Abstract:Purpose: Continuous peripheral pulse oximetry for monitoring adequacy of oxygenation is probably the most important technological advance for patients' monitoring and safety in the last decades. Pulse oximetry has the disadvantage of measuring the peripheral circulation, and the only mean to measure oxygen content of the central circulation is by invasive technology. Determination of blood oxyhaemoglobin saturation in the retinal vessels of the eye can be achieved noninvasively through spectrophotometric retinal oximetry which provides access to the central nervous system circulation. The aim of the thesis was to determine whether retinal oximetry technique can be applied for estimation of the central nervous system circulation which until now has only been possible invasively. This was achieved by measuring oxyhaemoglobin saturation in three adult subject study groups: in people with central retinal vein occlusion (CRVO) to observe local tissue hypoxia, in patients with severe chronic obstructive pulmonary disease (COPD) on long-term oxygen therapy to observe systemic hypoxaemia and in healthy subjects during hyperoxic breathing to observe systemic hyperoxemia. In addition, the fourth study that is mentioned was performed to test whether retinal oximetry is feasible for neonates. Methods: Retinal oximetry in central retinal vein occlusion: Sixteen subjects with central retinal vein occlusion participated in the study. The oxyhaemoglobin saturation of the central retinal vein occlusion affected eye was compared with the fellow unaffected eye. Retinal oximetry in healthy people under hyperoxia: Thirty healthy subjects participated in the study, and the oxyhaemoglobin saturation of retinal arterioles and venules was compared between normoxic and hyperoxic breathing. Retinal oximetry in severe chronic obstructive pulmonary disease: Eleven patients with severe chronic obstructive pulmonary disease participated in the study. Retinal oximetry measurements were made with and without their daily supplemental oxygen therapy. Retinal arteriolar oxyhaemoglobin saturation when inspiring ambient air was compared with blood samples from the radial artery and finger pulse oximetry and healthy controls. The healthy control group was assembled from our database for comparison of oxyhaemoglobin saturation of retinal arterioles and venules during the ambient air breathing. The retinal oximeter is based on a conventional fundus camera and a specialized software. A beam splitter coupled with two high-resolution digital cameras allows for simultaneous acquisition of retinal images at separative wavelengths for calculation of oxyhaemoglobin saturation. In addition, retinal images of 28 full-term healthy neonates were obtained with scanning laser ophthalmoscope combined with modified Oxymap analysis software for calculation of the optical density ratio and vessel diameter RESULTS: Retinal oximetry in central retinal vein occlusion: Mean retinal venous oxyhaemoglobin saturation was 31 ± 12% in CRVO eyes and 52 ± 11% in unaffected fellow eyes (mean ± SD, n = 14, p < 0.0001). The arteriovenous oxygen difference (AV-difference) was 63 ± 11% in CRVO eyes and 43 ± 7% in fellow eyes (p < 0.0001). The variability of retinal venous oxyhaemoglobin saturation was considerable within and between eyes affected by CRVO. There was no difference in oxyhaemoglobin saturation of retinal arterioles between the CRVO eyes and the unaffected eyes (p = 0.49). Retinal oximetry in healthy people under hyperoxia: During hyperoxic breathing, the oxyhaemoglobin saturation in retinal arterioles increased to 94.5 ± 3.8% as compared with 92.0 ± 3.7% at baseline (n = 30, p < 0.0001). In venules, the mean oxyhaemoglobin saturation increased to 76.2 ± 8.0% from 51.3 ± 5.6% (p < 0.0001) at baseline. The AV-difference was markedly lower during hyperoxic breathing as compared with the normoxic breathing (18.3 ± 9.0% versus 40.7 ± 5.7%, p < 0.0001). Retinal oximetry in severe chronic obstructive pulmonary disease: During ambient air breathing, chronic obstructive pulmonary disease subjects had significantly lower oxyhaemoglobin saturation than healthy controls in both retinal arterioles (87.2 ± 4.9% versus 93.4 ± 4.3%, p = 0.02, n = 11) and venules (45.0 ± 10.3% versus 55.2 ± 5.5%, p = 0.01) but the AV-difference was not markedly different (p = 0.17). Administration of their prescribed oxygen therapy significantly increased the oxyhaemoglobin saturation in retinal arterioles (87.2 ± 4.9% to 89.5 ± 6.0%, p = 0.02) but not in venules (45.0 ± 10.3% to 46.7 ± 12.8%, p = 0.3). Retinal oximetry values were slightly lower than finger pulse oximetry (mean percentage points difference = -3.1 ± 5.5) and radial artery blood values (-5.0 ± 5.4). Retinal oximetry study in neonates: The modified version of the retinal oximetry instrument estimated the optical density ratio in retinal arterioles to be 0.256 ± 0.041 that was significantly different from the 0.421 ± 0.089 in venules (n = 28, p < 0.001, paired t-test). The vascular diameter of retinal arterioles was markedly narrower than of venules (14.1 ± 2.7 and 19.7 ± 3.7 pixels, p < 0.001). Conclusion: The results of this thesis indicate that spectrophotometric retinal oximetry is sensitive to both local and systemic changes in oxyhaemoglobin saturation. Retinal oxyhaemoglobin saturation values are slightly lower than radial artery blood sample and finger pulse oximetry values. The discrepancies between the different modalities are expected to derive from countercurrent exchange between central retinal artery and vein within the optic nerve but calibration issues cannot be excluded as contributing to this difference. Despite these differences, the findings indicate the potential of retinal oximetry for noninvasive real-time measurements of oxyhaemoglobin saturation in central nervous system vessels. Following calibration upgrade and technological improvement, verification retinal oximetry may potentially be applied to critically ill and anaesthesia care patients. The study on combined scanning laser ophthalmoscope and retinal oximetry supports the feasibility of the technique for oximetry analysis in newly born babies.
A novel approach for measurement of retinal oxygen extraction based on laser speckle flowgraphy and retinal oximetry

