Abstract:Objective: maternal mortality remains a major public health problem in many countries. The aim of this paper is to describe the progress made in maternal health care in Zhejiang Province, China over 20 years in reducing the maternal mortality ratio (MMR).Setting: Zhejiang Province is located on the mid-east coast of China, approximately 180 km south of Shanghai, and has a population of 49 million. Almost all mothers give birth in hospitals or maternal and infant health institutes.Method: the annual maternal death audit reports from 1988 to 2008 were analysed. These reports were prepared annually by the Zhejiang Prenatal Health Committee after auditing each individual case.Measurements and findings: China has made considerable progress in reducing the MMR. Zhejiang has one of fastest developing economies in China, and since the 86 economic reforms of 1978, health care has improved rapidly and the MMR has declined. During the 19882008 period, 2258 maternal deaths were reported from 8,880,457 live births. During these two decades, the MMR decreased dramatically from 48.50 in 1988 to 6.57 per 100,000 in 2008. The MMR in migrant women dropped from 66.87 in 2003 to 21.67 per 100,000 in 2008. The rate of decline was more rapid in rural areas than in the city. There has been a decline in the proportion of deaths with direct obstetric causes and a corresponding increase in the proportion of indirect causes. The proportion of deaths classified as preventable has declined in the past two decades. Social factors are important in maternal safety, and on average 26.8% of maternal deaths were influenced by these factors.Conclusion: as the economy was developing, maternal safety was made a priority health issue by the Government and health workers. The provincial MMR has dropped rapidly and is now similar to the rates in developed countries and lower than that in the USA. Rising hospital delivery rates were financially supported not only by NCMS, but also by other local, regional, and national programmes, especially a national Ministry of Health program called “Reducing Maternal Mortality and Eliminating Neonatal Tetanus”, launched in 2000. The levelling out of urban maternal mortality in China in the past decade calls for increased attention. Abstract:Background China is one of the few Countdown countries to have achieved Millennium Development Goal 5 (75% reduction in maternal mortality ratio between 1990 and 2015). We aimed to examine the health systems and contextual factors that might have contributed to the substantial decline in maternal mortality between 1997 and 2014. We chose to focus on western China because poverty, ethnic diversity, and geographical access represent particular challenges to ensuring universal access to maternal care in the region.Methods In this systematic assessment, we used data from national census reports, National Statistical Yearbooks, the National Maternal and Child Health Routine Reporting System, the China National Health Accounts report, and National Health Statistical Yearbooks to describe changes in policies, health financing, health workforce, health infrastructure, coverage of maternal care, and maternal mortality by region between 1997 and 2014. We used a multivariate linear regression model to examine which contextual and health systems factors contributed to the regional variation in maternal mortality ratio in the same period. Using data from a cross-sectional survey in 2011, we also examined equity in access to maternity care in 42 poor counties in western China.Findings Maternal mortality declined by 8.9% per year between 1997 and 2014 (geometric mean ratio for each year 0.91, 95% CI 0.91-0.92). After adjusting for GDP per capita, length of highways, female illiteracy, the number of licensed doctors per 1000 population, and the proportion of ethnic minorities, the maternal mortality ratio was 118% higher in the western region (2.18, 1.44-3.28) and 41% higher in the central region (1.41, 0.99-2.01) than in the eastern region. In the rural western region, the proportion of births in health facilities rose from 41.9% in 1997 to 98.4% in 2014. Underpinning such progress was the Government's strong commitment to long-term strategies to ensure