Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aims:The aim of the present study was to analyse trends in full breastfeeding for at least 4 months across socioeconomic position in Denmark over a 17-year-long period from 2002 to 2019 using parental education as the indicator of socioeconomic position.Methods:The study used data on full breastfeeding collected between 2002 and 2019 by community health nurses in the collaboration Child Health Database, n=143,075. Data were linked with five categories of parental education from population registers. Social inequality was calculated as both the relative (odds ratio) and absolute social inequality (slope index of inequality). A trend test was conducted to assess changes in social inequality over time.Results:A social gradient in full breastfeeding was found for the entire study period. The odds ratio for not being fully breastfed for at least 4 months ranged from 3.30 (95% confidence interval 2.83–3.84) to 5.09 (95% confidence interval 4.28–6.06) during the study period for infants of parents with the lowest level of education (primary school) compared with infants of parents with the highest level of education (5+ years of university education). The slope index of inequality was between −38.86 and −48.81 during the entire study period, P=0.80. This indicated that both the relative and absolute social inequality in full breastfeeding to at least 4 months of age was unchanged in the study period from 2002 to 2019.Conclusions:This study showed a persistent relative and absolute social inequality in full breastfeeding for at least 4 months from 2002 to 2019 in Denmark.

Abstract:Background/objectives: The primary purpose of this study was to correlate the factors influencing the onset and duration of exclusive breastfeeding (EBF) and breastfeeding (BF) rates at different time points in baby-friendly hospitals (BFHs) in Greece. Methods: This study was conducted from October 2020 to January 2022. The sample consisted of 1201 mothers with corresponding newborn births out of the total 7201 that took place during the same period. We used a questionnaire that the mothers answered during the first hour after birth, before being discharged from the maternity hospital, and at the 2nd, 4th, and 6th month of the infant's life. The results were compared with the data of the national study from 2017 (concerning births from general maternity hospitals and not only from BFHs). Results: The EBF rate within 1 h after birth was 71.3%, which gradually declined to 21.2% in the 6th month. Between the two studies, differences were recorded in BF and EBF rates at the 6th and 4th month of the infants' life, respectively, especially in mothers who are less likely to breastfeed (e.g., those without breastfeeding experience, those over 40 years old, etc.). Conclusion: This study showed that EBF and BF rates are higher in infants born in BFHs compared to non-BFHs both during their hospitalization in the first days of life and during the first 6 months of life. BFHs also play a decisive role in mothers who, due to their socio-economic profile, have lower chances of starting to breastfeed.
Clinical experience of eclampsia at the University Teaching Hospital, Lusaka. An analysis of 50 cases.

Abstract:A clinical analysis of 50 cases of eclampsia for the period June 1971-June 1973 is presented. During the same period there were 32 011 deliveries--an incidence of 0,156%. Of these 50 cases, only 8 were booked. There were 40 primigravidas. The highest number of fits recorded in any one patient was 6, and 13 patients has had fits before admission. Thirty-four patients were in established labour on admission. Only 12 patients had postpartum fits. The basis of management was heavy sedation, anticonvulsant therapy and emptying of the uterus as soon as it could be achieved. Biochemical studies and urine production measurements were important supplementary measures. Twenty patients achieved vaginal delivery and 3 had symphysiotomy, assisted by vacuum extraction, low forceps and midcavity forceps, to complete the delivery. Seventeen patients were delivered by Caesarean section. The remaining 10 had vaginal delivery assisted by vacuum extraction. One mother did not survive. Of the 51 babies, 7 did not survive the neonatal period, and 4 were stillborn.
The Impact of Socio-Demographic Factors on Breastfeeding: Findings from the “Mamma & Bambino” Cohort