Abstract:Aims/Purpose: Several ocular diseases lead to a reduction in retinal oxygen metabolism. However, measurement of retinal oxygen extraction is mostly based on time‐consuming and custom‐built devices. Hence, the aim of this study is to assess retinal oxygen extraction based on measurement of retinal blood flow with the commercially available laser‐speckle flowgraphy (LSFG) and measurement of retinal oxygen saturation with the commercially available retinal oximetry. Methods: Ten young healthy subjects participated in the present study. Retinal blood flow and retinal oxygen saturation were measured in each vessel around the optic disc at the same position. The corrected arterial (co2, CRA) and venous (co2, CRV) oxygen content was estimated from the parameters evaluated and the difference between these two determined parameters (co2, DIFF) was assessed. Retinal oxygen extraction was calculated using mean vessel flow rate (MV) and cO2, DIFF. To validate the novel approach, baseline measurement and measurement during inhalation of 100% oxygen were performed, as retinal oxygen extraction is known to decrease during hyperoxia. Results: MV significantly decreased during 100% oxygen breathing (−23 ± 10%, p < 0.001). While cO2, CRA only slightly increased by 1 ± 2% ( p = 0.093), cO2, CRV increased by 9 ± 14% ( p = 0.050). Consequently, cO2, DIFF decreased by 10 ± 20% ( p = 0.087). This led to a pronounced decrease in retinal oxygen extraction by −31 ± 18% during hyperoxia ( p < 0.001). Conclusions: During breathing of 100% oxygen a significant decrease in TRBF and retinal oxygen extraction was assessed. The decrease in retinal oxygen extraction during hyperoxia is consistent with findings in literature and is physiologically expected. Therefore, the novel approach for measurement of retinal oxygen extraction appears to be feasible. Larger studies in patient cohorts using this method should be conducted in the future.

