Abstract:BACKGROUND:Delirium in patients in intensive care units (ICUs) is an acute disturbance and fluctuation of cognition and consciousness. Though increasing age has been found to be related to ICU delirium, there is limited evidence of the effect of age on delirium outcomes. The aim of this study is to investigate the relationship between age categories and outcomes among ICU delirium patients.METHODS:Data were extracted from the electronic ICU (eICU) Collaborative Research Database with records from 3,931 patients with delirium. Patients were classified into non-aged (<65 years), young-old (65-74 years), middle-old (75-84 years), and very-old (≥85 years) groups. A Cox regression model was built to examine the role of age in death in ICU and in hospital after controlling covariates.RESULTS:The sample included 1,667 (42.4%) non-aged, 891 (22.7%) young-old, 848 (21.6%) middle-old, and 525 (13.3%) very-old patients. The ICU mortality rate was 8.3% and the hospital mortality rate was 15.4%. Compared with the non-aged group, the elderly patients (≥65 yeras) had higher mortality at ICU discharge (χ2 =13.726, P=0.001) and hospital discharge (χ 2=56.347, P<0.001). The Cox regression analysis showed that age was an independent risk factor for death at ICU discharge (hazard ratio [HR]=1.502, 1.675, 1.840, 95% confidence interval [CI] 1.138-1.983, 1.250-2.244, 1.260-2.687; P=0.004, 0.001, 0.002 for the young-, middle- and very-old group, respectively) as well as death at hospital discharge (HR=1.801, 2.036, 2.642, 95% CI 1.454-2.230, 1.638-2.530, 2.047-3.409; all P<0.001).CONCLUSIONS:The risks of death in the ICU and hospital increase with age among delirious patients.

Abstract:Six Schistosoma mansoni isolates obtained from the same endemic area were compared experimentally; three were from patients with the hepatosplenic form and three from patients with the intestinal form of schistosomiasis. The following features were analyzed in mice: body weight, liver and spleen weights, mortality, infectivity, number of S. mansoni eggs/female worm, egg distribution in the viscera, and hemogram. Three groups of mice, used as controls, were infected with the LE strain of S. mansoni which is routinely maintained in the laboratory. The group of isolates from patients with the hepatosplenic form of schistosomiasis did not show important differences in relation to those from nonhepatosplenic patients; in almost all features analyzed the differences among these six isolates were within the variation limits observed with the LE strain of S. mansoni.
Delirium is prevalent in older hospital inpatients and associated with adverse outcomes: results of a prospective multi-centre study on World Delirium Awareness Day

Abstract:Abstract Background Delirium is a common severe neuropsychiatric condition secondary to physical illness, which predominantly affects older adults in hospital. Prior to this study, the UK point prevalence of delirium was unknown. We set out to ascertain the point prevalence of delirium across UK hospitals and how this relates to adverse outcomes. Methods We conducted a prospective observational study across 45 UK acute care hospitals. Older adults aged 65 years and older were screened and assessed for evidence of delirium on World Delirium Awareness Day (14th March 2018). We included patients admitted within the previous 48 h, excluding critical care admissions. Results The point prevalence of Diagnostic and Statistical Manual on Mental Disorders, Fifth Edition (DSM-5) delirium diagnosis was 14.7% (222/1507). Delirium presence was associated with higher Clinical Frailty Scale (CFS): CFS 4–6 (frail) (OR 4.80, CI 2.63–8.74), 7–9 (very frail) (OR 9.33, CI 4.79–18.17), compared to 1–3 (fit). However, higher CFS was associated with reduced delirium recognition (7–9 compared to 1–3; OR 0.16, CI 0.04–0.77). In multivariable analyses, delirium was associated with increased length of stay (+ 3.45 days, CI 1.75–5.07) and increased mortality (OR 2.43, CI 1.44–4.09) at 1 month. Screening for delirium was associated with an increased chance of recognition (OR 5.47, CI 2.67–11.21). Conclusions Delirium is prevalent in older adults in UK hospitals but remains under-recognised. Frailty is strongly associated with the development of delirium, but delirium is less likely to be recognised in frail patients. The presence of delirium is associated with increased mortality and length of stay at one month. A national programme to increase screening has the potential to improve recognition.

