Abstract:Objective: Bipolar radiofrequency (bRF) ablation is gaining popularity as a treatment modality for unstable knee chondral lesions of the knee. Limited reports of osteonecrosis and chondrolysis have been published; however, there is little data examining the safety of this treatment in larger series. This study aims to evaluate the safety and efficacy of bRF in the treatment of chondral lesions encountered during knee arthroscopy. Design: A retrospective evaluation of adverse outcomes in patients that underwent treatment of chondral lesions using bRF was undertaken. Secondary outcome measures included change in patient reported outcome scores and its correlation to patient demographics and quality of chondral and meniscal lesions using Chondropenia Severity Score. Results: Only 2.2% and 2.7% of the patients had a postoperative complication or required a reoperation, respectively. None of the complications were directly related to the use of bRF. A statistically significant difference was observed when comparing pre- and postoperative scores in all normalized categories ( P < 0.0001). No statistically significant correlation was found between change in self-reported scores and Chondropenia Severity Score. Conclusion: Bipolar radiofrequency ablation is a safe modality in treatment of chondral lesions.

Arthroscopic knee chondroplasty using a bipolar radiofrequency-based device compared to mechanical shaver: results of a prospective, randomized, controlled study

Abstract:Both mechanical shavers and radiofrequency-based devices are used to treat symptomatic partial thickness chondral lesions. Controversy exists as to which mode of treatment provides better outcomes. The purpose of this study was to compare clinical results after bipolar radiofrequency-based chondroplasty (RFC) to mechanical shaver debridement (MSD). Patients (n = 60) included in the study presented with knee pain associated with a medial meniscus tear and idiopathic ICRS grade III defect of the medial femoral condyle. Patients who had osteoarthritis, grade II or higher cartilage defects of the tibial joint surface, the lateral compartment, or the femoro-patellar joint, or had previously undergone surgery on the affected knee were excluded. Patients underwent partial meniscectomy; during the procedure, they were randomly assigned to receive bipolar RFC (Paragon, ArthroCare Corporation, Austin, TX) or MSD (Full radius resector LR 4.85 x 12.5 cm), Arthrex, Naples, FL). Postoperatively, the same physiotherapist provided instructions for a daily 2-h home training program consisting of isometric, isotonic, and leg lifting exercises; patients were provided the option of using crutches. Clinical outcomes were assessed using the Tegner score, visual analogue scale (VAS) score, and Knee and Osteoarthritis Outcome Score (KOOS) assessment, which consists of five principal domains including pain, symptoms, function in daily living (ADL), and knee related quality of life (QOL), where a score of 0 indicates extreme symptoms and 100 represents no symptoms. Age and time from injury onset did not differ significantly between the RFC and MSD groups (43 +/- 10 vs. 44 +/- 9 years, P = 0.732; 8 +/- 3 vs. 7 +/- 4 months, P = 0.279). No complications or adverse events were observed. Preoperatively, mean (+/-SD) scores for all KOOS principal domains were <20 and did not differ significantly (P > 0.05) between treatment groups. Postoperatively, the RFC patients returned to activity significantly earlier than MSD patients (17 +/- 7 vs. 22 +/- 6 days, P = 0.002). VAS pain scores at 6 h, 24 h, 6 weeks, and 1 year were significantly (P < 0.001) lower for the RFC group than for the MSD group (4 +/- 2, 2 +/- 0.5, 2 +/- 1, 2 +/- 1 vs. 8 +/- 1, 4 +/- 1, 4 +/- 1, 3 +/- 1, respectively). At 1 year, RFC patients had significantly better Tegner score (4.1 +/- 0.8 vs. 2.8 +/- 0.6, P < 0.001) and KOOS domain scores for pain, symptoms, ADL, QOL, respectively (81.1 +/- 8, vs. 59.4 +/- 12.8; 80.7 +/- 7.5 vs. 59.6 +/- 7.5; 81.5 +/- 6.5 vs. 60.1 +/- 6.9; 80 +/- 10 vs. 61.3 +/- 12.5; P < 0.001) than MSD patients. Significantly fewer RFC patients (2% vs 23%, p = 0.026) reported using NSAIDS for knee pain at 1 year. Patients undergoing radiofrequency-based chondroplasty for ICRS grade III medial femoral condyle chondral lesions in conjunction with partial meniscectomy had significantly better clinical outcomes through 1 year than patients with similar pathology receiving chondral debridement using the mechanical shaver.
Working Conditions of Bipolar Radiofrequency on Human Articular Cartilage Repair Following Thermal Injury During Arthroscopy