access to delivery care in health facilities-eg, professionalisation of maternity care in large hospitals, effective referral systems for women medically or socially at high risk, and financial subsidies for antenatal and delivery care. However, in the poor western counties, substantial disparity by education level of the mother existed in access to health facility births (44% of illiterate women vs 100% of those with college or higher education), antenatal care (17% vs 69%) had at least four visits), and caesarean section (8% vs 44%).Interpretation Despite remarkable progress in maternal survival in China, substantial disparities remain, especially for the poor, less educated, and ethnic minority groups in remote areas in western China. Whether China's highly medicalised model of maternity care will be an answer for these populations is uncertain. A strategy modelled after China's immunisation programme, whereby care is provided close to the women's homes, might need to be explored, with township hospitals taking a more prominent role. The analysis of Tibet Autonomous Region's process in achieving maternal and child health indicators under the United Nations Sustainable Development Goals

Abstract:Objective To investigate the process in achieving maternal and child health (MCH)indicators under seventeen sustainable development goals (SDGs)of 2030 agenda,analyze challenges and provide strategies for achieving goals of Tibet Autonomous Region. Methods Using data from statistical yearbooks of China and Tibet from 1991 to 2017,we extracted available data from Tibetan and national maternal and child health indicators,and estimated the missing data with interpolation method.We described the trend of maternal and child mortality in Tibet as well as its gap with the national average,analyzed the impact of MCH services and socioeconomic development on MCH status,and predicted Tibet's process towards SDGs based on historical data. Results From 1990 to 2015,there was a significant reduction of maternal mortality rate (MMR),infant mortality rate (IMR)and under-5 mortality rate (U5MR)in Tibet with average annual reduction rates of 7.6%,7.0% and 7.1% respectively.Tibet achieved MCH indicators under MDGs on schedule and the disparities between Tibet and the national average in maternal and child mortality continued decreasing,which was largely attributed to the rapid improvement of MCH services and socioeconomic development.As for SDGs,NMR and U5MR indicators had been achieved in 2014 and 2013 in Tibet,MMR indicator was estimated to be achieved in 2023 based on historical trends. Conclusion Tibet is on track to achieve MCH indicators under SDGs with significantly improved MCH status.Health assistance and active interventions on MCH and socioeconomic development can accelerate the progress towards SDGs.
The study on the effect of intervention strategies on reducing Maternal Mortality Ratio in rural area of Tibet in China

Abstract:Objective To predict the effects of maternal health interventions on reducing Maternal Mortality Ratio (MMR) in rural area of Tibet Autonomous Region in China with Lives Saved Tool (LiST),and provide evidence for policy making.Methods Data on maternal health intervention coverage and other related information from 2008 to 2015 were collected through literature review and expert consultation.LiST projection was modeled by using the data of 2008 as the baseline to predict the reduction in MMR and the outcomes of interventions from 2016 to 2020,and then intervention strategies that may be of priority were recommended.Results labor and delivery management was associated with the largest reduction in MMR,followed by the active management of the third stage of labor (AMTSL),hypertensive disorder case management,and MgSO4-management of pre-eclampsia.Conclusion Enlarging the coverage of labor and delivery management and AMTSL could reduce maternal death in rural area of Tibet effectively.Improving the quality of obstetric care in township hospitals was highly recommended in practice,because LiST does not take changes in quality of obstetric care into consideration,while the enlargement of coverage of labor and delivery management and AMTSL rely largely on medical institution.