Abstract:Similar to other organs, the retina relies on tightly regulated perfusion and oxygenation. Previous studies have demonstrated that retinal blood flow is affected in a variety of eye and systemic diseases, including diabetic retinopathy, age-related macular degeneration, and glaucoma. Although measurement of peripheral oxygen saturation has become a standard clinical measurement through the development of pulse oximetry, developing a noninvasive technique to measure retinal oxygen saturation has proven challenging, and retinal oximetry technology currently remains inadequate for reliable clinical use. Here, we review current strategies and approaches, as well as several newer technologies in development, and discuss the future of retinal oximetry.
Development of a new instrument to measure oxygen saturation and total hemoglobin volume in local skin by near-infrared spectroscopy and its clinical application

Abstract:Abstract The oxygen saturation (StO2) and total hemoglobin volume in cutaneous blood are closely related to cutaneous metabolism and are important factors in determining the skin color. Most conventional apparatuses for the measurement of cutaneous metabolism have been designed to evaluate qualitative changes in the oxyhemoglobin volume, deoxyhemoglobin volume, and their sum (total Hb volume) relative to their baseline values. In this study, we developed an instrument for non-invasive evaluation of individual and regional differences in StO2 and Hb volume, a system unaffected by melanin (Kao PSA system model III), and examined the validity of its application. First, changes in StO2 and total Hb volume in the antebrachial region during venous occlusion and devascularization by compression of the brachial region were evaluated. Changes in total Hb volume following venous occlusion were found to reflect the cutaneous blood flow. Also, StO2 was considered to reflect the state of oxygen consumption by the skin, because it was markedly reduced during devascularization. Next, the subjects were exposed to graded hypobaric conditions, and the relationships among StO2, arterial blood oxygen saturation (SaO2), and venous blood oxygen saturation (SvO2) were studied. StO2 showed significant positive correlations with SaO2 (r=0.811, P<0.001) and SvO2 (r=0.966, P<0.001), and its correlation with SvO2 was particularly strong. Therefore, StO2 was found to be closely dependent on SvO2. Lastly, StO2, total Hb volume, and other parameters were measured in healthy women (aged 20–69 years), and their regional differences and age- associated changes were evaluated. These regional differences (angle of mouth > cheek > forehead) and age-associated decreases in StO2 are considered to be caused by the age-associated decreases in the cutaneous blood flow.

Abstract:Impaired retinal blood flow (RBF) autoregulation plays a key role in the development and progression of several ocular diseases, including glaucoma and diabetic retinopathy. Clinically, reproducible RBF quantitation could significantly improve early diagnosis and disease management. Several non-invasive techniques have been developed but are limited for retinal microvasculature flow measurements due to their low signal-to-noise ratio and poor lateral resolution. In this study, we demonstrate reproducible vessel caliber and retinal blood flow velocity measurements in healthy human volunteers using a high-resolution (spatial and temporal) multimodal adaptive optics system with scanning laser ophthalmoscopy and optical coherence tomography.

Abstract:Quantitatively determining physiological parameters at a microscopic level in the retina furthers the understanding of the molecular pathways of blinding diseases, such as diabetic retinopathy and glaucoma. An essential parameter, which has yet to be quantified noninvasively, is the retinal oxygen metabolic rate (rMRO2). Quantifying rMRO2 is challenging because two parameters, the blood flow rate and hemoglobin oxygen saturation (sO2), must be measured together. We combined photoacoustic ophthalmoscopy (PAOM) with spectral domain-optical coherence tomography (SD-OCT) to tackle this challenge, in which PAOM measured the sO2 and SD-OCT mapped the blood flow rate. We tested the integrated system on normal wild-type rats, in which the measured rMRO2 was 297.86 ± 70.23 nl/minute. This quantitative method may shed new light on both fundamental research and clinical care in ophthalmology in the future.

