Abstract:Objective: The study aims to investigate the relationship between amplitude modulation (AM) of EEG and anesthesia depth during general anesthesia. Methods: In this study, Holo-Hilbert spectrum analysis (HHSA) was used to decompose the multichannel EEG signals of 15 patients to obtain the spatial distribution of AM in the brain. Subsequently, HHSA was applied to the prefrontal EEG (Fp1) obtained during general anesthesia surgery in 15 and 34 patients, and the alpha-theta and alpha-delta regions of feature (ROFs) were defined in Holo-Hilbert spectrum (HHS) and three features were derived to quantify AM in ROFs. Results: During anesthetized phase, an anteriorization of the spatial distribution of AMs of alpha-carrier in brain was observed, as well as AMs of alpha-theta and alpha-delta in the EEG of Fp1. The total power ( P-total ), mean carrier frequency ( M-FC ) and mean amplitude frequency ( MFAM ) of AMs changed during different anesthesia states. Conclusion: HHSA can effectively analyze the cross-frequency coupling of EEG during anesthesia and the AM features may be applied to anesthesia monitoring. Significance: The study provides a new perspective for the characterization of brain states during general anesthesia, which is of great significance for exploring new features of anesthesia monitoring.

Abstract:Background: The integrated information theory (IIT) of consciousness introduces a measure O to quantify consciousness in a physical system. Directly related to this, general anesthesia aims to induce reversible and safe loss of consciousness (LOC). We sought to propose an electroencephalogram (EEG)-based IIT index OEEG to evaluate various states of consciousness under general anesthesia.Methods: Based on the definition of mutual information, we estimated the OEEG by maximizing the integrated information under various time lags. We used the binning method to cut the nonGaussian EEG data for estimating mutual information. We tested two EEG databases collected from propofol-(n = 20) and sevoflurane-induced (n = 15) anesthesia, and especially, we compared the OEEG of drowsy (n = 7) and responsive participants (n = 13) under propofol anesthesia. We compared the effectiveness of OEEG with the estimated bispectral index (eBIS).Results: In all EEG frequency bands, we observed a negative correlation between OEEG and end-tidal sevoflurane concentration under sevoflurane-induced anesthesia (p < 0.001, BF10 > 6000). Under propofol-induced anesthesia, drowsy participants in moderate sedation (6.96 +/- 0.26(mean +/- SD)) showed decreased alpha-band OEEG compared with baseline (7.40 +/- 0.53, p = 0.016, BF10 = 3.58), no significant difference was observed for responsive participants. Oppositely, the responsive participants in moderate sedation (-5.32 +/- 0.38) showed decreased eBIS compared with baseline (-4.94 +/- 0.40, p = 0.03, BF10 = 2.41).Conclusions: These findings may enable monitors of the anesthetic state that can distinguish consciousness and unconsciousness rather than the changes of anesthetic concentrations. The alpha-band OEEG is promising for deriving the gold standard for depth of anesthesia monitoring.

Abstract:Background: Recent studies have demonstrated that changes in brain information processing during anesthetic-nduced loss of consciousness (LOC) might be influenced by phase-amplitude coupling (PAC) in electroencephalogram (EEG). However, most anesthesia research on PAC typically focuses on delta and alpha oscillations. Studies of spatial-frequency characteristics by PAC for EEG may yield additional insights into understanding the impaired information processing under anesthesia unconsciousness and provide potential improvements in anesthesia monitoring. Objective: Considering different frequency bands of EEG represent neural activities on different spatial scales, we hypothesized that functional coupling simultaneously appears in multiple frequency bands and specific brain regions during anesthesia unconsciousness. In this paper, PAC analysis on whole-brain EEG besides delta and alpha oscillations was investigated to understand the influence of multiple cross-frequency coordination coupling on information processing during the loss and recovery of consciousness. Method: EEG data from fifteen patients without cognitive diseases (7 males/8 females, aged 43.8 +/- 13.4 years, weighing 63.3 +/- 14.9 kilograms) undergoing lower limb surgery and sevoflurane anesthesia was recorded. To investigate the spatial-frequency characteristics of EEG source signals during loss and recovery of consciousness, the time-resolved PAC (tPAC) was calculated to reflect cross-frequency coordination in different frequency bands (delta, theta, alpha, beta, gamma) and different functional regions (Visual, Limbic, Dorsal attention, Ventral attention, Default, Somatomotor, Control, Salience networks). Furthermore, different patterns (peak-max and trough-max) of PAC were examined by constructing phase-amplitude histograms using phase bins to investigate the different information processing during LOC. The multivariate analysis of variance (MANOVA) and trend analysis were used for statistical analysis. Results: Theta-alpha and alpha-beta PAC were observed during sevoflurane-induced LOC, which significantly changed during loss and recovery of consciousness (E-4,E-70 = 16.553, p < 0.001 for theta-alpha PAC and F-4,F-70 = 12.446, p < 0.001 for alpha-beta PAC, MANOVA test). Simultaneously, PAC was distributed in specific functional regions, i.e., Visual, Limbic, Default, Somatomotor, etc. Furthermore, peak-max patterns of theta-alpha PAC were observed while alpha-beta PAC showed trough-max patterns and vice versa. Conclusion: Theta-alpha and alpha-beta PAC observed in specific brain regions represent information processing on multiple spatial scales, and the opposite patterns of PAC indicate opposite information processing on multiple spatial scales during LOC. Our study demonstrates the regulation of local-global information processing during sevoflurane-induced LOC. It suggests the utility of evaluating the balance of functional integration and segregation in monitoring anesthetized states.

