Abstract:Since the 1970s, the epidemic of hepatocellular carcinoma (HCC) has spread beyond the Eastern Asian predominance and has been increasing in Northern hemisphere, especially in the United States (US) and Western Europe. It occurs more commonly in males in the fourth and fifth decades of life. Among all cancers, HCC is one of the fastest growing causes of death in the US and poses a significant economic burden on healthcare. Chronic liver disease due to hepatitis B virus or hepatitis C virus and alcohol accounts for the majority of HCC cases. Incidence of nonalcoholic fatty liver disease has been on the risem and it has also been associated with the development of HCC. Its pathogenesis varies based on the underlying etiological factor although majority of cases develop in the setting of background cirrhosis. Carcinogenesis of HCC includes angiogenesis, chronic inflammation, and tumor macroenvironment and microenvironment. There is a significant role of both intrinsic genetic risk factors and extrinsic influences such as alcohol or viral infections that lead to the development of HCC. Understanding its etiopathogenesis helps select appropriate diagnostic tests and treatments.

Abstract:The virus-resistant papaya (Carica papaya L.), Huanong no. 1, was the genetically modified (GM) fruit approved for growing in China in 2006. To implement the labeling regulation of GM papaya and its derivates, the development of papaya endogenous reference gene is very necessary for GM papaya detection. Herein, we reported one papaya specific gene, Chymopapain (CHY), as one suitable endogenous reference gene, used for GM papaya identification. Thereafter, we established the conventional and real-time quantitative PCR assays of the CHY gene. In the CHY conventional PCR assay, the limit of detection (LOD) was 25 copies of haploid papaya genome. In the CHY real-time quantitative PCR assay, both the LOD and the limit of quantification (LOQ) were as low as 12.5 copies of haploid papaya genome. Furthermore, we revealed the construct-specific sequence of Chinese GM papaya Huanong no. 1 and developed its conventional and quantitative PCR systems employing the CHY gene as endogenous reference gene. This work is useful for papaya specific identification and GM papaya detection.
Effect of home-based pulmonary rehabilitation on health-related quality of life, lung function, exercise tolerance, and dyspnea in chronic obstructive pulmonary disorder patients in a tertiary care center in South India

Abstract:Context: Home-based pulmonary rehabilitation (PR) can increase compliance in chronic obstructive pulmonary disorder (COPD). There is paucity of data on this in India. Aims: To study the effectiveness of home-based PR on lung function, health-related quality of life, exercise tolerance and dyspnea among patients with COPD. Setting: The Department of Pulmonary Medicine, Christian Medical College, Vellore. Subjects and Methods: This was a quasi-experimental study which included COPD patients who found hospital-based PR impractical. Those willing for home-based PR constituted the intervention group and those unwilling, constituted the control group. Both groups received standard of care otherwise. At 6 weeks, forced expiratory volume in 1 sec (FEV1), St. George Respiratory Questionnaire (SGRQ), six-minute walk test distance (6MWD), COPD assessment test (CAT) score and modified Borg dyspnea scale were compared with their baseline values. Statistical Analysis: Student paired t-test. Results: A total of 42 patients completed the study, 21 in each arm. The baseline characteristics were comparable. The improvement in FEV1 was 90 ml in the intervention group and 4 ml in the control group (P = 0.01). There was no significant difference in 6MWD change. SGRQ score, Borg scale and CAT score improved significantly by 10.4, 2.4, and 3.4, respectively, in the intervention group when compared to the control group (P = 0.01). Conclusions: Home-based PR effectively provides tangible benefits in FEV1, QOL, exercise tolerance and dyspnea. It should be offered to those who are unable to avail institution-based PR, especially in this era of COVID pandemic.