Abstract:The retina is one of the most metabolically active tissues in the body. The dysfunction of oxygen kinetics in the retina is closely related to the disease and has important clinical value. Dynamic imaging and comprehensive analyses of oxygen kinetics in the retina depend on the fusion of structural and functional imaging and high spatiotemporal resolution. But it's currently not clinically available, particularly via a single imaging device. Therefore, this work aims to develop a retinal oxygen kinetics imaging and analysis (ROKIA) technology by integrating dual-wavelength imaging with laser speckle contrast imaging modalities, which achieves structural and functional analysis with high spatial resolution and dynamic measurement, taking both external and lumen vessel diameters into account. The ROKIA systematically evaluated eight vascular metrics, four blood flow metrics, and fifteen oxygenation metrics. The single device scheme overcomes the incompatibility of optical design, harmonizes the field of view and resolution of different modalities, and reduces the difficulty of registration and image processing algorithms. More importantly, many of the metrics (such as oxygen delivery, oxygen metabolism, vessel wall thickness, etc.) derived from the fusion of structural and functional information, are unique to ROKIA. The oxygen kinetic analysis technology proposed in this paper, to our knowledge, is the first demonstration of the vascular metrics, blood flow metrics, and oxygenation metrics via a single system, which will potentially become a powerful tool for disease diagnosis and clinical research.
In vivo photoacoustic imaging of chorioretinal oxygen gradients.

Abstract:Chorioretinal imaging has a crucial role for the patients with chorioretinal vascular diseases, such as neovascular age-related macular degeneration. Imaging oxygen gradients in the eye could better diagnose and treat ocular diseases. Here, we describe the use of photoacoustic ocular imaging (PAOI) in measuring chorioretinal oxygen saturation (CR - sO2) gradients in New Zealand white rabbits (n = 5) with ocular ischemia. We observed good correlation (R2 = 0.98) between pulse oximetry and PAOI as a function of different oxygen percentages in inhaled air. We then used an established ocular ischemia model in which intraocular pressure is elevated to constrict ocular blood flow, and notice a positive correlation (R2 = 0.92) between the injected volume of phosphate buffered saline (PBS) and intraocular pressure (IOP) as well as a negative correlation (R2 = 0.98) between CR - sO2 and injected volume of PBS. The CR - sO2 was measured before (baseline), during (ischemia), and after the infusion (600-μL PBS). The ischemia-reperfusion model did not affect the measurement of the sO2 using a pulse oximeter on the animal's paw, but the chorioretinal PAOI signal showed a nearly sixfold decrease in CR - sO2 (n = 5, p = 0.00001). We also observe a sixfold decrease in CR - sO2 after significant elevation of IOP during ischemia, with an increase close to baseline during reperfusion. These data suggest that PAOI can detect changes in chorioretinal oxygenation and may be useful for application to imaging oxygen gradients in ocular disease.

Abstract:The development of laparoscopic cholecystectomy has had a major impact on the management of patients with biliary tract disease. The evidence today suggests that the procedure is safe, effective, and has special advantages over open cholecystectomy in terms of patient discomfort and postoperative convalescence. Laparoscopic cholecystectomy has become the treatment of choice for patients with symptomatic cholelithiasis. However, the availability of this minimally invasive approach to gallstones should not alter the indications for operation. Minimally invasive techniques are increasingly being adapted to the management of patients with other biliary tract problems, including acute cholecystitis and choledocholithiasis. Continued surveillance of outcomes is warranted to ensure that these procedures are being applied safely and appropriately.
Noninvasive imaging of human skin hemodynamics using a digital red-green-blue camera

Abstract:In order to visualize human skin hemodynamics, we investigated a method that is specifically developed for the visualization of concentrations of oxygenated blood, deoxygenated blood, and melanin in skin tissue from digital RGB color images. Images of total blood concentration and oxygen saturation can also be reconstructed from the results of oxygenated and deoxygenated blood. Experiments using tissue-like agar gel phantoms demonstrated the ability of the developed method to quantitatively visualize the transition from an oxygenated blood to a deoxygenated blood in dermis. In vivo imaging of the chromophore concentrations and tissue oxygen saturation in the skin of the human hand are performed for 14 subjects during upper limb occlusion at 50 and 250 mm Hg. The response of the total blood concentration in the skin acquired by this method and forearm volume changes obtained from the conventional strain-gauge plethysmograph were comparable during the upper arm occlusion at pressures of both 50 and 250 mm Hg. The results presented in the present paper indicate the possibility of visualizing the hemodynamics of subsurface skin tissue.

