Abstract:Background: The association between ambient temperature and mortality has been well documented worldwide. However, limited data are available on nonfatal health outcomes, such as emergency department visits (EDVs), particularly from China.Objectives: To examine the temperature-EDV association in 12 Chinese cities; and to assess the modification effects by region, gender and age.Methods: Daily meteorological data and non-accidental EDVs were collected during 2011-2014. Poisson regression with distributed lag non-linear model was applied to examine the temperature-lag-EDV association in each city. The effect estimates were pooled using multivariate meta-analysis at the national and regional level. Stratified analyses were performed by gender and age-groups. Sensitivity analyses adjusting for air pollution and relative humidity were conducted.Results: A total of 4,443,127 EDVs were collected from the 12 cities. Both cold and hot temperatures were associated with increased risk of EDVs, with minimum-mortality temperature located at 64th percentile of temperature. The effect of cold temperature appeared on day 2 and persisted until day 30, causing a cumulative relative risk (RR) of 1.80 (1.54, 2.11). The effect of hot temperature appeared immediately and lasted until day 3, with a cumulative RR of 1.15 (1.03,1.29). The effect of temperature on EDVs was similar in male and female but was attenuated with increasing age. The effect of cold temperature on EDVs was greater in southern areas of the country whereas the hot effect was greater in northern cities. The association was robust to a large range of sensitivity analyses.Conclusions: In China, there is a U-shaped association between temperature and risk of EDVs that is independent of air pollution and humidity. The temperature-EDV association varies with latitude and age-groups but is not affected by gender. Forecasting models for hospital emergency departments may be improved if temperature is included as an independent predictor. (C) 2017 Elsevier Ltd. All rights reserved.
Ambient temperature variability and hospital admissions for pneumonia: A nationwide study

Abstract:Few investigations have assessed the impact of short-term ambient temperature change on pneumonia risk. We aimed to study the relation of temperature variability (TV) with daily hospitalizations for pneumonia in China. We conducted a time-series study in 184 major cities by extracting daily hospital data between 2014 and 2017 from a medical insurance claims database of 0.28 billion beneficiaries. TV was calculated as standard deviation of daily minimum and maximum temperatures over exposure days. We estimated associations of pneumonia admissions with TV for each city using over-dispersed generalized linear models controlling for weather conditions and ambient air pollution, and pooled city-specific estimates using random effects meta-analyses. We also investigated exposure-response relationship curve and potential effect modifiers. We identified 4.2 million pneumonia hospitalizations during the study period. TV was positively related to daily pneumonia admissions. At the national-average level, each 1-°C increase in TV at 0–6 days' exposure corresponded to a 0.65 % (95 % CI: 0.34 %–0.96 %) increase in pneumonia admissions. An approximately linear exposure-response curve for the relation of TV with pneumonia admission was noted. The relations were more evident in cities with larger average age ( P = 0.038). As the first study in China to assess the impact of temperature change on pneumonia on a national scale, our results indicated that acute TV exposure was related to higher admissions for pneumonia. Our findings should provide new insight into the health impacts associated with climate change.

Abstract:Chronic obstructive pulmonary disease (COPD) is a leading cause of death in people aged over 60 years old. Research has been reported that ambient temperature and diurnal temperature range (DTR), as representative indices of temperature variability, are contributors to the development and exacerbation of COPD. However, few studies are available in Chinese population. In this study, we aimed to assess the associations of temperature variability on COPD mortality in a fast developing city in China. Using the mortality surveillance system, we obtained a total of 7,863 deaths attributed to COPD from 2014 to 2016. Quasi-Poisson generalized linear regression with distributed lag non-linear model was applied to explore the associations between temperature variability and COPD deaths, after controlling for the potential confounders, including relative humidity, day of week, public holiday, and long-term trend. A J-shaped association of DTR and a reversely J-shaped association of temperature for COPD mortality were observed. Risk estimates showed that the relative risks (RRs) of COPD mortality with extreme high DTR at lag 0 and 0–7 days were 1.045 (95% CI: 0.949–1.151) and 1.460 (95% CI: 1.118–1.908), and the extreme high temperature at lag 0 and 0–7 days were 1.090 (95% CI: 0.945–1.256) and 1.352 (95% CI: 1.163–1.572). Our findings suggest that short-term exposure to extreme temperature was associated with mortality for COPD in Hangzhou. The evidence has implications for policy decision-making and targeted interventions.