Abstract:ABSTRACT Turing devised an electromechanical device, called a Bombe, to decipher German Enigma messages. His original design was considerably improved by the addition of Welchman’s diagonal board, allowing the use of much weaker menus and even menus without closures. Turing then made some extensive calculations to predict the performance of the improved Bombe. He provided a table, called the H-M factor, which allowed the expected number of stops to be predicted for menus containing a given number of letters and closures, but he gave no indication of how he constructed the table. This article is of technical interest and sheds some light on the H-M factor, including its variability with the tree-arrangement of the menu. It derives the H-M factor using a recursive technique for both the standard Bombe and the Jumbo Bombe, a later version incorporating the machine gun mechanism. These results may be of interest to those that code or run Bombe emulators.
Effects of Pulmonary Rehabilitation Program on Patients With Chronic Obstructive Pulmonary Disease

Abstract:Introduction: Chronic Obstructive Pulmonary Disease (COPD) impairs patients` quality of life and clinical outcomes. Pulmonary rehabilitation (PR) program can improve the functional capacity in patients with chronic lung disease. Thus, the study aimed to evaluate the effect of the PR program on the quality of life, anxiety, depression, and pulmonary function of patients with COPD. Materials and Methods: In this single-group before-and-after clinical trial, adult patients with COPD and recent history of exacerbation were recruited. The intervention was a PR program, including training of breathing exercises at home and aerobic exercise program, twice a week about 30 to 60 min for 8 weeks. The program was prepared according to the patient’s tolerance by a sports medicine specialist in a pulmonary rehabilitation clinic. The primary outcome was quality of life measured by the St. George’s Respiratory Questionnaire (SGRQ). Secondary outcomes were assessing anxiety, depression, pulmonary function, COPD status, the ability to walk, and shortness of breathing. All outcomes were measured before and one week after the program. Results: Twenty-two eligible patients of both genders (68% male and 32% female) with a Mean±SD age of 65.09±9.72 years finished the program. Quality of life was improved significantly following the intervention (51.49 [16.68] vs 4275 [15.63]; P<0.001]. Anxiety and depression (P<0.001), pulmonary function parameters, such as forced expiratory volume in 1 second (FEV1) (P<0.001) and FEV1/ forced vital capacity (FVC) ratio (P=0.015), COPD status (P=0.001), the ability of walk1ing (P<0.05), and shortness of breath (P=0.001) were improved significantly after the intervention. Conclusion: The PR program resulted in clinical improvement in patients with COPD. Thus, we recommend that it be used besides medical management.
Effects of comprehensive and intensive pulmonary rehabilitation and nutritional support on quality of life and functional status in patients with chronic obstructive pulmonary disease

Abstract:Objective To investigate the effects of pulmonary rehabilitation (PR) and combined nutritional support therapy on quality of life (QoL) and functional status in patients with chronic obstructive pulmonary disease (COPD). Methods This pre-and post-intervention prospective exploratory study involved 64 patients with stable stage three to four COPD. Oral nutritional support and personalized diet were combined with an intense and comprehensive PR program. Baseline and 8-week follow-up scores were compared for the 6-minute walk test (6MWT), incremental shuttle walking test (ISWT), St. George’s Respiratory Questionnaire (SGRQ), pulmonary function tests (PFT), PImax-PEmax, arterial blood gas (ABG), respiratory rate (RR), handgrip strength, Borg and modified Medical Research Council dyspnoea scale scores and fat-free mass index. Results Significant improvements were found in functional status (6MWT: 86.72 m, ISWT: 76.24 m), QoL (SGRQ total: 13.86), PFT, ABG, RR, dyspnoea, upper extremity muscle strength and hand-body composition. Conclusion Nutritional support with comprehensive and intensive PR can significantly improve physical performance, QoL, dyspnoea and body composition in COPD. The improvement in QoL was greater than that reported in previous studies. Because two modalities were combined in this study, future randomized controlled studies are needed to confirm the extent and contribution of these modalities to the outcomes.

