Abstract:BACKGROUND: From 2010 through 2013, integrated health and environmental responses addressed an unprecedented epidemic lead poisoning in Zamfara State, northern Nigeria. Artisanal gold mining caused widespread contamination resulting in the deaths of > 400 children. Socioeconomic, logistic, and security challenges required remediation and medical protocols within the context of local resources, labor practices, and cultural traditions.OBJECTIVES: Our aim was to implement emergency environmental remediation to abate exposures to 17,000 lead poisoned villagers, to facilitate chelation treatment of children ≤ 5 years old, and to establish local technical capacity and lead health advocacy programs to prevent future disasters.METHODS: U.S. hazardous waste removal protocols were modified to accommodate local agricultural practices. Remediation was conducted over 4 years in three phases, progressing from an emergency response by international personnel to comprehensive cleanup funded and accomplished by the Nigerian government.RESULTS: More than 27,000 m3 of contaminated soils and mining waste were removed from 820 residences and ore processing areas in eight villages, largely by hand labor, and disposed in constructed landfills. Excavated areas were capped with clean soils (≤ 25 mg/kg lead), decreasing soil lead concentrations by 89%, and 2,349 children received chelation treatment. Pre-chelation geometric mean blood lead levels for children ≤ 5 years old decreased from 149 μg/dL to 15 μg/dL over the 4-year remedial program.CONCLUSIONS: The unprecedented outbreak and response demonstrate that, given sufficient political will and modest investment, the world's most challenging environmental health crises can be addressed by adapting proven response protocols to the capabilities of host countries.CITATION: Tirima S, Bartrem C, von Lindern I, von Braun M, Lind D, Anka SM, Abdullahi A. 2016. Prevalence and determinants of evidence of silicosis and impaired lung function among small scale tanzanite miners and the peri-mining community in northern Tanzania

Abstract:Limited data among miners in Tanzania suggests prevalence of silicosis, obstructive lung disease and restrictive lung disease to be around 1.6%, 1.9% and 8.8% respectively. Our study aimed to determine the prevalence and factors associated with silicosis and impaired lung function among tanzanite mining community in northern Tanzania. We conducted a cross-sectional study, involving 330 miners and 330 peri-mining community members in Mererani mines. Silicosis was defined based on study participants' history of exposure to mining dust and digital chest radiological findings with reference to the 2011 ILO classification of pneumoconiosis. Impaired lung function was determined by spirometry using American Thoracic Society/European Respiratory Society recommended system 3. Association between evidence of silicosis/impaired lung function and presumed risk factors were determined using binary logistic regression analyses. The study found that 99/330 (30.0%) of miners had silicosis. Total of 65 (9.8%) participants had impaired lung function, of whom 29 (4.4%) had Chronic Obstructive Pulmonary Disease, 32 (4.8%) had restrictive lung disease and 4 (0.6%) had both obstructive and restrictive lung diseases. Unexpectedly, miners who have worked for more than 10years and those worked for 6 to 10 years had 64% (aOR 0.34, CI = 0.17-0.67, p = 0.002) and 48% (aOR 0.52, CI = 0.30-0.89, p = 0.018) lower odds of having silicosis respectively compared those worked for up to 5 years. Participants with more than 10 years of work duration had more than 3-times higher odds of impaired lung function compared to those who had worked for up to 5 years (aOR 3.11, CI = 1.53-6.34, p<0.002). We found a concerningly high prevalence of silicosis despite short durations of exposure to occupational silica dust. Immediate dust control measures including deployment of wet drilling, wearing of personal protective equipment and regular monitoring of dust exposure need to be enforced by the Occupational Safety and Health Authority-Tanzania.