Abstract:Background. In intensive care, monitoring the depth of anesthesia during surgical procedures is a key element in the success of the medical operation and postoperative recovery. However, despite the development of anesthesia thanks to technological and pharmacological advances, its side effects such as underdose or overdose of hypnotics remain a major problem. Observation and monitoring must combine clinical observations (loss of consciousness and reactivity) with tools for real-time measurement of changes in the depth of anesthesia. Methodology. In this work, we will develop a noninvasive method for calculating, monitoring, and controlling the depth of general anesthesia during surgery. The objective is to reduce the effects of pharmacological usage of hypnotics and to ensure better quality recovery. Thanks to the overall activity of sets of neurons in the brain, we have developed a BIS technique based on bispectral analysis of the electroencephalographic signal EEG. Discussion. By collecting the electrical voltages from the brain, we distinguish light sleep from deep sleep according to the values of the BIS indicator (ranging from 0 : sleep to 100 : wake) and also control it by acting on the dosage of propofol and sevoflurane. We showed that the BIS value must be maintained during the operation and the anesthesia at a value greater than 60. Conclusion. This study showed that the BIS technology led to an optimization of the anesthetic management, the adequacy of the hypnotic dosage, and a better postoperative recovery.

Abstract:Abstract Postoperative delirium is a serious sequela of surgery and surgery-related anaesthesia. One recommended method to prevent postoperative delirium is using bi-frontal EEG recording. The single, processed index of depth of anaesthesia allows the anaesthetist to avoid episodes of suppression EEG and excessively deep anaesthesia. The study data presented here were based on multichannel (19 channels) EEG recordings during anaesthesia. This enabled the analysis of various parameters of global electrical brain activity. These parameters were used to compare microstate topographies under anaesthesia with those in healthy volunteers and to analyse changes in microstate quantifiers and EEG global state space descriptors with increasing exposure to anaesthesia. Seventy-three patients from the Surgery Depth of Anaesthesia and Cognitive Outcome study (SRCTN 36437985) received intraoperative multichannel EEG recordings. Altogether, 720 min of artefact-free EEG data, including 210 min (29.2%) of suppression EEG, were analysed. EEG microstate topographies, microstate quantifiers (duration, frequency of occurrence and global field power) and the state space descriptors sigma (overall EEG power), phi (generalized frequency) and omega (number of uncorrelated brain processes) were evaluated as a function of duration of exposure to anaesthesia, suppression EEG and subsequent development of postoperative delirium. The major analyses involved covariate-adjusted linear mixed-effects models. The older (71 ± 7 years), predominantly male (60%) patients received a median exposure of 210 (range: 75–675) min of anaesthesia. During seven postoperative days, 21 patients (29%) developed postoperative delirium. Microstate topographies under anaesthesia resembled topographies from healthy and much younger awake persons. With increasing duration of exposure to anaesthesia, single microstate quantifiers progressed differently in suppression or non-suppression EEG and in patients with or without subsequent postoperative delirium. The most pronounced changes occurred during enduring suppression EEG in patients with subsequent postoperative delirium: duration and frequency of occurrence of microstates C and D progressed in opposite directions, and the state space descriptors showed a pattern of declining uncorrelated brain processes (omega) combined with increasing EEG variance (sigma). With increasing exposure to general anaesthesia, multiple changes in the dynamics of microstates and global EEG parameters occurred. These changes varied partly between suppression and non-suppression EEG and between patients with or without subsequent postoperative delirium. Ongoing suppression EEG in patients with subsequent postoperative delirium was associated with reduced network complexity in combination with increased overall EEG power. Additionally, marked changes in quantifiers in microstate C and in microstate D occurred. These putatively adverse intraoperative trajectories in global electrical brain activity may be seen as preceding and ultimately predicting postoperative delirium.
Changes in complexities and power spectrum of rat electroencephalogram under various anesthesia depth