Abstract:Journal of Mechanics in Medicine and Biology, Ahead of Print. The purpose of this study is to develop a near-infrared blood flow measurement algorithm and verify its performance. The gradient value of the measurement point was determined by using LED lights with wavelengths of 760[math]nm and 850[math]nm. This process involved measuring the gradient value for a significant number of regularly spaced cubic pixels centered around the chosen point. The measured gradient values for each pixel were then averaged to derive the final gradient value. Based on the measurement of the gradation value, when the concentration ratio of blood was altered by 20% between 0% and 100%, the gradation values at 760[math]nm and 850[math]nm changed significantly (more than [math]) in all cases. This study validated the efficacy of a self-developed near-infrared light system for detecting changes in blood flow. The ability to measure alterations in blood flow can be instrumental in predicting peripheral vascular disease (PVD) due to compromised circulatory conditions.

Abstract:Retinal vessel oxygen supply is important for retinal tissue metabolism. Commonly used retinal vessel oximetry devices are based on dual-wavelength spectral measurement of oxyhemoglobin and deoxyhemoglobin. However, there is no traceable standard for reliable calibration of these devices. In this study, we developed a fundus-simulating phantom that closely mimicked the optical properties of human fundus tissues. Microchannels of precisely controlled topological structures were produced by soft lithography to simulate the retinal vasculature. Optical properties of the phantom were adjusted by adding scattering and absorption agents to simulate different concentrations of fundus pigments. The developed phantom was used to calibrate the linear correlation between oxygen saturation (SO2) level and optical density ratio in a dual-wavelength oximetry device. The obtained calibration factors were used to calculate the retinal vessel SO2 in both eyes of five volunteers aged between 24 and 27 years old. The test results showed that the mean arterial and venous SO2 levels after phantom calibration were coincident with those after empirical value calibration, indicating the potential clinical utility of the produced phantom as a calibration standard.

Abstract:In this study, via the retinal functional imaging device, not only the structural information of the retina is imaged, but also the functional information such as the retinal oxygen saturation and vessel diameter is imaged and measured noninvasively and quantitatively. By comparing quantitatively the functional imaging results of diabetes patients and healthy controls, significantly elevated arterial oxygen saturation and arterial-venous oxygen saturation difference arc found in diabetes patients. The area of the foveal avascular zone is also significantly increased in diabetes patients. The result also shows that there arc significant differences between the average diameters of arteries and veins both in diabetes patients and healthy controls, and the arterial diameter is always larger than the venous diameter. The study demonstrates that by using the retinal functional imaging method, the clinical changes of retinal oxygen metabolism as well as the perfusion rate in diabetes patients can be detected, which helps doctors to better observe and evaluate the pathological changes of the retinal microvessels in diabetes patients. In addition, the retinal functional imaging technique has the application potential for the early diagnosis and prognostic evaluation of ophthalmic complications associated with diabetes.
Quantitative Volumetric Analysis of Retinal Ischemia with an Oxygen Diffusion Model and OCT Angiography