Abstract:BACKGROUND:The thermal injury during bipolar radiofrequercy results in chondrocyte death that limits cartilage repair. The purpose was to determine the effects of various factors of bipolar radiofrequency on human articular cartilage after thermal injury, offering suitable working conditions for bipolar radiofrequency during arthroscopy.METHODS:Osteochondral explants from 28 patients undergoing total knee arthroplasty (TKA) in Department of Orthopaedic, Peking University Reople's Hospital from October 2013 to May 2014, were harvested and treated using bipolar radiofrequency in a light contact mode under the following conditions: various power setting of levels 2, 4 and 6; different durations of 2 seconds, 5 seconds and 10 seconds; irrigation with fluids of different temperatures of 4°C, 22°C, and 37°C; two different bipolar radiofrequency probes ArthroCare TriStar 50 and Paragon T2. The percentage of cell death and depth of cell death were quantified with laser confocal microscopy. The content of proteoglycan elution at different temperatures was determined by spectrophotometer at 530 nm.RESULTS:Chondrocyte mortality during the treatment time of 2 seconds and power setting of level 2 was significantly lower than that with long duration or in higher level groups (time: P = 0.001; power: P = 0.001). The percentage of cell death after thermal injury was gradually reduced by increasing the temperature of the irrigation solutions (P = 0.003), the depth of dead chondrocytes in the 37°C solution group was significantly less than those in the 4°C and 22°C groups (P = 0.001). The proteoglycan elution was also gradually reduced by increasing the temperature (P = 0.004). Compared with the ArthroCare TriStar 50 group, the percentage of cell death in the Paragon T2 group was significantly decreased (P = 0.046).CONCLUSIONS:Thermal chondroplasty with bipolar radiofrequency resulted in defined margins of chondrocyte death under controlled conditions. The least cartilage damage during thermal chondroplasty could be achieved with lower power, shorter duration, suitable temperature of irrigation solutions and chondroprotective probes. The recommendations for the use of bipolar radiofrequency to minimize cartilage damage could be achieved with a power setting of level 2, treatment duration of 2 seconds, suitable fluid temperature (closer to body temperature of 37°C) and chondroprotective Paragon T2 probes.

Abstract:Degenerative knee osteoarthritis is a progressive debilitating condition that affects millions of people worldwide. Most patients experience limited range of motion, inflammation, swelling, and pain. Management options can lead to short- and long-term pain relief. Short-term pain relief usually involves conservative medical treatments such as NSAIDs, weight loss, physical therapy, and corticosteroid injections. The ultimate long-term pain relief treatment method involves total knee arthroplasty. The treatment algorithm for knee OA also includes managing pain until a patient is eligible for arthroplasty. Furthermore, about 20% of patients experience chronic pain after TKA without complications such as hardware loosening and infection with limited treatment options. The pathophysiology of this is unknown. Cooled radiofrequency ablation has been demonstrated to be clinically effective by disrupting the integrity of deep sensory nerves and hence interfering with transmission of pain signal. The analgesic effect after c-RFA has been reported up to 24 months. Here, we present an overview of the clinical application of cooled radiofrequency ablation and a summary of its effectiveness in the treatment of pain in the setting of advanced OA and symptomatic TKA.