Abstract:Epidemiological studies increasingly provide evidence about the adverse health effects of temperature variability (TV), which reflects short-term intra- and inter-day temperature change. However, calculation of TV only considers the temporal variability and lacks spatial variability. This study intends to investigate whether the lack of spatial variability in TV calculations has biased the health effect estimates. We collected daily data from the fine-gridded hourly temperatures and more than 2 million all-cause mortality counts in Zhejiang province in China from 2009 to 2015. A spatiotemporal TV index was developed by calculating the standard deviation of the hourly temperatures based on records from multiple sites. This new index could be compared to the two typical temporal TV indices that are calculated based on the hourly temperatures from one-site and area-average records. The three types of TV are compared using a three-stage analytical approach: district-specific time series Poisson regression, meta-analysis, and calculation of attributable mortality fraction. We observe that both spatiotemporal and temporal TVs produce very similar TV-mortality associations, attributable mortality fractions, and model fits at the district level. For example, the mortality increase associated for every increase of 1 degrees C during 0-7 exposure days is 1.53% (95% CI: 1.31, 1.73) in spatiotemporal TV, whereas it is 1.48% (95% CI: 1.27, 1.68) and 1.45% (95% CI: 1.24, 1.67) in the one-site and area-average temporal TV, respectively. Thus, time series models using temporal TV index are equally good at estimating the associations between TV and mortality as spatiotemporal TV at the district level in population-based epidemiological studies in China. Epidemiological studies using temperature from one site or the averages of multiple sites in TV calculation will not bias the effect estimates of TV. Our study could provide an important guidance method for future TV-related research in China and even in other countries.
Magnitude and direction of temperature variability affect hospitalization for myocardial infarction and stroke: population-based evidence from Guangzhou, China

Abstract:Relationships between nonoptimal temperatures and cardiovascular disease (CVD) mortality have been well documented. However, evidence of the association between temperature variability (TV) and CVD morbidity is limited. This study aimed to quantify the risk and burden of CVD-related hospitalization associated with the magnitude and direction of TV. Data on meteorology and population-based hospitalizations for myocardial infarction (MI) and stroke were collected in Guangzhou, China, from 2013 to 2017. Hourly temperature variability (HTV) was measured as the standard deviation of hourly temperature records over specific exposure days. The direction (upward or downward) of HTV was defined as the average daily mean temperature change relative to that of the previous day during the exposure period. Quasi-Poisson regression was applied to assess the impact of HTV after adjusting for the daily mean temperature, and the hospitalization fractions attributable to HTV were calculated. A 1 °C-increase in HTV was significantly associated with a 2.24% and 1.72% increase in hospitalizations for MI and hemorrhagic stroke (HS) at lag 0–1 d, respectively, and a 1.55% increase in hospitalizations for ischemic stroke (IS) at lag 0–3 d. During the study period, 5.99%, 4.64%, and 4.53% of MI, HS, and IS hospitalizations, respectively, were attributable to HTV. The upward TV exerts acute effects on CVD hospital admissions, whereas the impact of downward TV generally lags. These findings highlight the importance of the magnitude and direction of temperature fluctuations, in addition to the mean level, in assessing the adverse health impacts of temperature variations.

Effect of Ambient Temperature on Emergency Department Visits in Shanghai, China: a Time Series Study

Abstract:BACKGROUND:Many studies have examined the association between ambient temperature and mortality. However, less evidence is available on the temperature effects on gender- and age-specific emergency department visits, especially in developing countries. In this study, we examined the short-term effects of daily ambient temperature on emergency department visits (ED visits) in Shanghai.METHODS:Daily ED visits and daily ambient temperatures between January 2006 and December 2011 were analyzed. After controlling for secular and seasonal trends, weather, air pollution and other confounding factors, a Poisson generalized additive model (GAM) was used to examine the associations between ambient temperature and gender- and age-specific ED visits. A moving average lag model was used to evaluate the lag effects of temperature on ED visits.RESULTS:Low temperature was associated with an overall 2.76% (95% confidence interval (CI): 1.73 to 3.80) increase in ED visits per 1°C decrease in temperature at Lag1 day, 2.03% (95% CI: 1.04 to 3.03) and 2.45% (95% CI: 1.40 to 3.52) for males and females. High temperature resulted in an overall 1.78% (95% CI: 1.05 to 2.51) increase in ED visits per 1°C increase in temperature on the same day, 1.81% (95% CI: 1.08 to 2.54) among males and 1.75% (95% CI: 1.03 to 2.49) among females. The cold effect appeared to be more acute among younger people aged <45 years, whereas the effects were consistent on individuals aged ≥65 years. In contrast, the effects of high temperature were relatively consistent over all age groups.CONCLUSIONS:These findings suggest a significant association between ambient temperature and ED visits in Shanghai. Both cold and hot temperatures increased the relative risk of ED visits. This knowledge has the potential to advance prevention efforts targeting weather-sensitive conditions.