Abstract:Background and objectives: The World Health Organization (WHO) recommends women (1) to initiate breastfeeding within one hour of birth; (2) to exclusively breastfeed for the first six months; and (3) to continue breastfeeding until two years of age. However, women do not always adhere to these recommendations, threatening the health of their children. The present study aims to evaluate breastfeeding status and the main maternal factors associated with exclusive breastfeeding for six months among women from the “Mamma & Bambino” study, a prospective cohort settled in Catania, Italy. Materials and Methods: We used data from 220 women (median age = 37 years) enrolled in the “Mamma & Bambino” cohort during prenatal obstetric counselling. Self-reported breastfeeding status was collected during the follow-up interviews at 1 and 2 years, referring to breastfeeding status (i.e., yes or no) and type of breastfeeding (i.e., exclusive or predominant). We also collected data about duration of breastfeeding to classify women into those who adhered to the WHO recommendation and those who did not. Results: In the general population, we noted that the proportion of women who have breastfed increased with increasing educational level. Accordingly, logistic regression analysis demonstrated that medium (OR = 3.171; 95% CI = 1.285–7.822; p = 0.012) and high educational levels (OR = 4.549; 95% CI = 1.525–13.570; p = 0.007) were positively associated with breastfeeding if compared to low educational level. Among women who have breastfed, instead, the proportion of adherents to the WHO recommendation was higher among those with medium–high educational level and those who were employed. In line with this, we demonstrated that full-time employment (OR = 2.158; 95% CI = 1.033–4.508; p = 0.041) and medium educational level (OR = 4.632; 95% CI = 1.227–17.484; p = 0.024) were positively associated with exclusive breastfeeding for the first six months. Conclusions: Socio-demographic factors should be taken into account through public health strategies for improving maternal knowledge about health benefits of exclusive breastfeeding.

Abstract:Background: Data from the UK Infant Feeding Surveys indicate that breastfeeding initiation increased between 1985 and 2010. During this period, societal changes in GB also influenced the sociodemographic characteristics of women in the childbearing population. As breastfeeding behaviour is highly socially patterned in GB, the increasing trend in breastfeeding initiation may have hidden inequalities in breastfeeding practices. This study examines the sociodemographic inequalities in breastfeeding initiation in GB between 1985 and 2010, exploring whether and how this may have been influenced by social and policy changes. Methods: Data drawn from the nationally representative 1985, 1990, 1995, 2000, 2005 and 2010 Infant Feeding Surveys were used to estimate changes in the proportion of mothers in selected sociodemographic groups over time. Logistic regression models estimated the independent associations between breastfeeding initiation in each survey year and maternal sociodemographic characteristics. Associations were adjusted for maternal sociodemographic, pregnancy-related and support factors. Evidence of a change in the association between breastfeeding initiation and each sociodemographic characteristic over time was assessed using a test for statistical heterogeneity. Results: The sociodemographic characteristics of mothers in GB changed substantially between 1985 and 2010. Mothers were increasingly more likely to be 30 or over; have higher education and socioeconomic status; and be single or cohabiting. An increasing proportion of mothers in GB identified as being of black or minority ethnic origin. Reported smoking in pregnancy declined. These same characteristics independently predicted higher odds of breastfeeding initiation; the associations between these characteristics and breastfeeding initiation did not vary significantly over time. Conclusions: Marked inequalities in breastfeeding initiation persisted over the study period, hidden among the increasing initiation rate at the population level. The increasing overall rate of initiation was most likely driven by the rising prevalence of those groups of mothers who were, and remain, characteristically most likely to breastfeed.

Abstract:Background: Indicators to assess exclusive breast feeding (EBF) status are based on current status data according to World Health Organization (WHO), specially to avoid recall bias or imprecise reporting. Objective: To analyse the agreement between current status and retrospective data for prevalence and duration of EBF in low- and middle-income countries. Methods: Cross-sectional study using Demographic and Health Surveys (DHS) datasets of infants under six months from 10 low- and middle-income countries. It was applied two kinds of data about EBF: (1) current status data: variables about breast milk and foods offered in the previous day (yes or no) and (2) retrospective data: variables about age at which foods were offered the first time. The prevalence of EBF was estimated the same way for current status and for retrospective data. The median duration for current status data was calculated according to WHO recommendation, and retrospective data were calculated using survival analysis. The Kappa coefficient was applied to assess the agreement of prevalence using both kinds of data. Results: Prevalence of EBF was higher using current status data and differences between both data ranged from 0.5 to 6.4 percentage points. Kappa coefficient ranged from 0.74 (substantial) to 0.94 (almost perfect) in nine countries. Medians were higher using retrospective than current status data for two countries and lower for another two; however, differences were small (from 0.08 to 0.44 month). For six countries, breast-feeding practice was so low that it was not possible to calculate the median using either data. Conclusions: The difference between prevalence and median estimated using current status and retrospective data is little and the agreement ranged to substantial and almost perfect. We suggest the use of retrospective data to estimate duration of EBF in cross-sectional surveys.
Association Between Maternal Education and Breast Feeding Practices in China: a Population-Based Cross-Sectional Study