Catheter Radiofrequency Ablation of Recurrent Para-Hisian Accessary Pathways with Near Zero X-ray Exposure

Abstract:Introduction Radiofrequency (RF) ablation of parahisian accessory pathways (PHAPs) is a serious challenge, which is associated with high recurrence and fluoroscopy exposure. According to previous studies and case reports, catheter ablation of PHAPs is associated with a high post-ablation recurrence. The aim of this study is to assess the efficacy, safety and long-term outcome of radiofrequency ablation in patients with recurrent PHAPs after prior ablations with zero or extremely low fluoroscopy. Methods and Results A total of 58 patients (43 male, 75.4%) with PHAPs that had one or more previously unsuccessful RF ablations were enrolled. RF ablation procedures were performed under a three-dimensional guiding system (EnSite NavX, St. Jude Medical, MN) with extremely low exposure of fluoroscopy to femoral venous or arterial routes. The acute success rate was 96.6% (56/58). Two procedures were terminated during ablation because of high risk of complete AVB. In addition to a case of transient complete AVB during RF energy delivery, no minor or major complications were recorded during the procedure or long-term follow up. The mean fluoroscopy time was 98 seconds (0-129), and the mean radiation dose was 1.98 mGy (0-3.38) in all 58 cases. Fourteen patients underwent the EPS and RF ablation with zero X-ray exposure. The long-term success rate was 98.2% with a median follow-up period of 30 (12-45) months. Conclusions With a near zero X-ray exposure, RF ablation of recurrent PHAPs can be performed with a high acute success rate and convincing safety during long-term follow-up.

Abstract:Radiofrequency (RF) tumor ablation has been shown to be a useful method for creating thermally induced coagulation necrosis, using either a percutaneous approach with image-guidance, laparoscopic guidance, or direct surgical placement of thin electrodes into tissues to be treated. RF has gained widespread clinical use for the treatment of hepatic, renal, and bony malignancies. This article discusses the principles and technical considerations of RF ablation, with the goal of defining optimal parameters for the therapy of focal tumors. These parameters include innovations that permit large volume tumor ablation (i.e., multitined and internally cooled electrodes), as well as adjuvant therapies that can modulate tumor physiology to allow improved tumor destruction (i.e., altered tissue conductivity and blood flow). Recent adjuvant chemotherapy also will be reviewed. Finally, the principles governing the safe use of the system, including proper grounding pad placement, will be addressed.

Abstract:Radiofrequency ablation (RFA) is a minimally invasive clinical treatment that uses radiofrequency energy to generate heat, resulting in the thermal necrosis of targeted tissues. To enhance the therapeutic benefits of traditional RFA, cooled RFA (CRFA) technology has been developed. CRFA incorporates cooling technology to prevent thermal damage and rapid impedance changes caused by tissue overheating. This review article provides a comprehensive overview of various types of cooling electrode needles used in CRFA, as well as an evaluation of their efficacy and clinical applications. We discuss the advantages of CRFA, including its minimally invasive nature, improved safety profile, and highly effective treatment outcomes. Nevertheless, certain problems and limitations are also addressed to optimize the potential of CRFA as a clinical treatment option. Overall, CRFA has promising prospects. With continued advancements in technology and further research, this innovative treatment modality is expected to significantly impact the treatment of a wide range of medical conditions.

Abstract:BackgroundThe application of radiofrequency energy (RFE) has become widespread for surgical performed chondroplasty especially due to the anticipated sealing effect, however the safety of this procedure in the wrist remains unclear. The purpose of this study was to investigate the subchondral temperature during radiofrequency energy (RFE) application simulating chondroplasty in an arthroscopic setting of the wrist.MethodsA chondroplasty of the lunate fossa was performed during an arthroscopy setting on 14 cadaver arms using monopolar or biopolar RFE. The temperature was recorded simultaneously from 7 predefined anatomical landmarks.ResultsThe mean temperature for both application modes did not exceed more than 30°C at all measured points, except for the lunate fossa. The highest subchondral measured peak temperature was 49.35°C (monopolar) and 69.21°C (bipolar) in the lunate fossa. In addition, the temperature decreased for both radiofrequency (RF) devices depending on the distance of the sensors to the RF-probe.ConclusionIt remains to be questionable how safe RFE can be used for chondroplasty in wrist arthroscopy under continuous irrigation and constant movement to obtain the desired sealing effect. However, the bipolar device should be applied with more caution since peak temperature in the lunate fossa almost reached 70°C even under continuous irrigation.