Abstract:Introduction: After completing Pulmonary Rehabilitation (PR), there is a new challenge that involves maintaining the benefits and activity levels achieved during the program. The GOLD guidelines state that PR's aim is to translate physical conditions into an increase in levels of physical activity (PA). Objectives: The aim of this study was to analyse the impact of an outpatient PR on the PA and functional capacity of individuals with COPD. Materials and Methods: A quasi-experimental study was implemented using a convenience sample of individuals with COPD who attended a 12-week PR program (20 sessions). A component that supported educational and behavioural change was included in the PR, which included endurance and muscle strengthening sessions. A pedometer (Yamax EX 510) was used to assess PA by monitoring the number of steps. The 1-minute Sit-to-Stand Test (1MSTS) was used to assess functional capacity. Both measures were evaluated at the beginning and end of the PR. Results: The number of repetitions in the 1MSTS revealed statistically significant improvements in functional capacity (p-value 0.03). Despite not being statistically significant (pre-RR – 3690, and post-RR – 3079, p-value 0.06), there was a decrease in steps number for PA levels, as shown in the data. Conclusions: This study showed that the PR did not result in gains in PA levels despite improving functional capacity (assessed by the 1MSTS). In order to increase PA levels, it seems necessary for the PR program to be accompanied by follow-up strategies and support for an extended period.

Abstract:Background: Pulmonary rehabilitation (PR) is a widely recognized nonpharmacologic therapy for chronic obstructive pulmonary disease (COPD), but most of the current studies on whether PR can benefit COPD patients are based on the evaluation of symptoms and pulmonary function, which is limited to a certain extent. Because COPD is characterized by potential regional lung changes in morphology and pathophysiology, this study was designed to evaluate the effects of individualized PR on regional lung function in patients with stable COPD.Methods: In this study, patients with stable COPD who met the criteria were included, and they were treated with PR for 2 weeks using the respiratory rehabilitation training instrument. The symptoms, and global and regional lung function before and after 2 weeks of PR treatment were evaluated using surveys, spirometry, and electrical impedance tomography (EIT), respectively. The spatial coefficient of variation (CV) of regional spirometry parameters were calculated to quantify spatial heterogeneity of lung function. Temporal inhomogeneity was determined by the regional expiration time.Results: A total of 34 participants were recruited in this study, of whom 24 completed the PR. After 2 weeks of intervention, the modified Medical Research Council (mMRC) dyspnea scale and the COPD assessment test (CAT) score was significantly lower compared to those measured before the treatment (2.3±1.17 vs. 2.1±0.93, P=0.034; and 15.0±7.18 vs. 10.9±6.06, P<0.001, respectively). Global spirometry forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1 predicted percentage (%pred), and peak expiratory flow (PEF) were significantly better than they were pre-rehabilitation (2.1±0.86 vs. 2.3±0.90 L, P=0.018; 1.2±0.65 vs. 1.4±0.66 L, P=0.001; 46.8%±23.16% vs. 51.4%±24.41%, P<0.001; and 3.1±1.80 vs. 3.8±2.23 L/s, P=0.005, respectively). In addition, the CV for regional FEV1/FVC was significantly decreased after the PR treatment (0.26±0.161 vs. 0.17±0.077, P=0.002). Regional lung ventilation was more homogeneous and regional expiration time was shorter after 2 weeks of the PR treatment.Conclusions: Two weeks of PR treatment can improve both spatial and temporal regional ventilation in COPD. In addition, EIT may be useful in developing individualized PR treatment program to improve regional lung function in COPD.

Abstract:Objective: Pulmonary rehabilitation (PR) improves function, reduces symptoms and decreases healthcare usage in people with chronic obstructive pulmonary disease (COPD) following an acute exacerbation (AECOPD). However, rehabilitation uptake rates are low. This study aimed to address barriers to uptake and completion of PR following AECOPD. Methods: An action research approach was used to reflect on study feasibility, and to plan and implement an improved protocol. Phase I tested the feasibility of home-based PR started early after AECOPD. Phase II used qualitative interviews to identified potential barriers to program uptake. Phase III re-tested the program with changes to recruitment and assessment strategies. Results: Phase I: From 97 screened patients, 26 were eligible and 10 (38%) started home-based PR. Eight participants undertook ≥70% of PR sessions, achieving clinically meaningful improvement in 6-minute walk distance (mean (SD) change 76 (60) m) and chronic respiratory disease questionnaire total score (15 (21) units). Phase II: Potential barriers to uptake of home-based PR included access issues, confidence to exercise, and lack of information about PR benefits. Phase III: From 77 screened patients, 23 were eligible and 5 (22%) started the program. Discussion: Home-based PR improved clinical outcomes, but program eligibility and uptake remain challenging. Efforts should be made to ensure PR program eligibility criteria are broad enough to accommodate patient needs, and new ways of engaging patients are needed to improve PR uptake after AECOPD.