Abstract:The algorithmic complexity, the approximate entropy and the power spectrum of rat electroencephalogram (EEG) under different stages of anesthesia were calculated and analyzed. For synchronous EEGs, both complexity values decreased significantly with the development of anesthesia depth from light to deep. Although powers of synchronous EEG in high frequency bands of theta, alpha and beta gradually decreased, its total power and its power in delta band did not decrease until the deepest anesthesia. For de-synchronous EEGs, the complexity values did not change much with the development of anesthesia depth, and after the anesthesia was deep enough the de-synchronous EEGs would not appear any more, which indicated that the activities in the midbrain reticular formation had been inhibited. Powers of desynchronous EEG in high frequency bands of alpha and beta decreased gradually from light anesthesia stages. These findings suggest that more EEG features can be revealed by applying both methods of the complexity measures and the power spectral analysis.

Abstract:Electroencephalogram (EEG)-guided anesthesia is indispensable in modern operating rooms and has become established as the standard form of monitoring. Many anesthesiologists rely on processed EEG indices in the hope of averting anesthesia-related complications, such as intraoperative awareness, postoperative delirium and other cognitive complications in their patients. This educational review aims to provide information on the five most prevalent monitors used to guide depth of sedation during general anesthesia. This article elucidates the principles underpinning the application of these monitors where known, which are generally based on power in various EEG frequency bands and on the burst suppression pattern. Convinced that EEG-guided anesthesia has the potential of benefitting many surgical patients, it is felt that many basic principles and shortcomings of processed EEG indices need to be better understood in the clinical practice. After discussing the different monitors and clinically relevant data from the literature, the article gives a short practical guidance on how to critically interpret processed EEG information and troubleshooting of confounded indices in the context of clinical situations.

Abstract:BACKGROUND: Perioperative depth monitoring techniques, such as electroencephalography bispectral index (BIS), entropy index, and auditory evoked potential, are commonly used to assess anesthesia depth. However, the influence of patient positioning changes, particularly in gynecological surgeries where a head-down position is often required, on the accuracy of these monitoring indices remains unexplored. OBJECTIVE: The aim of the our study was to observe the impact of patient position changes on the monitoring value of entropy and BIS to identify a more sensitive method of anaesthesia depth monitoring for gynaecological surgery patients. METHODS: We conducted a study involving 40 women undergoing general anesthesia, during which routine monitoring of vital signs, including electrocardiogram (ECG), heart rate (HR), noninvasive arterial blood pressure (NIBP), oxyhemoglobin saturation (SpO2), and end-expiratory carbon dioxide (PetCO2), was initiated. Entropy and BIS devices were affixed to the patients' foreheads after alcohol sterilization to record brain activity. Tracheal intubation was performed following anesthesia induction. Throughout anesthesia maintenance, the value of BIS and response entropy (RE) were monitored and maintained between 40 and 50 by adjusting the infusion rate of propofol and remifentanil with Target Controlled Infusion (TCI, Mintopharmacokinetics model). Dosing for infusion control utilized corrected weight (height-105). Data were recorded before and after position changes, including tilting the operating table to head-down positions of 15 and 25 degrees, returning to a supine position, and elevating the head to 15 and 25-degree angles. BIS and entropy values at different time points were compared between the groups. RESULTS: Both BIS and entropy values increased from supine to head-down position and decreased from supine to head-up position, with entropy changes preceding those of BIS. Heart rate increased after head-up and decreased after head-down, while mean blood pressure (MBP) exhibited the opposite effect on heart rate. Significant correlations were found between heart rate and BIS (correlation coefficient: -0.43) and RE (correlation coefficient: -0.416), as well as between MBP and BIS (correlation coefficient: 0.346) and RE (correlation coefficient: 0.384). CONCLUSION: Changes in patient position can significantly affect the value of RE and BIS, as changes in entropy occur earlier than changes in the BIS.