Introduction Twenty years into the Safe Motherhood Initiative, countries continue to struggle to reduce the high burden of maternal death. The fifth Millennium Development Goal (MDG-5) has set a target of reducing maternal mortality by 75% between 1990 and 2015, but progress has been slow. (1,2) Effective technical interventions exist, but their application requires considerable skill. The training, deployment and retention of skilled health personnel remain huge challenges, and inequities, insufficient financial resources and lack of political will continue to limit progress. (3,4) China represents one of the few success stories in maternal health. The maternal mortality ratio (MMR), estimated at 1500 deaths per 100 000 live births in the 1950s, (3,5) decreased to an estimated 88 deaths per 100 000 live births in 1990. (6-8) The factors explaining this decline are complex, but the successful expansion of rural health services, with an effective referral system from villages to township and county hospitals, is thought to have been a major factor. (9-11) During the past 25 years China has undergone unprecedented economic growth, and the health reforms introduced concurrently have raised concerns over rising inequalities. (12,13) There is ample evidence of inequalities between regions in access to obstetric care and maternal survival, but whether gaps increase over time is unknown. (7,13-15) As in other countries, in China the accuracy of maternal mortality estimates is uncertain. The recent Countdown, which tracks coverage of child and maternal health indicators in 68 countries, including China, was unable to track progress towards MDG-5 because of the huge margin of uncertainty in maternal mortality estimates. (16) China's maternal mortality estimates are typically obtained from a sentinel surveillance system covering a sample of 37 urban and 79 rural sites. (17) The generalizability of these data has been called into question, and discrepancies have been noted in the classification of the causes of death. (7) In this study we used a different source of data to examine trends and variations in the MMR in China between 2000 and 2005. The National Maternal and Child Health Routine Reporting System, established in the 1980s, covers the entire population of China. (18) We report trends in maternal mortality by province and region, and explore the extent to which the observed trends are explained by changes in the proportion of institutional births, the crude birth rate and economic growth over time. Methods Data We obtained data on live births and maternal deaths in each province between 2000 and 2005 from the National Maternal and Child Health Routine Reporting System, which falls under the responsibility of the Ministry of Health. (13,18) The system differs somewhat in urban and rural areas. In urban areas, all pregnancies are registered and community doctors keep a log of all pregnancy outcomes. When the pregnancy outcome is not known, they call the woman's home to update the information. Once a month they also visit obstetric and emergency departments in their catchment area and check death certificates at the police departments to further identify maternal deaths. Community doctors send monthly reports to sub-district health managers, who forward summary reports to district health managers twice a year. In rural areas, village doctors use their extensive community networks to identify births and deaths within their catchment area. Data are forwarded monthly to township hospitals and twice a year to the county health department. Maternal deaths are defined as the death of a woman while pregnant or within 42 days of pregnancy termination, irrespective of pregnancy duration or termination method, excluding deaths from intentional and unintentional injuries. …

Liang and colleagues provide the first estimates of the progress of maternal mortality ratios and MDG 5 and SDG 3 at the county level in China. However, a question worth exploring is whether it is appropriate to calculate and compare maternal mortality ratios by county. The population differences between provinces in China are large, and between counties are even greater. The largest county in China has a population of 2·44 million, while the smallest has a population of less than 10 000. Abstract:Abstract Background: The maternal mortality ratio (MMR) is not only an important indicator of maternal and infant safety, but also a sign of the development of economy, education, and medical care in a country. In the last 60 years, the Chinese government has implemented various strategies and policies to reduce the MMR, especially in the rural areas. Aim: This study aimed to discuss the strategies developed by the Chinese government, showing the successful experience of Chinese intervention programs and highlighting the challenges to the government in the context of current economic and social status. Method: This study probed into the Chinese government’s efforts and achievements in the MMR reducing by reviewing the relevant health policies, extracting the data from China Health Statistics Yearbook of 2015 , analyzing the reduction of maternal death in rural areas and the major causes from 1991 to 2015, comparing the MMR trend in urban and rural areas, and discussing the changes of the situation in China. Finding: Although it seems that Chinese government’s efforts have brought evangel to the rural pregnant women and significantly reduced rural maternal mortality, the government still needs to develop more equitable and flexible primary health care policies to narrow the imbalance in health resource allocation and pay more attention to the health care for the rural-to-urban migration in China.