Changes in COPD-related anxiety symptoms during pulmonary rehabilitation: a prospective quantitative and qualitative study

Abstract:Background: Fear-avoidance in COPD can have detrimental effects on pulmonary rehabilitation (PR) outcomes and is therefore important to address. This prospective study examined changes in and management of COPD-related anxiety symptoms over the course of a PR program. Methods: Patients with COPD referred to 9-weeks of PR in the municipality of Vejle, Denmark from January to December 2022 completed a six-minute walk test (6MWT) and the following questionnaires, both before and after PR: COPD Anxiety Questionnaire 20-item version (CAF-R), measuring COPD-related anxiety; COPD Assessment Test (CAT), measuring COPD-related disability; 12-Item Short-Form Health Survey (SF-12), measuring health-related quality of life (HR-QoL); sociodemographic and disease-related information. After PR, a subsample of the patients took part in semi-structured interviews exploring their understanding of how they managed COPD-related anxiety during PR. Pre- and post-assessment of COPD-related anxiety and other PR outcomes were analysed with t-tests and correlation analyses. Qualitative interviews were analysed using a thematic analysis approach. Results: A total of 72 patients with COPD (mean ± SD age 71 ± 8, 53% female) were included in the study, and 13 took part in qualitative interviews. A significant decrease in COPD-related anxiety was observed from before to after PR, corresponding to a small effect size (Cohen's d = 0.32; p = 0.018). Reductions in COPD-related anxiety were not associated with improvements in COPD-related disability, HR-QOL, or functional exercise capacity. The qualitative findings identified four anxiety management strategies, i.e., "planning", "problem-solving", "accepting", and "confronting", which were influenced by interactions with healthcare professionals and co-patients as well as patients' own perception. Conclusions: COPD-related anxiety symptoms was reduced after PR, potentially through the use of various management strategies. The strategies appeared to be influenced by interactional factors during the PR program.

Abstract:Introduction: Home-based pulmonary rehabilitation (PR) offers potential benefits for COPD patients, but its effectiveness across severity levels remains unclear. PR components are known. Results may be improved if the patient is more engaged with the intervention and receptive to change. This study aims to analyse which patients most benefit from home-based PR programmes regarding COPD severity. Methods: We conducted a quasi-experimental study where an 8-week PR programme was delivered. Regarding functional capacity accessed by the 6mWT, the 1-minute Sit-to-Stand Test, and the physical activity level, we divided the groups according to the cut-offs in the literature for the COPD population indicating poor prognosis. We categorised the patients into severe and non-severe patients. The primary outcome was functional capacity. Secondary outcomes included the impact of COPD and HRQoL. Results: Both groups showed relevant improvements in 6MWD at eight weeks. However, the severe group had a more significant improvement compared to the non-severe. Similar trends were observed for secondary outcomes, with greater improvements in the severe group regarding the impact of COPD and HRQoL. Conclusions: This study suggests that home-based PR can improve outcomes for both severe and non-severe COPD patients, with potentially greater benefits for severe patients. Further research is warranted to confirm these findings and optimise PR programs for different severity levels.