Abstract:Purpose: Retinal ischemia is a major feature of diabetic retinopathy (DR). Traditional nonperfused areas measured by OCT angiography (OCTA) measure blood supply but not ischemia. We propose a novel 3-dimensional (3D) quantitative method to derive ischemia measurements from OCTA data. Design: Cross-sectional study. Participants: We acquired 223 macular OCTA volumes from 33 healthy eyes, 33 diabetic eyes without retinopathy, 7 eyes with nonreferable DR, 17 eyes with referable but nonvision-threatening DR, and 133 eyes with vision-threatening DR. Methods: Each eye was scanned using a spectral-domain OCTA system (Avanti RTVue-XR, Visionix/Optovue, Inc) with 1.6-mm scan depth in a 3 × 3-mm region (640 × 304 × 304 voxels) centered on the fovea. For each scanned OCTA volume, a custom algorithm removed flow projection artifacts. We then enhanced, binarized, and skeletonized the vasculature in each OCTA volume and generated a 3D oxygen tension map using a zero-order kinetics oxygen diffusion model. Each volume was scaled to the average retina thickness in healthy controls after foveal registration and flattening of the Bruch's membrane. Finally, we extracted 3D ischemia maps by comparison with a reference map established from scans of healthy eyes using the same processing. To assess the ability of the ischemia maps to grade DR severity, we constructed receiver operating characteristic curves for diagnosing diabetes, referable DR, and vision-threatening DR. Main outcome measures: Spearman correlation coefficient and area under receiver operating characteristic curve (AUC) were used to quantify the ability of the ischemia maps to DR. Results: The ischemia maps showed that the ischemic tissues were at or near pathologically nonperfused areas, but not the normally nonvascular tissue, such as the foveal avascular zone. We found multiple novel metrics, including inferred 3D-oxygen tension, ischemia index, and ischemic volume ratio, were strongly correlated with DR severity. The AUCs of ischemia index measured were 0.94 for diabetes, 0.89 for DR, 0.88 for referable DR, and 0.85 for vision-threatening DR. Conclusions: A quantitative method to infer 3D oxygen tension and ischemia using OCTA in diabetic eyes can identify ischemic tissue that are more specific to pathologic changes in DR. Abstract:PURPOSE:To evaluate the possibility as well as the usage of adaptive optics in high-resolution retinal imaging.METHODS:From March to November 2001, the fundus of 25 adults were checked by using Optic Adaptive Retinal Imaging System (OAS). The age of the subjects varied from 18 - 48 years. All had normal visual acuity from 0.9 to 1.0. No abnormality was found in the ocular examination, and their medical as well as ocular history was unremarkable.RESULTS:High-resolution images of the retinal cells, photoreceptor and bipolar cell, were analysed. In these images, the cells are clearly resolved. The density of the photoreceptor at area 1.5 degree from the foveloa is around 40 000 - 50 000/mm2. At area 3 degree, it drops to less than 30 000/mm2.CONCLUSION:Optic Adaptive Retinal Imaging System (AOS) is able to get high-resolution image of retinal cells in living human eyes. It may be widely used in ophthalmology experimentally and clinically.
Non-invasive structural and metabolic retinal markers of disease activity in non-proliferative diabetic retinopathy

Abstract:Purpose: Metabolic and structural microvascular retinal alterations are essential components in diabetic retinopathy (DR). The present study aimed to measure changes at different stages of non-proliferative DR (NPDR) and to explore interactions of imaging-based metrics. Methods: This cross-sectional, cohort study included 139 eyes from 80 diabetic patients. Each patient underwent dilated fundal examinations including colour fundus photography, retinal oximetry and optical coherence tomography angiography (OCTA), analysed by semi-automated and automated software. Diabetic retinopathy (DR) severity was classified according to the International Clinical Diabetic Retinopathy (ICDR) Severity Scale, ranging from no DR to severe NPDR (level 0-3). Retinal metabolism was evaluated by oximetry as retinal arteriolar (raSatO2 ) and venular oxygen saturation (rvSatO2 ), and macular microvascular structure was measured by OCTA as the area of foveal avascular zone (FAZ), vessel density (VD), vessel diameter index (VDI), FAZ circularity and fractal dimension (FD) in the superficial and deep retinal capillary plexus. Results: A trend for increasing rvSatO2 was found with increasing DR severity (51.3%, 53.3%, 54.2%, 59.8%, p = 0.02). Increasing severity of DR associated with decreasing FD in the superficial and deep plexus (p < 0.001 and p = 0.014), and in the superficial plexus decreasing VD (p < 0.001), increasing VDI (p = 0.003) and decreasing FAZ circularity (p = 0.006). A few interactions were identified between raSatO2 , rvSatO2 and VDI, but only in the deep capillary plexus (p < 0.01 and p < 0.01). Conclusion: Alterations of the venular retinal vascular oxygen saturation and microvascular structural abnormities were found continuously throughout the DR-spectrum. Given the sparse correlations between metabolic and structural abnormalities, it seems that these occur independently in DR.