Abstract:This study assessed the links between daily mean temperature and emergency room (ER) admissions for total and cause-specific cardiovascular diseases (CVD) in Lanzhou, China from 2013 to 2019. A quasi-Poisson Generalized Additive Model (GAM) and a Distributed Lag Non-Linear Model (DLNM) were used to determine the effects of temperature on total and cause-specific cardiovascular emergency visits. The relative risks (RR) at cold (hot) temperatures were calculated by comparing the 5th (95th) centile of temperature with the minimum morbidity temperature (MMT). Exposure–response curves demonstrating an inverted U-shape or an irregular M-shape association were observed between temperature and total and cause-specific CVD. The study found that both cold and hot temperatures had negative impacts on emergency room visits for various cardiovascular diseases. For people with total CVD, heart rhythm disturbances (HRD), or cerebrovascular diseases (CD), females were more sensitive to temperature than males, while for ischemic heart disease (IHD) and heart failure (HF), males were more vulnerable to temperature. The < 65 years old with total CVD, IHD, HRD, or CD was more susceptible to the effects of temperature. The results indicated that the relationship between temperature and total and cause-specific CVD was nonlinear, and susceptibility to temperature varied across disease subtype, gender, and age.

Temperature Variability and Mortality in Rural and Urban Areas in Zhejiang Province, China: an Application of a Spatiotemporal Index

Abstract:Background: Temperature variability (TV) is a potential trigger for death in urban areas, but there is little evidence of this in rural areas. In addition, a typical TV index only considers the temporal variability of temperature and ignores its spatial variability, which should be considered due to the effects of human mobility. Here this study aimed to 1) develop a novel spatiotemporal TV index accounting for human mobility; and 2) based on this index, explore the urban-rural differences in TV-mortality associations in China. Methods: We collected daily data on fine-gridded hourly temperatures and >2 million deaths that occurred in Zhejiang province, China from 2009 to 2015. A spatiotemporal TV index was developed by calculating the standard deviation of the hourly temperatures from multi-site records over the course of several exposure days. A three-stage analysis was performed to estimate the mortality risks and mortality burdens of TV. Stratified analyses were performed by cause-specific mortality, urban/rural district, age and gender. Results: Significant associations were found between TV and all types of targeted diseases, age groups, and genders. Percentage increase inmortality associated with a 1 degrees C increase in TV at 0-7 exposure days were found to be higher for rural dwellers than urban dwellers in the warm season [ for all-cause mortality, 2.07% (95% CI: 1.49%, 2.64%) vs. 1.16% (95% CI: 0.70%, 1.62%)]. An estimated all-causemortality fraction of 5.33% was attributable to TV, with 4.99% in urban areas and 6.02% in rural areas. The elderly (aged 65+years) and females were more sensitive to TV than young people and males, respectively. Conclusions: A spatiotemporal TV index was developed, considering both the temporal and spatial variability of temperatures. TV is an independent health risk factor. In China, rural areas generally suffer greater TV-related mortality risks than urban areas in the warm season. Our findings have important implications for developing area-, cause-, and group-specific adaptation strategies and emergency planning to reduce TV-related mortality. (C) 2018 Elsevier B.V. [Association between ambient temperature and hospital emergency room visits for cardiovascular diseases: a case-crossover study].