Abstract:Abstract Background Although pulmonary rehabilitation (PR) is considered a key component in managing chronic obstructive pulmonary disease (COPD) patients, uptake remains suboptimal. This systematic review aimed to determine the effectiveness of home-based PR (HBPR) programs for COPD patients. Methods A systematic review of scholarly literature published within the last 10 years from the conception of this project was conducted using internationally recognized guidelines. Search strategies were applied to electronic databases and clinical trial registries through March 2020 and updated in November 2021 to identify studies comparing HBPR with ‘usual care’ or outpatient pulmonary rehabilitation (OPR). To critically appraise randomized studies, the Cochrane Collaboration risk of bias tool (ROB) was used. The quality of non-randomized studies was evaluated using the ACROBAT-NRSI tool. The quality of evidence relating to key outcomes was assessed using Grading of Recommendations, Assessment, Development and Evaluations (GRADE) on health-related quality of life (HRQoL), exacerbation frequencies, COPD-related hospital admissions, and program adherence. Three independent reviewers assessed methodologic quality and reviewed the studies. Results Twelve randomized controlled trials (RCTs) and 2 comparative observational studies were included. While considerable evidence relating to the effectiveness of HBPR programs for COPD patients exist, overall quality is low. There were no differences between HBPR and OPR in terms of safety, HRQoL, functional exercise capacity and health care resource utilization. Compared to usual care, functional exercise capacity seemed to significantly improve after HBPR. While patient compliance with HBPR is good, two factors appeared to increase the ‘risk’ of non-compliance: expectations of patients to 1) complete daily diaries/activity logs and 2) engage in solely unsupervised exercise sessions. Conclusion The overall quality for most outcomes was low to very low; however, HBPR seems to offer comparable short-term benefits to OPR.

Abstract:Background Pulmonary (PR) and cardiac rehabilitation (CR) are recommended in the management of chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF); the impact of coexisting COPD and CHF on completion and outcomes of rehabilitation programmes is unknown. We examined enrolment, completion and clinical outcomes of CR and PR in adults with COPD, CHF and coexisting COPD and CHF. Methods The National Audit of CR and National COPD Audit Programme: clinical audits of PR were analysed (211 PR and 237 CR programmes); adults with a diagnosis of CHF, COPD or coexisting COPD and CHF were identified (COPD+CHF or CHF+COPD according to database). Propensity matching was conducted (age, sex, body mass index and functional status) between COPD+CHF and COPD, and CHF+COPD and CHF. Group by time interaction was examined using mixed 2x2 analysis of variance. Results Those with CHF+COPD had lower enrolment and completion of CR compared to those with CHF; there were no differences in PR enrolment or completion between the two groups. Adults with COPD made a significantly larger gain in the incremental shuttle walk test compared to adults with COPD+CHF following PR (59.3 m versus 37.4 m); the improvements following CR were similar (CHF 77.3 m versus CHF+COPD 58.3 m). Similar improvements were made in the 6-min walk test following CR (CHF 45.1 m versus CHF+COPD 38.8 m) and PR (COPD 48.2 m versus COPD+CHF 44.0 m). Comparable improvements in quality of life and mood state were made following CR and PR, regardless of diagnosis. Conclusion We have demonstrated that multi-morbid adults benefit from exercise-based rehabilitation, yet efforts are needed to promote completion. These findings support group-based, tailored, multi-morbid exercise rehabilitation.
The Effects of Digital Health Interventions for Pulmonary Rehabilitation in People with COPD: A Systematic Review of Randomized Controlled Trials

Abstract:Background and Objectives: Chronic Obstructive Pulmonary Disease (COPD) is the third most common cause of death globally. Pulmonary rehabilitation (PR) programmes are important to reduce COPD symptoms and improve the quality of life of people with COPD. Digital health interventions have recently been adopted in PR programmes, which allow people with COPD to participate in such programmes with low barriers. The aim of this study is to review and discuss the reported effects of digital health interventions on PR outcomes in people with COPD. Materials and Methods: To achieve the study goals, a systematic literature search was conducted using PubMed (MEDLINE), CINAHL, AMED, SPORTDiscus and the Physiotherapy Evidence Database. Randomised clinical trials (RCTs) were included if they met specified criteria. Two reviewers independently checked titles, abstracts, and performed full-text screening and data extraction. The quality assessment and risk of bias were performed in accordance with the PEDRO scale and Cochrane Risk of Bias tool 2, respectively. Results: Thirteen RCTs were included in this systematic review with 1525 participants with COPD. This systematic review showed the potential positive effect of digital health PR on the exercise capacity-measured by 6- and 12-min walking tests, pulmonary function, dyspnoea and health-related quality of life. There was no evidence for advantages of digital health PR in the improvement of anxiety, depression, and self-efficacy. Conclusions: Digital health PR is more effective than traditional PR in improving the pulmonary and physical outcomes for people with COPD, but there was no difference between the two PR programmes in improving the psychosocial outcomes. The certainty of the findings of this review is affected by the small number of included studies.
Self-reported maintenance training in patients with COPD - a 56 months follow-up study after initial pulmonary rehabilitation