Abstract:BackgroundMaternal mortality in China has decreased strikingly over the past 20 years, and China is one of the few countries achieving Millennium Development Goal 5. Between 1996 and 2013, China substantially strengthened the maternal-health sector: a large number of funds were invested in maternal health, maternal health facilities were substantially improved, and maternal-health professionals received increased training. China also made substantial investments in the overall health-systems reform, including increases in overall health expenditure, universal health insurance coverage, and the number of health professionals per person. All of these health-systems improvements are thought to have contributed to reductions in maternal mortality in China, but which improvements carried the greatest weight were not known. The aim of this study was to examine health-systems determinates of declines in maternal mortality in China.MethodsWe used a panel dataset of 31 provinces from China between 1996 and 2013 to examine the effects of health-systems factors on maternal mortality. Our data on health systems were obtained from China Statistical Yearbooks, China Health Statistical Yearbooks, and other national and provincial publications. Data on maternal mortality were obtained from the Annual Report of Maternal and Child Health information. We ran fixed-effects regressions using the logarithm of maternal mortality as the dependent variable. The health-system determinants studied were health expenditure, number of hospital beds, human resources, and length of highways. We controlled for income, female literacy, birth rate, and local tax revenue capacity. All data used were publicly available, aggregate secondary data; therefore, this study was exempt from ethics approval. No individual patient data were used, so no informed consent from patients was necessary.FindingsMaternal mortality fell from 66 to 14 deaths per 100 000 livebirths between 1996 and 2013 (32 to 8 deaths per 100 000 livebirths in eastern region, 52 to 13 deaths per 100 000 livebirths in central region, and 109 to 23 deaths per 100 000 in western region). Hospital beds per 1000 population (elasticity –0·78 [p=0·023]) and length of highways (–0·31 [p=0·010]) were significantly associated with maternal mortality. Health expenditure and the density of health workers had no effect on maternal mortality."InterpretationAccess to health care is important in accounting for variation in maternal mortality between provinces and over time in China. Abstract:Abstract Introduction China is one of the few countries to achieve Millennium Development Goal 4 and 5. This study aimed to access the levels and trends of maternal mortality ratio (MMR) in China from 1990 to 2018. Methods Using descriptive epidemiology, we collected the data from the China Health Statistics Yearbooks and the China Statistical Yearbooks to describe changes in MMRs, hospital delivery rate, per capita GDP, policies, health expenditure indictors and per capita annual income by region from 1990 to 2018. Spearman correlation analysis was used to assess the relevance between MMR and health expenditure indicators. Results The MMR decreased by 79.4% from 1990 to 2018 in China, and the MMR was remarkably lower in the eastern China than that in the western China. In the context of the widening gap between urban and rural wealth from 1990 to 2018, the urban-rural MMR gap narrowed. The MMR in China has always been higher in rural areas than in urban areas. After the implementation of the Two-Child policy in 2015, the urban MMR continued to decline in 2015-2018, and the rural MMR rebounded in 2017. The hospital delivery rate in China has been on the rise, with almost all pregnant women giving birth in hospital by 2018. The MMR was negatively correlated with the percentage of government health expenditure in the total health expenditure. Obstetric hemorrhage has always been the leading cause of maternal death from 1990 to 2018 in urban and rural areas. Conclusions China has made remarkable progress in maternal survival, despite regional differences still exist. The implementation of the Two-Child policy slowed down the decline of MMR in rural areas. Chinese government should focus on the maternal health in western provinces, rural areas, and the floating population in urban areas.