Abstract:ObjectiveTo investigate the association between maternal education and breast feeding in the Chinese population, with a consideration of household income and health-seeking behaviours.DesignA nationally representative population-based cross-sectional study.Setting77 counties from 12 geographically distinct regions in China.Participants10 408 mothers with children from 0 to 12 months of age, aged 15–53 years old (mean: 29.15, SD: 5.11) were classified into primary school or below group (n=781), middle school group (n=3842), high school/vocational school group (n=1990), college or above group (n=3795), according to their highest completed education.OutcomesFive breastfeeding outcomes, including early initiation of breast feeding (EIB), exclusive breast feeding (EBF) under 6 months, predominant breast feeding under 6 months, current breast feeding and children ever breast fed, were calculated based on the standardised questionnaire from the WHO and Wellstart International’s toolkit for monitoring and evaluating breastfeeding activities.ResultsThe absolute risk of EIB and EBF in the lowest maternal education level was 64.85% and 26.53%, respectively, whereas the absolute risk of EIB and EBF in the highest maternal education level was 77.21% and 14.06%, respectively. A higher level of maternal education was positively associated with EIB (risk ratio (RR): 1.22; 95% CI: 1.12 to 1.30) and was inversely associated with EBF (RR: 0.59; 95% CI: 0.38 to 0.88). Stratified by household income, a positive association with EIB was observed only in the group with the highest household income and an inverse association with EBF was found in both low household and high household income groups.ConclusionsMothers with a higher education were more likely to initiate early breast feeding when they were also from a high-income household while also being less likely to exclusively breast feed their babies. Routine and successful nursing is crucial for the health of infants and is influenced by maternal education. Future public health interventions to promote breast feeding should consider the issues related to the educational level of mothers.

Abstract:Breastfeeding promotion and support in hospitals is expected to have a positive impact on maternal breastfeeding outcomes. The objective of this study is to examine the association between breastfeeding promotion in maternity hospitals in Germany and exclusive breastfeeding (EBF) rates during the first 4 months. Thus, a nationwide cross-sectional web-based survey of breastfeeding promotion was conducted in 103 hospitals. Mother-infant pairs (n = 962) were recruited at these hospitals for a prospective web-based survey of breastfeeding status at five-time points, that is, during a hospital stay, at discharge as well as after 0.5, 2, and 4 months. The hospital analysis was based on the "10 Steps to Successful Breastfeeding" of the World Health Organization and the United Nations Children's Fund, adapted for Germany. Their degree of implementation was stratified by a breastfeeding promotion index (BPI) as low (≤5 steps), medium (6-8 steps), and high (≥9 steps). The association between the BPI and the odds of EBF at each of the five-time points was estimated by multivariable regression models, adjusting for various maternal factors. At all time points, the proportion of EBF among mothers from high BPI hospitals exceeded the proportion of those from medium or low BPI hospitals. A high BPI was associated with higher odds of EBF during the hospital stay and at discharge, while maternal factors for EBF such as breastfeeding experience and no early use of a pacifier persisted beyond. The high commitment of hospitals and tailored support of mothers is essential for EBF.

Abstract:Background: This study aims to investigate the measurement of breastfeeding prevalence indicators using Demographic and Health Surveys (DHS) data, focusing on early initiation, exclusive breastfeeding, and continued breastfeeding indicators as reported by the World Health Organization (WHO) and the United Nations Children's Fund (UNICEF) and on the discrepancies arising from small changes in their definition. Methods: Two hundred sixty DHS samples from 78 countries were analyzed to re-calculate usual indicators reported by WHO and UNICEF: early initiation of breastfeeding (EIB), exclusive breastfeeding under 6 months (EBF), and continued breastfeeding between 1 and 2 years (CBF12 and CBF24). Additionally, alternative estimates of the same indicators, slightly changing their definition, were calculated to test their robustness. Results: The WHO and UNICEF indicators for early initiation (EIB) primarily capture cases where breastfeeding is initiated "immediately" after birth, omitting those initiated within 0 or 1 hour. This discrepancy leads to substantial underestimation of levels in some regions, particularly South Asia, and in trends. Furthermore, sizable discrepancies between exclusive breastfeeding (EBF) indicators arise from the inclusion or exclusion of plain water in the definition, with significant variations across regions, especially in West and Middle Africa. However, continued breastfeeding indicators showed consistency across definitions, proving them robust for international comparisons and time trend estimations. Conclusion: This study highlights the importance of understanding how breastfeeding indicators are defined and calculated using DHS data. Researchers should be cautious when using WHO and UNICEF indicators for early initiation and exclusive breastfeeding, as they may underestimate prevalence due to their narrow definition. Continued breastfeeding indicators, on the other hand, are less affected by small changes in definitions and provide reliable measures for cross-country comparisons and trend analyses. These findings underscore the need for standardized robust definitions and transparent reporting of breastfeeding indicators in global health assessments.