Abstract:In our previous RCT, short-term effects of pulmonary rehabilitation (PR) for chronic obstructive pulmonary disease (COPD) were associated with attendance rate in a dose-response pattern, regardless of whether PR contained conventional physical exercise training or Singing for Lung Health. In this 56 months' follow-up study, we aimed to investigate if current self-reported maintenance training was correlated with initial PR adherence. We hypothesised that maintenance training was related to PR adherence in the past. In 2023, RCT participants, who were still alive, were contacted (n=196; 73% of 270), included (n=159; 81% of 196), and interviewed via telephone. We explored data using stratified analyses and logistic regression. In total, 30 participants had dropped out during the initial RCT. At long-term follow-up, these non-completers were currently more frequently smokers (p=0.04) and less active than completers (38% vs 60%; p=0.03), regardless of initial RCT randomisation. Moreover, they had decreased QoL (St George's Respiratory Questionnaire): mean change from since RCT: Non-completers: 4.0 (SD 15.6); Completers: -3.5 (SD 13.9). Current maintenance training was related to maintained QoL (p=0.04; OR: 2.4, 95% CI [1.0; 5.4]). Our study suggests that long-term maintenance training and maintenance of PR effects are positively associated with completion of an initial PR programme – irrespectively of randomisation to SLH or PExT. Therefore, we see a need for future PR programmes to include support of maintenance training after PR, and additional attention – including smoking cessation support - should be given to activate those who do not complete PR.

Abstract:Introduction: Outpatient pulmonary rehabilitation (PR) is being developed in order to offer this treatment to as many patients as possible. We present the results of a study that evaluated the reasons Method: A regional multicenter study including COPD patients referred for PR in the Lille and Valenciennes metropolitan PR centers, France, was conducted between October 2018 and March 2021 (NCT03453606). At the time of inclusion, a preference questionnaire covering 9 choice criteria and the SURE test were completed. In the event of a strong preference, the patient was referred to the local PR team offering the modality of his or her choice. Otherwise, the patient was randomized between home- and center-based PR. Results: 209 patients were included (65% male, mean age 64 years, mean FEV1 and BMI 42% and 26.3 kg/m 2), 188 of whom made a strong choice for one modality, 124 of which for home-based PR. Criteria most frequently associated with the choice of home-based PR were mobility capacities (87.1%), time constraints (83.9%) and constraints on personal life (77.4%). Criteria most frequently associated with the choice of center-based PR were expected supervision (93.8%), psychological support (82.8%), access to equipment (82.8%) and security(79.7%). 96.8% and 90.6% of patients who chose home-and center-based PR, respectively, were certain which was the best choice for them. Conclusion: The majority of patients have a preference between home- and center-based PR when choice criteria are proposed to them.

Abstract:Background: Pulmonary rehabilitation (PR) is second in importance to smoking cessation treatment in the management of chronic obstructive pulmonary disease (COPD). Access to the service is limited and less than half of those referred complete the treatment. Aims: We assessed the obstacles to participation in PR among COPD patients in a community-based PR programme and associated general practices. Methods: A qualitative interview study was conducted among COPD patients who completed the PR treatment, those who did not complete or declined treatment, and patients never referred. Participants were invited by letter from their own general practitioners or from the PR service. Views on exercise, disease education, social contact, group activity, accessibility, location, role of referrer, and support for participation were assessed. Data were analysed using the framework approach. Results: Twenty-four patients (28%, 13 male, 12 not referred) were interviewed. The acceptability of PR was the major concern. Feasibility of attending was an issue for some. Perceptions of PR and of exercise were highlighted. How a smoker might be seen, the suitability of group activity, and the views of professionals were influential, as were positive and negative recommendations. The location of the centre was important. Participants' willingness or reluctance to take on something new was a central element of the decision. Many would welcome the role of experienced patients in introducing the treatment. Conclusions: For patients who refused referral to PR, had not completed a course, or had yet to be referred, the way the service was introduced was an important determinant of willingness to participate.
Functional status following pulmonary rehabilitation in people with interstitial lung disease: A systematic review and meta-analysis