Abstract:Artisanal and small-scale miners (ASMs) labour under archaic working conditions and are exposed to high levels of silica dust. Exposure to silica dust has been associated with an increased risk of tuberculosis and silicosis. ASMs are highly mobile and operate in remote areas with near absent access to health services. The main purpose of this study was to evaluate the prevalence of tuberculosis, silicosis and silico-tuberculosis among ASMs in Zimbabwe. A cross-sectional study was conducted from 1 October to 31 January 2021 on a convenient sample of 514 self-selected ASMs. We report the results from among those ASMs who attended an outreach medical facility and an occupational health clinic. Data were collected from clinical records using a precoded data proforma. Data variables included demographic (age, sex), clinical details (HIV status, GeneXpert results, outcomes of chest radiographs, history of tuberculosis) and perceived exposure to mine dust. Of the 464 miners screened for silicosis, 52 (11.2%) were diagnosed with silicosis, while 17 (4.0%) of 422 ASMs were diagnosed with tuberculosis (TB). Of the 373 ASMs tested for HIV, 90 (23.5%) were sero-positive. An HIV infection was associated with a diagnosis of silicosis. There is need for a comprehensive occupational health service package, including TB and silicosis surveillance, for ASMs in Zimbabwe. These are preliminary and limited findings, needing confirmation by more comprehensive studies.

Abstract:Artisanal and small-scale mining is characterized by excessive exposure to physical, chemical, ergonomic, psychosocial and biological hazards. There is a high burden of tuberculosis (TB), human immunodeficiency virus (HIV) infections and silicosis among artisanal and small-scale miners (ASMs). The aim of this project report is to describe lessons learned from strategies implemented to reach ASMs with screening services for TB, HIV and silicosis in Zimbabwe through the Kunda-Nqob'i TB (KNTB) project supported by the United States Agency for International Development (USAID). The intervention package for screening ASMs for TB, HIV and silicosis included service provision through two occupational health clinics at two provincial hospitals and a mobile workplace-based screening (WBS) facility at the mining sites. From 1 October 2020 to 30 September 2023, 10,668 ASMs were screened, with a high number of cases of silicosis (21%) and TB (7.4%). There was a high burden of HIV (30%) in ASMs attending the occupational health clinics. The two occupational health clinics screened 3453 ASMs, while the mobile WBS activities screened 7215 ASMs during the period. A total of 370 healthcare workers (doctors/clinical officers, nurses, environmental health technicians and district tuberculosis and Leprosy control officers) were trained on TB and the fundamental diagnostic principles of silicosis. The KNTB project has been successful in reaching out to many ASMs operating in remote and hard-to-reach mining areas. The KNTB project has brought to light the positive health-seeking behavior of ASMs operating in remote areas. The project has brought to the fore the effectiveness of multi-stakeholder engagement and collaboration in reaching out to ASMs in remote areas with health screening services. There is a high burden of TB, HIV and silicosis in ASMs. Screening for TB, HIV and silicosis using workplace-based screening and occupational health clinics is an effective strategy and should be rolled out to all areas with high artisanal and small-scale mining activity.

Abstract:An estimated 44 million artisanal and small-scale miners (ASM), largely based in developing economies, face significant occupational risks for respiratory diseases which have not been reviewed. We therefore aimed to review studies that describe silicosis and tuberculosis prevalence and respirable crystalline silica (RCS) exposures among ASM and use background evidence to better understand the relationship between exposures and disease outcomes. We searched PubMed, Web of Science, Scopus and Embase for studies published before the 24 th March 2023. Our primary outcome of interest was silicosis or tuberculosis among ASM. Secondary outcomes included measurements of respirable dust or silica, spirometry and prevalence of respiratory symptoms. A systematic review and narrative synthesis was performed and risk of bias assessed using the Joanna Briggs Prevalence Critical Appraisal Tool. Logistic and Poisson regression models with predefined parameters were used to estimate silicosis prevalence and tuberculosis incidence at different distributions of cumulative silica exposure. We identified 18 eligible studies that included 29,562 miners from 13 distinct populations in 10 countries. Silicosis prevalence ranged from 11 to 37%, despite four of five studies reporting an average median duration of mining of <6 years. Tuberculosis prevalence was high; microbiologically confirmed disease ranged from 1.8 to 6.1% and clinical disease 3.0 to 17%. Average RCS intensity was very high (range 0.19–89.5 mg/m 3 ) and respiratory symptoms were common. Our modelling demonstrated decreases in cumulative RCS are associated with reductions in silicosis and tuberculosis, with greater reductions at higher mean exposures. Despite potential selection and measurement bias, prevalence of silicosis and tuberculosis were high in the studies identified in this review. Our modelling demonstrated the greatest respiratory health benefits of reducing RCS are in those with highest exposures. ASM face a high occupational respiratory disease burden which can be reduced by low-cost and effective reductions in RCS.
Personal respirable dust and respirable crystalline silica exposure in two shafts and a concentrator of a Zambian copper mine