Abstract:Minimally invasive interventional radiology procedures play an adjunctive role in treating the symptoms of osteoarthritis (OA) with the hopes of delaying total knee arthroplasty (TKA). However, currently available intra-articular injections offer only short-term benefits. This has led to evolution of new techniques such as genicular artery embolization and genicular nerve ablation, which show benefit in pain control and quality of life, especially for mild-to-moderate OA, positioning these techniques as potential alternatives to intra-articular injections to help delay TKA.

Abstract:Acoustic emission technology has been developed and extensively used as a non-destructive method of testing within engineering. In recent years, acoustic emission has gained popularity within the field of Orthopaedic research in a variety of situations. It is an attractive method in the detection of flaws within structures due its high sensitivity and non-destructive nature. The aim of this article is firstly to critically review the research conducted using acoustic emission testing in a variety of Orthopaedic-related situations and to present the technique to the wider Orthopaedic community. A summary of the principles and practical aspects of using acoustic emission testing are outlined. Acoustic emission has been validated as a method of early detection of aseptic loosening in femoral components in total hip arthroplasty in several well-conducted in vitro studies [1-3]. Other studies have used acoustic emission to detect microdamage in bone and to assess the biomechanical properties of bone and allografts [9]. Researchers have also validated the use of acoustic emission to detect and monitor fracture healing [4]. Several studies have applied acoustic emission to spinal surgery and specifically to assess the biomechanical environment in titanium mesh cages used in spinal surgery [10, 11]. Despite its growing popularity within Orthopaedic research, acoustic emission remains are relatively unfamiliar technique to the majority of Orthopaedic surgeons.

Abstract:Abstract Purpose of Review Augmented reality (AR) is becoming increasingly popular in modern-day medicine. Computer-driven tools are progressively integrated into clinical and surgical procedures. The purpose of this review was to provide a comprehensive overview of the current technology and its challenges based on recent literature mainly focusing on clinical, cadaver, and innovative sawbone studies in the field of orthopedic surgery. The most relevant literature was selected according to clinical and innovational relevance and is summarized. Recent Findings Augmented reality applications in orthopedic surgery are increasingly reported. In this review, we summarize basic principles of AR including data preparation, visualization, and registration/tracking and present recently published clinical applications in the area of spine, osteotomies, arthroplasty, trauma, and orthopedic oncology. Higher accuracy in surgical execution, reduction of radiation exposure, and decreased surgery time are major findings presented in the literature. Summary In light of the tremendous progress of technological developments in modern-day medicine and emerging numbers of research groups working on the implementation of AR in routine clinical procedures, we expect the AR technology soon to be implemented as standard devices in orthopedic surgery.

Abstract:[Objective] To evaluate the clinical value of Radiofrequency coblation for laparoscopic surgery. Thirty eight patients were operated laparoscopically by Radiofrequency coblation, Including cholecystectomy(21), radical resection for early gallbladder cancer(1), partial thyroidectomy(4), fenestration of liver cyst(3), Nissen fundoplication(1), excision of duodenal adenoma(1), appendectomy(1), adhesiolysis(1), laparoscopic exploration and biopsy(2), ovariocystectomy(1), proctectomy(2). The operations were all successfully. accomplished. Gastric tube were taken out 6~12 hours after operation. The patients could leave bed and take some fluid 24 hours later. The abdominal drainage tube was pulled out 1~3 days postoperation. The patients were discharged 2~5 days after the operation. No operative complications were occured. [Conclusions] Radiofrequency coblation can precisely melt, cut and coagulate to the tissue in lower temperature and is a minimally invasive approach. So it has distinctive and practive value in the clinic.

Abstract:Abstract Radiofrequency ablation is an interventional technique that in recent years has come to be employed in very different medical fields, such as the elimination of cardiac arrhythmias or the destruction of tumors in different locations. In order to investigate and develop new techniques, and also to improve those currently employed, theoretical models and computer simulations are a powerful tool since they provide vital information on the electrical and thermal behavior of ablation rapidly and at low cost. In the future they could even help to plan individual treatment for each patient. This review analyzes the state-of-the-art in theoretical modeling as applied to the study of radiofrequency ablation techniques. Firstly, it describes the most important issues involved in this methodology, including the experimental validation. Secondly, it points out the present limitations, especially those related to the lack of an accurate characterization of the biological tissues. After analyzing the current and future benefits of this technique it finally suggests future lines and trends in the research of this area.