Abstract:HI returns: Hydroiodic acid is a highly selective reducing reagent for a wide variety of substrates. Its application is limited by the formation of iodine and the difficulty in reconverting that iodione back to HI in situ. We report the facile conversion of I(2) to HI by metal-catalyzed hydrogenation in the presence of water, and demonstrate the utility of this process in the conversion of fructose to 5-methyfurfural and glycerol to 2-iodopropane.
[Results of hearing preservation in surgery of vestibular schwannoma. Value of combined retrosigmoid and middle fossa approaches].

Abstract:We report our experience of hearing preservation in acoustic neurinoma surgery, using combined retrosigmoid and middle fossa approaches. Fifty neurinomas operated on between 1987 and 1994 were included in this retrospective study. Hearing preservation surgery was performed for patients with grade II or grade III tumors (mean average tumor diameter: 14.4 mm), presenting with normal or serviceable pre-operative hearing (pure tonal average decrease less than 50 dB, speech discrimination score better than 50%). Isolated middle fossa approach was used in 3 cases, isolated retrosigmoid approach in 2 cases. The 45 other cases were operated on using both routes during the same procedure. Total removal of the tumor with anatomic facial preservation was performed in all cases. No death occurred. The facial function assessed 3 months after surgery was good in 84% of cases (House-Brackmann grades I or II). The mean follow-up was 42 months. Post-operative hearing was measurable in 48% of cases and serviceable in 30% of cases. The size of the tumor and the level of preoperative hearing appear to be the most important predictive criteria for successful hearing preservation.

Abstract:(1) Background: Exclusive breastfeeding is not only the cornerstone of a child's healthy development, but it is also the most economical and planet-friendly way to feed an infant. This study aims to assess the current situation regarding breastfeeding in Romania. (2) Method: The authors searched five databases using the search terms "breastfeeding OR lactation" AND Romania. Studies have been limited to those published in English since 2000. In vitro animal studies, case studies, reviews, meta-analyses, book chapters, and guidelines were excluded. (3) Results: 19 studies were included in the final review. For the purpose of this systematic review, studies were subgrouped into studies on attitudes, practices, and behaviors regarding breastfeeding and the composition of human milk. This review demonstrates a slight tendency to increase interest in breastfeeding in Romania in the last years. Interventional studies conducted so far included mainly urban populations with good socioeconomic levels and a high level of education. Studies about the impact of early intervention in maternal education concluded that the level of education is positively correlated with the interest in education regarding breastfeeding. On the other hand, the Romanian population has similar characteristics to the European population in terms of the factors that influence human milk composition. (4) Conclusions: The review underlines strengths and weaknesses of the Romanian situation regarding breastfeeding and makes recommendations for future directions for research and health education.

Abstract:In terms of breastfeeding (BF) practices in Romania, there is a lack of up-to-date data. The aim of the present study was to assess current BF practices, and to investigate the factors associated with exclusive BF (EBF) under 6 months of age in northwestern Romania. A structured questionnaire was used to collect data among 1399 mothers of children aged 0-23 months, recruited between March and June 2019, from the community. BF practices were evaluated based on the World Health Organization indicators for assessing infant and young child feeding practices, whereas determinants of EBF were explored using logistic regression models. Almost all mothers (95.7%) breastfed their child at least once. The EBF rate was 46.7%, the continued BF rate at one year of age was 54.2%, and the continued BF rate at 2 years of age was 30.3%. The place of delivery and parental leave duration were strong determinants of EBF. The improving rates observed in this study for all the BF practices assessed suggest the continuation of efforts to develop effective national policies and programs for promoting, protecting, and supporting BF in Romania. Particular emphasis should be given to the creation in maternity hospitals of an environment that is supportive towards breastfeeding.
Factors affecting exclusive breast-feeding during the first 6 months in Korea

