but the lumbar lordosis distance reduced by 10.6 ± 6.8 mm. However, no significant increase in intervertebral disc loading was observed, but contact pressure increased significantly, which could cause discomfort and health problems.

Automated driving: A biomechanical approach for sleeping positions

Abstract:Occupants of autonomous vehicle have frequently indicated the desire to sleep or rest while driving, yet little has been known regarding the suitable design criteria for a biomechanically reasoned in-vehicle sleeping position. This study was aimed at evaluating the biomechanical quality of different backrest and seat pan angle combinations, and at predicting the most favourable sleeping positions based on vehicle restriction. More specifically, the interface pressure distribution and subjective suitability rating of 23 subjects was assessed in a total of nine (3 × 3) combinations of seat pan (20°, 30°, 40°) and backrest (145°, 155°, 165°) angles. Biomechanical quality was evaluated with an interface pressure score (IPS) based on sensitivity weighted pressures and the total contact area. Two-way repeated measures ANOVA revealed that IPS significantly improves with increasing seat pan angle whereas backrest angles of 155° or 165° lead to significant better IPS compared to flatter ones (145°). The overall highest IPS was observed for a 40°-seat pan angle in combination with a 155°-backrest angle. Subjective suitability rating revealed that people prefer a combination of 165° backrest angle with a seat pan of 20°; however, eight of nine combinations can be considered as suitable for sleeping. Therefore, the combination of a 40°-seat pan angle and 155° backrest is recommended by the present study for an in-vehicle sleeping position due to the increased biomechanical quality.

Abstract:In order to solve the difficult portability problem of traditional non-invasive sleeping posture recognition algorithms arising from the production cost and computational cost, this paper proposes a sleeping posture recognition model focusing on human body structural feature extraction and integration of feature space and algorithms based on a specific air-spring mattress structure, called SPR-DE (SPR-DE is the Sleep Posture Recognition-Data Ensemble acronym form). The model combines SMR (SMR stands for Principle of Spearman Maximal Relevance) with horizontal and vertical division based on the barometric pressure signals in the human body's backbone region to reconstruct the raw pressure data into strongly correlated non-image features of the sleep postures in different parts and directions and construct the feature set. Finally, the recognit-ion of the two sleep postures is accomplished using the AdaBoost-SVM integrated classifier. SPR-DE is compared with the base and integrated classifiers to verify its performance. The experimental results show that the amount of significant features helps the algorithm to classify different sleeping patterns more accurately, and the f1 score of the SPR-DE model determined by the comparison experiments is 0.998, and the accuracy can reach 99.9%. Compared with other models, the accuracy is improved by 2.9% ~ 7.7%, and the f1-score is improved by 0.029 ~ 0.076. Therefore, it is concluded that the SMR feature extraction strategy in the SPR-DE model and the AdaBoost-SVM can achieve high accuracy and strong robustness in the task of sleep posture recognition in a small area, low-density air-pressure mattress, taking into account the comfort of the mattress structural design and the sleep posture recognition, integrated with the mattress adaptive adjustment system.