Abstract:Abstract Objectives Since the 1920s, Zambia’s mining sector has experienced growth, which has increased the number of mine workers employed in the industry. Consequently, the potential for occupational exposure and prevalence of occupational diseases have also increased. Unfortunately, Zambia does not currently have legislative guidelines for workplace air monitoring and compliance. This study’s objectives were to evaluate copper miners’ personal exposure to respirable dust and respirable crystalline silica (RCS) and to assess workplace compliance using the European Standard for workplace air monitoring and measurement (EN689:2018). Methods This cross-sectional study collected 100 personal respirable dust exposure samples at a Zambian copper mine in 2023. These samples were weighed using NIOSH method 0600 and analyzed for crystalline silica using Fourier transform infrared spectroscopy (KBr pellet) (NIOSH method 7602). Additionally, 253 respirable dust exposure measurements collected at the mine between 2017 and 2022 were included for comparison. Results The median respirable dust exposure for the 2023 exposure measurements was 0.200 mg/m3 (95th percentile 2.871 mg/m3) compared to 0.400 mg/m3 (95th percentile 3.050 mg/m3) for the historic data. The median RCS exposure was 0.012 mg/m3 (95th percentile 0.163 mg/m3). Using EN689:2018, it was found that from 15 work areas, only six work areas complied with the standard for respirable dust exposure and only seven work areas complied with the standard for RCS exposure. Conclusions At the mining site, several work areas had substantial exposure to respirable dust and RCS. Therefore, management needs to prioritize these areas when implementing control measures to reduce dust exposure. For the Zambia mining industry to manage exposure to respirable dust and RCS, it is necessary to implement standardized monitoring strategies. This study has demonstrated that EN689:2018 can be used successfully to determine compliance among Zambian mining work areas.

Abstract:Background. Underground miners can experience occupational health diseases due to exposure to particulate matter hazards. Objectives. The aim of the present study was to examine occupational exposures of underground miners to dust and diesel particulate matter and to identify exposure groups with high potential to develop associated health effects due to the presence of dust and diesel particulate matter (DPM) hazards in an underground gold mine in Ghana. Methods. Purposive sampling was employed using gravimetric air samplers over an 8-hour time weighted average period. The National Institute for Occupational Safety and Health (NIOSH) analytical Chapter Q and 5040 were used in determining crystalline silica dust and diesel particulate matter fractions, respectively. Structured questionnaires were administered to gather data on workers' level of awareness to dust and DPM exposures. Results. It was found that 41% of the sampled groups were exposed to higher crystalline silica levels above the (NIOSH) permissible exposure limit (PEL) level of 0.05 mg/m 3 . For DPM, 49% of these groups had exposures above the Mine Safety and Health Administration (MSHA) PEL level of 160 μg/m 3 . Among the 94 mine workers who responded to this study, 62% were found to be aware of the presence and hazardous nature of silica dust, 28% had minimal knowledge and the remaining were found to be unaware. Conclusions. There are varying levels of dust and DPM due to the presence of silica-bearing rocks, the production of diesel fumes and inefficiencies of available mitigation measures. Research carried out over the past decades has found confirmed cases of silicosis and lung cancer due to high dust exposure levels. Rock drillers, blast men and shotcrete operators were found to be exposed to higher levels of dust and diesel particulate matter and are at greater risk of silicosis. Participant Consent. Abstract:Background: The 2010 lead poisoning outbreak that claimed the lives of more than 400 children in artisanal gold mining villages in Zamfara, Nigeria is the tragic result of high gold prices, a geologic anomaly, and processing of ores in residential areas. Today, these villages face a growing crisis related to conflict and climate change. While the situation in Zamfara is unparalleled in many ways, the interactions between climate change, conflict, and mining consistently overlap a global scale. The scope of this analysis extends beyond the Nigerian crisis.Objectives: Understanding the complexities of challenges faced in Zamfara provides insight into how these issues impact vulnerable communities globally, and which strategies should be considered to solve this wicked problem.Methods: Analysis of the relationships between climate change, conflict, and mining in Zamfara and globally via literature review and examination of current events in the Sahel region.Findings: Supporting healthy artisanal mining communities, as was prioritized in Zamfara, must be a focus of environmental, health, and mineral management policies. This includes the consideration of multiple environmental health challenges, the protection of vulnerable groups, government-supported formalization programs, and meaningful involvement of local leadership in developing, implementing, and sustaining intervention strategies to enshrine ASM as a poverty reduction, climate change adaptation strategy.Conclusions: Rapidly rising metal prices and demand will continue to fuel environmental health crises associated with mining. Given Africa's growing role in the global mineral economy and the massive number of subsistence communities who will continue to be impacted by climate change, strategies that support responsible artisanal mining are both a necessity for preventing future health crises and an opportunity for promoting regional stability and peace.