Abstract:OBJECTIVE:To explore the association between ambient average temperature and hospital emergency room visits for cardiovascular diseases (International Classification of Diseases, Tenth Vision ICD-10: I00 - I99) in Beijing, China. METHODS:Data was collected on daily hospital emergency room visits for cardiovascular diseases from Peking University Third Hospital, including meteorological data (daily average temperature, relative humidity, wind speed, and atmospheric pressure) from the China Meteorological Data Sharing Service System, and on air pollution from the Beijing Municipal Environmental Monitoring Center. Time-stratified case-crossover design was used to analyze data on 4 seasons. RESULTS:After adjusting data on air pollution, 1 degree ( degrees C) increase of ambient average temperature would associate with the emergency room visits of odds ratio (ORs) as 1.282 (95%CI: 1.250 - 1.315), 1.027 (95%CI: 1.001 - 1.055), 0.661 (95%CI: 0.637 - 0.687), and 0.960 (95%CI: 0.937 - 0.984) in spring, summer, autumn, and winter respectively. After controlling the influence of relative humidity, wind speed, and atmospheric pressure, 1 degrees C increase in the ambient average temperature would be associated with the emergency room visits on ORs value as 1.423 (95%CI: 1.377 - 1.471), 1.082 (95%CI: 1.041 - 1.124), 0.633 (95%CI: 0.607 - 0.660) and 0.971 (95%CI: 0.944 - 1.000) in spring, summer, autumn, and winter respectively. CONCLUSION:These data on outcomes suggested that the elevated level of ambient temperature would increase the hospital emergency room visits for cardiovascular diseases in spring and summer while the elevated level of ambient temperature would decrease the hospital emergency room visits for the cardiovascular diseases in autumn and winter, suggesting that patients with cardiovascular diseases should pay attention to the climate change.
Vulnerability to the impact of temperature variability on mortality in 31 major Chinese cities.

Abstract:Few studies have analyzed the health effects of temperature variability (TV) accounting for both interday and intraday variations in ambient temperature. In this study, TV was defined as the standard deviations of the daily minimum and maximum temperature during different exposure days. Distributed lag non-linear Poisson regression model was used to examine the city-specific effect of TV on mortality in 31 Chinese municipalities and provincial capital cities. The national estimate was pooled through a meta-analysis based on the restricted maximum likelihood estimation. To assess effect modification on TV-mortality association by individual characteristics, stratified analyses were further fitted. Potential effect modification by city characteristics was performed through a meta-regression analysis. In total, 259 million permanent residents and 4,481,090 non-accidental deaths were covered in this study. The effect estimates of TV on mortality were generally increased by longer exposure days. A 1 °C increase in TV at 0–7 days' exposure was associated with a 0.60% (95% CI: 0.25–0.94%), 0.65% (0.24–1.05%), 0.82% (0.29–1.36%), 0.86% (0.42–1.31%), 0.98% (0.57–1.39%) and 0.54% (−0.11-1.20%) increase in non-accidental, cardiovascular, IHD, stroke, respiratory and COPD mortalities, respectively. Those with lower levels of educational attainment were significantly susceptible to TV. Cities with dense population, higher mean temperatures, and relative humidity and lower diurnal temperature ranges also had higher mortality risks caused by TV. This study demonstrated that TV had considerable health effects. An early warning system to alert residents about large temperature variations is recommended, which may have a significant impact on the community awareness and public health.
Temperature Fluctuation and Acute Myocardial Infarction in Beijing: an Extended Analysis of Temperature Ranges and Differences

Abstract:PurposeFew studies examined the relationship between temperature fluctuation metrics and acute myocardial infarction (AMI) hospitalizations within a single cohort. We aimed to expand knowledge on two basic measures: temperature range and difference.MethodsWe conducted a time-series analysis on the correlations between temperature range (TR), daily mean temperature differences (DTDmean), and daily mean-maximum/minimum temperature differences (TDmax/min) and AMI hospitalizations, using data between 2013 and 2016 in Beijing, China. The effects of TRn and DTDmeann over n-day intervals were compared, respectively. Subgroup analysis by age and sex was performed.ResultsA total of 81,029 AMI hospitalizations were included. TR1, TDmax, and TDmin were associated with AMI in J-shaped patterns. DTDmean1 was related to AMI in a U-shaped pattern. These correlations weakened for TR and DTDmean with longer exposure intervals. Extremely low (1st percentile) and high (5°C) DTDmean1 generated cumulative relative risk (CRR) of 2.73 (95% CI: 1.56–4.79) and 2.15 (95% CI: 1.54–3.01). Extremely high TR1, TDmax, and TDmin (99th percentile) correlated with CRR of 2.00 (95% CI: 1.73–2.85), 1.71 (95% CI: 1.40–2.09), and 2.73 (95% CI: 2.04–3.66), respectively. Those aged 20–64 had higher risks with large TR1, TDmax, and TDmin, while older individuals were more affected by negative DTDmean1. DTDmean1 was associated with a higher AMI risk in females.ConclusionTemperature fluctuations were linked to increased AMI hospitalizations, with low-temperature extremes having a more pronounced effect. Females and the older adult were more susceptible to daily mean temperature variations, while younger individuals were more affected by larger temperature ranges.
Dynamic Total Order Protocol Replacement