Abstract:Abstract Introduction Unsubstantiated claims about the potential benefits of various mattress types can make it challenging for consumers to choose a mattress that fits their personalized needs. Services that provide information and recommendations regarding mattress selection may help solve this problem. However, more evidence and research are necessary to understand such services' potential to support better sleep. This study examined the sleep of research participants using mattresses selected during an in-store matching process compared to using their original mattresses. Methods Healthy adults (75% female; ages 27-73, mean=46) participated in a 10-week field study, using a pre-post intervention design. The intervention consisted of being guided in person through the Mattress Firm Mattress Matcher process. During the 4-week baseline period, participants used their regular mattress at home. After being matched at a store near them with a new mattress, participants slept on that mattress for 6 weeks, including a 2-week adjustment period. Sleep was measured objectively using SleepScore Max every night and by daily and pre-post self-report. Multilevel regression and paired t-tests were used to test for statistical significance. Results There were over 1,189 nights of tracked sleep across 29 participants. Objective sleep measurements showed that deep sleep improved when using the matched mattress compared to baseline, indicated by 3 metrics: deep sleep in minutes (5% increase, p=.003), deep sleep as a proportion of the night (4% relative increase; p=.030), and BodyScore (1% increase; p=.022). Improvements also were found for a variety of self-reported sleep outcomes, including feeling better able to sleep through the night without tossing and turning, waking up feeling refreshed and well-rested, feeling satisfied with sleep more often, and perceiving better overall sleep quality (all ps<.001). Additionally, compared to baseline, the matched mattress felt more comfortable, pressure relieving, supportive, and cooler (all ps<.001). Conclusion Objectively measured deep sleep and numerous aspects of self-reported sleep improved among healthy adults using a mattress selected via an expert, in-person mattress matcher process. Additionally, positional adjustment of a sleep surface can help improve sleep and alleviate discomfort. This study compared sleep in an inclined position on an adjustable bed base to participants’ prior sleep when sleeping flat in their usual bed environment. Methods Healthy adults (71% male; ages 27-75, mean age=44) reporting difficulties with frequent awakenings or time spent awake at night participated in an 8-week field study, using a pre-post intervention design. During the 4-week baseline period, participants slept flat on their regular bed. During the 4-week intervention period, they slept in an inclined position of their choosing using Sleepy’s Adjustable Bed Base. Sleep was measured objectively using a PSG-validated, non-contact biomotion device, SleepScore Max, every night and by daily and pre-post self-report. Multilevel regression and paired t-tests were used to test for statistical significance. Results There were 951 nights of tracked sleep across 27 participants. Objective sleep measurements showed increased total sleep time (+6 minutes, p=.038) and an increase in light sleep duration (+5 minutes, p=.045) compared to baseline. Also, improvement was observed in BodyScore, an objective measure of deep sleep quality (+2%, p=.030). Self-report measures revealed that using the bed base felt more comfortable compared to sleeping flat using the original bed base (p=.011). Improvements also were found for a variety of perceived sleep outcomes, including the perception of falling asleep more easily, less tossing and turning, better sleep quality, and feeling more well-rested in the morning (all ps<.001). Conclusion Several important aspects of objectively measured sleep and self-reported sleep improved when using the adjustable bed base compared to sleeping flat on the original bed base. Abstract:The purpose of this study was to determine the biomechanical mechanisms driving passengers' lower-limb postural shifts during seated sleep on a flight to prevent negative effects on passengers' physical health. Twenty subjects participated in an observational study and a subsequent experiment on fatigue development and tissue oxygenation changes during seated sleep in an economy-class aircraft seat. Three of the most frequently used postures, which involved four targeted muscles of the legs and the thigh-buttock region, were selected and examined in the experiment with the following measures: muscle electromyogram, tissue oxygenation, and body contact pressure distribution. The results showed that the fatigue of the tibialis anterior and gastrocnemius and the compression of the region under the medial tuberosities were relieved by alternations among the three positions-position 1 (placing the shanks forwards), position 2 (placing the shanks neutrally), and position 3 (placing the shanks backwards). This research reveals the mechanical properties of the biomechanical factors functioning in lower-limb postural shifts during seated sleep and provides design optimization strategies for economy-class aircraft seats to reduce the negative effects on passenger health.