Abstract:One of the fundamental questions of clinical pharmacology is: Why do different persons respond differently to the same dose of the same drug? Pioneers in this field, such as Dr. B. B. Brodie, discovered that different persons had different plasma concentrations of drugs when'given identical doses and that these differences in concentrations were often due to individual differences in the rates of metabolism of these drugs. Drug metabolism and disposition then became a legitimate area for medical research. Otherpioneers, such as Dr. R. T. Williams, began a study of the chemical reactions of drug metabolism and identified reaction pathways by which drugs were biotransformed. Then investigators began to study the factors controlling the rates of these pathways to learn some of the reasons for the individual differences in rates of drug metabolism. A person's inheritance appeared to be one of the important factors controlling the rate of drug metabolism and, through this, the drug conceptration in the body and the intensity of drug effects.

Abstract:The electroencephalographic (EEG) activity during anesthesia emergence contains information about the risk for a patient to experience postoperative delirium, but the EEG dynamics during emergence challenge monitoring approaches. Substance-specific emergence characteristics may additionally limit the reliability of commonly used processed EEG indices during emergence. This study aims to analyze the dynamics of different EEG indices during anesthesia emergence that was maintained with different anesthetic regimens. We used the EEG of 45 patients under general anesthesia from the emergence period. Fifteen patients per group received sevoflurane, isoflurane (+ sufentanil) or propofol (+ remifentanil) anesthesia. One channel EEG and the bispectral index (BIS A-1000) were recorded during the study. We replayed the EEG back to the Conox, Entropy Module, and the BIS Vista to evaluate and compare the index behavior. The volatile anesthetics induced significantly higher EEG frequencies, causing higher indices (AUC > 0.7) over most parts of emergence compared to propofol. The median duration of "awake" indices (i.e., > 80) before the return of responsiveness (RoR) was significantly longer for the volatile anesthetics (p 0.6), with SE having the weakest correlation. For propofol, the correlation was lower (rs < 0.6). SE was significantly higher than BIS and, under propofol anesthesia, qCON. Systematic differences of EEG-based indices depend on the drugs and devices used. Thus, to avoid early awareness or anesthesia overdose using an EEG-based index during emergence, the anesthetic regimen, the monitor used, and the raw EEG trace should be considered for interpretation before making clinical decisions.

Abstract:Purpose: Processed electroencephalogram (EEG) is used peri-operatively for monitoring depth of anaesthesia. Because these utilise EEG data, attempts have been made to investigate their use in diagnosing and monitoring seizures. This is important as formal EEG monitoring can be hard to obtain in many critical care environments. We undertook a scoping review of the evidence for using processed EEG (pEEG) from depth of anaesthesia monitors for this indication. Methods: Medline, Psych INFO, and Embase were searched for peer-reviewed journals until 20 March 2021. Data and conclusions taken from the study of pEEG in both critical care and peri-operative settings have been included in a qualitative synthesis about the current evidence for the use of pEEG in the detection and monitoring of seizures. Results: Searches yielded 8 observational studies, 1 randomised trial and 15 case reports in which the use of pEEG in critical care and peri-operative medicine was described. Most concerned the Bispectral Index (BIS) device. The majority of observational studies reported the use of BIS for optimisation of burst suppression in patients with refractory status-epilepticus (RSE), or in the comparison of pEEG data with conventional EEG during epileptic activity. Multiple case reports describe the application of pEEG in the presence of disorders of consciousness as a tool for detection of non-convulsive status-epilepticus, finding variable trends in the pEEG output. Conclusions: Processed EEG may be helpful in monitoring pharmacologically induced burst suppression. Despite this, its use in the diagnosing or monitoring seizure activity is controversial and currently not evidenced, with numerous confounding variables that requires systematic assessment in future studies.