Abstract:Abstract Background Most maternal deaths in developing countries can be prevented. China is among the 13 countries with the most maternal deaths; however, there has been a marked decrease in the maternal mortality ratio (MMR) over the last 3 decades. China's reduction in the MMR has contributed significantly to the global decline of the MMR. This study examined the geographic and rural-urban differences, time trends and related factors in preventable maternal deaths in China during 1996-2005, with the aim of providing reliable evidence for effective interventions. Methods Data were retrieved from the population-based maternal mortality surveillance system in China. Each death was reviewed by three committees to determine whether it was avoidable. The preventable maternal mortality ratio (PMMR), the ratios of PMMR (risk ratio, RR) and 95% confidence intervals (CI) were used to analyze regional disparities (coastal, inland and remote regions) and rural-urban variations. Time trends in the MMR, along with underlying causes and associated factors of death, were also analysed. Results Overall, 86.1% of maternal mortality was preventable. The RR of preventable maternal mortality adjusted by region was 2.79 (95% CI 2.42-3.21) and 2.38 (95% CI: 2.01-2.81) in rural areas compared to urban areas during the 1996-2000 and 2001-2005 periods, respectively. Meanwhile, the RR was the highest in remote areas, which was 4.80(95%CI: 4.10-5.61) and 4.74(95%CI: 3.86-5.83) times as much as that of coastal areas. Obstetric haemorrhage accounted for over 50% of preventable deaths during the 2001-2005 period. Insufficient information about pregnancy among women in remote areas and out-of-date knowledge and skills of health professionals and substandard obstetric services in coastal regions were the factors frequently associated with MMR. Conclusions Preventable maternal mortality and the distribution of its associated factors in China revealed obvious regional differences. The PMMR was higher in underdeveloped regions. In future interventions in remote and inland areas, more emphasis should be placed on improving women's ability to utilize healthcare services, enhancing the service capability of health institutions, and increasing the accessibility of obstetric services. These approaches will effectively lower PMMR in those regions and narrow the gap among the different regions.

Abstract:BackgroundIn order to achieve the Millennium Development Goals (MDG) 4 and 5, the Chinese Government has invested greatly in improving maternal and child health (MCH) with impressive results. However, one of the most important barriers for further improvement is the uneven distribution of MCH human resources. There is little information about the distribution, quantity and capacity of the Chinese MCH human resources and we sought to investigate this.MethodsCities at prefectural level were selected by random cluster sampling. All medical and health institutions providing MCH-related services in the sampled areas were investigated using a structured questionnaire. The data were weighted based on the proportion of the sampled districts/cities. Amount, proportions and numbers per 10,000 population of MCH human resources were estimated in order to reveal the quantity of the Chinese MCH human resources. The capacity of MCH human resources was evaluated by analyzing data on the education level and professional skills of the staff.ResultsThere were 77,248 MCH workers in our investigated institutions in 2010. In general, 67.6 % and 71.9 % of the women’s and children’s health care professionals had an associate degree or higher, whereas around 30 % had only high-school or lower degrees. More than 40 % of the women’s health workers were capable of providing skilled birth attendance, but these proportions varied between different institutions and locations.ConclusionsEvidence from this study highlights that Chinese MCH human resources are not in shortage in the national level. However, the quantity and capacity of MCH human resources are not evenly distributed among different institutions and locations. Finally there is a need in the improvement of the MCH services by improving the quality of MCH human resources.

Abstract:Background The Chinese government introduced the maternal health services as part of the Basic Public Health Service (BPHS) project in 2009. While China has made progress in reducing maternal mortality, the longitudinal association between the utilization rate of the maternal health services of the BPHS project and such reduction was rarely evaluated with robust methods. Methods We conducted a longitudinal study on the maternal health service utilization rates of the BPHS project and the maternal mortality ratios (MMR) in mainland China and its 31 provincial regions. The data were extracted from the National Health Statistic Yearbooks (2009–2016). Panel data models were used to evaluate the association between the utilization rate and the MMR after adjusting for available covariates on healthcare resources. Stepwise regression models were used to gauge the direction and magnitude of omitted variable bias. Results It was found that the service utilization rate increased from 79.24% in 2009 to 91.67% in 2016, while MMR decreased from 30.90 to 17.88 per 100,000 livebirths at the national level. The results of the fixed effects panel data model revealed that the utilization rate was associated with MMR deduction continuously. With every 1% increase in utilization rate, the maternal death would decrease by 0.35 per 100,000 livebirths after having the health resources variables adjusted. Conclusions The utilization of maternal health services increased continuously after the introduction of BPHS project in 2009 and MMR went downward from then on. The utilization of maternal health services did have significant association with MMR and could reduce MMR effectively. Such improvement could be attributed to the fact that this program was designed to serve the targeted population throughout the continuum of maternal care and the government’s rigorous efforts in stressing equality and standard care in program implementation.