Abstract:Background Delirium in older hospitalized patients (> 65) is a common clinical syndrome, which is frequently unrecognized. Aims We aimed to describe the detailed clinical course of delirium and related cognitive functioning in geriatric patients in a mainly non-postoperative setting in association with demographic and clinical parameters and additionally to identify risk factors for delirium in this common setting. Methods Inpatients of a geriatric ward were screened for delirium and in the case of presence of delirium included into the study. Patients received three assessments including Mini-Mental-Status-Examination (MMSE) and the Delirium Rating Scale Revised 98 (DRS-R-98). We conducted correlation and linear mixed-effects model analyses to detect associations. Results Overall 31 patients (82 years (mean)) met the criteria for delirium and were included in the prospective observational study. Within one week of treatment, mean delirium symptom severity fell below the predefined cut-off. While overall cognitive functioning improved over time, short- and long-term memory deficits remained. Neuroradiological conspicuities were associated with cognitive deficits, but not with delirium severity. Discussion The temporal stability of some delirium symptoms (short-/long-term memory, language) on the one hand and on the other hand decrease in others (hallucinations, orientation) shown in our study visualizes the heterogeneity of symptoms attributed to delirium and their different courses, which complicates the differentiation between delirium and a preexisting cognitive decline. The recovery from delirium seems to be independent of preclinical cognitive status. Conclusion Treatment of the acute medical condition is associated with a fast decrease in delirium severity. Given the high incidence and prevalence of delirium in hospitalized older patients and its detrimental impact on cognition, abilities and personal independence further research needs to be done.

Abstract:Abstract Background Inpatient delirium is common and associated with poor outcomes. Although most organisations have evidence-based guidelines to improve delirium prevention and management, delirium rates and outcomes have remained relatively unchanged over time. A lack of understanding of healthcare providers’ experience of caring for people with delirium and its integration into existing guidance may explain some of the slow progress in improving delirium care. Objective To review and synthesise existing qualitative evidence on healthcare providers’ experience of caring for inpatients with delirium within and across disciplines. Methods We systematically searched OVID Medline, CINAHL, Embase, Emcare, PsychINFO, AMED and Web of Science databases for articles published between January 1990 and November 2022. Article inclusion and study quality were assessed by two independent reviewers. Both thematic synthesis and content analysis were then conducted to synthesise findings from included studies. Results Within the 25 included studies, the experience of nurses was the most commonly studied perspective, followed by medical and allied health. Nursing, medical and allied health staff all reported that their experience of caring for people with delirium was challenging, highlighting difficulties in delirium recognition and that they felt unsupported at organisational and local levels. Attitudes towards older people and the importance of delirium influenced identification and prioritisation. Conclusions Healthcare providers often find caring for hospitalised patients with delirium challenging and complex. Although good communication within multidisciplinary teams was deemed helpful, more work is required to understand how to achieve this, recognising the unique perspectives of individual disciplines.

Abstract:Delirium is a common condition in the elderly, affecting up to 30% of all older patients admitted to hospital. There is a particularly high risk of delirium in surgical inpatients, especially following operations for hip fracture or vascular surgery, but also for patients in the intensive care unit. Patients with delirium have higher morbidity and mortality rates, higher re-admission rates, and a greater risk of long term institutionalisation care, thereby having a significant impact on both health and social care expenditure. Delirium frequently goes unrecognised by clinicians and is often inadequately managed. Recent evidence suggests that a better understanding and knowledge of delirium among health care professionals can lead to early detection, the reduction of modifiable risk factors, and better management of the condition in the acute phase. Many cases of delirium are potentially preventable, and primary and secondary care services should be taking active steps in order to do prevent this condition.

Abstract:The present review aims to highlight this intricate syndrome, regarding diagnosis, pathophysiology, etiology, prevention, and management in elderly people. The diagnosis of delirium is based on clinical observations, cognitive assessment, physical, and neurological examination. Clinically, delirium occurs in hyperactive, hypoactive, or mixed forms, based on psychomotor behavior. As an acute confusional state, it is characterized by a rapid onset of symptoms, fluctuating course and an altered level of consciousness, global disturbance of cognition or perceptual abnormalities, and evidence of a physical cause. Although pathophysiological mechanisms of delirium remain unclear, current evidence suggests that disruption of neurotransmission, inflammation, or acute stress responses might all contribute to the development of this ailment. It usually occurs as a result of a complex interaction of multiple risk factors, such as cognitive impairment/dementia and current medical or surgical disorder. Despite all of the above, delirium is frequently under-recognized and often misdiagnosed by health professionals. In particular, this happens due to its fluctuating nature, its overlap with dementia and the scarcity of routine formal cognitive assessment in general hospitals. It is also associated with multiple adverse outcomes that have been well documented, such as increased hospital stay, function/cognitive decline, institutionalization and mortality. In this context, the early identification of delirium is essential. Timely and optimal management of people with delirium should be performed with identification of any possible underlying causes, dealing with a suitable care environment and improving education of health professionals. All these can be important factors, which contribute to a decrease in adverse outcomes associated with delirium.