More empirical research is needed to establish the impact of particular mattresses on both objective and self-reported measures of sleep quality. This study compared sleep on an innerspring mattress to participants’ prior sleep on their original mattress. Methods Healthy adults (68% female, ages 23-69) participated in a 10-week field study, using a pre-post intervention design. During the 4-week baseline period, participants used their regular mattress at home. During the 4-week intervention period, which occurred following a 2-week adjustment period, they used a Sleepy’s by Sealy mattress at home. Sleep was measured objectively using SleepScore Max every night and by daily and pre-post self-report. Multilevel regression and paired t-tests were used to test for statistical significance. Results There were over 757 nights of tracked sleep across 18 participants. Objective sleep measurements showed decreased WASO, both in duration (-13 minutes, p=.024) and proportion of the night (p=.025). Also, there was an increase in deep sleep (+12 minutes, p=040). Self-report measures revealed that the innerspring mattress felt more comfortable (p<.001). Improvements also were found for a variety of perceived sleep outcomes, including time to fall asleep, waking less often, spending less time awake, longer sleep duration, better sleep quality, and feeling more rested in the morning (ps<.05). Conclusion Objectively-measured sleep and self-reported sleep improved when using the innerspring mattress compared to healthy adults’ original mattresses. Abstract:Getting proper rest is an essential issue, but it is neglected as the major component of every person's overall health and well-being. Enough proper rest has a lot of proven health benefits. This paper aims at proposing a systematic framework to find the suitable pillows and mattresses for individuals to have proper rest and sleep. The proposed system includes a set of force-sensing resistors (FSR), thermal sensors, humidity-thermal sensors, and a central unit to process the acquired data. Customized software in LabVIEW is developed to monitor the results and ultimately identify the proper ergonomic mattresses and pillows for users. Initial experiments were performed based on zonal performance and multitasking to design, construct, and test each system as a novel hypothesis. The studies and tests were carried out in two steps. The first step included hardware testing and sensor arrangement, and the second step involved processing the acquired data to identify a good product. In this study, we propose a feasible framework to select convenient mattresses and pillows for each person.

Abstract:Background Sleep is essential for human health. Considerable effort has been put into academic and industrial research and in the development of wireless body area networks for sleep monitoring in terms of nonintrusiveness, portability, and autonomy. With the help of rapid advances in smart sensing and communication technologies, various sleep monitoring systems (hereafter, sleep monitoring systems) have been developed with advantages such as being low cost, accessible, discreet, contactless, unmanned, and suitable for long-term monitoring. Objective This paper aims to review current research in sleep monitoring to serve as a reference for researchers and to provide insights for future work. Specific selection criteria were chosen to include articles in which sleep monitoring systems or devices are covered. Methods This review investigates the use of various common sensors in the hardware implementation of current sleep monitoring systems as well as the types of parameters collected, their position in the body, the possible description of sleep phases, and the advantages and drawbacks. In addition, the data processing algorithms and software used in different studies on sleep monitoring systems and their results are presented. This review was not only limited to the study of laboratory research but also investigated the various popular commercial products available for sleep monitoring, presenting their characteristics, advantages, and disadvantages. In particular, we categorized existing research on sleep monitoring systems based on how the sensor is used, including the number and type of sensors, and the preferred position in the body. In addition to focusing on a specific system, issues concerning sleep monitoring systems such as privacy, economic, and social impact are also included. Finally, we presented an original sleep monitoring system solution developed in our laboratory. Results By retrieving a large number of articles and abstracts, we found that hotspot techniques such as big data, machine learning, artificial intelligence, and data mining have not been widely applied to the sleep monitoring research area. Accelerometers are the most commonly used sensor in sleep monitoring systems. Most commercial sleep monitoring products cannot provide performance evaluation based on gold standard polysomnography. Conclusions Combining hotspot techniques such as big data, machine learning, artificial intelligence, and data mining with sleep monitoring may be a promising research approach and will attract more researchers in the future. Balancing user acceptance and monitoring performance is the biggest challenge in sleep monitoring system research.