Abstract:The dynamic total order protocol replacement mechanism proposed in this paper allows an application to change, in run-time, the total order broadcast protocol it is using. In a conventional setting, without such a replacement mechanism, the application should be stopped, reconfigured and then restarted, in order to be adapted to a changing environment. The problem of this stop-and-restart approach is that it stops the normal operation of the application, reducing its availability. A dynamic replacement mechanism allows the application to switch from the current total order protocol it is using to a different one that performs better under the new operation conditions. Moreover, the switch should be performed in a transparent manner, from both the application and its users. This paper presents a software architecture for dynamically switching total order protocols, including the switching algorithm, its correctness justification and an experimental evaluation of its performance.
Perspectives in Diabetes The Aldose Reductase Controversy

Abstract:ext to the debate as to whether chronic hyperglycemia is the primary cause of the late vascular complications of diabetes, the longest running controversy among researchers and clinicians studying this disease is the role of the sorbitol pathway, particularly its initial enzymatic step, the reduction of glucose to sorbitol by aldose reductase in the pathogenesis of these complications. Because the outcome of the Diabetes Control and Complications Trial (DCCT) (1) appears to have effectively resolved the first issue, the aldose reductase controversy now takes center stage. The sorbitol pathway hypothesis is not the only mechanism for the pathogenesis of the complications of diabetes to have achieved serious consideration. Among others, for example, nonenzymatic glycation of proteins (2) is a plausible, if still largely untested, possibility. But over the more than 30 years since the British biochemist, Ruth vari Heyningen, first demonstrated an excess of sorbitol in the lenses of diabetic rats (3), a substantial accumulation of evidence has supported the sorbitol pathway as the principal actor in at least some of the late complications of diabetes in humans and experimental animals. 'This view is not unchallenged, however. In this discussion, I will summarize some of the evidence supporting the role of aldose reductase in producing several of the late complications of diabetes, but also point out those contradictory findings that render the sorbitol pathway hypothesis controversial.

Abstract:This study assessed the impact of various temperature indices, including mean temperature (MT), diurnal temperature range (DTR), and temperature changes between neighboring days (TCN) on hospitalization rates for cardiovascular system diseases among residents of Zhangye City, a typical western city in China. The Quasi-Poisson generalized additive regression model (GAM) in conjunction with a distributed lag nonlinear model (DLNM) was applied to estimate the association of temperature indices with CVD hospitalization rates in Zhangye City during the periods of 2015–2021. The exposure–response relationship and relative risk were discussed and stratified analyses by age and gender were conducted. We found that the hospitalization rates of cardiovascular disease (CVD) patients in Zhangye City was significantly related to different temperature indicators (MT, DTR, TCN). Both low and high MT, DTR, and TCN increased the risk of cardiovascular disease (CVD) among residents. Besides, different demographic populations exhibited distinct sensitivities to temperature conditions. Relevant authorities should devise corresponding preventive and control measures to protect vulnerable populations.

Abstract:This study aimed to evaluate the effects of temperature on CVD hospitalizations in suburban farmers of Zhangye, Wuwei, and Longnan with different climatic zones in Gansu Province, China. We obtained daily CVD hospitalization data for the three cities from the medical records of inpatients with the New Rural Cooperative Medical Scheme from the period January 1, 2018 to December 31, 2019. Generalized additive model based on Quasi-Poisson distribution and distributed lag nonlinear model were used to evaluate the effects of temperature on CVD hospitalizations, with analyses stratified by gender and age (adults: <65 years; the elderly:≥65 years). The effect of minimum-morbidity temperature was used as the reference value to calculate estimates. M-shaped exposure-response curves were observed between temperature and CVD hospitalizations in the three cities. There are some differences in the susceptible populations of temperature in different regions, gender and age. This study showed that both low and high temperatures could increase the relative risks for CVD hospitalizations in the three cities. High temperatures have more significant and long-lasting effects than low temperatures.