Abstract:A remedial device for hand insufficiency includes a glove trank having a foundation and allowing the palm of a hand to be applied thereto, the thumb and fingers of the hand to be fixed to the foundation with finger stoppers, and the wrist and forearm to be fixed to the foundation with an arm stopper; a plurality of first bag bodies accommodating finger-spreading air sacks and disposed on the front side of the foundation; a pair of second bag bodies accommodating joint-extending air sacks and disposed on the back side of the foundation; and a compressed air feed and discharge device for inflating and deflating the air sacks in the first and second bag bodies to effect a spreading of the fingers and an extension and dorsiflexion of hand joints. Each of the second bag bodies includes two bag pieces which are each formed of cloth cut into a foundation. The second bag bodies each have a concavity where the foundation is folded about itself and the bag pieces are adjoined to each other through the fold forming the concavity. The arm stopper includes a wrist strap extending laterally from one side of the foundation and having a length sufficient to be wound around the wrist at least one and a half times and a pair of forearm straps.
Retinal Oximetry and Vessel Diameter Measurements With a Commercially Available Scanning Laser Ophthalmoscope in Diabetic Retinopathy

Abstract:Purpose: To test the hypothesis that retinal vascular diameter and hemoglobin oxygen saturation alterations, according to stages of diabetic retinopathy (DR), are discernible with a commercially available scanning laser ophthalmoscope (SLO). Methods: One hundred eighty-one subjects with no diabetes (No DM), diabetes with no DR (No DR), nonproliferative DR (NPDR), or proliferative DR (PDR, all had photocoagulation) underwent imaging with an SLO with dual lasers (532 nm and 633 nm). Customized image analysis software determined the diameters of retinal arteries and veins (DA and DV) and central retinal artery and vein equivalents (CRAE and CRVE). Oxygen saturations of hemoglobin in arteries and veins (SO2A and SO2V) were estimated from optical densities of vessels on images at the two wavelengths. Statistical models were generated by adjusting for effects of sex, race, age, eye, and fundus pigmentation. Results: DA, CRAE, and CRVE were reduced in PDR compared to No DM (P ≤ 0.03). DV and CRVE were similar between No DM and No DR, but they were higher in NPDR than No DR (P ≤ 0.01). Effect of stage of disease on SO2A differed by race, being increased relative to No DM in NPDR and PDR in Hispanic participants only (P ≤ 0.02). Relative to No DM, SO2V was increased in NPDR and PDR (P ≤ 0.05). Conclusions: Alterations in retinal vascular diameters and SO2 by diabetic retinopathy stage can be detected with a widely available SLO, and covariates such as race can influence the results.

Abstract:Retinal oximetry imaging of retinal blood vessels measures oxygen saturation of hemoglobin. The imaging technology is non-invasive and reproducible with remarkably low variability on test-retest studies and in healthy cohorts. Pathophysiological principles and novel biomarkers in several retinal diseases have been discovered, as well as possible applications for systemic and brain disease. In diabetic retinopathy, retinal venous oxygen saturation is elevated and arteriovenous difference progressively reduced in advanced stages of retinopathy compared with healthy persons. This correlates with pathophysiology of diabetic retinopathy where hypoxia stimulates VEGF production. Laser treatment and vitrectomy both improve retinal oximetry values, which correlate with clinical outcome. The oximetry biomarker may allow automatic measurement of severity of diabetic retinopathy and predict its response to treatment. Central retinal vein occlusion is characterized by retinal hypoxia, which is evident in retinal oximetry. The retinal hypoxia seen on oximetry correlates with the extent of peripheral ischemia, visual acuity and thickness of macular edema. This biomarker may help diagnose and measure severity of vein occlusion and degree of retinal ischemia. Glaucomatous retinal atrophy is associated with reduced oxygen consumption resulting in reduced arteriovenous difference and higher retinal venous saturation. The oximetry findings correlate with worse visual field, thinner nerve fiber layer and smaller optic disc rim. This provides an objective biomarker for glaucomatous damage. In retinitis pigmentosa, an association exists between advanced atrophy, worse visual field and higher retinal venous oxygen saturation, lower arteriovenous difference. This biomarker may allow measurement of severity and progression of retinitis pigmentosa and other atrophic retinal diseases. Retinal oximetry offers visible light imaging of systemic and central nervous system vessels. It senses hypoxia in cardiac and pulmonary diseases. Oximetry biomarkers have been discovered in Alzheimer's disease and multiple sclerosis and oxygen levels in the retina correspond well with brain.