Analysis of respirable dust exposure data collected at a Zambian copper mine between 2017 and 2022

Abstract:Mine workers are occupationally exposed to respirable dust, which can cause irreversible lung diseases and controlling exposure concentrations to as low as reasonably practicable is, therefore, essential. To implement exposure reduction strategies and adequately manage exposure to hazardous chemicals, exposure needs to be measured and recorded according to a standard exposure management plan. This study aimed to assess the available respirable dust exposure data measured between 2017 and 2022 in various work areas and job categories at two mining shafts and a concentrator plant of a Zambian copper mine. Additionally, the monitoring program implemented at the mine was assessed for appropriateness. Descriptive data analysis was used to create an exposure matrix comprising 253 exposure measurements. Approximately 5.6% of the measured concentrations exceeded the South African time-weighted average occupational exposure limit (TWA-OEL) of 3 mg/m 3 . The geometric means of respirable dust exposure for shaft A, shaft B and the concentrator plant were 0.44 mg/m 3 , 0.44 mg/m 3 , and 0.68 mg/m 3 , respectively. The work areas with the highest maximum exposure results were the tipping area (18.0 mg/m 3 ) at shaft A, the sump and waste bin (8.05 mg/m 3 ) at shaft B and the screening (10.8 mg/m 3 ), secondary crusher (14.0 mg/m 3 ), foreign ore bin (4.43 mg/m 3 ), and tertiary crusher (5.07 mg/m 3 ) areas at the concentrator plant. It was found that the monitoring strategy implemented at the mine was flawed and did not collect a sufficient number of samples in each area during each year to make rigorous statistical assessment possible. This study highlights the sampling strategy's shortcomings and recommends adopting a standardized monitoring strategy, such as EN689, to improve the respirable dust monitoring program at the mine. Additionally, this strategy can be implemented across Zambia and, if implemented correctly, it has the potential to be improve exposure monitoring across the country as no standard sampling strategy is currently enforced by the Government.

Respiratory Health in a Community Living in Close Proximity to Gold Mine Waste Dumps, Johannesburg, South Africa

Abstract:The effects on respiratory health in populations living close to silica-rich gold mine dumps are unknown. This pilot study related respiratory health and exposure to mine dump dust using two measures of exposure: exposure group, based on distance lived from the mine dump-high (n = 93) (home <500 m from a mine dump), moderate (n = 133) (500-1.5 km), and low (n = 84) (>15 km, control group); and cumulative exposure index (CEI) derived from exposure group and number of years of residence in each exposure group. Participants were interviewed about respiratory symptoms and had chest X-rays and spirometry. We adjusted for key respiratory confounders. No subject had radiological features of silicosis. The high relative to low exposure group had significantly elevated adjusted odds ratios (aORs) for upper respiratory symptoms (aOR: 2.76, 95% CI: 1.28-5.97), chest wheezing (aOR: 3.78; 95% CI: 1.60-8.96), and spirometry-diagnosed chronic obstructive pulmonary disease (COPD) (aOR: 8.17; 95%CI: 1.01-65.85). These findings were similar for the high relative to medium exposure group, but no significant associations were found for the medium versus low exposure group. Chronic bronchitis and tuberculosis risks did not differ significantly among groups. CEI and exposure group produced similar results. In conclusion, residents residing <500 m from mine dumps had elevated adverse respiratory health effects.