Abstract:ISEE-0083 Background and Objective: Temperature is increasingly being recognized as an important determinant of cardiocerebrovascular diseases. This study explores the association between the daily minimum temperature in summer seasons and the hospital emergency room visits for cardiocerebrovascular diseases in Beijing, China. Methods: The data of the daily hospital emergency room visits for cardiocerebrovascular diseases (ICD-10: I00∼I99) from one of Peking University Hospitals, the data of relevant air pollution from the Beijing Municipal Environmental Monitoring Center and the data of meteorological index from China meteorological data sharing service system were collected respectively for statistic analysis with conditional logistic regression model, based on time-stratified case-crossover design. Results: After adjusting the level of SO2, NO2, and PM10 in the air, the results showed that each 1°C increase in the daily minimum temperature in the summer was positively associated with the intraday hospital emergency room visits of the total cardiocerebrovascular diseases (P < 0.05) and the value of ORs is 1.039 (95%CI:1.008∼1.071). There were no significant associations between the coronary heart disease, cardiac arrhythmia, heart failure disease, cerebrovascular diseases and the daily minimum temperature in the summer (P > 0.05). In multi-factor model, considering relative humidity, 1°C increase in the daily minimum temperature was associated with the coronary heart disease (ORs:1.095, 95%CI:1.001∼1.075) and the cerebrovascular diseases (ORs:1.050, 95%CI:1.002∼1.100) (P < 0.05) respectively. Conclusion: The increase of daily minimum temperature in the summer can raise the hospital emergency room visits of the total cardiocerebrovascular diseases, and the coronary heart disease and cerebrovascular diseases together with relative humidity.
Long-term Variations in the Association Between Ambient Temperature and Daily Cardiovascular Mortality in Shanghai, China

Abstract:Objective: The objective of this study was to explore the long-term variation in the association between ambient temperature and daily cardiovascular (CVD) mortality in Shanghai, China.Materials and methods: We collected daily data on ambient temperature and CVD mortality from a central urban district in Shanghai, China, during the period from 1981 to 2012. We used overdispersed generalized additive Poisson regression together with a distributed lag nonlinear model to estimate potentially lagged and nonlinear effects of temperature on CVD mortality after controlling for the seasonality, relative humidity, day of the week, holidays and population size. To allow for the evaluation of long-term variations in the effects, we divided the entire study period into six sub-periods (1981-1985, 1986-1990, 1991-1995, 1996-2000, 2001-2005, and 2006-2012) and analyzed the effect estimates in each sub-period separately.Results: The association between temperature and daily CVD mortality was J-shaped with both low and high temperatures increasing the risk of CVD deaths. The effects of extremely low temperatures were delayed and persisted for two weeks, while extreme hot effects were limited to the first five days followed by a significant mortality displacement (9 days). The relative risks (RRs) of extremely low, moderately low, moderately high, and extremely high temperatures comparing the 1st, 10th, 90th, and 99th percentile with the reference temperature (26 degrees C) over the cumulative lags of 0-14 days were 1.95 [95% confidence interval (CI): 1.84,2.07], 1.61(95% CI: 1.57,1.66), 1.03 (95% CI: 1.01,1.05), and 1.14(95% CI: 1.07,1.21). The RRs for extremely low and moderately low temperature attenuated substantially from 9.78 and 5.52 in 1981-1985 to 1.42 and 1.18 in 2006-2012, respectively, but the RRs remained almost stable for extremely high and moderately high temperatures.Conclusions: This time-series study suggested that there might have been some human adaptation to low ambient temperature in Shanghai, China, over the last 3 decades. Hourly Personal Temperature Exposure and Heart Rate Variability: A Multi-Center Panel Study in Populations at Intermediate to High-Risk of Cardiovascular Disease