Delirium: the lived experience of older people who are delirious post-orthopaedic surgery

Abstract:Delirium is a common, potentially preventable and reversible cause of postoperative functional disability, morbidity, and mortality. It can lead to increased health-care use and also poses a substantial challenge for nurses caring for patients who experience delirium after surgery. Predominantly, the published work concentrates on diagnosis, reduction of the modifiable risk factors, and treatments. Compared with this body of published work, the experience of delirium from a patient's perspective has been largely ignored except for a limited number of qualitative research reports. The importance of researching the lived experience of delirium is that a better understanding may lead to more empathic, therapeutic nursing care and help other sufferers to know they are not alone. The aims of the study were to explore and clarify the lived experience of delirium. Eleven patients were recruited to the study following discharge post-surgery from an orthopaedic ward of a major tertiary hospital. The study used a qualitative descriptive approach and incorporated grounded theory data analysis processes. The findings of this study provide an insight into the incomprehensible emotional pain suffered by patients while they were delirious and the disparate feelings of remorse, guilt, and shame they experienced after the episode of delirium. Following this study, recommendations for nursing practice include formal follow-up support for patients with post-delirium episodes and more research into the long-term impact of the experience of delirium.
A survey of non-consultant hospital doctors' perspectives, knowledge, and practices toward delirium in a large Irish hospital

Abstract:Background: Delirium is a common condition in hospitals, particularly among older people. This refers to a dramatic decline in mental capabilities, which is marked by diminished concentration and consciousness. Aim: The purpose of this study was to assess the views, knowledge, and behavior of non-consultant hospital doctors in managing delirium in a large Irish hospital. Methods: Questionnaires were administered to 28 healthcare professionals from various departments according to Davis and MacLullich (Age Ageing 38(5):559-563, 2009). It was conducted between July and September 2023, with an emphasis on determining the prevalence rate, diagnostic criteria, and management strategies for delirium. Results: The study established that the majority of respondents recognized the importance of delirium, but there appears to be a gap in the practical management of this clinical syndrome. Although many doctors agreed that delirium was significant, most lacked confidence in its diagnosis and management. The use of standardized assessment tools, such as the 4AT, was limited. Conclusions: This study highlights the disparity between what is known and practiced by hospital doctors regarding delirium care. This implies increased training for delirium management with frequent use of assessment tools and ongoing education aimed at enhancing patient outcomes in cases of delirium.

About 2–5% of older people world-wide live in nursing homes and are characterized by functional impairment, cognitive decline, dementia, comorbidities, and polypharmacotherapy, all factors which influence the development of delirium. Abstract:Objectives Delirium is common in older hospitalised patients and is associated with significant distress in family members who witness it. Education‐based interventions can support the family of people with delirium. This study aimed to understand the perspective of family members of people with delirium regarding the information they received and opportunities for improvement. Methods We performed 10 semi‐structured interviews of 11 family members (one mother–daughter dyad) of people with delirium greater than 65 years of age across two sub‐acute Geriatric Evaluation and Management units in Victoria, Australia. Interviews were inductively analysed by the lead investigator to identify themes related to the information provided and opportunities for improvement. Results We identified three themes related to delirium information provision: (1) delirium knowledge is not common; (2) information should consider the broad complex of the person; and (3) lived experience of delirium was helpful. We identified five themes with regard to opportunities to improve delirium information provision: (1) Good communication skills are important; (2) Regular communication is helpful; (3) Family wish to be present and know how to be involved; (4) Information provision should include prognosis; (5) Written information can support verbal information. Conclusions Information provision to family members should take into account their existing knowledge and experience and consider their family member as a whole. High quality, regular, written and oral communication from health‐care professionals is likely to prove helpful.