Abstract:Abstract There is no Occupational Health and Safety (OHS) system for the mining sector in Cameroon, and miners are exposed to numerous health and safety risks inherent from Artisanal Gold Mining (AGM) activity. Also very few studies have addressed the health problems in AGM in Cameroon. This study seeks to create an awareness of the health and environmental issues surrounding the mining sector in Cameroon so as to foster its development. It was a cross-sectional, observational, and communitybased study involving 273 respondents from three different communities. Questionnaires and interviews were used to collect data. OHS risk assessment was conducted through direct observation and inspection of five different workplaces. Blood samples were collected and analyzed for the presence of mercury and lead. Gold miners in Batouri are exposed to the risk of physical injuries, respiratory conditions, diarrheal conditions, psychotic disorders, dermatophyte infections, helminthic infestations, malaria and musculoskeletal problems. The most common health problems among miners are musculoskeletal disorders (35.6%), malaria (26.4%) and hernias (14.9%), while malaria (18.7%) and musculoskeletal disorders (15.4%) are common among stakeholders. The mean blood mercury and lead levels among miners is 2.27 ± 8.85 μg/L and 12.73 ± 32.73 μg/L respectively, and 9.1% of them are chronically intoxicated with mercury, reporting ≥ 4 symptoms possibly related to mercury intoxication. The major environmental problems are deforestation, land degradation, water pollution, air pollution and water-logged pits. AGM in Batouri, therefore, has a negative impact on human health and environmental sustainability.
A New Long-Term Care Facilities Model in Nova Scotia, Canada: Protocol for a Mixed Methods Study of Care by Design

Abstract:Background Prior to the implementation of a new model of care in long-term care facilities in the Capital District Health Authority, Halifax, Nova Scotia, residents entering long-term care were responsible for finding their own family physician. As a result, care was provided by many family physicians responsible for a few residents leading to care coordination and continuity challenges. In 2009, Capital District Health Authority (CDHA) implemented a new model of long-term care called “Care by Design” which includes: a dedicated family physician per floor, 24/7 on-call physician coverage, implementation of a standardized geriatric assessment tool, and an interdisciplinary team approach to care. In addition, a new Emergency Health Services program was implemented shortly after, in which specially trained paramedics dedicated to long-term care responses are able to address urgent care needs. These changes were implemented to improve primary and emergency care for vulnerable residents. Here we describe a comprehensive mixed methods research study designed to assess the impact of these programs on care delivery and resident outcomes. The results of this research will be important to guide primary care policy for long-term care. Objective We aim to evaluate the impact of introducing a new model of a dedicated primary care physician and team approach to long-term care facilities in the CDHA using a mixed methods approach. As a mixed methods study, the quantitative and qualitative data findings will inform each other. Quantitatively we will measure a number of indicators of care in CDHA long-term care facilities pre and post-implementation of the new model. In the qualitative phase of the study we will explore the experience under the new model from the perspectives of stakeholders including family doctors, nurses, administration and staff as well as residents and family members. The proposed mixed method study seeks to evaluate and make policy recommendations related to primary care in long-term care facilities with a focus on end-of-life care and dementia. Methods This is a mixed methods study with concurrent quantitative and qualitative phases. In the quantitative phase, a retrospective time series study is being conducted. Planned analyses will measure indicators of clinical, system, and health outcomes across three time periods and assess the effect of Care by Design as a whole and its component parts. The qualitative methods explore the experiences of stakeholders (ie, physicians, nurses, paramedics, care assistants, administrators, residents, and family members) through focus groups and in depth individual interviews. Results Data collection will be completed in fall 2013. Conclusions This study will generate a considerable amount of outcome data with applications for care providers, health care systems, and applications for program evaluation and quality improvement. Using the mixed methods design, this study will provide important results for stakeholders, as well as other health systems considering similar programs. In addition, this study will advance methods used to research new multifaceted interdisciplinary health delivery models using multiple and varied data sources and contribute to the discussion on evidence based health policy and program development.
Respiratory health and silicosis in artisanal mine workers in southern Brazil