Abstract:Background: Individuals with interstitial lung disease (ILD) often experience worsening symptoms and activity avoidance. Limited data exists on outcome measures for assessing functional status (capacity and performance), as well as on the effectiveness of pulmonary rehabilitation (PR) in improving these outcomes in ILD. Aim: This review aimed to systematically assess the effects of PR on both functional capacity and performance in individuals with ILD. Methods: Randomised controlled trials involving pulmonary rehabilitation (PR) in adults with ILD, which included at least an exercise training component and education and/or psychosocial support, were included. Risk of bias and quality of evidence were assessed. Mean changes from baseline and standard deviations were retrieved for each group, and a random-effects model was applied. Results: Eight studies were included, mostly involving individuals with idiopathic pulmonary fibrosis (n = 5). PR duration ranged from 3 to 26 weeks. Seven studies used the 6MWT to evaluate functional capacity and one also used the 30-s STS. Two studies assessed functional performance, measuring time spent in moderate physical activity with the SenseWear Armband, number of steps per day with the same device, and energy expenditure in MET-min using the international physical activity questionnaire. PR improved functional capacity (6MWT-MD 45.82 m, 95%CI [26.14; 65.50], I2 = 71.54%, p < .001; 30-s STS- PR: 3.7 ± 2.6 reps; control group: -0.4 ± 2.5 reps, p < .001) compared to usual care. Only self-reported physical activity levels increased after PR (PR: 51.4 ± 57.7MET-min; control group: 20.9 ± 37.2MET-min, p = .03). Conclusion: PR is effective at improving functional capacity; however, functional performance is often overlooked, resulting in limited and inconclusive findings.

Abstract:Purpose: This study examined whether health-related quality of life (HRQL) and depression assessed prior to pulmonary rehabilitation (PR) participation (ie, at baseline) predicted change in 6-min walk distance (6MWD) from baseline to end of PR. Methods: Patients with pulmonary disease were consecutively referred/enrolled in a PR program from 2009-2022 (N = 503). Baseline 6MWD was assessed along with self-report measures of HRQL (St George's Respiratory Questionnaire [SGRQ]) and depression (Geriatric Depression Scale [GDS]). The SGRQ total score was used to assess overall HRQL, and SGRQ subscales assessed pulmonary symptoms, activity limitations, and psychosocial impacts of pulmonary disease. Multiple linear regression was used to examine whether baseline SGRQ scores and depression predicted Δ6MWD. Results: Baseline SGRQ total score ( F (1,389) = 8.4, P = .004) and activity limitations ( F (1,388) = 4.8, P = .03) predicted Δ6MWD. Patients with an SGRQ activity limitation score ≤ 25th percentile showed the most 6MWD improvement (mean = 79.7 m, SE = 6.7), and significantly more improvement than participants scoring between the 50-75th percentiles (mean = 54.4 m, SE = 6.0) or >75th percentile (mean = 48.7 m, SE = 7.5). Patients scoring between the 25-50th percentiles (mean = 70.2 m, SE = 6.1) did not differ significantly from other groups. The SGRQ symptoms and impacts subscales were unrelated to Δ6MWD ( F (1,388) = 1.2-1.9, P > .05), as was depression ( F (1,311) = 0.0, P > .85). Conclusions: Patients with greater HRQL at baseline may experience greater physical functioning improvement following PR. Additional support for patients with lower HRQL (eg, adjunctive self-management interventions) may enhance PR outcomes, particularly for patients who report greater activity limitations. Alternatively, early referral to PR (ie, when less symptomatic) may also benefit physical function outcomes.