During the 4-week baseline period, participants used their regular mattress at home. During the 4-week intervention period, which occurred following a 2-week adjustment period, participants slept on a TEMPUR-LuxeBreeze Medium Hybrid mattress. Sleep was measured objectively using a PSG-validated, non-contact biomotion device, SleepScore Max, every night and by daily and pre-post self-report. Multilevel regression and paired t-tests were used to test for statistical significance. Results There were 1,331 nights of tracked sleep across 32 participants. Compared to baseline, objective sleep measurements showed increased time spent in bed (+8 minutes, p=.029) and improved SleepScore, an overall sleep quality metric (+1%, p=.044). Self-report measures revealed improvements in perceived coolness, comfort, support, pressure relief, sleep quality, amount of tossing and turning, satisfaction with sleep, and feeling well-rested in the morning (all ps<.001). Conclusion Participants reported a more comfortable sleep, in numerous ways, when using the mattress being tested compared to baseline. How do sleepwear and bedding fibre types affect sleep quality: A systematic review

Abstract:Sleepwear and bedding materials can affect sleep quality by influencing the skin and body temperature and thermal comfort. This review systematically evaluates the impact of sleepwear or bedding of different fibre types on sleep quality. A systematic search was conducted in six data bases plus Google Scholar and manual searches. Original articles that compared human sleep quality between at least two fibre types of bedding or sleepwear were included, resulting in nine eligible articles included in the review. The fibre types included cotton, polyester, wool, and blended materials for sleepwear; cotton, duck down, goose down, polyester and wool for duvet; and linen and a combination of cotton and polyester for bedding. The interplay between fibre materials and sleep quality is complex. Blended sleepwear demonstrated potential benefits for specific populations. Wool sleepwear showed benefits for sleep onset in adults (cool conditions) and in older adults (warm conditions). Linen bedsheets improved sleep quality under warm conditions in young adults. Goose down-filled duvets increased slow-wave sleep under cool conditions in young adults. However, a systematic comparison of fibre types is challenging due to the diverse nature of the studies evaluating sleep quality. Further research employing standardised methodologies with standard fibre samples in different populations and in different temperature conditions is imperative to elucidate comprehensively the effects of fibre choices on sleep quality. Despite the limitations and heterogeneity of the included studies, this analysis offers valuable insights for individuals seeking to optimise their sleep experiences and for manufacturers developing sleep-related products.

Abstract:Humans spend one-third of their lives in the sleep environment; however, research on how its interior arrangement affects individuals’ preferences and perceptions of sleep quality has not been emphasised. We predict that the Form School Feng Shui model corresponds to a mother’s embrace, which may fit most users’ desired paradigm in a sleep environment, and is, thus, probably supportive to better sleep quality. This paper investigates whether the desirable bedroom layout for humans corresponds to the Form School Feng Shui model, and whether the embraced arrangement in a sleep environment can enhance individual sleep quality. A nationwide questionnaire survey was conducted to investigate the perceptions of adults from multiple backgrounds, with a response rate of 27.0% (N = 405). The results showed the bedroom layout that was designed based on the Form School Feng Shui model was indicated as highly preferred but had no significant implications for better sleep quality. The findings supported the claim that the Form School Feng Shui model is broadly preferred for its application in the sleep environment; however, the hypothesis that such a model will contribute to better sleep quality is not established.

Sleep disruption and sleep position: Increased wake frequency in supine predicts lateral position preference

Abstract:Little is known about the physiological and biomechanical factors that determine individual preferences in lying posture during sleep. This study investigated relationships between position preference and position-specific arousals, awakenings, limb movements and limb movement arousals to explore the mechanisms by which biomechanical factors influence position preference. Forty-one mature-aged adults underwent 2 nights of at-home polysomnography ~2 weeks apart, on a standardised firm foam mattress, measuring nocturnal sleep architecture and position. The lateral supine ratio and restlessness indices specific to lateral and supine positions including limb movement index, limb movement arousal index, arousal index, wake index, respiratory arousal index and apnea-hypopnea index were calculated and analysed via linear mixed-effects regression. In the supine position, all restlessness indices were significantly increased compared with the lateral position, including a 379% increase in respiratory arousals (β = 7.0, p < 0.001), 108% increase in arousal index (β = 10.3, p < 0.001) and 107% increase in wake index (β = 2.5, p < 0.001). Wake index in the supine position increased significantly with more lateral sleep (β = 1.9, p = 0.0013), and significant correlation between lateral supine ratio polysomnography 1 and lateral supine ratio polysomnography 2 (β = 0.95, p < 0.001) indicated strong consistency in sleep preference. Overall, the findings suggest that some individuals have low tolerance to supine posture, represented by a comparatively high wake index in the supine position, and that these individuals compensate by sleeping a greater proportion in the lateral position.
Biomechanical design optimization and experimental verification of Bezier curve based two-sectional cervical pillow with variable-density cellular structure