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Artisanal and small‐scale mining (ASM) are an important source of employment in southern Brazil. Mining workers are frequently exposed to unhealthy work conditions which increase the risk of occupational diseases. In this study, we assessed the association of sociodemographic factors and the occupational history of artisanal mining workers with the risk of adverse respiratory outcomes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The study was conducted with 258 artisanal mining workers in southern Brazil, who were exposed to dust (mainly crystalline silica) in their work. Information on sociodemographic variables and occupational histories was collected between 2017 and 2018. To estimate the worker's exposure to inhalable dust we use the Advanced REACH (Registration, Evaluation, and Authorization of Chemicals) Tool (ART).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Study participants were all men, with an average age of 40 years. Median crude dust exposure estimated by ART was 13.2 mg/m³ and median crude crystalline silica exposure was 1.6 mg/m<sup>3</sup>. The prevalence ratio (PR) for self‐reported silicosis was 3.08 (95% confidence interval, 1.39–7.17) in workers with 20 years or more of mining work. Factors associated with silicosis were age, pack‐years of tobacco use, and body mass index. Smokers were over twice as likely to report respiratory symptoms. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Our findings indicate that a high prevalence of silicosis and other associated diseases in mining workers is associated with both unhealthy work environment conditions and the health profile of workers. This study is an important step for understanding health outcomes from work in ASM.</p></section>

Abstract:Background: Workers in the informal economy often incur exposure to well-documented occupational health hazards. Insufficient attention has been afforded to rigorously evaluating intervention programs to reduce the risks, especially in artisanal and small-scale gold mining (ASGM).Objectives: This systematic review, conducted as part of the World Health Organization's Global Plan of Action for Workers' Health, sought to assess the state of knowledge on occupational health programs and interventions for the informal artisanal and small-scale gold mining (ASGM) sector, an occupation which directly employs at least 50 million people.Methods: We used a comprehensive search strategy for four well-known databases relevant to health outcomes: PubMed, Engineering Village, OVID Medline, and Web of Science, and employed the PRISMA framework for our analysis.Findings: Ten studies met the inclusion criteria of a primary study focused on assessing the impact of interventions addressing occupational health concerns in ASGM. There were no studies evaluating or even identifying comprehensive occupational health and safety <i>programs</i> for this sector although target interventions addressing specific hazards exist. Major areas of intervention-education and introduction of mercury-reducing/eliminating technology were identified, and the challenges and limitations of each intervention taken into assessment. Even for these, however, there was a lack of standardization for measuring outcome or impact let alone long-term health outcomes for miners and mining communities.Conclusion: There is an urgent need for research on comprehensive occupational health programs addressing the array of hazards faced by artisanal and small-scale miners.

Abstract:Background. Respirable dust, diesel particulate matter, crystalline silica and noise pollution are the most common causes of health issues experienced by underground mine workers. Assessment of exposure levels in relation to standard regulatory body permissible levels is essential for the safety of mine workers. Objectives. The present study compared exposure levels of diesel particulate matter, crystalline silica dust and noise experienced across different underground mine worker job titles. Methods. Subjective sampling was employed using gravimetric air samplers over an 8-hour time weighted average for two periods designated as period 1 (first half of the year) and period 2 (second half of the year). A comparative analysis of exposure levels between job titles and in relation to the National Institute for Occupational Safety and Health (NIOSH) permissible exposure levels (PELs) was performed. Results. In the present study, 90% of the selected job titles were over-exposed to noise and 80% were over-exposed to diesel particulate matter. The highest exposures for crystalline silica dust and diesel particulate matter were found in the 40–49-year-old age group. Conclusions. The present study of exposure levels of diesel particulate matter, respirable dust, crystalline silica, and noise during underground gold mining demonstrates that better control mechanisms are needed to protect workers. Abstract:Artisanal and small-scale gold mining (ASGM) poses a significant global threat due to mercury emissions and resulting health hazards. This study focuses on assessing these risks in the Abu Hamad ASGM community in Sudan. Utilizing the Mercury Analyzer 3000 (NIC), analyses of twelve soil samples (including one tailings sample) and seven water samples revealed the highest concentrations near amalgam burning locations: 34.8 mg/kg in soil (S06) and 3.26 μg/L in water (W03). Concentrations decrease with distance, with soil near burning exceeding tailings (S05 = 19.0 mg/kg). Hazard quotients indicate mercury vapor inhalation as the primary exposure route from soil, with the Hazard Index reaching 5.34 for adults and 33.4 for children close to amalgam burning sites. Water samples generally pose little risk except for W03, where children face potential danger via ingestion (HI = 1.74). These findings emphasize the urgent need for adopting retorts and eco-friendly practices to reduce mercury emissions and protect ASGM communities.