Abstract:Background: Several studies reported temperature exposure was associated with altered cardiac automatic function, while this effect of temperature on hourly heart rate variability (HRV) among populations with cardiovascular risks was seldom addressed.Methods: We conducted this panel study in four Chinese cities with three repeated visits among 296 participants at intermediate to high-risk of cardiovascular disease (CVD). Real-time temperature level and 24-h ambulatory electrocardiogram were monitored during each seasonal visit. Linear mixed-effects models were used to investigate associations between individual temperature and HRV parameters, and the seasonal effects and circadian effect were also evaluated.Results: We found the overall downward trend of hourly HRV associated with acute exposure to higher temperature. For each 1 degrees C increment in temperature of 1-3 h prior to HRV measurements (lag 1-3 h), hourly standard deviation of normal-to-normal intervals (SDNN) decreased by 0.38% (95% confidence interval [CI]: 0.22, 0.54), 0.28% (95% CI: 0.12, 0.44), and 0.20% (95% CI: 0.04, 0.36), respectively. Similar inverse associations between temperature and HRV were observed in stratified analyses by temperature level. Inverse associations for cold and warm seasons were also observed, despite some effects gradually decreased and reversed in the warm season as lag times extended. Moreover, HRV showed a more significant reduction with increased temperature during daytime than nighttime. Percent change of hourly SDNN was -0.41% (95% CI: -0.62, -0.21) with 1 degrees C increment of lag 1 h during daytime, while few obvious changes were revealed during nighttime.Conclusions: Generally, increasing temperature was significantly associated with reduced HRV. Inverse relationships for cold and warm seasons were also observed. Associations during daytime were much more prominent than nighttime. Our findings clarified the relationship of temperature with HRV and provided evidence for prevention approaches to alleviate cardiac automatic dysfunction among populations at intermediate to high-risk of CVD.

Abstract:In the context of global climate change, far less is known about the impact of long-term temperature variability (TV), especially in developing countries. The current study aimed to estimate the effect of long-term TV on the incidence of cardiovascular disease (CVD) in China. A total of 23,721 individuals with a mean age of 56.15 years were enrolled at baseline from 2012-2016 and followed up during 2017 to 2019. TV was defined as the standard deviation of daily temperatures during survey years and was categorized into tertiles (lowest≤ 8.78°C, middle = 8.78 to 10.07°C, highest ≥ 10.07°C). The Cox proportional hazards regression was used to estimate the multivariable-adjusted hazard ratio (HR) between TV and CVD. During the median follow-up of 4.65 years, we ascertained 836 cases of incident CVD. For per 1°C increase in TV, there was a 6% increase of CVD (HR =1.06 [95% confidence interval (CI): 1.01-1.11]). A significant positive trend was observed between CVD risk and increasing levels of TV compared to the lowest tertile [HR = 1.34 (95% CI: 1.13–1.59) for the medium tertile, HR = 1.72 (95% CI: 1.35–2.19) for the highest tertile, Ptrend < 0.001]. Exposure to high TV would lose 2.11 disease-free years for the population aged 35 to 65 years and 66 CVD cases (or 7.95% cases) could been attributable to TV higher than 8.11 °C in the current study. The current findings suggested that long-term TV was associated with a higher risk of CVD incidence, it is needed to reduce the TV-related adverse health effect.

Abstract:Rheumatic heart disease (RHD) remains a serious public health burden in developing countries. We conducted a time-series study to explore the association between ambient temperature and daily hospital admissions for RHD in Shanghai, China. We collected data on daily hospital admissions for RHD from 2013 to 2015 from the database of Shanghai Health Insurance System. We applied the generalized additive models together with the distributed lag nonlinear model to estimate the association between temperature and RHD hospital admissions after controlling for relative humidity, time trend, day of the week, and holidays. Stratification analyses by age and gender were performed to evaluate their potential effect modification. A total of 4178 cases of RHD hospitalizations were identified over the study period. There were almost linear, positive, and significant associations between daily mean temperature and RHD hospital admissions with higher risks at hotter days. Compared to reference temperature (0 °C), the cumulative risks of moderate heat (the 90th percentile of temperature, 28.0 °C) and extreme heat (the 99th percentile of temperature, 33.5 °C) over lags 0–5 days were 2.55 (95% confidence interval 1.14, 5.73) and 3.22 (95% confidence interval 1.36, 7.61), respectively. These associations were significantly stronger in older people than in younger people. This study indicated larger risks of RHD hospital admissions associated with higher temperature, especially in older people. Our findings provided first-hand epidemiological evidence regarding the effects of ambient temperature on RHD incidence.