Abstract:OBJECTIVE: To examine trends and variations in maternal mortality in China between 2000 and 2005.METHODS: We used Poisson regression analysis of data from the Chinese National Maternal and Child Health Routine Reporting System between 2000 and 2005 to identify time trends in the maternal mortality ratio (MMR) by province and region.FINDINGS: The MMR declined by an average of 5% per year (crude relative risk, RR: 0.95; 95% confidence interval, CI: 0.94-0.97). There was no interaction between region and year (P = 0.2311). Mortality declined by 5% per year in the eastern region (crude RR: 0.95; 95% CI: 0.92-0.97), by 5% per year in the central region (crude RR; 0.95; 95% CI: 0.94-0.96), and by 4% per year in the western region (crude RR: 0.96; 95% CI: 0.94-0.98). The absolute difference in MMR between the western and eastern regions declined from 65.4 deaths per 100,000 live births in 2000 to 49.4 per 100,000 live births in 2005.CONCLUSION: China is making good progress towards achieving the fifth Millennium Development Goal, and there is no evidence of a widening gap between better-off and economically more deprived provinces.

Abstract:BackgroundWith the introduction of the Millennium Development Goals 4 and 5 in 1990, analyses of progress in maternal and newborn health have focused on the last 20 years. However, China already had low maternal and neonatal mortality rates in 1990, so it is important to understand the health policy environment and health systems inputs that have underpinned progress since 1949.MethodsWe used standard tools developed by Countdown to 2015's Health Systems and Policies Technical Working Group to examine national-level changes to the health system and health policies in China since 1949 and to describe the translation of macropolicies and strategies into the implementation of maternal and newborn health programmes. We used data from the Chinese Health and Family Planning Statistical Yearbook 2013 to calculate national and subnational health workforce densities. This study used open access secondary (and anonymous) data so did not require ethical approval.ResultsThe Chinese maternal and newborn policy and programmatic environment from 1949 to present is complex. Initially, training of traditional midwives and barefoot doctors aimed to decrease maternal mortality and neonatal tetanus in rural areas. With vast economic growth from the late 1970s, well coordinated health initiatives, such as a perinatal health programme and cross-cutting strategies including the one-child policy, were implemented nationally. The 1995 Law on Maternal and Infant Health Care focused on training and service delivery for maternal and new-born health, and the 1999–2009 Safe Motherhood programme focused on increasing facility births. From 2003, the Rural Cooperative Medical System contributed to increases in hospital delivery by improving quality of service delivery, providing subsidies to pregnant women, and focusing on community health education. As a result, hospital births increased nationally from 61·7% of all live births in 1997 to 99·5% in 2013, and in western Provinces from 47·9% in 1997 to 98·5% in 2013. Enabling health system and policies substantially improved facility delivery rates in Western provinces, although large urban, rural, and provincial disparities still exist. In 2012, the density of health workforce per 1000 population in urban areas was nearly three times that of rural areas (8·54 vs 3·41), and the density of health workforce per 1000 population in Beijing Province was twice that of Sichuan Province (9·48 vs 4·82).InterpretationChina's successes in implementing health services and policies to improve maternal and newborn health could be useful to other countries. More needs to be done to decrease inequities within the country. A strength of this study is its use of standardised, novel tools to examine the effect of health system and policies on maternal and newborn health in China over a long time frame (1949 to present). Abstract:Purpose Many public health systems have struggled with the dual questions of (1) why the uptake rate of maternal health services is low among some subpopulations; and (2) how to raise it. The objective of this study is to assess the uptake rate of a new set of maternal health services in poor rural areas of China. Design/methodology/approach The analysis is based on the survey responses of women’s representatives and village cadres from almost 1000 villages in June 2012 as part of a wide-scale public health survey in Sichuan, Gansu and Yunnan provinces in the western part of China. Findings We find that the uptake rate of maternal health services (including in-hospital delivery, antenatal care visits and post-partum care visits) in poor rural areas of western China are far below average in China, and that the rates vary across provinces and ethnic groups. Our analyses demonstrate that distance, income, ethnicity and availability appear to be systematically correlated with low uptake rates of all maternal health services. Demand-side factors seem to be by far the most important sources of the differences between subpopulations. We also find that there is potential for creating a Conditional Cash Transfer program to improve the usage of maternal health services. Originality/value We believe that our results will contribute positively to the exploration of answers to the dual questions that many public health systems have struggled with (1) why the uptake rate of maternal health services is low among some subpopulations; and (2) how to raise it.