Abstract:Delirium is a serious consequence of many acute or worsening chronic medical conditions, a side effect of medications, and a precipitant of worsening functional and cognitive status in older adults. It is a syndrome characterized by fluctuations in cognition and impaired attention that develops over a short period of time in response to an underlying medical condition, a substance (prescribed, over the counter, or recreational), or substance withdrawal and can be multi-factorial. We present a narrative review of the literature on nonpharmacologic and pharmacologic approaches to prevention and treatment of delirium with a focus on older adults as a vulnerable population. Older adult patients are most at risk due to decreasing physiologic reserves, with delirium rates of up to 80% in critical care settings. Presentation of delirium can be hyperactive, hypoactive, or mixed, making identification and study challenging as patients with hypoactive delirium are less likely to come to attention in an inpatient or long-term care setting. Studies of delirium focus on prevention and treatment with nonpharmacological or medication interventions, with the preponderance of evidence favoring multi-component nonpharmacological approaches to prevention as the most effective. Though use of antipsychotic medication in delirium is common, existing evidence does not support routine use, showing no clear benefit in clinically significant outcome measures and with evidence of harm in some studies. We therefore suggest that antipsychotics be used to treat agitation, psychosis, and distress associated with delirium at the lowest effective doses and shortest possible duration and not be considered a treatment of delirium itself. Future studies may clarify the use of other agents, such as melatonin and melatonin receptor agonists, alpha-2 receptor agonists, and anti-epileptics.

Abstract:Background Many patients in the Intensive Care Unit (ICU) experience delirium. Understanding the patient perspective of delirium is important to improve care and reduce suffering. The aim of our study was to investigate the subjective patient experience of delirium, delirium‐related distress, and delirium management in ICU. Methods Our study had a qualitative multicenter design applying individual interviews and thematic analysis. Participants were critically ill adult patients that were determined delirium positive according to validated delirium screening tools during ICU admission. The interviews were conducted after ICU discharge when patients were delirium‐free as assessed by the "Rapid clinical test for delirium" (4AT) and able to participate in an interview. Results We interviewed 30 patients choosing the main themes deductively: Delirium experience; Delirium related distress; and Delirium management. Despite variations in recollection detail, ICU survivors consistently reported delirium‐related distress during and after their ICU stay, manifesting as temporal confusion, misinterpretations, and a sense of distrust towards ICU staff. Delusions were characterized by a blend of factual and fictional elements. Impaired short‐term memory hindered communication and intensified feelings of isolation, neglect, and loss of control. Conclusion The ICU survivors in our study recalled delirium as an unpleasant and frightening experience, often leading to delirium‐related distress during and after their ICU stay, indicating the necessity for enhanced assessment and treatment.

The association of dementia and delirium with health outcomes in a sample of older inpatients in a New Zealand hospital setting

Abstract:Background Both dementia and delirium are seen frequently in older hospital inpatients. Dementia is present in up to one-third of older hospital inpatients, and delirium also occurs in around one-third of older inpatients, often alongside dementia. Patients’ health outcomes, such as mortality, length of stay, and discharge to a care home, may be worse in patients with dementia and/or delirium. Objective(s) To describe the association of dementia and delirium with health outcomes (length of stay, mortality, care home placement) in a sample of patients from a New Zealand hospital setting. Methods Routinely collected data from a consecutive sample of 2248 older patients assessed in a New Zealand memory service from 2013 to 2021 were extracted to examine the associations of delirium and dementia with patients’ health outcomes: length of stay, care home placement, and mortality. Results Of the 2248 patients assessed, 75% were hospitalised after diagnosis, and half of these had delirium screening in hospital. People with dementia had three times the risk of experiencing delirium (OR=3.0, 95% CI:2.0-4.6, p<0.001). In Pacific people compared to NZ Europeans, the adjusted relative risk ratios for having dementia only or delirium superimposed on dementia, compared to those with neither, were 2.3 (p<0.001) and 2.9 (p<0.001), respectively. Compared to people with neither diagnosis, people with delirium (with or without dementia) had a longer mean length of stay, were two to four times more likely to be placed in a care home, and the risk of death was 1.7-3.1 times higher. Conclusion Delirium is very common in older hospital inpatients and has a major impact on patient outcomes and health costs. Pacific peoples seem to have greater risk of both dementia and delirium, associated with worse clinical outcomes.