Abstract:Background: The aim of this study was to identify the status of exclusive breast-feeding (EBF) in Korea and analyze the factors affecting exclusive breast-feeding at 6 months of age. Method: This study was based on data obtained from the Korea National Health and Nutrition Examination Survey (KNHANES) IV, conducted from January 2007 to December 2008. A total of 404 mother-infant pairs were recruited. Exclusive breast-feeding was defined according to the criteria established by the World Health Organization. Result: The rate of EBF was 60.9% for 1 month, 55.0% for 3 months, 35.4% for 6 months, 3.7% for 9 months and 1.2% for 12 months after birth. According to a stepwise logistic regression analysis, factors that were positively associated with EBF at 6 months were younger maternal age (odds ratio [OR] = 0.85, 95% confidence interval [CI]: 0.79-0.92), higher maternal education level (OR = 2.29, 95%CI: 1.17-4.46) and living in a capital city (OR = 2.64, 95%CI: 1.46-4.75). Conclusion: The rate of EBF in Korea is still suboptimal. To promote EBF, persistent and systematic education and campaigns for breast-feeding should be provided, particularly in vulnerable regions.

Abstract:The supernatants collected from cisplatin, lipopolysaccharide (LPS), muramyl dipeptide (MDP) or recombinant interferon gamma (rIFN-gamma) treated non-adherent mononuclear cells (with NK cell activity) enhanced thymocyte proliferation by a submitogenic concentration of concanavalin A as compared to untreated nMNC. Supernatants collected from cisplatin or rIFN-gamma treated nMNC also demonstrated enhanced cytotoxicity against actinomycin-D treated L929 cells, suggesting that cisplatin or rIFN-gamma treated nMNC release tumor necrosis factor (TNF) into the culture supernatant. On the other hand, supernatant collected from untreated nMNC showed little TNF activity. Treatment of nMNC with cisplatin, LPS, MDP or rIFN-gamma resulted in enhanced release of lysozyme into the culture medium as compared to untreated nMNC.
Exclusive Breastfeeding Rates and Associated Factors in 13 “Economic Community of West African States” (ECOWAS) Countries

Abstract:Exclusive breastfeeding (EBF) has important protective effects on child survival and also increases the growth and development of infants. This paper examined EBF rates and associated factors in 13 “Economic Community of West African States” (ECOWAS) countries. A weighted sample of 19,735 infants from the recent Demographic and Health Survey dataset in ECOWAS countries for the period of 2010–2018 was used. Survey logistic regression analyses that adjusted for clustering and sampling weights were used to determine the factors associated with EBF. In ECOWAS countries, EBF rates for infants 6 months or younger ranged from 13.0% in Côte d’Ivoire to 58.0% in Togo. EBF decreased significantly by 33% as the infant age (in months) increased. Multivariate analyses revealed that mothers with at least primary education, older mothers (35–49 years), and those who lived in rural areas were significantly more likely to engage in EBF. Mothers who made four or more antenatal visits (ANC) were significantly more likely to exclusively breastfeed their babies compared to those who had no ANC visits. Our study shows that EBF rates are still suboptimal in most ECOWAS countries. EBF policy interventions in ECOWAS countries should target mothers with no schooling and those who do not attend ANC. Higher rates of EBF are likely to decrease the burden of infant morbidity and mortality in ECOWAS countries due to non-exposure to contaminated water or other liquids.

Abstract:Breastfeeding is the single intervention with the largest impact on the health of a new-born baby. Evidence has to be generated to convince the mother as well as policy-makers for the promotion of exclusive breastfeeding (EBF) for 6 months. This study aimed to assess the evidence for the effectiveness of breastfeeding education compared with standard hospital information among mothers on the rate of EBF. The study design is systematic review. Trials which are randomized or cluster randomized which studied the effect of educational interventions for mothers on EBF were searched for. Two databases were searched, namely PubMed and Cochrane. Manual search of reference lists of all included studies in Google scholar and Clinical Trial Registry was done. Two reviewers independently assessed the quality of the included studies. Data were extracted using a table format set by the reviewers referring the previously reported high-quality systematic reviews. Out of the 12 comparisons of nine studies included for review, eight studies report that breastfeeding education has added advantage on increasing the rate of EBF. We conclude that there is evidence to support breastfeeding education versus standard hospital information across diverse settings.