Abstract:Mercury contamination stemming from artisanal and small-scale gold mining (ASGM) operations poses significant environmental and health concerns. This study focuses on the Darmali area in River Nile State, Sudan, where the reprocessing of amalgamation tailings has led to mercury contamination. This study assessed the mercury content in soil and tailings samples, as well as in tap and groundwater, to evaluate the human health risks from ASGM activities and assess contamination levels within the study area. Soil and water samples were collected from various locations, including agricultural, residential, and tailings sites, as well as groundwater and tap water from the Nile. Mercury analysis was conducted using MA-3000 (NIC), and geo-accumulation index analysis revealed extreme pollution levels in areas with tailings and moderate pollution levels in agricultural and residential areas. Hazard quotients were applied to assess health risks, with inhalation of mercury vapor identified as the primary exposure route. The results indicated that tailings pose significant health risks, particularly for children, while water samples and soil from agricultural and residential areas did not pose significant risks. These findings underscore the urgent need for authorities and local communities to address mercury contamination by removing and treating tailings from affected areas to mitigate health risks.

Abstract:The 2021 tuberculosis (TB) preventive treatment guidelines in India included silicosis as a screening group, yet latent TB infection (LTBI) testing for silica-dust-exposed individuals is underemphasized. Focusing on an estimated 52 million silica-dust-exposed workers, particularly agate-stone workers in Khambhat, Gujarat, our study aims to estimate LTBI prevalence, identify predictors, and gather insights from TB and silicosis experts. Employing a sequential explanatory mixed-methods approach, a cross-sectional study involved 463 agate-stone workers aged ≥ 20 years in Khambhat, using IGRA kits for LTBI testing. In-depth interviews with experts complemented quantitative findings. Among agate-stone workers, 58% tested positive for LTBI, with predictors including longer exposure, type of work, and BCG vaccination. Our findings reveal a nearly double burden of LTBI compared to the general population, particularly in occupations with higher silica dust exposure. Experts advocate for including silica-dust-exposed individuals in high-risk groups for LTBI testing, exploring cost-effective alternatives like improved skin sensitivity tests, and shorter TB preventive treatment regimens to enhance compliance. Future research should explore upfront TB preventive treatment for silica-dust-exposed individuals with high LTBI prevalence and optimal exposure duration. This study underscores the urgent need for policy changes and innovative approaches to TB prevention among silica-dust-exposed populations, impacting global occupational health strategies.

Abstract:This research quantifies the health risks associated with exposure to heavy metals in the Matjhabeng Local Municipality, a gold mining region in South Africa, utilising a deterministic source–pathway–receptor approach. This study uniquely integrates both non-carcinogenic and carcinogenic risk assessments across multiple heavy metals, providing a comprehensive perspective on health impacts in mining-impacted communities. The study measured concentrations of arsenic (As), cadmium (Cd), lead (Pb), cobalt (Co), chromium (Cr), copper (Cu), iron (Fe), nickel (Ni), selenium (Se), and zinc (Zn) in soil samples, with mean values found to be 3.2 mg/kg, 2.5 mg/kg, 45 mg/kg, 17 mg/kg, and 25 mg/kg for As, Cd, Pb, Co, and Cr, respectively. The deterministic assessment revealed minimal non-carcinogenic risks for ingestion across all demographics (HQ 1), particularly for the adult group. The inhalation pathway emerged as a critical exposure route, with HQ values ranging from 5 to 15 and chronic hazard index values significantly exceeding safe limits (CHI > 5). The carcinogenic risk through inhalation notably surpassed the acceptable thresholds set by the United States Environmental Protection Agency (1 × 10−4 to 1 × 10−6), with a calculated lifetime cancer risk far exceeding the limit for As, Cd, Cr, Co, and Ni for both adults and juveniles. These findings underscore the urgent need for targeted risk mitigation strategies in the community to address the significant health risks posed by airborne heavy metal exposure.