Abstract:Background Delirium is a common and serious neuropsychiatric disorder. The prognosis of delirium in older patients living at home has not been studied often before. The aim of this study is to examine the prognosis of delirium in patients attending a memory clinic of a psychiatric hospital. Method The study population consisted of 85 outpatients diagnosed with delirium between October 2013 and October 2014. Seventeen patients had already been diagnosed as having dementia. Three months after the diagnosis, consenting patients underwent a follow‐up visit. We recorded delirium status (remitted or not), new dementia diagnosis, subjective cognitive functioning compared to baseline and to before delirium, level of daily functioning, and place of residence. Results After 3 months, 45 (53%) had recovered from delirium, 19 (22,4%) had persistent/recurrent delirium, 12 (14%) patients had died, and another nine (11%) could not be revisited for other reasons than death. None of the 64 re‐examined patients reported that their cognitive functioning had recovered to the pre‐delirium level, and the mean level of daily functioning did not substantially improve either. The rate of diagnosed dementia increased to 63.8%, and 18 patients (28.1%) had moved to a nursing home. Conclusions Delirium in older outpatients has a poor prognosis. A larger study on the risk factors of the prognosis of delirium in older persons living at home is advised.

Abstract:Abstract Delirium, a form of acute brain dysfunction, is very common in the critically ill adult patient population. Although its pathophysiology is poorly understood, multiple factors associated with delirium have been identified, many of which are coincident with critical illness. To date, no drug or non-drug treatments have been shown to improve outcomes in patients with delirium. Clinical trials have provided a limited understanding of the contributions of multiple triggers and processes of intensive care unit (ICU) acquired delirium, making identification of therapies difficult. Delirium is independently associated with poor long term outcomes, including persistent cognitive impairment. A longer duration of delirium is associated with worse long term cognition after adjustment for age, education, pre-existing cognitive function, severity of illness, and exposure to sedatives. Interestingly, differences in prevalence are seen between ICU survivor populations, with survivors of acute respiratory distress syndrome experiencing higher rates of cognitive impairment at early follow-up compared with mixed ICU survivor populations. Although cognitive performance improves over time for some ICU survivors, impairment is persistent in others. Studies have so far been unable to identify patients at higher risk of long term cognitive impairment; this is an active area of scientific investigation.

Abstract:Abstract Background According to the ‘Second Irish National Audit of Dementia Care in Acute Hospitals (INAD-2), it is estimated that 20% of all adults admitted to hospital have a diagnosis of delirium during admission. Local HSE guidelines advise that all older adults (>65 years) should have a formalised delirium screening tool completed within the first 24 hours of presenting to the Emergency Department (ED). Methods We reviewed the medical admission documentation and subsequent medical notes in the charts of current inpatients on three Geriatric Medicine wards in the hospital. Data was collected from 56 patients over the age of 65 over a two day period. Results Data was collected from 56 patients. 21.4% (n = 12) had a delirium screening tool documented. Of those, 100% used 4AT testing and 100% were completed by the OPAL team (Older Person Assessment and Liaison Team)/ED Frailty Intervention Team. 5.4% (n = 3) met diagnostic criteria for delirium. 21.4% (n = 12) had delirium documented as a diagnosis on their medical admission, of which only 3 had delirium screening completed. 26.7% (n = 15) had a documented pre-existing diagnoses of dementia. Of those, 2 had a formal delirium screen documented. 25% (n = 14) were diagnosed with delirium later in their admission. Of those, 3 had a delirium screening tool completed at any point. The vocabulary used to describe delirium was also examined. Most commonly the words confusion (10) and agitated (4) were used instead of delirium. 4 had delirium subcategorised as hypoactive. Conclusion Delirium continues to be prevalent among our older inpatients and has been shown to be directly related to increased mortality. It is vital that we make use of the tools that are available to us to identify those at risk and those already suffering from this condition. We should prioritise appropriate delirium screening with standardised assessment tools in order to correctly identify those at risk.

Abstract:Abstract In Brazil there is scarce data about the occurrence of delirium among hospitalized elderly patients. Objective: This study aimed to evaluate the prevalence of delirium among elderly patients hospitalized in clinical wards. Methods: This cross-sectional study examined a sample of elderly inpatients admitted to three clinical wards of a general hospital between July 2011 and May 2012. The presence of delirium was detected by applying the Confusion Assessment Method (CAM). Dementia diagnosis was conducted in two steps: screening and diagnosis (Cambridge Examination, CAMDEX, was applied during hospitalization at a second timepoint). Other medical diagnoses and medications in use were extracted from medical records. Results: A sample of 173 elderly inpatients was examined; mean age 71.2 years (SD: 7.8; 60-92 years); 64.2% male. Thirty-one patients were diagnosed with delirium; prevalence of 17.9% (95% CI: 12.2-23.6). Delirium was directly associated with Urinary Tract Infection, Renal Failure and Dementia (p<0.05). Conclusion: The principal findings of this study were a high prevalence of delirium and the identification of associated factors, helping to guide preventive approaches and clinical management for at-risk patients in a Brazilian sample.