Abstract:Abstract Background: Post-operative cognitive dysfunction/decline is a well-known phenomenon and of crucial importance, especially in the elderly, emphasizing the significance of selecting the proper anesthetics. General anesthesia can be accomplished by inhalation-based (volatile) or total intravenous anesthesia (TIVA). While their effects on post-operative symptoms have been investigated, little is known about their influence on brain functionalities during the surgery itself. Objective: To assess differences in brain activity between volatile and TIVA anesthetics during surgery. Methods: Seventeen patients who were electively scheduled for laparoscopic cholecystectomy in Galilee Medical Center, Nahariya, gave written informed consent to participate in the study and were randomly divided to receive either volatile anesthesia ( n = 9), or TIVA ( n = 8). The level of anesthesia was kept equal in both groups. A single bipolar EEG electrode (Aurora by Neurosteer®) was placed on each participant’s forehead. The electrode presented real-time activity and collected participants’ data during the surgery. The dependent variables included frequency bands (delta, theta, alpha, and beta), and three features (VC9, ST4, and A0) previously extracted with the Aurora device and provided by Neurosteer®. Results: All surgeries were uneventful, and all patients showed bispectral index (BIS) scores less than 60. Feature activity under volatile anesthesia (in comparison to TIVA) was significantly lower for the delta, theta, and alpha frequency bands and for the three features (VC9, ST4, and A0). Further analysis showed that the largest difference between anesthesia types was for feature A0. Conclusions: Both EEG frequency bands and novel brain activity features provide evidence that volatile anesthesia further reduces components of brain activity in comparison to TIVA anesthesia. Specifically, A0, which previously showed a correlation with cognitive decline severity and cognitive load, exhibited the most prominent difference between anesthesia types. Together, this study suggests that measuring brain activity during anesthesia using sensitive features can reveal that different anesthesia types may affect brain activity differently, which could affect a patient’s recovery from anesthesia, and consequently reduce post-operative cognitive decline.

Abstract:Maser et al. (2009) identify several problems with the categorical DSM, and suggest that a shift to a mixed categorical-dimensional system is warranted. Maser et al. support their argument by citing evidence related to mood and anxiety disorders, among other conditions. In this commentary, I consider the applicability of several issues raised by Maser et al. to two disruptive behavior disorders in youth, oppositional defiant disorder (ODD) and conduct disorder (CD). The issues include paradigm shifts concerning (a) the diagnostic threshold, (b) symptoms, and (c) distress/psychosocial impairment. Within each topic, several developmental psychopathology principles that parallel and extend the Maser et al. issues are presented and described. This commentary also provides examples of dimensions that could be useful for conceptualizing ODD and CD within a mixed categorical-dimensional classification system.

Abstract:Background: Monitoring the electroencephalogram (EEG) during general anesthesia can help to safely navigate the patient through the procedure by avoiding too deep or light anesthetic levels. In daily clinical practice, the EEG is recorded from the forehead and available neuromonitoring systems translate the EEG information into an index inversely correlating with the anesthetic level. Electrode placement on the forehead can lead to an influence of electromyographic (EMG) activity on the recorded signal in patients without neuromuscular blockade (NMB). A separation of EEG and EMG in the clinical setting is difficult because both signals share an overlapping frequency range. Previous research showed that indices decreased when EMG was absent in awake volunteers with NMB. Here, we investigated to what extent the indices changed, when EEG recorded during surgery with NMB agents was superimposed with EMG. Methods: We recorded EMG from the flexor muscles of the forearm of 18 healthy volunteers with a CONOX monitor during different activity settings, that is, during contraction using a grip strengthener and during active diversion (relaxed arm). Both the forehead and forearm muscles are striated muscles. The recorded EMG was normalized by z -scoring and added to the EEG in different amplification steps. The EEG was recorded during anesthesia with NMB. We replayed these combined EEG and EMG signals to different neuromonitoring systems, that is, bispectral index (BIS), CONOX with qCON and qNOX, and entropy module with state entropy (SE) and response entropy (RE). We used the Friedman test and a Tukey-Kramer post hoc correction for statistical analysis. Results: The indices of all neuromonitoring systems significantly increased when the EEG was superimposed with the contraction EMG and with high EMG amplitudes, the monitors returned invalid values, representative of artifact contamination. When replaying the EEG being superimposed with "relaxed" EMG, the qCON and BIS showed significant increases, but not SE and RE. For SE and RE, we observed an increased number of invalid values. Conclusions: With our approach, we could show that EMG activity during contraction and resting state can influence the neuromonitoring systems. This knowledge may help to improve EEG-based patient monitoring in the future and help the anesthesiologist to use the neuromonitoring systems with more knowledge regarding their function.