Abstract:BACKGROUND:Universal maternal health coverage is a proven, effective strategy for maternal survival. This study aimed to describe the changes and determinants of maternal health service use between 1991 and 2015 in central China.METHODS:The study was conducted in Enshi Prefecture. Women were eligible for inclusion if they were rural women who lived in villages, had live births during 1991-2015, could recall their maternal care histories, and had no communication problems. This retrospective study included 470 rural women in 9 villages and collected 770 records. The conceptual framework was designed based on the Society Ecosystem Theory. The determinants included micro-factors (individual characteristics), meso-factors (family factors, community factors, healthcare factors), and macro-factors (government-run maternal and child health programs, abbreviated as MCH programs). Multivariate logistic regressions were applied to analyze the determinants of maternal health service utilization.RESULTS:The utilization of maternal healthcare has improved in Enshi. The hospital birth rate was 98.1% in 2009 and mostly 100% in subsequent years. The prenatal examination rate, the postpartum visit rate, and the continuum of maternal health service (CMHS) rate increased to 73.3%, 67.7%, and 53.4%, respectively, in 2009-2015. The utilization of maternal health services was affected by macro-factors, meso-factors, and micro-factors, with macro-factors being the most notable contributors.CONCLUSIONS:Despite the remarkable improvements in antenatal care (ANC) use and hospital birth, gaps in postpartum visits remain. Promoting the integrated continuum of maternal and child healthcare in ethnic minority rural areas requires the joint efforts of the government, health and other sectors, communities, families, and individuals.

Abstract:Background: China has made impressive progress towards the Millennium Development Goal (MDG) for maternal and reproductive health, but ensuring that progress reaches all segments of the population remains a challenge for policy makers. The aim of this review is to map disadvantaged populations in terms of maternal health in China, and to explain the causes of these inequities to promote policy action. Methods: We searched PUBMED, Popline, Proquest and WanFang and included primary studies conducted in mainland China. Experts were also contacted to identify additional studies. Disadvantaged populations in terms of MDG 5 and the reasons for this disadvantage explored by authors were identified and coded based on the conceptual framework developed by the WHO Commission on the Social Determinants of Health. Results: In China, differences in maternal health service utilization and the maternal mortality ratio among different income groups, and among regions with different socio-economic development still exist, although these differences are narrowing. Groups with low levels of education and ethnic minorities utilize maternal health care less frequently and experience higher maternal mortality, although we could not determine whether these differences have changed in the last decade. Rural-to-urban migrants use maternal health care and contraception to a lower extent than permanent residents of cities, and differential maternal mortality shows a widening trend among these groups. Gender inequity also contributes to the disadvantaged position of women. Intermediary factors that explain these inequities include material circumstances such as long distances to health facilities for women living in remote areas, behavioral factors such as traditional beliefs that result in reduced care seeking among ethnic minorities, and health system determinants such as out-of-pocket payments posing financial barriers for the poor. Conclusions: Inequity in maternal health continues to be an issue worthy of greater programmatic and monitoring efforts in China.