An Audit of Treatment of Genital Warts: Opening the Feedback Loop

Abstract:An audit of the treatment of patients (100 men and 90 women) presenting with a first episode of anogenital warts to the Genitourinary Medicine Department at Leeds General Infirmary was performed. Treatment of patients was monitored for a period of 6 months from the time of presentation. The management of patients with genital warts lacked a clearly defined strategy and treatment was unselective and poorly monitored. Excluding patients who defaulted, at follow-up 44 (44%) men and 36 (38%) women still had genital warts at 3 months. Of those patients clear of warts at 3 months, the mean time to remission for men and women was 7.1 and 8.3 weeks respectively. Podophyllin 25% in tincture of benzoin was by far the predominant therapeutic modality used. A total of 96 (96%) men and 76 (84%) women received treatment with podophyllin. Both male and female patients had a mean of 5 treatments with podophyllin 25% (range 1–19 and 1–12 respectively). Physical methods of treatment i.e. cryotherapy and electrocautery, were underutilized, both as primary therapies and when topical agents had failed. Patients saw an average of 3 (range 1–7) doctors over the course of their treatment. Patients with warts affecting 2 or more sites, male patients with anal/perianal warts, and female patients with cervical and vaginal warts had higher failure rates from treatment at 3 months. On the basis of these findings, specific treatment protocols for the management of anogenital warts have been devised.

Abstract:For patients with knee osteoarthritis (OA), radiofrequency ablation may reduce pain and improve function compared to other nonsurgical interventions without increasing adverse events. There is insufficient evidence to suggest that radiofrequency ablation reduces pain or improves function among patients with chronic hip pain. We did not find any studies or guidelines on the clinical effectiveness of radiofrequency ablation for treating chronic shoulder pain that met the inclusion criteria for this review. Three guidelines conditionally recommend the use of radiofrequency ablation for patients with knee OA, and 1 guideline conditionally recommends the use of radiofrequency ablation for hip joint pain following diagnostic blocks.

Abstract:Over the past 2 decades there has been significant progress in the treatment of articular cartilage pathology with the introduction of cell biology, tissue engineering, and biomaterials to address long standing challenges in this debilitating condition. However, cell therapies and products combining cells, other biologics, devices, and/or drugs do not easily fit within traditional regulatory frameworks of the US Food and Drug Administration (FDA) or agencies outside the United States. Regulations have evolved to specifically address these types of products in terms of their unique scientific and clinical issues. This paper reviews the characteristics of cell and combination products that distinguish them from more traditional medical devices and drugs, FDA regulatory pathways for marketing approval of these new types of products, and the implications of these regulations for orthopedic product development and practice.

Abstract:Acute stimulation of cardiac beta1-adrenergic receptors (beta1ARs) by norepinephrine represents the strongest endogenous mechanism for increasing cardiac function, but long-term stimulation induces cardiomyocyte apoptosis and contributes to cardiac disease. These effects have been attributed to coupling of the beta1AR to the stimulatory G protein (Gs) and classical cAMP-mediated signaling. In this issue of the JCI, Noma and colleagues report that cardiomyocyte beta1ARs may in addition deliver an antiapoptotic signal through transactivation of EGFRs (see the related article beginning on page 2445). Their findings provide a perspective for a novel class of receptor ligands that may direct beta1AR signaling toward alternative signaling pathways.

Innovations: Technology and Techniques in Cardiothoracic and Vascular Surgery

Abstract:The surgical treatment of atrial fibrillation has evolved over the past 2 decades due to the advent of ablation technology, and the introduction of less invasive surgical approaches. Current devices produce ablation lines that aim to replace the incisions of traditional surgical ablation strategies, such as the Cox-Maze procedure. This has helped to simplify and shorten surgical ablation procedures and has allowed for the development of minimally invasive surgical techniques. This review discusses surgical ablation energy sources and devices, providing background on device characteristics, mechanism of tissue injury, and success in creating transmural lesions.