Abstract:Abstract Electroencephalograms (EEGs) display a mixture of rhythmic and broadband fluctuations, the latter manifesting as an apparent 1/f spectral trend. While network oscillations are known to generate rhythmic EEG, the neural basis of broadband EEG remains unexplained. Here, we use biophysical modelling to show that aperiodic neural activity can generate detectable scalp potentials and shape broadband EEG features, but that these aperiodic signals do not significantly perturb brain rhythm quantification. Further model analysis demonstrated that rhythmic EEG signals are profoundly corrupted by shifts in synapse properties. To examine this scenario, we recorded EEGs of human subjects being administered propofol, a general anesthetic and GABA receptor agonist. Drug administration caused broadband EEG changes that quantitatively matched propofol’s known effects on GABA receptors. We used our model to correct for these confounding broadband changes, which revealed that delta power, uniquely, increased within seconds of individuals losing consciousness. Altogether, this work details how EEG signals are shaped by neurophysiological factors other than brain rhythms and elucidates how these signals can undermine traditional EEG interpretation.

Abstract:The growing use of neuromonitoring in general anesthesia provides detailed insights into the effects of anesthetics on the brain. Our study focuses on the processed EEG indices State Entropy (SE), Response Entropy (RE), and Burst Suppression Ratio (BSR) of the GE Entropy TM Module, which serve as surrogate measures for estimating the level of anesthesia. While retrospectively analyzing SE and RE index values from patient records, we encountered a technical anomaly with a conspicuous distribution of index values. In this single-center, retrospective study, we analyzed processed intraoperative electroencephalographic (EEG) data from 15,608 patients who underwent general anesthesia. We employed various data visualization techniques, including histograms and heat maps, and fitted custom non-Gaussian curves. Individual patients' anesthetic periods were evaluated in detail. To compare distributions, we utilized the Kolmogorov–Smirnov test and Kullback–Leibler divergence. The analysis also included the influence of the BSR on the distribution of SE and RE values. We identified distinct pillar indices for both SE and RE, i.e., index values with a higher probability of occurrence than others. These pillar index values were not age-dependent and followed a non-equidistant distribution pattern. This phenomenon occurs independently of the BSR distribution. SE and RE index values do not adhere to a continuous distribution, instead displaying prominent pillar indices with a consistent pattern of occurrence across all age groups. The specific features of the underlying algorithm responsible for this pattern remain elusive.

Abstract:Objective: Monitoring the depth of anesthesia (DoA) plays an important role for administering the drug injection during a surgery, i.e., preventing undesired awareness and inordinate anesthetic depth. Although the bispectral index (BIS) monitor is the golden standard system for the DoA monitoring, it is still not affordable for the developing countries. Alternatively, a low-cost electroencephalogram (EEG) headband can be used. The objective of this paper is to present a new algorithm for estimating the BIS values using a single frontal EEG channel. Method: In the first step, the EEG signal is filtered for the elimination of artifacts and is split into its sub-bands. In the second step, several linear and nonlinear features are extracted from each sub-band and fed to a random forest regression model in order to estimate the BIS. The performance of the proposed algorithm is assessed using EEG data recorded from twenty-four subjects during the general anesthesia and is validated in terms of correlation coefficient (CC) and absolute error (AE) between the reference and estimated BIS values. Results: The proposed algorithm achieved the mean CC of 0.83 and AE of 6.5 for intra subject variability and mean CC of 0.87 and AE of 5.5 for inter subject variability. Significance: Given the similar results for both intra and inter subject variability, the proposed algorithm has the potential to be used in the real-world scenario.

Abstract:Electroencephalogram (EEG) has been widely used to measure the effect of anesthetics on the central nervous system. However, EEG signals are very prone to artifacts. In this article, EEG variability (EEGV) is proposed, and nonlinear models are used to analyze EEGV for measuring the depth of anesthesia (DoA). First, the time intervals between successive local maxima of EEG are extracted. Then, resampling and differentiation are applied to reconstruct the EEGV time series. Sample entropy (SampEn), permutation entropy (PeEn), detrended fluctuation analysis (DFA), and Poincaré plots (PoPs) are used to analyze EEGV. The area under the curve (AUC) and the correlation analysis between proposed measures and conscious level indicated by the bispectral index and expert-labeled data are investigated. A long short-term memory network is further used to combine multiple features for predicting DoA. The results from 59 patients show that these four indices can differentiate awake and unconscious states and track changes in anesthesia states. SampEn, DFA, and PoP derived from EEGV achieve significantly higher AUCs and correlation coefficients than those derived from EEG. The fusion of EEG and EEGV can measure DoA more precisely. In summary, it is the first time that four nonlinear methods are used to analyze the reconstructed EEGV signals instead of traditionally used raw EEG, which provides a robust way to measure DoA.