Abstract:The fundamental function of an optimal cervical pillow is to provide sufficient support to maintain normal spinal alignment and minimize biological stress on the contact surface throughout sleep. The recent advancements in cervical pillows have mainly focused on the subjective and objective evaluations of support comfort, as well as the relationship between pillow height and cervical vertebrae posture. However, only a few studies have addressed shape design guidelines and mechanical performances of the pillows themselves. In this study, a two-sectional contour cervical pillow comprising an arc and a Bezier curve is designed to support the head and neck. The design of the arc-shaped neck section incorporates the Cobb's angle and Borden value from healthy individuals to reflect the consistency of normal cervical anatomical features. The Bezier curve-based head section takes the head length and neck depth into account as significant individual differences. Static analysis and lattice optimization are performed in ANSYS Workbench to develop a variable-density cellular structure, aimed at improving air permeability and reducing the risk of pressure ulcers associated with the cervical pillow. The rapid prototyping technique fused deposition modeling (FDM) and thermoplastic material polylactic acid (PLA) are employed for fabricating different cellular structures. The results demonstrate that the neck section experiences less stress and greater deformation in comparison to the head section, indicating good comfort and support provided by the designed cervical pillow. Additionally, the compressive, bending, and cushion properties of the 3D-printed cervical pillow with variable-density cellular structure are experimentally validated, further confirming its effectiveness.
Methodological issues of systematic reviews and meta-analyses in the field of sleep medicine: A meta-epidemiological study

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><p>An increasing number of systematic reviews and meta-analyses (SRMAs) have been published in the field of sleep medicine. We evaluated the methodological issues of these SRMAs. A protocol was developed in advance. Three databases were searched from inception to October 2019 for SRMAs published in major academic journals of sleep medicine that assessed healthcare interventions. The AMSTAR 2.0 instrument was used to evaluate the methodological issues and a multivariable regression analysis was conducted to investigate potential measures associated with methodological validity. We identified 163 SRMAs. The median number of missing safeguards of these SRMAs was 7 out of 16 (Interquartile range, IQR: 6-9), and on average, 2 of these missing safeguards were critical weaknesses. Our regression analysis suggested that SRMAs published in recent years (ß=0.16; 95%CI: 0.08, 0.24; p=0.002), with the first author from Europe (ß=0.08; 95%CI: 0.02, 0.14; p=0.013) tend to have higher relative methodological ranks. In conclusion, the methodological validity for current SRMAs in sleep medicine was poor. Further efforts to improve the methodological validity are needed.</p>

Abstract:This paper presents a novel system for automatic sleep system recommendation using RGB, depth and pressure information. It consists of a validated clinical knowledge-based model that, along with a set of prescription variables extracted automatically, obtains a personalized bed design recommendation. The automatic process starts by performing multi-part human body RGB-D segmentation combining GrabCut, 3D Shape Context descriptor and Thin Plate Splines, to then extract a set of anthropometric landmark points by applying orthogonal plates to the segmented human body. The extracted variables are introduced to the computerized clinical model to calculate body circumferences, weight, morphotype and Body Mass Index categorization. Furthermore, pressure image analysis is performed to extract pressure values and at-risk points, which are also introduced to the model to eventually obtain the final prescription of mattress, topper, and pillow. We validate the complete system in a set of 200 subjects, showing accurate category classification and high correlation results with respect to manual measures.
